Ingrown toenails are painful. It’s as simple as that. If you’ve ever experienced an ingrown toenail, you know first hand how uncomfortable they feel. Not only do you have to experience the pain of having one, but you also have to deal with having ghost symptoms. For example, you’ll be positive you are getting another ingrown toenail, but it’s just your shoe rubbing against your toe. The relief that is felt by not having an ingrown toenail can only be known from the memory or fear of having one.

Why Do I Get Ingrown Nails?

Cutting nails wrong. Getting ingrown nails is an easy mistake if you are cutting your toenails wrong. While rounded nails may seem fashionable, it is better to cut straight across to avoid angling the nail into the skin.

Poor footwear. Though shoes can cause ghost-like symptoms, they can cause ingrown nails as well. It is important to choose footwear that doesn’t put too much pressure on your toes, high heels are one example.

Injuries. An ingrown toenail may also be the result of an injury, like stubbing your toe, dropping something on it, or repeated movements using your feet. For example, some athletes are more prone to developing an ingrown toenail due to the requirements of their sports. Soccer players, ballerinas, and football players are all at higher risk. The pressure and trauma on their toes ultimately leads to the beginning of an ingrown toenail.

How Do I Know if I Have an Ingrown Toenail?

An ingrown toenail, usually found on the big toe, will make the skin around it feel tender or hard. You may even notice some fluid building up around the nail. If not treated, an infection may develop and cause puss to drain from the area. It is important to treat an ingrown toenail as untreated nails may result in necessary surgical treatment.

How Can I Treat a Non-Infected Ingrown Toenail at Home?

Soaking your feet can help relieve some of the pain and fluid build-up around the toenail and help soften it away from the skin. You can also try pushing the toe away from the skin with a cotton ball or dental floss. If you are worried about infection, use a topical antibiotic to prevent it. While home remedies are helpful and can relieve pain, not everybody reacts to the treatments. You may need to see a doctor or podiatrist.

What is an ingrown toenail?

An ingrown toenail occurs when the side of the nail curls down and grows into the skin around the nail.

Any toe can be affected but it commonly occurs in the big toe.

An ingrown toenail can become painful and inflamed (tender, red and swollen).

Sometimes, it can become infected, which, if left untreated, can spread and infect the underlying bone.

What are the symptoms of an ingrown toenail?

If you have an ingrown toenail, the area may be:

  • painful or tender
  • red
  • swollen
  • surrounded by hardened skin

If the area becomes infected, you may notice fluid or pus, and symptoms that get worse rather than better.

An ingrown toenail occurs when the side of the nail grows into the skin around the nail. It can become painful and sometimes become infected.

What causes ingrown toenails?

You can get an ingrown toenail if you:

  • have tight fitting shoes, socks, or tights that crowd your toes, putting pressure on your toenails
  • cut your toenails too short or not straight across
  • injure your toe, for example by stubbing it
  • pick or tear the corners of your toenails
  • have sweaty feet, making your skin soft and easier for toenails to dig in
  • have toenails with naturally curved edges or that are fan-shaped

Ingrown toenails become more common in older people as the nails get thicker with age. They are more likely to become infected in people who have diabetes or circulation problems.

How are ingrown toenail treated?

If you have diabetes, nerve damage in your leg or foot, poor blood circulation to your foot or an infection around the nail, see your doctor or podiatrist (a trained therapist who diagnoses and treats foot conditions) immediately.

Otherwise, try this:

  • Soak your feet in warm water for 15 to 20 minutes, 3 or 4 times a day.
  • Then use a cotton bud to gently push away the skin from the nail.
  • Repeat each day for a few weeks, allowing the nail to grow.
  • As the end of the nail grows forward, push a tiny piece of cotton wool or dental floss under it to help the nail grow over the skin and not grow into it. Change the cotton wool or dental floss each time you soak your foot.
  • Do not pick at the toenail or try to remove it yourself.

An infected toenail may be treated with antibiotics. Your doctor or podiatrist may need to drain the pus. In severe cases, part or all of nail may need to be removed.

