This medication killed my mother. She was one that believed in her doctor and did not so much research on prescribed medications. Her doctor also did not do much research about adverse effects with her other medications. 2 weeks after starting this medication she had every side effect. 2 weeks later she was dead. She went in only because 1 finger was giving her problems with rheumatoid arthritis. This medication cost my mother her life.

Was diagnosed with Polyarticular Juvenile Idiopathic Arthritis at the age of 16. Within about 8 months the slight lower back aches and an ache in left ankle had gotton worse, spread to my other ankle, my knees, jaw, wrists, and all fingers. Doc confirmed diagnoses with MRI of the lower spine to find years worth of arthritis damage in the SI joints and immediately prescribed Enbrel. For about 3 months it worked perfectly and I was able to exercise again until it started to wear off before the next injection and eventually stopped working altogether. Blood test revealed I had developed antibodies to the Enbrel, making it useless as a treatment. It was worth it during the time it did work, some moderate side effects but nothing intolerable like head or stomach ache. Still worth it. Bummer that my body had to reject the medication like that. I am wondering about others thoughts on the length of time needed for enbrel to start working. I have been on Enbrel for 15 weeks (almost 4 months now) to treat my RA. I am only 28 years old, and was previously on Plaquenil and Prednisone. Neither of which worked for me. I am still having a lot of pain in the joints in my hands and feet , as well as my jaw. About 8 weeks ago, I started having sever back/spine pain. And xray showed that the vertebrae in my neck and rather close together, indicating arthritis. My rheumotologist has said, on more than one occasion that back pain is NOT common in people in RA which I DO NOT believe. Is anyone else with RA experienceing this kind of stabbing/burning/numb pain in their back/neck and spine? On Enbrel for 6 years years. Topicals did almost nothing for it. The joint pain was miserable. I went on methotrexate for about a year, which helped get rid of the pain and some of the plaque psoriasis. I have been on Enbrel for about 6 years now and generally the joint pain is gone. I saw the posts on the potentials and hope the cancers and other bad things don’t surface. I don’t know what to take that will work but plan to talk to my doctor and see if he has other offers. The cost is extremely high and once the ACA prevented people on Medicare from getting drug company assistance the cost was full retail ( $41k ) and way beyond my ability to pay. At the start Enbrel did a nice job of reducing plaque and, as I cannot take NSAIDs, controlling the arthritis. After a year or so its effectiveness gradually reduced to where I discussed a change with my dermatologist. The weekly shots were another negative though the auto inject system was pretty good. I switched to Cosentyx which, after 15 months, continues to be effective. Sure like the monthly injections, even if there are two of them.

I have been taking Enbrel a little over 12 years and have loved it! When I was diagnosed with rheumatoid arthritis at age 19, I could hardly eat because of jaw pain and could barely walk without crutches. Since I have been on Enbrel,I am able to do anything I want with no pain at all. Every once in awhile I’ll get a stiff knee but it’s very rare and normally only if it’s really humid. The past 2 months I have developed Urticaria. My rheumatologist is concerned the Enbrel has triggered a reaction and has instructed me not to take it anymore. I am concerned because the reason we started the injections was because nothing else seemed to work for me. The injections are quick and easy and are no worse than any other injection. I love my Enbrel and don’t want to give it up. All blood work has been very normal with no problems, I am very active and so far very healthy. My husband was diagnosed 12years ago with ankylosis spondylitis and prescribed Ebrel. It worked great! 15 months ago it suddenly stopped working, or so we thought. Dr changed his melds but did not do any scans or X-rays. Wish he had. 12 months ago his collar bone broke spontaneously. He was using the mouse on his PC. At the emergency room they found shadows in his lungs. CT-Scan showed stage 4 metastasized lung cancer. 4 months ago he died at the age of 50. The rheumatologist never said this could happen. I had normal WBC counts before I was put on Enril for 3 months. My WBC dropped to 2.3, and I noticed my hair falling out during that time. My doctor immediately took me off the Enbril, and after being off a month my WBC count is now up to 4.2 and my hair has stopped falling out. I was previously on Humira with no known side effects, and hope to go back on it one my count goes higher.

