That Anal Itch: How to Diagnose and Permanently Get Rid of Pinworms

That mom’s face said it all — her expression changed from worried to grossed-out as I showed her the squiggling worm protruding from her daughter’s anus under the bright light of my examining room.  She had brought her toddler to the emergency room because she was awakening in the night and scratching her bottom and complaining of rectal itch.  Could it be sexual abuse, she asked?  No, I reassured her, just pinworms.

An estimated 20-40% of American children carry pinworms at any given time.  They are also known as threadworms, or enterobius vermicularis.  They are ubiquitous in our community and anyone can catch them.  Pinworms live in the digestive tract and especially the anus.  Pinworms can also infest the vagina in females.  The best prevention is strict hygiene, but let’s face it, what toddler has perfect hygiene?  If your child is a nail biter or thumb sucker, it’s nearly impossible to prevent pinworms.  And once pinworms are in your house, they often spread to the entire family, including parents.

Here’s the good news: pinworms are easily treated with either over-the-counter or prescription medications.  Before you rush off to Walgreens to buy Reece’s Pinworm Medicine or Pin-X, make sure you really have pinworms.  It’s easy to diagnose at home, and these tricks work for grown-ups, too.

How to tell if your child has pinworms

If your child is complaining of anal itching, especially at night, check on them in their bed about 2-3 hours after they fall asleep.  Pick a night when they didn’t have a bath, if possible.  Bring a good flashlight, and have your child lay on their belly with their buttocks in the air.  Spread the butt cheeks and get a good look at the anus.  Ask your child to push out like they are having a bowel movement while you are looking, which will help expose the anus.

Look carefully for one or more white worms about 4-12 mm long and about as thick as a piece of white thread or dental floss.   If you see it move, it’s a worm.  Pick up the worm with a piece of scotch tape (the transparent kind) so that you can prove your diagnosis to your physician, your child, or anyone else who doesn’t want to believe the ugly truth of pinworms.  If you don’t see anything, or if you’re not sure what you’re looking at, just apply a piece of scotch tape to the anal area, crossing from about 2 cm on one side of the anus to about 2 cm on the other side of the anus.  Press firmly, then stick the tape to a glass microscope slide, sticky side down.  If you don’t have a glass slide, you can get one from your physician.  The pinworm eggs will stick to the tape and your doctor will be able to see them under a microscope, diagnosing the pinworms.  We call this the “scotch tape test.”

How to treat pinworms at home without a prescription

If you found a worm and saw it move, you probably don’t need to see your physician.  Pinworms can usually be treated completely with two doses of over-the-counter medicine called pyrantel pamoate, available in the United States under the brand names Pin-X and Reese’s Pinworm Medicine.  Take one dose immediately and another dose two weeks later.  If you are having persistent symptoms (anal pain, abdominal pain, blood in stool, or rectal bleeding), be sure to seek medical care immediately.

What prescription medications are used for pinworms?

Mebendazole, also called Vermox, is a prescription medication that very effectively kills pinworms.  As of December, 2011, mebendazole is no longer available from any manufacturer in the USA, although it can be made by compounding pharmacies with a doctor’s prescription. No reason was given publicly for this discontinuation in the United States.  Mebendazole is still available internationally and is recommended by the World Health Association as an essential medication for a basic medical system.

If one member of your household has pinworms, you need to treat your entire family.   You may need to treat your pets, too (ask your vet).  Pinworms lay eggs that spread easily to children (and adults) who touch their anal area and then don’t wash their hands and fingernails well.  Many (if not most) people with pinworms have no symptoms, so the person who keeps spreading the eggs around your house may not be the person with symptoms.  It’s a bit painful to buy medicine for the whole household and get your kids to take it, but you’ll be glad when you don’t have a recurrence in a few months.

Can infants get pinworms?

Yes, but it is uncommon. Infants who are not yet crawling, drinking water, or eating solid food probably will not ingest the pinworm eggs and, hence, are unlikely to contract the illness.  Rarely pinworm eggs can be inhaled, which is a potential mechanism of infant infection.  Infants generally have a very clean anal area because their caregivers wipe their anus with baby wipes with every diaper change, making re-infection unlikely.  Once an infant starts crawling and putting things in their mouth, they are at risk of pinworms.

What is the treatment for pinworms during pregnancy?

Pinworms are common in pregnancy, as many expectant moms also have pinworm-laden toddlers in the house.  Pinworms cannot hurt or infect a baby before birth, but they are uncomfortable and just plain disgusting for a pregnant mother.  Sadly, neither pyrantel pamoate nor mebendazole is recommended during either pregnancy or breastfeeding.  Breastfeeding mothers can take either medication if they “pump and dump” afterward.  The good news is that you can still eradicate pinworms during pregnancy (or breastfeeding) using the strict hygiene approach to treatment (see below).  Some people also recommend enemas for flushing the pinworm eggs out of the rectum, although I have not found any research that supports this treatment.  The lack of research doesn’t mean this approach doesn’t work — a study to show effectiveness would be very hard to do.

Prevention — how to keep the pinworms out of your home

Strict hygiene measures can clear up pinworm infection without medication, and reduce the risk of reinfection. The worm has a lifespan of about six weeks, therefore the strict hygiene needs to last that long. Everyone in the household has to adhere to strict hygiene, which can be very hard if you have a nail-biter or thumb sucker.  But do the best you can and take advantage of the opportunity to motivate your children to improve their hygiene.

  • Wash all bed linen, pajamas, and anything your child sleeps with.
  • Vacuum your carpets and mop your floors regularly (at least weekly).  Clean every floor in your whole house, especially bedrooms and bathrooms.  Your goal is to get the eggs out of your carpets and off your floors.  Pinworm eggs can live 2-3 weeks outside the body.
  • Clean surfaces in the kitchen and bathroom daily, especially faucet knobs and the toilet flush handle.  Use Clorox wipes or a rag with hot water.
  • Do not shake things that may have eggs on them, such as clothing, underwear, pajamas, bed linen or towels.
  • Do not eat in the bedroom. There is a risk of swallowing eggs that have shaken off the bed linen.
  • Make sure everybody’s fingernails are cut short.  Teach children to wash under their nails with a nail brush daily.  If you have a nail biter or finger sucker, consider this a motivational opportunity to quit.
  • Teach children to wash hands thoroughly after going to the bathroom and before eating.  Sing “Happy Birthday to You” and don’t quit scrubbing your hands and nails until you finish the song.
  • Always wear tight-fitting underwear (not boxer shorts).  Change your underwear every morning.
  • Cotton gloves may help prevent scratching during sleep.
  • Everyone in the household needs a bath or shower daily.  Teach your children to thoroughly soap up and rinse their rectum and genital areas.
  • Do not share towels or washcloths.   Wash bath towels and washcloths after each use.  Change hand towels daily.
  • Airborne eggs can theoretically land on your toothbrush.  Keep toothbrushes in a closed cabinet and/or rinse them well before each use.

