What Causes Back Acne? Bacne Triggers and Treatment

Meanwhile, fluctuating hormone levels can be a contributing factor to bacne flare-ups. “The most common pattern is for a flare to happen 7 to 10 days before the onset of [menstrual] bleeding and subside once bleeding starts,” Hadley King, M.D.9, clinical instructor of dermatology at the Weill Medical College of Cornell University in New York, tells SELF. This is because there’s a mid-cycle rise in progesterone that stimulates the production of sebum. Androgens (typically referred to as “male” hormones) like testosterone also stimulate sebum production during this time. The potential result? Oily skin, clogged pores and a new round of bacne10.

You might notice bacne flareups during other hormonal changes too (think during or after pregnancy or during perimenopause and menopause). Hormone-related conditions, such as polycystic ovary syndrome, can also spur bacne thanks to the intense hormonal shifts it can cause.

How your particular genetic makeup and hormones interact with other aspects of your lifestyle adds an additional layer of potential back acne causes. If stress is an ongoing problem for you, for instance, this could be one of the reasons why bacne is too—the body produces more androgens in response to stress, which peer pressures oil glands to kick into high gear7.

Excessive sweating is another common bacne precursor—say, from work or living in a warm climate. “When sweat sits on the skin for an extended period of time, it can lead to clogged pores,” Dr. Garshick says. Add to the mix tight athletic clothing causing friction as you exercise (and trapping heat and sweat against your skin in the process), and you’ve got a perfect storm for bacne breakouts4.

An uptick in back acne can also be a side effect of certain medications, including drugs that contain corticosteroids, testosterone, or lithium, according to the Mayo Clinic.

Though research is still shaky on the subject, there’s a chance your bacne breakouts could be linked to certain aspects of your diet as well. Studies suggest that foods with a high glycemic index (white bread, white potatoes, pastries, white rice, milkshakes) may be prominent acne triggers. “This is probably because of the blood sugar spike that results and how this affects our insulin levels and other hormones,” Dr. King says. Ditto dairy, she adds: “Milk and dairy products may promote insulin secretion and the production of hormones, such as IGF-111, which is known to be a major contributor to acne development12.”

How many types of acne are there?

There are several types of acne that can become a series regular on your back. And it’s helpful to understand what type you have so you can better treat your acne. Read on to learn how to recognize each type of bacne you might experience.


Also known as closed comedones, whitehead pimples strike when pores become clogged with excess oil, bacteria, and dead skin cells. “Just as they do on other areas of the body, whiteheads present on the back as a closed pore with a raised white or skin-colored tip on them13,” Deanne Robinson, M. D.14, assistant clinical professor of dermatology at Yale University of Medicine in New Haven, Connecticut, tells SELF.


Like whiteheads, blackheads (or open comedones) are a result of clogged pores. But unlike whiteheads, “there’s an opening that exposes the oil and dead skin cells trapped within the pore to the air, causing the buildup to oxidize and turn a dark color13,” Dr. Garshick says. (Both whiteheads and blackheads are non-inflammatory forms of acne, Dr. Robinson says, which means you may not know they’re even on your back unless someone else sees them.)


This type of acne is what most people refer to as pimples. “Papules occur when excess oil, dead skin cells, and bacteria become trapped deeper into the skin and lead to inflammation (redness and swelling),” Jacquelyn Sink, M.D.15, board-certified dermatologist at Northwestern Medicine Central DuPage Hospital in Winfield, Illinois, tells SELF. “They manifest as small, firm bumps13.” Depending on your skin color, papules can be red, brown, or other hues.


These zits present much in the same way as papules, but are larger in size and contain a yellow- or white-ish fluid. “Pustules typically appear as a bump that may have a central pus bump,” Dr. Garshick says. “Not only can these be present in acne, but in various types of folliculitis (inflammation of the hair follicle)13.”


A highly inflamed form of acne, nodular acne are deeper, firmer bumps located underneath the skin. “Nodules may appear skin-colored or red, depending on how inflamed they are, and may be associated with tenderness to the touch,” Dr. Garshick says. They tend to be larger than papules and pustules and require treatment by a dermatologist to prevent longterm scarring13.


Cystic acne involves larger, more painful lumps underneath the skin caused by deep-seated inflammation. Because they’re filled with pus, cysts tend to be softer than nodules and may be associated with redness or inflammation, Dr. Garshick says. They may drain or have an overlying crust and can cause scars when they heal.

