Hair Loss? It May Be Iron Deficiency

May 17, 2006 – If you’re losing hair, you may have an iron deficiency.

A review of 40 years of research shows that iron deficiency has a much closer link to hair loss than most doctors realize. It may be the key to restoring hair growth, Cleveland Clinic dermatologists find.

“We believe that treatment for hair loss is enhanced when iron deficiency, with or without anemia, is treated,” Leonid Benjamin Trost, MD; Wilma Fowler Bergfeld, MD; and Ellen Calogeras, RD, MPH, write in the May issue of the Journal of the American Academy of Dermatology.

It’s a controversial issue. Not every study shows a link between iron deficiency and hair loss. Trost says there’s not enough hard evidence — yet — to make iron-deficiency screening a routine procedure for people with hair loss.

But study researcher Bergfeld has been doing this for years. And she’s finding that whatever the cause of hair loss — for both women and men — having too little iron in the blood makes it worse.

“What Dr. Bergfeld has found in decades of experience, is when she treats patients for iron deficiency –even in the absence of anemia — it can maximize their ability to regrow hair,” Trost tells WebMD. “It is not the silver bullet for baldness, but it can definitely help maximize how a patient regrows hair.”

The Cleveland Clinic isn’t alone in doing this. George Cotsarelis, director of the University of Pennsylvania Hair and Scalp Clinic, has studied iron supplementation in women with various forms of hair loss.

“From our clinic’s experience, it is clear to me that if you replenish hair-loss patients’ iron stores with iron supplements, they are more likely to regrow hair, or at least stop hair shedding,” Cotsarelis tells WebMD. “And they don’t have to be anemic. That is the biggest mistake doctors make.”

An even bigger mistake would be for balding people to take iron supplements on their own. Unless you have iron deficiency — diagnosed by a doctor — iron supplements can lead to a very dangerous condition from iron overload.

Hair Loss May Be a Symptom of Serious Illness

A sensitive way to check total body iron stores is to measure the amount of ferritin in the blood. Ferritin is a protein that plays an important role in iron storage. As a general rule, the less ferritin in the blood, the less iron a body has stored up.

Cotsarelis and Trost say that what most doctors consider to be a normal ferritin level is, in fact, too low. Ferritin levels of 10-15 ng/mL are within the “normal” range. Cotsarelis says a ferritin level of at least 50 ng/mL is needed to help replenish hair. Trost and Bergfeld shoot for 70 ng/mL.

“Doctors see ferritin levels in the normal range, and don’t do anything,” Cotsarelis says. “But the normal range is wrong, I think. The normal range for women is 10-120 ng/ML, and for men it is 30-250 ng/mL. Why should a man’s be lower than a woman’s? Mostly because women are iron deficient. It is almost a public health problem. Hair loss is only an indication of this.”

Cotsarelis and colleagues have found that women with hair loss have significantly lower iron stores than women without hair loss. Surprisingly, this was particularly true for women with alopecia areata, a form of hair loss caused by haywire immune responses.

“Our theory is that lower iron levels decrease the threshold for developing hair loss of any kind in genetically predisposed individuals,” Cotsarelis says. “So people prone to develop even hereditary hair loss, if their iron levels are low, it accelerates that process. We think it’s because the hair follicles grow so much, they require a lot of iron.”

Women who frequently have heavy menstrual periods often become iron deficient. “If you have a healthy woman with hair loss, you can assume iron deficiency,” Trost says.

Iron deficiency is less common in men and postmenopausal women than in women of childbearing age. But it’s something Cotsarelis and Trost see often in people with hair loss. Especially when it gets to the point of anemia, iron deficiency can be a symptom of very serious illness. It’s important for a doctor to find out why this is happening.

“If you have a man or a postmenopausal woman with iron-deficiency anemia, you need to do a workup to find out why,” Trost says. “Say you have a 55-year-old man with iron-deficiency anemia — it could be caused by bleeding due to colon cancer. Believe it or not, someone can come in complaining of hair loss, and find out it is something serious.”

