What Is Laryngitis?

Laryngitis is inflammation of the voice box (larynx). This organ sits in your upper neck just past the back of your throat. Swelling of the vocal cords muffles sound, and you are hoarse. When you try to talk, all that comes out is a whisper or squeak.

Swelling of the voice box can be triggered by an infection, such as a cold, the flu, or bronchitis. Or the problem could be something as simple as overuse.

Laryngitis usually isn’t a serious problem. With proper treatment, acute (short-lived) laryngitis should go away in no more than 3 weeks. But sometimes, laryngitis lasts longer and becomes chronic. But there are ways to help yourself feel better.

Laryngitis Symptoms

Laryngitis is often related to another illness, such as a cold, the flu, or bronchitis. Symptoms in children and adults are usually similar. Laryngitis symptoms include:

Laryngitis Causes

The most common cause of acute laryngitis is a viral infection, like an upper respiratory infection. You’re more likely to get laryngitis if you’re prone to:

Other causes of acute and chronic laryngitis are:

  • Smoking or vaping
  • Overuse or misuse of the voice, such as screaming, loud cheering, singing; infants or children may get laryngitis from constant crying or changing their voice to mimic animals or cartoon characters
  • Allergies
  • Throat irritation caused by inhaled medications, like asthma inhalers
  • A fungal infection, such as thrush
  • An injury, such as a hit to the throat
  • Inhalation of chemical fumes
  • Sinus disease

Acid reflux can also play a role. Strong acids can travel up from the stomach into your throat and get all the way to your larynx. This can irritate it and make you lose your voice.

Rarely, laryngitis may be caused by infection by bacteria.

Some health conditions, including certain cancers, can also make you more likely to get laryngitis.

Laryngitis Diagnosis

Because viral laryngitis usually goes away within a couple of weeks, you probably don’t need to see a doctor. If you end up needing to make a visit, a doctor is likely to do the following:

  • Examine your throat and take what’s called a culture. The culture will likely grow out the bacteria or virus that is causing the laryngitis.
  • Use an endoscope, a narrow tube, equipped with a camera. This is called a laryngoscopy. They thread it into your throat through your nose or mouth. You’re given something to numb you so you won’t feel any pain. This way, the doctor can get a close-up look at your vocal cords.
  • If you have a suspicious lump or nodule in your throat or voice box area, your doctor may recommend taking a sample of tissue for examination (biopsy).

The doctor may also do a skin allergy test or an X-ray to rule out other issues.

Laryngitis Treatments

The best treatment is to rest your voice. Without the stress of everyday use, it will often recover on its own.

Medical treatments

  • Corticosteroids. If your need to speak clearly is urgent, a doctor may prescribe corticosteroids. This is a class of man-made drugs that mimic hormones, such as cortisol, that your body makes naturally. They reduce swelling.
  • Antibiotics. If you have a bacterial infection, you may be given antibiotics. But laryngitis is very rarely caused by bacteria. And antibiotics will not help with viral laryngitis.
  • Pain medications. If you’re in pain, you can take acetaminophen or ibuprofen. Always follow the instructions about how often and how much to take.
  • Voice therapy. A speech-language therapist teaches you how to care for your voice and reduce behaviors that strain it.

Home care

You can try a number of home remedies to aid in your healing:

  • Drink plenty of fluids. Early on, swallowing may be painful, but the more you’re hydrated, the better. But avoid alcohol and caffeine.
  • Use humidifiers and menthol inhalers. Moisture is your friend, and menthol can be soothing.
  • Gargle with warm salt water. The salinity not only soothes the area, but also reduces swelling.
  • You may also suck on throat lozenges, which often contain herbs such as eucalyptus and mint, known for calming sore throats.
  • Avoid dry, smoky, or dusty rooms.
  • Stay away from decongestants. They dry you out when your throat wants moisture.
  • Don’t whisper. That actually puts more strain on your vocal cords.

Certain herbs — such as licorice, marshmallow, and slippery elm — have reputations as throat pain relievers, but they interact with some medications. Talk to your doctor before taking them.

Laryngitis Complications

Laryngitis can be very serious in children. Watch for fever and call a doctor if your child is:

  • Younger than 3 months old and has a temperature of 100 F or higher, or is older than 3 months and has a fever of 102 F or higher
  • Having trouble swallowing or breathing, is making high-pitched sounds when inhaling, or is drooling more than usual

In kids, it may lead to croup, a narrowing of the airways, or epiglottitis, an inflammation of the flap at the top of the larynx. This condition can be life-threatening, so get emergency treatment if you or a child in your care has had laryngitis and starts gasping or having any trouble breathing.

