DOSAGE AND ADMINISTRATION
Ascorbic acid (vitamin c) is usually administered orally. When oral administration is not feasible or when malabsorption is suspected, the drug may be administered IM, IV, or subcutaneously. When given parenterally, utilization of the vitamin reportedly is best after IM administration and that is the preferred parenteral route.
For intravenous injection, dilution into a large volume parenteral such as Normal Saline, Water for Injection, or Glucose is recommended to minimize the adverse reactions associated with intravenous injection.
The average protective dose of vitamin C for adults is 70 to 150 mg daily. In the presence of scurvy, doses of 300 mg to 1 g daily are recommended. However, as much as 6 g has been administered parenterally to normal adults without evidence of toxicity.
To enhance wound healing, doses of 300 to 500 mg daily for a week or ten days both preoperatively and postoperatively are generally considered adequate, although considerably larger amounts have been recommended. In the treatment of burns, doses are governed by the extent of tissue injury. For severe burns, daily doses of 1 to 2 g are recommended. In other conditions in which the need for vitamin C is increased, three to five times the daily optimum allowances appear to be adequate.
Parenteral drug products should be inspected visually for particulate matter and discoloration prior to administration, whenever the solution and container permit.

- Kidney stones or other very bad kidney problems have rarely happened in people taking high doses of ascorbic acid (vitamin C) injection for a long time. The risk may
be higher in people who have had kidney problems or kidney stones. The risk may also be higher in people 65 or older and children younger than
2 years of age.
- Do not take ascorbic acid (vitamin C) injection for longer than you were told by your doctor.
- Be careful if you have G6PD deficiency. Anemia may happen.

Intravenous Vitamin C – Paradigm Naturopathic Medicine
It is becoming apparent that the classic high dose-model of administering vitamin C-based treatments is not typically needed and may in fact be detrimental or harmful. Higher-dose treatments (i.e. beyond 50-75 grams/infusion) can place the body in an increased state of stress and actually further exhaust the body over time – more is not necessarily better. This is one of the reasons why we do not typically go beyond 25 grams of vitamin C-based infusions with patients and yet the benefits remain (and the results are in fact better). Other experienced physicians who have been using vitamin C-based treatments in people with cancer well over a decade are also noticing a similar trend. Experience is crucial in this area.
While the more standard oncology community feels that such treatments should not be combined with either chemotherapy or radiation, a growing body of data is showing that this recommendation is incorrect or wrong. Clinical experience clearly demonstrates that a combination approach allows for a “synergistic-effect” to, for example, enhance chemotherapy while at the same time protecting the body (i.e. decrease negative side-effects). Case after case is demonstrating how the effectiveness of chemo can go along long way when taken together with intravenous vitamin C treatment. Radiation is showing a similar trend however the data is more preliminary.
Dr. Spooner believes that the greatest benefit of vitamin C lies in its combination with conventional treatment versus using each one on its own.
Cancer cells respond very differently to intravenous vitamin C than when compared to the body’s normal cells. For example, a cancer cell makes a rather rapid and sustained increase in hydrogen peroxide in response to vitamin C which results in a “rusting-effect” known as oxidative damage (i. e. this is how many chemo agents work to kill cancer). Normal or healthy cells do not respond this way with vitamin C. This phenomenon makes intravenous vitamin C a very unique and targeted treatment unlike any other chemotherapy-like medicine.
It is important that each intravenous treatment be tailored to the individual patient and particular situation. Protocols are adjusted based on how the patient is feeling, the type of cancer(s), conventional treatments they are receiving, and what is financially feasible over time. Supportive ingredients alongside the vitamin C assist in enhancing the effectiveness of the treatment.
Typically, a person would receive 2-3 treatments per week and the duration depends on how a patient responds. In the beginning, it is advised that a person try a treatment cycle for at least 8 weeks to help assess the potential benefit of treatment. Doses are started low and gradually increased to better customize a person’s individual tolerance.
Similar to a treating medical oncologist, there may be a need to adjust or change a treatment protocol at any time. We have many, many protocols to use in cancer care, and the art of medicine is to determine which one works best for you.