I was diagnosed with lupus/severe arthritis after the birth of my daughter 21 years ago. I took methotrexate, plaquenil, and pred for three issues with terrible side effects scaling back to pred to have my second daughter. 6 mos after my second daughter was born, dr called telling me a about this trial drug. I immediately went on it because I couldn’t bear the thought of mehotrexate. It’s been 15 yrs! I made a decision that day for quality of life – I have my life back and I’ve raised two children doing all the crazy activities you do with kids! I’ve never had the side effects and I would fight my dr if she tried to take me off it! I started this drug at 38 and I’m 53 now. This drug gave me my life back and if I eventually develop a side effect, I still would not regret the decision I made. I’ve lived almost pain free for many years and once you lived with the severe pain of RA, you may think quality over quantity. My baby girl has JIA junior inflammatory arthritis. She was diagnosed at 6 month’s old now four years old and been on Enbrel for 2 and a half years along with methotrexate. We have no choice but to give it to her. With out it she’s paralyzed.I’m so afraid that what we’re giving her is gonna eventually take her life. Reading all these reviews breaks my heart. I was one of the original users of this medication. I developed RA at 18, although I wasn’t diagnosed until 22 and was already in the “moderate to severe” category in every mobile joint in my body other than my spine. I tried everything on the market, and at best was only “surviving.” I also have Type I Diabetes, so I can’t take prednisone for flares (instantly develop high sugars that do not respond to correction). After I started taking a combination of methotrexate and Enbrel, I started having “okay” days. Then most days were okay, with a few “good” days. Then mostly “good” with some “what RA?” days. After faithfully taking this for 5+ years, I had to have some x-rays taken, and the radiologist noted that there was little to no evidence of RA in many of my joints! My only complaint is that the sure-click thingy injects way too fast and it hurts like hell for about 12 hours, in addition to an itchy lump under the skin. I’ve only been on Enbrel for 2 months, but I feel like there has been some improvement along with the methotrexate. I was a bit leary about doing the sureclick the first time, but like someone else stated, it’s a piece of cake! I would’ve never been able to do the syringe thing because of the immobility in my hands and fingers. Thanks Enbrel!

I just started this drug about 2 weeks ago.. I thought i would have faster progress.. but I’m finding out different. This medication is to expensive to not work. But i’m hoping by my first month is over of my weekly shot .. that it will bwe working.. I’ve been on Embrel for two years. Now my copay is over $3,000. Who can afford that. My insurance has paid over $30,000 this year for my prescription. Now Amgen says I no longer have any benefits remaining on my “copay card”. Amgen is a giant company who doesn’t care about patients only profits. How could they be allowed to profit from insurance then tell me my benefits are exhausted. Sounds like a intentional effort to extort patients who need ongoing medication. Enbrel has work extremely well for over 10 years now. Reduces and controls severe plaque psoriasis but doesn’t eliminate. Works extremely well for psoriatic arthritis. Completely eliminates pain and swelling in all joints. Takes effect in as little as 24 hours if I haven’t had a does in a couple of weeks. SureClick devices are virtually foolproof.

After my group insurance provider began covering the drug, I used it for a year and it did help my RA immensely. However; the cost has increased to prices that range from $3,500 to $6,000 per month and insurance companies are not covering it or the co-pay is around $1,000 per month. This is especially true with Medicare Part D coverage. Unfortunately, it is not financially feasible to continue this medication.

Before Enbrel I tried four other medicines that did not work. Since starting Enbrel 6 months ago my condition has greatly improved. I have noticed a marked improvement in pain and swelling is now way less frequent. I am very happy with Enbrel.

I have used it for three weeks have seen no improvement yet. Injection site get a big rash and hurts going in. Get dizzy for a couple of days after but am hopefully well get results in a couple of months.

I have ankylosing spondylitis which is a stiffening of the spine and also a rheumatoid arthritis. I have been taking enbrel for about 9 years now and it has been a miracle for me. No problems at all. I now get out of bed with no pain. And no more stomach problems which I got with the pills.

