Because the diet is so high-fat and low-carb, having trouble in the bathroom in the form of “keto diarrhea” isn’t at all uncommon.

“Constipation is often the biggest problem with the keto diet due to the lack of fiber, but in fact, artificial sweeteners and sugar alcohols, as well as a very high fat diet, can also cause diarrhea,” says Ginger Hultin, RD, a spokesperson for the Academy of Nutrition and Dietetics and owner of Champagne Nutrition.

Keto diarrhea is exactly what it sounds like: diarrhea that happens because of the switch to the keto diet. “It could be anything from noticing looser or more frequent bowel movements to full-on diarrhea, which is generally defined as having three or more loose, liquid bowel movements in a day,” Hultin says.

But what is it about the keto diet that can cause such a crappy situation, and is there anything that can be done about it? Here’s everything you need to know about keto diarrhea.

What causes keto diarrhea?

Some people, not all, experience keto diarrhea as an unpleasant side effect when switching to the keto diet. There are a few possible reasons for keto diarrhea, Hultin says:

  1. Fat can be hard to digest. In general, fat takes a bit longer for the body to digest. On a high-fat diet like keto, you’re all of a sudden eating lots more avocado, nuts, seeds, meat, olive oil, and other fats than normal—which can disrupt digestion. “When you switch to this high fat of a diet, it can cause diarrhea,” says Hultin.
  2. Artificial sweeteners and sugar alcohols can be rough on the stomach. Additionally, like Hultin said earlier, people often eat higher amounts of sugar alcohols and other artificial sweeteners while on the keto diet. These sweeteners, while low-carb and generally considered safe, can potentially cause bloating and diarrhea when consumed in excess. So if someone is eating lots of keto-friendly products high in sugar alcohols, they might struggle with keto diarrhea.
  3. Keto may disrupt the gut microbiome. The keto diet can also impact the composition of your gut microbiome, which has implications for digestion, too. “There have been some small studies that have found that after three months on the ketogenic diet, the bacteria in the gut change quite a bit, and that could cause diarrhea in some people,” Hultin says. If your body is not used to eating more fats, this could alter the gut bacteria, causing diarrhea and looser stools from poor digestion. (More research needs to be done before this can be considered a conclusive issue.)

Thankfully, like keto breath, keto diarrhea is not a permanent issue. “I would expect it to occur around the time that you’re changing your diet the most so in the beginning, probably the first one to four weeks until the body is able to adjust,” says Hultin.

  1. Add more fiber to your diet. “Soluble fiber is the best thing to help calm diarrhea because it absorbs water,” says Hultin. “These options are limited on a ketogenic diet but you could include: citrus, berries, avocado, broccoli, and chia seeds for example,” says Hultin.
  2. Drink more water and electrolytes. You can lose a lot of fluids if you have diarrhea—which is why the Mayo Clinic recommends consuming lots of liquids (water for hydration, and soups and fruit juice for electrolytes and sodium) to replenish your body.

At what point should I be concerned about this side effect?

However, if you have a stubborn case of keto diarrhea, you should consider seeing your doctor. “If you have diarrhea, especially multiple times per day, for over two to three days and if you’re experiencing any symptoms of dehydration—dark colored urine, dry skin, headache, feeling dizzy or light-headed, etc.—then you should see a doctor right away,” says Hultin. You might need more serious interventions to prevent serious dehydration and to rule out other potential causes of the diarrhea.

If keto diarrhea persists, it’s worth rethinking the eating plan, says Hultin. “If the body is reacting to the high fat nature of the diet, that probably can’t be changed because there’s a certain ratio that must be met,” she says. No one eating plan is right for everyone, and your body might just not be cut out for high amounts of fat. That extra fat that the body doesn’t use comes out in your stools, making them, uh, more fluid than usual. (To be clear: Boules doesn’t advocate or encourage the keto diet because of that high fat content. “We have no idea what effect a diet that high in fats has on long-term health,” she says.)

Another keto diarrhea culprit: Using non-nutritive sweeteners (think: aspartame or Splenda) or sugar alcohols (like sorbitol or xylitol) to replace real sugar on keto might also cause digestive troubles. “Sorbitol in particular is notorious for causing GI upset,” she says, like cramping and bloating, in addition to diarrhea.

Wait…diarrhea isn’t a symptom of keto flu?

Not necessarily. While, yes, diarrhea is a symptom of the keto flu, you’ll also have other symptoms like headaches, difficulty focusing, nausea, and trouble sleeping.

But, while the keto flu typically resolves in two weeks—after your body gets used to being in ketosis (a.k.a. the state of burning fat for fuel instead of carbs)—keto diarrhea doesn’t necessarily have a stopping point. Especially if your body has trouble processing fat.

“Any time you radically change what you’re eating, your GI system may react. This can happen when starting probiotics, or a high-fiber diet as well,” Boules says.

