There’s no quick fix, but your first step is to follow up with your doctor, who may recommend treatment for low testosterone. Aside from causing a drop in your sex drive and energy levels, low testosterone can increase your risk for heart disease, depression, type 2 diabetes, and osteoporosis.

How to Treat and Manage Low Testosterone: First Steps

If your low testosterone is caused by an underlying health condition, such as obesity, type 2 diabetes, or sleep apnea, treating that condition may help improve low testosterone, says Ahmer Farooq, DO, assistant professor of urology at Loyola University Medical Center in Maywood, Ill.

Many doctors also recommend an overall healthy lifestyle as a first approach. But even if you’re prescribed testosterone treatment, diet, exercise, stress management, and other healthy habits are important to keep symptoms of low testosterone at bay as well.

“Eat foods that are low in saturated fat and exercise regularly,” Dr. Farooq says.

A 2013 study published in The Journal of Sexual Medicine found that men who engaged in moderate-intensity aerobic exercise for more than 3½ hours each week had greater improvement in testosterone levels, sexual function, and losing weight and belly fat than men who got less than 2½ hours of physical activity a week.

Treatment Options for Low Testosterone

Treatment for low testosterone may involve hormone replacement therapy. “Essentially, the body is being given the testosterone it’s lost,” says Farooq.

For some men, testosterone replacement therapy can help improve sex drive, ease symptoms of depression, and increase energy levels. Treatment may also boost muscle mass and bone density.

Testosterone replacement therapy is available in several different forms, Dr. Farooq says. The most common form is a topical gel. About 70 percent of men on testosterone replacement therapy apply these gels daily.

“Gels are used in places where men are less likely to come into contact with women or children, such as the inner thigh, shoulder, or armpit,” Farooq says. And the gel should only be applied to areas of the body covered by clothing to avoid contact with others.

About 17 percent of men treated for low T receive testosterone injections every 7 to 22 days. Testosterone levels peak two to three days after the injection and then slowly drop until the next dose of the hormone is given.

There are also testosterone patches that men can use. These patches can be placed on areas of the body such as the back, stomach, upper arm, or thigh. About 10 percent of men with low testosterone use the patches. The skin absorbs the testosterone, which is slowly released into the bloodstream.

An uncommon option, used by no more than 3 percent of those men being treated for low T, is implanted testosterone pellets. Over the course of three to four months, the pellets slowly release testosterone into the body. The procedure must then be repeated.

Only rarely are oral medications used to treat low T because these medications have potentially harmful side effects to the liver.

When Will Treatment for Low Testosterone Start Working?

For men who are prescribed testosterone replacement therapy, it usually takes three to six months to work, Farooq says.

“Some men may see improvement sooner, but for most men it’s a gradual process,” Farooq says. “Don’t expect a miracle overnight.”

Research published in 2011 in the European Journal of Endocrinology, however, suggested that the effects of testosterone replacement therapy on men’s libido and symptoms of depression may begin after three weeks.

Treatment for low testosterone is an ongoing process and usually will continue long term. As with many other chronic conditions, if you stop the therapy, the problem returns — your testosterone levels will drop back down to below-normal levels.

Potential Risks of Low Testosterone Treatment

The number of men using testosterone replacement therapy in the United States has increased dramatically over the past several years. Several short-term studies have shown it to be an effective treatment in appropriately selected individuals, but the long-term effects remain unclear.

Like any other medication, testosterone replacement therapy carries certain risks. Although research has shown mixed results, it could possibly stimulate growth of existing prostate cancer. Men who’ve been diagnosed with breast cancer or prostate cancer aren’t candidates for testosterone replacement therapy, Farooq says.

Testosterone replacement could also contribute to sleep apnea. Farooq says that men should be screened for sleep apnea before receiving treatment for low testosterone. Treatment for low testosterone could also limit sperm production, cause your body to make too many red blood cells, or lead to acne or enlargement of the breasts.

