What is oxcarbazepine?

Oxcarbazepine is an anticonvulsant. It works by decreasing nerve impulses that cause seizures and pain.

Oxcarbazepine is used either alone or with other medicines to treat partial seizures.

The Trileptal brand of oxcarbazepine is used as a single medicine in adults and children who are at least 4 years old. Trileptal is used with other medicines in adults and children who are at least 2 years old.

The Oxtellar XR brand of oxcarbazepine is used with other medicines in adults and children who are at least 6 years old.

Oxcarbazepine may also be used for purposes not listed in this medication guide.

Warnings

Seek medical treatment if you have symptoms of a serious drug reaction: skin rash, fever, swollen glands, flu-like symptoms, unusual bruising or bleeding, or yellowing of your skin or eyes.

Call your doctor right away if you have symptoms of low sodium levels in your body, such as nausea, confusion, severe weakness, muscle pain, or increased seizures.

Some people have thoughts about suicide while taking seizure medicine. Stay alert to changes in your mood or symptoms. Report any new or worsening symptoms to your doctor.

Do not stop using oxcarbazepine suddenly.

Before taking this medicine

You should not take oxcarbazepine if you are allergic to oxcarbazepine or eslicarbazepine.

Tell your doctor if you have ever had:

  • liver disease;

  • kidney disease;

  • mood problems or suicidal thoughts; or

  • an allergy to carbamazepine (Carbatrol, Tegretol).

Some people have thoughts about suicide while taking oxcarbazepine. Your doctor will need to check your progress at regular visits. Your family or other caregivers should also be alert to changes in your mood or symptoms.

Do not start or stop taking oxcarbazepine during pregnancy without your doctor’s advice. Having a seizure during pregnancy could harm both mother and baby. Tell your doctor right away if you become pregnant while taking oxcarbazepine for seizures.

If you are pregnant, your name may be listed on a pregnancy registry to track the effects of oxcarbazepine on the baby.

Oxcarbazepine can make birth control pills less effective. Ask your doctor about using a non-hormonal birth control (condom, diaphragm with spermicide) to prevent pregnancy.

You should not breast-feed while you are taking oxcarbazepine.

Do not give this medicine to a child without medical advice. There are specific age restrictions for the use of oxcarbazepine in children, depending on the dose form and whether it is used alone or with other medicines.

How should I take oxcarbazepine?

Follow all directions on your prescription label and read all medication guides or instruction sheets. Your doctor may occasionally change your dose. Use the medicine exactly as directed.

Shake the oral suspension (liquid) before you measure a dose. Use the dosing syringe provided, or use a medicine dose-measuring device (not a kitchen spoon).

Give the liquid directly from the oral syringe, or mix the medicine with a small glass of water. After using the syringe, rinse it with water and allow it to air dry.

Take the oxcarbazepine extended-release tablet on an empty stomach, at least 1 hour before or 2 hours after a meal.

You may take the oxcarbazepine oral liquid or regular tablet with or without food.

Swallow the extended-release tablet whole and do not crush, chew, or break it.

You may need frequent blood tests.

Do not stop using oxcarbazepine suddenly, even if you feel fine. Stopping suddenly may cause increased seizures. Follow your doctor’s instructions about tapering your dose.

In case of emergency, wear or carry medical identification to let others know you use oxcarbazepine.

Seizures are often treated with a combination of drugs. Use all medications as directed by your doctor. Read the medication guide or patient instructions provided with each medication. Do not change your doses or medication schedule without your doctor’s advice.

Store at room temperature away from moisture and heat. Throw away any unused liquid 7 weeks after opening the bottle.

What happens if I miss a dose?

Take the medicine as soon as you can, but skip the missed dose if it is almost time for your next dose. Do not take two doses at one time.

What happens if I overdose?

Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

What should I avoid while taking oxcarbazepine?

Do not drink alcohol. Drinking alcohol can increase certain side effects, and may increase the risk of seizures.

Avoid becoming overheated or dehydrated during exercise, in hot weather, or by not drinking enough fluids. Follow your doctor’s instructions about the type and amount of liquids you should drink.

Avoid driving or hazardous activity until you know how oxcarbazepine will affect you. Your reactions could be impaired.

Oxcarbazepine side effects

Get emergency medical help if you have signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat.

Seek medical treatment if you have a serious drug reaction that can affect many parts of your body. Symptoms may include: skin rash, fever, swollen glands, flu-like symptoms, muscle aches, severe weakness, unusual bruising, or yellowing of your skin or eyes.

Oxcarbazepine can reduce the sodium in your body to dangerously low levels, which can cause a life-threatening electrolyte imbalance. Call your doctor right away if you have nausea, lack of energy, confusion, feeling tired or irritable, severe weakness, muscle pain, or increased seizures.

Report any new or worsening symptoms to your doctor, such as: mood or behavior changes, depression, anxiety, or if you feel agitated, hostile, restless, hyperactive (mentally or physically), or have thoughts about suicide or hurting yourself.

Common side effects of oxcarbazepine may include:

  • dizziness, drowsiness, tiredness;

  • balance or coordination problems;

  • nausea, vomiting;

  • tremors or shaking;

  • double vision; or

  • rash.

This is not a complete list of side effects and others may occur. Call your doctor for medical advice about side effects. You may report side effects to FDA at 1-800-FDA-1088.

What other drugs will affect oxcarbazepine?

Using oxcarbazepine with other drugs that make you drowsy can worsen this effect. Ask your doctor before using opioid medication, a sleeping pill, a muscle relaxer, or medicine for anxiety or seizures.

