Structure of the Male Reproductive System – Men’s Health Issues

The male reproductive system includes the penis, scrotum, testes, epididymis, vas deferens, prostate, and seminal vesicles.

  • The penis and the urethra are part of the urinary and reproductive systems.

  • The scrotum, testes (testicles), epididymis, vas deferens, seminal vesicles, and prostate comprise the rest of the reproductive system.

The penis consists of the root (which is attached to the lower abdominal structures and pelvic bones), the visible part of the shaft, and the glans penis (the cone-shaped end). The opening of the urethra (the channel that transports semen and urine) is located at the tip of the glans penis. The base of the glans penis is called the corona. In uncircumcised males, the foreskin (prepuce) extends from the corona to cover the glans penis.

The penis includes three cylindrical spaces (blood-filled sinuses) of erectile tissue. The two larger ones, the corpora cavernosa, lie side by side. The third sinus, the corpus spongiosum, surrounds most of the urethra. When these spaces fill with blood, the penis becomes large and rigid (erect).

The scrotum is the thick-skinned sac that surrounds and protects the testes. The scrotum also acts as a climate-control system for the testes because they need to be slightly cooler than body temperature for normal sperm development. The cremaster muscles in the wall of the scrotum relax to allow the testes to hang farther from the body to cool or contract to pull the testes closer to the body for warmth or protection.

The testes are oval bodies that average about 1.5 to 3 inches (4 to 7 centimeters) in length and 2 to 3 teaspoons (20 to 25 milliliters) in volume. Usually the left testis hangs slightly lower than the right one. The testes have two primary functions:

  • Producing sperm (which carry the man’s genes)

  • Producing testosterone (the primary male sex hormone)

The epididymis consists of a single coiled microscopic tube that measures almost 20 feet (6 meters) in length. The epididymis collects sperm from the testis and provides the environment for sperm to mature and acquire the ability to move through the female reproductive system and fertilize an ovum. One epididymis lies against each testis.

The vas deferens is a firm tube (the size of a strand of spaghetti) that transports sperm from the epididymis. One such duct travels from each epididymis to the back of the prostate and joins with one of the two seminal vesicles. In the scrotum, other structures, such as muscle fibers, blood vessels, and nerves, also travel along with each vas deferens and together form an intertwined structure, the spermatic cord.

The urethra serves a dual function in males. This channel is the part of the urinary tract that transports urine from the bladder and the part of the reproductive system through which semen is ejaculated.

The prostate lies just under the bladder and surrounds the urethra. Walnut-sized in young men, the prostate enlarges with age. When the prostate enlarges too much, it can block urine flow through the urethra and cause bothersome urinary symptoms.

The seminal vesicles, located above the prostate, join with the vas deferens to form the ejaculatory ducts, which travel through the prostate. The prostate and the seminal vesicles produce fluid that nourishes the sperm. This fluid provides most of the volume of semen, the fluid in which the sperm is expelled during ejaculation. Other fluid that makes up a very small amount of the semen comes from the vas deferens and from Cowper glands in the urethra.

Sexual anatomy that’s typically called male includes your penis and scrotum (external male genitalia) and internal reproductive organs like the testicles.

What are the external parts?

Your penis and scrotum are the two parts of the male (or what’s typically called male) external sex anatomy (outside your body).

Your penis is made of 3 layers of spongy tissue. When you get turned on, blood fills these tissues. This makes your penis get harder and stand up, which is also called getting an erection or hard-on.

The average size of an adult erect (hard) penis is 5 to 7 inches long. The size of your penis when it’s flaccid (soft) doesn’t have much to do with its size when it’s hard. Some penises get much bigger when they get hard. Others stay pretty much the same size.

About half of all penises in the US are circumcised, and half are uncircumcised (still have foreskin) — so both types are common. Some people call circumcised penises “cut,” and uncircumcised penises “uncut.”

Every penis looks a little different. For example, some curve like a banana when they’re hard. Others are straighter. All penises have the same parts though:

  • Glans
    Your glans is also called the head or tip of your penis. The opening of your urethra is here. This is where pre-ejaculate (precum) and semen (cum) come out of, and it’s where you pee out of. For many people, it’s the most sensitive part of the penis.

  • Shaft
    The shaft of your penis extends from the tip to where it connects to your lower belly. It looks like a tube. Your urethra is inside the shaft.

  • Foreskin
    The foreskin is a patch of skin that covers and protects the head (AKA glans). When your penis gets hard, the foreskin pulls back and the tip is exposed. Sometimes foreskin is circumcised (when a doctor surgically removes your foreskin) soon after birth, so not everyone has it.

