Q. I’ve been plagued by plantar’s warts on the bottom of both feet for years. I’ve been treated three times by different doctors who have either removed these warts using a laser or have cut them out. Each time they returned.

I’m tired of the pain of foot surgery, but want to get rid of this problem. Are there any other methods for getting rid of these warts once and for all?

A. Plantar warts can be tough to get rid of. I’ll go over the different treatments for them, but if they’re not bothering you, it may be best to leave them alone.

About half of all warts will disappear by themselves within two years. Unless they’re painful or unsightly, you may be able to ignore them. But if you do get treatment, make sure it isn’t worse than the condition you started with.

Plantar warts are warts located on the sole or plantar surface of your foot. Many people mistakenly call them planter’s warts, and perhaps think they have something to do with working in the garden. The pressure of your weight makes them grow flat, under the skin, rather then heaped up, as they can on your fingers or other places.

Like all warts, plantar warts are caused by a virus. The virus is slow to spread, but it can break out on other parts of your body, as it has on both of your feet. It can also spread to another person, but not easily. One way of getting plantar warts is in communal showers.

Plantar warts are particularly hard to treat. The reason is that you may get rid of the wart, but the virus lies dormant beneath the skin. Weeks to months later, it can trigger another wart to grow.

The goal of treatment is to destroy the wart and its virus while causing as little damage as possible to healthy skin. Because you have to put weight on your feet, many treatments cause some degree of pain until your foot heals.

Doctors treat warts by several different methods. No one is obviously better than another, which is why so many treatments exist. They include:

Caustic chemicals, like salicylic acid and dichloroacetic acid, which destroy the top layer of skin without scarring the lower layers.

Blistering agents, like cantharone, which form a painful blister on the skin, lifting the wart off without leaving a scar.

Podophyllin, which selectively destroys wart tissue more than it does normal skin.

Freezing, using liquid nitrogen applied directly to the wart.

Burning, using an electric current or laser beam applied to anesthetized skin.

Surgery, cutting or scraping out the wart and leaving a scar.

When possible, it’s best to start with simpler treatments and progress to more destructive ones. Not only is that less expensive, but if you’re left with a scar, it can occasionally be painful.

The simplest remedy is home therapy using a non-prescription wart medicine like Compound W, which is salicylic acid. If your doctor recommends this treatment, you can apply it to the wart according to directions in the package. Other liquid treatments must be applied by your doctor, which adds the expense of several office visits. The most expensive, though not necessarily best treatments are surgery and laser therapy.

Before treating yourself, check with your doctor. You first need to make sure that your problem is indeed a wart and not some other skin condition. You’ll also be able to get some advice on how to take care of your particular problem.

5 Tips for Getting Rid of Warts

Having a wart can be cause for feeling embarrassed and self-conscious, especially as warts on the hand, an area of your body that is hard to cover up. The good news is that warts are easy to remove. Discover 5 tips on how to remove a wart, including home remedies, treatments from a doctor, and tips to prevent the spread of new warts.

What Is a Wart?

There are a few types of warts, which include the common wart, the plantar wart, and genital warts. The common wart is a benign (non cancerous), raised bump on your skin, often rough on the surface and flesh colored. They can also be dark or have a speckled appearance caused by small blood clots in the skin. The common wart is caused by a virus (human papillomavirus or HPV) and is spread by touch. You are more likely to get the virus if you have a cut on or damage to your skin, such as skin broken by a hangnail. Usually, common warts appear on your hands and fingers and are likely to be acquired if you bite your nails, which can damage the skin around your fingers. Children are more likely to get warts because their immune system is still developing. People with compromised immune systems, like those with HIV, also have a higher likelihood of contracting the HPV virus and getting warts.

Plantar warts are caused by the same virus as common warts, but usually grow on the bottom of your feet, especially on the weight-bearing parts of your feet, such as your heel. Plantar warts can grow in clusters and tend to be flat because the pressure on your feet causes the wart to grow inward. You will know they are there by the hard, thickened skin above the wart. They are also often painful or tender, especially when pressure is applied while walking or doing other activities while on your feet. The small black dots (often called “seeds”) that appear on the plantar wart are clots in small blood vessels. It is usually best to seek the help of a dermatologist in plantar wart treatment.

Genital warts are a common, sexually transmitted condition of the HPV virus. Like plantar warts, they can either grow alone or grow in a cluster, often looking like cauliflower. Often though, they are too small and flat to see. In both men and women, they can appear wherever sexual activity has taken place, such as the groin, the mouth, or the anus. In women, they appear on the vulva, vagina, cervix and groin, while in males, they are found on the groin, penis, scrotum and thigh. Genital warts often cause itching, tenderness, pain, or bleeding.

Home Treatments

Typically, if you have a strong immune system, your warts will go away on their own, but it can take a while, especially as warts can spread to other parts of your skin. It is always best to see a dermatologist to get a proper diagnosis and treatment plan for your warts, but there are some things you can do to get rid of your common warts at home. These treatments are not recommended for genital warts, which should be attended to by a dermatologist.

