General Hospital Services Admission to the Hospital

If your hospitalization was scheduled in advance, you have already had preadmission screening. The information requested at that time and on the day of your admission will help us speed your admission and make your stay more comfortable. This information is also necessary for the hospital to maintain accurate records and to meet accreditation requirements.

Planning Your Arrival

For information regarding your surgical admission time, call the operating room at 707-393-4670. For your own protection, please notify the Admitting Department if you are being hospitalized due to a work related injury or occupational accident.

Photo Identification

You will be asked for photo identification the day of your admission. The parent or legal guardian of a minor will be asked for a photo identification.

Permit for Treatment

You will be asked to sign a Permit for Treatment during the pre-admission or admission procedure. The permit for a minor must be signed by a parent or legal guardian. The Permit for Treatment form acknowledges your responsibility for payment of non-covered expenses incurred while you are hospitalized, and relieves the hospital of responsibility for personal belongings or valuables which are kept at your bedside. If you are scheduled for surgery, your physician will ask you to sign an additional informed consent form after discussing the surgical procedure and alternative methods of treatment with you. During your hospitalization, you may be asked to sign other permits for treatment that involve special laboratory tests, X-rays, or other procedures.

At the time of your admission, you may specify which of the following information may be disclosed to the public: your admission to the hospital, your room number/location, your religious affiliation and/or condition.

Advance Healthcare Directive Kit

If you are unable to make health decisions yourself, you may choose someone to make them for you. This person, often a family member or friend, could decide what types of medical care or how much treatment you should receive.

If you do not have someone to make decisions for you, and you are without an Advance Directive, we will provide the best possible care for you.

You can print your own Advance Healthcare Directive or pick one up at Patient Relations, Admitting and the Health Education Center. If you have any questions about the forms or process, please speak to an attorney.

For patients having surgery, do not resuscitate (DNR) directives will be suspended during the perioperative period.

Surgical and anesthetic interventions may cause loss of cardiopulmonary function in some situations, and there are definitive means available in the surgical setting which can often allow physicians to promptly restore these functions, which may allow for full recovery of the patient to their prior state.

Copayment/Deposits

You will be asked for the applicable copayment/deposit payment on the day of admission if not already paid. You may pay by check, credit card, or cash. If you have any questions, call 707-393-4666.

Laboratory Tests and X-rays

Laboratory tests, X-rays or other procedures may be required before hospitalization. Your physician will provide you with appropriate orders, dates and instructions for such procedures. In some instances, these procedures may be performed on the day of admission before you go to your room.

Hospital admissions – NHS

Depending on your circumstances, you could be admitted as

  • an outpatient – you’ll go to hospital for an appointment, but not stay overnight
  • a day patient (day case) – you’ll be given a hospital bed for tests or surgery, but will not stay overnight; this can include treatments such as minor surgery, dialysis or chemotherapy
  • an inpatient – you’ll stay in hospital for 1 night or more for tests, medical treatment or surgery

What’s an admission letter?

Once your hospital appointment is booked, you’ll receive an admission letter, which provides you with details such as your hospital admission date or the ward you’re going to be on.

If you need to follow any specific instructions before your treatment, those are also included in the letter.

Contents of an admission letter:

  • the date and time of the appointment
  • contact details of the hospital department or ward in charge of your care
  • information about where you have to go on the day
  • the name of the consultant-led team in charge of your care
  • information about any tests you may need to have before your appointment
  • information about any samples (pee and poo) or medicines you may have to bring with you on the day
  • information about whether you can or cannot eat or drink before your hospital appointment, and how long for

Besides the items listed on your appointment letter, you may also want to bring the following with you:

  • a small amount of money in case you need to buy a drink or snack
  • information about any changes to your personal details, such as a new address or GP
  • proof of entitlement to free prescriptions, if appropriate
  • proof of entitlement to free travel or help with travel, if appropriate

Call the hospital in advance if you have any special needs or require a translator.

If you have any communication difficulties, you should be given information you can easily understand and any support you need to communicate effectively with the people caring for you.

Cancelling and rearranging appointments

If you’re unable to attend your appointment, tell the hospital in advance and they’ll try to arrange a new appointment.

Many appointments are wasted each year because patients do not turn up on the day.

If you do not come for your appointment, you’ll lose your referral and will have to ask your GP for a new appointment. This also means the waiting time clock will start again.

If you happen to fall ill in the weeks before your appointment, let the hospital know, especially in cases of diarrhoea and vomiting.

You may be asked not to come and be offered a new appointment. This is to help prevent and control the spread of infections in the hospital.

Pre-admission assessment

Sometimes you’ll be asked to attend a pre-admissions assessment (PAA). This may be an appointment with a nurse or doctor, or a telephone assessment.

You’ll be asked questions about your health, medical history and home circumstances.

During the PAA, you’ll be given advice about your admission, including where to report to.

You may be asked not to eat or drink before coming into hospital, as this may interfere with your test or operation.

You’ll also be given advice about when to take your normal medicines, if you have any.

You may be screened for MRSA and assessed for your risk of hospital-acquired blood clots.

During the PAA, the nurse or doctor can decide whether you’re suitable for a day procedure or whether you’ll need to stay in hospital to have your operation.

  • having a carer present with you at certain times