“It’s important to look at a man’s low testosterone levels and weigh the risks and benefits of treatment,” Farooq says. He also says that bringing your spouse or partner with you to checkups can help if you feel apprehensive about discussing your concerns or symptoms with your doctor.

When to Schedule Follow-Ups With Your Doctor

“During the first year of treatment for low testosterone, men are usually seen every three months to check their blood and testosterone levels,” Farooq says. Among the tests you can expect are the prostate-specific antigen (PSA) screening test for prostate cancer and blood tests to measure lipids, liver function, and red blood cell count.

“If you’re healthy after one year, follow-up visits may be spaced out to every six months,” Farooq says. “It’s important to make sure you’re not developing any of the potential side effects of treatment.”

What It’s Like to Inject Yourself

Afraid of needles? Don’t worry, injections are not nearly as petrifying as they may seem at first. We’re not trying to sugarcoat this process, because injections are certainly a hurdle for many beginners on TRT. Even those who have been on TRT for years still have a little apprehension when it comes time to inject themselves. 

In reality, though, the gauge of the needle is very small when injecting testosterone; once you get past the initial puncturing of the skin, the needle will glide smoothly into your muscle tissue without much sensation at all. The best advice is to take deep breaths and be steady when inserting the needle.

Intuitively, you might just think that jabbing the needle in as fast as possible will bypass any sort of sensation you might feel, but that’s also not a very controllable approach. Use a firm grip on the barrel of the syringe, breath, and firmly penetrate your skin. The needle will smoothly glide into your muscle, at which point you will apply pressure to the plunger and dispense the testosterone.  (Your muscle might twitch slightly but it’s not a painful sensation for most people, more so a reflex.)

The entire injection process generally takes no more than 15 seconds. Always be sure to maintain a sterile environment. Wipe the injection spot with an alcohol wipe beforehand to disinfect the skin and do not reuse needles or syringes as this greatly increases the risk of infections.

When you inject testosterone, you’re putting a depot of oil into muscle tissue which is slowly dispersed into the bloodstream (for several days, or even weeks, depending on the ester). As a side effect of this, you might experience injection site soreness for a few days after the fact. Don’t worry, though; this is completely normal and subsides on its own. Massaging the muscle and applying a warm gel pack for 5-10 minutes can also help alleviate any soreness or stiffness at the injection site.

Unfortunately, there isn’t really a black and white answer to this question. Most physicians assert that patients will experience improvement within 4-6 weeks.

But, like most treatment procedures, Low Testosterone Treatment (TRT) is dependant on the individual.

Since this answer is a little ambiguous, we will discuss the signs of testosterone kicking in.

After TRT, patients typically experience similar responses from their bodies. When you start having these experiences, it’s likely your treatment is beginning to kick in.

How Long Does Testosterone Take to Work: Timeline

Improved Sex Drive and Better Quality Sleep

The first two signs to look for is an improved sex drive and the ability to sleep better at night.

The frequency of erection and ejaculation increases usually around the 3 week mark. However, sometimes it can occur a little sooner or later as well.

Around 12-21 weeks, men are expected to experience steady levels of frequency with ejaculation. Also, around the 3 week mark, men also experience a stronger libido and have more sexual fantasies.

Restlessness among men with low testosterone is common. It prohibits the brain from falling into a deep sleep cycle.

During the early stages of TRT, men typically feel less anxious and more relaxed. This allows them to fall into better sleep patterns.

Better Mood

After a consistent improvement in sex drive and sleep, a patient’s mood changes.

Low testosterone in men often causes them higher levels of depression than others. However, around the 6 week mark of TRT, patient’s mood increases significantly.

Men tend to continue feeling less anxious and unclear throughout this period. They are much less irritable and also experience greater levels of sociability.

After a year of TRT, men tend to experience the full benefits of treatment.

Patient’s mental state is much more balanced and their brains are able to think much clearer. They are much more positive, happier, and see an increase in their overall well-being.

Their sleep habits are fully restored. This also boosts morale and greater feelings of enjoyment.

