So, you finally got inked. You chose a design, picked out a parlor, and “sat” like a champ. (That’s tattoo artist-speak for grinning and bearing it through hours of pain.) Then you spent a few weeks diligently washing and moisturizing it while it healed. Now, save for moments you catch a glimpse of the design in the mirror, you usually forget the whole thing happened. What’s done is done, right?

Not always. In fact, skin irritation or a full-blown condition can develop months, years, even decades after the initial tattooing process. “Tattoos breach the protective layer of the skin, increasing your risk of skin complications,” says David Lortscher, a dermatologist based in San Diego and San Francisco and co-founder of Curology. If you start to see redness, bumps, or even burns on or around a long-healed tattoo, one of these issues could be the culprit, and you should see your physician or dermatologist as soon as possible.

Your tattoo is infected.

You’ve heard horror stories of peoples’ ink getting infected and warping the appearance of the design. But while this typically occurs during the initial healing process, an infection is still possible even months later, according to the American Academy of Dermatology. Some signs to look out for: pain or redness that gets worse rather than better; a rash with itchy, red bumps; open sores; pus; and a fever with chills.

You’ve developed an allergy to the ink.

“Though it’s rare, a reaction called a pseudolymphomatous reaction can occur in response to red ink,” says plastic surgeon David L. Cangello of Cangello Plastic Surgery in New York City. Essentially, this is a delayed hypersensitivity to the ink.

“The exact etiology is unknown, but it’s thought that the red ink acts as an antigen, or something that stimulates an immune response from the body,” says Cangello. “Cells called lymphocytes infiltrate the skin in the area of the antigen — or red pigment in this case — and cause an inflammatory reaction.” Likely, the response has been developing for some time but took months or years to appear on the surface of the skin.

You’re predisposed to a skin condition.

Shockingly, tattoos can cause skin conditions such as eczema and psoriasis and even vitiligo to crop up for the first time. “This centers around something called the Koebner phenomenon,” says Dhaval Bhanusali, a dermatologist in New York City. “Particularly with psoriasis and vitiligo, the idea is that any epidermal disruption can trigger disease, including a tattoo. Eczema is probably more reflective of an allergic reaction.”

Tattoos & Body Piercings – Sepsis Alliance

Tattoos and body piercings provide an opening in the skin that may allow germs to enter your body and cause infections. These infections could cause sepsis. It is for this reason that anyone who receives a tattoo or piercing must take special care to reduce the risk of contracting an infection.

Sometimes incorrectly called blood poisoning, sepsis is the body’s often deadly response to infection. Like strokes or heart attacks, sepsis is a medical emergency that requires rapid diagnosis and treatment.

Sepsis and septic shock can result from an infection anywhere in the body, such as pneumonia, influenza, or urinary tract infections. Like strokes or heart attacks, sepsis is a medical emergency that requires rapid diagnosis and treatment. Worldwide, one-third of people who develop sepsis die. Many who do survive are left with life-changing effects, such as post-traumatic stress disorder (PTSD), chronic pain and fatigue, organ dysfunction (organs don’t work properly), and/or amputations.

Tattoos and piercings create openings in the skin

The art of tattooing and the practice of body piercing go back thousands of years. A tattoo is an image made by permanent ink placed directly under the outermost layer of your skin, the epidermis, into the dermis. Tattoos range from permanent eyeliner or eyebrows to full arm sleeves and more. Regardless of the type of permanent tattoo, in order for the ink or dye to get under the skin, the tattoo artist must make thousands of tiny holes or pin pricks to make openings for the ink to enter. Until each one of these tiny holes in the skin heals, they make you susceptible to infection.

A body piercing involves creating an opening in your skin or cartilage so you can insert a piece of jewelry. Piercings can be done just about anywhere, from the more traditional ear lobes to other parts of the body, such as the tongue, navel, and even genitals. These breaks in the skin are, as with tattoos, a source for infection until they are fully healed. Depending on the location of the piercings, some are also easier to tear or chafe even after they have healed, which again makes them susceptible to infection. This includes infections such as cellulitis and necrotizing fasciitis.

Preventing infection after tattoos and body piercings

If you are considering a tattoo or body piercing, there are some steps you can take before you get the tattoo and after to reduce the risk of contracting an infection .