Watch the pinworm movie with your kids and learn the pinworm lifecycle

Understanding these nasty creatures, their mating cycles, and how they lay eggs will motivate you and your kids to improve your hygiene.  There is a great YouTube video on the enterobius lifecycle. Watch it together as a family, then smile while your kids wash their hands and start cleaning.

How To Tell If You Have Tapeworms or Pinworms

As a kid, you may have heard that only dirty people get worms. In fact, you can get pinworms and tapeworms no matter how often you take a bath. But most of the time, these parasites are easy to get rid of.

Pinworms are also called “threadworms.” They’re the most common type of intestinal worm infection in the U.S., and one of the most common in the world. They’re thin and white, and about one-quarter to one-half inch long — about as long as a staple.

Tapeworms are flat worms that look a bit like ribbons. Their bodies are made up of segments, and each segment is about the size of a grain of rice. Adult tapeworms can grow to be 30 feet — almost as long as the average school bus. Fortunately, infections caused by them are rare in the U.S.

How Do You Get Them?

Pinworms are spread from feces to the mouth. It can happen directly, like when your dirty fingers touch your mouth or food. It can also happen indirectly, like when you touch an object that’s contaminated with eggs. Tiny pinworm eggs can live on surfaces — toys, bathroom fixtures, school desks, or bed linens — for up to 3 weeks.

Continued

Once inside your body, pinworms make a home in your colon and rectum. At night, female worms come outside to lay eggs in the skin around your anus. If you scratch the area, the eggs get on your fingers or under your nails, then wipe off on the next thing you touch. This is why pinworms are spread so easily among young children.

Tapeworms can live outside for months, waiting for a host to come along. You’re most at risk if you work around livestock or travel to a country where hygiene is poor. You can also be infected if you eat or drink something that contains tapeworm eggs or larvae, like raw or undercooked beef or pork.

Once inside your body, the tapeworm head attaches to the wall of your intestines. It uses the food you eat to grow new segments. The older segments, which contain eggs, then break off and leave your body with your poop.

What Are the Symptoms?

Most of the time, there aren’t any. You might have anal itching, especially at night. You could also have stomach pain, nausea, or vaginal itching. Sometimes pinworms can be seen around your anus or on your underwear or bed sheets about 2 to 3 hours after you’ve gone to bed.

Most of the time, pinworms don’t cause major problems. But in rare cases, they can cause infections of the vagina and uterus.

If you have a tapeworm infection, you may not have any symptoms. But some people have nausea, stomach pain, weakness, or diarrhea. You might notice a change in appetite (eating more or less than usual). And since the tapeworm keeps your body from absorbing nutrients from food, you may lose weight.

If you get tapeworm from eating pork (it’s actually called the pork tapeworm), the eggs enter your blood stream and hatch in your tissues. There, they form fluid-filled cysts, which doctors call “cysticercosis.” This can cause a wide range of possible symptoms, depending on where the cysts develop and how much inflammation they cause. Some of these may include vision changes, lumps on the skin, neurologic changes, or seizures.

How Are You Diagnosed?

If your doctor suspects you have pinworms, they may ask you to do a “tape test.” As soon as you wake up in the morning, you’ll place a piece of clear tape around your anus, then gently peel it off. Any pinworm eggs will stick to the tape, which your doctor can see under a microscope in a lab.

A tapeworm infection is usually diagnosed by finding eggs or tapeworm segments in the stool. Your doctor may ask you to bring in a sample so a lab can look for eggs, larvae, or tapeworm segments. A blood test can spot antigens, foreign substances that let your doctor know your body is trying to fight the infection. Sometimes, an MRI or CAT scan can find cysts formed by the pork tapeworm.

What’s the Treatment?

Some over-the-counter medicines kill pinworms. Your doctor also could prescribe you an anti-parasite medicine. The most common one is albendazole. Everyone in your family should be treated — even if they don’t have symptoms. Two doses are often needed to ensure the infection is gone for good.

Continued

Sometimes you don’t need to do anything about a tapeworm. It could leave your body on its own. But if your doctor finds it, they can prescribe a medicine like praziquantel or nitazoxanide. These will either kill the adult worms or cause you to poop them out. But they won’t kill the eggs, which can still cause infections. You’ll likely need to give your doctor a stool sample for a few months to make sure all the worms are gone.

It’s harder to treat an infection caused by tapeworm cysts. In addition to the medicine that kills the tapeworm, you may need medicine to reduce inflammation or other symptoms, like seizures, that you’re having. Depending on where your cysts are and how many you have, you may need surgery to remove them.

How Do You Prevent Worms?

To avoid being infected with either pinworms or tapeworms, it’s crucial that you wash your hands with soap and water before eating or handling food and after going to the bathroom or changing diapers.

Here are some other ways to prevent them:

  • Keep your fingernails clipped short.
  • Take a shower or bath every day (in the morning is best).
  • Try not to scratch around your anus or between your legs.
  • Wash your clothes and bed linens often.

To help prevent tapeworms, try these things:

  • Don’t eat raw or undercooked beef, pork, or fish.
  • If you have a dog with tapeworm, treat it right away.

Kristen Bell Caught ‘Anal Worms’ — and They Are As Weird As They Sound

  • Kristen Bell recently shared that she got ‘anal worms’ from her daughter.
  • During an appearance on “The Joel McHale Show,” the actress said she found the worms after noticing her 3-year-old daughter scratching her butt.
  • The worms are technically known as pinworms, which are a common and highly contagious human worm.
  • “You have to monitor it and look in their poop and when you wipe their butt. Sure enough, I wiped and saw a little white worm.”