Laryngitis in adults is not serious, but you should see a doctor if you’ve been hoarse for more than 2 weeks, are coughing up blood, have a temperature above 103 F, or are having trouble breathing.

Laryngitis Prevention

Follow these steps to keep your voice healthy and prevent dryness and irritation that can lead to laryngitis.

  • Don’t drink coffee, soda, or other products that have caffeine, which dries out the throat.
  • Stay hydrated by drinking plenty of water during the day.
  • Don’t smoke, and stay away from secondhand smoke. Smoking is bad for your health in general, but it also irritates the vocal cords.
  • Don’t clear your throat. Ahem, doing so creates abnormal vibrations that trigger irritation and swelling of the vocal cords.
  • Wash your hands often and properly, especially if you’ve been around someone who is sick.

You may experience hoarseness or lose your voice (get laryngitis) when the tissue covering your vocal cords becomes inflamed or swollen.

In another scenario with long-term, heavy voice use, callus-like growths, known as nodules can form on the vocal cords and cause hoarseness.

When this happens, your vocal cords don’t vibrate as easily. This can show up as vocal fatigue, vocal breaks or cause your voice to sound abnormal.

A symptom with many causes

Losing your voice is a symptom and not a condition itself, says voice care specialist Claudio Milstein, PhD.

“If you have lost your voice, you might find that your voice sounds rough, raspy, tired or feel like it takes a lot of effort to speak,” he says.

Most often, one of these causes is to blame:

  • An upper respiratory infection such as a cold, cough, bronchitis, laryngitis or sinusitis.
  • Seasonal allergies that cause sinus drainage, throat clearing and laryngitis.
  • A vocally demanding job that requires you to use your voice frequently over the course of several hours, such as teaching or working in a call center.
  • Talking loudly, yelling or cheering, such as at a sporting event.

If you can trace your voice problems back to one of these sources, then losing your voice once in awhile likely isn’t serious, Dr. Milstein says. If you rely on your voice in your job, you may find that it happens more often for you and may be more of a problem if you rely on your voice daily.

But, occasionally, losing your voice might signal that precancerous or cancerous cells are forming. So you shouldn’t ignore a problem that persists beyond two to four weeks.

How long should you wait it out?

Hoarseness is more likely to happen during an upper respiratory illness. You’re coughing, clearing your throat a lot and your voice may start to sound croaky or weak. This can linger from a few days to a few weeks, or even longer in some cases, Dr. Milstein says.

How long should you wait to see if it will go away on its own? It depends.

“If you rely on your voice for your job, make plans to see your doctor if your voice isn’t better in two weeks,” he says.

Risk factors such as a history of smoking, a history of cancer or other health concerns should prompt an evaluation for hoarseness that lingers beyond two to four weeks.

Here’s what to do in the meantime

While there is no quick fix to help get your voice back, Dr. Milstein offers these tips to soothe inflamed vocal cords:

  1. Rest. Aim to rest your voice as much as possible. Talk quietly and only talk half as much as you ordinarily would, or less if possible.
  2. Seek out quiet. Avoid loud environments. They can often force you to talk more loudly (or with greater effort) than usual.
  3. Stay hydrated. Drink plenty of water. Avoid dehydrating beverages, such as alcohol and those that contain caffeine.
  4. Use a humidifier. This will help keep the air you breathe moist, which can help soothe inflamed vocal cords.
  5. Medicate. Try an over-the-counter medication, such as acetaminophen or ibuprofen. This can help ease discomfort and vocal cord inflammation.

If your hoarseness persists for longer than two weeks to a month, talk with your doctor. He or she may suggest a laryngoscopy, a procedure that will offer a better view of your larynx.

“Our ability to better visualize your larynx can help us more quickly diagnose something that might need surgical management or medication,” Dr. Milstein says.

Some experts recommend visualization of the vocal cords before beginning prescription medications, such as anti-reflux medications, steroids or antibiotics.

The bottom line? Most of the time your laryngitis will resolve itself in a week or two, with no lasting effects. But if the problem lingers on toward a month, it’s a good idea to get checked by a otolaryngologist (ear, nose, and throat) doctor to make sure there isn’t something serious at work.

What Causes me to Lose my Voice?

Even though we commonly associate a lost voice to getting sick, there can be a variety of reasons why someone loses their voice. Some of the reasons are very common while others are more rare but may be more serious.

