If you have seen blood in the toilet after a bowel movement, or on the toilet paper after you wipe, this may or may not be a serious problem. In most cases, the blood will appear bright red. It might also be accompanied by stools that are maroon in color. And while pain or discomfort may occur with the bowel movement, it also might not.

Minor rectal bleeding is typically caused by abrasions on the outside of the anus, which often clear up without treatment, as well as hemorrhoids (a swollen vein or veins in the rectum or anus) or anal fissures (tears in the anal tissue). It’s important to pay close attention to any blood you see in your stool because, if it’s coming from the colon or further up the gastrointestinal tract, it could indicate a more serious problem. Mild cases of rectal bleeding will usually clear up within a day or two either without treatment or with over-the-counter medications.

What triggers rectal bleeding?

There are several likely causes of mild rectal bleeding that does not require treatment, such as temporary constipation or hemorrhoids, as well as dehydration, which can result in hard stools that injure the anus or rectum while passing. In most cases, the bleeding stops once the underlying cause has been resolved.

On the other hand, cases that require a physician’s attention include:

  • Cancer in the colon, rectum, or anus. When diagnosed early, which may require a colonoscopy, these are among the more treatable forms of cancer. With colorectal cancer and many types of anal cancer, the growth rate is very slow, which is why early diagnosis is so important.
  • Colon polyps. Polyps are small growths of cells that sometimes form on the lining of the colon. For the most part, they are benign and harmless, especially when detected early. Over time, however, some polyps can develop into colon cancer. Typically, polyps can be seen easily and removed during a colonoscopy.
  • Crohn’s disease. A type of inflammatory bowel disease, Crohn’s causes inflammation along the digestive tract resulting in severe diarrhea, pain in the abdomen, fatigue, and even malnutrition. It can occur in several different areas of the digestive tract. The inflammation often extends deep into the layers of the bowel tissue, leading to debilitating pain.
  • This condition is also known as diverticular disease, and occurs when pockets called diverticula create bulges in the walls of the digestive tract, usually the colon. If material gets trapped in these pockets during digestion, it can cause pain and inflammation, a condition known as diverticulitis. Diverticulosis is often undetected unless it shows up on an x-ray or during a colonoscopy.
  • Ulcerative colitis. Patients suffering from ulcerative colitis often experience diarrhea, abdominal cramping, pain and/or bleeding in the rectum, and problems with bowel movements, including an inability to defecate despite a feeling of urgency.
  • Anal fissures. An anal fissure is when a small tear forms in the thin lining of the anus or mucosa. It often happens as a result of passing hard or large stools, causing pain and bleeding during a bowel movement. Usually, this condition gets better with simple treatment.

What you should do.

For non-severe cases of constipation, the best action you can take is to start eating plenty of fiber-rich foods – such as leafy greens, fresh fruit, whole-grain breads, and nuts – and drink plenty of water to avoid dehydration. Also, try to maintain regular bowel habits.

To treat non-severe hemorrhoids, get up and walk around every so often if you sit a lot. Fiber-rich foods are also important when you have hemorrhoids, and avoid heavy lifting as much as possible. Also, when going to the bathroom, don’t push too strenuously or for to long during a bowel movement.

If you experience severe abdominal pain or a fever with rectal bleeding, see a doctor as quickly as possible as this may indicate a problem that needs immediate attention. You should also see a physician if you experience black, tarry stool or persistent bleeding from the rectum or anus, or get dizzy while you have any of these symptoms.

Rectal bleeding refers to any blood that passes from the anus (where stool, or poop, exits the body). It can show up in the stool, on toilet paper, or in the toilet, and can range from bright red to almost black.  

Kids can have rectal bleeding for different reasons, most of which are not serious.

Causes

Different things can cause rectal bleeding; these are some of the most common:

  • Anal fissures: These small tears are the most common cause of rectal bleeding in children. They can hapen when passing a large or hard stool, which stretches the lining of the anus until it tears, or when frequent diarrhea irritates the lining. The tears can cause pain or itching in the area, especially during and after a bowel movement (BM).

    Fissures are very common in babies, and usually heal completely with basic care. In older kids and teens, the cuts can take several weeks or longer to heal and sometimes tear open again. The three F’s (fluids, fiber, and fitness) and, in some cases, stool softeners can help make BMs easier to pass. Keeping the area clean and applying ointments can relieve pain and speed healing.

  • Constipation: This is when someone has painful (hard, dry, and unusually large) or less frequent bowel movements (BMs). The three F’s — fluids, fiber, and fitness — can help prevent and control most cases of constipation.
  • Hemorrhoids: A frequent complaint of pregnant women, but not common among kids, these are varicose veins in the anus or rectum. They may bleed, itch, or sting, especially during or after a bowel movement. Again, fluids, fiber, and fitness can help prevent constipation (a leading cause of hemorrhoids) and control many instances of hemorrhoids.
  • Polyps: These small growths of tissue in the lining of the rectum or colon may bleed during or after a bowel movement.