Kristen Bell doesn’t hide much from the world. Whether she’s sharing her marital struggles, talking about depression and anxiety, or telling a story about her kids walking in during sex, the actress is an open book. So it should come as no surprise that Bell openly discussed her experience with anal worms during a recent appearance on “The Joel McHale Show.”

In the interview, Bell revealed that she caught anal worms — also known as pinworms — from her 3-year-old daughter, Delta. The mother of two said that the preschool her daughter attends had a case of pinworms, which can live on surfaces like clothing, bedding or carpets according to Mayo Clinic.

But it wasn’t until her daughter began scratching herself that Bell knew there was a problem. “You have to monitor it and you have to look in their poop,” she told McHale. “Sure enough, I wiped and saw a little white worm.”

Her entire family, including her husband Dax Shepard, had to take a shot of “chalky nonsense from the drugstore” to make sure the worms were gone for good.

Kristen Bell provided a gross parenting lesson on “The Joel McHale Show.

Netflix

The star of “The Good Place” said they weren’t painful, but they were very itchy. She also pointed out something more concerning: that most people with anal worms don’t have any symptoms.

Anal worms – or pinworms – can live in the colon and are easy to contract.

According to the Centers for Disease Control and Prevention (CDC), pinworms are small, thin, white roundworms that live in the colon and rectum of humans. Pinworms can be contracted if you swallow or breathe in their eggs, the CDC notes. So it makes sense that preschoolers, who often put their hands in their mouths or the mouths of their parents, could get them.

“Your kids get them because they put their hands in their mouth and they lay eggs,” Bell told McHale.

You can also get pinworms if you accidentally ingest contaminated foods or drinks. The eggs can also be transferred from fingers to food, liquids, clothes or other people.

Once swallowed, the eggs hatch in the intestines and mature into adult worms within a month, according to the CDC. Female pinworms then leave through the anus and lay their eggs in the area, which causes itching. The eggs cling to your fingernails when you scratch the itch, making them easy to spread when you touch other objects.

Unfortunately, as Bell and the CDC note, there are usually no symptoms that will let you know if you have pinworms. You may, however, experience anal itching, restless sleep, abdominal pain or nausea, or irritability, according to Mayo Clinic.

If you suspect you have pinworms, you can perform a ‘tape test,’ which requires pressing a piece of transparent tape to the skin around the anus for three days in a row and take the pieces of tape to your doctor. Your doctor should be able to look at the tape under a microscope to see if there are any pinworm eggs.

Although common, pinworms aren’t serious and can be easily prevented.

Thankfully, pinworm infections don’t usually cause serious health problems and treatment may only require you (and any other household members) to take an over-the-counter anti-parasite medication, according to Mayo Clinic.

To prevent pinworms, make sure you regularly clean your house, change your underwear and bed linens daily, use hot water to do your laundry, avoid scratching, wash your hands, and of course, washing your anal area in the shower.

Pinworms

Is this your child’s symptom?

  • Tiny, harmless worms that can cause anal itching
  • The pinworms live in the colon (large bowel)

Symptoms of Pinworms

  • Itching and irritation of the anus and buttocks is the main symptom
  • Sometimes, moves to the vagina and causes vaginal itching or discharge

Cause

  • A white, very thin, threadlike worm, about ¼ inch (6 mm) long.
  • It moves. If it doesn’t wiggle, it’s probably lint or a thread.
  • The worm may be seen around the anus or on the child’s bottom. It is especially active at night or early morning.
  • Rarely, the pinworm is seen on the surface of a stool.
  • The pinworm’s secretions are a strong skin irritant and cause the itching.

Care Advice

Treatment for Pinworms

  1. What You Should Know About Pinworm Treatment:
    • Pinworms are the most common worm infection in the US.
    • Pinworms can cause anal itching.
    • Pinworms do not carry any diseases.
    • Treatment is very helpful.
    • Here is some care advice that should help.
  2. Pinworm Medicine:
    • If a pinworm was seen, your child’s doctor will suggest a special pinworm medicine. Pinworm medicines are available without a prescription (such as Reese’s Pinworm Medicine). There are also prescription medicines that treat pinworms. Your doctor will decide which one is best for your child. Take as directed.
    • Give a repeat dose of the pinworm medicine in 2 weeks. Reason: To prevent the pinworms from coming back.
    • The repeat dose is needed because eggs can live for 1 to 2 weeks. Temperature and humidity levels also affect how long the eggs can stay alive.
  3. Treating Close Contacts:
    • There is a slight risk that Pinworms may spread to others.
    • Treat family members only if they have symptoms.
    • If another child sleeps with the infected child, they also should be treated.
    • If any of your child’s friends have symptoms, be sure to tell their parents. These children should get tested or checked for pinworms.
  4. Return to School:
    • Children with pinworms do not need to miss any child care or school.
  5. What to Expect:
    • After taking the pinworm medicine, itching should stop in 5 to 7 days.
  6. Prevention of Pinworms:
    • Wash hands and fingernails well before meals and after using the toilet.
  7. Call Your Doctor If:
    • Anal symptoms last over 1 week after treatment
    • You think your child needs to be seen
    • Your child becomes worse

Treatment for Anal Itching Without Pinworm Being Seen

  1. What You Should Know About Anal Itching:
    • There are many causes of itching around the anus. Some are more common than pinworms.
    • The most common cause is stool that has been left on the skin. Stool contains chemicals and germs that can cause itchy skin rashes. Try to prevent this by washing the skin off. After wiping off stool with toilet tissue, cleanse the skin with warm water.
    • Bubble bath can also cause an itchy bottom. Children with dry skin are at increased risk. Avoid bubble bath or any soapy bath water.
    • If these changes don’t get rid of the anal itching, get a pinworm test.
  2. Pinworm Checks: Check your child for pinworms.
    • Examine the area around the anus, using a flashlight.
    • Look for a ¼-inch (6 mm), white, threadlike worm that moves.
    • Do this a few hours after your child goes to bed. Check him 2 nights in a row. Also, check him first thing in the morning for 2 days.
  3. Scotch Tape Test:
    • If no adult pinworm is seen, call your doctor’s office. Ask for instructions on doing a Scotch-tape test for pinworm eggs. You can also use the technique below:
    • Take a piece of clear Scotch tape with the sticky side down. Touch it to the skin on both sides of the anus. Do this in the morning soon after your child has awakened. Also, do this before any bath or shower.
    • Put the sticky tape side that touched the skin on a slide. If you don’t have a slide, put it on a second piece of tape.
    • Do it 2 mornings in a row.
    • Bring the 2 samples in to be looked at under a microscope.
  4. Steroid Cream for Itching:
    • For the itching, wash the skin around the anus with warm water.
    • For severe itch, use 1% hydrocortisone cream (such as Cortaid) 2 times per day. Use for 1 or 2 days. No prescription is needed.
  5. Call Your Doctor If:
    • Pinworm is seen
    • Skin around the anus gets red or tender
    • Anal itching lasts more than 1 week
    • You think your child needs to be seen
    • Your child becomes worse

