Overview

Hidradenitis suppurativa is a chronic condition of the sweat glands. The sweat glands become clogged due to a variety of reasons, leading to enlargement (dilatation) of the ducts behind them. This dilatation predisposes the area to inflammation and infection by a variety of bacteria. Those with hidradenitis suppurativa typically have multiple large, red bumps on the buttocks, breasts, groin, and armpits. The bumps gradually get larger and drain pus. After multiple bouts of this cycle of plugging, enlargement, and drainage, there may be scarring.

While there is no cure for hidradenitis suppurativa, you can work with your doctor to treat existing lesions and prevent new ones.

Who’s at risk?

Hidradenitis suppurativa usually starts soon after puberty and continues into adult life. It is more common in women and in African Americans. Hair removal from shaving or using depilatories, deodorants, and irritation from anything rubbing against the affected area can worsen the condition. Hidradenitis suppurativa is often associated with smoking, obesity, and increased hormones. Additionally, it often runs in families.

Signs and Symptoms

The groin and armpits have high concentrations of sweat glands, which is why these areas are most severely affected by hidradenitis suppurativa. There may also be lesions between the buttocks or under the breasts due to the friction from skin rubbing together, which is why people who are obese are more predisposed to this disease.

The lesions of hidradenitis suppurativa are firm tender, red bumps with extensive scar and sinus tracking under the surface of the skin. Often, you will see a pair of blackheads within one lesion, and the lesions may drain pus. Hidradenitis suppurativa lesions are rarely seen on the trunk of the body or on the scalp or legs.

Some people may experience only one or few lesions, while others may have severe disease, affecting many areas. Increasing hormones and excessive perspiration can cause more lesions.

Self-Care Guidelines

Make sure to wash any red, draining area(s) of hidradenitis suppurativa with an antibacterial soap, and then apply antibiotic ointment and clean bandages. If there is a large amount of drainage, change the gauze pads and dressings often. Warm compresses and ibuprofen can help reduce the swelling. Avoid tight-fitting clothing to prevent further irritation.

Weight loss may decrease lesions by decreasing skin folds and, thus, friction on the skin. Smoking should be avoided.

When to Seek Medical Care

Some cases of hidradenitis suppurativa can be treated with self-care measures. Healing of individual lesions takes about 1–2 weeks. Make sure to seek medical care if you are not able to control the pain with ibuprofen and warm compresses. Medical treatment may be necessary if the condition appears in multiple areas or continues to come back (recur). If there is redness, excessive pain, excessive drainage, red streaking, you have a fever, or there are other factors that may indicate there is an infection, see your doctor. If you have been given antibiotics and the area does not improve within a few days, return your doctor.

Treatments Your Physician May Prescribe

Your doctor may prescribe a topical antibiotic solution for mild areas or a long course of oral antibiotics to treat more severe areas. Your doctor may also send drainage for laboratory analysis to make sure that you are on the appropriate antibiotics. You may be given an injection of steroids into the deep, painful hidradenitis suppurativa lesions. If excessive hormones are thought to be the problem in a woman, she may be prescribed hormonal therapy. Oral tretinoin, used in severe acne, may be used to treat plugged sweat glands and keep them from plugging in the future. Steroids and immunosuppressive medications may be used in severe cases.

Surgical removal is often the ideal treatment for severe cases of hidradenitis suppurativa that continue to come back.

Also known as skin abscesses, boils form as a result of a cut or break in the skin, which leads to a bacterial infection. They are characterized as a red, tender area with a painful, pus-filled center that can open spontaneously or by surgical incision. Some boils are caused by an ingrown hair. Others are caused by plugged-up sweat glands, such as some types of cystic acne. Anyone can get a boil. They grow quickly and are usually painful until they drain. However, left alone a boil will naturally come to a head and burst open, allowing the pus to drain and the skin to heal. People with weakened immune systems are more susceptible to boils than the general population.