  • Salicylic acid: you can get this over-the-counter treatment without a prescription. It will either be in a gel, as a liquid, or on a pad or patch. You will need to apply the acid once a day, and should do so after soaking your wart in warm water. You can get rid of the dead skin that develops on top of the wart by filing the skin with an emery board or pumice stone. Make sure to use disposable emery boards or pumice stones, as re-using them can cause the wart(s) to spread.
  • Wart freeze: you can freeze your wart with an over-the-counter product that uses liquid nitrogen as a gel or a spray.
  • Duct tape: you may be able to get rid of your wart by covering it with duct tape. The Mayo Clinic recommends covering the wart with silver duct tape for 6 days, soaking it in water and filing away the dead skin, leaving it exposed for 12 hours and then repeating the process until the wart is gone.
  • Prevent the spread of new warts: there are many things you can do to prevent getting warts and to prevent new warts from forming. These preventative measures include: cover your wart to keep the virus from spreading, don’t pick at your wart, keep your hands and feet clean and dry, don’t bite your nails, wash your hands regularly, wear shoes when in public showers or pools, and use protection during sexual activity.
  • See a doctor: there are a few reasons why you should see a doctor about your wart. If you are at all unsure as to whether the bump is a wart, see a dermatologist — it could be a sign of skin cancer. You should also see a doctor if you have gential warts, if your wart bleeds or is particularly painful, if it is in a sensitive place like your face, if you have diabetes, or if you have a lot of warts. Plantar wart removal is best done by a doctor. Whatever type of wart you have, a dermatologist can treat the wart with a stronger acid than is available to you as an over the counter treatment. They can even perform minor surgery or use a laser to burn the wart off.

The Best Topical Treatments for Plantar Warts

Thankfully, plantar warts are easy to heal and only require one or several trips to the podiatrist’s office for topical treatments. What is a plantar wart?

A wart on the foot is known as a plantar wart. They are small, rough growths caused by a virus that has entered broken skin. Human papillomavirus, or HPV, is the direct cause of plantar warts. There are over 150 kinds of HPV affecting different parts of the body, but only a few are known to cause plantar warts.

Are plantar warts dangerous?

Certain types of HPV can cause cancer later in life, but the strain of HPV that causes plantar warts is benign. Warts are dangerous only because they are highly contagious. It’s possible for new warts to spread to the rest of the body without treatment. Plantar warts can be more painful than warts on other parts of the body because of the constant pressure placed on them from walking and other activities.

Despite being highly contagious and bothersome, most warts are not harmful. They will eventually go away within two years, but it is still recommended to see a podiatrist.  

Topical treatment options for plantar warts

Depending on the size and location of the plantar wart, your podiatrist may prescribe a topical treatment medication to apply at-home or perform a treatment in-office.

Cryotherapy

Cryotherapy is a topical treatment for plantar warts that deep-freezes an affected area. Typically, podiatrists use liquid nitrogen in cryotherapy to treat plantar warts, which is fast and effective. Some warts may disappear after one treatment, but other warts may require two or three treatments in total. Cryotherapy is an excellent topical treatment because it has very little risk for scarring and heals quickly.

The downside of cryotherapy is that it can be painful, particularly in areas where the skin is thicker, such as on calluses on the feet. A blister may also develop. Despite these downsides, the few treatments make cryotherapy a great option for treating plantar warts.

Salicylic acid

Another popular topical treatment for plantar warts is salicylic acid. Salicylic acid is a peeling medication that is applied over the wart in liquid, cream, or patch form. There are over-the-counter prescriptions for salicylic acid, but if the plantar wart is located on the sole of the foot where the skin is thick, a podiatrist will recommend a stronger prescription to effectively remove the wart.

It’s recommended to soak the wart in warm water before application or apply the treatment after a shower or bath. After patting the area dry, apply the treatment. Once the treatment has sat for the prescribed amount of time, exfoliate the area to remove dead skin cells from the wart. This treatment should be applied daily for the fastest results.

Salicylic acid is an easy treatment to do at home after the podiatrist has reviewed the wart. There are a few risks associated with this treatment, such as irritation of the skin, in which treatment should be stopped immediately and you should contact your podiatrist. Also, salicylic acid may take up to a few weeks of daily application for the wart to fall off. However, there is little to no pain and can be applied at home.

Electrodesiccation and curettage

These topical treatment options are more involved than salicylic acid and cryotherapy. They are generally recommended as the next step if the previous treatments don’t work.

Electrodesiccation and curettage are topical treatments that use an electric needle to cauterize the affected area and a small cutting tool to manually remove the wart. They are usually performed at the same time. Podiatrists will apply local anesthesia and use a low to medium current to burn the wart until it turns white. They will then cut the wart using the curettage tool and apply a postoperative ointment to reduce pain and scarring.