Treatment for Pinworm Exposure

  1. Low Risk for Getting Pinworms:
    • Your child has had contact with a child with pinworms, but no symptoms now. Your child probably won’t get them. This is especially likely if over a month has passed.
    • If contact is within the last month, your child may get pinworms. This risk is small.
    • Pinworms are harmless. They are never present very long without being seen or causing anal itching.
  2. Scotch Tape Test:
    • If you’re still worried, call your doctor’s office. Ask for instructions on doing a Scotch tape test. This can be done to look for pinworm eggs.
    • Do this about 1 month after contact.
    • Reason: The swallowed egg will not become an adult pinworm for 3 or 4 weeks.
  3. Call Your Doctor If:
    • Pinworm is seen (white, ¼ inch or 6 mm, and moves)
    • Anal itching lasts more than 1 week
    • You think your child needs to be seen

Treatment for Reducing Reinfection or Spread to Others

  1. Preventing Pinworm Infections:
    • Infection is caused by swallowing pinworm eggs.
    • A child can get pinworms no matter how carefully you clean.
    • The following hygiene measures, however, can help to reduce the chances of reinfection. It also can reduce the chance of new infections in other people.
    • Pets don’t carry pinworms.
  2. Wash Hands:
    • Have your child scrub the hands and fingernails well before each meal. Also, wash the hands well after each use of the toilet.
    • Keep the fingernails cut short, because eggs can collect here.
    • Help your child give up thumb sucking and nail biting.
  3. Shower:
    • Each morning give your child a shower.
    • Always rinse the anal area.
    • Do this for 3 days after taking the pinworm medicine.
  4. Vacuum:
    • Vacuum or wet-mop your child’s bedroom once a week.
    • Any eggs left on the floor can still infect others for 1 or 2 weeks.
  5. Wash Clothes:
    • Wash clothes and bedding at a hot temperature. This will kill any eggs left in them.
  6. Return to School:
    • Children with pinworms do not need to miss any child care or school.

And remember, contact your doctor if your child develops any of the ‘Call Your Doctor’ symptoms.

Pinworm Infection

Last Reviewed: October 2011

What are pinworms?

Pinworms are white, parasitic worms that can live in the large intestine of humans. They are about one-half inch long. While the infected person sleeps, female pinworms leave the intestinal tract and lay their eggs on the skin around the anus. The eggs are laid in a sticky, jelly-like substance that, along with the wriggling of the female pinworm, causes severe itching.

Who gets pinworm infection?

Pinworm is the most common worm infection in the United States. School-age children, followed by preschoolers, have the highest rates of infection. Cases of pinworm infection are seen most often at schools, daycare centers and other institutional settings.

What are the symptoms of a pinworm infection?

Pinworm infection may cause:

  • Itching around the anal area, difficulty sleeping and irritability.
  • If it is a severe infection, symptoms may include:
    • nervousness
    • restlessness
    • loss of appetite
    • weight loss
    • girls may experience vaginal itching and irritation (vaginitis), if pinworms are near the vagina.

How does someone get pinworms?

Pinworms are spread when an infected person, most often a child, has scratched his/her bare anal area and the eggs get under his/her fingernails. Pinworms can then be spread in the following ways:

  • By an infected child not washing hands after using the bathroom. If the child then touches playmates or toys, he/she may pass on the eggs.
  • Pinworm eggs can also be transferred to the fingers from clothing or bedding, and then spread around the home.
  • Eggs may be inhaled from the air or deposited onto food and swallowed.
  • Pinworms can survive up to two weeks on clothing, bedding or other objects, if kept at room temperature.

The eggs may hatch while still attached to the skin around the anus. They then move through the rectum to the lower intestine, where they grow to adult size within two to six weeks. Pinworm infections can be spread as long as either worms or eggs are present.

How is a diagnosis of pinworm infection made?

Finding the female worm or the eggs confirms the diagnosis of pinworms. To find a female worm:

  • At night, the adult worms can sometimes be seen directly around the anal area or in pajamas. The worm (one-quarter to one-half inch long) is clearly visible to the naked eye. Finding a worm confirms the diagnosis.
  • If adult worms are not visible, conduct a tape test in the morning. Apply a piece of transparent tape against the folds of skin around the anus to pick up any eggs or worms. Seal in a plastic bag.
  • Take the tape to a health care provider. The eggs and worms caught on the tape can be identified under a microscope.

Pinworms are rarely spotted in stool samples. Because bathing or a bowel movement can remove the eggs, the tape test should be done as soon as the person wakes up in the morning.

How is a pinworm infection treated?

Treating pinworms involves either prescription or over-the-counter drugs. Consult a health care provider before treating a suspected pinworm infection. Follow these treatment steps:

Step 1: Treat the infected person/any infected family members

  1. The infected person should take the medicine orally. It is given in two doses. The second dose should be given two weeks after the first.
  2. Bathe first thing in the morning to reduce egg contamination.
  3. Wash hands and under the fingernails thoroughly, after using the bathroom, before eating and after changing diapers.
  4. Discourage nail biting and scratching bare anal areas to avoid re-infection.
  5. Keep fingernails trimmed very short.
  6. Infection often occurs in more than one family member. Treat all infected family members at the same time.

Step 2: Treat the household

  1. Change and wash underwear and pajamas in hot water daily.
  2. Machine-wash sheets, blankets, towels and clothing in hot water to destroy eggs. Machine-dry at high temperature.
  3. Eggs are sensitive to sunlight, so open blinds in bedrooms in the daytime.
  4. 4. Since pinworm eggs are light and scatter easily, dust should be removed carefully from all surfaces in the home. Careful vacuuming or the use of an oiled cloth (which may be boiled or destroyed later) will help prevent the eggs from scattering.

