This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.

Before using this medication, tell your doctor or pharmacist your medical history, especially of: breathing problems (such as emphysema, asthma), high pressure in the eye (glaucoma), high blood pressure, kidney problems, liver problems, seizures, stomach/intestine problems (such as ulcer, blockage), overactive thyroid (hyperthyroidism), difficulty urinating (for example, due to enlarged prostate).

Hydroxyzine may cause a condition that affects the heart rhythm (QT prolongation). QT prolongation can rarely cause serious (rarely fatal) fast/irregular heartbeat and other symptoms (such as severe dizziness, fainting) that need medical attention right away.

The risk of QT prolongation may be increased if you have certain medical conditions or are taking other drugs that may cause QT prolongation. Before taking hydroxyzine, tell your doctor or pharmacist of all the drugs you take and if you have any of the following conditions: certain heart problems (heart failure, slow heartbeat, QT prolongation in the EKG), family history of certain heart problems (QT prolongation in the EKG, sudden cardiac death).

Low levels of potassium or magnesium in the blood may also increase your risk of QT prolongation. This drug may make you dizzy or drowsy or blur your vision. Alcohol or marijuana (cannabis) can make you more dizzy or drowsy. Do not drive, use machinery, or do anything that needs alertness or clear vision until you can do it safely. Avoid alcoholic beverages. Talk to your doctor if you are using marijuana (cannabis).

Before having surgery, tell your doctor or dentist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products).

Liquid products may contain sugar and/or alcohol. Caution is advised if you have diabetes, liver disease, or any other condition that requires you to limit/avoid these substances in your diet. Ask your doctor or pharmacist about using this product safely.

Once I saw it could cause heart disturbances I refused to take this drug. I am using benedryl instead. The risks seem to be very high with this drug with what I see as a low effectiveness for the risks at hand.

I was prescribed this only as a sleep aid, for no other reason. I took the 25MG strength. The side effects I experienced were increased visits to the bathroom, almost every two hours, and restlessness. Unfortunately, it only made me drowsy but couldn’t really help me get a good night’s sleep.

Asked for this back in 2016 for longstanding anxiety that wasn’t fully controlled by daily sertraline + gabapentin. On the plus side, it does help my anxiety and doesn’t muddle my thoughts like Xanax. On the downside, the drowsiness from this medication noticeably reduces my desire to do anything and can last for up to two days after a single dose – and I’m convinced that my daily use of this medication between 2016 and early 2020 contributed to two depressive episodes.

I still use it when my anxiety is really bad, but I prefer to use Xanax. Even though Xanax makes me feel like I’ve dropped a few IQ points and has an addiction risk, I still prefer that over feeling lifeless and risking another depressive episode.

Everyone is different, though. My mother and my best friend both take hydroxyzine for anxiety. My mom only feels tired for a couple of hours and my best friend gets a dry mouth. Don’t be like me and rush into it blindly, but please don’t be scared away from this medication, either. Just make sure to discuss the potential benefits and side effects of this drug with your doctor before you pick it up from the pharmacy. Read More Read Less

The traffic and noises outside my house have been extremely frustrating to me, enough to cause anxiety like I’ve never experienced. My doctor prescribed this and it works. I could take it as many as four times a day but the most I’ve taken it was in the afternoon and evening once. Otherwise I take it as needed once a day.

While this drug does keep the itching at bay, it also causes drowziness. Therefore, I take it after a meal and often follow it with a cup of black tea. This combination works well for me.

I have eczema that I treat with a combination prescription Betaderm Ointment 0.1% with Hydroxyzine 25mg. I have been on these prescriptions close to 20 years and yet my condition continues to only get worse and keeps spreading. I am supposed to take one tablet a day but when I get flareups I have to cheat and double the dose for a couple days or so before I get any relief. Starting to wonder if I have developed some type of immunity or resistance on hydroxyzine

The first day it was pretty good, took it at night and slept really well. Now I’m on the 3rd day and it is giving me one heck of a headache, which I rarely get. How should I use this medication?

For adults and children weighing more than 40 kg, the recommended dose of hydroxyzine when taken by mouth is up to a maximum of 100 mg per day, divided into 3 or 4 doses.   For people over the age of 65 years, the maximum daily dose is 50 mg.