Boils tend to occur on parts of the body that have hair or sweat glands and are exposed to friction, typically on the face, neck, armpits or buttocks. There are a variety of different types of boils:

Furuncle or Carbuncle. These abscesses are caused by the Staphylococcus aureus bacterium. A furuncle is an individual boil; carbuncles are deep clusters of boils that most often form on the back of the neck, shoulders or thighs.

Pilonidal Cyst. An infected hair follicle around the buttocks area caused by long periods of sitting. Pilonidal cysts almost always require medical treatment.

Hidredenitis Suppurativa. These are multiple abscesses that form from blocked sweat glands in the armpits or groin areas.

Cystic Acne. These boils are situated more deeply into skin tissue than the more superficial forms of acne. It typically occurs among teenagers.

Boils respond well to home remedies. To promote healing, apply heat to the boil in the form of hot soaks or compresses. Keep the area clean, apply over-the-counter antibiotics and then cover with gauze. Do not puncture or squeeze the boil because it can lead to further infection. If the boil does not go away within two weeks, is accompanied by a fever or is painful, contact your dermatologist. The doctor will clean, lance and drain the boil and prescribe an antibiotic to alleviate the infection.

What is Hidradenitis Suppurativa (HS)?

HS is a chronic disease that presents with sore red lumps that look like “boils”. These very painful lesions are found under the arms and breasts and in the groin, buttock and around the anus, basically the follicles that are in the hot, moist, sweaty gland-bearing areas. These spots appear suddenly, increase in size rapidly and can then rupture sideways under the surface of the skin or drain to the surface. This condition is sometimes referred to as “inverse acne” because it presents like the severe cystic acne vulgaris usually seen on the face, chest and back in teenagers, with painful cysts, sinuses, blackheads and scars, but in unexpected (inverse) areas. It is more common in women than in men. The pattern of involvement varies from patient to patient.

What causes HS?

This condition develops due to blockage of the ducts leading to the skin pores. The pores are the tiny openings to the skin surface from the oil glands and hair follicles below. When the duct ‘lining cells’ block the ducts, a plug builds up in those blocked areas and starts expanding the duct, which leaks its contents, causing inflammation and irritation. The oil duct and hair follicle eventually explode sideways underneath the skin. The body develops a strong irritant and allergic reaction to this foreign material under the skin. The reaction is like that from a buried splinter or ingrown hair with the development of large, red, hot, painful swellings that eventually break down and drain pus, even though this is not an infection. This problem can be hereditary, so ask other family members about having similar problems or “boils”.

Do you know the symptoms of HS?

First there are red, swollen, bumps that develop into “boils”. These heal with pitted scars. The “boils” can be solitary, scattered in an area or grouped together. Blackheads may be seen. Some patients have 1-2 areas involved while others will have many areas extensively involved. The grouped lesions may form a large swollen area that is connected by small tunnels under the skin, called sinuses. If you press on any areas of involvement you can probably find pus coming out of openings nearby.

Worsening factors include

In addition to the family history, this condition is related to hormones. It commonly starts around puberty. It often gets worse each month with the menstrual cycle. Stress can be a factor. Friction in areas of involvement is often a problem. The walls of the pores or ‘follicles’ in the sweaty areas of the body seem to be “weaken” and easily rupture. Anything that rubs the areas (tight clothing, menstrual pads, etc.) can cause the plugged and swollen ducts to break down more easily. Try to avoid any friction or irritation. Squeezing these “boils” or “cysts” will always make things worse. Sweating can trigger a flare.

Hurley Staging

To decide the best treatment for you it is important to know how severe your problem is, and this is done with a staging system that is referred to as Hurley’s Criteria.

Hurley’s Stage I – there are some abscesses (“boils”), one or several, but they do not have the small tunnels under the skin and scarring is fairly minor.

Hurley’s Stage II – there are recurrent abscesses (“boils”) with small sinuses under the skin and scarring. There may be one or several of these complexes scattered in different areas, or small groups of them.

Hurley’s Stage III – there are large areas involved with multiple interconnected tunnels and tracks and draining lesions with a lot of scarring.

How is HS diagnosed?

