“Some natural and over-the-counter remedies can completely put some IBS patients in remission,” says Brenda Powell, MD, medical director at Lower Elwha Klallam Tribe Health Department in Port Angeles, Washington.

1. A Specialized Diet Might Be Enough

New guidelines from the American College of Gastroenterology (ACG) suggest trying a low-FODMAP diet, which cuts out foods that are difficult for the body to absorb. While the diet may seem daunting because it eliminates so many foods you may be used to, it revolves around basic healthy eating habits.

2. Psyllium Powder for Added Fiber

It’s based on the principle that qi flows through various pathways in the body called meridians.

A review published in September 2018 in Neurogastroenterology and Motility looked at various exercises spanning a wide range of intensity from tai chi to mountaineering. It found that each of the exercises helped reduce IBS symptoms and improved mood.

But is one exercise better than another when it comes to IBS pain relief?

“The best exercise is the one that you will do and stick with,” says Megan Riehl, PsyD, a gastrointestinal psychologist at Michigan Medicine in Ann Arbor, Michigan.

Here are five expert recommended activities proven to improve IBS symptoms.

One caveat: Always talk to your doctor before beginning a new exercise routine, especially if abdominal pain, IBS symptoms, or gastrointestinal (GI) distress have kept you from working out for an extended period.

1. Yoga Improves Digestion and Mood

While there are many types of yoga, some more aerobic than others, they all follow the basic principles of connecting the brain, body, and breath through a series of stationary poses and active movements.

A review published in December 2019 in Digestive Diseases and Sciences found that study participants who started a yoga practice improved their digestion, reduced their IBS symptoms, and felt stronger. They also reported less depression and anxiety.

“We believe yoga is a very effective treatment for IBS because the focus on the mind-body connection improves the messages traveling from the brain to the gut and the gut to the brain,” says Brian E. Lacy, MD, PhD, a gastroenterologist at the Mayo Clinic in Jacksonville, Florida.

If you’re just starting out, try classes for beginners, which are sometimes labeled “gentle,” or ones that focus on stress relief, until you’re ready for a more intense class.

2. Tai Chi Helps Connect the Body to the Breath

Like yoga, tai chi also involves connecting the brain, body, and breath through a series of slow, graceful movements coupled with deep breathing into the diaphragm. The review published in Neurogastroenterology and Motility looked at a previous study that tracked two groups of IBS patients for eight weeks — both groups took medication, but one group did tai chi while the other didn’t. The group that did tai chi for eight weeks had fewer and less severe symptoms than the control group.

Tai chi is effective, because it helps reduce stress.

“Physical and emotional stress doesn’t help any of us,” says Dr. Riehl. “It raises our cortisol levels and that can impact our digestive system. People with IBS have trouble down-regulating digestive stress. So, checking in with your breath, slowing your breathing down and bringing the breath into the diaphragm kick-starts the parasympathetic system, calms everything down, and gives a nice massage to the digestive organs that can reduce spasms and urgency.”

A study published in May 2020 in PLoS One found that the more steps people with IBS took per day, the less severe their symptoms were. And in a study published in February 2018 in the Journal of Bodywork and Movement Therapies, participants who did six weeks of exercise on a treadmill for just 30 minutes three times a week saw significant improvement in their IBS symptoms and their mood.

“Taking a walk aids digestion,” says Dr. Lacy. “It can help relieve constipation and it can help move gasses to alleviate abdominal pain. Plus, being outside in the fresh air and moving is good for your mental health. And, that’s good for your GI health.”

Riehl notes that swimming rarely, if ever, leads to cramping or urgent diarrhea for people with IBS.

Moderation Matters When It Comes to Exercise

Stories of cyclists and marathon runners losing control of their bladder or defecating on themselves are not uncommon. According to Dr. Spiegel, when you exercise your body endures a physical stress that does not prioritize the gut, which decreases blood flow to the abdomen.

“Pushing it to an extreme can lead to urgent diarrhea from something called localized ischemia in the colon,” says Lacy. “It underscores the need to exercise in moderation rather than push yourself to an extreme level.”

Just 30 minutes on a treadmill three times a week was enough to reduce IBS symptoms in the Journal of Bodywork and Movement Therapies study.

Irritable bowel syndrome – Diagnosis and treatment

Diagnosis

There’s no test to definitively diagnose IBS. Your doctor is likely to start with a complete medical history, physical exam and tests to rule out other conditions, such as celiac disease.

