The Link Between Gastrointestinal Issues and Kidney Stones: Preva Surgicare

What are kidney stones?

Kidney stones are painful accumulations of minerals that form in your kidneys and can cause serious pain. Often smaller stones can pass freely through your urinary tract without symptoms, but larger stones can become stuck in the urinary tract causing severe abdominal or lower back pain and other symptoms.

GI issues caused by kidney stones 

Normally, kidney stones cause symptoms such as pressure and pain in your lower back, fever, frequent urination, discomfort urinating, and bloody or discoloured urine. However, sometimes kidney stones can cause gastrointestinal symptoms such as nausea, vomiting, and stomach discomfort. 

If you’re experiencing sudden low back pain and gastrointestinal discomfort, don’t ignore the possibility that it might be kidney stones. 

Links between GI issues and increased risk of kidney stones

While it’s possible to experience gastrointestinal symptoms due to your kidney stones, there’s another way these two types of conditions are linked: having a history of gastrointestinal issues can increase your risk of kidney stones. 

Inflammatory bowel diseases

If you have inflammatory bowel diseases like Crohn’s disease or ulcerative colitis, you may experience frequent loose stools and diarrhea. On top of being uncomfortable, this can lead to dehydration which can lead to concentrated urine and kidney stone formation.

Additionally, you may experience malabsorption where your small intestine struggles to absorb nutrients. This could lead to fat in your small intestine binding to calcium, creating oxalates which then accumulate in the kidneys.  

Bariatric surgery

If you’ve recently had bariatric surgery for weight loss, it can affect the functioning of your GI tract. Normally, your GI tract absorbs particles called oxalates, which are the primary substance found in kidney stones. 

After a bariatric surgery, you can reduce your risk of developing kidney stones by drinking lots of water, reducing your sodium intake, and limiting foods with oxalates such as beets, spinach, nuts, chocolate, and tea. 

High protein diets 

If you’ve increased your protein intake with a diet such as the paleo diet or the keto diet, the increased animal proteins could increase your risk for kidney stones. These animal proteins can lead to an accumulation of uric acid in your urine. 

That’s why it’s important to discuss any new diets with your doctor and to make sure you limit your portions of meat to about the size of a deck of cards per meal.

They can be as small as a grain of sand, but kidney stones can pack a punch. One minute, you feel completely fine. The next, you’re doubled over in intense pain and searching for the strength to make it to the ER.


How can something so small hurt so much?


“Typically, you start getting symptoms once you get obstruction,” explains Steven Rosenberg, MD, a urologist at The Iowa Clinic. “The kidney kind of has a funnel where urine exits that’s called the ureteropelvic junction. An obstruction usually occurs when the stone passes out of the kidney and gets stuck in that funnel or in the ureter, the tube that’s between the kidney and the bladder.”


“You can have a kidney stone for 20 years and never know it. Then, all of the sudden, for no reason, it just decides to pass. And that’s the moment you develop pain.”


Kidney stone pain is not always severe — or easy to identify.


The kidneys are located in an area of the torso called the flank. They sit below the lower ribcage in the back. While you can certainly feel intense pain on either side of the back (depending on which kidney the stone is in), that’s not always the case.


“Kidney stones are almost always on one side, so the pain is either on the right or the left. Usually, you feel it in your back or on your side.” Dr. Rosenberg says. “But the human body is weird. It doesn’t always behave like the books say it should.”


You may experience abdominal pain. Or groin pain. It can start higher up near the kidneys and then move down to the groin. Your pain may be intense and short-lived. Or it could be vague and more like a stomach ache that just won’t go away.


“Non-obstructive kidney stones can sometimes cause aching or discomfort. Acute pain occurs when there’s obstruction, which means they’re blocking the kidney or passing,” Dr. Rosenberg says. “Usually kidney stone pain is sporadic. You can have intense pain followed by periods of no pain. It comes and goes.”


Everyone experiences pain differently and has different thresholds. So it can be hard to understand what’s going on with your body based on pain alone. You might think you’re fine once the pain lessens, only to have it come back just as suddenly as before. Because you haven’t fully passed the kidney stone until you see it.


Other warning signs of kidney stones may be more noticeable.