Don’t Take Iron Supplements Without Doctor Visit

Iron supplements are not a cure for baldness. But as part of a multipronged approach, Cotsarelis and Trost say, supplements can be a big help.

So is a diet full of iron-rich foods, such as tofu, lentils, beans, oysters, spinach, prunes, raisins, and, yes, lean beef. Learn more about the connection between your diet and alopecia.

Trost says he and Bergfeld usually recommend these foods, plus supplementation with ferrous sulfate, 325 milligrams per day taken on an empty stomach.

It’s not an easy supplement to take.

“Iron supplements cause constipation and gastrointestinal upset,” Cotsarelis says. “We try different preparations, but they but all seem to have similar problems. And there is some anecdotal evidence that orange juice, vitamin C, or lysine, if take together with the iron, helps the absorption.”

Do not take iron supplements unless a doctor has told you that you have iron deficiency, Trost warns.

“Iron supplements are available over the counter, but we recommend you take them only under the supervision of a doctor,” he says. “It is safe, when used appropriately, but if taken when inappropriate it can cause some harm. If you take a too-high dose of vitamin C, your body eliminates it — but iron doesn’t work that way. Your body can regulate how quickly it uptakes iron, but has no way to get rid of it quickly. If you are not deficient, you can get iron overload, which can be dangerous.”

Do Iron Pills Stimulate Hair Growth in Women?

Hair loss is a real concern for many women, and iron pills have been shown to help with hair growth. Doctors can test for iron deficiency and will determine if supplements will help. In addition to iron, there are other vitamins that may help hair grow stronger and thicker.

Tip

Increasing your iron levels through supplements or eating iron-rich foods, like red meat and dark leafy greens, can help with hair loss if you have low iron levels. Vitamin C and vitamin D have also been shown to help with hair loss.

Benefits of Iron for Hair

A recent study showed the benefits of iron for hair growth. One of the most common types of hair loss for women is called telogen effluvium (TE), which is hair loss that often occurs at the crown or top of the scalp. A September 2017 study in the Egyptian Journal of Dermatology and Venerology showed that premenopausal women who had low iron levels had a higher chance of having hair loss.

This doesn’t mean that you should automatically start popping iron supplements. It is important that you first get your levels checked to see if iron could help with your hair growth. Ask your doctor for a serum ferritin test to see what your iron level is. In the study, women whose levels were below 20 ng/ml had a higher chance of having hair loss.

Signs of Iron Deficiency

According to Harvard Health, the main reason women in the United States have iron deficiency or anemia is from childbirth or heavy menstrual periods. If you are a vegetarian, you could also be a risk for having an iron deficiency. Women who are older than 65 may have iron deficiency from internal bleeding, so it is important to get checked out by your doctor.

Those who have an iron deficiency may notice that, in addition to hair loss or thinning, they may also have fatigue, dizziness, weakness, shortness of breath or a pale complexion. If you notice these symptoms, get tested to see if low iron is the cause.

It is important not to start taking iron supplements without getting tested first, as too much iron can cause side effects, including constipation and nausea.

Iron for Hair Growth

How much iron should you take for hair growth? Iron is not excreted from your body at a high rate, so too much can build up and become toxic. According to the US Department of Health and Human Services, women between the ages of 19 and 50 should get 18 mg of iron per day. Women older than 50 need just 8 mg per day. If you are pregnant, however, your iron requirements bump up to 27 mg per day.

According to Cleveland Clinic, the best way to amp up your iron levels is by incorporating iron-rich foods, such as meat and seafood, into your diet. Eating a four-ounce portion of red meat twice a week should give you the iron you need. If you are vegetarian, incorporate plenty of dark leafy greens, such as spinach, as well as beans into your diet. Many cereals are now iron fortified as well. Avoid eating your iron-rich meal in the morning with your coffee, as a compound in coffee inhibits the absorption of iron.

If your doctor recommends an iron supplement for hair growth, then 18 mg of iron in a vitamin will supply 100 percent of your recommended daily value.