I have AS. Enbrel gave me my life back. I have been diagnosed after being in pain for years. I don’t know if it’s because I am a girl or that I was showing enough wear in my xrays but when I was finally diagnosed and given the medication I had immediate relief. Within the first 2 days I was able to roll over in bed. Do you know what its like to get a good nights sleep after being tired for so long? I do and it’s because of Enbrel and my Rhueumatologist. I can tell when I am getting to the end of the week because I start getting sore but when I take the shot I’m all fixed up again. My reaction to the 3rd injection was a huge melon size area of redness, swelling, heat & pain. Even my Rheumy had never seen an injection site that bad. I may have had an allergic reaction to a property in the injection. I started taking OTC Benadryl. Thank goodness I had a Lidocaine (Lidoderm) patch which saved me from scratching it raw. OTC Hydrocortisone cream didn’t help at all. I secured the patch with scotch packing tape around my leg on top & bottom. It kept the patch from falling off during the night & I was able to sleep. The swelling continued to the back of my leg the next day but it wasn’t as red & painful. The swelling went down in a week. The discoloring of the area eventually faded after 3 weeks. I am sorry that I reacted to the injection. My 1st injection hurt a lot even when the thigh area was iced prior. My 2nd injection had a small amount of swelling & redness at the site about the size of a baseball which is what the studies reported so I didn’t sweat it. I was feeling much, much better and I was doing more than I have in many years with a lot less pain! I wish I could somehow continue with Enbrel. I also seemed to retain more water using Enbrel. Had been on methotrexate for 6 months but had to come off it following a severe allergic reaction. Have now been on Enbrel for 15 months, and it is working very well. It took 3/4 months to kick in so recommend patience! Well worth the 8 seconds of “ouch” each week to have my life back.

