Recognizing hemorrhoids

Simply put, a hemorrhoid is a bulging and swollen vein in your anorectal region. Hemorrhoids are very similar to varicose veins, and you can have them anywhere from your lower rectum to the area just outside your anus. If you have internal hemorrhoids, they occur inside your body, while external hemorrhoids form underneath the skin of your anal opening.

Depending on the location of your hemorrhoids, you may experience excessive bleeding with mild discomfort, or they can become extremely painful. As with many medical conditions, seeking treatment early for your hemorrhoids can spare you needless discomfort and pain.

4 signs your hemorrhoids need medical attention

Here are four signs that Dr. Paonessa says indicate your hemorrhoids require medical intervention and why early treatment matters.

1. Bleeding when you have a bowel movement

There are many underlying causes of rectal bleeding, including hemorrhoids. If you have bleeding from internal hemorrhoids, it’s usually bright red, occurs with bowel movements, causes no pain, and ranges from mild to moderate in quantity.

But never assume rectal bleeding is due to hemorrhoids. If you’ve never been diagnosed with hemorrhoids — or you have noticed a change in the characteristics of the bleeding you’ve had with hemorrhoids in the past — see us for an accurate diagnosis.   

2. Noticing an anal bulge that retracts after passing stools

When you have internal hemorrhoids, they can prolapse (or push outside) your anal opening during a bowel movement. At first, the tissue may retract on its own, or you can easily push it back inside. But a prolapse can worsen over time and can eventually cause significant irritation, bleeding, itching, and pain as this internal tissue becomes trapped outside of your anus.

3. Feeling you can’t empty your stool completely

If you have prolapsed internal hemorrhoids, they can cause the sensation that you have stool stuck at your anus. You may also have difficulty cleaning yourself or notice a mucous discharge following a bowel movement because of the displaced tissue.

What are hemorrhoids?

Hemorrhoids are swollen veins, similar to varicose veins, found in your anus and lower rectum. To say they’re quite common is no understatement. Nearly three-quarters of all adults will have hemorrhoid symptoms at some point in their lives. They can develop on the outside of your anus — external hemorrhoids — or inside your rectum — known as internal hemorrhoids.

Your hemorrhoids can also become thrombosed. A thrombosed hemorrhoid is when blood gets trapped in the swollen vein and a blood clot forms. Thrombosed hemorrhoids are painful but not usually dangerous.


What are the common symptoms of hemorrhoids?

You may have hemorrhoids and not experience any symptoms, especially if they’re internal. However, some of the symptoms of hemorrhoids include:

  • Itching or irritated feeling around your anus
  • Painless bleeding during bowel movements
  • Pain or discomfort when sitting
  • A lump near your anus


External and thrombosed hemorrhoids are more likely to cause painful symptoms. You should make an appointment with Dr. Jaffke when your condition causes pain or rectal bleeding.


What causes hemorrhoids?

Hemorrhoids usually come from lifestyle factors including:

  • Chronic diarrhea or constipation
  • Straining to pass a stool
  • Sitting for extended periods of time
  • Obesity
  • Pregnancy
  • Low-fiber diet
  • Anal intercourse


The reason why some people develop hemorrhoid symptoms and others don’t isn’t apparent. However, you can reduce your risk by eating a high-fiber diet or taking a fiber supplement and drinking at least eight glasses of water every day.


How are hemorrhoids treated?

Dr. Jaffke offers a variety of in-office and outpatient surgical treatments to remove hemorrhoids. The treatments are available on an outpatient basis so you can go home to relax and recover on the same day.

Rubber band ligation

Dr. Jaffke places a rubber band around the base of your hemorrhoid. This procedure cuts off the blood supply to your hemorrhoid, causing it to wither and fall off in a week.

Hemorrhoids, or piles, are one of mankind’s most common and nagging disorders. By themselves, hemorrhoids are rarely serious, but they can be extremely troublesome. In some instances, they may mask a more serious disorder, such as colon or rectal cancer. Therefore, hemorrhoids require the proper diagnosis and treatment by a physician.

Hemorrhoids are dilated (enlarged) veins which occur in and around the anus and rectum. They may be external (outside the anus) or internal and slip to the outside. In both of these instances, the hemorrhoids can be felt and seen as lumps or knots. Hemorrhoids also may remain inside the rectum and so cannot be felt or seen. These are called internal hemorrhoids.

What Causes Hemorrhoids?

A common cause of hemorrhoids is simply the standing position, in which all the blood above the rectum exerts pressure on the rectal and anal areas. Other conditions which contribute to hemorrhoids are: poor bowel habits, constipation, protracted diarrhea, pregnancy, obesity, and especially frequent straining when having a bowel movement. However, some patients will have none of these conditions and still develop hemorrhoids.