After other conditions have been ruled out, your doctor is likely to use one of these sets of diagnostic criteria for IBS:

  • Rome criteria. These criteria include abdominal pain and discomfort lasting on average at least one day a week in the last three months, associated with at least two of these factors: Pain and discomfort are related to defecation, the frequency of defecation is altered, or stool consistency is altered.
  • Type of IBS. For the purpose of treatment, IBS can be divided into three types, based on your symptoms: constipation-predominant, diarrhea-predominant or mixed.

Your doctor will also likely assess whether you have other signs or symptoms that might suggest another, more serious, condition. These signs and symptoms include:

  • Onset of signs and symptoms after age 50

Your doctor might recommend endoscopy if celiac disease is suspected.

Laboratory tests can include:

  • Lactose intolerance tests. Lactase is an enzyme you need to digest the sugar found in dairy products. If you don’t produce lactose, you may have problems similar to those caused by IBS, including abdominal pain, gas and diarrhea. Your doctor may order a breath test or ask you to remove milk and milk products from your diet for several weeks.
  • Breath test for bacterial overgrowth. A breath test also can determine if you have bacterial overgrowth in your small intestine. Bacterial overgrowth is more common among people who have had bowel surgery or who have diabetes or some other disease that slows down digestion.
  • Stool tests. Your stool might be examined for bacteria or parasites, or a digestive liquid produced in your liver (bile acid), if you have chronic diarrhea.

Treatment

Treatment of IBS focuses on relieving symptoms so that you can live as normally as possible.

Mild signs and symptoms can often be controlled by managing stress and by making changes in your diet and lifestyle. Try to:

  • Avoid foods that trigger your symptoms
  • Eat high-fiber foods
  • Drink plenty of fluids
  • Exercise regularly
  • Get enough sleep

Your doctor might suggest that you eliminate from your diet:

  • High-gas foods. If you experience bloating or gas, you might avoid items such as carbonated and alcoholic beverages and certain foods that may lead to increased gas.
  • Gluten. Research shows that some people with IBS report improvement in diarrhea symptoms if they stop eating gluten (wheat, barley and rye) even if they don’t have celiac disease.
  • FODMAPs. Some people are sensitive to certain carbohydrates such as fructose, fructans, lactose and others, known as FODMAPs — fermentable oligosaccharides, disaccharides, monosaccharides and polyols. FODMAPs are found in certain grains, vegetables, fruits and dairy products.

A dietitian can help you with these diet changes.

If your problems are moderate or severe, your doctor might suggest counseling — especially if you have depression or if stress tends to worsen your symptoms.

In addition, based on your symptoms your doctor might suggest medications such as:

  • Fiber supplements. Taking a supplement such as psyllium (Metamucil) with fluids may help control constipation.
  • Laxatives. If fiber doesn’t help constipation, your doctor may recommend over-the-counter laxatives, such as magnesium hydroxide oral (Phillips’ Milk of Magnesia) or polyethylene glycol (Miralax).
  • Anti-diarrheal medications. Over-the-counter medications, such as loperamide (Imodium A-D), can help control diarrhea. Your doctor might also prescribe a bile acid binder, such as cholestyramine (Prevalite), colestipol (Colestid) or colesevelam (Welchol). Bile acid binders can cause bloating.
  • Anticholinergic medications. Medications such as dicyclomine (Bentyl) can help relieve painful bowel spasms. They are sometimes prescribed for people who have bouts of diarrhea. These medications are generally safe but can cause constipation, dry mouth and blurred vision.
  • Tricyclic antidepressants. This type of medication can help relieve depression as well as inhibit the activity of neurons that control the intestines to help reduce pain. If you have diarrhea and abdominal pain without depression, your doctor may suggest a lower than normal dose of imipramine (Tofranil), desipramine (Norpramin) or nortriptyline (Pamelor). Side effects — which might be reduced if you take the medication at bedtime — can include drowsiness, blurred vision, dizziness and dry mouth.
  • SSRI antidepressants. Selective serotonin reuptake inhibitor (SSRI) antidepressants, such as fluoxetine (Prozac, Sarafem) or paroxetine (Paxil), may help if you are depressed and have pain and constipation.
  • Pain medications. Pregabalin (Lyrica) or gabapentin (Neurontin) might ease severe pain or bloating.