Men’s sex lives reach a peak, and they continue to experience great levels of sexual intimacy. Not only does sexual desire increase, but their physical attractiveness continues to develop.

Men will see continued growth in their muscle strength and have much stronger bones.

Their lean body mass is much more developed than it was at the 3-month mark. This leads to a reduction in overall body fat.

Prostate levels do not continue to grow after its initial spike. They have leveled out at this point in treatment.

Blood pressure, cholesterol, and triglyceride levels are also at a healthy state. Lastly, patients will see increased red blood cell formation.

Important Reminders During Low Testosterone Treatment

Upon receiving treatment, you must remember that TRT is not an overnight process. You aren’t going to wake up the next morning with increased body mass and a high sex drive.

Moreso, it’s a gradual process that requires frequent care and attention. Instead of asking “How long does testosterone take to work?”, ask “How is my body processing the treatment?”.

“Because of the marketing, men have been flooded with information about the potential benefit of fixing low testosterone, but not with the potential costs,” says Dr. Carl Pallais, an endocrinologist and assistant professor of medicine at Harvard Medical School. “Men should be much more mindful of the possible long-term complications.”

If a man’s testosterone looks below the normal range, there is a good chance he could end up on TRT hormone supplements — often indefinitely. “There is a bit of a testosterone trap,” Dr. Pallais says. “Men get started on testosterone replacement and they feel better, but then it’s hard to come off of it. On treatment, the body stops making testosterone. Men can often feel a big difference when they stop therapy because their body’s testosterone production has not yet recovered.”

This wouldn’t matter so much if we were sure that long-term hormone therapy is safe, but some experts worry that low-T therapy is exposing men to small risks that could add up to harm over time.

What are the risks of testosterone therapy?

A relatively small number of men experience immediate side effects of testosterone treatment, such as acne, disturbed breathing while sleeping, breast swelling or tenderness, or swelling in the ankles. Doctors also watch out for high red blood cell counts, which could increase the risk of clotting.

Men on long-term using forms of testosterone therapy long term appear to have a higher risk of cardiovascular problems, like heart attacks, strokes, and deaths from heart disease. For example, in 2010, researchers halted the Testosterone in Older Men study when early results showed that men on testosterone replacement therapy had noticeably more heart problems. “In older men, theoretical cardiac side effects become a little more immediate,” Dr. Pallais says.

Some physicians also have a lingering concern that testosterone therapy could stimulate the growth of prostate cancer cells. As with the hypothetical cardiac risks, the evidence is mixed. But because prostate cancer is so common, doctors tend to be leery of prescribing testosterone to men who may be at risk.

For male patients with low blood testosterone levels, the benefits of hormone replacement therapy for men usually outweigh potential risks. However, for most other men it’s a shared decision with your doctor. It offers men who feel lousy a chance to feel better, but that quick fix could distract attention from unknown long-term hazards. “I can’t tell you for certain that this raises your personal risk of heart problems and prostate cancer, or that it doesn’t,” Dr. Pallais says.

So, keep risks in mind when considering testosterone therapy or other therapies . “I frequently discourage it, particularly if the man has borderline levels,” Dr. Pallais says.

  • Pace yourself: Spread out activities throughout the day.

  • Take a walk: It gives you a lift when you feel pooped out.

  • Snack smart: Have a snack with fiber and some protein between meals.

Take a cautious approach

A large, definitive trial for hormone treatment of men is still to come. Until then, here is how to take a cautious approach to testosterone therapy.

Take stock of your health first
  • Have you considered other reasons why you may be experiencing fatigue, low sex drive, and other symptoms attributable to low testosterone? For example, do you eat a balanced, nutritious diet? Do you exercise regularly? Do you sleep well? Address these factors before turning to hormone replacement therapy for men.

  • If your sex life is not what it used to be, have you ruled out relationship or psychological issues that could be contributing?

  • If erectile dysfunction has caused you to suspect “low T” as the culprit, consider that cardiovascular disease can also cause erectile dysfunction.