Not all kidney stones cause pain. Some don’t present any symptoms whatsoever. But when symptoms do appear, they are clear signs that something’s off and that you need to see a doctor:


1. Nausea and Vomiting


Kidney stones can make you feel sick to your stomach. But not in the way you normally would from the stomach flu or food poisoning. The pain can be so excruciating that it makes you queasy or even makes you puke. If the pain is radiating from your right side, these symptoms might lead you to think that your appendix burst — even more reason to get checked out.


2. Blood in the Urine


Seeing your pee take on a shade of pink or red is alarming. But it actually takes only a little bit of blood to change the color or urine. Some medications and red foods like beets and rhubarb can have this effect but you should get it checked out no matter what. Blood in the urine could be from a kidney stone or many other problems — some serious like kidney, bladder or prostate cancer.


3. Cloudy or Foul-Smelling Pee


Urine can change in other ways too. Kidney stones are born out of the crystallization of concentrated minerals in your pee. Concentrated urine is darker, cloudier and stinkier — similar to when you’re dehydrated. The strong odor is often compared to ammonia, but it’s more likely that the smell stems from a urinary tract infection than a kidney stone.


4. Problems with Flow


An obstruction by a kidney stone doesn’t only cause pain. It can create a variety of pee problems. As the stone moves further down the urinary tract close to the bladder, you may feel more urgency to go more often and feel pain when you pee. Kidney stones can even be on both sides at once and stop the flow of urine altogether, which, although rare, is a medical emergency.


5. Fever and Chills


Some people complain of flu-like symptoms when they have kidney stones. And that’s a bigger problem than a simple stone. If you’re running a fever, having chills or shivering, you may have an infection in your kidneys or urinary tract as well. You should get urgent medical attention.


Kidney stone symptoms and risk varies between men and women.


Men are twice as likely to develop kidney stones than women. But the gap has been closing in recent years. It’s not clear what causes kidney stones or the rates at which men and women get them.


Genetics play a big role for both sexes. Men who have a family medical history of kidney stones, are two to three times as likely to get them. Other risk factors relate to the conditions that contribute to stone formation: diet, dehydration, warm weather (which also causes dehydration) and the side effects of medications.


“The biggest risk factor is dehydration,” Dr. Rosenberg says. “People who work outside often get kidney stones because they’re chronically dehydrated. They need to drink more water because they’re losing fluid in a lot of ways.


The differences in the male and female anatomy can lead to different symptoms, too. For men, it’s like going into labor. They feel pain in their abdomen, lower back or groin as the stone passes through the narrow ureter and beyond. That can also cause some gastric discomfort, which is centered in the upper abdomen and can be dull and achy or throbbing pain. Women, on the other hand, may compare kidney stones to menstrual cramps that can escalate from dull aches to wincing pain.


Both men and women agree on one thing: kidney stones really, really hurt.


“Women who’ve had kidney stones and babies often say kidney stone pain is worse than labor pain,” Dr. Rosenberg says.


There’s always someone to talk to about your symptoms.


Kidney stone pain usually comes and goes. But you don’t have to wait for it to pass or wait for other signs to appear to see if you have a stone. Any time you’re experiencing severe pain, you should get help. Because you never know if it will actually pass or how long it will take.


And if you have pain with or without changes to your urine, nausea or vomiting — and especially fevers and chills — call your doctor. 


“No matter where you need to go — Primary Care, Urgent Care if it’s on the weekend or the ER if you’re miserable — we’ll help you and tell them you’re coming,” Dr. Rosenberg says.

You lose a little bit of everything as you get older. Life experiences make you much wiser but many aspects of your health peak before you even reach middle age.


Your kidneys peak at 30. After that, they begin a long, gradual decline, losing a little functionality over time. In most cases, you don’t even notice it — even when the decline is quicker and more problematic. By that time, the damage is done and your kidney condition is chronic.


What is chronic kidney disease?


It’s when your kidneys suffer permanent damage and loss of functionality over time. According to the National Kidney Foundation, 15 percent of adults in the U. S. have chronic kidney disease (CKD), making it more common than both prostate cancer and breast cancer.


But just because your kidneys don’t work like they used to, doesn’t mean you have CKD. Anyone can get it at any age but you’re at greater risk if you’re older than 60, have a family member with kidney disease or have diabetes, high blood pressure or heart disease.