Can ingrown toenails be prevented?

To help prevent an ingrown toenail:

  • wear shoes that fit properly
  • keep your feet clean and dry
  • trim your nails properly — briefly soak your foot in warm water before trimming, and make sure you cut straight across, without tapering or rounding the corners or cutting them too short

People with diabetes or persistent foot problems should see a podiatrist regularly for routine foot checks and nail care.

An ingrown toenail is one of the most common issues that can affect your feet. In some cases, if you apply proper home care — like keeping the area clean and dry — an ingrown toenail will correct itself. 

But if your ingrown toenail doesn’t go away quickly, it can leave you with discomfort when you wear shoes or walk. And left untreated, an ingrown toenail can create an infection that may spread to your bone.

Common causes of ingrown toenails 

You have an ingrown toenail when the front edge or side of a toenail digs painfully into the soft tissue at the outer edges of your nail bed. Healthy toenails, on the other hand, grow straight out from the nail grooves.

Some of the biggest factors that contribute to the development of ingrown toenails include:

  • Shoes that crowd or pinch your toes together
  • Cutting your toenails too short
  • Cutting your toenails with a curve instead of straight across
  • Toenail injury (e.g., from stubbing your toe or dropping something heavy on it)
  • Hereditary factors making your nails to curve rather than grow straight across

Ingrown toenails are most common on the big toe, but any toenail can grow inward. Your nail might be ingrown on one or both sides of the nail.  

The symptoms of an ingrown nail

In the beginning, the symptoms of an ingrown toenail are usually mild. You might experience a twinge of discomfort when you touch the affected nail, wiggle your toes, or squeeze into any shoes with a tight toe box, for example.   

Over time, though, the ingrown toenail can get worse, causing symptoms like:

  • Persistent pain that occurs in your toe along one or both sides of the nail
  • Swelling and redness of the affected toe
  • Drainage, pus, plus other symptoms of infection surrounding the nail

When to visit us 

If you have diabetes or another medical condition that affects circulation in your feet, the American Podiatric Medical Association (APMA) recommends seeing a podiatrist as soon as you notice the ingrown toenail. 

If you have healthy feet, you can wait to make an appointment with Dr. Kaplan until the ingrown toenail causes pain, or you notice redness, swelling, or other signs of problems. 

Generally, you can try at-home care for about a week, but don’t wait to seek treatment if it doesn’t heal by then. Even otherwise healthy individuals can develop an infection that may quickly spread to the bone of the affected toe. 

Preventing ingrown toenails

You can do a lot to lessen the likelihood of ingrown toenails, including:

  • Cutting your toenails regularly 
  • Trimming your toenails straight across 
  • Wearing well-fitting shoes with a roomy toe box that doesn’t crowd your toes
  • Checking your feet regularly (daily if you have diabetes) for nails that are growing inward

While an ingrown nail can occur on either the hands or the feet, they’re most common in the toes, especially in the big toes. Ingrown toenails occurs for one of two reasons:

  1. The skin grows over the nail itself, on one or both sides
  2. The nail grows into the surrounding skin

No matter the cause, the initial result is usually a painful, red swelling at the corner of the nail. This may be followed by an infection. If an infection occurs, you’ll likely see a small amount of pus ooze from the cut.

The stages of an ingrown toenail

Ingrown toenails usually progress in three stages:

Stage 1

As the nail first digs into the tissue, typical symptoms include pain at the corner of the nail and inflammation and swelling of the toe.  

Stage 2

If the toe isn’t treated in the first stage, the inflammation often leads to infection in the skin. The toe may leak blood, pus, or both.

Stage 3

If the infection isn’t treated, you may develop granulation tissue, which is new skin that grows over the open infection, which can perpetuate and worsen the infection.