The Science Behind a Lost Voice

When you lose your voice, in most cases the larynx and area around the voice box has become irritated, inflamed or swollen. When the vocal cords swell, they cannot produce normal sound. Irritated or inflamed vocal folds cannot vibrate properly and the sound they produce is abnormal and sounds “hoarse.” When this occurs, even though you are trying to speak normally, it may only come out as a whisper. Common Causes of a Lost Voice

There can be a number of conditions which cause an individual to lose their voice. These might include things like:

  • Acute Laryngitis
  • Chronic Laryngitis
  • Reflux or GERD
  • Misuse or Overuse of the Voice

In a vast majority of cases, a lost voice is caused by a virus or bacteria infecting the larynx. The virus typically comes from some other type of illness like a cold, bronchitis or the flu. The most common reason a person loses their voice is acute laryngitis. This is usually brought on by a viral infection, causing the inflammation in the area of the vocal cords.

Misusing the Voice

Speaking may seem like a simple task, but along with breathing it incorporates the use of numerous muscles. Just like muscle groups, the voice can be affected if not used properly. Overusing the voice might include speaking too loudly for a long period of time, talking for extended periods of time throughout the day, using a pitch that is too high or too low, or yelling excessively. This can often be a result of occupational voice use.

Serious Conditions Associated with a Lost Voice

There can also be some very serious conditions associated with losing your voice. These might include:

  • Vocal Cord Lesions (Benign)
  • Vocal Cord Hemorrhage
  • Vocal Cord Paralysis and Paresis
  • Laryngeal Cancer

In some cases, overuse or misuse of the voice can cause benign vocal cord lesions. These are like bumps that appear on the vocal cords due to trauma. Nodules, polyps or cysts are the most common types of lesions on the vocal cords. Microsurgery may be required to treat the lesions. Sometimes when a person is yelling or shouting they lose their voice suddenly. This can be the result of injuring the vocal cords and causing a hemorrhage. If you lose your voice suddenly after doing something strenuous like screaming or yelling, it is important to speak to your doctor as soon as possible.

Risk Factors for Losing Your Voice

There are some factors that can cause you to lose your voice. The three most common factors include: smoking, overusing the voice, and upper respiratory infections such as colds, bronchitis or the flu.   • Avoid whispering (this causes too much strain on the vocal cords)
  • Don’t smoke
  • Avoid “clearing” your throat
  • Do not drink alcohol
  • Use a humidifier

You may have a voice disorder if you have a problem with pitch, volume, tone, and other qualities of your voice. These problems occur when your vocal cords don’t vibrate normally.

Your voice is the sound that air makes when it is forced out of your lungs and passes over your vocal cords. Vocal cords are the 2 folds of tissue inside your larynx, also called the voice box. The vibration of those cords is what produces speech.

Examples of voice disorders include:

  • Laryngitis. Laryngitis is when your vocal cords swell. It makes the voice sound hoarse. Or you may not be able to speak at all. Acute laryngitis happens suddenly, often because of a virus in the upper respiratory tract. It often lasts just a few weeks. Treatment is to rest the voice and drink plenty of fluids. Chronic laryngitis is when the swelling lasts for a long time. Common causes include a chronic cough, using inhalers for asthma, and GERD. Treatment of chronic laryngitis depends on the cause.
  • Vocal cord paresis or paralysis. The vocal cords can be paralyzed, or partially paralyzed (paresis). This can be caused by a viral infection that affects your vocal cord nerves, an injury to a nerve during surgery, stroke, or cancer. If one or both of your vocal cords are paralyzed in a nearly closed position, you may have noisy or difficult breathing. If they are paralyzed in an open position, you may have a weak, breathy voice. Some people will get better over time. In other cases, the paralysis is permanent. Surgery and voice therapy may help improve the voice.
  • Spasmodic dysphonia. This is a nerve problem that causes the vocal cords to spasm. It can make the voice sound tight, quivery, or jerky, hoarse, or groaning. At times, the voice may sound normal. Other times, the person may not be able to speak. Treatment may include speech therapy and injections of botulinum toxin to the vocal cords.                                                                     

What causes voice disorders?

For normal speech, your vocal cords need to touch together smoothly inside your larynx. Anything that interferes with vocal cord movement or contact can cause a voice disorder. Many voice disorders can be cured with treatment when diagnosed early.