These risk factors speed up the normal, gradual decline of the kidneys. If you have any of them, you should be aware of the signs of kidney disease.


What are the symptoms of kidney disease?


Because of its slow-developing nature, 90 percent of people who have CKD don’t even know it. Most people show no signs of disease until it’s too late and the kidneys are badly damaged.


You can’t reverse kidney damage. You can only keep it from progressing. So if you see these signs, you can catch CKD early and start treatment to prevent your kidneys from getting worse.


1. Your pee changes.


Your kidneys are like garbage collectors for your blood. They get rid of all the waste and extra fluids. They remove it from the blood and turn it into urine to be expelled from your body. So you can see signs of kidney issues in your urine or bathroom habits. You might:


  • Produce too much or too little urine.
  • Wake up in the night to pee.
  • Notice your pee is too foamy or bubbly.
  • Have red, purple or brown pee due to blood in your urine.
  • Feel pressure or have trouble urinating.

2. You have less energy. 


The kidneys have another important job — telling the body to when to make red blood cells, which carry oxygen throughout your body. Damaged kidneys stop producing the hormone that signals your body to make red blood cells, leading to less oxygen in the rest of your cells.


When the cells are tired, your body is tired. You feel fatigued, get exhausted easily or have trouble catching your breath. You may sleep even more without ever feeling rested. Since your brain is also not getting enough oxygen, you can get dizzy, faint and have trouble concentrating.


3. Your feet and ankles swell up.


Without proper disposal, the extra fluid and salt in your body can deposit in your lower limbs, causing swelling in your feet and ankles. You may also experience swelling elsewhere, in your face, legs, arms or hands. Swelling is more common in the late stages of kidney disease.


4. You don’t feel like eating.


The build-up of waste affects your other organs. It causes inflammation in your stomach. This makes you nauseous so you don’t want to eat anything at all. And when you do, you may end up vomiting.


Even your favorite foods can be less appealing. The waste affects your sense of taste and smell. It also causes bad breath, leaving you with a bad taste in your mouth overall. Many people stop enjoying the taste of meat and struggle with maintaining their weight.


5. You’re itchy all over. 


Poorly functioning kidneys throw off the balance of your blood. In addition too much waste, there can be too much of other minerals or too little of other essential nutrients. Since blood reaches every part of your body, so do the excess minerals like phosphorus that cause a rash and make your skin itch.


The itchiness can affect your whole body or be localized to one annoying spot, most commonly the back, abs, head and arms. And it tends to get worse at night, which can cause sleep issues.


Do you even need your kidneys?


Technically, no. You can survive without your kidneys, which is why it’s the most commonly donated organ. You can go on living a normal, healthy life with just one kidney. The remaining kidney will pick up the slack and continue filtering out the waste in your body.


On the flip side, kidneys are commonly donated because they are so necessary to your body functions. When you have kidney failure — the last and most severe stage of chronic kidney disease — your kidneys have completely stopped working. And you have to continue their critical work somehow, either through a kidney transplant or dialysis.


Kidney failure is almost always permanent. Without a new kidney through transplant surgery, you have to regularly undergo dialysis, where a machine does the important work of removing waste, balancing the chemicals in your blood and helping to control your blood pressure.


Is there anything you can do to stop the decline of your kidneys?


Some decline is to be expected. To prevent chronic kidney disease or kidney failure, you need to control for the other health factors that cause further damage — high blood pressure, diabetes and heart disease. Taking these steps not only save your kidneys, but also improve your overall health:


  1. Exercise often. Physical activity helps you manage your blood pressure and blood glucose. It also helps you maintain a healthy weight because carrying too much weight can overwork the kidneys. Exercising for at least 30 minutes on most days of the week will make you and your kidneys healthier.
  2. Quit smoking. Cigarettes damage your heart and kidneys as well as your lungs. If stop smoking, you can stop kidney damage from getting worse.
  3. Stick to a kidney-friendly, renal diet. By limiting the sodium, phosphorus and potassium in your diet, you help keep the kidneys from having to filter three minerals that cause health issues. You need to eat a variety of fresh fruits and veggies, pasta, white bread and rice, beans and lean meat. You may also have to limit your fluids or adjust your diet to balance out other nutrients in your body.
  4. Manage your medication. Everything that gets into your bloodstream eventually gets filtered by your kidneys. Many prescription and over-the-counter medicines like ibuprofen and naproxen can damage your kidneys if used long-term.

