Summer is the season of holidays, fruits, beaches and… colds. Although autumn-winter is considered the traditional season for acute respiratory diseases (ARI), we also often catch colds in the summer. I ate my favorite ice cream, plunged into a pleasant cool river, drank cold water, sat under the air conditioner – and please, temperature, runny nose, cough … It would seem that it is a common thing – we all had acute respiratory infections more than once. However, in people with diabetes, a common cold can lead to serious consequences. What are these consequences and how to avoid them – we will tell in this article.
Dangerous acute respiratory disease
An infectious process triggers a cascade of reactions that promote, among other things, the active synthesis of a hormone that has a contra-insular effect: they block the work of insulin at the cellular level and inhibit its formation in the pancreas. At the same time, their total effect significantly exceeds the effect of insulin [1].
Accordingly, the level of sugar in the blood rises. And severe hyperglycemia, in turn, adversely affects the pancreas, further suppressing insulin production. As a result, hyperglycemia increases, which can lead to the accumulation of excess amounts of ketones, in extreme cases, to ketoacidosis and even ketoacidotic coma [2]. At the same time, it is not uncommon for a deterioration in the condition of a person with DM due to the development of these complications to be taken as the phenomena of viral intoxication, and an immediate threat to life is diagnosed late.
Another danger lurks, oddly enough, in the treatment of acute respiratory infections. Most of the existing antiviral drugs only work against the influenza virus, which makes it difficult to prescribe them. The drugs themselves are quite aggressive, have a long list of contraindications and side effects. Thus, their administration should be avoided in cases of kidney and liver disorders, which often accompany diabetes [1].

“Diabetic kidney disease” – medical news.

Scientists have discovered a new method to slow the progression of diabetic kidney disease (KDD), which affects 40% of diabetic patients. The study, led by experts from the University of Bristol, is published in JCI Insight and on the medical portal Medical Xpress.
Recent clinical trials have shown that the widely used antihypertensive and diuretic drug known as spironolactone is an effective treatment for DKD by reducing urinary protein. However, despite the positive effect, the drug can also have side effects, including hyperkalemia, which is why doctors are reluctant to use it.
Researchers at Bristol Medical School collaborated with international colleagues to understand how spironolactone prevents kidney damage.
They found that the drug has a protective effect, helping to maintain a gel-like layer of glycocalyx on the surface of blood vessels inside the kidney. The use of new alternative therapeutics to slow the degradation of the glycocalyx layer in diabetes may now help them recreate the same effect, but without the side effects.
To conduct this study, the team used kidney biopsies from patients to develop a new method to measure changes in the depth of the glycocalyx and to confirm that diabetes damages the endothelial layer of the glycocalyx and that spironolactone prevents this damage. The results showed that spironolactone reduces the activity of a group of enzymes called matrix metalloproteases, helping to maintain a gel-like layer of glycocalyx on the surface of blood vessels in the kidney, preventing disease progression.
“Our next steps will be to explore the repurposing of drugs that target matrix metalloproteases [MMPs] to see if they can benefit patients with kidney disease and avoid the side effects associated with mineralocorticoid receptor blockers. If we see the same level of protection with these more specific drugs, then patients will experience significant benefits while avoiding the risks associated with high blood potassium,” said one of the co-authors of the study.