Medications specifically for IBS

Medications approved for certain people with IBS include:

  • Alosetron (Lotronex). Alosetron is designed to relax the colon and slow the movement of waste through the lower bowel. Alosetron can be prescribed only by doctors enrolled in a special program, is intended for severe cases of diarrhea-predominant IBS in women who haven’t responded to other treatments, and isn’t approved for use by men. It has been linked to rare but important side effects, so it should only be considered when other treatments aren’t successful.
  • Eluxadoline (Viberzi). Eluxadoline can ease diarrhea by reducing muscle contractions and fluid secretion in the intestine, and increasing muscle tone in the rectum. Side effects can include nausea, abdominal pain and mild constipation. Eluxadoline has also been associated with pancreatitis, which can be serious and more common in certain individuals.
  • Rifaximin (Xifaxan). This antibiotic can decrease bacterial overgrowth and diarrhea.
  • Lubiprostone (Amitiza). Lubiprostone can increase fluid secretion in your small intestine to help with the passage of stool. It’s approved for women who have IBS with constipation, and is generally prescribed only for women with severe symptoms that haven’t responded to other treatments.
  • Linaclotide (Linzess). Linaclotide also can increase fluid secretion in your small intestine to help you pass stool. Linaclotide can cause diarrhea, but taking the medication 30 to 60 minutes before eating might help.

Potential future treatments

Researchers are investigating new treatments for IBS, such as fecal microbiota transplantation (FMT). Considered investigational at this time, FMT restores healthy intestinal bacteria by placing another person’s processed stool into the colon of a person affected by IBS. Clinical trials to study fecal transplants are currently underway.

Lifestyle and home remedies

Simple changes in your diet and lifestyle often provide relief from IBS. Your body will need time to respond to these changes. Try to:

  • Experiment with fiber. Fiber helps reduce constipation but also can worsen gas and cramping. Try slowly increasing the amount of fiber in your diet over a period of weeks with foods such as whole grains, fruits, vegetables and beans. A fiber supplement might cause less gas and bloating than fiber-rich foods.
  • Avoid problem foods. Eliminate foods that trigger your symptoms.
  • Eat at regular times. Don’t skip meals, and try to eat at about the same time each day to help regulate bowel function. If you have diarrhea, you may find that eating small, frequent meals makes you feel better. But if you’re constipated, eating larger amounts of high-fiber foods may help move food through your intestines.
  • Exercise regularly. Exercise helps relieve depression and stress, stimulates normal contractions of your intestines, and can help you feel better about yourself. Ask your doctor about an exercise program.

Alternative medicine

The role of alternative therapies in relieving IBS symptoms is unclear. Ask your doctor before starting any of these treatments. Alternative therapies include:

  • Hypnosis. A trained professional teaches you how to enter a relaxed state and then guides you in relaxing your abdominal muscles. Hypnosis may reduce abdominal pain and bloating. Several studies support the long-term effectiveness of hypnosis for IBS.
  • Peppermint. Studies show that, in people who have IBS with diarrhea, a specially coated tablet that slowly releases peppermint oil in the small intestine (enteric-coated peppermint oil) eases bloating, urgency, abdominal pain and pain while passing stool.
  • Probiotics. Probiotics are “good” bacteria that normally live in your intestines and are found in certain foods, such as yogurt, and in dietary supplements. Recent studies suggest that certain probiotics may relieve IBS symptoms, such as abdominal pain, bloating and diarrhea.
  • Stress reduction. Yoga or meditation can help relieve stress. You can take classes or practice at home using books or videos.

Preparing for your appointment

You may be referred to a doctor who specializes in the digestive system (gastroenterologist).

What you can do

  • Be aware of any pre-appointment restrictions, such as restricting your diet before your appointment.
  • Write down your symptoms, including any that may seem unrelated to the reason why you scheduled the appointment.
  • Write down any triggers to your symptoms, such as specific foods.
  • Make a list of all your medications, vitamins and supplements.
  • Write down your key medical information, including other conditions.
  • Write down key personal information, including any recent changes or stressors in your life.
  • Write down questions to ask your doctor.
  • Ask a relative or friend to accompany you, to help you remember what the doctor says.

Questions to ask your doctor

  • What’s the most likely cause of my symptoms?
  • What tests do I need? Is there any special preparation for them?
  • What treatment approach do you recommend? Are there any side effects associated with these treatments?
  • Should I change my diet?