For children weighing less than 40 kg, the dose is based on body size, and is calculated as 2 mg per kilogram of body weight. This dose is then divided into 3 or 4 doses to be taken spaced out throughout the day. The maximum daily dose of hydroxyzine for this group is 80 mg per day.

Use an oral syringe to measure each dose of the liquid, as it gives a more accurate measurement than household teaspoons.

Hydroxyzine may be taken with or without food.

Many things can affect the dose of a medication that a person needs, such as body weight, other medical conditions, and other medications. If your doctor has recommended a dose different from the ones listed here, do not change the way that you are taking the medication without consulting your doctor.

It is important to take this medication exactly as prescribed by your doctor. If you miss a dose, take it as soon as possible and continue with your regular schedule. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one. Store this medication at room temperature, protect it from light and moisture, and keep it out of the reach of children.

Who should NOT take this medication?

Do not take this medication if you:

  • are allergic to hydroxyzine or any ingredients of the medication
  • are in early pregnancy
  • have had an allergic reaction to aminophylline or cetirizine
  • have asthma and have had a breathing problem relating to histamine or allergies
  • have a history of irregular heartbeat, other heart problems, or a family history of sudden cardiac death
  • have untreated low levels of potassium or magnesium,
  • have porphyria

What side effects are possible with this medication?

Many medications can cause side effects. A side effect is an unwanted response to a medication when it is taken in normal doses. Side effects can be mild or severe, temporary or permanent. The side effects listed below are not experienced by everyone who takes this medication. If you are concerned about side effects, discuss the risks and benefits of this medication with your doctor.

The following side effects have been reported by at least 1% of people taking this medication. Many of these side effects can be managed, and some may go away on their own over time.

Contact your doctor if you experience these side effects and they are severe or bothersome. Your pharmacist may be able to advise you on managing side effects.

  • constipation
  • drowsiness
  • dry mouth, nose, or throat
  • flushing
  • headache
  • itching

Although most of these side effects listed below don’t happen very often, they could lead to serious problems if you do not seek medical attention.

  • difficult or painful urination
  • dizziness
  • slow, fast, or pounding heartbeat
  • hallucinations (seeing or hearing things that aren’t there)

Stop taking the medication and seek immediate medical attention if any of the following occur:

  • seizures
  • signs of an allergic reaction (e.g., rash, itching, or hives)
  • signs of a severe skin reaction such as blistering, peeling, a rash covering a large area of the body, a rash that spreads quickly, or a rash combined with fever or discomfort

Abnormal heart rhythms: This medication can cause abnormal heart rhythms. Certain medications (e.g., sotalol, quinidine, thioridazine, chlorpromazine, droperidol, pimozide,  moxifloxacin, mefloquine, pentamidine, arsenic trioxide, probucol, tacrolimus) can increase the risk of a type of abnormal heart rhythm called QT prolongation and should not be used in combination with hydroxyzine. You are more at risk for this type of abnormal heart rhythm and its complications if you:

  • are female
  • are older than 65 years of age
  • have a family history of sudden cardiac death
  • have a history of heart disease or abnormal heart rhythms
  • have a slow heart rate
  • have congenital prolongation of the QT interval
  • have diabetes
  • have had a stroke
  • have low potassium, magnesium, or calcium levels
  • have nutritional deficiencies

If you have heart disease and abnormal heart rhythms, or people are taking certain medications (e.g., verapamil, atazanavir), discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.

Breathing problems: People with breathing problems such as asthma, emphysema, or chronic bronchitis may find that hydroxyzine makes breathing more difficult. It can dry the mucus in the lungs, making it more difficult to clear the lungs. If you experience difficulty breathing, stop taking the medication and contact your doctor.

Digestive system problems: Antihistamines, such as hydroxyzine, cause the passage of food though the digestive system to slow down. If you have any condition that already slows down the movement of food through the stomach and intestines, hydroxyzine may make this worse.

Drowsiness/reduced alertness: Hydroxyzine may cause drowsiness or dizziness, affecting your ability to drive or operate machinery. Avoid driving, operating machinery, or performing other potentially hazardous tasks until you have determined how you are affected by this medication.