Causes of ingrown toenails

There are a number of reasons why ingrown toenails can occur, including the following:

  • Cutting nails below the edge of the toe
  • Rounding the edges instead of cutting them straight across
  • Wearing shoes that are too tight or socks that push the nail into the toe
  • Repeatedly stressing your toes, such as with ballet, running, or soccer
  • Genetics that predispose you to ingrown toenails

At-home treatments

As soon as you notice your toenail is becoming ingrown, you can start with at-home remedies to prevent progression to an infection. Some suggestions include:

  • Soak your toe in warm water 15-20 minutes, 3-4 times a day
  • Use Epsom salts in the water
  • Make sure your affected foot can breathe, and don’t let it sweat or become damp
  • Wear shoes with plenty of toe room
  • Apply antibiotic cream and cover with a bandage to protect the toe
  • Take over-the-counter pain relievers to reduce tenderness and swelling

If you don’t see an improvement in 2-3 days, or if the condition worsens, including becoming infected, make an appointment to see Dr. Wright.

When to see a physician

If you have swelling, pain, excessive inflammation, and discharge from your ingrown nail, you need to see a physician, because these symptoms indicate that there’s an infection.

If a skin infection is left untreated, it could lead to a bone infection in the affected toe. A toenail infection could also lead to open sores, known as foot ulcers, and a loss of adequate blood flow to the infected area. In addition, tissue decay and tissue death at the infection site could be possible.

An infection could be even more serious if you have diabetes. Because of a lack of blood flow to the extremities, an infection could quickly worsen. Furthermore, if you have diabetes, you have a higher chance of having peripheral neuropathy.

With peripheral neuropathy, there’s a lack of nerve sensitivity in the extremities, which means you may not feel the ingrown toenail in your foot, and an infection could worsen without your knowledge. Because of this, it’s imperative that you check your feet daily if you have diabetes.

Professional treatments for ingrown toenails

Dr. Wright has a number of treatment options at his disposal. What he recommends for you will depend on your situation. Among the options are the following:

  • Taking oral antibiotics to clear up the infection
  • Partially or completely removing the nail
  • Removing part of the underlying nail bed
  • Removing adjacent tissue
  • Removing part of the growth center

If the ingrown toenail causes you a lot of pain or if the infection keeps returning, Dr. Wright may recommend removing part of the nail (partial nail avulsion). With this treatment, Dr. Wright injects your toe with an anesthetic and uses scissors to cut away the ingrown part. According to the UK National Health Service, partial nail avulsion is 98% effective in preventing future ingrown toenails.

The Ins and Outs of Ingrown Toenails

You can’t go through life without something causing a pinch or irritation now and then…but you just don’t expect your own toenail to turn on you!

An ingrown toenail, however, is literally that—when the nail has curved and is starting to grow into the surrounding skin. The medical name for this is “onychocryptosis,” but people usually start backing away from us when we bring out that fact at parties.

While the big toe is the most frequent sufferer of an ingrown nail, it is not unheard of to see it on other toes as well. And which of your toenails has become ingrown often is related to the causes behind this problem developing in the first place.

The more we know about the causes of ingrown toenails, the more we can do to treat and prevent them. Here are some fast and useful facts on this condition, as well as what you can do about it.

What Does an Ingrown Toenail Look Like?

We won’t spend too much time on this one. Most people have a good idea of when they have an ingrown nail. What they might not know is when an ingrown toenail looks especially bad.

A standard ingrown toenail will usually exhibit:

  • Redness around the ingrown part of the nail.
  • Some swelling up around the nail.
  • Pain and tenderness, either by pressing against the skin or bearing weight on the toe.

These symptoms are typical and not usually much reason for concern. However, there are times when the case is especially severe or the toe might become infected. It is important to see us if you have any of the following problems:

  • Your toe is causing you a great deal of pain, or the pain is interfering with your daily life.
  • Your toe is oozing pus or discharge.
  • There is a growing or radiating redness that seems to be spreading away from the toe.
  • You have diabetes or another condition that interferes with your circulation, making any kind of sore or injury on your foot more dangerous.

What Causes Ingrown Toenails?

There are a few different ways that a toenail can start “going rogue” and digging into the surrounding flesh. In most cases, you can blame excess force or injury of some sort.