The diagnosis is made by recognizing the typical skin changes in the typical locations. The pattern of recurrent “boils” in these particular areas, especially when they do not respond to standard antibiotics, is a good clue.

Normal boils are caused by bacteria and respond quite well to antibiotics. Normal boils usually do not recur in areas after treatment. Normal boils ‘point’ and discharge vertically to the surface and do not make horizontal tunnels under the skin.

What are the treatment options?

Treatment depends on severity. Treatment may just involve medications that are used on the skin or taken orally. Long term and very low dose isotretinoin (formerly Accutane) or acitretin can help prevent new plugging. Surgery of various types is useful in all stages.

General treatment

  • It is important to reduce any friction in the areas where you have recurrent spots. Change to loose cotton clothing such as loose boxer shorts, avoid underwear with seams that rub in areas that give you new lesions. Try to wear clothing that is loose and cool so that you are not overheated and sweating in those areas. Take fresh underwear and a ZiplocR bag to work and change during the day.
  • Wash all clothing, towels, pajamas, sheets, etc. in a laundry detergent free of enzyme. ALLR Free / Clear is the only one we know of.
  • Try hard to reduce obesity and get down to ideal weight. The best diet is the Paleolithic diet (www.thepaleodiet.com) but the most important part is TOTAL avoidance of all dairy products, and a slow steady weight loss to ideal weight.
  • Antiseptic washes can be helpful. These do not cure anything but can help in areas of odor and drainage. Use a triclosan-containing cleanser like DialR antiseptic daily. Otherwise use a mild soap-free cleanser such as DoveR for Sensitive Skin – Fragrance Free or CetaphilR Cleanser for Sensitive Skin.
  • Stopping all nicotine from any source is essential. Nicotine stimulates plugging of the pores. Also, toxins in smoke appear to interfere with proper healing. We have never successfully cleared a smoker of this disease.
  • For women it is best to block male hormones.
    • This can be done using birth control pills (BCPs) such as Yasmin (or Ocella) or Yaz (or its generic)
    • We can add an extra hormone blocker called spironolactone to the BCP.
    • There are other male hormone blockers called finasteride and dutasteride but they are less used than the BCP blockers. (Pregnancy must be avoided with the use of hormone blockers). Dutasteride has made a big difference in some male patients.
  • For men
    • The male hormone blocker called dutasteride works well in men.
  • To decrease inflammation, antibiotics are recommended.
    • Antibiotics on the skin – 1% clindamycin lotion applied morning and night
    • Antibiotics by mouth – antibiotics have traditionally been used for short periods of time or sometimes for weeks or months. Doxycycline, clindamycin, amoxicillin/clavulinic acid, Bactrim or combinations like clindamycin and rifampin are anti-inflammatory but are not capable of curing the disease because this disease is NOT due to infection.
    • Zinc gluconate by mouth with food – 30 mg three times a day.
    • For painful spots, an injection of cortisone called triamcinolone acetonide (KenalogR) may be used, and it may quickly take the inflammation down over 2-3 days.
    • Oral cortisone (prednisone or medroxyprednisolone) may be given for short periods or intermittently to temporarily cool inflammation.
  • For Hurley Stage II – systemic anti-inflammatory antibiotics are used as in Stage I, for weeks or months. Clindamycin may be combined with rifampin. Intralesional triamcinolone may be used. Dapsone can be very useful.
  • Surgical unroofing is a simple, yet very effective technique that can stop forever the progression of the treated lesions. It is far more effective than prolonged antibiotics and anti-inflammatory therapy, which do not treat the cause. It is done in the office, under local anesthesia, and healing is usually complete in less than 14 days. Pain is usually controlled with Tylenol and antibiotics are not normally required.
  • TNF blockers: these medicines block TNF (Tumor Necrosis Factor). This molecule is made by our bodies and stimulates inflammation. There are possible side effects and reasons why you may not be able to take these medicines. They include Remicade®, Enbrel®, Simponi® or Humira®.
  • Preliminary work suggests that a zero-dairy diet may lead to clearing in some patients. The diet needs to be really well controlled. Nothing at all from cow source can be used. In addition to no milk, cream, cheese, butter, yoghurt, cream cheese, cottage cheese, ice cream and derivatives like cheeseburgers and lasagna, this means checking protein powders and protein drinks to be sure there is no casein, whey, milk protein isolate, or milk solids. Soy and other vegetable and nut-sourced drinks and foods are acceptable, but should be the unsweetened variety only. No French vanilla; limit the sweetened chocolate.
  • Staged CO2 laser marsupialization surgery for scarred and persistent lesions is now available. It is done in the office under local numbing, although IV sedation is available. The entire involved piece of skin is removed down to the deep dermal layer, and all edges are explored to find any hidden tracts or extensions. This reduces the possibility of recurrence.   • Cryoinsufflation with liquid nitrogen is a technique used to treat smaller areas with multiple sinus tracts or fistulas. It is done in the office under local numbing, although IV sedation is available. The liquid nitrogen is sprayed into the tracts via a small needle on an extension. It will course through all hidden tracts or extensions, causing them to scar down. Post-procedure pain is minimal, and while scarring, color change and injection can occur, they are very rare. Three sessions are down at monthly intervals.