  • Choose a reputable artist. Ask for recommendations. If you have friends or family members with tattoos or piercings you like, ask if they would recommend the studio and artist again. Ask about their experiences and if they have ever had an infection or problem following the procedure.
  • Check to see if your state or local region regulates shops and studios that offer tattoos or piercings. Not all states have this requirement. Check for how many years the artist has been in business.
  • Visit the studio or shop. Look for signs of proper cleaning and maintenance. Ask questions about what type of equipment the artists use. You are looking for artists who use single, “throw away” kits. For tattoos, these contain disposable needles and tubes, and for piercings they contain needles. Does the staff use an autoclave to sterilize non-disposable equipment? Check to see where they place their used needles. The shop or studio must have a separate, clearly marked red container marked “sharps disposal.” Razors must also be disposable. The artist should also wash his or her hands and use a new pair of sterile disposable gloves before touching a new client or if he or she steps away and returns to continue the procedure. If these basic steps of infection control are not present, go to another establishment.
  • Do not use a DIY tattoo kit. The Food and Drug Administration (FDA) recommends against using do-it-yourself kits for tattoos. The inks are associated with infections and allergic reactions, and the agency is concerned that tattoos done at home may not follow all infection-prevention strategies. Self-piercing is also discouraged because of the increased risk of infection when a piercing is not done in the proper environment.
  • Sterilize jewelry before insertion into the new hole.

Follow after-care instructions

Even if you have had multiple piercings or tattoos and you’ve never had an infection before, you still must follow the after-care instructions every time you have a body modification. It’s important to keep in mind that no two tattoos or piercings heal at the same rate.

Tattoos

You should receive instructions on how to care for the tattoo until it has healed. These are the most common instructions:

  • Once complete, your artist should wrap your tattoo to keep it clean and prevent it from rubbing against your clothing or coming into contact with some other objects that could irritate the skin further or introduce germs. Your tattoo artist will tell you how long to keep the new art covered, which is usually about 24 hours.
  • Once you remove the covering, you should gently clean the tattoo with warm water and unscented soap. Always make sure you have washed your hands before touching your tattoo. Pat your skin dry with a clean towel you don’t share with someone else – do not rub. Rubbing may affect the tattoo and it may cause more irritation to the skin as well. You may want to apply some antibiotic ointment.
  • If any scabs form, do not pick at them.
  • Avoid swimming, using a hot tub, or getting the tattoo wet other than with regular washing. Wait until the skin has healed.
  • If you show any signs of infection, see your doctor or nurse practitioner right away, or go to an urgent care clinic. Signs include skin getting redder around the tattoo, redness spreading, increased pain, pus from the tattoo, fever.

Piercings

As with tattoos, follow the after-care instructions for your particular piercing. Always wash your hands before handling your piercing. The instructions usually include:

  • If you have had an oral piercing, use a new soft bristle toothbrush. This avoids introducing bacteria from an older brush into your piercings. The piercing also needs to be cleaned with an alcohol-free antiseptic mouthwash after you eat and before bed. Swish the mouthwash for 30 seconds to a minute. You can also rinse twice a day with a salt-water rinse, about a quarter of a teaspoon of table salt dissolved in a cup of warm water. After the piercing heals remember to take out the jewelry every night to clean it. This prevents plaque from building up.
  • For skin piercings, clean the site a few times a day. Use a salt water solution or warm water and non-scented soap. Do not go swimming, use a hot tub, or expose your piercing to water other than to clean it until the skin has healed. Don’t remove the piercing until the skin has healed.*
  • For all piercings, it is important to not touch, twist, or fiddle with the jewelry as the hole heals.
  • If you show any signs of infection, see your doctor or nurse practitioner right away. An urgent care clinic is another option. Signs include skin getting redder around the piercing, swelling, increased pain, pus or discharge, fever.

*If you develop an infection, you may need to remove the jewelry in order to treat the infection effectively.

Treating tattoo or piercing infections

You may need an antibiotic cream first. You may get oral antibiotics if the infection is more serious or in a place where it is impossible to apply a cream. Follow the instructions for the antibiotics to work. You may have to remove your piercing

Once the tattoo or piercing is healed, the skin is intact and should not be at risk for infection any more. However, some types of body piercings are at a higher risk of injury and infection well after the piercing has healed. Earrings can rip accidentally from an ear lobe. Navel piercings can become irritated and raw from rubbing against clothing. If you have a piercing, always be aware of the signs and symptoms of an infection.