What to expect from your doctor

Your doctor is likely to ask you a number of questions. Being ready to answer them may leave time to go over points you want to spend more time on. You may be asked:

  • What are your symptoms, and when did they begin?
  • How severe are your symptoms? Are they continuous or occasional?
  • Does anything seem to trigger your symptoms, such as foods, stress or — in women — your menstrual period?
  • Have you lost weight without trying?

How to Heal IBS Naturally

If you haven’t realized by now, I’m pretty passionate about gut health. After dealing with my own gut issues for years, I’ve done a lot of work to heal them naturally and I’m at a good place today and feel amazing almost everyday!

Irritable bowel syndrome or IBS is an intestinal disorder that can cause abdominal cramps, irregular bowel movements, gas and bloating in your small intestine and large intestine. People with IBS know how debilitating and embarrassing it can be. But, sadly, the exact cause isn’t really known.

Your nervous system is on high alert because the nerves in your gut and digestive tract are more sensitive, and some things like gas can trigger extreme pain in those with IBS, while someone without IBS wouldn’t be bothered by it.

My last few years in college, plus a few after college, I was dealing with a lot of IBS symptoms, while I was never formally diagnosed, it’s was obvious that was what I was dealing with.

I’d tried probiotics, which helped a bit, but wasn’t living the healthiest lifestyle. Going out a lot, drinking multiple nights a week, eating foods that I knew would upset my stomach and not getting enough rest or exercise. When I look back at that phase of my life, I don’t know how I did it. I was literally running on fumes and pushing my body past its limits on a regular basis.

Thankfully, I finally had a wake up call that really made me think twice about how I was living, so I started making some major lifestyle and diet changes.

Today, I never get those awful IBS symptoms anymore. It didn’t go away overnight, but I’m proud that I was able to heal my gut, which in turn reversed my IBS for good!

At the end of the day, our gut health is one of the most important things in keeping us healthy. The majority of our immune system lives in our gut, and the gut is also known as the second brain, so if you’re not taking care of it, it’s time you start prioritizing it! The best thing about healing my gut was that I had all these positive side effects. All of my other weird symptoms—brain fog, chronic fatigue, breakouts, poor sleep, anxiety and more literally faded away.

So, let’s talk about how to heal IBS and boost your gut health with natural remedies!

How to Heal IBS Naturally

While I’m not against Western medicine, I have learned first hand that a pill will not cure you of IBS for good. It may mask your symptoms and make you feel better, but if you don’t fix the real issue at hand—poor gut health—those symptoms will always come back.

Before we jump in, this is not medical advice and if you are having extreme symptoms, please see a doctor!

Probiotics

Probiotics are so buzzy these days, but they can do wonders for improving your gut health. They add good bacteria to your gut flora and help create balance. If you have bad bacterial overgrowth, it can throw your system out of whack and a probiotic can help you find balance again.

If you’re having a lot of symptoms, I highly recommend you start with a round of this probiotic. It has 60 billion CFU (colony forming units) and packs a punch. If your gut is in distress, this will do wonders!

After a round of this, I recommend this one that’s not quite as potent, but will keep things balanced moving forward. It has more CFUs than your average drugstore probiotic.

Prebiotics

While probiotics will definitely help ease your discomfort, they’re not a magic pill. The thing that really changed the game for me was a prebiotic. Prebiotic are a fiber supplement that feeds the good bacteria and probiotics and allow them to be more productive. A prebiotic will help establish better gut flora that lasts!

I put a scoop of this prebiotic powder in my smoothie every morning. It has completely changed my gut health in the best way possible. Do note that if you don’t currently eat much fiber, start with a little and work your way up!

Synbiotics

I’ve also heard really great things about synbiotics, which are basically a prebiotic and probiotic in one! I haven’t tried the brand Seed myself, but I’m considering it! People seem to love it.

Eat More Fiber

This past year, I’ve adopted a mostly plant-based diet. No rules or restrictions, I just aim to eat at least 2 plant-based meals a day. The biggest reason for it? When I eat a high fiber diet, I feel SO much better. Making one of my meals a big green smoothie, and another one a big salad with greens, chickpeas, seeds, veggies and herbs not only keeps me full, but seriously makes me feel freaking amazing!

Think about your own diet and how much fiber you’re getting in on an average day, could you use more? Most Americans don’t get enough, so make sure to add some fiber! For me, the easiest way to do this is a smoothie every morning—load it up with flax, spinach, cauliflower and chia seeds—this gut-healing smoothie one is my favorite.

