Urethritis is infection of the urethra, the tube that carries urine from the bladder out of the body.

  • Bacteria, including those that are sexually transmitted, are the most common cause of urethritis.

  • Symptoms include pain while urinating, a frequent or urgent need to urinate, and sometimes a discharge.

  • Antibiotics are usually given to treat the infection.

Urethral Discharge in Females | 8 Causes for Urethritis in Women

Symptoms of menopause

Menopause is the name for the natural process by which the menstrual cycle (period) stops happening in a woman. Usually, the process is gradual (takes months or years) and occurs from the age of 45 to 55 years. Menopause is officially diagnosed once a woman stops having a period for 12 months continuously. A woman with menopause will notice a decrease in the number and regularity of her periods until they completely stop. In addition, she may notice a number of symptoms that occur as a result of decreased estrogen levels, such as hot flashes, changes in mood, sleep problems, vaginal dryness, changes in libido, and changes in sexual function. Certain medications exist that can decrease these symptoms.

Rarity: Common

Top Symptoms: fatigue, delay in or irregular periods, vaginal discharge, anxiety, trouble sleeping

Symptoms that always occur with symptoms of menopause: delay in or irregular periods

Urgency: Self-treatment

Yeast infection

A vaginal yeast infection, also called genital/vulvovaginal candidiasis, is actually caused by the fungus Candida albicans and is very common. The organism is a normal inhabitant of the mouth, gastrointestinal tract, and vagina.

Antibiotics can kill off the healthy bacteria in the vagina, allowing overgrowth of the fungus. Women who are pregnant, on the birth control pill, or diabetic are more prone to yeast infections, as are those who have weakened immune systems. It can also be transmitted through sex or through mouth-to-genital contact.

Symptoms include itching, burning, pain, and soreness inside the vagina and on the external tissues (the vulva,) and a thick, white vaginal discharge.

If not treated, the yeast infection can become “complicated,” severe, and difficult to cure.

Most yeast infections are diagnosed simply through the patient’s description of symptoms. Recurrent infections may be diagnosed through pelvic examination and vaginal swab.

Treatment often is just an over-the-counter cream, though oral anti-fungal medications are sometimes prescribed.

Vaginal trichomonas infection

Trichomonas vaginalis infection (or “Trichomoniasis” or “trich”) is a common sexually transmitted disease caused by a parasite of the same name. It affects over 3 million people per year, but only about 30% have any symptoms.

You should go see your primary care doctor in the coming days. He or she may order a number of tests to identify if the bug is actually the cause of your symptoms. Treatment involves prescription metronidazole or tinidazole.

Urinary tract infection

A urinary tract infection, or UTI, can involve any or all parts of the urinary system but most often affects the bladder and urethra.

Bacteria from the gastrointestinal tract – especially Escherichia coli (E. coli) – are the most common cause of UTIs. These bacteria spread from the anus to the urethra. Sexual activity can do this, but a UTI is not considered a sexually transmitted disease.

Women are more at risk for UTI than men. Due to female anatomy, the urethral opening is a short distance from the anus. Anyone who uses catheters to urinate is also prone to UTIs.

Common symptoms of less-serious UTIs include lower abdominal discomfort and pressure; burning or discomfort on urination; and cloudy or discolored urine.

Left untreated, the infection could spread to the kidneys and cause a medical emergency.

Diagnosis is made by having the patient describe the symptoms and by testing a urine sample for bacteria.

UTIs are caused by bacteria and so can be treated with antibiotics.

Prevention involves good hygiene and drinking plenty of water.

Symptoms of menopause

Menopause, or “change of life,” refers to the time when a woman no longer has menstrual cycles and can no longer bear children.

It is a normal occurrence and usual happens between ages 45 to 55. Menopause can be artificially induced by surgical removal of both ovaries, and by chemotherapy and/or radiation therapy for cancer.

Symptoms usually begin many months before periods actually stop. There will be irregular periods, vaginal dryness, hot flashes, weight gain from slower metabolism, and dry skin.