What Are the Complications?

Hemorrhoids can produce several uncomfortable, but non-serious problems.

Thrombosis and pain — A blood clot in the hemorrhoid may cause severe pain and usually demands immediate medical attention.

Bleeding — Hemorrhoids can ooze fresh red blood, whether located externally or internally. External hemorrhoids often cause dripping of blood from the anus while sitting on the toilet. The blood might also be seen as soiling of the underwear. Internal hemorrhoids that bleed may produce fresh blood in the stool.

Itching and irritation — External hemorrhoids can be itchy, especially if the area is moist and irritated.

Do Hemorrhoids Cause Cancer?

Hemorrhoids do not develop into cancer. However, both hemorrhoids and cancer can cause rectal bleeding. In fact, many disorders can be the cause of rectal bleeding. When rectal bleeding occurs in persons over age 30, and especially in those over age 50, it should be considered a serious problem until an exact diagnosis is made. The physician who directly examines the rectal area can often make the specific diagnosis.


Treatment of hemorrhoids varies depending on where they are, what problems they are causing, and how serious they are. Often, time and the normal process of healing clear hemorrhoids with little or no specific treatment. When hemorrhoids require treatment, the following general measures are recommended:

Conservative Treatment

  • Keep the anal area clean, using a mild soap or baby wipes and gentle dabbing after a bowel movement. Avoid vigorous rubbing of the area.
  • Keep the anus and hemorrhoids as dry as possible, using talcum powder and a pad of soft tissue to absorb moisture.  Medicated pads containing witch hazel (Tucks) can also be used.
  • Eat a diet high in fiber (bran) and roughage. Fiber and bran retain water in the stool, producing soft, bulky stools which are easier to pass and reduce the tendency to develop hemorrhoids.
  • Bulking agents, such as Metamucil, Konsyl, Citrucel, and Fibercon, are available in drug stores. These also come in less expensive generic versions.
  • Avoid straining when having a bowel movement.
  • When thrombosis, pain, and tenderness occur, a 10 to 20 minute hot tub bath two to four times daily brings heat to the area, provides relief from the pain, and promotes healing. This is called a hot Sitz bath.


A common method of treating internal hemorrhoids is to use a small rubber band to tie off the base of the swollen vein. The blood circulation stops and the hemorrhoid then falls off. Repeat treatments are sometimes necessary.

Infrared Photocoagulation

An infrared light source can be used to coagulate internal hemorrhoidal veins. Often, more than one treatment is necessary.


A laser directs a high-intensity light beam at tissue and so produces heat, thereby coagulating or cauterizing the hemorrhoid. This technique is still somewhat new, but may have application in the future.


Surgery is sometimes recommended in treating hemorrhoids. It usually is reserved for:

  • Acute, painful clot (thrombosis) of hemorrhoids
  • Profusely or continuously bleeding hemorrhoids
  • Longstanding, irreversible, and large hemorrhoids
  • When other treatment is unsuccessful

The physician evaluates each patient’s case to determine the appropriate treatment.

To Prevent Hemorrhoids…

  • Eat plenty of fiber, bran, or roughage, or use a bulking agent to maintain regular, soft bowel movements.
  • Do not delay or try to prevent a bowel movement when the urge is present.
  • Exercise, especially aerobic, may help produce more regular bowel movements.
  • Drink plenty of liquids and eat regularly scheduled meals.
  • Keep the area around the anus clean and dry.


Hemorrhoids are an especially common disorder and often clear up by themselves or with minimal treatment. Treatment typically is simple and effective, although surgery occasionally is necessary. Hemorrhoids can mask a more serious disorder and, therefore, must be evaluated and diagnosed properly by a physician. People with hemorrhoids who work closely with their physician are usually assured a good outcome and relief from this common disorder.

Diagnosing Hemorrhoids

Treating Hemorrhoids

Simple diet and lifestyle changes often reduce the swelling of hemorrhoids and relieve hemorrhoid symptoms. Eating a high-fiber diet can make stools softer and easier to pass, reducing the pressure on hemorrhoids caused by straining.

Other changes that may help relieve hemorrhoid symptoms include:

  • Drinking six to eight 8-ounce glasses of water or other nonalcoholic fluids each day
  • Sitting in a tub of warm water for 10 minutes several times a day
  • Exercising to prevent constipation
  • Not straining during bowel movements

Over-the-counter creams and suppositories may temporarily relieve the pain and itching of hemorrhoids. These treatments should only be used for a short time because long-term use can damage the skin. If at-home treatments do not relieve symptoms, medical treatments may be needed.

What are hemorrhoids?

Hemorrhoids are swollen veins in your lower rectum and anus. Your rectum is the lowest portion of your digestive tract and holds your stools before they leave your body by way of your anus. Hemorrhoids can develop inside your rectum or underneath the skin surrounding your anus.