Get an accurate assessment
  • Inaccurate or misinterpreted test results can either falsely diagnose or miss a case of testosterone deficiency. Your testosterone level should be measured between 7 am and 10 am, when it’s at its peak. Confirm a low reading with a second test on a different day. It may require multiple measurements and careful interpretation to establish bioavailable testosterone, or the amount of the hormone that is able to have effects on the body. Consider getting a second opinion from an endocrinologist.

  • After starting therapy, follow-up with your physician periodically to have testosterone checks and other lab tests to make sure the therapy is not causing any problems with your prostate or blood chemistry.

Be mindful of unknown risks of testosterone replacement therapy
  • Approach testosterone therapy with caution if you are at high risk for prostate cancer; have severe urinary symptoms from prostate enlargement; or have diagnosed heart disease, a previous heart attack, or multiple risk factors for heart problems.

  • Ask your doctor to explain the various side effects for the different formulations of testosterone. The different treatments include testosterone injections, gels and patches. Know what to look for if something goes wrong.

Have realistic expectations

Testosterone therapy is not a fountain of youth. There is no proof that it will restore you to the level of physical fitness or sexual function of your youth, make you live longer, prevent heart disease or prostate cancer, or improve your memory or mental sharpness. Do not seek TRT therapy with these expectations in mind.

If erectile function has been a problem, testosterone therapy might not fix it. In fact, it might increase your sex drive but not allow you to act on it. You may also need medication or other therapy for difficulty getting or maintaining erections.

Often one of the most frequently asked questions for patients starting on testosterone replacement therapy (TRT) is ow long will it take before I feel the benefits?

How long does it take to experience testosterone benefits?

There is no definitive time frame that patients will start to experience the positive benefits of testosterone replacement therapy, but on average 4-6 weeks of starting the program is the most common answer from our patients and the industry as a whole (1,2).

We tracked 14 of our male patients who had hypogonadism (low T) through their initial six weeks of treatment to determine just how long it took them to report a symptomatic improvement in 4 of the above measures. Patients were asked to create a journal over the period of their treatment and in particular note at the end of each week, for 6 weeks, the reported change in 4 markers.

Please note: Percent improvement in the study is measured subjectively. Reported improvement is only accounted if there is at least a 50% improvement in symptom. This is not a controlled study and does not account for the placebo effect.

How long before TRT starts to work: What the science journals say

Scientific literature shows a considerable variation in the time TRT takes to show effects across a range of body functions.

A systematic study published in the European Journal of Endocrinology(2) revealed the following results:

Sex drive

Improved quality of life

Patients begin to feel positive testosterone benefits on quality of life within 3–4 weeks, but maximum benefits take longer.

Depressive moods begin to lift after 3–6 weeks with a maximum after 18–30 weeks. Effec erythropoiesis (the production of red blood cells)are evident at 3 months, peaking at 9–12 months.

Body mass and muscle strength

Effects on lipids (fats) appear after 4 weeks and reach a peak after 6–12 months. Posivite changes in fat mass, lean body mass, and muscle strength occur within 12–16 weeks, stabilize at 6–12 months, but can marginal tinue over years.

Effects on inflammation occur within 3–12 weeks and benefits to bone are detectable after 6 months while continuing at least for 3 years.

Insulin sensitivity may improve within few days, but effects on glycemic control are only obvious after 3–12 months, making it easier for your body to absorb sugars and increase your body’s optimum performance.

Regardless of the time to experience the positive benefits of TRT, the vast majority experience profound changes in their mental outlook, sense of vitality and physical make-up.

How Long For TRT to Work?

In this article we will address the question of how long for TRT to work.  Many of my patients ask similar questions about how long it is going to take before they see the benefits of testosterone injections or topical testosterone.  The answer really depends on a few different things.  I will say right out of the gate that initially it is going to take about three weeks before you notice the benefits.  However, you may not get the full benefits for almost three months.  In between that time there are a few different things that you want to make sure are optimized and a few different things that can accelerate the benefits.  That’s what most of this article is going to be about. 