MULTIPLE HORDEOLA AND PRESEPTAL CELLULITIS FOLLOWING PERMANENT EYELINER TATTOO

Abstract

BACKGROUND: Permanent eyeliner tattoo (aka blepharopigmentation, micropigmentation) is a cosmetic procedure in which pigment is deposited along the eyelash line. Originally performed by physicians, the procedure is now commonly performed by non-medical personnel, e.g. cosmetologists. A variety of complications can arise from the procedure. A case of bilateral multiple hordeola and preseptal cellulitis following permanent eyeliner is presented.

CASE REPORT(S): A 28 year old Hispanic female presented with a chief complaint of severe pain, swelling, and redness of the upper and lower eyelids of both eyes. The symptoms had onset 3 days following application of permanent eyeliner, eyebrow, and lip pigmentation (tattoo). The patient had no significant ocular or medical history. Examination revealed visual acuity of 20/20 in each eye. Extraocular motility, color vision, and pupil function were normal. Body temperature was 98.2 F. Gross examination revealed bilateral eyelid edema and erythema, with swelling extending inferiorly through the right cheek. There were no apparent problems at the lip or eyebrow tattoo sites. Slit lamp examination revealed multiple external hordeola, and a clear conjunctiva, cornea, and anterior chamber. The optic nerve was normal, without disc edema, in each eye. The patient was diagnosed with multiple hordeola OU and preseptal cellulitis OD. She was treated with oral amoxicillin/clavulanic acid (Augmentin)for 10 days, with complete resolution of the condition.

CONCLUSIONS: Complications may occur following eyeliner “tattoo” procedures. Although most commonly cited are allergic/granulomatous reactions, other reported complications include eyelid necrosis, cicatricial entropion, and inoculation/infectious complications. The optometrist should be familiar with the potential risks involved with this procedure in order to counsel patients and to manage any complications that arise.

The Non-Healing Tattoo | Emergency Physicians Monthly

The Case: A 25-year-old male presented to the emergency department rapid care center complaining of an infection to his left lower extremity. He had a tattoo placed with red and green ink 2 weeks ago, and has noticed progressively worsening redness and pruritis developing, and a nonhealing wound with elements of the tattoo sloughing off. He denied fever, sore throat, nausea, vomiting, and shortness of breath, abdominal pain, hives and joint pain. The present discomfort is mild and somewhat pruritic. Scratching and pressure on site aggravate the area, and no relief was obtained with Keflex x 1 week.

The patient has not had similar symptoms in the past. There was no history of trauma prior to the placement of the tattoo.

The patient denies smoking, alcohol or drug abuse nor any past medical/surgical history or allergies.

On physical examination the patient was in moderate discomfort. His blood pressure was 151/51, pulse 90, respiratory rate 18, pulse oxygenation 98%, and temperature 97.2F. His pain scale was documented as 7/10.

The tattoo was a cartoon character located on his lateral left lower extremity wearing a hat and shirt in red dye ink, and other portions of the cartoon being in neon green ink. Erythema and mild edema were isolated to the borders of the tattoo and did not extend between the ink portions. The hat and shirt areas were open lesions, moist and tender with clear discharge. The areas with neon green ink was actively sloughing off and moist and tender. No fluctuance was noted.

Dx: Hypersensitivity to Tattoo Ink

Although an acute inflammatory reaction commonly occurs due to the soft tissue injury from the injected pigments, these usually resolve after several weeks. This patient had a hypersensitivity reaction to tattoo ink. It has been reported in the literature that tattoo allergy typically develops as a reaction to the ink in the tattoo (Hansan & Voutsalath, 2009) and most frequently is a contact dermatitis or a photoallergic dermatitis. The pigment dyes used for tattooing are prepared from metal salts. Red tattoo pigments, especially those made from mercury sulfide (cinnabar) cause the most frequent allergic reactions whereas yellow tattoo pigment (cadmium sulfide) is responsible for most photoallergic reactions. Other pigments utilized to create blue (cobalt aluminate), green (chromium oxide, lead chromate, or phthalocyanine) and black tattoos (logwood, iron oxide, or carbon) are much less common. Black tattoo pigment has been rarely reported to cause an allergic reaction. Phototoxic reactions may also occur in red tattoos due to the addition of cadmium, which is added in small amounts to intensify the red tattoo pigment.