Stay Hydrated

Extra fiber is amazing for your gut, but if you’re not drinking enough water, it’s not going to feel good. Make sure to drink plenty of fluids throughout the day. My hack for getting in enough water is to drink one huge 32oz. bottle first thing in the morning. If I get that in, I’m more thirsty throughout the day and it forces me to remember to drink more!

Move your Body

If you’re feeling bloated and crampy, the last thing you want to do is go workout, but do it anyway. Nothing crazy, just a walk will do! Daily movement always boosts my energy and digestion, and good digestion is key to healing your IBS.

Get Enough Sleep

Our bodies heal while we rest, so if you’re not getting adequate sleep on a regular basis, you’re never going to feel your best. Aim to go to bed and wake up at generally the same time every night. Don’t look at your phone around bedtime, dim the lights and make sure your room is dark and cool, all these things help. And, if you’re still struggling, a magnesium supplement, dropper full of CBD oil and 1mg of melatonin always do the trick for me!

Cut out Inflammatory Foods

For me, a big one was dairy and red meat. I also admit that when I was at the peak of my worst IBS symptoms, it was in college and I now realize that drinking (a lot of) alcohol was only making things worse. I also quit caffeine and that made a big difference in my energy levels, focus and gut health.

Everyone will be different in what they can tolerate, but dairy, red meat and gluten are always good places to start. You can also look into food allergies or sensitivities as those may trigger abdominal pain.

Use Essential Oils

In clinical trials, peppermint oil has been shown to help reduce symptoms and ease discomfort. This won’t cure you of IBS, but may help with pain and cramping and ease your symptoms while you work to fix the problem at the root.

The goal of IBS treatment is to provide relief from your symptoms. Your exact course of treatment will depend on the type and severity of your symptoms.

The success of the treatment often depends on having a good understanding of what IBS is and how it is treated. Fortunately, there are dietary, pharmacologic and behavioral approaches that can help, and they should be individualized to you. So ask your doctor lots of questions and help your doctor get to know what is important to you. Patients with better relationships with their medical provider often report that they have better symptom control.

Many patients worry about their symptoms and what will happen to them in the future. IBS is troubling and uncomfortable, but the condition itself does not increase your risk of any future health difficulties.

Treatment of IBS and associated symptoms may include:

  • Dietary changes
  • Medications
  • Psychotherapy
  • Alternative therapies

Dietary Changes

Bloating, abdominal pain and diarrhea may respond to dietary modification. For example, caffeine or fatty foods stimulate colonic contractions, so someone with IBS and diarrhea might have improvement by reducing their daily intake of caffeinated beverages and rich foods.

Those who suffer with bloating and abdominal discomfort might benefit from eating less food containing carbohydrates that are not digested well in the small intestine. Eating foods that contain large quantities of lactose, fructose and sorbitol, for example, may increase intestinal gas production, triggering more bloating, diarrhea and/or abdominal discomfort in those with IBS.

Many healthy foods, including some fruits and vegetables, contain these types of fermentable carbohydrates, in addition to fiber, another source of intestinal gas production. Finding the right balance of healthy foods can be a challenge if you have IBS. Ask your doctor if you might benefit from a trial of the low FODMAPs (fermentable oligosaccharides, disaccharides, monosaccharides and polyols) diet that has been shown in research studies to help reduce gassiness, abdominal pain and bloating in patients with IBS.

Medications

There are several medications that can be used to treat IBS.

  • Smooth muscle relaxants: These are best for relieving or preventing intestinal cramping.
  • Antidiarrheal medications: Medications for those with diarrhea slow intestinal transit and reduce the frequency of bowel movements while improving the consistency of the stool.
  • Laxatives: For patients who have constipation as the predominant symptom.
  • Antibiotics: To attempt to alter the composition of the gut flora that might be responsible for the fermentation of poorly digested carbohydrates.
  • Low-dose antidepressants: If pain and diarrhea are your predominant symptoms, you may find relief with these medications that work on the gut’s nervous system to make it less reactive to foods you eat or to emotional stress.

Psychotherapy

There is a strong connection between the nervous system and colonic function. Stress plays an important role in the frequency and severity of symptoms in IBS patients. A history of stressful life events or a current stressful situation can often precede IBS. Some patients who lost loved ones report the onset of symptoms shortly after the loss. Others with a history of depression notice that when the depression returns their symptoms worsen. Sometimes anxiety or depression occur with the onset of IBS symptoms.