So if this interests you keep reading, we are going to get into the details. 

How Long For TRT To Work

Many people starting testosterone replacement therapy wonder how long for their TRT will start to work.   On average I expect people to feel some improvements in about three weeks.   For some it can take longer up to three to six months.  In the case you start feeling better early on, you may not get the full benefits until several months down the road.  Firsts let’s look at  what symptom improvements  you are expecting to see on TRT or testosterone replacement therapy. 

 

Benefit Timeline of TRT

The main benefits I expect to see when someone has low testosterone and they start testosterone replacement therapy are  increased energy, drive, and mood.  Along with this you will also typically see improvements in erectile dysfunction and libido.  The benefit that most people notice first is with energy mood and drive.  How long it takes depends on:

  • If your dose is correct.
  • If your estrogen levels are not going up.
  • What’s happening with your androgen receptors. 

These three things are important but I also wanted to point out that most of us think a little bit is good, so more is even better.  That’s not always the case for testosterone replacement therapy.  The reason it is not in this case, is tied in with the downstream hormone metabolites of testosterone and other hormones.  If you’re on a high dose of testosterone and your levels are really high,  your enzymes will be saturated.  These are enzymes like alpha five reductase and aromatase.  The alpha reductase turns testosterone into dihydrotestosterone (DHT) and the aromatase makes estrogen or estradiol. The more testosterone in your system the more DHT and estradiol.  These can cause some side effects. 

 

The Role of Estradiol 

In the case of estradiol, it can diminish the positive effects of the testosterone.  So if you’re getting a lot of estrogen production with the increased testosterone, it may negate some of the benefits.  Sometimes you just decrease the dose testosterone and things even out.  Most people know that high estrogen negates some of the benefits of their testosterone replacement therapy.    What they sometimes forget is that, you have to check the levels at different times.  You have to make sure you check it mid week and several months into the treatment.  Once you get the estradiol optimized, you will see more of the benefits.  A high estradiol will delay the time it takes you to see benefits.  That doesn’t necessarily mean you have to discount the total time and start back from square one.  Once optimized it may take an additional 3 weeks after the estradiol is normalized. 

 

The Role of Your Dose

The other thing to look at when we’re trying to understand how long this might take for you to realize the full benefits, is your actual dose of testosterone.   The benefits of testosterone replacement therapy are all tied in with the androgenic and anabolic effects of testosterone.  One of the metabolites of testosterone is DHT which has stronger androgenic effects.  Testosterone replacement therapy can lead to higher levels of DHT.  If it gets too high, this can lead to more irritability and anxiety.   The outcome will really depend on how active your enzymes are.   Regardless of your enzymes, higher levels of testosterone will lead to higher DHT levels.  If you are taking it for mood reasons, pay attention to your mood in relation to your dose.  Generally it helps with mood and positive outlook but it can make things worse too. 

If your testosterone levels are still low after you check it six weeks later, your tissues may not be getting fully saturated with those androgens.  In this case, it will take longer than you expect to get to the full benefits.   Typically it’s going to take about two to threes months to get the full benefits if everything is optimized. 

Another thing that is important is the number of androgen receptors.  The main two hormones are testosterone  and dihydrotestosterone.   The dihydrotestosterone binds to the androgen receptor stronger than the regular testosterone.  As you’re saturating your tissues with more androgens from the testosterone replacement therapy that saturation has the effect on the cells and the tissues.   However, if you don’t have very many androgen receptors then you have less tissue saturation.  You can enhance the saturation and shorten the duration of time we expect to see benefits when you have more androgen receptors.   These receptors increase with more resistance exercise and to a lesser extent exercise in general.  The exercise likely creates more androgen receptors in all of your tissues, but it is specifically documented in muscles. 

The last thing i wanted to note is about cortisol levels.  Very high testosterone levels can cause high cortisol.  When your testosterone levels are low you may notice difficulty sleeping and waking up sluggish.  When your testosterone levels get too high the high cortisol can interfere with your sleep.  If you are having more issues with sleep when you start the testosterone that may be a problem.