More serious causes of rectal bleeding include:

  • Inflammatory bowel disease (IBD), which refers to two chronic diseases that cause inflammation of the intestines: ulcerative colitis and Crohn’s disease. Both can cause frequent diarrhea, so blood often appears in the stool. In severe cases, continued small amounts of blood loss can lead to anemia.
  • Intestinal infections caused by bacteria (such as shigella, salmonella, and campylobacter), viruses, or parasites. Many of these can be prevented with good hand-washing and food safety habits.

Sometimes, food allergies and blood-clotting problems also can lead to rectal bleeding.

Treatment

Drinking plenty of fluids, eating foods with fiber, and exercising regularly can help treat and prevent constipation, hemorrhoids, and anal fissures. Keeping the area clean and applying ointments can relieve pain and speed healing. Rarely, a fissure doesn’t heal and the doctor may recommend surgery.

Bleeding from the bottom (rectal bleeding)

A small amount of one-off bleeding from the bottom is not usually a serious problem. But a GP can check.

Check if you’re bleeding from the bottom

You might be bleeding from the bottom if you have:

  • blood on your toilet paper
  • red streaks on the outside of your poo
  • pink water in the toilet bowl
  • blood in your poo or bloody diarrhoea
  • very dark, smelly poo (this can be blood mixed in poo)

A small amount of one-off bleeding can often go away on its own without needing treatment.

Non-urgent advice: See a GP if:

  • your child has blood in their poo
  • you’ve had blood in your poo for 3 weeks
  • your poo has been softer, thinner or longer than normal for 3 weeks
  • you’re in a lot of pain around the bottom
  • you have a pain or lump in your tummy
  • you’ve been more tired than usual
  • you’ve lost weight for no reason

Anal bleeding can be a sign of many different problems. Some causes can be serious, which is why it should be checked out by your doctor.

Symptoms

Bright red blood may be seen only on toilet paper or in the toilet bowl after a bowel movement – not mixed freely with the bowel motion. This type of bleeding is usually caused by haemorrhoids (piles) or anal fissure.

  • A haemorrhoid is a swollen vein inside the anus.
  • A fissure is a split in the lining of the anus.
  • Both can be caused by constipation, and may cause difficult and painful bowel movements.

Blood mixed into the bowel movement usually means trouble further up from the anus: it may indicate an inflamed rectum or colon (large intestine or bowel). This type of bleeding can be caused by a serious condition, and you may have other symptoms as well.

Other symptoms:

  • sharp anal pain
  • mucus and pus in the bowel motions
  • constipation
  • abdominal pain
  • cramping
  • fever
  • diarrhoea
  • tiredness and being unusually pale – which may be a sign of anaemia (low blood count).

Blood mixed into the bowel motion may also indicate bowel cancer. Bowel cancer is the second leading cause of cancer death in New Zealand. Polyps may be found before the cancer develops. Polyps are small growths from the inner lining of the colon. They may cause minor bleeding.

Maroon or black blood is usually caused by bleeding higher in the gut.

  • Bleeding ulcers in the stomach or high in the small intestine cause black or tarry bowel motions.
  • Overuse of alcohol, aspirin or other anti-inflammatory medication can cause bleeding higher in the gut.
  • Iron pills cause black bowel motions but don’t cause bleeding.

When to see your doctor

If you notice either bright red or black/tarry anal bleeding, see your doctor as soon as possible. If your bleeding is non-stop or in large amounts, go to the emergency department immediately.

You may need tests to find the cause of the bleeding. These could include X-rays or endoscopic procedures such as a colonoscopy. A colonoscopy is the passage of a long, flexible tube through the colon, which allows doctors to see what is happening inside it.

Rectal bleeding

Definition

Rectal bleeding is when blood passes from the rectum or anus. Bleeding may be noted on the stool or be seen as blood on toilet paper or in the toilet. The blood may be bright red. The term “hematochezia” is used to describe this finding.

Considerations

The color of the blood in the stools may indicate the source of bleeding.

Black or tarry stools may be due to bleeding in the upper part of the GI (gastrointestinal) tract, such as the esophagus, stomach, or the first part of the small intestine. In this case, blood is most often darker because it gets digested on its way through the GI tract. Much less commonly, this type bleeding can be brisk enough to present with bright rectal bleeding.

With rectal bleeding, the blood is red or fresh. This usually means that the source of bleeding is the lower GI tract (colon and rectum).

Eating beets or foods with red food coloring can sometimes make stools appear reddish. In these cases, your doctor can test the stool with a chemical to rule out the presence of blood.