Enlarged prostate and urinary difficulties: Hydroxyzine may worsen urinary difficulties. If you have an enlarged prostate or difficulty starting urination, including a blockage from the bladder, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.

Epilepsy: Hydroxyzine should be used with caution by people with epilepsy, as it may increase the risk for seizures. If you have a history of seizures, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.

Glaucoma: This medication may cause a slight increase in pressure in the eye and therefore should be used with caution by anyone with glaucoma. If you have glaucoma, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.

If you notice vision changes while you are taking this medication, contact your doctor.

Kidney function: The kidneys are partially responsible for removing this medication from your body. Kidney disease or reduced kidney function may cause this medication to build up in the body, causing side effects. If you have reduced kidney function or kidney disease, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.

Liver function: Liver disease or reduced liver function may cause this medication to build up in the body, causing side effects. If you have liver problems, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.

Myasthenia gravis: Hydroxyzine may worsen the muscle weakness experienced by people with myasthenia gravis. This medication should be used with caution if you have myasthenia gravis.

Sedation: The sedating effect of hydroxyzine must be considered when the medication is used at the same time as other sedating medications such as narcotics, non-narcotic pain relievers, sleeping pills, sedatives, barbiturates, or alcohol.

Thyroid disease: Hydroxyzine may cause symptoms of an overactive thyroid gland (hyperthyroidism). If you have thyroid problems, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed.

Pregnancy: Hydroxyzine should not be used during early pregnancy. It should be used during later pregnancy only if the potential benefits outweigh the risks.

Breast-feeding: It is not known if hydroxyzine passes into breast milk. If you are a breast-feeding mother and are taking this medication, it may affect your baby. Talk to your doctor about whether you should continue breast-feeding.

Seniors: Seniors may be at increased risk for the sedating and impaired coordination effects of this medication. Seniors should use extra caution, for example, when getting up during the night.

There may be an interaction between hydroxyzine and any of the following:

How Vistaril Is Used to Treat Anxiety

Your doctor may prescribe Vistaril as a short-term treatment for anxiety or as an alternative to other habit-forming anxiety medications, such as benzodiazepines. If you have combined allergies and anxiety, this medication may also be a good choice. Vistaril is not a good choice for long-term treatment of anxiety, as it has not been shown to be effective for this use.

Taking Vistaril

If you have been prescribed Vistaril, you will typically take it either in tablet or capsule form. Vistaril comes in 25 mg two-tone green capsules and 50 mg green and white capsules. Vistaril is also available as a syrup or suspension liquid. You can continue your normal diet while taking Vistaril.

Research on Vistaril has primarily focused on its effectiveness in the treatment of generalized anxiety disorder.  There are no studies to indicate the effectiveness of Vistaril for social anxiety disorder.

You should not be prescribed Vistaril for treatment lasting more than 4 months, as this use has not been supported by systematic clinical studies. There is also some research evidence to suggest that long-term use, particularly in older adults, could be linked to dementia. There are many other alternatives for treating your anxiety long-term.

Vistaril may be combined with other medications, such as antidepressants, depending on your specific symptoms. It may also be combined with other treatments, such as psychotherapy.

Dosage Guidelines

A typical adult dose for Vistaril for anxiety and tension is 50 to 100 mg up to four times a day. Dosages for children under 6 years old are 2 mg per kg per day in divided dosages, not to exceed 50 mg per day. Vistaril may be used in children both to relieve anxiety and also as an antihistamine.

If you forget to take a dose, take it as soon as you remember it, but not if it is near the time for your next dose. In that case, skip the dose rather than taking a double dose.

Who Shouldn’t Take Vistaril

You should not take Vistaril if you have a hypersensitivity to hydroxyzine, Zyrtec (cetirizine), or Xyzal (levocetirizine). It is not recommended if you are pregnant or breastfeeding; it is not known whether the medicine passes into breast milk or could harm your baby or cause birth defects.

Those with a heart condition that has a long QT interval should not take this medication as it can lead to serious heart problems. Discuss any history of heart disease or a slow or irregular heartbeat with your doctor.