What will happen to me?

Some patients clear with early care and avoidance of all the irritants, the dairy and highly refined flour and sugar-containing foods, hormones, smoking, and stresses that appear to trigger the problem. They can be kept in Stage I or actually cleared. Patients in Stage II can be brought back to Stage I with aggressive care. And even some Stage III patients can be brought back to Stage II and Stage I. The drugs and diets and hormones need to be used for many years. This usually cannot be done with drugs and diet alone. Surgical treatment of the areas is often necessary but is usually nowhere near as difficult as most patients imagine.

Introduction

Acne conglobata is a rare but severe form of acne. It usually presents with deep burrowing abscesses that interconnect with each other. Scar formation and disfigurement of the body are common with this type of acne. The comedones often occur in groups of three and the cysts often contain purulent, foul-smelling material that is discharged on the skin surface. Acne conglobata nodules are found on the shoulder, chest, upper arms, buttocks, face, and thigh. Acne conglobata may occur following the sudden worsening of pustular acne, or the disorder may occur gradually following the recrudescence of acne that has been quiet for many years. [1][2][3]

It is important not to confuse acne conglobata with acne fulminans. They both present with inflammatory nodules on the trunk. The major difference is that the former induces polyporous comedones and non-inflammatory cysts, whereas the latter does not.

Acne conglobata resembles hidradenitis suppurativa with large tender nodules and draining sinus tracts. The involved area is intensely inflamed and fluctuant. Foreign body granulomas are common.

Etiology

It is believed that Propionibacterium acnes, the same organism implicated in acne vulgaris, may play an important role in the disease by changing its reactivity as an antigen. The hypersensitivity to this antigen induces an intense immunological reaction that presents with a chronic inflammatory state. The presence of the bacteria also leads to an infectious process with the development of pus and sinus tracts. The pus is usually foul-smelling and putrid.[4][5][6]

Ingestion of thyroid medication and exposure to halogenated aromatic hydrocarbons may trigger acne conglobata. Other factors that can provoke acne conglobata include androgens (e.g., androgen-producing tumors), and anabolic steroids.

Acne conglobata has also been reported to occur in individuals who stop anabolic hormones or as a reaction to other hormonal agents.

Some people with acne conglobata have the XYY karyotype. The skin disorder has been linked to HLA phenotypes.

Epidemiology

Acne conglobata is not very common. Overall, it is more common in men than women. Over the past 30 years, many reports have been published on athletes with this type of acne, and this is felt to be primarily to the use of anabolic steroids. Outside the US, not much is known about acne conglobata. The condition is usually seen in young adults and very rarely seen in children or elderly people. The onset is usually in the second and third decade of life. Whether it occurs more frequently in any specific race is not known.[7]

Pathophysiology

Acne conglobata presents with deep burrowing abscesses that interconnect with each other via sinus tracts. Initially, the nodular lesion may mimic a pimple, but underneath there is a vigorous inflammatory reaction and formation of pus. Over time, the pus pushes into the adjacent tissues and extrudes on the skin surface. Scar formation and disfigurement of the body are common with this type of acne. The comedones often occur in groups of three, and the cysts often contain purulent, foul-smelling material that is discharged on the skin surface.