Basic Facts Why Intravenous vs. Oral Vitamin C is Key in the treatment of Cancer:
Vitamin C levels in the body are tightly controlled by the bowels and the kidney’s (i.e. the body gets rid of it quickly)
Even by taking up to 18000mg per day orally in divided doses throughout the day, the blood levels do not change and do not exceed 0.2 mMol/L
Intravenous vitamin C bypasses the tight control by the body leading to a 70-fold higher blood levels with only a 2000mg injection
A simple injection of 5000mg of vitamin C produces a blood value of 3mMol/L and the research has shown that you can kill most cancer cells in the laboratory with values between 0.5 to 3mMol/L
How Intravenous Vitamin C (IVC) may work in killing cancer (chemotherapeutic action):
Increased production of hydrogen peroxide production (pro-oxidant)
Anti-angiogenesis (stop the blood supply that feeds cancer)
Immune system support (interferon, interleukin, etc. )
And many, many, many other ways
Safety of Intravenous Vitamin C:
A 2010 study evaluating the use of vitamin C amongst practitioners revealed, after calculating over 750,000 yearly sales and estimated yearly doses of over 350,000 in 2008, and after evaluating over 9000 patients, only minor side-effects were noted that included lethargy/fatigue, change in mental status, and vein irriation. Clinically, if these effects occur they are temporary and easily corrected.

The negative side effects of vitamin C IV are rare. However, there are concerns and potential side effects to be considered:
Although it has been reported only once in the literature (Scot Med J 1979;24:151), tumor necrosis, hemorrhage, and subsequent death after a single intravenous 10 gram dose of vitamin C should be the highest priority for safety. This is why we always begin with a small dose.
Another report described acute oxalate nephropathy in a patient with bilateral ureteric obstruction and renal insufficiency who received 60 gram IVC. Consequently, we need to ensure that your kidneys are working well enough before beginning and especially with a higher-dose infusion.
A rare hemolysis can occur in patients with a red cell glucose-6-phosphate dehydrogenase deficiency (G6PD) and so this may be screened in select cases.
Localized pain at the infusion site can occur if the infusion rate is too high or the pH too low. This is easily corrected by either slowing the rate or adjusting the formula.
Vitamin C may decrease the levels of calcium, chloride, and potassium and as a consequence some patients may experience shakiness or ache. This is treated by further adjusting the formula with those ingredients.
Given the amount of fluid and sodium that is used as the carrier for vitamin C, any condition which could be affected by the increased fluid load (i. e. congestive heart failure, ascites, edema, pleural effusion, etc.) needs to be monitored more closely and a tailored protocol given accordingly.

Vitamin C, also known as ascorbic acid, is currently making headlines in the fight against COVID-19. Reports say that intravenous (IV) vitamin C may help people suffering from this disease.
Recently, IV vitamin C clinical trials began in China and Italy. Doctors in hard-hit areas of the United States are now reporting using it. Furthermore, IV vitamin C appears in some COVID-19 critical care guidelines, like at the Eastern Virginia Medical School.
Even though using IV vitamin C is nothing new, there is not enough scientific evidence to determine if it works against COVID-19. Read on for more information.
What is IV vitamin C?
A solution of vitamin C is delivered directly into the bloodstream through a vein, typically in the arm. A physician’s order is required to receive this treatment. It is important to get IV vitamin C only in a clinic or other medical setting.
Why use IV vitamin C?
IV vitamin C goes straight into the bloodstream. Because it skips the stomach and intestines, vitamin C in the blood rises to very high levels and very quickly.
What do we know about IV vitamin C in COVID-19?
The clinical data from IV vitamin C trials will not be available for many months. So far, doctors and researchers cannot tell how effective IV vitamin C is or how it might work.
There is some scientific evidence that IV vitamin C can be useful in the treatment of other infections. Most of the recorded use of IV vitamin C has been in the treatment of cancer.
To be clear: IV vitamin C is not a cure for COVID-19.
What about vitamin C supplements?
It is hard to compare them. You cannot get blood vitamin C levels nearly as high as IV vitamin C when taking vitamin C supplements.
Despite label claims, no supplement is equal to IV vitamin C – no matter how much you take or what formula you use.
Can vitamin C supplements help your immune system? Yes, but for different reasons.
Caution: Be careful taking very high doses of vitamin C supplements. They can also cause unwanted side effects such as abdominal cramping and diarrhea.
How might IV vitamin C work in COVID-19 patients?
There are several theories about how IV vitamin C may help treat COVID-19:
- Very high vitamin C levels may create free radicals that destroy viruses and bacteria. Our body’s cells have defenses against these free radicals, but viruses do not.
- Another possibility is IV vitamin C renews the body’s antioxidant protection. Serious infections can use up our body’s vitamin C and other antioxidants very quickly.
Vitamin C is a vital part of the immune system, but there is no reason to think IV vitamin C can boost your immune system beyond normal, healthy status. Daily vitamin C from food or supplements is enough to support healthy immunity.
Is IV vitamin C safe?
IV infusions always require proper administration by a medical professional. Patients need to be screened to make sure it is safe for them. Some conditions might prevent a person from getting IV vitamin C.