If emotional stress is a trigger for your symptoms, there are several psychological interventions that might be used. The most studied is cognitive behavioral therapy, which has been shown to be effective for IBS. This type of therapy is provided by a trained mental health professional. Hypnotherapy has also been shown to help manage IBS symptoms.

Alternative Therapies

  • Certain probiotics have been shown to be helpful in managing some symptoms of IBS.
  • Acupuncture may be helpful in managing anxiety, fibromyalgia, migraines and insomnia associated with IBS. Acupuncture also can have a direct gastrointestinal effect by altering GI motility and pain perception.
  • Therapeutic massage can help reduce anxiety and relieve stress.

Understanding Irritable Bowel Syndrome with Constipation (IBS-C)

What is Irritable Bowel Syndrome With Constipation (IBS-C)?

Irritable Bowel Syndrome (IBS) is a common digestive disorder affecting 7-21% of the general population. Symptoms include frequent abdominal pain, bloating or discomfort, and changes in the appearance or frequency of bowel movements. IBS-C is a type of IBS in which the abdominal discomfort or bloating happens with constipation. Generally, constipation is when stools don’t pass often enough (less than three times per week).  Having hard stools that are difficult to pass or the feeling of an incomplete bowel movement are signs of constipation.

What are the Symptoms of IBS-C?

The symptoms of IBS-C include abdominal pain and discomfort, along with changes in bowel function.  Bloating and/or gas also may happen. Changes in bowel function may include straining, infrequent stools, hard or lumpy stools, and/or a feeling that the bowel does not empty completely. Some people may feel as if there is a “blockage” preventing them from passing stools. They may need to press on a part of their body or change body position to help them complete their bowel movement. How often a person passes stool, or the way it appears, may be different when abdominal discomfort is happening. With IBS-C, abdominal discomfort often improves after a bowel movement. In most cases, symptoms are ongoing (chronic), but they may come and go.

What Causes IBS-C-?

The cause of IBS-C is not known. Some experts think that it relates to changes in how the intestines move and contract, or changes in how the gut senses pain. In some patients, IBS-C may happen after a past infection in the gut. It could also be related to changes in the messages between the brain and the intestines. There is evidence that bacteria which are normally found in the gut, or changes to the composition of those bacteria, play a role. In addition, researchers are looking into possible roles of genetics and/or changes in the immune system.

How Do Doctors Diagnose IBS-C?

The diagnosis is based on a thorough medical history and physical exam. Doctors use a tool called the Rome criteria, a list of specific symptoms and factors that can help determine if someone has IBS-C. The most important of these criteria include the presence of abdominal pain or discomfort and change in bowel habits. There are no lab tests or imaging studies that can confirm a diagnosis of IBS-C.

In certain situations, a limited amount of testing (such as blood work or imaging studies) may be needed to make sure some other condition is not present. There are also “alarm symptoms” that suggest that something other than IBS-C may causing a person’s symptoms. Examples of alarm symptoms include weight loss, bleeding, or if IBS-C symptoms start after age 50 in a person that did not frequently have these symptoms before.  

What Is the Treatment for IBS-C?

IBS-C is not life-threatening. It can, however, affect a person’s quality of life. There is no cure, so the   goal of treatment is to reduce symptoms as much as possible. Treatments may be focused on improving the uncomfortable symptoms of IBS-C – such as abdominal pain and bloating –  or on improving bowel function. Treatments include lifestyle modifications, dietary changes, psychosocial therapy, and medications.   

Lifestyle modifications  

Lifestyle modifications for IBS-C include reducing or avoiding alcohol and tobacco products, improving sleep habits and getting regular exercise.

Dietary therapy

Some people find that certain foods can “trigger” or set off symptoms of IBS-C. However, the specific foods that bring on symptoms can vary widely from person to person.  Increasing the amount of soluble fiber in one’s diet (for example, oats, psyllium, and flax) can be helpful. Cutting down on caffeine, soda and gas-producing foods is often recommended. Specific diets have been studied and have shown benefit for some patients. Examples include the “low FODMAP” diet (a diet that is low in certain fermentable sugars) and the gluten free diet (a diet that excludes certain proteins that are found in many grains).