  • Frenulum
    The frenulum is where your foreskin meets the underside of your penis. It looks like a small V just below the head. Usually part of it remains after circumcision. And for many people, it’s very sensitive.

Other external parts include:

  • Scrotum
    The scrotum (AKA ballsack) is the sac of skin that hangs below your penis. Your scrotum holds your testicles and keeps them at the right temperature. If it’s too cold, your scrotum pulls your testicles closer to your body. If it’s too warm, your testicles hang away from your body.

    Your scrotum is covered with wrinkly skin and hair. Your scrotum can be big or small, have a little or a lot of hair, and vary in color. Some people’s scrotum is larger on one side than the other.

    The scrotum is super sensitive, so any hitting or twisting is extremely painful. But many people like having their scrotum gently touched during sex.

  • Anus
    The anus (AKA butthole) is the opening to your rectum. The anus has lots of sensitive nerve endings, so some people experience sexual pleasure from anal stimulation.
     

What are the internal parts?

The internal parts of male sex anatomy are made up of:

  • Testicles 
    The testicles (AKA balls) are 2 ball-like glands inside your scrotum. They make sperm and hormones like testosterone.

  • Epididymis
    The epididymis is a tube where your sperm matures. It connects each testicle to each vas deferens. And it holds your sperm before you ejaculate (come).

  • Vas Deferens
    A vas deferens is a long, narrow tube that carries sperm from the epididymis to the seminal vesicles when you ejaculate (come). There are 2 of them — one connected to each epididymis.

  • Seminal Vesicles
    Seminal vesicles are 2 small organs that produce semen, the fluid that sperm moves around in. They’re located below your bladder.

  • Prostate Gland
    The prostate gland makes a fluid that helps your sperm move. It’s about the size of a walnut or golf ball. The prostate gland is sensitive to pressure or touch in a way that many people find pleasurable.

  • Cowper’s glands
    The Cowper’s glands produce a fluid called pre-ejaculate or precum. This fluid prepares your urethra for ejaculation (coming). It reduces friction so your semen can move more easily. The Cowper’s glands are under the prostate and attach to your urethra. They’re also called bulbourethral glands.

  • Urethra
    The urethra is the tube that carries urine (pee), pre-ejaculate, and semen to your urethral opening and out of your body.

  • Cremaster
    The cremaster is a muscle that moves your scrotum and testicles closer to your body. This happens when you’re cold, you’re aroused, or when someone touches your inner thigh.

Anus

The opening at the end of the digestive tract where bowel contents leave the body.

Bladder

A triangular shaped, hollow organ located in the lower abdomen. It is held in place by ligaments that are attached to other organs and the pelvic bones. The bladder’s walls relax and expand to store urine and contract and flatten to empty urine through the urethra.

Epididymis

A long tube that is located near each testicle. The epididymis is the tube in which the sperm from the testicles are removed away from the testicles.

Penis

The external reproductive organ of the male. The penis is made up of two parts, the shaft and the glans. The glans is the tip of the penis, while the shaft is the main part of the penis and contains the tube (urethra) that drains the bladder. All boys are born with a foreskin, or a covering over the tip of the penis. Some boys are circumcised, which means that this covering of skin is removed. Other boys are not circumcised and may have skin that covers the tip of the penis. If circumcision is not done the skin must be pulled back for proper cleaning. 

Prostate Gland

A sex gland in men. It is about the size of a walnut, and surrounds the neck of the bladder and urethra. It is partly muscular and partly glandular, with ducts opening into the urethra. The prostate gland secretes a slightly alkaline fluid that forms part of the seminal fluid, a fluid that carries sperm.

Rectum

The lower end of the large intestine, leading to the anus.

Scrotum

The bag of skin that holds and helps to protect the testicles. The testicles make sperm, and to do this, the temperature of the testicles needs to be cooler than the inside of the body. This is why the scrotum is located outside of the body.

Seminal Vesicles

The sac-like glands that lie behind the bladder and release a fluid that forms part of semen.

Testes (Testicles)

The testes are two small organs that are found inside the scrotum. The testes are responsible for making sperm and are also involved in producing a hormone called testosterone. Testosterone is an important hormone during male development and maturation — for developing muscles, deepening the voice, and growing body hair.

Urethra

The tube that allows urine to pass outside the body. 

Vas Deferens

Where the sperm is stored; a tube that carries the sperm out of the scrotal sac. The vas deferens is between the epididymis and the urethra and connects these together.

The Male Reproductive System – Sexual Health

For reproduction to occur, the female sex cell, called the egg, must be fertilized by a male sex cell, called the sperm. Not only are the sex cells different, but the organs that produce and store them are different as well.