Paraphenylenediamine (PPD) is a common allergen found in tattoo ink, temporary tattoo inks, and henna tattoos (Hansan & Voutsalath, 2009). Thimerosal is a mercury based chemical also reported to cause allergic contact dermatitis reactions in tattoos, however, mercury derived inks have been removed from the market due to their adverse side affects (Breithaupt & Jacob, 2008).

Allergic reactions tend to worsen over time; each exposure to the offending agent will cause an even worse reaction (Cowley, 2008). It is not uncommon for a patient to report having had previous tattoos with no complications, then reporting redness, nonhealing, pruritis with a recent addition of tattoo ink (Cowley, 2008). One person reported a new tattoo causing previously described symptoms in a new tattoo, as well as a 4-year-old tattoo with colored ink (Wiki, 2011). This person was treated with silver sulphadiazine dressings for 2 weeks with the older tattoo remaining intact with color and the new tattoo losing all color (Wiki, 2011).

The literature recommends acute treatment with antibiotic ointment, hydrocortisone creams, anti-itch creams and cold compresses; some dermatologists would also prescribed some form of systemic steroid for treatment (Cowley, 2008). Laser therapy is also an option in removing the offending agent by fragmenting the ink particles to a size where the body’s immune system can remove them (Cowley, 2008). Vitamin E, aloe vera and Qeurcitin (flavonoid) are also beneficial in aiding the immune system in rejuvenation and healing of skin (Cowley, 2008).

The wound cleansed with soap and water, and xeroform dressing applied to wound. The patient was referred to the dermatology clinic for a wound follow up as well as laser removal of the tattoo.

Tattoo Infection 101 – How Do You Know If Your New Ink Is Infected

You don’t have to roll around in the mud like a pig to get a skin infection. In reality, complications are common when you or your artist forget to use your noggins.

Most people think that avoiding a tattoo infection during the tattoo healing and aftercare process is as simple as avoiding trashy tattoo parlors, or by declining requests from ink-aspiring artists that go by the name of “friends.”

The truth is, it’s just not that simple!

While skin infections reported are rather small, there are still a handful of ways to get an infected tattoo unexpectedly. Most of which will probably surprise you, and make you go, “why didn’t I even think of that?”

Today, I’d like to share with you all the ways to avoid a nasty infection, alongside all the symptoms to watch out for. From serious to minor signs, I’m going to cover them all below. Additionally, I’m going to give you an insider look at what treatment is like and cover the right time to seek help from a medical professional.

If you’re worried your new ink is already infected, this guide is for you. If you want to avoid infections altogether, either now or in the future, this guide is for you!

With that said, let’s dive into the world of unfortunate mistakes and their unintended consequences.

 

What causes infected tattoos and how to prevent it from happening to you:

By far, the number one cause of infected tattoos today stems from one thing: Improper aftercare.

I highly suggest and urge you to read the definitive guide to tattoo aftercare. I give it away for free because I got so sick and tired of hearing about people doing the wrong things with their new ink! It is by far, the most comprehensive guide on the planet! Most importantly, one you can actually trust because I follow it step by step myself and continually update it with the times.

 

The general causes:

Old school habits of giving out terrible advice.

1. Let’s face it, not every single tattoo artist out there is going to be up to date with the latest and greatest in ink aftercare. They just aren’t. A fair number are still stuck in their old school ways and have been giving out the same (terrible) advice for years and years. Most are simply tattoo artists, not medical professionals.

Unfortunately, the medical community doesn’t operate in the same way as they do. Health experts, doctors, and dermatologists are always discovering new treatments and putting closes to countless old wives tales of the past.

For instance, years ago everyone once believed that the skin needed to “breathe” after it was wounded. By that I mean you’d take the bandage off and expose an open flesh wound to the air. After studying the subject intensively, everyone in the medical realized the idea of letting a wound breathe was absolute madness! It’s far better to keep it wrapped up for a whole host of reasons. Many of which I cover extensively in the aftercare guides I’ve written.

In reality, you’ll still find artists out there who tell clients to let their new tattoo breathe. In fact, some stress it like doing so is super important to the healing process. It’s the kind of nonsense that makes you go what the heck!

