Understanding Epidermoid Cysts

An epidermoid cyst is a benign, slow-growing cyst that develops from the outermost layer of the skin, known as the epidermis. These cysts are typically filled with keratin, a protein found in the skin, hair, and nails, as well as lipid-rich debris. Epidermoid cysts are also commonly referred to as epidermal cysts, epidermal inclusion cysts, or sebaceous cysts (though this term is inaccurate, as the cysts do not involve sebaceous glands).

Who Develops Epidermoid Cysts?

Epidermoid cysts most commonly occur in adults, particularly in young to middle-aged individuals. Men are affected twice as frequently as women. Certain genetic disorders, such as Gardner syndrome, Pachyonychia congenita type 2, and Basal cell nevus syndrome, can increase the risk of developing multiple epidermoid cysts.

Causes of Epidermoid Cysts

Epidermoid cysts generally result from an occluded pilosebaceous unit, which is the structure that includes the hair follicle and sebaceous gland. In non-hair-bearing areas of the body, such as the buttocks, palms, or soles, epidermoid cysts may develop due to the traumatic implantation of epidermal cells into the deeper layers of the skin, where keratin can accumulate within an epithelium-lined sac.

Clinical Features of Epidermoid Cysts

The clinical features of epidermoid cysts include a firm, flesh-colored or yellowish, round papule or nodule that is fixed to the skin surface but typically mobile over deeper layers. The cysts are usually 1-3 cm in diameter and may have a central punctum, from which a foul-smelling, cheesy debris can be expressed. Epidermoid cysts are most commonly found on the central trunk (chest and shoulders) and face, but can occur almost anywhere on the body, including the scrotum and vulva. They may be solitary or multiple and are generally asymptomatic.

Complications of Epidermoid Cysts

The primary complications of epidermoid cysts include:

  • Rupture of the cyst contents into the dermis, resulting in swelling, redness, and tenderness, often due to bacterial infection
  • Bleeding, secondary bacterial infection, and scarring following surgical excision
  • Recurrence of the cyst if the entire capsule is not removed
  • Rare development of cutaneous squamous cell carcinoma within the cyst

Diagnosis of Epidermoid Cysts

The diagnosis of an epidermoid cyst is typically made through clinical examination. Biopsy is usually not required, but the lesion may be surgically excised for cosmetic reasons or due to complications. Histological examination of the cyst would reveal a cystic structure in the dermis, a single cavity (unilocular), and an epithelial lining without rete ridges and with a granular layer containing keratohyaline granules. Ultrasound can be used in the initial evaluation of a soft tissue mass, but is not typically necessary for a typical epidermoid cyst.

Differential Diagnosis

The differential diagnosis for an epidermoid cyst includes:

  • Lipoma: a mobile, 2-10 cm, dome or egg-shaped subcutaneous lump with a rubbery or soft, smooth consistency
  • Trichilemmal cyst: a firm, mobile, 0.5-5 cm subcutaneous nodule without a central punctum, usually presenting on the scalp, with a thick capsule and less prone to rupture
  • Acne pseudocyst: lacks a capsule and is associated with other signs of acne
  • Myxoid pseudocyst: a shiny papule arising at the end of a digit
  • Dermoid cyst: has epidermal and dermal components and arises in early childhood
  • HPV-related epidermal cyst: a lesion with a hard, keratinous surface

Treatment of Epidermoid Cysts

Most small, uncomplicated epidermoid cysts do not require treatment. The most effective treatment for an epidermoid cyst is complete surgical excision with an intact cyst capsule. Removal of the entire cyst lining decreases the rates of recurrence. In some cases, the cyst contents may be aspirated, but this is not a long-term solution, as the cyst will often refill and may become inflamed. Topical or intralesional corticosteroids can be used to reduce inflammation in the short term, but do not provide a definitive cure.

What is an epidermoid cyst?

An epidermoid cyst is a benign cyst derived from the infundibulum or upper portion of a hair follicle, encapsulated in a thin layer of epidermis-like epithelium. Epidermoid cysts are typically filled with keratin and lipid-rich debris [1,2].

Synonyms for an epidermoid cyst include:

  • Epidermal cyst
  • Epidermal inclusion cyst
  • Epithelial cyst
  • Follicular infundibular cyst
  • Infundibular cyst
  • Keratin cyst
  • Sebaceous cyst (this is a common misnomer, as these cysts do not involve sebaceous glands, nor do they contain sebum).

Who gets an epidermoid cyst?

Epidermoid cysts most commonly occur in adults, particularly when young-to-middle aged. They occur twice as frequently in men than in women [3].

Genetic disorders which may increase the risk of developing multiple epidermoid cysts include [1]:

  • Gardner syndrome
  • Pachyonychia congenita type 2
  • Basal cell naevus syndrome.

Epidermoid cysts are most common on the central trunk (eg, chest and shoulders) and face but can occur almost anywhere on the body. Epidermoid cysts are common the scrotum and vulva. They may be solitary or multiple, and are generally asymptomatic.

What are the complications of an epidermoid cyst?

Rupture of the cyst contents into the dermis results in swelling, redness, and tenderness. This can be due to trauma or bacterial infection, commonly by Staphylococcus aureus, Escherichia coli, and group A streptococcus [1].

Surgical excision can lead to bleeding, secondary bacterial infection, and scarring. If the cyst has ruptured, or the capsule is not removed in its entirety, the cyst may recur.

