REPORT

October 05, 2017 for doctors – dermatovenerologists, cosmetologists and doctors of other specialties, in accordance with the order of the Minister of Health of the Samara Region dated 01.10.2017 . No. 776-r, the public organization “Samara Regional Association of Doctors” together with the Ministry of Health of the Samara Region and the Samara State Medical University held a scientific and practical conference on the topic: “Order” for the provision of cosmetic care to the population of the Samara Region. Standards, protocols and algorithms of modern therapeutic approaches in the practice of dermatovenereologists and cosmetologists” within the framework of the twenty-eighth educational week, as well as the XII specialized interregional exhibition “Sharm-PROFI”.

The chief freelance specialist of the Ministry of Health of the Samara Region in dermatovenereology and cosmetology, chief physician of the Samara Regional Dermatovenerologic Dispensary, Doctor of Medical Sciences, Professor Shakurov I.G.

Orlov E.V. , Doctor of Medical Sciences, Professor, Head of the Department of Skin and Venereal Diseases, Samara State Medical University of the Ministry of Health of Russia, spoke about professional retraining, advanced training for a cosmetologist.

Then there were 3 reports.

1. “Adjuvant therapy of acne as an integral part of the treatment protocol”

Kruglova Larisa Sergeevna, Professor, MD, Head of the Department of Dermatovenereology and Cosmetology of the Central State Medical Academy of the UDP RF.

Acne is a chronic disease in which the skin barrier is damaged. Most acne medications address up to 3 of the 4 pathogenic causes of acne. To restore the skin barrier, care for the skin and minimize the adverse effects of acne drug therapy, it is necessary to use specialized concomitant therapy products that provide gentle cleansing, sebum control and an acceptable cosmetic effect. It is important that these products are non-comedogenic, well tolerated and have a balanced composition.

2. “Modern aspects of topical therapy of steroid-sensitive dermatoses in accordance with clinical guidelines”

Matushevskaya Elena Vladislavovna, MD, Professor of the Department of Dermatovenereology and Cosmetology FSBEI DPO IPK FM BA.

More than 50 years have passed since the Nobel Prize Ph. Hench and E. Kendall for introducing glucocorticosteroid hormones into clinical practice. The pronounced anti-inflammatory effect and high efficiency have made this group of drugs one of the most commonly used drugs.

The development of glucocorticosteroid therapy has come a long way from the creation of the first topical preparations with a large number of side effects to the synthesis of steroids with high efficiency and minimal local adverse reactions. In this regard, of particular interest is 0.1% methylprednisolone aceponate, which has been used in clinical practice since 1994.

Methylprednisolone aceponate belongs to the medium strength class. This is a new generation non-halogenated topical glucocorticosteroid with a proven combination of high activity and minimal risk of systemic and local adverse reactions, even with long-term use. The almost complete absence of systemic effects is due to the insignificant penetration of the drug from the skin into the blood, a short half-life in circulating blood, as well as strong binding to the transport protein transcortin and subsequent inactivation in the liver.

Of all existing glucocorticosteroids, methylprednisolone aceponate has the lowest therapeutic index, that is, a good ratio of the risk of side effects and therapeutic efficacy. The drug does not affect the level of endogenous cortisol, which is especially important in pediatrics, since children have a higher absorption of drugs due to a larger ratio of skin area to body weight. In addition, they have a weaker system of metabolism of exogenous steroids than adults.

Practitioners will be interested in a new line of domestic products based on 0.1% methylprednisolone aceponate for topical therapy of steroid-sensitive dermatoses: within the framework of import substitution programs.

3. “The role and place of antihistamines in the treatment of allergic dermatosis”

Matushevskaya Elena Vladislavovna, Doctor of Medical Sciences, Professor of the Department of Dermatovenereology and Cosmetology FGBOU DPO IPK FMBA.

Antihistamines (AHPs) are widely used in dermatology and are first-line drugs for many diseases. These dermatoses (dermatitis, urticaria, pruritus, eczema, scabies, skin lymphomas), despite differences in pathogenesis and clinical manifestations, are united by one symptom – itching.

Itching is defined as a sensation that causes a conscious or reflex desire to scratch. The sensation of itching is directly related to the skin and to the production of humoral factors, the spectrum of which is different in different pathologies. In many dermatoses (contact dermatitis, toxicoderma, urticaria), itching is caused by the release of histamine. Accordingly, antihistamines are the first line of therapy for these dermatoses.