History and Physical

Acne conglobata may follow the use of androgenic anabolic steroids and is quite common in bodybuilders. Many young adult males will present to the dermatology clinic complaining of severe acne and facial scars. It is important to seek a thorough history of the use of anabolic steroids because discontinuation of these agents is vital for treatment.

Acne conglobata may also occur in patients with hidradenitis suppurativa and pyoderma gangrenosum. When the condition develops following puberty, the nodules will gradually coalesce and increase in severity over the ensuing years. Active nodule formation usually persists for the first three decades of life and then becomes quiescent.

A physical exam will usually reveal a severe form of acne. The nodular lesions are tender and dome-shaped. When the nodules have broken down, there will be the presence of discharge that is foul-smelling pus. After the pus has drained, crusting of the lesion is common, followed by the formation of large irregular scars.

A classic feature of the disorder is the presence of paired or aggregates of blackheads on the trunk, neck, upper arms, and buttocks.

Evaluation

The diagnosis is made clinically, and the discharge should be cultured. Appropriate antibiotics should be started in the presence of putrid discharge. One should not wait for culture results before starting antibiotic therapy.

Treatment / Management

The treatment of choice for acne conglobata is with the use of retinoids like isotretinoin for 20 to 28 weeks or in some cases even longer. Some experts even recommend the use of oral prednisone (1 mg/kg/d) for 14 to 28 days. Steroids have been shown to be effective when there are systemic constitutional symptoms such as general malaise, fever, weight loss, and anorexia. Topical retinoids are not as effective as oral retinoids. It is important not to administer retinoids to women of childbearing age in the absence of effective contraception as these drugs are known to be teratogenic.[8][9]

  • Other options include the use of minocycline, tetracycline, or doxycycline. The tetracyclines should not be combined with oral isotretinoin because there is a real potential to induce pseudotumor cerebri.

  • In cases that are unresponsive to the above antibiotics, dapsone is an option. There are also case reports of treatment of acne conglobata with infliximab and carbon dioxide laser with or without isotretinoin.

  • In severe cases of acne conglobata that do not respond to the above treatments, another option is external beam radiation.

Surgery

  • Once the lesions have healed, dermal fillers can be used to improve the scars. Recently the FDA approved the use of the bovine collagen filler, Bellafill, for the treatment of acne scarring.

  • When nodules are large and fluctuant, they can be aspirated. Sometimes providers may use cryotherapy or intralesional triamcinolone. The large nodules can also be excised surgically.

Differential Diagnosis

The differential diagnoses of acne conglobata include but are not limited to the following:

Prognosis

Acne conglobata can produce marked disfigurement. Severe scarring causes psychological impairment; individuals with acne conglobata often feel excluded. Acne conglobata has also been responsible for anxiety and depression in many patients.

Complications

The majority of patients with acne conglobata are shy and embarrassed about their skin condition. Many give a history of being withdrawn and isolated. Suicidal ideations are also common in this population. Thus, it is vital to offer them some mental health counseling.

Deterrence and Patient Education

Once the diagnosis of acne conglobata is made, these individuals should receive psychological counseling because of depression and anxiety resulting from body disfigurement. The tendency is for these patients to hide the body disfigurement and skin lesions with garments which often leads to excess warmth and humidity, which worsens the skin condition. Hence, patients are educated on skin hygiene and counseling is recommended.

Patients with acne usually present to their primary care provider or nurse practitioner. But these healthcare workers should be aware that there are types of acne that are very serious and need an appropriate consult with a dermatologist. Acne conglobata is best managed by a dermatologist as it requires more aggressive therapy with close follow-up.