If not treated, some symptoms may affect quality of life. Hot flashes and hormonal imbalances can disrupt sleep, sexual function, and emotional health.

At menopause, risks of heart disease, osteoporosis, and urinary incontinence increase. Because periods can become irregular while pregnancy is still possible, testing is advisable before any medical treatment is done.

Menopause is diagnosed when an entire year has gone by without the woman experiencing a menstrual period. Blood testing for hormone levels can confirm menopause.

Treatment can be done for any troublesome symptoms, including hormone replacement therapy to ease hot flashes.

Pelvic inflammatory disease

Pelvic inflammatory disease, or PID, is the general term for a bacterial infection of a woman’s reproductive organs.

PID is most often a complication of a sexually transmitted disease (STD) such as gonorrhea or chlamydia. However, it is possible to get PID from other causes.

Any woman can be affected. It is most often found in sexually active women under age 25, especially those who have had PID before, have multiple partners, and/or douche frequently.

Symptoms include fever, lower abdominal pain, foul-smelling vaginal discharge, pain and/or bleeding during sex, and pain on urination.

Untreated PID can cause infertility due to damaged tissue in the reproductive tract, as well as chronic pelvic and abdominal pain. Unprotected sex partners will be infected as well.

Diagnosis is made through symptoms, pelvic examination, vaginal and cervical swabs, and urine tests.

Treatment is with a course of antibiotics. Be sure to finish all of the medication as directed, even when you begin feeling better.

To prevent PID, have all partners (male or female) tested for STDs and avoid unprotected sexual contact.

Rarity: Common

Top Symptoms: fever, abdominal pain or unusual vaginal discharge, vaginal discharge, nausea or vomiting, vaginal bleeding, pelvis pain

Symptoms that always occur with pelvic inflammatory disease: fever, abdominal pain or unusual vaginal discharge

Urgency: In-person visit

Normal case of vaginal discharge

It is perfectly normal for every woman to have a clear or thin white vaginal discharge which is more or less constant.

The body protects the vaginal tissues by producing this light mucus from glands in the cervix and in the vaginal walls. This keeps the tissues lubricated so that they do not dry out and become irritated, and keeps the tissues slightly acidic because that helps to kill off any harmful germs.

The discharge is also a cleaning mechanism, clearing away any dead cells or bacteria as it moves out of the vagina. Douching is not necessary for normal discharge.

Vaginal discharge may change at different stages of life. During pregnancy, it becomes white and milky in appearance.

During and after menopause, the discharge lessens due to the drop in estrogen levels. If the dryness causes irritation or difficulty with sexual activity, a gynecologist can recommend an appropriate remedy.

Normal vaginal discharge never causes itching or has a foul smell, and is never any color other than clear or white.

Rarity: Common

Top Symptoms: vaginal discharge, mild vaginal discharge, white/gray vaginal discharge, clear vaginal discharge, severe vaginal discharge

Symptoms that always occur with normal case of vaginal discharge: vaginal discharge

Symptoms that never occur with normal case of vaginal discharge: vaginal itch or burning, painful urination, severe vaginal discharge, vaginal pain, abdominal pain (stomach ache), bleeding after sex, missed period, vulvovaginal odor

Urgency: Wait and watch

Disseminated gonococcal infection

Gonococcal cervicitis is an inflammation of the cervix – the passageway at the lower end of the uterus – caused by the bacteria Neisseria gonorrhoeae, or gonorrhea.

Gonorrhea is a sexually transmitted disease (STD) and spreads through unprotected sexual contact.

Symptoms include abnormal vaginal discharge, pain or discomfort during sex, and vaginal bleeding after sex. However, some women have few or no symptoms.

If not treated, gonococcal cervicitis can lead to further infection of the reproductive tract and to pelvic inflammatory disease, which can cause sterility.

Diagnosis is made after a cervical swab is taken and tested.

Treatment is through a course of oral antibiotics. Women diagnosed with gonococcal cervicitis should be further tested for other common STDs such as chlamydia and trichomoniasis, as they are often found at the same time.