Male Sexual Anatomy

The organs and glands that make up the male sexual anatomy include:

  • Testicles — After puberty, a man’s testicles, located at the base of the penis, produce male sex cells called sperm. Also starting at puberty, testicles produce testosterone, the male sex hormone. A man’s sperm production, once started, continues throughout his life; sexually mature males produce millions of sperm cells each day. The testicles are located below the penis, outside the body, where the appropriate temperature to make sperm may be maintained as it is several degrees too hot for sperm to be viable (able to fertilize eggs) inside the body.
  • Scrotum — The testicles are covered by a pouch of skin called the scrotum. The scrotum and the muscles surrounding it can pull the testicles toward the body when they are too cold, and relax away from the body when the testicles are too warm. The scrotum also holds the epididymis.
  • Epididymis and vas deferens — The epididymis stores the sperm after the testicles produce them, and the vas deferens transports the sperm from the epididymis to the urethra.
  • Urethra — The urethra is a duct, or tube, that transports fluids from the inside of the body to the outside. In both men and women, the urethra is connected to the bladder and is used to pass urine out of the body. In males, however, the urethra is also connected to the “accessory glands,” which produce semen, and to the vas deferens, the duct that brings the sperm from the epididymus.
  • Penis — The penis is perhaps the most visible part of the male sexual anatomy. It is made up of two parts, the shaft and the head (also called the glans.) The shaft houses the corpora cavernosa (two flexible cylinders comprised of erectile tissue that run the length of the penis and support erections), and the corpus spongiosum (erectile tissue surrounding the urethra). In its reproductive capacity, the urethral opening at the tip of the penis delivers sperm into the vagina. Urine also flows out of the body through the urethral opening.
  • Accessory glands — There are several glands that work together to produce semen, or seminal fluid. Sperm can live inside the female reproductive system for up to 48 hours, and seminal fluid helps the sperm move around and stay nourished. The seminal vesicle produces a fluid that provides energy to the sperm as they seek out the female sex cell, or the egg. The prostate gland makes a different fluid that helps the sperm move more quickly through the female reproductive system. Another set of glands, called bulbourethral or Cowper’s glands, makes a small quantity of fluid that helps protect the sperm on its way through the urethra by neutralizing any leftover traces of acidic urine.

fetus  (Adj. fetal) The term for a mammal during its later-stages of development in the womb. For humans, this term is usually applied after the eighth week of development.

gene   (adj. genetic) A segment of DNA that codes, or holds instructions, for producing a protein. Offspring inherit genes from their parents. Genes influence how an organism looks and behaves.

genitals or genitalia    The visible sex organs.

gonads  The reproductive organs that make eggs (in females) and sperm (in males).

hormone    (in zoology and medicine)  A chemical produced in a gland and then carried in the bloodstream to another part of the body. Hormones control many important body activities, such as growth. Hormones act by triggering or regulating chemical reactions in the body.

intersex    Animals or humans that display characteristics of both male and female reproductive anatomy.

masculinize  (in biology) For a female person or animal to take on physical, behavioral or physiological traits considered typical of males.

mutation  Some change that occurs to a gene in an organism’s DNA. Some mutations occur naturally. Others can be triggered by outside factors, such as pollution, radiation, medicines or something in the diet. A gene with this change is referred to as a mutant.

ovary  (plural: ovaries) The organ in the females of many species that produce eggs.

pediatrics  A field of medicine that has to do with children and especially child health.

psychiatry    A field of medicine where doctors study and treat diseases of the human mind. Treatments may consist of talking therapies, prescription drugs or both. People who work in this field are known as psychiatrists.

sex   A person’s biological status, typically male or female. There are a number of indicators of biological sex, including sex chromosomes, gonads, internal reproductive organs, and external genitals.

testis   (plural: testes) The organ in the males of many species that makes sperm, the reproductive cells that fertilize eggs. This organ also is the primary site that makes testosterone, the primary male sex hormone.

testosterone  Although known as male sex hormone, females make this reproductive hormone as well (generally in smaller quantities). It gets its name from a combination of testis (the primary organ that makes it in males) and sterol, a term for some hormones. High concentrations of this hormone contribute to the greater size, musculature and aggressiveness typical of the males in many species (including humans).

transgender  Someone who has a gender identity that does not match the sex they were assigned at birth.

urology     The medical field that deals with diseases of the male and female urinary tract and conditions affecting male reproductive organs. Doctors who specialize in this area are known as urologists.

womb   Another name for the uterus, the organ in which a fetus grows and matures in preparation for birth.