Sometimes it is not safe to use certain medications at the same time. Some drugs can affect your blood levels of other drugs you take, which may increase side effects or make the medications less effective.

Many drugs can affect oxcarbazepine. This includes prescription and over-the-counter medicines, vitamins, and herbal products. Not all possible interactions are listed here. Tell your doctor about all your current medicines and any medicine you start or stop using.

Trileptal ® may be used alone or in combination with other antiepileptic drugs. In both cases, the course of treatment begins with a clinically effective dose, the frequency of administration is 2 times a day. The dose may be increased depending on the response to therapy. When replacing another antiepileptic drug with Trileptal ® at the beginning of taking the drug Trileptal ® should gradually reduce the dose of the replaced drug. When using the drug Trileptal ® as part of combination therapy, it may be necessary to reduce the dose of concomitant antiepileptic drugs and / or slower increase in the dose of the drug Trileptal ® .

The recommendations below apply to patients with normal renal function. For this category of patients, there is no need to control plasma concentrations of the active substance in order to optimize therapy with Trileptal 9.0007®.

The following dosing recommendations (in adults, children and adolescents, and patients ≥65 years of age) are based on the use of these doses in clinical studies conducted in all age groups. However, if necessary, lower initial doses can be used.

Tablets are scored and can be broken into 2 pieces for easier swallowing.

When using the drug Trileptal ® in children under 3 years old who cannot swallow tablets, and also in cases where it is impossible to measure the required dose when using the drug in the form of tablets, prescribe the drug Trileptal ® in the form of a suspension for oral administration.

Adults and elderly patients

Monotherapy: starting dose is 600 mg/day (8-10 mg/kg/day) divided into 2 doses. A good therapeutic response has been observed in the dose range of 600–2400 mg/day, with most patients having a good clinical effect at a dose of 900 mg/day. If necessary, a gradual increase in dose is possible. The dose is increased by no more than 600 mg / day, with an interval of approximately 1 week, until the desired therapeutic response is achieved.

In patients who have not previously received antiepileptic therapy, the effective dose is 1200 mg / day, in patients who have previously received but poorly responded to therapy with other antiepileptic drugs – 2400 mg / day.

Combination therapy: starting dose is 600 mg/day (8-10 mg/kg/day) divided into 2 doses. A good therapeutic response was observed in the dose range of 600–2400 mg/day. If necessary, a gradual increase in dose is possible. The dose is increased by no more than 600 mg / day, with an interval of approximately 1 week, until the desired therapeutic response is achieved.

The use of the drug Trileptal ® at a daily dose of 2400 mg as part of combination therapy without dose reduction of another antiepileptic agent was accompanied by poor tolerability in most patients, mainly due to the development of adverse events from the nervous system. The use of the drug Trileptal ® in a daily dose above 2400 mg has not been studied.

Children

With monotherapy and with the use of the drug as part of combination therapy: The recommended starting dose is 8-10 mg/kg/day for 2 doses.

If necessary, to achieve the desired therapeutic effect, a gradual increase in dose is possible: at intervals of approximately 1 week, the dose is increased by a maximum of 10 mg / kg / day, up to a maximum daily dose of 60 mg / kg.

Trileptal ® is intended for use in children 1 month of age and older. The use of the drug in children under 1 month of age has not been studied in controlled clinical trials.

When Trileptal ® is used as monotherapy and as part of combination therapy, when adjusted for body weight, apparent clearance in children decreases significantly with increasing age. Children aged 1 month to 4 years may require a dose of the drug, 2 times the adult dose, when adjusted for body weight; children aged 4 to 12 years may require a dose 50% higher than the adult dose, adjusted for body weight.

In children aged 1 month to 4 years, the effect of antiepileptic drugs – inducers of liver enzymes – on their apparent clearance is more pronounced than in children of older age groups (when adjusted for body weight). When using the drug Trileptal ® in children aged 1 month to 4 years in combination with antiepileptic drugs – inducers of liver enzymes – a dose of oxcarbazepine may be required 60% higher (when adjusted for body weight) than with monotherapy with Trileptal ® or when used in combination with antiepileptic drugs that do not induce enzymes. For children of older age groups, during combination therapy with Trileptal ® with liver enzyme inducers, a slight increase in the dose of the drug may be required compared with monotherapy.

In children under 3 years of age, the drug should be used in the form of a syrup due to the difficulties of using solid dosage forms in this age group.

Special patient populations

Patients ≥65 years of age. Special adjustment of the dosing regimen for this category of patients is necessary in case of impaired renal function or if there is a risk of developing hyponatremia.

Impaired liver function. No dosage adjustment required in patients with mild to moderate hepatic impairment. There are no data on the use of the drug Trileptal ® in patients with severely impaired liver function, and therefore care must be taken when using the drug in patients of this category.

Impaired kidney function. For patients with impaired renal function (Cl creatinine less than 30 ml / min), the recommended initial dose is 300 mg / day; the dose should be increased slowly, at intervals of at least 1 week, until the desired therapeutic response is achieved. Careful monitoring of patients during dose selection is necessary.

Before taking the oral suspension, shake the bottle thoroughly and immediately measure the required amount of suspension. The required dose (ml) is taken from the vial using the supplied syringe. When using a 10 ml syringe (supplied with a 250 ml vial for adults and older children), the amount of suspension should be rounded up to 0. 5 ml. When using the 1 ml syringe (supplied with the 100 ml bottle for young children), the amount of suspension should be rounded up to 0.1 ml. After each use, close the vial tightly and wipe the syringe with a clean, dry cloth. The suspension can be taken directly from the syringe or diluted with a little water before taking. Store an open bottle for no more than 7 weeks.