The main target of antihistamine action is the blockade of the inflammatory process caused by the binding of IgE antibodies to the allergen. As a result of the action of antihistamines, itching and swelling are reduced. Second-generation antigens, unlike first-generation antigens, practically do not penetrate the BBB and do not cause a sedative effect. II generation antihistamines are able to selectively block h2-receptors, quickly have a clinical effect within 24 hours, practically do not cause tachyphylaxis and have a high safety profile. Levocetirizine occupies a special place among the drugs of the second generation. Along with all the advantages of non-sedating antihistamines, levocetirizine has not only high selectivity for H1 receptors, but also the fastest and longest antihistamine effect when taken once a day. There is a high adherence of patients to the drug due to the high safety profile and the possibility of long-term use of levocetirizine.

Generic levocetirizine Allerway (Dr. Reddy’s Laboratories) is available on the domestic market. Allerway is bioequivalent to the original levocetirizine. It has a wide range of indications and is recommended for the treatment of pollinosis, symptoms of allergic rhinitis and allergic conjunctivitis, urticaria and other allergic dermatoses accompanied by itching and rashes.

Gribanov A.V., Vice-President of the “National Council of the Beauty Industry”, head of the section of cosmetologists at the Samara branch of the Russian Association of Dermatovenereologists and Cosmetologists also made a report on the topic “New trends in the development of lawmaking in the beauty industry. What should a law-abiding cosmetologist do?

The final speech was made by the chief freelance specialist of the Ministry of Health of the Samara region in dermatovenereology and cosmetology, chief physician of the Samara Regional Dermatovenerologic Dispensary, Doctor of Medical Sciences, Professor Shakurov I.G., in which he thanked everyone those present for their participation and wished them success in their future work.

Abstract

Data obtained on cellular pharmacological models indicate that the polyphenol
Zero extracts of oats Rhealba have an anti-inflammatory effect on important pathophysio-
logical links involved in the pathogenesis of atopic dermatitis. These extracts restore
function of the epidermal barrier and eliminate skin dryness, which leads to a decrease in skin itching,
scratching and the risk of infection. The use of oat extracts in the clinical setting suppresses
relieves manifestations of atopic dermatitis, including itching and scratching. Clinical trials showing
emollients containing Rhealba oat extract are highly effective
steroid-substituting agents and are considered as first-line agents in the management of patients
atopic dermatitis, strongly recommended by international and national consensus
telny documents and manuals.

of the skin of the forearms was evaluated according to the results of laser Doppler flowmetry in standard
conditions. Then, 20 µl of a 0.5 M ACH solution was injected intradermally into the back surface of the left
shoulders (for control, it was injected into the symmetrical point of the right forearm 0.9% NaCl solution in the same
volume) and within 20 minutes the reaction to ACH was evaluated by the appearance of a local sensation (according to
visual analog scale) and measurement of microcirculation (in the area 1 cm proximal to the
administration of ACH).
Results. It was found that at the injection site of ACH, patients with AD experienced a feeling of itching, and persons
suffering from hay fever, and healthy – tingling and burning. The duration of this response is
answered: patients with AD > patients with hay fever > healthy. Microcirculation changes in response
on ACH in patients with AD in the acute stage were more pronounced than in the remission stage, and in pain>
hay fever in the stage of remission is more pronounced than in healthy individuals.
Conclusions. The nature of the reaction to ACH in patients with AD (itching sensation) is different from that in patients with gender
linosis and healthy individuals (tingling and burning sensation). This difference is not related to sensitization>
tion as such and can explain the nature of the main symptom of AD – pruritus.

Abstract

Allergic rhinitis is a widespread disease. Diagnose allergies
rhinitis and adequate therapy can be prescribed already at the outpatient stage, not by allergists,
and doctors of other specialties. Cytological examination plays an important role in establishing the diagnosis.
following the material taken from the nasal mucosa. And the study of the cellular composition, in addition to the clinical
pictures, helps in the choice of therapy. After a 2-week course of intranasal use of Flixonase
significantly decreased the number of eosinophils and inflammatory cells in the nasal mucosa, which was
indicator of the effectiveness of the treatment of allergic rhinitis.