The first thing to know about how to get rid of back acne is that it’s not like the pimples you get on your face. Yes, it’s similar in that the pores clog and bacteria is often to blame, and also you get incredibly confused and angry and emotional about it (especially if you’re on steroids). But it’s different in that it comes with its own rulebook for treating and preventing it.

Luckily bacne is very treatable and very preventable. And I didn’t know all the ways to keep it at bay until I sourced a bunch of board-certified dermatologists for the answers. We got the advice they often give their own patients in regards to back acne, and now they’ve got your back, too: Here’s how to get rid of back acne.

Swap Out Your Shower Products

Start with a benzoyl peroxide (5-10%) body wash:

Nearly all of the dermatologists cited benzoyl peroxide body wash as their top suggestion for clearing bacne, since it aggressively kills the bacteria that cause you to break out back there. But be warned: It can stain your towels and clothing, so rinse thoroughly when done, and maybe wear a white t-shirt to bed afterwards.

“Low glycemic index choices (meaning low in simple carbohydrates) have good results. So does increasing omega 3s which we can find in salmon, avocado, and walnuts,” says Dr. Sara Greer.

Mind Your Hygiene

Wash your towels and clothes frequently:
“It is important to wash your towels frequently or switch to an antimicrobial fabric so that bacteria doesn’t sit and then keep getting transferred back to the skin,” says Dr. Kavita Mariwalla. Ditto for your clothes, especially anything you’ve sweat in: It becomes a breeding ground for bacteria.

You can keep it from the general public, but it’s hard to conceal back acne in the bedroom or at the beach.

To banish “bacne”, you need to first understand its various causes. In some cases, minor changes to your hygiene regimen or diet might clear everything up. Here are three of the things that conspire behind your back, each of them a likely candidate for big breakouts. Take notes, make adjustments, and soon you’ll be back in business.

1. Genetics:

Acne—of all kinds—is often genetic. If you have oily or acne-prone skin on your face, you may have it on your back and shoulders, too. Similarly, everyone’s body reacts differently to certain foods. One guy might be able to eat a block of cheese with no effects to his complexion, while the next might suffer the consequences for a week.

How to minimize breakouts:

If you’re naturally acne-prone, you have to be very proactive with your skincare regimen—more than everyone else. Try showering with mild water (not burning hot!) twice daily. Use non-comedogenic soaps and body cleansers (meaning, they are engineered to keep your pores unclogged). You can stick to some of the name brands, like Dove Men+Care’s Clean Comfort bodywash. Also, speak with a dermatologist or dietician about the foods you eat, to pinpoint which ones might be best avoided.

2. Poor hygiene and diet:

First, a note on your diet: Minimize sugary foods, and drink a lot of water. Most foods play into the “genetics” category, and affect people differently (dairy is one such example). But high sugar intake will almost always contribute to a breakout, as will dehydration.

As for hygiene, wear clean shirts every day. Wash your bed sheets once every week, or sleep with a clean t-shirt every night if you prefer to wash your sheets less frequently. (But really, wash them frequently.) You need to shower daily—and immediately following a workout—to keep pores unclogged. Shower morning and night with mild water if you’re naturally oily. If you shave or wax your back, treat it with a witch-hazel aftershave to disinfect the pores. (Also, be sure you’re using a clean razor.) Then, let everything breathe (in other words, go shirtless, if you can) for a few hours afterwards. Prevent sweat from getting in the pores as they close, and you stand much better odds of keeping your skin healthy.

If you have a severe breakout—particularly after shaving or waxing—try using a drug-store disinfectant like Hibiclens until everything clears up.

3. Hormones:

There’s a reason a lot of bodybuilders get major bacne: They’re taking steroids or hormonal enhancements, and the body has to get rid of excess testosterone through the skin. This isn’t to say that guys who have higher natural levels of testosterone will get bacne more often. Instead, it means that when your body experiences periodic spikes—which happens naturally to all of us—you’re more prone to a single breakout. But apart from not taking performance enhancers, you can’t really control when your body experiences spikes or lulls in testosterone.