Voice disorders can be caused by many factors. In some cases, the cause of a voice disorder is not known. Possible causes can include:

  • Growths. In some cases, extra tissue may form on the vocal cords. This stops the cords from working normally. The growths can include fluid-filled sacs called cysts, wart-like lumps called papilloma, or callus-like bumps called nodules. There may be patches of damaged tissue called lesions, or areas of scar tissue. In some people, a band of tissue called a web can grow between the vocal cords. Other growths include a small area of chronic inflammation called a granuloma, and small blisters called polyps. Growths can have many causes, including illness, injury, cancer, and vocal abuse.
  • Inflammation and swelling. Many things can cause inflammation and swelling of the vocal cords. These include surgery, respiratory illness or allergies, GERD (acid reflux), some medicines, exposure to certain chemicals, smoking, alcohol abuse, and vocal abuse.
  • Nerve problems. Certain medical conditions can affect the nerves that control the vocal cords. These can include multiple sclerosis, myasthenia gravis, Parkinson disease, Amyotrophic lateral sclerosis (ALS), and Huntington disease. Nerves can also be injured from surgery or chronic inflammation of the larynx (laryngitis).
  • Hormones. Disorders affectingthyroid hormone, female and male hormones, and growth hormones can cause voice disorders.
  • Misuse of the voice. The vocal cords can be stressed by using too much tension when speaking. This can cause problems in the muscles in the throat, and affect the voice. Vocal abuse can also cause a voice disorder. Vocal abuse is anything that strains or harms the vocal cords. Examples of vocal abuse include too much talking, shouting, or coughing. Smoking and constant clearing of the throat is also vocal abuse. Vocal abuse can cause the vocal cords to develop calluses or blisters called nodes and polyps. These change how the voice sounds. In some cases, a vocal cord can rupture from vocal abuse. This causes the cord to bleed (hemorrhage), and can cause loss of voice. Vocal cord hemorrhage needs to be treated right away.

What are the symptoms of a voice disorder?

If you have a voice disorder, your voice may:

  • Have a quivering sound
  • Sound rough or harsh (hoarseness)
  • Sound strained or choppy
  • Is weak, whispery, or breathy
  • Is too high or low or change in pitch

You may have tension or pain in your throat while speaking, or feel like your voice box is tired. You may feel a “lump” in your throat when swallowing, or feel pain when you touch the outside of your throat.

How are voice disorders diagnosed?

If you have a voice change that lasts for a few weeks, your healthcare provider may send you to see a throat specialist called an otolaryngologist (Ears, Nose and Throat specialist or ENT).  An otolaryngologist will ask you about your symptoms and how long you’ve had them. He or she may examine your vocal cords and your larynx using certain tests. These may include:

  • Laryngoscopy. This lets the doctor view the throat. With indirect laryngoscopy, the healthcare provider holds a small mirror at the back of the throat and shines a light on it. With fiberoptic laryngoscopy, a thin, lighted scope called a laryngoscope is used. The scope is put through your nose down into your throat, or directly down into your throat.  
  • Laryngeal electromyography, or EMG. This test measures electrical activity in the muscles of the throat. A thin needle is put into some of the neck muscles while electrodes send signals from the muscles to a computer. This can show nerve problems in the throat. 
  • Stroboscopy. This test uses a strobe light and a video camera to see how the vocal cords are vibrating during speech.
  • Imaging tests. X-rays and MRI can show growths or other tissue problems in the throat.

How are voice disorders treated?

Treatment for a voice disorder depends on what’s causing it. Treatment may include:

  • Lifestyle changes. Some lifestyle changes may help reduce or stop symptoms. These can include not yelling or speaking loudly, and resting your voice regularly if you speak or sing a lot. Exercises to relax the vocal cords and muscles around them can help in some cases. Warm up the vocal cords before extensive periods of speaking. Stay hydrated.      
  • Speech therapy. Working with a speech-language pathologist can help with certain voice disorders. Therapy may include exercises and changes in speaking behaviors. Some of these may include maneuvers that time deep breaths to power vocalizations with adequate breathing.
  • Medicines. Some voice disorders are caused by a problem that can be treated with medicine. For example, antacid medicine may be used for GERD, or hormone therapy for problems with thyroid or female hormones.
  • Injections. Your doctor can treat muscle spasms in the throat with injection of botulinum toxin. In some cases, your doctor can inject fat or other fillers into the vocal cords. This can help them close better.
  • Surgery. Your doctor can remove some tissue growths. If cancer causes the growths, you may need other treatment, such as radiation therapy.

What Does Being Hoarse Mean?

A hoarse voice is when someone sounds rough, raspy, or breathy when speaking. Hoarseness happens because the vocal cords are strained. In kids, this can happen from everyday things like cheering loudly at a ballgame, belting out a favorite song in the shower, or calling out to friends on the playground.

Sounding hoarse for a few hours or the day after a big game is nothing to worry about. Usually, the voice returns to normal on its own.

What Are the Vocal Cords?

The vocal cords are delicate bands of tissue in the larynx (LAIR-inks), or voice box. When we speak, air pushes out of the lungs. In the larynx, the vocal cords — a “V”-shaped band of muscle — prepare to make sound by tightening up and moving closer together. As air passes through them, they vibrate. This vibration, combined with the movement of the tongue, lips, and teeth, is what makes the sound of the voice.