The male sex organs allow men to have children. But they have other important jobs, too: The sex organs produce hormones, control the process of boys maturing into grown men, and make sex and sexual pleasure possible. Like women, men have external and internal sex organs.

What jobs do the external sex organs do?

The external male sex organs include the penis and the scrotum. The main purpose of the external sex organs is to allow for sexual intercourse and sexual pleasure. The structure of the penis is similar to that of the female clitoris, apart from the fact that the clitoris is mostly inside the body. During the time in the womb, they both develop from the same organ.

The penis can carry and release the man’s sperm into a woman’s vagina. The round glans (head) of the penis is located at the tip of the longer shaft of the penis. It is covered with a mucous membrane. The movable foreskin (partially) covers the head of the penis. In some men, the foreskin has been shortened or removed in a medical procedure known as circumcision.

A tube called the urethra runs through the inside of the penis. Urine leaves the body through the urethra, and so does semen (the fluid that carries sperm) during ejaculation. The shaft and the head of the penis contain erectile tissues called corpora cavernosa. These tissues are like sponges. During sexual arousal, blood builds up in the corpora cavernosa until they are full and firm. This allows the penis to become erect and stiff (erection). In most men, it also becomes longer and thicker.

The scrotum is a bag of skin that surrounds the testicles, the epididymis and the start of the vas deferens.

The head of the penis and the skin on the penis, scrotum and the surrounding area contain a dense network of nerve fibers. This makes the external sex organs very sensitive. As a result, touching and rubbing this area can cause sexual arousal and increased pleasure that may lead to orgasm and trigger ejaculation.

What jobs do the internal sex organs do?

The main internal male sex organs include:

  • Testicles

  • Epididymis

  • Vas deferens tubes

  • Seminal vesicles

  • Prostate

The two round or egg-shaped testicles are the male reproductive glands (gonads): This is where the sperm cells are produced. Only these cells are capable of fertilizing a female egg cell. The testicles also produce the male sex hormone testosterone. They start to develop inside the abdominal cavity of a male embryo. As the embryo grows, they drop down out of the abdominal cavity and into the scrotum through the inguinal canals.

A worm-shaped epididymis is attached to each testicle. The epididymis takes the sperm cells from the testicles and allows them to mature into sperm that can move. The mature sperm are stored in the epididymis until the next ejaculation.

Both of the thin vas deferens tubes lead from the epididymis to the prostate. There they join the urethra tube, through which sperm can leave the body.

Just before the vas deferens tubes reach the urethra, they pass by the seminal vesicles. The seminal vesicles produce a fluid that mixes with the sperm cells to form semen. One part of the semen is produced by smaller glands near the base of the urethra and in the prostate. During ejaculation, the muscles in the pelvic floor contract. This causes the semen to be squirted out of the urethra. The released semen is called ejaculate (or “come”).

What possible problems can occur?

Various problems may occur if the male sex organs change or are affected by medical conditions:

  • Pain, for example constant or only during sex

  • Itching, red skin and swelling

  • Discharge from the urethra

  • Burning sensation and difficulties urinating (peeing), urinary retention

  • Painful, long-lasting erection (priapism)

  • Hormonal imbalances

Some men experience problems related to other underlying conditions – such as erection problems in men who have diabetes. Other men have problems because changes occur in only a single organ, such as a benign enlarged prostate.

In general, problems in the genital area can be caused by many different things, such as viral, bacterial or fungal infections. Typical examples include genital warts, inflammation of the head of the penis (balanitis) or inflammations caused by sexually transmitted infections like gonorrhea.

As a result of developmental disorders or malformations, the testicles may not drop down from the abdominal cavity into the scrotum before birth. This can cause infertility. If the foreskin – which is still attached to the head of the penis in young boys – does not loosen during the first few years of the boy’s life, he will have a tight foreskin (phimosis). In some men, the connective tissue in the penis becomes hardened and tight, causing the penis to curve.

Problems are often associated with the blood vessels, too. Varicose veins on the testicles (varicoceles) can cause swelling, pain and infertility, for instance. One testicle can become painfully twisted and then no longer get a proper supply of blood. If that happens, some testicle tissue may die off. Erection problems are often caused by chronically damaged blood vessels in the penis.

If an erect penis is suddenly bent sharply or squashed, the blood-filled erectile tissue may become damaged. This can cause major bruising (“penile fracture”).

Non-cancerous growths may occur in tissue like the skin or the connective tissue of the penis or scrotum. They may also develop in the gland tissue of the internal male sex organs, like the testicles or the epididymis. The most common cancerous tumors that affect the male sex organs include prostate cancer, testicular cancer and skin cancer on the penis. Tumors on the penis often develop due to abnormal changes in skin cells or mucous membrane cells. Known as dysplasia, these changes make the cells multiply faster than normal. Dyplasia on the penis is also called penile intraepithelial neoplasia (PIN).