Here are some of the most common causes of ingrown toenails:

  • Wearing shoes that too tight or crowd your toes. Toes need space to move a bit inside your shoe. If they remain crammed up against each other for long periods of time, that’s placing a lot of force on them that may direct their nails to start growing inward.
  • Trimming your toenails incorrectly. Cutting your toenails too curved can encourage them to grow against the skin as well. If you cut your nails too short, you also risk causing minor trauma to the nail bed that can also encourage abnormal growth.
  • Injuring your nail. If cutting too close can injure your nail enough to make it become ingrown, just imagine what dropping something heavy on your toe can do! Trauma doesn’t just have to come from an accident, however. The repetitive impact of your toes striking the front of your shoe—such as from running—can cause the type of trauma that can lead to ingrown toenails as well.
  • Simply being born that way. Some people just have genetics that leave their toenails more prone to becoming ingrown. While shoe choices and preventing injuries can help keep more from happening, it is likely not to put a full stop to the condition.

What to Do About an Ingrown Toenail

Most ingrown toenails can be treated easily enough at home—as long as you’re not exhibiting any of the problem signs mentioned above.

  • Soak your foot in warm water for 15-20 minutes, 2-3 times per day. This will soften the nail, and can help relieve pain and some swelling, too. Feel free to add Epsom salts to the soak, if you wish. Completely dry the foot when done.
  • Carefully slide dental floss or tiny pieces of cotton between the ingrown nail and the skin. You can soak the cotton in a bit of water or antiseptic first, if you would like.
  • Use antibiotic ointment on the toe to help keep a potential infection at bay.
  • Cover the toe with a Band-Aid for some added protection and a little more comfort.

If it has been a few days and you have not seen any improvement—or if things have even gotten worse!—it’s time to give us a call.

One thing you should never do is try to dig into the nail with clippers or other instruments and try to remove a chunk of the nail. What Is an Ingrown Toenail?
When a toenail is ingrown, it is curved and grows into the skin, usually at the nail borders (the sides of the nail). This “digging in” of the nail irritates the skin, often creating pain, redness, swelling, and warmth in the toe.

If an ingrown nail causes a break in the skin, bacteria may enter and cause an infection in the area, which is often marked by drainage and a foul odor. However, even if the toe isn’t painful, red, swollen, or warm, a nail that curves downward into the skin can progress to an infection.

Causes
Causes of ingrown toenails include:

  • Heredity. In many people, the tendency for ingrown toenails is inherited.
  • Trauma. Sometimes an ingrown toenail is the result of trauma, such as stubbing your toe, having an object fall on your toe, or engaging in activities that involve repeated pressure on the toes, such as kicking or running.
  • Improper trimming. The most common cause of ingrown toenails is cutting your nails too short. This encourages the skin next to the nail to fold over the nail.
  • Improperly sized footwear. Ingrown toenails can result from wearing socks and shoes that are tight or short.
  • Nail Conditions. Ingrown toenails can be caused by nail problems, such as fungal infections or losing a nail due to trauma.

Treatment
Sometimes initial treatment for ingrown toenails can be safely performed at home. However, home treatment is strongly discouraged if an infection is suspected, or for those who have medical conditions that put feet at high risk, such as diabetes, nerve damage in the foot, or poor circulation.

Home care:
If you don’t have an infection or any of the above medical conditions, you can soak your foot in room-temperature water (adding Epsom’s salt may be recommended by your doctor), and gently massage the side of the nail fold to help reduce the inflammation.

Avoid attempting “bathroom surgery.” Repeated cutting of the nail can cause the condition to worsen over time. If your symptoms fail to improve, it’s time to see a foot and ankle surgeon.

Physician care:
After examining the toe, the foot and ankle surgeon will select the treatment best suited for you. If an infection is present, an oral antibiotic may be prescribed.

  • Proper trimming. Cut toenails in a fairly straight line, and don’t cut them too short. You should be able to get your fingernail under the sides and end of the nail.
  • Well-fitted shoes and socks. Don’t wear shoes that are short or tight in the toe area. Avoid shoes that are loose, because they too cause pressure on the toes, especially when running or walking briskly.