How can pinworm infection and reinfection be prevented?

  • Wash hands and under fingernails frequently.
  • Encourage children to avoid scratching their bare anal areas.
  • • Pinworm eggs continue to be present (excreted) in the feces of an infected person for up to a week after the treatment, so precautions should be taken to prevent reinfection by washing hands thoroughly, especially under the nails.
  • Bathe daily.
  • Change and wash clothing and bedding frequently.

What if the pinworm infection occurs again?

If infection occurs again, consult your health care provider. In some cases, it may be necessary to treat the patient and close family contacts more than once.

For more information contact your local health department or go to the Centers for Disease Control and Prevention (CDC) website at http://www.cdc.gov/parasites/pinworm/.

Pinworms: They only come out at night

So many of us or our family members have experienced this relatively mild though distressing infection. Pinworm is the most common worm infection in North America with up to 50% of some groups of school aged children getting infected. It is estimated that there are 500 million infections worldwide and 40 million at any one time in the U.S.

E. vermicularis image/CDC

Pinworm, or Enterobius vermicularis, is a small intestinal roundworm about the size of a staple. The female adult worms leave the anus in the middle of the night while the person is sleeping to deposit her eggs around the skin of the perianal region.

The eggs become infectious within a couple of hours after being deposited and can survive for about 2 weeks outside the host.

Many people may be asymptomatic. In others there may be rectal itching and eczema, disturbed sleep, irritability, and secondary bacterial infection of scratched skin. Rarely worms are known to migrate into other areas like the vagina in young girls. Pinworms should be suspected in young girls with persistent vulvovaginitis.

The mode of infection with pinworm is through transfer of infective eggs by hand to mouth, frequently after scratching the perianal region and indirectly through clothing, bedding, food and other articles contaminated with eggs. Eggs get dispersed around the house, especially in the bedroom.

Pinworm infection is most common in school aged children, followed by preschoolers and is lowest in adults except for mothers of infected children. Whole families can get infected with this worm.

This infection is easily diagnosed by applying scotch-tape or a pinworm paddle to the perianal area and examining this microscopically for characteristic eggs. Specimen should be collected early morning before bathing or the passage of stools. Eggs are rarely found in stool samples.

In children over age two years, treatment usually includes either mebendazole (Vermox) or Pin-X (pyrantel pamoate), an over-the-counter alternative to Vermox that is available as a liquid.

One of the keys to treatment of pinworms is to repeat the dose in 2 weeks to be sure and kill newly hatched eggs. Also, since some family members may have a pinworm infestation without symptoms, it is sometimes recommended that the whole family be treated, taking caution in treating children under age 2 years and pregnant women (ask your doctor first).

Reinfection with pinworm occurs rather easily. Strict observance of good hand hygiene is essential (e.g. proper handwashing, maintaining clean short fingernails, avoiding nail biting, avoiding scratcing the perianal area) to prevent reinfection.

Other preventive measures include:

• Daily morning bathing and changing of underclothes helps remove a large proportion of pinworm eggs and can help prevent infection and reinfection. Showering may be preferred to avoid possible contamination of bath water.
• Careful handling (avoid shaking) and frequent laundering of underclothes, night clothes, towels, and bed sheets using hot water also helps reduce the chance of infection and reinfection by reducing environmental contamination with eggs.
• Clean and vacuum house daily for several days after treatment.

90,000 ENTEROBIOSIS

17 July 2020

ENTEROBIOSIS

Almost all families with children are familiar with enterobiasis. Annually, about 6 thousand patients with enterobiasis are registered among the children of St. Petersburg. The incidence of enterobiasis is comparable to the incidence of influenza.

Enterobiasis is a helminthic disease caused by pinworms parasitizing in the human intestine, and is widespread among people of all ages, especially among children. The causative agent is a pinworm – a small, white helminth, up to 1 cm long. In the human body, from several hundred to a thousand pinworms can simultaneously parasitize.

The source of the disease is only a sick person . The main transmission factor is contaminated hands. Infection with enterobiasis occurs when viable pinworm eggs enter the mouth. By combing itchy areas, infected people can contaminate the subungual spaces, fingers, and then household items and food with helminth eggs.

Pinworm eggs fall on the hands of a healthy child both upon contact with an invasive child (when shaking hands) and from objects in the external environment that the patient touched. Contamination of objects in the external environment is also possible when shaking the patient’s bed linen. If the rules of personal hygiene are violated, the patient with unwashed hands can re-enter the eggs of pinworms in the mouth, while repeated infections (autosuperinvasion) occur, and in the future a persistent course of the disease is observed.

Can enterobiasis be contracted from animals?

Often, parents associate the onset of the disease in a child with communication with animals, but cats and dogs do not get sick with enterobiasis. But on their wool, as on any objects of the external environment, under favorable conditions, pinworm eggs can maintain their viability for 15-18 days. Therefore, animals that children play with must be washed.

Who is sick with enterobiasis?

Most often, children of preschool and primary school age from 3 to 8 years old, attending preschool institutions, primary classes of secondary schools, are ill.

What are the main symptoms of enterobiasis?

The main clinical symptoms of enterobiasis are: itching in the perianal region, disorders of the digestive system and the nervous system. Recently, there has been an asymptomatic course of the disease.

1. Perianal itching (itching in the anal region), and in girls – itching in the genital area occurs in the evening or early night hours. As a rule, during sleep, when the child is relaxed, female pinworms, overflowing with eggs, can move along the damp skin along the hips, lower back and crawl into the genitals.Dry skin stimulates egg-laying, during which the female secretes formic acid, which causes itching. Laying 10-12 thousand eggs, the female dies.

Itching can be so excruciating that it interferes with sleep. The child becomes restless, tosses and turns in bed, cries, jumps up. This is the cause of neurasthenia, bedwetting, insomnia, headaches, dizziness, memory impairment, and in schoolchildren – a decrease in academic performance. Itching can persist after treatment as a result of the formation of a persistent focus of excitation in the cerebral cortex. With a low intensity of invasion, itching is repeated for several days and then disappears.