The best prevention for gonorrhea is the use of a condom during sex, as well as testing of all sex partners so that they can be treated and not re-infect anyone.

Cervicitis in general can be prevented by not exposing the cervix to douching or other irritants.

Rarity: Ultra rare

Top Symptoms: fatigue, joint pain, fever, chills, moderate fever

Urgency: Primary care doctor

Chlamydia infection

Chlamydia is a common bacterial infection that is spread through unprotected sex. Each year, over 1 million Americans are diagnosed with this STD.

You should visit a physician to confirm the diagnosis where an antibiotic will be prescribed.

Bacterial vaginosis

Bacterial vaginosis is an inflammation of the vagina due to overgrowth of the bacteria which are normally present.

It is not considered an STD (sexually transmitted disease) but nearly all cases are found in women after unprotected sexual contact, especially with multiple partners.

Frequent douching is also a factor.

Male partners do not carry this condition, but it can spread between female partners.

Common symptoms include an itchy, foul-smelling discharge that may look grayish or greenish, as well as burning during urination. However, some women have no symptoms.

The greatest risks of bacterial vaginosis are secondary. The symptoms can be similar to actual STDs and so should not be ignored. This condition makes a woman more vulnerable to actual STDs, as well as to pelvic inflammatory disease and to infections following any gynecologic surgery.

Pregnant women with bacterial vaginosis are at risk for premature or low-birth-weight babies.

Diagnosis is made through symptoms, and/or pelvic exam and vaginal swab.

Treatment consists of oral medication, and sometimes a cream or gel that is inserted into the vagina.

Rarity: Uncommon

Top Symptoms: vaginal itch or burning, vulvovaginal odor, bloody vaginal discharge, white/gray vaginal discharge, thick vaginal discharge

Urgency: Primary care doctor

Atrophic vaginitis

Atrophic vaginitis is a condition causing dryness of the vagina. It is caused by a drop in estrogen levels, which happens due to menopause, stress, medication side effects, and childbirth. Other symptoms include painful sex and vaginal soreness.

You should consider visiting a medical professional to discuss your symptoms. Atrophic vaginitis is evaluated with a review of your symptoms and a pelvic exam. Once diagnosed, it can be treated with estrogen replacement, moisturizers, and lubricants. Avoiding douching and perfumes may help dryness from worsening.

Normal discharge from the penis in men is considered to be clear discharge during an erection. This phenomenon is called physiological urethrorrhea and is a product of the activity of the urethral glands. White, yellowish, frothy or purulent discharge indicates some kind of pathological process. By color, smell, consistency, one can judge the causative agent of the infection.

Important!

The information in this article should not be used for self-diagnosis or self-treatment. For the correct diagnosis and treatment, you should always consult a doctor.

IMPORTANT! Sexually transmitted infections are the fate of people who frequently change sexual partners. Not all of these infections have pronounced symptoms, which is why they are called latent infections. Therefore, when changing partners, it is recommended to be tested for sexually transmitted diseases, even if there are no obvious signs of infection.

In most cases, unusual mucus is a sign of a sexually transmitted disease or sexually transmitted infection. The incubation period is 4 to 15 days after infection. Additional symptoms in some cases (see below) are redness of the head of the penis, itching, burning. When the infection spreads to the urethra, normal urination is disturbed: it becomes painful, frequent, difficult. There may be pain in the testicles, penis, pain in the lower abdomen.

Bloody discharge from the urethra indicates hematorrhoea, a non-inflammatory disease caused most often by mechanical damage to the urethra. Such an injury can occur, for example, during an instrumental medical examination of the penis.

Sperm discharge from the urethra without sexual intercourse or spermatorrhea is a sign of a weak tone of the vas deferens, which in turn occurs as a result of chronic inflammatory processes.

White discharge , milky purulent-mucous nature, which is a mixture of leukocytes, mucus and serous exudate, is a symptom of non-gonococcal urethritis, that is, urethritis caused by non-gonococcus. Most often found in chlamydia, ureaplasmosis, mycoplasmosis. With such discharge, no other symptoms are observed – cramps, pain, itching.