Forgetting to give out aftercare advice in the first place.

2. While bad advice is a surefire way to end up with a tattoo infection, your chances increase when you come across an artist who either forgets to explain the process to you, or simply tells you to not even worry about it.

Personally, I’ve gotten tattoos where the artist acted as if aftercare was no big deal at all. Had I not brought up the question of what to do afterward, I would have left assuming the large flesh wound on my body would just magically heal up on its own, with no issues! While the body is capable of repairing itself, there are countless ways to hinder that process and really shoot yourself in the foot.

 

The specific and actual causes:

You:

1. Not using bandages or plastic wrap to create an occlusive seal which protects your flesh wound.

2. Removing the bandage too early. You must wait at least 4 to 5 hours before peeling it off. It’s the bare and absolute minimum. Ideally, you want to aim for around 8 hours. Or, removing the bandage too late. After 24 hours problems can start to occur. (This won’t directly cause an infection but bad habits can play a role.)

4. Not washing after removing the bandage, or failing to loosen up and remove dried lymph and coagulated blood from the surface. You can really set yourself up for an infection if you don’t wash your hands first or use a dirty sponge or washcloth. Never use a sponge or washcloth, only your hands. Your hands should be deep cleaned, with the knuckles, palms, fingernails, fingertips scrubbed and spotless. You must always, use soap!

5. Taking long showers and baths will make the healing process more difficult. However, things like swimming in pools, hot tubs, ponds and lakes are surefire ways to actually getting an infection.

6. Drying with a dirty hand towel or bath towel. Always ensure you are using fresh, just out of the washer and dryer towel or a brand new roll of paper towels to pat dry. Remember, an old towel that’s lying around could have been walked over by your dog, brushed up against the toilet, or used after someone failed to wash their hands thoroughly.

7. Allergic reactions can be confused with infections when it comes to using ointment. In some cases, using Neosporin for example, can be toxic when the body rejects it. It makes for a fatal situation that isn’t just red bumps popping up, it’s you popping into the hospital door for help.

8. Failing to keep your new tattoo clean. It’s critical that you wash, every couple of hours for the first three days. With soap, not just water! If you’re not sure which soap to use, try Tattoo Goo Deep Cleansing Soap. It’s specially designed for cleaning tattoos and is antibacterial, antimicrobial and pH-balanced.

9. Using expired or contaminated lotion. Check the expiration date on your bottles before applying any type of product to your skin. Believe it or not, but lotion isn’t just lotion in the sense. The ingredients in the formula can spoil and bacteria/mold can begin to form. Manufacturers can make accidental mistakes at their factories too.

10. For foot tattoos, infection can occur when wearing no socks or protective wrap with old, musty shoes. Consider all the times you tied your laces without washing your hands, or how often they’ve slid across dog poop in the grass.

11. Wearing dirty or soiled clothing. Remember, it’s going to rub up against your flesh wound. If you’re not protected, it’s going to invite bacteria in.

12. Exercising with a new tattoo a gym and failing to realize that an arm tattoo for instance, may rub up against a machine that thousands of other people have sweated all over and rubbed against. Some of which may have not taken a bath in a long time. If you don’t want to wear a long-sleeved t-shirt at the gym, try wearing a tattoo protection sleeve like this one from Tough Outdoors.

13. A dirty home. More specifically, dirty pillows and bed sheets. So many people forget to wash these after coming home with new ink! If you live with dogs you know how dirty bedding can get. Remember to always use the steam option on your washer/dryer if you have it, as it will cut back on any stubborn leftover bacteria.

14. Showing off your new tattoo to friends and actually letting them touch it! You don’t know where their hands have been at or what they have touched! In some cases people will peel back the wrap without realizing they just held their phone a moment earlier. Remember, your phone is swamping with bacteria, don’t do that!

15. Being in places or around people that are either ill or dirty. That could mean washing the dishes at a sink, going into a unsanitary public place, or perhaps scrubbing toilets.

16. Picking at scabs or scratching excessively.

 

Your artist:

For the most part, tattoo artists today are clean, abide by various health and safety regulations and don’t wish for anyone to get an infection. They want to make your experience as smooth as possible. While it’s possible for them to make mistakes from time to time, unless you’re in a really disgusting/trashy tattoo parlor, most of the time infections are caused by you, not your artist!