2. Abdominal pain is a frequent symptom of enterobiasis, which is associated with the mechanical action of the parasite. As a rule, pain occurs during meals and is localized in the navel. In some patients, intestinal disorders, frequent mushy stools, tenesmus, cramping pains, which may be the reason for seeking surgical help, come to the fore, but instead of pathology, an accumulation of gas is found. In addition to frequent repeated infections, the cause of the persistent course of enterobiasis is a violation of the intestinal biocenosis, including the intestinal microflora, which increases the susceptibility to pathogens of acute intestinal infections.

3. In the majority of infested children, the number of Escherichia coli decreases and the proportion of lactonegative intestinal microflora increases. As a result of a decrease in the activity of intestinal enzymes, the processes of absorption and digestion of nutrients are disrupted, which leads to a decrease in body weight, a delay in the growth and development of the child.Dysbacteriosis in people infected with pinworms is detected 4 times more often than in non-invasive ones.

What complications are possible with enterobiasis?

Vulvovaginitis and, in rare cases, infertility is a complication of enterobiasis in girls. The causative agent of enterobiasis can be found in various organs and tissues (abdominal cavity, ovary, fallopian tube, endometrium, liver).

Parasitizing pinworms in children leads to suppression of nonspecific immunity. This is evidenced by the fact that among patients with enterobiasis there are 6 times more often sick children. The presence of enterobiasis leads to a decrease in the effectiveness of preventive vaccinations. The immune layer against diphtheria in children with enterobiasis is much lower than among healthy children.

Enterobiasis can cause the development of allergic diseases in children and a decrease in the level of trace elements in the blood.

How to establish the diagnosis of Enterobiasis?

Diagnosis of enterobiasis is simple and consists in the detection of pinworm eggs in a scraping or imprint from perianal folds. These studies can be carried out in the laboratory of the polyclinic at the place of residence or other institutions. Sometimes after eating certain foods (oranges, carrot juice, mineral water) or raising the body temperature, pinworms are released with feces – small mobile worms, up to 1 cm long.

How to treat enterobiasis?

You do not need a prescription to purchase antihelmintic drugs, but before treatment, you should consult with your local doctor or infectious disease doctor. The effectiveness of modern drugs is high – 95-98%, they are taken once. In the case of a persistent course of enterobiasis, treatment is carried out three times at intervals of 2 weeks. The most common anthelmintic drugs: Pirantel, Gelmintoks, Vermox, Wormin, Nemazol. Treat only as directed by a doctor!

Since the manifestations of enterobiasis are associated with disorders of the intestines, it is undoubtedly necessary to correct the microflora by implanting “normal” intestinal microorganisms. Therefore, today, in the treatment of enterobiasis, probiotics are widely used – bacterial dry or liquid preparations from live microbial cultures.

For active correction of microflora, Lactobacillus is used – suspension of dried live lactobacilli, which have high activity against pathogenic microorganisms. It is also important that the effect of Lactobacterin does not decrease when treated with anthelmintic drugs. Lactobacillus has an antiviral effect, prevents the incidence of influenza and acute respiratory viral infections, eliminates intoxication.

Are there folk remedies for the treatment of enterobiasis?

Known food products and medicinal plants that can be used for treatment and prevention! enterobiasis. Carrots and carrot juice have a good effect. Children are recommended to use 30-50 ml of juice per day for several days.

You can use the anthelmintic activity of walnuts, wild strawberries, pomegranate, garlic and lovage. From medicinal plants used St. John’s wort, elecampane, centaury in the form of decoctions, infusions and tea. However, we do not recommend garlic enemas, which can burn the intestinal mucosa!

What hygiene measures should be taken after treatment of a patient with enterobiasis?

It is advisable to wash the child, change underwear and bed linen, carry out a general cleaning in the apartment. For 3-5 days for the child at night in the anus to lay a tampon smeared with petroleum jelly, put on tight-fitting panties, change or iron the bed linen daily.

What events are held

in childcare facilities?

Parents of children attending kindergartens and primary schools should be aware that in September-December, all institutions carry out annual planned activities to improve the health of children from enterobiasis. Before the planned examination, parental meetings are held, and after the examination, conversations with the parents of the infested children are held.Children with enterobiasis detected during routine examination are treated in a child care facility. After treatment in groups, final anti-epidemic and disinfection measures are carried out. At the same time, it is necessary to carry out hygienic measures in the families of the infested and the examination of the contact. If the parents refuse to be examined, it is possible to take an anthelmintic drug in the presence of a medical worker.

Who else should be examined for enterobiasis?

Unorganized children under 3 years of age are annually examined for enterobiasis in polynics at the place of residence during preventive examinations.

Children applying for enterobiasis are subject to examination in children’s institutions, swimming pools, entering inpatient treatment in children’s hospitals and sanatoriums, in children’s out-of-town health institutions, according to clinical indications (if there are complaints) and in contact with a patient with enterobiasis.

Advice to parents

To prevent infection with enterobiasis in the family, the following rules should be followed:

  • Teach children the skills of a personal hyena.
  • Wash hands before eating and after using the toilet.
  • Cut your nails short.
  • To wean children from the habit of taking fingers, toys, foreign objects in their mouths.
  • Thoroughly wash the child in the morning and evening.
  • Change your child’s underwear daily.
  • Iron bed linen daily with a hot iron and do not shake it out in the room.
  • Keep the apartment clean, carry out wet cleaning with hot soap and water solution, use a vacuum cleaner.
  • Wash or treat children’s toys regularly.

A significant part of parents underestimate the problem of enterobiasis, the need for timely treatment and implementation of preventive measures in the family. It is very important to educate the child in the need for correct hygienic behavior. Make him want to follow the rules of hygiene, and not you force him to do it. Remember that a child learns mainly through imitation of adults close to him, especially his parents. If adults meet the hygiene requirements, then the baby will begin to wash his hands regularly to feel “big”.

Pinworms

Pinworms are one of the most common intestinal worms.

Pinworms are tiny parasitic nematodes (2-14 mm) that live in the intestines and lay eggs on the skin around the anus.

Pinworm infection is also known as enterobiasis.

The disease is most common in children aged 5 to 10 years.

A person becomes infected by swallowing helminth eggs.

In the intestine, larvae emerge from the eggs, which parasitize in the lower parts of the small intestine, and in the large intestine they turn into an adult.

At night, a mature female leaves the anus and lays eggs on the skin in the folds of the skin around the anus, perineum, buttocks, lays eggs (from 2 to 15 thous.).