From research on vitamin C and cancer, we know that IV vitamin C is safe when medical guidelines are followed.
Although doctors are using IV vitamin C, there is no published scientific evidence about the use of IV vitamin C in COVID-19. Therefore, we cannot fully evaluate its effectiveness.
IV vitamin C is not the same as taking vitamin C supplements. Vitamin C supplements may help the immune system, but vitamin C blood levels that are far lower than IV vitamin C.

High Dose Vitamin C IV
Vitamin C has such broad applications because it is a key factor in collagen production (skin, blood vessels, ligaments and bones), adrenalin and carnitine synthesis (energy), it directly boosts the immune system, and it has a potent antioxidant function. So let’s look at each of these in turn.
Collagen is the structural protein found in all animal tissue. It holds our muscles to our bones, provides structural support for veins and arteries, and more.
Vitamin C is required for collagen synthesis along with the amino acids proline and lysine (1). When you don’t have enough collagen, you may have thin skin and weaker blood vessels, bones, tendons and ligaments. This can lead to aneurysms (blood vessel wall thinning), possible stroke, and even the ruptured ligaments and tendons commonly seen in sports injuries.
High doses of vitamin C when administered by IV can bolster skin and connective tissue and prevent these outcomes.
Vitamin C is required in the breakdown of fat for energy use. It takes part in the synthesis of a molecule called carnitine which serves as a shuttle bringing fats into the mitochondria in our cells for energy production.
This means it can have a powerful influence on our energy levels. For example, a 2011 scientific study showed high dose (10 grams) vitamin C given by IV reduced fatigue in office workers after 2 hours and continued its effect for 24 hours (2)
Vitamin C may have a direct effect against infectious agents like bacteria, viruses, and even cancer cells. (The use of Vitamin C in cancer is still controversial but is being studied and is promising… read more below.) At very high doses it can generate peroxide (h3O2) which may account for this effect.
Before the polio vaccine, vitamin C was used as a high dose intramuscular injection every 8 hours in the treatment of polio with some success. More recently in mice genetically modified to lose the ability to make vitamin C, external vitamin C was shown to increase the mice’s ability to increase anti-influenza cytokines (powerful immune system cells) and fight influenza virus infections (3).
Vitamin C is a potent antioxidant that helps protect cell structures like collagen, proteins, and DNA from damage by free radicals and reactive oxygen species (ROS) which are common in high stress states, chronic illness and infections.
Research has shown that the antioxidant properties of vitamin C can reduce the severity of symptoms associated with h2N1 influenza virus and is considered an important add-on therapy in surviving the flu (4).
Cancer and Chemotherapy
The role of vitamin C as a therapy for cancer is very controversial. No large scale studies have been done to definitively conclude one way or another if vitamin C has a role in the treatment of cancer. Here is what we do know:
- Vitamin C even in high doses of 1.5 grams/ kg (over 100 grams) is safe and tolerated very well with very few side effects. Even the myth of kidney stones (oxalic acid) has been dispelled in recent studies which have shown that oxalic acid levels remain well controlled even with high doses of vitamin C. (5)
- IV vitamin C becomes an oxidant at high doses as opposed to its antioxidant function at lower doses. This is because vitamin C interacts with oxygen and iron to cause the formation of high concentrations of peroxide (h3O2) in tissues. Peroxide is used by immune cells to kill foreign invaders and cancer cells. Vitamin C also depletes ATP and causes DNA damage to cancer cells. (7)
- High dose IV vitamin C can be used safely in conjunction with chemotherapy (except methotrexate) and radiation in certain situations in which the patient’s oncological team is aware of and has agreed to incorporate vitamin C as a treatment modality. (5, 6)
- High dose IV vitamin C may improve mood and energy for patients undergoing cancer treatment using other modalities. (5)
- High dose IV vitamin C can be used safely in treatment of terminal cancers which have not responded to chemotherapy and radiation treatment even if it may be of limited use in further killing these cancer cells. (5)
Some of the most active treatment and research for the use of high dose IV vitamin C is coming out of The Riordan Clinic in Wichita, Kansas where they have been using it for decades. Vitamin C (ascorbate) is a powerful antioxidant and several critical enzymes require vitamin C in the synthesis of collagen from proline and lysine. Vitamin C is, therefore, required for the maintenance of normal connective tissue as well as for wound healing since synthesis of connective tissue is the first event in wound tissue remodeling. Vitamin C is also necessary for bone remodeling due to the presence of collagen in the organic matrix of bones.