Common hemorrhoid symptoms include:

  • Rectal bleeding with bowel movements, especially small, bright red blood on your toilet paper
  • Swelling around your anus

What causes hemorrhoids?

How do you diagnose hemorrhoids?

Dr. Bazalgette can usually diagnose hemorrhoids during a physical examination. If you have external hemorrhoids, he can often see them during your exam. Additional tests might include a digital rectal exam, where Dr. Bazalgette checks your rectum for signs of abnormalities.

Some hemorrhoids can be difficult to detect during a physical exam, so Dr. Bazalgette could also use a proctoscope, anoscope, or sigmoidoscope to get a closer look at your rectum and lower areas of your colon.

In some cases, Dr. Bazalgette might also recommend a colonoscopy to evaluate your entire colon.

How do you treat hemorrhoids?

    Almost everyone is bothered at some time or another by hemorrhoids. Reports suggest that 75 percent of people will experience enlarged hemorrhoids at some point in their lives. The term refers to a condition that occurs when veins around the anus (external hemorrhoids) or just inside the anus (internal hemorrhoids) become distended and inflamed. They can cause a number of symptoms – usually just irritating, not dangerous, and treatable at home – but sometimes they can become infected or bleed heavily, prompting urgent intervention and even surgery.

    What are the symptoms of hemorrhoids?

    Depending on whether they are internal or external, these inflamed veins can cause itching, rectal bleeding, and pain that can sometimes be severe. The classic symptom of internal hemorrhoids is rectal bleeding. Most often it is usually noticed as bright red streaks on toilet paper, but blood can also be found on the stool, underwear, or in the toilet bowl. Internal hemorrhoids can sometimes bulge outside of the anus, rarely leading to the formation of a blood clot.

    External hemorrhoids are more frequently associated with pain, and if irritated from straining or excessive rubbing or cleaning, may also itch and bleed. Blood clots – also known as thrombosed hemorrhoids – are more commonly formed in external hemorrhoids. They can often feel like a hard, painful grapelike lump on the anus, and sometimes need to be removed surgically.

    What are the causes?

    The most common causes of hemorrhoids are prolonged sitting, straining at stool, and constipation. Irritants in the diet can exacerbate hemorrhoids, including strong spices such as red pepper and mustard, and drinks such as coffee, decaffeinated coffee, and alcohol. Stress can cause hemorrhoids, as can pregnancy (due to pressure of the fetus in the abdomen, as well as hormonal changes that cause blood vessels to enlarge – these hemorrhoids usually disappear after delivery).  Obesity, chronic diarrhea and anal intercourse can also lead to hemorrhoids.

    What is the conventional treatment of hemorrhoids?

    If natural home remedies for hemorrhoids (see below) don’t improve constipation, you can help lessen pressure and straining with bowel movements by taking an over-the-counter stool softener such as docusate sodium (Colace).

    An effective natural symptomatic hemorrhoids treatment often recommended in conventional settings is the sitz bath: sit in a bathtub with enough warm water to cover the anal area for fifteen minutes several times a day. Application of warm compresses for 10 minutes at a time throughout the day can also offer similar relief. Local heat increases blood flow to the area, promoting healing.

    Most hemorrhoids disappear on their own after a few days, some however, may become thrombosed and require surgical intervention to remove clotted blood; still others can persist becoming hard and painful. If the above home remedies for hemorrhoids are not effective, you may require more invasive hemorrhoids treatment options. One method is ligation, in which a rubber band is placed around the base of the hemorrhoid. The band cuts off blood circulation and, as a result, the hemorrhoid withers away within days. Another method is sclerotherapy – injection of a chemical solution around the blood vessel to shrink it. A third technique relies on infrared coagulation to destroy the hemorrhoid by burning. And lastly, a surgical procedure known as hemorrhoidectomy is occasionally recommended for extensive or severe hemorrhoids to completely remove them. All of these are last resorts for persistent or severe cases that do not respond to simpler methods.

    What therapies does Dr. Weil recommend for hemorrhoids?

    • Dietary changes: To prevent hemorrhoids as well as treat them, you should increase the amount of fiber in your diet in order to relieve constipation (and consequent straining, which increases pressure in the veins surrounding the anus). You can do this by eating plenty of fruits, vegetables and whole grains. If that’s not possible, take psyllium seed husks, in any of the forms available in drugstores and health-food stores. Drink lots of water, more than you think you need. Yet another healthy option is freshly ground flaxseed, a great source of both soluble and insoluble fiber in addition to omega-3 fatty acids and lignans. Flax helps promote bowel motility as well as softening stool. Try one to two tablespoons daily with food. Avoid dietary irritants as much as possible.
    • Exercise: Regular exercise promotes bowel function and health.
    • Supplements: Triphala, a combination of three tonifying herbs from the Ayurvedic tradition, is another excellent bowel regulator. You can buy it in capsules in health food stores. Follow the dosage and directions on the label. If you can tolerate it, try taking two to three ounces of liquid pure aloe vera juice – without additives – after meals.
    • Also: Apply aloe vera gel to the anal area frequently; it feels good and promotes healing. Avoid tobacco use in all forms. Instead of dry toilet paper, use compresses of witch hazel (which acts as an astringent) to clean the anal area after bowel movements. You can get medicated witch hazel pads and towlettes at most drugstores.