After 4-5 hours, the eggs become “invasive” (infectious) for humans, because during this time, larvae mature in the eggs.

If an infected person touches household items such as bedding, clothing, toilet seats, or toys, eggs are transferred to those items.

Eggs can also be transferred directly from contaminated fingers to food.

Although rare, adults can also inhale airborne eggs by shaking contaminated bedding, towels, or clothing.

The causative agent of enterobiasis is resistant to various disinfectants. On toys, bedding, carpets and other household items, the causative agent of enterobiasis remains viable for up to 21 days, on environmental objects – in the upper layers of the soil of playgrounds, sand from sandboxes – up to 14 days, in tap and waste waters – up to 7 days. The stability of pinworm eggs in the external environment increases as they mature. At a temperature of plus 22-28 ° C and a decrease in humidity to 60%, pinworm eggs remain viable for up to 8 days.

Who is at risk of contracting enterobiasis?

Pinworms affect people of all ages, enterobiasis is widespread.

Most susceptible to infection:

  • children attending organized institutions (especially kindergartens, camps)
  • family members or caregivers of infected people
  • people living in public institutions such as dormitories
  • people who do not follow the rules of personal hygiene, namely regular and thorough washing of hands before meals
  • children who have a habit of sucking their fingers, biting their nails

Can pinworms be infected by pets?

No, the only source of infection is humans.

Symptoms of enterobiasis:

  • Itching, scratching accompanying pinworms crawling onto the skin
  • Combing the skin provokes the spread of pinworm eggs and repeated self-infection, as a result of which the disease can last for a long time. The eggs can survive for several hours on the hands, under the nails.
  • Allergic reaction, rash
  • Irritation, tearfulness, restless sleep
  • Pinworms in feces

Some people with enterobiasis may not experience any symptoms.

The danger of enterobiasis lies in the fact that pinworms, parasitizing in the human intestine, produce toxins – waste products that cause intoxication of the body. Pinworms, parasitizing on the mucous membrane, injure it, which can lead to the attachment of a secondary fungal or bacterial infection

The diagnosis is made on the basis of laboratory tests.

Treatment is prescribed by a doctor.

Children infested with pinworms, which are the sources of the spread of enterobiasis, are not admitted to preschool educational institutions for the period of treatment and control laboratory examination.

In addition to following the doctor’s recommendations, it is necessary to prevent the spread of infection and the risk of repeated self-infection.

Washing hands with soap and warm water after using the toilet, changing diapers, and before eating is the most effective way to prevent pinworm infection.

Parents are advised to ensure that children do not bite their nails.

Follow the rules of personal hygiene and change your underwear daily.

Infected people should not swim with other people.

Those infected should avoid bathing. It is necessary to take a shower every morning.

Prevention of enterobiasis includes a set of measures:

  • identification of patients (parasitic carriers) with enterobiasis
  • treatment of identified infested persons and prevention of persons in contact with infested
  • Monitoring the circulation of enterobiasis pathogen in groups at increased risk of infection

Disinfestation is carried out in the focus of infection:

  • furniture and other surfaces are wiped with special disinfectants
  • soft toys, upholstered furniture and carpets are processed with a vacuum cleaner, followed by dust disinfection. During the processing, it is recommended to remove carpets and toys for 3 days.
  • Linen, bed linen – boil
  • Hard toys – wash with detergents.

90,000 Enterobiasis and its prevention

Enterobiasis is a parasitic disease caused by roundworms called pinworms, characterized by intestinal damage and allergy to the body.

Enterobiasis is widespread in all latitudes of the globe where humans exist, and a high level of infestation by pinworms is recorded in most countries of the world.

The spread of enterobiasis depends on the sanitary condition of the home, institution and personal hygiene habits of people. It was found that pinworms contribute to a more frequent occurrence in invasive somatic and infectious diseases.

The only source of pinworm infestation is an infested human. A person becomes infected by swallowing eggs. Fingers contaminated with pinworm eggs play the most important role in the transmission of the invasion. Enterobiasis is rightly considered a disease of dirty hands.Due to the intolerable itching associated with the crawling of female pinworms from the rectum and their movement, the invasive contaminates the hands when scratching. With dirty hands, helminth eggs are carried onto other parts of the body – the skin of the abdomen, thighs, face, and also fall on underwear and bedding, and when shaken they rise with dust and settle on household items, floor, furniture, creating conditions for the patient’s self-infection – reinvasion and for widespread infection of others.

When studying various objects of the external environment, pinworm eggs were found on dishes, toys, tables, desks, money, on food, where they fell from dirty hands and with dust.Flies play a role in the spread of pinworm eggs. On human skin, underwear, pinworm eggs ripen quickly (4-6 hours) and become capable of infecting humans. The main factor in the transmission of infestation are hands, as well as toys, food contaminated with hands. Infestation of children in swimming pools is possible. The possibility of infection by inhalation of dusty eggs is not excluded.

In case of primary infection, the clinical incubation is 2 days, the acute phase is 5-7 days. The main clinical symptoms of the acute phase of enterobiasis are abdominal pain, morning intestinal discomfort, nausea, and frequent bowel movements up to 4 times a day. These phenomena last up to 5 – 7 days, the stool remains speeded up until the end of the invasion.

The nature of clinical manifestations in the chronic phase is ambiguous. In some of the infected, enterobiasis proceeds without complaints, in the absence of clinical symptoms. The intensity of symptoms in the chronic phase of enterobiasis depends primarily on the intensity of infection and the frequency of repeated infections

With mild invasion, the main complaint is perianal itching.arising more often in the evening before bedtime, when falling asleep, sometimes abdominal pain. After 2-3 days, all these symptoms disappear and. usually repeated every 3-4 weeks. This frequency is associated with the completion of the development of a new generation of helminths as a result of re-infection of

With more intense and prolonged invasion, perianal itching disturbs the invader not only in the evening and night hours, but also during the day. Abdominal pain intensifies, more often in the right ileocecal region, reminiscent of pain in appendicitis, occur! tenesmus.flatulence, loose stools. Intolerable itching interferes with sleep. As a rule, neurological symptoms are expressed, such as headache, dizziness, memory loss. Children often suffer, they become capricious, whiny, get tired quickly, and do not perceive useful skills well. Instability of attention, inability to concentrate reduces academic performance, in adults – disability.