How to minimize breakouts:

Since breakouts will inevitably happen, your goal should be to lessen their effects when they pop up. Develop a healthy, sustainable skincare routine, adjust your shower temp (not too hot, not too cold) so that you can rid your skin of excess sebum (the result of testosterone being excreted through your pores). Do this twice daily if you take any testosterone boosters, and always shower immediately after working out (good hygiene is still key). Also, it should go without saying: Don’t take hormonal enhancements if you can avoid it. If you have sever hormonal acne, a dermatologist can prescribe more intense topical treatments.

If you say that you have never ‘faced’ acne at any age, then you are probably were a blessed star-child with extra-terrestrial genes, because a majority of human being have dealt with acne at some point or the other in their lives.

Case study: Koninica Dasgupta, 20, got up to itchy, scratchy, back that had developed small nodules. She and her mother surmised that it was some form of allergy but a visit to a dermatologist revealed that Koninica was suffering from back/truncal acne. What is truncal acne?

Doctors say that acne vulgaris is a common skin condition that is characterized by an area of seborrhoea, comedones, papules and nodules that occur on the face, chest, back and upper arms. Dr Sadaf Rangooni, consultant dermatologist says, “Of this truncal acne i.e acne on the back and chest are estimated to occur in 61% patients.”

Why does back acne occur?

Dr S S Trasi, dermatologist explains, “Back acne, like acne of face and elsewhere are caused due to the activity of the oil glands. We have maximum oil glands on the face, shoulders, the back and the buttocks, hence acne appear on these areas more.

Men are more prone to back acne

“Acne is basically hormonal in nature. Usually, women get pimples more on the face and men get more pimples on the back,” says Dr Trasi.
Causes of acne on the back

1. Dandruff
Pityriasis Capitis (dandruff) is characterized by excessive flaking and abnormal shedding of the skin.

2. Seborrheic dermatitis
Seborrheic dermatitis- is an inflammatory condition characterized by scaling and erythema.

PCOD- polycystic ovarian diseases also causes Truncal acne.

4. Laser treatment
Acne post laser treatment- it can cause folliculitis due to the heat generated by the laser therapy.

5. Shaving and waxing
Shaving and waxing can also lead to folliculitis.

6. Friction or heat
Any kind of occlusion, friction or heat can also cause acne.

7. Acne prone skin
Also seen in patients who have acne prone skin.


It is a little difficult to treat acne on the back because skin is thicker in the are- feels Dr Trasi. She suggests, “Usually acne leave scars are pigmentation which can be treated with lasers.”

Talking about how to get rid of acne on your back, Dr Sadaf says, “The patient should be put on topical combination of clindamycin and benzoyl peroxide or retinoic acid derivative. In resistant cases, systemic antibiotic should be given.” He adds that the patient should use benzoyl peroxide bathing soap and a salicylic acid peel should be done in an interval of 15 days. Also, you can use these following home remedies to get rid of the bacne:

1. Baking Soda
Baking soda can be miraculous when it comes to acne. It helps in exfoliating the skin and neutralize the pH parity of skin. Just apply a paste of 4 tsp and water on the affected area and wash after few minutes.

2. Apple Cider Vinegar
Often used for getting rid of the acne generally, apple cider vinegar can also be used for getting rid of the bacne (back acne). It helps in neutralizing the pH of the skin and has amazing antibacterial properties. Blend water and vinegar, fill it into a spray bottle and put it into a refrigerator. Every night spray the mixture on the affected area before sleeping. Do it 2-3 times a day.

3. Lemon Juice
Lemon Juice is loaded with citric acid and is very effective in cleansing the pores deeply. Just cut a lemon in two halves and rub one half on the affected area. Leave it for a few hours and wash with cold water after that. Make sure you don’t overdo as it may leave your skin dried up.

4. Honey
Honey has amazing medicinal properties and is a popular ingredient in most home remedies. A mixture of honey and oats will help you cleanse oil and the pores altogether, soothing your back. Just boil oats and add honey to it. Keep aside to cool. If the mixture dries after cooling, add more honey. Apply this mixture once every day and leave for 20 minutes. Wash it later.

Precautions and prevention

Doctors say that prevention in entirety is not possible since it is a condition driven by hormones. However, the effects can be reduced to some extent if care is taken. “As soon as you see the first pimples appear, you can start using anti-acne creams like retinol and benzol peroxide creams. It can reduce the intensity of breakouts and the subsequent scarring,” says Dr Trasi. It is important to treat the underlying cause i.e dandruff, seborrhoea, PCOD advise experts. “You should moisturise your skin before any parlour treatment. Application of ice or aloe vera gel post laser treatment is important,” says Dr Sadaf. Applying sunscreen regularly is very important say dermatologists.