While electrodesiccation and curettage are not the first options recommended for patients with plantar warts, they are highly effective. The risks for these treatments include scarring and postoperative pain; however, the pain is lessened by an ointment and is overall less painful than cryotherapy.

Tips to prevent warts

The best practice for plantar warts is to prevent them from forming in the first place.

  • Wear shoes in public pools, bathing areas, and other public settings. If you are walking around a public swimming pool or taking a shower at a campground, be sure to wear flip flops or other shoes to protect your feet from plantar warts.
  • Bandage any cuts on the foot with a triple antibiotic ointment. Plantar warts arise from the HPV virus entering broken skin. To keep your feet clear of warts, apply a triple antibiotic ointment to heal the wound and bandage accordingly.
  • Wash your feet after being exposed to public areas. If you have been in a public setting and your feet were exposed, wash your feet with soap and warm water to keep your skin clean and free of unwanted germs.
  • Don’t touch other people’s warts or your own warts. This is obvious, but avoid touching other peoples’ plantar warts, as well as touching or scratching your own, as this increases the risk of spreading the virus.
  • Keep your feet dry. Warts love a moist environment, so prevent them from developing by keeping your feet dry after water activities or exercise.

Warts are noncancerous tissue abnormalities and are growths caused by a virus. What Caused My Plantar Wart?

Plantar warts are caused explicitly by direct contact with the human papillomavirus, the most commonly transmitted infection. Many people with HPV don’t develop symptoms, but can still infect others with the virus through direct contact. While there is no cure for HPV, many treatments for the symptoms involve wart removal. Vaccines are often the best preventative measure against HPV for children, and for those with plantar warts, it can happen in these variations:

  • Solitary – Also known as a single wart, these types of plantar warts increase in size and may eventually multiply and spread to other areas of the body.
  • Mosaic – Mosaic warts are multiple warts clustered together in one area. These formations are often more challenging to treat because of their proximity.

Plantar warts are often confused with corns or calluses due to their appearance. To best determine if you have a plantar wart, these symptoms most often occur:

  • Thickening skin – Plantar warts tend to be hard and flat, while corns and calluses have raised surfaces that develop like layers on top of the skin. Warts have a tough, thick tissue that’s grown and have well-defined features. 
  • Sharp-shooting pain – Sharp, burning pain often occurs with plantar warts due to their proximity to the foot’s weight-bearing areas. Any pressure placed on the wart causes intense pain and doesn’t have the irritating feel that corns and calluses produce. 
  • Tiny black dots – Plantar warts often appear gray, brown, or black and have pinpoints of dried blood underneath the surface.

How are Plantar Warts Treated?

It’s essential to note that plantar warts can be quite resistant to treatment and tend to reoccur due to the presence of the virus in the body. We do not advise at-home treatment due to the possibility of destroying healthy skin and tissue. Many at-home medications contain acids and chemicals that should be avoided by people with diabetes, cardiovascular, or circulatory disorders.

 Plantar warts are typically hard and grainy growths that appear on the heels and balls of the feet. Warts occur when the human papilloma virus (HPV) enters your body through cuts or weak spots on the bottom of the feet. Once plantar warts start to grow, they can take as long as two years to go away if not treated. Luckily there are plenty of treatments to reduce, remove, and prevent the inconvenience. Cryotherapy, also known as freezing therapy, is the most widely used method is removing warts, but it may not always work if the treatment does not reach the entire wart. About Plantar Warts

Warts are growths that are the result of a virus causing rapid growth of cells on the outer layer of skin. The virus, human papillomavirus (HPV), has 60+ types. While warts are commonly found on the hands or feet, certain types of HPV can cause warts in the genital area. Warts are contagious via direct contact with the virus. This can occur not only from person to person, but can also spread on the same person to other body parts that are in contact with existing warts. Following is information on the types of warts and ways to treat them.

Types of Warts

  • Common Warts: Common warts are usually found on fingers and toes, and have a rough, grainy, round appearance. The color is usually a little grayer than the surrounding skin.
  • Plantar Warts: Plantar warts are usually flesh-colored or light brown. They can have little black dots which represent tiny, clotted blood vessels. They are found on the soles of the feet and are not raised like other types. They can make walking uncomfortable.
  • Flat Warts: Flat warts are common in teens and children. They are smoother, flatter, and smaller than other types of warts. They are pink, brown, or yellow. Flat warts can occur on the face, arms, or legs.
  • Filiform Warts: Filiform warts are small flaps of skin (skin tag) that are found around the mouth, nose, or neck. They are usually skin color.
  • Periungual Warts: Periungual warts are found underneath and surrounding toe/finger nails. They can be painful and affect nail growth.
  • Genital Warts: Genital warts is the most common sexually transmitted infection in the U.S. It is characterized by small raised growths, usually red, pink, or skin colored, and can occur in clusters of 3-4. It can be spread rapidly to other parts of the skin, or to intimate partners. They can be found on the genitals, in or around the anus, and for women, in or around the vagina. Genital warts are not usually painful, but can sometimes cause mild pain, bleeding, or itching.