1. Failing to keep their workspace clean. If their shop has dirty floors, countertops with disgusting clutter, and a soiled chair run out the door!

2. Not performing any spore testing, or failing to do it at least once a month. Failing to use an Autoclave or Statim to sterilize their tools. Don’t be afraid to request the most recent test!

3. Failing to unseal a needle in front of you. Never let an artist use a needle that isn’t sealed or packaged.

4. Failing to pour pigments from single-use plastic vessels, or using pigment stored and removed from an unsanitary area. In rare cases, ink can be sealed, yet still contaminated from manufacturing errors unrelated to your artist.

It happens: A study in Europe examined 58 sealed bottles and found 10% were contaminated with bacteria known as a “mycobacterium chelonae,” alongside 17% of previously opened bottles. Granted, this does not happen every day, nor often enough to panic over.

5. Not thoroughly washing their hands/using gloves. Or simply eating, drinking, handling their phone, or touching pens and pencils, etc. while inking.

6. Using a dirty towel or dirty paper towels to remove oozing plasma or blood while inking.

 

Warning Signs of An Infected Tattoo And Symptoms

1. Overly Itchy: During the healing process you’re going to itch, especially when you start the peeling phase. Relax, it’s normal. If lotion and proper moisturizing aren’t working for you, then hydrocortisone cream (short term) may do the trick for the worst case scenario. However, if that doesn’t work, or the itchiness continues to persist at an uncomfortable level consult a health care professional.

2. Oozing: A bit of clear plasma and a little bit of red blood will happen to ooze out for a rather short period. It’s normal. However, when you see yellow, brown or green tinted slime or pus discharging from your flesh wound, you’ll need to stop and see a doctor immediately. While you might think it’s the color of the ink pigment leaking out, it’s not. Even in tiny amounts, you should take any discolored oozing seriously.
Oozing may add another symptom to take note of: Foul odor.

3. Tenderness and Pain: Your new ink will be a tad sore, it’s a flesh wound after all. That’s quite normal. If you start to feel deep pain or intense sunburn like surface pain that stings beyond your comfort level, go to the doctor. Even if you do not notice any oozing or other symptoms, pain can often be a good indication of an infected tattoo.

4. Red streaks: A minor redness for a short period is normal. However, red streaks, on the other hand, can indicate an infection that’s spreading around. It’s a classic, tell-tale sign to take note of. If you don’t notice streaks, you may see instead, a rash that seems to spread larger and larger. The rash can appear red and inflamed. However, an infection can be present even without immediate scabbing on the surface.

5. Swelling, Blistering, and Boils: Small bulges can indicate significant future problems. When a tattoo gets infected, the skin can balloon up like a pimple and fill with puss that will need to be drained by a medical professional. When the infection is dreadful, it can flow out like a rushing river from all the buildup.

7. Warmth: More than often it’s accompanied by other symptoms which are more noticeable like the red streaks for example.

8. Systemic Infection: It means the entire body is affected by things like weakness, fever, tiredness, or muscle aches, and so on. Take sepsis for example, which happens when chemicals are pumped into the bloodstream to ward off an infection. It triggers full body inflammation and starts a chain of reaction, which can lead to things like organ failure or worse. Understand that just because your flesh wound is in one single spot, doesn’t mean that your entire body is somehow going to be magically unaffected!

Other reactions: Let’s face it, allergies happen. Some of which have delayed reactions or immediate reactions. Things like cement dermatitis, eczematous eruptions, parakeratosis, and keloids are a reality for some people. Not to mention, things like hematoma, where a blood vessel getting punctured can leave things like scary looking bruises. To the untrained eye, simply eye-balling new body art issues is not going to paint a full picture. It won’t determine if something’s truly an allergic reaction or bacterial infection; it will just give you clues. Ultimately, a doctor can put together a puzzle from those clues, and give you the correct answer as to what’s really going on.

What does an infected tattoo look like?

When it comes to infected tattoos, treatment isn’t something you can DIY. Period! You must go to a hospital or doctor to restore your skin back to a clean bill of health. I don’t care if you think it’s too expensive, too time-consuming, or if you’re afraid of looking like an idiot who overacted. Trust me; you won’t!