White frothy or purulent discharge from the urethra, sometimes with an admixture of blood in the urine or streaks of blood in the semen, is noted in the inflammatory process caused by trichomoniasis. Additional symptoms of infection: burning, pain, feeling of heaviness, discomfort in the perineum or deeper in the pelvic region. Such symptoms are not always found in trichomoniasis.

FACT: According to WHO, more than 250 million patients with trichomoniasis are registered annually in the world.

Yellow, yellow-green, yellow-brown purulent discharge, thick, consisting of a mixture of epithelium, mucus and leukocytes is the main symptom of gonorrhea (gonococcal urethritis). With purulent discharge, pronounced subjective symptoms are noted: sharp pains, pain, itching.

FACT: According to statistics, in the last few years, the incidence of asymptomatic gonorrhea has increased to almost 90%.

White, greenish and yellow mucous discharge, also occurs in inflammatory diseases: epididymitis, orchitis, vesiculitis, prostatitis in acute form.

Other changes

Along with the discharge from the urethra, there may also be a change in the shape of the urethral opening, the so-called “sponges”. Depending on how acute the disease is, the intensity of changes also differs. Patients experience edema, swelling, as well as a change in the color of the urethral opening, redness on the skin and some other symptoms.

Who treats?

With any of the symptoms listed above, you need to contact a urologist. The list of mandatory studies to determine the type of infectious agent includes tests for latent infections. The sooner the diagnosis is established and therapy is started, the higher the likelihood of a quick and easy cure.

Doctor Rylchikov Ivan Vladimirovich

I want to express my deep gratitude to the doctor Rylchikov Ivan Vladimirovich. In May 2020, I contacted him about testicular pain. Ivan Vladimirovich immediately identified a malignant neoplasm, determined that the tumor was localized within the organ, gave emergency recommendations for treatment, chose the right words so that I would not panic, but act. As a result, in August 2020, I underwent surgery, underwent three courses of chemistry and now I am completely healthy!!!!!!!!!!!!!!! Thank you Ivan Vladimirovich, you saved my life.

Congratulations on the upcoming holidays to all the personnel of the Altermed division on Prosveshcheniya avenue, in particular Ivan Vladimirovich Rylchikov, the personnel of the treatment room, administrators. In this unit, I encountered a friendly and correct attitude of people who know and love their job. Good luck and happiness in the coming year, good health, great mood. THANK YOU!

What can you say? A DOCTOR with a capital letter, who knows his business at 5+, did not regret that he made an appointment with him. You can write a lot, but his deeds usually speak for the doctor, and the patient’s condition, at the moment there is a clear improvement in well-being. I am sure that subsequent visits will solve the problem.
This, in principle, is not a criticism, but a wish, I am completely satisfied with the quality of service in Altermed, friendly staff, a doctor who knows his business perfectly, improving well-being, this is all great, of course, but the prices of course for some categories of people are either simply not affordable, or they really force strain your budget, why not, for example, make discounts for pensioners? After all, our pensions, alas, do not allow us to relax. Of course, you also have a discount card, but, alas, it cannot be a significant help to the poor, especially since in addition to paying for doctor’s appointments and procedures, you also have to buy medicines.

Doctor Rylchikov Ivan Vladimirovich

I have been seeing a doctor for right-sided pyeloctasia and chronic pyelonephritis for the fourth year already. During therapy, there was not a single attack and exacerbation. I know that in my case only surgery will help, but so far nothing bothers me, I prefer therapy. After the operation, a difficult rehabilitation is ahead, so I’m delaying it as much as I can. Ivan Vladimirovich is very helpful.

Doctor Rylchikov Ivan Vladimirovich

Hello! I want to say a big thank you to the urologist Rylchikov Ivan Vladimirovich for the correct diagnosis and the appointment of a successful treatment. And also to the nurses of the treatment room for their attentiveness and professionalism.

Wonderful clinic, polite and friendly staff, reasonable prices.