Your kidneys work silently, filtering out the bad to leave you feeling good. Making the move to a healthier lifestyle now can make you feel even better and preserve your kidney function longer.

How to Get Rid of Kidney Stones

Passing a kidney stone is a painful experience. Depending on the size of the stone, it’s an impossibility without proper treatment.

Spend some time outdoors and you’re bound to get a rock in your shoe. Every single step sends pain through the sole of your foot. You can’t walk again until you shake that little thing out of your shoe, often to find that it’s much smaller than the pain would make you think.


Now, imagine one of those pebbles being lodged in your kidneys or slowly moving through your urinary tract. There’s no way to shake it loose like a rock in your shoe. But there are a number of ways to get rid of it to get relief.


1. Pass the kidney stone.


Once you have kidney stone pain, it’s actually already passing. It may have been stuck inside your kidney for years before finally working it’s way down to the ureter (the tube connecting the kidney to the bladder), where it causes an obstruction and that terrible pain. From there, the stone might be able to pass the rest of the way on its own.


“Your probability of passing a kidney stone is based on its size. Small stones usually pass. Bigger stones usually don’t. But it’s not completely predictable,” says Steven Rosenberg, MD, a urologist at The Iowa Clinic.


A kidney stone of 4 millimeters (about the width of two stacked nickels) or smaller is highly likely to pass. But it can take up to a month. Stones of 6 millimeters or more are not likely to make it. For stones in that 4 to 6 millimeter gray area, just over half do pass naturally in about 45 days.


But even tiny kidney stones can cause considerable pain. So it’s less a question of size and more a matter of how you feel during this time. If taking an over-the-counter pain medication like acetaminophen or ibuprofen isn’t enough to ease your discomfort, you need to consider your other options.


2. Pulverize the kidney stone into tiny, passable pieces.


Lithotripsy is a procedure that breaks down kidney stones into fragments that are small enough to pass or easy to remove in a kidney stone surgery. One form of the procedure, extracorporeal shock wave lithotripsy (ESWL) is commonly used for stones in the kidneys and upper ureter.


“It’s completely noninvasive. We use an X-ray to localize the stone and a machine that generates shock waves focused on the stone,” explains Dr. Rosenberg. “It pulverizes the stone into little pieces that can then pass. It’s useful for small or medium-sized stones that are up in the kidney.”


ESWL has about a 90 percent success rate in breaking up kidney stones in just one treatment, he says. No invasive surgery is involved — it’s all external — so there are no restrictions or recovery afterward. Most people don’t experience significant pain passing the fragmented kidney stones, either.


3. Break up the kidney stone and pull out the pieces.

  


There’s another form of lithotripsy that uses a laser at the end of a scope that is passed up the urethra, through the bladder and into the ureter. Once the stone is located, it’s broken to bits with the laser, and the pieces are extracted.


“We can sometimes pull stones out with just a ureteroscope. But because of the size, position or shape, sometimes pulling them out isn’t possible,” says Dr. Rosenberg. “We do an ureteroscopy with lithotripsy to break the stone up into smaller pieces and then pull those out. And we usually place a stent to help the kidney drain while the swelling goes down.”


Ureteroscopy is typically used when a stone has already passed out of the kidney, down into the ureter. It’s a minimally invasive kidney stone removal procedure. There are no incisions, so it takes less than a week to recuperate.


“The nice thing about ureteroscopy is that the stone is removed. You don’t have to wait for it to pass,” Dr. Rosenberg says.  “All you have to do is recover from the procedure, and that’s usually pretty quick.”


4. Undergo kidney stone removal surgery.


More invasive surgery is the least likely scenario, reserved for large kidney stones that can’t be removed through other means.


“There’s a less common procedure called percutaneous nephrolithotripsy. We pass a needle into the kidney through the back. We enlarge the needle tract and then put a scope down into the kidney and break up big stones with lasers and other means. Then, we pull them out through a tiny incision in the back,” Dr. Rosenberg explains.