If you have checked off all these boxes and you are not noticing the benefits of testosterone replacement therapy maybe there’s another problem going on.  That doesn’t necessarily mean you don’t need the testosterone at all.  If your levels started off  low it may be just a piece of the puzzle.  You may feel ten or thirty percent better but you’re still not quite where you think you should.  In this case, there’s probably another thing going on there you haven’t figured out yet.  If all the hormone levels are optimized, you know you have to look at other possibilities to what could be going wrong.

In 2020, the Turkish TRT media holding launched a project for post-Soviet countries – the Internet channel “TRT in Russian”.

In 2020, TRT decided to launch a Russian-language platform, aiming to “bring the voice of Turkey to the whole world”, using the image of Recep Tayyip Erdogan. At the same time, the creators promised that the channel would become “a source of objective and verified information for residents of Russian-speaking countries.”

The channel’s slogan is “We speak the same language”, but there is ambiguity in the news feed. If we are talking about Turkey, the channel mostly talks about the tourism sector, sights and cultural life of the country, and about 30% of publications are devoted to the military-industrial complex of Turkey.

But when it comes to Russia, an “objective” source of information tells about negative events: accidents, problems of citizens, crimes and disasters.

From such information it turns out that everything is not very good in Russia: people are dying, while the authorities do nothing to save their citizens.Other methods of influencing perception are also used, for example, the sound scale – when the talk on the channel comes to Russia, the background music becomes “an amateur”, and during the stories about Turkish military equipment, the music is replaced by a solemn one.

The channel works in contrast: women in labor are dying in Russia, while in Turkey, at this time, doctors are performing unique operations. Turkey plans to create a unique helicopter by 2023, while in Russia a CXT-40 rocket has punched a hole in a residential building in Saratov. There is a fire in a nursing home in Russia, and a Turkish-assembled fire engine entered the top ten in the world.

Examples can be given for a long time, but everyone understands that the presentation of news is rather one-sided. At the same time, the countries are not in a state of enmity, and the Foreign Ministries of Russia and Turkey report on the development of a close partnership.

Friendly relations with Ukraine is one of the main topics of the materials of “TRT in Russian”. The channel calls it a “strategic partner.” TRT editors reiterate that Turkish-Ukrainian relations have been strengthened after the “annexation of Crimea”. If the publications are talking about Ukraine, then not a word is said about the crisis.But on the Russian-language resource one by one there are such headlines: “ Ukrainian woman creates hats and ornaments from flowers” , “” Kiev artist creates Christmas tree decorations from cotton wool. ”

The face of the channel is the Ukrainian journalist Anton Ravitsky, along with Pavel Kazarin is known for his dubious rhetoric towards Russia. TRT is present on YouTube, Facebook, Twitter, Instagram and Telegram, and broadcasts in English, German and Arabic, and plans to launch in Russian in the spring.

Launching is obviously not cheap. The channel’s management has already agreed on financial support. There are rumors that Mikhail Khodorkovsky will be the sponsor of TRT in Russian. Previously, he has already supported opposition publications in the Russian Federation. The interests of the fugitive oligarch and the owners of the media holding coincided: he got a high-quality platform, and they – experts in the Russophobic information space.

Recently, the editors of TRT in Russian have referred to Russian opposition Internet resources. For example, in an interview with Novaya Gazeta, which, according to media reports, is sponsored by Khodorkovsky.

Any information about suspicion of someone in espionage by the Russian authorities is questioned, but if it comes to the same thing, only from Turkey, the information is given without any hesitation.

The topic of “spy mania” in Russia throughout 2020 was raised in the MBKh Media edition, founded by the same Khodorkovsky.

Recently TRT took abstracts from the material of “MBH Media”. This “argumentation” was based on the words of the FBK lawyer (who, apparently, also lives thanks to Khodorkovsky’s investments), as well as Alexey Navalny’s video .