Medication Interactions

Vistaril can potentially interact with several medications including narcotics, antihistamines, analgesics, and barbiturates. The effect of alcohol may also be increased when taking Vistaril. Be sure to tell your doctor about any nonprescription or prescription drugs you are taking or plan to take. Your dosages may need to be adjusted or you may need to be monitored for side effects.

Side Effects

Common side effects of Vistaril include dry mouth and drowsiness. However, these effects are usually mild and short-lived. In older adults, there may be constipation or confusion. Dizziness and headache are also reported. Rarely, tremor or convulsions can develop, usually with high dosages.

Call your doctor immediately if you experience trembling or seizures. Stop taking Vistaril immediately and call your doctor if you develop a rash, pus-filled sores, swelling, and fever.

Possible Risks

Caution should be exercised if driving, operating dangerous machinery, or participating in hazardous activities while using Vistaril. Risk of overdose is low, with the result generally being extreme sedation.

Symptoms of an overdose include stupor, dizziness, nausea, vomiting, or seizures. You should contact a poison control center or 911 in the event of an overdose. Usually, they will induce vomiting if it hasn’t occurred already.

Is Hydroxyzine Effective for Treating Allergies?

Hydroxyzine is a first-generation, sedating antihistamine, which means it has similar side effects to Benadryl. It is marketed under the brand names Atarax and Vistaril but is also available in generic form. Hydroxyzine was originally developed as a sedative during the 1950s but was found to have significant antihistamine properties.

Hydroxyzine for Allergies

Hydroxyzine is commonly prescribed for the treatment of various allergic conditions, particularly hives, but is also commonly used to treat itching, anxiety, insomnia, as well as nausea and vomiting. The dose of hydroxyzine depends on the condition being treated, although a common dose is 25 to 50 milligrams every six hours. Hydroxyzine is also used for children, although the dose is calculated based on a child’s weight. However, the effects of sedation and drowsiness may lead instead to a prescription for one of the second-generation antihistamines such as Xyzal (levocetirizine.)

How It Works

Hydroxyzine works by blocking the h2 receptor, binding to them and decreasing the activity of histamine. Histamine allows more fluid to escape from the capillaries into the tissues, and it is this fluid you experience as a runny nose and watery eyes when you have an allergic reaction. Histamine also induces swelling and the production of wheels in rashes.

By blocking the h2 receptor, hydroxyzine is effective against these allergy symptoms. This can also help when itching (pruritis) is caused by allergies.

But hydroxyzine also crosses into the brain where it has further effects to cause drowsiness and sedation. These effects may not be wanted when treating allergies.

Second-Generation Antihistamines Derived From Hydroxyzine

The active metabolite of hydroxyzine is cetirizine (Zyrtec), which is available over-the-counter as a low-sedating antihistamine.

The active isomer of cetirizine is levocetirizine (Xyzal), which is available only by prescription and has become available in generic form. It doesn’t cross into the brain as readily as hydroxyzine, and therefore it doesn’t produce the same sedation. It also does not produce the anti-anxiety effects of hydroxyzine for the same reason.

Zyrtec and Xyzal are better for the treatment of allergic rhinitis than hydroxyzine since they have fewer side effects and a longer duration of action. They are also effective for the treatment of hives and itching.

Zyrtec and Xyzal are not effective for the treatment of anxiety, insomnia or nausea, and vomiting, conditions for which hydroxyzine or other first-generation antihistamines may be prescribed. (Despite hydroxyzine being a 50-year-old medication, it still has benefit for the treatment of various medical conditions.)

A review of studies of Xyzal (levocetirizine) in 2009 found that 5 mg/d was effective in reducing symptoms of seasonal allergic rhinitis (hay fever), perennial allergic rhinitis, and chronic idiopathic urticaria (chronic hives of unknown cause, improving the quality of life, with an acceptable tolerability profile).

An advantage of the second-generation antihistamines (Zyrtec and Xyzal) is the lack of unwanted side effects such as sedation, drowsiness, and difficulty in concentrating and learning. These side effects are of special concern when school-aged children are treated for long periods with hydroxyzine.