Cutaneous squamous cell carcinoma may very rarely arise within an epidermoid cyst [5].

The diagnosis of an epidermoid cyst is usually made clinically.

The most effective treatment for an epidermoid cyst is complete surgical excision with an intact cyst capsule. Removal of the entire cyst lining decreases rates of recurrence [6]. This can be difficult to achieve following cyst rupture. Histological examination of the surgical specimen is recommended due to the small risk of malignant transformation and misdiagnosis [6].

In cases of infection, initial antibiotics, incision and drainage may be indicated.

What is the outcome for an epidermoid cyst?

Epidermoid cysts are typically benign and slow growing, rarely undergoing malignant transformation. Occasionally, they resolve spontaneously without intervention [1].

Female Genital Sores: Pictures, Causes, and Diagnosis

Female genital sores may be the result of an infected scratch, a sexually transmitted infection (STI), a chronic skin condition, or vulvar cancer. See a doctor for a diagnosis, especially if the sores bleed or do not heal.

Female genital sores are bumps and lesions in or around the vagina or vulva. These sores can extend from the genital area to the anus.

Some may be itchy, painful, tender, or produce a discharge. And some may not cause any symptoms.

Genital sores can occur for no reason and resolve on their own. But some may be due to certain skin disorders or be a symptom of a sexually transmitted infection (STI).

Genital sores may appear as small, red, or flesh-colored bumps and blisters. Sores may also change appearance and become crusty or larger.

They can also be accompanied by other symptoms, such as:

  • itchiness
  • pain at the site
  • pelvic pain
  • burning
  • bleeding
  • discomfort when urinating

STIs, in general, are also associated with symptoms, such as:

  • painful intercourse
  • discomfort when urinating
  • increased or foul-smelling vaginal discharge

But some STIs don’t have any symptoms, which make them undetectable without testing.

The most common causes of female genital sores are STIs, which can be spread through oral, vaginal, or anal sex, as well as through the sharing of sex toys.

STIs that can cause female genital sores include:

  • genital herpes, which is caused by a virus
  • genital warts, which is caused by a virus
  • chancroid, a bacterial disease
  • syphilis, a bacterial infection
  • molluscum contagiosum, a viral skin infection with raised, flat pearly nodules

Some chronic skin conditions may also lead to genital sores:

  • eczema, a skin inflammation often caused by allergies
  • vulvovaginitis, an inflammation of the vulva and vagina
  • contact dermatitis, a sensitivity to chemicals, detergents, and perfumes
  • cysts
  • ingrown hairs

Sometimes, even a scratch can become infected and cause a genital sore.

Bumps and lumps around your vulva that bleed or don’t go away can also be signs of vulvar cancer and will require immediate medical attention.

As there are a number of potential causes of female genital sores, it’s important to see a doctor for diagnosis.

They’ll need to physically examine you via a pelvic exam and ask about your medical history. They may also perform blood tests or swab the affected area and test it for the presence of bacteria or viruses.

A doctor should evaluate any genital bumps or sores to determine the cause and prevent any medical complications. It’s also important to find out if the cause is an STI, so you can be treated and avoid spreading it to sexual partners.

While you’re waiting for your appointment, a sitz bath can help relieve any pain or discomfort.

You can make a sitz bath at home by filling the bathtub with warm water that goes up to your hips when you’re seated. Add a mild saline solution or baking soda to the water. You can also purchase a small basin for a sitz bath from a drugstore and use it instead of a bathtub.

The exact form of treatment depends on the cause of the genital sores. For example, some STIs, such as genital herpes, are incurable — but outbreaks can be treated with medication.

Topical and oral medications may treat the sores and relieve pain. Your doctor may prescribe:

  • antibiotics
  • antiviral medications
  • a corticosteroid, like hydrocortisone
  • pain relievers
  • other anti-itch drugs

Other genital sores, such as noncancerous cysts, don’t always require treatment. But you can have them removed if you prefer.

Practicing safer sex by using condoms can help stop the spread of STIs that cause genital sores.

If you have an STI, try to be open with your sexual partner(s) and talk about getting tested and treated.

You and your partner(s) should also avoid sexual contact until after treatment, as it’s possible to spread an STI back and forth.

Genital sores due to skin conditions or allergic reactions may be more difficult to prevent. Avoid known irritants, such as abrasive soaps or strong fragrances.

The chance of cysts and ingrown hairs, meanwhile, can be reduced by carefully washing the genital area and not shaving red, swollen, or affected areas.

It’s best to see a doctor as soon as possible. That way, they can properly diagnose genital sores and prescribe an appropriate course of treatment.

So if you notice a new genital sore, a change in an existing sore, genital itching, bleeding, pain, or fever alongside sores, book an appointment straight away.

Seeing a doctor is particularly important if the cause is an STI. Young people assigned female at birth (AFAB) are especially at risk for developing serious long-term health complications from untreated STIs, according to the Centers for Disease Control and Prevention (CDC).

The long-term outlook depends on the cause. In many cases, female genital sores can be cured with treatment. However, some conditions, such as genital herpes or chronic skin issues, can be lifelong, leading to recurring sores.

Your outlook also depends on the timing of treatment. Untreated STIs can cause serious health complications for AFAB folks, including:

  • pelvic inflammatory disease (PID)
  • infertility
  • scarring of the reproductive organs
  • increased risk of ectopic pregnancy

Your doctor can discuss long-term treatment options for these types of conditions to manage symptoms, prevent complications, and help avoid outbreaks.