Dealing with acne scars

I’m going to come out and say it: I have back acne. I started to notice it about a year ago, when my quarantine workouts got more intense. Knowing that back acne is common doesn’t make it any less annoying. I’m not the sort of guy who takes his shirt off in public very much (or, let’s get real, in private either), but I’m already stressed about the coming months. I don’t want to be on the beach worried the whole time that people are staring at my back pimples, you know what I mean? Even if no one else can see my bacne on a daily basis, I can—every time I look in the mirror. And I want it gone, if for no reason other than my own peace of mind. So, I asked Dr. Bhanusali how to squelch those zits for good.

Before knowing how to treat back acne (or any kind of body acne, for the record), you have to understand what’s causing it. Just like the pimples on your face, back acne can be caused by “bacteria, sweat, oil, and clogged pores,” says Dr. Bhanusali. Some people just have oil-prone skin all over their bodies and other factors like hormones can play a part (testosterone particularly can increase oil production). But in most cases, sweat is a major factor; that’s why back acne is so common in people who work out frequently. “Sweat can cause blockage of the pores which can lead to bacterial overgrowth [that ultimately leads to pimples],” he says. The longer sweat sticks around on your skin, the more time the bacteria has to thrive.

To make matters a bit more complicated, the pimples you see on your back may not be actual acne. They could be folliculitis, which is inflammation of the hair follicles. Think about it: men have hair everywhere, which means we have hair follicles everywhere. Similar to a clogged pore, these follicles can also get clogged with things like sweat and oil, which invite bacteria to infect the follicles. There’s also something called yeast folliculitis, says Dr. Bhanusali, which is when yeast gets down in there instead of bacteria. “Sweat creates an environment that’s ideal for yeast growth,” he says and can happen anywhere sweat sticks around (in another area of the body it has a different name: jock itch).

There’s also acne mechanica, a type of acne caused from something rubbing against your body for long periods of time. It’s usually seen in athletes who wear pads and helmets and “do the same thing over and over causing repeated trauma and friction to the area,” says Dr. Bhanusali. In non-athletic life, it could come from a backpack or sitting in a chair for a long time while you’re sweaty.

If all of this sounds complicated to you, don’t worry. You don’t need a medical degree to treat back acne. While the pimples on your back might be caused by a variety of things, the treatment itself is pretty much the same. Follow these steps to clear up bacne for good and prevent it down the road.

Benzoyl Peroxide Is Your Friend

Like many acne-fighting ingredients, benzoyl peroxide helps to clear away oil and dead skin cells that are caught inside your pores, but it’s also antibacterial, so it helps kill the bacteria that’s caught in there too. Using a body wash or soap that contains benzoyl peroxide is a “tried and true” solution, says Dr. Bhanusali, but warns to be careful with your towels. Retinoids help to stimulate cell turnover and remove dead cell buildup from your skin (which, when left there, can cause blockages in your pores). It’s commonly used on the face, but Dr. Bhanusali says it can work just as well on your body. “Apply it generally in the areas where you have acne,” he advises, not as a spot treatment. “You might get a pimple in one place today and another place tomorrow,” so by using it over the whole area, “the goal is to prevent that.”

Spot Treat Like On Your Face

Generally, you want to use products that can easily cover a large area like your back, but if you have just a few pimples here and there—or a few especially large ones—you can use the same spot treatments that you use on your face. “Like, right away,” says Dr. Bhanusali. The longer sweat sticks around on your body, the more chance it has to get down into your pores and the more likely it is that bacteria will grow. Even if you’re pressed for time after a workout, do a quick rinse with soap or acne-fighting body wash to get it off your skin as soon as possible.

If you’re using the right products and taking the right steps, but your bacne sticks around, think about other aspects of your life that could be contributing to it. Hormone fluctuations and diet can play a role in chronic acne. “High-glycemic-index diets tend to cause acne and are pro-inflammatory,” says Dr. Bhanusali, who also points out that the studies focus on facial acne, but there is likely some impact on body acne as well. Consult your dermatologist if your acne persists to see what changes they would suggest.