Xyzal (levocetirizine) is the only antihistamine with which there are no clinically relevant adverse effects on physical and psychomotor development in children aged six months to 12 years, according to studies.

That said, hydroxyzine continues to have non-allergy uses for the treatment of mild anxiety, insomnia, and nausea and vomiting. It may also be an inexpensive short-term medication for adults with allergies when the side effects of sedation may be beneficial (such as with insomnia or anxiety).

Take this medicine by mouth with a full glass of water. Follow the directions on the prescription label. You may take this medicine with food or on an empty stomach. Take your medicine at regular intervals. Do not take your medicine more often than directed.

Talk to your pediatrician regarding the use of this medicine in children. Special care may be needed. While this drug may be prescribed for children as young as 6 years of age for selected conditions, precautions do apply.

Patients over 65 years old may have a stronger reaction and need a smaller dose.

Overdosage: If you think you have taken too much of this medicine contact a poison control center or emergency room at once.

What should I watch for while using this medicine?

Tell your doctor or health care professional if your symptoms do not improve.

You may get drowsy or dizzy. Do not drive, use machinery, or do anything that needs mental alertness until you know how this medicine affects you. Do not stand or sit up quickly, especially if you are an older patient. This reduces the risk of dizzy or fainting spells. Alcohol may interfere with the effect of this medicine. Avoid alcoholic drinks.

Your mouth may get dry. Chewing sugarless gum or sucking hard candy, and drinking plenty of water may help. Contact your doctor if the problem does not go away or is severe.

This medicine may cause dry eyes and blurred vision. If you wear contact lenses you may feel some discomfort. Lubricating drops may help. See your eye doctor if the problem does not go away or is severe.

If you are receiving skin tests for allergies, tell your doctor you are using this medicine.

Side effects that you should report to your doctor or health care professional as soon as possible:

Pharmacological action

Pharmacokinetics

Dosing regimen

Use during pregnancy and lactation

Hydroxyzine is contraindicated for use during pregnancy, during childbirth, during breastfeeding.

Use with caution in patients with hepatic impairment; dose reduction is required.

Symptoms
Symptoms observed after a significant overdose of hydroxyzium are associated with excessive m-anticholinergic blocking action, suppression or paradoxical stimulation of the central nervous system.
These symptoms include nausea, vomiting, tachycardia, pyrexia, drowsiness, impaired pupillary reflex, tremors, confusion, or hallucinations.
Subsequently, depression of consciousness, respiration, convulsions, a decrease in blood pressure or arrhythmia, including bradycardia, may develop.
Possible aggravation of coma and cardiopulmonary collapse.

Treatment
It is necessary to monitor the state of the respiratory tract, the state of respiration and blood circulation using ECG monitoring, to ensure adequate oxygenation.
Basic supportive measures should include monitoring of vital signs and observation of the patient until symptoms of intoxication disappear and for the next 24 hours.
In case of a mental disorder, it is necessary to exclude the intake of other drugs or alcohol, if necessary, the patient should be inhaled with oxygen, administered naloxone, dextrose (glucose) and thiamine.
In case of need to obtain a vasopressor effect, norepinephrine or metaramenol is prescribed.
Epinephrine should not be used.
Do not administer ipecac syrup if patients have symptoms or if lethargy, coma, or seizures develop rapidly, as this can lead to aspiration pneumonia.
In case of ingestion of a significant amount of the drug, gastric lavage with previous endotracheal intubation can be performed.
The use of activated carbon is possible, however, there is not enough evidence of its effectiveness.
No specific antidote exists.
Hemodialysis is not effective.
Literature data indicate that in the case of the development of severe, life-threatening, intractable m-anticholinergic effects that cannot be stopped by other drugs, it is possible to use a therapeutic dose of physostigmine. Physostigmine should not be used solely to awaken the patient. If the patient is taking tricyclic antidepressants, the use of physostigmine can provoke seizures and irreversible cardiac arrest.Physostigmine should also be avoided in patients with cardiac conduction disorders.

  • Use with caution in patients with renal failure, myasthenia gravis, difficulty urinating, constipation, dementia, a tendency to seizures, arrhythmias or taking antiarrhythmic drugs, with impaired liver and / or kidney function.