Vitamin C is required for conversion of tyrosine to epinephrine (adrenalin), and the synthesis of the bile. It is present in high amounts in the adrenal gland cortex and these levels are depleted after adrenocorticotropic hormone (ACTH) stimulation of the gland and synthesis of the stress hormone cortisol, which is why it’s important to replete vitamin C level during times of high stress.

patients’ care, included ARDS of diverse etiology. In the context of a coronavirus pandemic, with an elevated
morbimortality and pressure over the sanitary system, it is of vital importance to use every available resource to
improve patients’ outcomes in an accessible and safe way. In this article, I briefly analyze the evidence around the
use of vitamin C in the critical patient and its potential benefits on admission time, intubation time and mortality on
patients affected by ARDS.
Vitamin C was discovered in the 30s by Albert Szent-Györgyi. The first therapeutic function known was to treat scurvy, first described on ship crews who couldn’t access fresh fruits and vegetables during long seasons on the sea. Later on, Nobel Prize winner Linus Pauling performed numerous studies on the physiological and therapeutic effects of vitamin C, concluding that we were just beginning to understand its full potential. Humans are one of the few vertebrates that can’t synthesize vitamin C, therefore it is considered to be an essential nutrient. It’s estimated that 7% of the general population is deficient in vitamin C, but this percentage increases to 47% in admitted patients. ₍₁₎₍₂₎
Vitamin C’s enteral absorption is limited and its plasmatic levels are strictly regulated. A standard dietary intake maintains plasmatic levels around 70 ɥmol/L, but intensive oral supplementation of 3 grams a day only rises this level up to a maximum of 220 ɥmol/L. Therapeutic effects are achieved with plasmatic levels in the range of 20-49 mmol/L (100 times higher than those achieved by oral intake) only possible with intravenous infusion₍₃₎₍₄₎₍₅₎.
Evidence shows that the therapeutic effects take place with a minimum dose of 50mg/k/day and a maximum of 200mg/k/day, divided into four doses given every 6 hours. The largest randomized study performed so far, CITRIS-ALI ₍₆₎, found that vitamin C supplementation failed to modify biomarkers such as thrombomodulin, C reactive protein and organ failure scores (SOFA), but successfully reduced mortality by 16.5%, respiratory assistance requirements by 2.5 days, ICU stay by 3.2 days and hospital admission by 6.7 days, compared to placebo. This is more relevant than never in view of the worldwide shortages in beds and equipment. Some authors suggest that the reason why biomarkers weren’t modified was that the samples were taken at 96 hours, time when more severely ill patients had died and were excluded from the study (survivorship bias)₍₇₎₍₈₎. Two meta-analyses that included 685 and 147 critical patients concluded that intravenous vitamin C showed no adverse reactions, reduced the need for fluids and vasopressor support and reduced intubation time up to 25%₍₉₎₍₁₀₎.
A series of case reports on ADRS of diverse etiology (aspirative after seizures, inflammatory during a psoriasis flare-up, and viral) describes a fast improvement following vitamin C administration, even when the treatment was started after ECMO was required. In every case there was a symptomatic and radiologic improvement in 24-48 hours after the initial dose and good evolution at discharge, without signs of fibrotic sequels₍₁₁₎₍₁₂₎₍₁₃₎. Marik et. al. found a 30% mortality reduction in septic patients treated with vitamin C, hydrocortisone and thiamine, when compared to patients treated under standard procedures. In addition, the treated group required 50% less vasopressor support than the control group₍₁₄₎.
Regarding the evidence around vitamin C’s mechanisms of action, certain preclinical findings might explain the effects observed on respiratory distress. Vitamin C down-regulates inflammatory genes and inhibits the cytokine storm responsible for the activation of pulmonary neutrophils, therefore protecting alveolar capillaries from inflammatory damage. In addition to this, it enhances alveolary fluid clearance by increasing the water transporter channel expression₍₁₅₎₍₁₆₎.
In regards to its safety, most studies report no adverse effects on large doses of vitamin C. On rare occasions, the following have been described: Hypersensitivity, oxalate urolithiasis, iron overload in haemochromatosis and anaemia among others, most of them with a prevalence less than 1%. It has also been described the inaccuracy of bedside glucometry when using vitamin C and it is advised to corroborate findings with laboratory results₍₅₎₍₁₇₎₍₁₈₎₍₁₉₎.
We live in times of incalculable need. Worldwide medical supplies are in shortage, costs threat to crush even the wealthiest of health care systems, and above all the wellbeing of millions of humans is at risk. Treatment of severe ARDS from COVID-19 is an ongoing challenge and a specific treatment could be months ahead. The evidence around vitamin C is scarce but promising.