Don’t be surprised if the high-fat diet also makes your poop float. While it could be harmless, floaters might point at malabsorption and/or too much gas. If it lasts for more than two weeks, then you should see your doc (or consider revising your dietary plan).

How can I stop keto diarrhea?

If you’re committed to giving keto a go and you initially experience diarrhea, “aim to stay hydrated and increase fiber,” says Boules, as that can help your body adjust to ketosis, bulk up your poop, and help reduce bathroom trips.

You can increase your fiber intake by adding more veggies to your plate, like leafy greens and broccoli. If that’s not your jam, you can add a fiber supplement to your diet.

Keep in mind, though, that taking an anti-diarrheal medicine like Immodium is basically a bandaid—it can put a pause on your bowels, but it won’t fix the root problem (i.e., the diet choices leading to diarrhea in the first place).

If your diarrhea persists after the keto flu period, however, it might be time to start looking at your food choices—like cutting back on the artificial sweeteners and focusing on healthy fats (like avocado and olive oil) instead of (still keto) but less-healthy ones (like various cheeses and bacon).

But you might also have to accept that keto just isn’t your thing: “If the diarrhea continues, this food philosophy probably isn’t the best fit for you,” Boules says.

And, really, the most successful diet is one that you’ll feel comfortable sticking to—which likely won’t make you run to the bathroom every five minutes.

The keto diet is a high fat diet. In fact, it is recommended that 70% to 80% of your daily calorie intake is from fats. (1)

The high fat intake is the most straightforward reason why you’re experiencing diarrhea. When you consume fat, your body produces bile, a substance that is responsible for breaking down fats. (2)

Bile is good, and your body needs it to digest the fats properly you’re consuming. However, it also acts as a lubricant for your colon.

Diet Changes and Digestion

Any time you make a change to your eating habits, you risk disrupting your digestive system’s homeostasis — or balance. The ketogenic diet is an extreme low-carb diet that cuts carbs to a maximum of 50 grams daily, but often much less than that. The keto diet keeps protein intake at a moderate level — about 35 percent, according to an article published online in ​StatPearls​ in January 2019. This is at the high end of the range of 10 to 35 percent of calories recommended for the general population by the National Academy of Medicine. But, unless your protein intake was previously very low, this amount isn’t likely to cause problems.

But once you banish these carbs from your diet, your fiber intake takes a big hit, too, according to Niket Sonpal, MD, a New York-based internist and gastroenterologist.

Fiber is the nutrient responsible for keeping your poops consistent, and in order to maintain regularity, you should aim to eat about 25 grams of fiber per day, according to the FDA. But when you cut carbs, you also cut a lot of key fiber sources, like whole grains, fruits and legumes.

So, if you adopt a low-carb, high-fat/protein diet, don’t be surprised if you see a reduced number of bowel movements each week, Dr. Sonpal says. You can combat these effects by eating more high-fiber low-carb foods, like avocados (about 10 grams of fiber per cup) or raspberries (about 8 grams per cup).

When to worry:​ If you fall below three bowel movements per week, you’ll want to consult your doctor, advises Dr. Sonpal. Especially if your movements were more frequent before you began the diet.

On the complete opposite end of the spectrum, some people might experience bouts of diarrhea after starting a low-carb, high-fat diet, Dr. Sonpal says. “This happens when the body doesn’t digest all the fat that it is consuming, leading to an upset stomach,” he explains.

Chronic diarrhea may also be a sign of steatorrhea, which is when you have too much fat in your stool, according to the International Foundation for Gastrointestinal Disorders. Statorrhea is a sign of malabsorption, which happens when your body doesn’t break down or absorb the foods and nutrients you consume. Over time, this can lead to nutrient deficiency or dehydration.

Steatorrhea can be caused by a variety of factors, according to an October 2019 study published in ​StatPearls​. But if you only began experiencing symptoms after dramatically shifting your diet, it’s probably the reason you’re experiencing diarrhea.

When to worry:​ Typically, keto diarrhea is temporary and can be avoided by introducing the diet slowly. But if these symptoms persist longer than a few days, it’s probably best to consult your doctor and revert back to your usual eating habits.

3. Keto and Yellow (or Smelly) Poop

Yellow or particularly smelly poop is another side effect you may experience after taking on a low-carb, high-fat, high-protein diet. As with symptoms of diarrhea, your stool can become yellow in color or foul in odor due to excess fat, according to the Mayo Clinic.

While yellow or foul-smelling poop isn’t dangerous per se, it does indicate that your intestines aren’t absorbing enough nutrients or your liver isn’t producing enough bile to break down the fat in your diet, according to the University of Rochester Medical Center.

While the keto diet can help you lose weight, it may not be the best diet for your body if you’re experiencing problematic digestion. “It may be helpful in losing stubborn weight, but it may be dangerous if you have a chronic condition like IBS,” Dr. Sonpal says.