Advice for those who are just thinking about whether or not to go to a urologist? Of course, go! It’s up to you to go to this clinic or another, but the sooner you go, the easier it will be to treat inflammation, incipient prostatitis, etc. I understood this, and now I will go to the urologist regularly, although before that the last time I visited a urologist was more than 10 years ago.

Thanks again to all Altermed employees.

Sincerely, Alexander (37 years old)

Doctor Rabinyuk Sergey Anatolyevich

From the bottom of my heart I would like to thank the urologist Rabinyuk Sergey Anatolyevich. In February 2020, by intuition, I signed up for this doctor and I don’t regret it at all! I knew right away that he was a professional. He identified all the problems, prescribed treatment (he didn’t prescribe anything superfluous), said which pills / injections are not required, but would not interfere. And cystitis, as it was not! Finally, I started to live a normal life and stopped looking for latrines everywhere, sorry for the details, but I write as it is. Therefore, if you are concerned about cystitis, then I sincerely advise you to make an appointment with Sergey Anatolyevich! I will definitely not go to another doctor)

Urethritis

Urethritis in men and women: causes, symptoms and signs, effective methods of diagnosis and treatment

Urethritis is an inflammation of the urethra (urethra). Urethritis can occur in both women and men. Most often, infection of the urethra (urethra) occurs during sexual contact. Urethritis can occur against the background of various sexually transmitted diseases, and is often the main manifestation of a sexual infection. The main symptoms of urethritis are pain or burning when urinating and discharge from the urethra (urethra).
Treatment of urethritis is carried out depending on the cause of the disease and includes antibiotics, hygiene measures.

What is urethritis?

Urethritis is an inflammation of the urethra. Most often, due to the anatomical features of the structure of the urethra, urethritis occurs in women (the urethra in women is wider and shorter, as a result of which it is easier to become infected).
In urethritis, the walls of the urethra and its glands become inflamed. In the lumen of the urethra, purulent or bloody discharge accumulates, which are released drop by drop when pressing on the urethra or during urination.

What are the causes of urethritis?

As mentioned above, urethritis is an inflammation of the urethra, which occurs as a result of its infection with various microbes. Depending on the cause of urethritis (causative microbe), specific and nonspecific urethritis are isolated.
The cause of specific urethritis is certain sexually transmitted infections (STDs): gonorrhea, herpes, chlamydia, urogenital mycoplasmosis, trichomoniasis. These forms of urethritis are called specific, since they occur with some characteristic features for each case and require special treatment.
Nonspecific urethritis is usually caused by streptococci, staphylococci, E. coli and other bacteria. Regardless of the causative agent of the disease, nonspecific urethritis always proceeds in the same way and is treated according to a certain scheme.

Risk factors that contribute to the development of urethritis

Due to the fact that the urethra is located in close proximity to the anus, which serves as a constant source of microbes (causative agents of nonspecific urethritis), infection in the urethra is observed quite often, but infection alone is not enough to develop urethritis. Generally, in healthy people, immune defenses in the urethra block infection and prevent urethritis from developing. There are a number of factors that can reduce the immune defense of the whole body or immunity at the level of the urethra. It is these factors, in combination with infection, that contribute to the development of urethritis. As a rule, urethritis occurs against the background of:

  • Genital injuries (most often during intercourse)
  • Hypothermia of the human body (especially the perineal and genital areas)
  • Sexual promiscuity (a large number of sexual partners increases the risk of contracting a sexual infection)
  • Improper diet, chronic overwork, stress, lack of vitamins (reduce the body’s immune defenses)

Symptoms and signs of urethritis

The first signs and symptoms of urethritis appear several hours or days after infection of the urethra with microbes. Symptoms of urethritis in men and women have their own characteristics.