Causes

Rectal bleeding causes include:

  • Anal fissure (a cut or a tear in the anal lining, often caused by straining hard, hard stools or frequent diarrhea). It may cause sudden onset of rectal bleeding. There is most often pain at the anal opening.
  • Hemorrhoids, a common cause of bright red blood. They may or may not be painful.
  • Proctitis (inflammation or swelling of the rectum and anus).
  • Rectal prolapse (rectum falls out through the anus).
  • Trauma or foreign body.
  • Colorectal polyps.
  • Colon, rectal, or anus cancer.
  • Ulcerative colitis.
  • Infection in the intestines.
  • Diverticulosis (abnormal pouches in the colon).

When to Contact a Medical Professional

Contact your health care provider if there is:

  • Fresh blood in your stools
  • A change in the color of your stools
  • Pain in the anal area while sitting or passing stools
  • Incontinence or lack of control over passage of stools
  • Unexplained weight loss
  • Drop in blood pressure that causes dizziness or fainting

You should see your provider and have an exam, even if you think that hemorrhoids are causing the blood in your stool.

In children, a small amount of blood in the stool is most often not serious. The most common cause is constipation. You should still tell your child’s provider if you notice this problem.

What to Expect at Your Office Visit

You may be asked the following questions:

  • Have you had any trauma to the abdomen or rectum?
  • Have you had more than one episode of blood in your stool? Is every stool this way?
  • Have you lost any weight recently?
  • Is there blood on the toilet paper only?
  • What color is the stool?
  • When did the problem develop?
  • What other symptoms are present (abdominal pain, vomiting blood, bloating, excessive gas, diarrhea, or fever?

You may need to have one or more imaging tests to look for the cause:

  • Digital rectal exam.
  • Anoscopy.
  • Sigmoidoscopy or colonoscopy to look inside your colon using a camera at the end of a thin tube to find or treat the source of bleeding may be needed.
  • Angiography.
  • Bleeding scan.

You may have one or more lab tests before, including:

  • Complete blood count (CBC)
  • Serum chemistries
  • Clotting studies
  • Stool culture

Any loss of blood is harmful to the body

The intensity of bleeding can be different: from marks on toilet paper to heavy bleeding when blood runs in a trickle or a person goes to the toilet with blood clots.

Potential diseases

There are several reasons for the discharge of blood from the rectum:

  • some of them are deadly;
  • from others you cannot die, but they significantly reduce the quality of life.

Causes of blood with pain

This can manifest itself: anal fissure, hemorrhoids, anal polyps, condylomas. And also some other proctological diseases, including advanced stage of rectal cancer.

Causes of blood without pain

Most often this indicates the presence of a chronic disease – for example, a chronic fissure or chronic hemorrhoids.

Less commonly – about the fimbria in the stage of inflammation, which can occur in the absence of adequate treatment for a long time.

Only a doctor can establish an accurate diagnosis, detect diseases dangerous to life and health in time and carry out the necessary treatment.

Hemorrhoids

The most common cause is of course hemorrhoids. With it, the release of blood can be either one of the characteristic symptoms of the disease (while blood appears almost daily, the intensity is insignificant), and its serious complication (a large volume of blood is released once).

Fissure

Also, the cause of blood from the anus can be an anal fissure, when the mucous membrane is torn and the wound bleeds.

Cancer

The most serious cause is colon cancer. The tumor first grows, then its surface is covered with “ulcers” and begins to bleed. You need to know that cancer can be cured, especially at an early stage. That is why, if blood appears from the anus, you must immediately consult a doctor.

Excessive bleeding – life threatening

A one-time loss of a large amount of blood can lead to death of the patient. In such a situation, urgent help from a proctologist is needed.

Fortunately, heavy bleeding is rare.

Frequent small amounts of blood are dangerous

More often people suffer from recurrent small amounts of blood. This condition can last for months or years.

In the first months of the disease, it usually bleeds only with constipation.

But further bloody discharge can also occur with an ordinary soft stool – during or after a bowel movement.

Over time, bleeding gets worse and more frequent. They can appear under any load (sitting on a chair, walking, lifting weights).

Anemia (or “anemia”, lack of hemoglobin), oxygen starvation, tissue malnutrition, problems with infections – this is just a small list of the negative consequences of frequent low blood volume losses.

Suppositories and pills do not help

Do not try to stop the blood on your own. Tablets, suppositories and ointments do not relieve the cause of the bleeding.They only temporarily ease the symptoms, driving the disease deep into the body. The disease progresses, the stages increase, and the person loses precious time. It must be remembered that blood is just an external manifestation of problems that have already “settled” inside the rectum.

When the first signs of blood from the anus appear, you should immediately make an appointment with the proctologist and undergo an examination.