It may be helpful to work with a dietitian to better understand possible food triggers. In addition, diets that exclude whole groups or types of food can be difficult to follow, and a dietitian can help make sure your modified diet is both nutritious and safe.

Psychological Therapies

Various therapies focus on treating the central nervous system and have helped patients with IBS-C. Some of these work on how the brain and mind interpret sensations, such as discomfort or bloating. Examples of psychological therapies include hypnotherapy, cognitive behavioral therapy, multicomponent psychological therapy and dynamic and/or psychotherapy. For some patients, psychological stressors can worsen IBS, such as a history of physical, mental or sexual abuse. Psychiatric conditions such as post-traumatic stress disorder (PTSD), anxiety and depression are sometimes found along with IBS-C, and treating these problems may also improve the IBS symptoms.

Treatments Targeted at Bacteria

Bacteria are naturally present in the intestines or “gut” of humans, and they play an important role in normal bowel health and function.  ”Probiotic” products are foods or pills that contain live bacteria that may  promote gut health. They are sometimes recommended with the goal of changing of the types of gut bacteria in the intestine. This can sometimes reduce abdominal discomfort, bloating and gas from IBS-C. Experts are not sure of the overall benefit of probiotics for people with IBS; the most beneficial types and amounts of probiotic foods or supplements also are not known.  

Antibiotics are another way to change the population of bacteria in the gut, but there is disagreement about this approach.  While antibiotics sometimes provide symptom relief of IBS symptoms, there are potential risks associated with frequent use.  If antibiotics are used too often, they can become less effective, and the risk for developing serious infections increases.

Herbal therapies

There is some evidence that peppermint oil or Chinese herbal formulas, such as a supplement called STW5, can improve IBS-C symptoms.

Medical therapy

Medications for abdominal discomfort:

Antispasmodics are a group of medicines thought to relax smooth muscle in the intestine. They are commonly used to treat IBS-C. They may have some benefit in reducing the symptoms of abdominal pain, discomfort and bloating. Certain medications designed to improve bowel function (see below) can also improve symptoms of abdominal discomfort.

Medications to improve bowel function:

Laxatives and stool softeners often are the first drugs used for patients with IBS-C to stimulate bowel movements. This is because of they are relatively safe, inexpensive and widely available.  Of these medicines, polyethylene glycol (found in some products sold over the counter) has the best evidence behind it, but other laxatives may be used. It is worth noting that certain non-digested sugars (such as lactulose) or sugar alcohol laxatives can increase bloating. Sorbitol is an example of a sugar alcohol and is found in many sugar-free gums and sweets.

Pro-secretory agents are a newer group of medications that moisten both the stool and the intestine so that bowel movements are easier and more frequent. These include the prescription medicines lubiprostone and linaclotide. Both medications have shown some effect in reducing abdominal discomfort and improving bowel function.

Antidepressants may reduce pain and other symptoms in some patients with IBS-C.  They can modify how the gut feels pain, improve mood, and change how the intestine works to move stool along.  Examples of these medications include low dose tricyclic antidepressants (TCA) and selective serotonin reuptake inhibitors (SSRI). However, in some patients, tricyclic antidepressants can actually cause constipation, so SSRI may be better suited for treatment of IBS-C.

When to See the Doctor for IBS-C

If you have chronic symptoms of constipation and discomfort, see your doctor for a diagnosis and treatment plan to help reduce and manage symptoms, improve bowel function and increase comfort. It can make your quality of life better.

90,000 Belarus: “My doctors told me this is the only chance.”

Yuri is 38 years old, he was diagnosed with a form of tuberculosis resistant to 1st and 2nd line drugs. After two years of treatment that did not work, he had no options, but in 2015, Doctors Without Borders began a new program in the country.

Yuri is 38 years old. He is the first patient to complete treatment under the MSF anti-tuberculosis program, which the organization implements in the Republic of Belarus in close cooperation with the country’s Ministry of Health.The program began in the summer of 2015, when revolutionary anti-TB drugs were brought into the country through the endTB project. Prior to that, Yuri, together with doctors, had been fighting the disease for two years: treating extensively drug-resistant tuberculosis is a long and difficult job. Since this form of the disease is resistant to both first and second line drugs.