Peculiarities of symptoms of urethritis in men

The main symptoms of acute urethritis in men are:

  • Burning (itching) when urinating. Due to the anatomical features of the urethra in men, the burning sensation in urethritis in men is more pronounced than in women
  • Pain when urinating
  • Discharge from the urethra (purulent or bloody). As a rule, discharge with urethritis occurs in the morning. Most often, with urethritis, abundant discharge of a blue-green or yellowish color with an unpleasant odor is observed.
  • Adhesion of the external opening of the urethra (mainly in the morning)

Acute urethritis is dangerous because sometimes (in mild cases) it occurs without obvious symptoms.
Often, with urethritis in men, the infection spreads to neighboring organs with the occurrence of prostatitis, pyelonephritis, cystitis.

Features of symptoms of urethritis in women

The main symptoms and signs of urethritis in women include:

  • Discharge from the urethra (the nature of the discharge depends on the causative agent of urethritis, most often there is a greenish or white-yellow or bloody discharge with an unpleasant odor)
  • Pain in the lower abdomen. As a rule, pain in urethritis in women is localized in the lower abdomen. Pain with urethritis in women, constant low intensity
  • Burning (itching) in the urethra, redness of the outlet of the urethra.

Most often, the symptoms of urethritis in women appear hours or days after sexual intercourse.
In view of the anatomical features of the structure of the urethra in women already described above, their urethritis is often associated with cystitis. Read more about the symptoms and treatment of cystitis in the Cystitis section.

General symptoms of urethritis

In acute urethritis, both men and women do not have an increase in body temperature, a decrease in appetite, weakness.

If left untreated, urethritis can become chronic. The main reasons that lead to severe urethritis include:

  • Mistreatment of acute urethritis
  • Spread of infection to neighboring organs. In acute urethritis, very often the infection in men spreads to the prostate gland (prostatitis), the penis. In women, as a rule, an infection with urethritis can go to the glands in the labia (Bartholin’s glands)
  • Weakening of immunity (chronic infections, stress, overwork)

Candidal urethritis

Candidal urethritis occurs as a result of infection of the urethra with yeast fungi against the background of thrush.
Symptoms and signs of candidal urethritis are usually not very pronounced (itching or burning in the urethra, scanty cheesy white discharge from the urethra), discomfort felt in the lower abdomen and deep in the perineum.
Treatment of candidal urethritis is reduced to the use of antifungal drugs (ketoconazole, miconazole, etc.).

Diagnosis of acute urethritis

As a rule, 1 examination is enough for a patient to make a diagnosis of urethritis. To determine urethritis in a patient, a sample is taken, and then the sowing of discharge from the urethra. As a result of this examination, the cause of urethritis is identified, and the correct treatment is prescribed. Diagnosis and treatment of urethritis are carried out by urologists, gynecologists and andrologists.

Chronic urethritis

With inadequate or untimely treatment, urethritis can become chronic (chronic urethritis). As a rule, chronic urethritis is characterized by the same signs and symptoms, but less pronounced than acute urethritis (see above).
Chronic urethritis can last for quite a long time (several months, years) and can lead to narrowing and deformation of the urethra (urethral structure). In the presence of stricture (narrowing) of the urethra in chronic urethritis, most doctors will offer to perform bougienage (expansion) of the urethra with a special probe. This procedure is painless and can replace a complex operation to restore the patency of the urethra.

Treatment of urethritis in men and women

If you have symptoms and signs of urethritis, you should consult a doctor, self-treatment of urethritis is unacceptable.
First of all, with urethritis, it is necessary to follow a diet (spicy and salty foods, as well as alcohol, smoking, are excluded from the diet). In addition, with urethritis, patients should drink plenty of fluids (juices, cranberry juice).
Various drugs for the treatment of urethritis in men and women are used in the form of tablets and injections (shots). The main principles of treatment of patients with urethritis include:

  • Antibiotic therapy (removal of the cause of the disease)
  • Topical treatment of urethritis
  • Improving the immunity of the human body

Medicines for urethritis

The main direction in the treatment of urethritis in men and women is the use of antibiotics. The appointment of antibiotics by a doctor is carried out taking into account the causative agent of urethritis. The dose of drugs for urethritis is selected by the doctor individually.
If the cause of urethritis in men and women is gonorrhea (or chlamydia), then Doxycycline + Deoxyribonuclease (25 mg for 6 days) is prescribed.