Contraindications

With caution:
Demyelinating diseases, chronic heart failure (CHF), immunodeficiency conditions, diseases predisposing to the development or activation of infections (diabetes mellitus, hepatitis), moderate and severe alcoholic hepatitis, hepatitis C, blood dyscrasia, nervous diseases (multiple sclerosis, optic neuritis, transverse myelitis).
Pregnancy and lactation:
The use of Enbrel® is contraindicated during pregnancy and in women planning pregnancy, as there is no experience of using this drug in pregnant women.
Etanercept crosses the placenta.
Newborns whose mothers received etanercept during pregnancy have been found to have etanercept in plasma. The clinical significance of this is unknown, but these children may be more susceptible to infections. It is not recommended to give live vaccines to newborns within 16 weeks after their mothers received their last dose of etanercept.
Women of childbearing potential should use a reliable method of contraception during Enbrel® therapy and for three weeks after stopping it. Etanercept is excreted in breast milk after subcutaneous injection. Since immunoglobulins, like many other drugs, can be excreted in human milk, either stop breastfeeding or stop taking Enbrel® during breastfeeding, taking into account the importance of therapy for the mother and the risk to the child.
Composition
1 ml solution contains:
active ingredient: etanercept 50.0 mg;
excipients: arginine hydrochloride 5.3 mg, sodium chloride 5.8 mg, sucrose 10.0 mg, sodium hydrogen phosphate dihydrate 1.2 mg, sodium dihydrogen phosphate dihydrate 2.9 mg, water for interventions up to 1.0 ml.
Pharmacodynamics
Tumor necrosis factor (TNF-α, TNF) is the main cytokine that supports the inflammatory process in rheumatoid arthritis. An increase in the concentration of TNF was also found in synovial membranes and psoriatic plaques in patients with psoriatic arthritis, as well as in the blood plasma and synovial tissues of patients with ankylosing spondylitis. Etanercept is a competitive inhibitor of the binding of TNF to its receptors on the cell surface and thus inhibits the biological activity of TNF. TNF and lymphotoxin are pro-inflammatory cytokines that bind to two distinct tumor necrosis factor receptors (TNFORs) on the cell surface: 55-kilodalton (p55) and 75-kilodalton (p75). Both FNORs are present in the body in membrane-bound and free forms. Soluble FNOR regulate the biological activity of TNF.
TNF and lymphotoxin exist predominantly as homotrimers, their biological activity depends on the cross-linking of FNOR located on the cell surface. Dimeric soluble receptors, such as etanercept, have a greater affinity for TNF than monomeric receptors and are therefore much more potent competitive inhibitors of TNF binding to their cellular receptors. In addition, the use of the Fc fragment of immunoglobulin as a binding element in the structure of the dimeric receptor lengthens the half-life from blood serum.
A significant part of the pathological changes in the joints in rheumatoid arthritis and ankylosing spondylitis, as well as skin changes in the form of psoriatic plaques, occur due to exposure to pro-inflammatory molecules involved in the TNF-controlled system. The mechanism of action of etanercept appears to be competitive inhibition of TNF binding to TNF receptors on the cell surface. Thus, etanercept prevents TNF-mediated cellular response by promoting biological inactivation of TNF. Etanercept may also modulate biological responses controlled by additional downstream signaling molecules (eg, cytokines, adhesion molecules, or proteinases). And these responses can either stimulate or control TNF.
In patients with active psoriatic arthritis, Enbrel® inhibits the progression of structural damage, improves physical activity, and reduces the likelihood of peripheral joint damage.
Exacerbation of the disease is possible after stopping therapy with Enbrel® within a month. The effectiveness of re-administration of the drug within 24 months after discontinuation of therapy is comparable to that for patients who did not interrupt treatment.
Pharmacological action
Etanercept is slowly absorbed from the subcutaneous injection site, reaching peak concentrations approximately 48 hours after a single dose of Enbrel®. Absolute bioavailability is 76%. Dosing Enbrel® twice a week achieves twice the steady-state concentrations than those observed after a single dose. After a single subcutaneous injection of 25 mg of Enbrel®, the mean maximum plasma concentration of etanercept was 1.65 ± 0.66 μg / ml, the area under the concentration-time curve (AUC) was 235 ± 96.6 mcg-hour/ml. No apparent saturation of clearance within the dose range was observed.
The time dependence of etanercept concentration is described by a bi-exponential curve. The mean value of the volume of distribution is 7. 6 liters, while the volume of distribution at steady state is 10.4 liters.
Etanercept is excreted slowly from the body. The half-life (T1 / 2) is about 70 hours. In patients with rheumatoid arthritis, the clearance is approximately 0.066 l / h, which is slightly lower than its value of 0.11 l / h in healthy volunteers. The pharmacokinetic characteristics of etanercept in patients with rheumatoid arthritis, ankylosing spondylitis and psoriasis are similar.
Enbrel® 50 mg single dose is bioequivalent to two 25 mg injections given almost simultaneously. Despite the fact that in patients and healthy volunteers after the introduction of labeled etanercept, the radioactive label is eliminated through the kidneys, in patients with acute renal or hepatic insufficiency, no increase in plasma concentrations of etanercept was observed. In patients with acute renal or hepatic insufficiency, an increase in the concentration of etanercept is not observed.
There are no apparent differences in the pharmacokinetics of etanercept between men and women.
Elderly patients
Clearance and apparent volumes of distribution of etanercept in patients 65 to 87 years of age are similar to those in patients younger than 65 years of age.
Children
Juvenile idiopathic polyarthritis
The serum concentration profile is similar to that of adult patients with rheumatoid arthritis. Modeling suggests that in older children (10-17 years) and adult patients, the concentration of etanercept in the blood serum is approximately the same, and in younger children it will be significantly lower.
Psoriasis
Steady-state serum concentrations of etanercept in children aged 4 to 17 years with psoriasis and children with juvenile idiopathic arthritis who received Enbrel®, respectively, at a dose of 0.8 mg/kg once a week (maximum dose 50 mg per week) and 0.4 mg/kg twice a week (maximum dose of 50 mg per week) for 48 and 12 weeks were similar (1.6-2.1 mcg/ml). The value of this indicator coincided with that in adult patients with psoriasis, who were administered Enbrel® at a dose of 25 mg 2 times a week.