Percutaneous nephrolithotripsy is primarily used for kidney stones that are 2 centimeters or larger — or about the same size as a marble. These stones are way too big to pass and are stuck up in the kidney.


Percutaneous nephrolithotripsy may also be necessary for an oddly-shaped kidney stone that is unable to pass. It’s the most invasive of the kidney removal procedures, and it can take several weeks to recover.


5. Prevent kidney stones before they form.


The best — and least painful — way to get rid of kidney stones is to stop them before they start. Because once you’ve had one kidney stone, you’re more likely to get another. Some people are just predisposed to getting them.


“People who form stones, there’s a reason for it. And it’s usually something metabolic. Their body is putting out too much calcium or they don’t have enough citrate,” says Dr. Rosenberg.


The kidneys’ job is to process waste. And when your body sends too much of a mineral (like calcium or sodium) to the kidneys to flush out in the urine, it can collect and crystallize into a stone. While there are medications that help pull out some of the excess minerals or dilute concentrated urine, there are several simpler strategies to prevent kidney stones from forming:


  1. Drink (more than) enough water. You want to get enough fluids that you’re able to fill a one-liter bottle more than two times with urine each day, Dr. Rosenberg says. That’s not as simple as drinking two liters of water, which is just more than the often recommended eight glasses. You have to drink enough to consistently produce that output in your urine and not lose it through sweat or by other means.
  2. Splash in some lemon juice. “Citrate interferes with calcium stone crystallization and the best dietary form of citrate is lemon,” Dr. Rosenberg says. Drop a slice of lemon in your water, drink real lemonade and squirt lemon juice on your meals to get plenty of citrate.
  3. Hold the salt. Excessive sodium in your diet is another source of kidney stones. So you need to restrict sodium and be cautious eating at restaurants where meals are filled with salt. “The more you can eat fresh fruits and vegetables, cook your own food and manage the sodium intake, the better,” Dr. Rosenberg says.
  4. Moderate high-oxalate foods. Oxalate, or oxalic acid, is an antinutrient that’s bound to minerals in many plants that can also cause kidney stones. Dr. Rosenberg advises patients to limit food sources high in oxalates like spinach, rhubarb, beets, sweet potatoes, peanuts and almonds. Since the foods are very healthy, you don’t want to avoid them entirely.
  5. Eat less meat. Up to 85% of kidney stones are made of calcium. The other 15% are most often made of uric acid, which is commonly found in animal protein. Limit your intake of meat, including eggs and seafood, to a quantity no bigger than a deck of cards.

Most kidney stones are not dissolvable, Dr. Rosenberg says. So these dietary tips are unlikely to get rid of a stone after it forms. They do, however, reduce the amount of minerals in your kidneys and keep them from growing.


“We try to manage your diet and hydration first. It’s the best thing you can do to lower your risk of kidney stones,” he says. “It’s like a big chemistry lab in your urine. The idea is to keep the concentration low enough that the minerals float around instead of crystalizing and forming stones.”


If you’ve passed a kidney stone before, there very well could be others up there waiting like ticking timebombs of pain. You need to be diligent with your diet and keep an eye out for the early signs of kidney stones.

He finds great joy in the one on one contact with his patient inside the exam or operating room and development of long term patient relationships. At this point in his career, Dr. Rosenberg has had patient relationships lasting more than two or three decades. Dr. Rosenberg practices general urology with a particular focus on pediatric urology, open cancer surgery, stones, and treatment of benign prostatic hyperplasia.

In his free time, Dr. Rosenberg loves to travel, especially to Santa Fe, New Mexico to feed his love for American Indian art and culture. Most people have two kidneys located behind their abdominal organs along the middle of their back. The purpose of your kidneys is to filter blood and send any waste that’s collected into your urine.

Sometimes, these waste products (usually salts or minerals) build up and form small, hard deposits called kidney stones. Stones develop inside kidneys, but they exit the body through the urinary tract.

Because the ureter is small, passing a stone can cause complications inside the urinary tract, as well as a range of unpleasant symptoms from abdominal pain to nausea. About 1 in 10 Americans will experience at least one kidney stone in their lifetime, but learning to recognize the telltale signs of a kidney stone isn’t always easy — especially if you’ve never had one.