Enterobiasis has a significantly negative effect on behavioral reactions and neuropsychic development of preschool children. Cases of fainting and epileptiform seizures in children, causally associated with enterobiasis, have been described.

Severe complications of enterobiasis include appendicopathies and appendicitis, proctitis and paraproctitis. dermatitis and eczema of the perianal region with their characteristic symptoms.

A variety of clinical manifestations leads to the inevitable appeal of the invasive to doctors of different specialties: gastroenterologists, obstetricians-gynecologists, neuropathologists, allergists, dermatologists, urologists, surgeons, etc.And only the correct orientation of such a specialist to the need to exclude enterobiotic invasion provides qualified assistance to the patient.

The diagnosis of enterobiasis is established by laboratory research for enterobiasis.

A set of measures to combat enterobiasis is aimed at identifying and improving the source of the invasion – a patient with enterobiasis – and preventing the transmission of the pathogen. Identification of infested in a planned manner concerns primarily the most epidemiologically significant contingents: children of preschool institutions.schoolchildren of the first to fourth grades and personnel of preschool institutions should be examined once a year. Workers of the food group and persons equated to them, as well as persons visiting swimming pools, are subject to examination for enterobiasis. All children admitted to hospitals of any profile are examined for enterobiasis. In polyclinics, the examination is carried out according to clinical indications.

For the prevention of enterobiasis, simple rules must be followed:

• Teach children personal hygiene skills.Wean children from the habit of putting fingers and toys in their mouths.

• Wash your hands often with soap, soap twice and do as much as possible

more often.

• Cut your nails short.

• Thoroughly wash your baby in the morning and evening.

• Change or wash your baby’s undergarments daily.

• Change bed linen frequently (be sure to carefully remove it from bed without excessive shaking in the room), wash the linen at a temperature not lower than 60 ° C, iron it with a hot iron.

• Carry out regular damp cleaning of the premises with frequent water changes or rinsing floor cloths under running water. When cleaning, use active detergents (detergents, soda, mustard), which effectively remove helminth eggs. Since these eggs are covered with a sticky fat-like substance that firmly fixes them on any surface, it is almost impossible to do without these tools during the cleaning process. Vacuum or knock out carpets. A detrimental effect on pinworm eggs is provided by keeping bedding and soft toys outside in the cold or in the summer in the bright sun for 2-3 hours.

• Wash children’s toys.

• It is recommended to introduce carrots, strawberries, pomegranate juice into the child’s diet in the absence of allergies. walnuts, St. John’s wort tea, which help cleanse the body oi helminths.

• The child needs names, a separate bed and his own towel.

Enterobiasis – Infectology – Multifunctional diagnostic and treatment center for the whole family HEALTH PLUS in Pyatigorsk

Infection occurs orally, while enterobiasis is considered a disease of dirty hands. In each case, the body of a person affected by this disease reacts in completely different ways. The duration of the primary invasion is approximately two days. During the next 5-7 days, its acute phase is noted. Then comes the period of parasitological incubation, which lasts from 35 to 70 days.

This disease is accompanied by intolerable itching arising from the movement of pinworms and their crawling out of the rectum. The patient involuntarily scratches the corresponding area, thereby contaminating the hands.Of course, after this, the eggs of pinworms will certainly end up on other parts of the body – on the skin of the abdomen, face, on the thighs. In addition, helminth eggs fall on underwear and bed linen, spreading further on other household items (for example, when shaking the linen). Thus, other people who find themselves in these conditions are at risk of contracting these parasites.

MAIN SYMPTOMS OF ENTEROBIOSIS IN CHILDREN

Children are most often susceptible to enterobiasis. The main symptom of this disease is severe itching in the anus and a feeling of discomfort. If the child is constantly bothered by continuous itching (especially at night), then this means that the disease is massive in nature. As a result of constant itching, scratches appear in children, which can later become infected. This can provoke dermatitis, paraproctitis, vulvovaginitis, or sphincteritis. In addition, the allergic readiness of the child’s body may appear or increase. If a child has been infected with enterobiasis for a long time, then he has a deterioration in appetite, a sharp depletion of the body, which is provoked, first of all, by poisoning with the waste products of helminths.

The more significant the invasion, the more intense itching, and it can manifest itself not only at night, but also during the day. Abdominal pain is common, and stools become frequent and runny. With this disease, frequent headaches, dizziness and memory impairment are also noted, from which the child becomes whiny and capricious, and immunity is significantly reduced.

MAIN SYMPTOMS OF ENTEROBIOSIS IN ADULTS

Symptoms of enterobiasis in adults are the same as in children, but the course of this disease is much easier.Sometimes patients have the absence of any symptoms indicating enterobiasis. Still, it is worth noting the main ones. First of all, this is itching in the anus at night, insomnia, irritability, urinary incontinence, intestinal upset, lack of appetite. In addition, in women, pinworms can enter the genital tract and cause vulvovaginitis.

If the number of pinworms in the body of an adult is significant, then this can provoke the following symptoms:

    • severe itching;
    • abdominal pain on the right side;
    • flatulence;
    • false urge to empty the bowels;
    • allergic reactions;
    • Frequent and loose stools;
    • headache;
    • dizziness; insomnia; disturbed sleep;
    • irritability;
    • disorder of memory, attention.

COMPLICATIONS OF ENTEROBIOSIS

Enterobiasis provokes the following diseases: appendicitis, proctitis, various inflammatory skin diseases, inflammation of the external genital organs (in women).

TREATMENT OF ENTEROBIOSIS

In order to avoid infection with enterobiasis, it is necessary to observe preventive measures – observe a hygienic regime. If infestation has occurred, then it is worth taking care of measures aimed at getting rid of these parasites. In the event that there is a suspicion of infection with helminths, it is necessary to contact an infectious disease specialist.

If any of the listed symptoms are detected, you should contact an infectious disease doctor

The spectrum of action on parasites in Bactefort (divorce or truth, we will find out below) is rather narrow, that is, it will not help in the fight against all types of helminths. The greatest effect is achieved against: Ascaris. Pinworms. Vlasoglavov. Ankylostom. If we are talking about lamblia, liver fluke, amoebae or Chinese fluke, the use of “Baktefort” will definitely not give the expected effect. Also, in the case when helminthiasis is associated with a secondary infection, the drug will not help.