What Causes Chronic Hoarseness?

Chronic (long-lasting or ongoing) misuse of the vocal cords — from things like a lot of yelling or using the voice in an unnatural way — puts excess wear and tear on the vocal cords. They may stretch too far or rub together, causing small irritations. If these aren’t allowed to heal, they turn into small calluses, or vocal cord nodules. Vocal cord nodules are the top cause of chronic hoarseness in children.  

Nodules can happen when kids do things like these for long periods:

  • yelling, screaming, cheering, or crying
  • raising or lowering the pitch of the voice
  • speaking in a strained voice, like imitating animals or cartoon characters
  • repetitive coughing or throat clearing
  • singing or talking without taking a good breath
  • talking extremely loud or fast
  • starting words forcefully

How Are Vocal Cord Nodules Diagnosed?

Doctors will want kids with chronic hoarseness to be checked by an otolaryngologist (an “ears, nose, and throat” specialist, or ENT). The ENT will:

  • ask about the child’s
    medical history
  • listen for strain or breathiness in the voice
  • do a diagnostic test to get an internal view of the voice box and check vocal cord function

The diagnostic test options include:

Flexible laryngoscopy. During this exam, a tiny, flexible fiber-optic tube with a camera attached to the end (called a scope) is passed through a nostril and into the throat. It gives a magnified view of the larynx. Kids are asked to speak, sing, sniff, cough, and make other sounds that make the vocal cords vibrate. Movements are recorded on a computer monitor so that specialists can analyze them later.

Rigid laryngoscopy. This can be done in the office for older kids, but very young children and others who can’t tolerate a flexible laryngoscopy will have this test in an operating room under general anesthesia. The rigid scope passes through the mouth to provide the best view of the vocal cords. Images of the vocal cords at rest are recorded.

Laryngeal stroboscopy. To get an accurate view of the vocal cords in action, doctors pass either a rigid or flexible scope attached to a strobe light into the throat. The strobe light flickers in sync with the vibration of the vocal cords as a person speaks. This makes it easier for doctors to determine the frequency of movement. Because individual vibrations are too fast for the naked eye to see, the strobe technology projects the vibrations in slow motion.

How Is Chronic Hoarseness Treated?

Treatment for hoarseness caused by vocal cord nodules involves making behavioral changes so that the vocal cords can heal. Speech therapists work one-on-one with kids and their families to promote good vocal habits, or what’s called “vocal hygiene.”

A typical vocal hygiene program involves:

  • treating any medical conditions (like GER) that aggravate vocal problems
  • changing behaviors that distress the vocal cords, like yelling
  • learning how to take deep breaths and relax the muscles in the throat
  • taking time to speak slowly and clearly
  • staying hydrated throughout the day
  • avoiding caffeinated beverages, like soda, which dry out the vocal cords

It might be hard for kids with established bad habits — like talking loudly when they’re excited or clearing their throats when they’re nervous — to make changes. The first step is to make kids aware of the behavior and see how often they do it.

Older kids might learn to keep track of how often they engage in the behavior (perhaps keeping notes in a diary), which many do without realizing. Then they can practice the skills they’ve learned in therapy when at home, at school, and spending time with friends.

How Can Parents Help?

Parents can encourage good vocal habits by setting a good example themselves. For example, rather than calling to kids from another room, walk into the other room to talk to them. Promote quiet times (perhaps for half an hour each day) and using an “inside voice” when indoors.

Reward systems that encourage these new behaviors can help elementary school-age kids succeed. Offer stickers, tokens, extra TV time, a later bedtime, or similar “prizes” to get kids on board with the new, healthier habits.

The most common treatments for nodules, polyps, and cysts are voice rest, voice therapy, and surgery to remove the tissue.

Vocal fold hemorrhage. Vocal fold hemorrhage occurs when a blood vessel on the surface of the vocal fold ruptures and the tissues fill with blood. If you lose your voice suddenly during strenuous vocal use (such as yelling), you may have a vocal fold hemorrhage. Researchers funded by the NIDCD are working with teachers to devise strategies to help them reduce the stress and strain on their voices. In one study, the teachers use a voice “dosimeter” that takes into account the frequency and loudness of their speech along with the duration of vibration in their vocal folds to determine their daily “dose” of vocal use. The researcher hopes to establish a safe level of voice use, as well as recommended recovery times from prolonged speaking.

In another study, researchers are working with two groups of student teachers in the United States and China to test the effectiveness of voice hygiene education on its own and with voice production training.