The male reproductive system | Male Sexual Organs

Several different organs and structures make up the male reproductive system. These include:

The scrotum, or scrotal sac. This is the loose bag of skin which hangs under the main body cavity between the upper thighs. It is divided into two, and each side contains one testicle (testis).

The testicles (testes). There are two testes. Each is an egg-shaped structure located in the scrotum. The testes produce sperm and also produce male hormones. The testes start developing inside the body cavity in a growing baby (fetus). About two months before a male baby is born, the testes start to drop down into the scrotal sacs. Because they are outside the main body cavity the testes are slightly cooler. This difference in temperature helps sperm production.

The epididymis. This is the tube attached to the testis where sperm is stored.

The vas deferens. This is the tube which carries the sperm from the epididymis. It may also be called the sperm duct. It meets a tube from the seminal vesicle (see below) to form a short tube called the ejaculatory duct. This then opens into the urethra, which is the tube that takes the sperm outside the body.

The accessory sex glands. There are three glands which produce fluids that mix with the sperm to make up semen. Semen is the liquid which is ejected from the penis during ejaculation. The three glands are located close to the bladder and are called:

  • The seminal vesicles
  • The prostate gland. The prostate gland lies just beneath the bladder (see diagram). It is normally about the size of a chestnut.
  • The bulbourethral glands

The penis. The penis has a single tube in it called the urethra. There are three main parts of the penis – the root, body and glans. The root is the part attached to the skin at the top of the scrotum. The body of the penis is made up of a spongy type of tissue, which swells when blood enters during an erection. The glans penis is the slightly larger area towards the end of the penis and contains the opening of the urethra.

The urethra. This is the tube which passes from the bladder down the penis to the outside. It carries both urine and semen.

What does the male reproductive system do?

The main function is to give men the ability to fertilise a woman’s egg (ovum) by producing and delivering semen. The testicles (testes) also make hormones which help men develop the characteristics associated with being male. This includes:

  • The development of hair in a male distribution – for example, on the chest, under the arms, on the face and in the pubic area.
  • Enlargement of the penis.
  • Deepening of the voice.
  • Muscle growth.
  • Bone growth and increased height.

How does the male reproductive system work?

During puberty, the level of a hormone called gonadotrophin-releasing hormone (GnRH) increases. GnRH is produced in a part of the brain called the hypothalamus. In turn this causes an increase in the production of two hormones from another part of the brain, called the pituitary gland. These hormones are called luteinising hormone (LH) and follicle-stimulating hormone (FSH). LH in the bloodstream causes cells in the testicles (testes) to make and release testosterone, the main male hormone.

FSH and testosterone work together to stimulate the testes to produce sperm. Each sperm cell takes between 65-75 days to form and around 300 million are produced every day. Inside the testes sperm is made in structures called the seminiferous tubules. At the top and to the back of each testicle (testis) is the epididymis, which stores sperm.

Leading from the epididymis is the vas deferens. The vas deferens carries sperm towards the penis. Along the way it joins other tubes and during ejaculation collects fluids from the accessory sex glands. The mixture of sperm and fluids from the accessory sex glands is called semen. About two thirds of the volume of semen come from the seminal vesicles. The semen is then passed into the urethra.

When sexually aroused, a number of changes occur inside the penis. The arteries supplying the penis get bigger, allowing more blood to enter its tissues. The extra blood flow causes the penis to enlarge and to become more rigid. The extra blood flow plus signals from the nervous system and chemical changes cause an erection.

Ejaculation is the term for the contractions that release semen. This is a reflex action, which means it is not consciously controlled. As part of the reflex action, the opening that drains the bladder is closed. This means that urine is not released at the same time as semen. The volume of semen in a typical ejaculation is between 2.5-5 millilitres (mL). There are normally more than 20 million sperm in each mL of semen. During sexual intercourse, the penis of the male enters the vagina of the female, carrying the sperm to the neck of the womb (cervix) to fertilise the woman’s egg.

WHAT HORMONES REGULATE THE MALE REPRODUCTIVE SYSTEM

?

The male reproductive system is primarily dependent on three hormones. Luteinizing (LH) and follicle-stimulating (FSH) hormones are produced by the pituitary gland, a gland located at the base of the brain. FSH stimulates sperm formation, LH stimulates testosterone synthesis.

Testosterone is necessary for the production of sperm, as well as the emergence of secondary sexual characteristics: increased muscle mass, male body hair growth, sex drive for women, low voice, etc.