Yuri and I are sitting on a bench on the bank of the river on the edge of a large children’s park in the center of Minsk and talking in the hubbub of children’s voices.Children rush past us on scooters, their mothers in light summer dresses hardly keep up with them. The sun is hot, but Yuri does not take off his thick jacket. Explains: “I am afraid that, due to my own stupidity, it will go through somewhere again …” He says, while undergoing treatment in the MSF program, he promised himself to treat his health with caution. “I thought it was over.”

Yuri learned that he was sick with tuberculosis in 2013.“For some reason, my appetite began to disappear, weakness. I began to lose weight, it was noticed from the outside. The temperature has risen high. I went to the clinic, I thought I had a cold. They made an X-ray and even a small hole was found on it. As soon as I did a “fluorography”, they immediately say: “Do you know what you have …?” I say: “How do I know ?!” “You have a suspicion …”, – talking about the disease, Yuri rarely calls her by name. He says, having learned about her, he was very frightened, for several months he was close to panic. First of all, he was not afraid for his life, he was afraid of the reaction of others.

“I thought it was over. That everyone will turn away. As they say, I will be a leper if someone finds out. I hid it for a long time, already when I was in the hospital. From relatives – no. On the contrary, I told them that this is the situation. To be on the safe side, too. You never know what … And he hid from the rest. ”

The first attempt at treating tuberculosis ended in failure for Yuri. He had a severe form of the disease, which is not sensitive to the drugs available at that time.But this became clear only after a year of grueling treatment.

“As they poured me, there are more than 20 pieces of , my eyes already darkened. Well, it’s bad from them, of course, very much. This is such a dose right away. And sick, and weakness. Such a state that God forbid. Constantly bad. It was not only with me there, everyone had it. In the morning they take their medicine, and nobody is visible until evening. Because everyone is on the bed. Someone immediately vomits, someone later, ”says Yuri.

“This is the only chance.”

Several times it seemed that the disease had receded, the tests came “clean”. But after a few months – bad again. As he almost lost hope, the doctors spoke about the imminent opening of the MSF program. Yuri immediately agreed to participate in it.

“My doctors told me that this is the only chance. I started such a process that the cavities were already enlarging. In general, it is getting worse and worse. There was drug resistance. They said that the old medicines will not help anymore, ”says Yuri.- “Everything literally in a moment – after a couple of days I was given port <, an implantable system for long-term intravenous infusions – approx. ed > and the treatment started. Imipinem, bedaquiline, a few more pills, that’s all. And my improvement went in a moment. I didn’t feel better, I had no appetite, I had nothing. But according to analyzes, X-rays – everyone was surprised! They say wow! It is fantastic! You have such a dynamic! Well, great, in general. And that’s it, literally all my crops were clean already in October.In general, everything is clean. Everyone was surprised – nurses, doctors. Naturally, they said, do not try to miss the treatment, because you need to fix it. ”

Dr. Mikhail Khmyz, MSF doctor, says: “We had our first patients, those who are undergoing palliative care. That is, they receive symptomatic treatment. And they no longer receive anti-tuberculosis treatment ”. This happens if the patient has repeatedly failed in treatment, and it is obvious to doctors that this strain is resistant to the drugs at their disposal.The last hope for such patients is new drugs against tuberculosis, which appeared in the world a few years ago for the first time in 50 years – bedaquiline and delamanid.

The Ministry of Health of Belarus also has at its disposal new anti-tuberculosis drugs since 2015, thanks to a grant from the Global Fund. But this is not enough to treat all the patients who need them, and doctors have to make difficult choices. One of the factors here is adherence to treatment in the previous stages, and one of the main risk factors in this aspect, according to the results of a study conducted by MSF in 2016, is alcohol abuse.

“The main barrier to treatment adherence in TB patients here is alcohol abuse. The Ministry of Health of Belarus is aware of this and has expressed its readiness to work with MSF to solve this problem. We are now discussing an approach that will enable an alcohol-dependent TB patient to achieve good treatment outcomes, ”said Dr. Parvati Nair, Medical Leader for MSF’s country project.

The goal of the program in Belarus, which MSF is implementing in close collaboration with the country’s Ministry of Health, is to improve the chances of patients with multidrug-resistant and extensively drug-resistant tuberculosis (MDR and XDR-TB), including those at risk, to cure and improve adherence treatment.The former is facilitated by the use of new regimens for the treatment of tuberculosis within the program, including the use of bedaquiline and delamanid. The second is a patient support service organized by Médecins Sans Frontières, whose counselors and social workers provide patients with not only psychological, but also social support.
