Indications for the use of the drug NICORETTE ^®

For the treatment of tobacco dependence:

– reduction of withdrawal symptoms that occur with the complete cessation of smoking in patients who decide to quit smoking;

– with a temporary cessation of smoking;

– to reduce the number of cigarettes smoked by those who cannot or do not want to completely stop smoking.

Pharmacological action

Drug for the treatment of nicotine addiction. After abruptly quitting smoking, patients who use nicotine-containing products daily for a long time may develop a withdrawal syndrome, which includes dysphoria, insomnia, irritability, anxiety, impaired concentration, decreased heart rate, increased appetite and weight gain, and also desire to smoke.

In the treatment of nicotine dependence, nicotine replacement therapy can reduce the need for smoking (and the number of cigarettes smoked), and reduce the severity of withdrawal symptoms that occur with complete smoking cessation in those who decide to quit smoking. Facilitates temporary abstinence from smoking and helps to reduce the number of cigarettes smoked in those who cannot or do not want to completely stop smoking.

Pharmacokinetics

Absorption

Chewing gum nicotine is rapidly absorbed through the buccal mucosa and is detected in the blood after 5-7 minutes.

The distribution of

V _d nicotine when administered intravenously is about 2-3 l/kg. The binding of nicotine to plasma proteins is less than 5%. In this regard, violations of nicotine binding with the simultaneous use of other drugs or changes in the amount of protein in the plasma in various diseases should not have a significant effect on the kinetics of nicotine.

Metabolism

Nicotine is metabolized in the liver, kidneys and lungs. More than 20 metabolites have been identified that are inferior in activity to nicotine. The concentration of the primary metabolite – cotinine – exceeds the concentration of nicotine by 10 times.

Excretion

Excreted mainly by the liver. The average plasma clearance is about 70 l/h. T _1/2 – about 2 hours1/2 – 15-20 hours) and trans-3-hydroxy-cotinine (45% of the dose). 10-30% of a dose of nicotine is excreted in the urine unchanged.

Dosing regimen

Nicorette ^® should be used whenever there is an overwhelming desire to smoke. The gum should be chewed slowly until a sharp taste appears, then chewing should be stopped and the gum should be placed between the gum and the inside of the cheek. After the taste disappears, chewing should be resumed. For 1 reception, you should use only 1 chewing gum.

With complete cessation of smoking the average daily dose is 8-12 gum, the maximum is 15 gum. It is necessary to continuously use the drug in the first 3 months after quitting smoking. Then you should gradually reduce the amount of gum consumed. When the daily intake of the drug drops to 1-2 gum, its use should be discontinued.

When reducing the number of cigarettes smoked chewing gum should be used between smoking episodes to lengthen the intervals between smoking cigarettes in order to reduce cigarette consumption as much as possible. Overdose

Excessive intake of nicotine from replacement therapy and/or smoking may cause overdose symptoms.

Drug interactions