    Finally, many over-the-counter products are sold for symptomatic relief of hemorrhoids. Dr. Weil recommends avoiding those containing benzocaine or other topical anesthetics. Although it may contribute to the loss of marine species, Preparation H (which contains shark liver oil) is a safe and effective remedy.

    Hemorrhoids Treatment at University of Utah Health

    Hemorrhoid surgery is highly effective in addressing long-term issues and preventing further problems in the future. Most of our patients will return home the same day of surgery.

    We use surgery to treat problematic hemorrhoids that are:

    • extremely painful,
    • contain a blood clot, or
    • associated with a large amount of redundant (extra) tissue.

    You will be given medication during surgery so you will not feel pain. You will discuss your best option for pain management with your anesthesiologist on the day of your surgery.

    During the procedure, your surgeon will make small incisions (cuts) on the external tissue around the hemorrhoids. The enlarged vein inside the hemorrhoid will be tied to prevent bleeding, and then your surgeon will remove the hemorrhoid. Your surgeon may also use an advanced energy device (surgical tool) to remove the hemorrhoid and seal the blood vessel at the same time. Our care team will go over the procedure in detail and answer any questions you have.

    Your hemorrhoid removal may or may not require stitches. If you receive stitches, they will dissolve after surgery.

    Preparing for Hemorrhoid Surgery

    Minimal preparation is required for hemorrhoid surgery. We may only ask you to:

    • stop eating and drinking the night before the surgery and
    • discontinue certain medications, such as blood thinners.

    How Painful is Hemorrhoid Surgery?

    One to two weeks after your hemorrhoid surgery, you may experience increased discomfort, such as difficulty sitting or muscle spasms close to the surgical area. Pain medication and warm water soaks will help manage your symptoms.

    What to Expect After Hemorrhoid Surgery

    Your provider will give you detailed aftercare instructions, including:

    • how to clean the surgical site;
    • a medication schedule; and
    • pain management techniques (such as taking over-the-counter pain relievers, bathing in warm water, or using a warm compress).

    We may prescribe you stronger pain medication to use for up to five days following your surgery. You’ll be scheduled for a four-week follow-up visit with your provider when your surgical site has healed.

    Hemorrhoids in a certain sense can be called a unique disease. Due to its location in the anus, discussing this disease with other people, even with a doctor, is not easy for most people. As a result, there are a lot of rumors about hemorrhoids that have nothing to do with reality.

  5. Today we would like to discuss with you such a problem as hemorrhoids. There is evidence that 70–80% of people develop hemorrhoids during their lifetime, although no more than 10–15% seek medical help. How accurate are these numbers? What are the causes of hemorrhoids?
  6. In the United States, it is estimated that about 4-10% of the population suffers from some form of hemorrhoids. In Russia, sometimes there is evidence that 80% of our people suffer from hemorrhoids. Hearing such numbers, I keep repeating a standard joke: maybe it is “mental hemorrhoids”, “hemorrhoids of thoughtfulness and reflection”? It happens like this: a person comes in, and it is clear that he has “hemorrhoids” in life, and he does not have physical hemorrhoids, but he does have mental ones.So the data that 80% of the population suffers from hemorrhoids does not correspond to the true incidence. Although, of course, if we are talking about people over 50, then among them the number of people suffering from hemorrhoids is really high. By this age, hemorrhoidal complaints such as blood from the anus, itching near the anus, discomfort, prolapse of nodes, in Europe have about 30-40% of mature people.

    Let me remind you that in the distal part of the rectum (the upper part of the surgical anal canal), covered with the rectal mucosa and not having pain sensitivity, there are internal hemorrhoidal plexuses – internal hemorrhoids, and in its lower part, covered with skin richly innervated by pain receptors, are located external hemorrhoidal plexus – external hemorrhoids.Outer and inner nodes are separated by a “jagged” line. This is the confluence of the mucosa and anoderm. The height of the entire anal canal in women is about 2-3 cm, in men – up to 5 cm, therefore, with significant edema, thrombosis or a long history of the disease, external and internal merge (combined form), even surgeons find it difficult to differentiate groups of nodes.