Proven Vitamins for Hair Growth

In addition to iron supplements, a March 2019 Dermatology and Therapy study showed that vitamin C is beneficial for those who have both TE hair loss and another type of hair loss called alopecia areata (AA), in which your immune system attacks the hair follicle. Vitamin D is another vitamin that helps in hair growth, especially for those who have AA.

Contrary to popular belief, the study showed that biotin does not help with hair growth. One vitamin tested, vitamin A, actually caused hair loss when taken in too high of a dose. If you are experiencing hair loss, talk to your doctor to get to the root cause. If you do have low iron, supplements can help with hair growth.

Why is Iron Important for Hair?

Iron is a mineral that is crucial to our health. Its most important function in our body is to aid in the production of red blood cells. These cells carry oxygen (haemoglobin) around the body via our bloodstream to all our tissues and organs — including our hair follicles. Iron also optimises energy levels, nourishes your muscles (including your heart), and helps your immune system. In other words, it is an all-round essential for your general fitness and well-being.

If you do not have enough iron in your body, you may develop a condition called iron deficiency anaemia. This can cause your hair to shed.

Iron Deficiency Anaemia and Hair Loss

Iron deficiency anaemia is a condition in which your body lacks iron to make enough red blood cells. These are the cells that carry oxygen [haemoglobin] around your body to your tissues, allowing them to function.

How does a lack of red blood cells affect your hair? When your body is short of oxygen, it channels its available supply to your vital organs to keep them alive, rather than to your hair follicles. Without oxygen, your hair follicles cease to function properly, and your hair may fall out. In fact, a large proportion of people who come into our Trichological clinics for hair-loss treatment have some degree of anaemia.

The symptoms of iron deficiency anaemia include [but are not limited to]:

Extreme tiredness

Diffuse hair loss from the scalp

Anaemia can occur if your diet lacks iron-rich foods, or if you are not absorbing iron correctly. Other causes of anaemia include loss of blood (particularly in menstruating women), growth spurts in children, illness, and pregnancy. In fact, pregnant women need increased iron to deliver blood and oxygen to their baby, and therefore often become anaemic.

If you are losing your hair, or notice it is not growing as long or as fast as it used to, we suggest you ask your doctor to test your iron levels. Iron levels should be analysed by checking the following blood levels:

TIBC (Total Iron Binding Capacity)

If you discover you are anaemic, try not to worry — hair loss due to anaemia is reversible. Your GP can prescribe you an iron supplement. It is important to follow the dosage you are given, because high iron levels can also be dangerous, and taking more than prescribed will not lead to a faster result. You may also want to ask your doctor to suggest changes to your diet. 

The most beneficial treatment for anaemia is when you receive iron through good nutrition as well as a suitable supplement.

What is Ferritin?

Ferritin is a protein in your body that stores iron and releases it when your body needs it. Good ferritin levels indicate that your body’s iron levels are balanced and healthy, which in turn is essential for healthy hair.

Why is Ferritin Important for Hair?

A certain amount of ferritin is stored in your hair bulb. Your hair bulb uses it, when needed, in order to form the proteins that hair is made of — thereby allowing your hairs to grow. However, when you are not getting enough iron through your diet, your body takes ferritin stored in non-essential tissue (like your hair bulb) and gives it to essential tissue, such as your heart. This often means that the growth (anagen) phase of your hair growth cycle is shortened, causing your strands to shed prematurely before they reach their maximum length.

If you notice that your hair is shedding, and/or not growing as long as it used to, and/or that you have short, wispy hairs around your temples and hairline, you may have low ferritin levels. Ask your GP or Trichologist to check them. (It is important to note that optimum ferritin levels for your hair are higher than those for your body. Again, this is because your hair is non-essential tissue.)

If your ferritin is low, try not to worry — your doctor or Trichologist can prescribe an iron supplement. Hair loss due to low ferritin is entirely reversible, and relatively easy to correct.

6 Supplements or Vitamins for Hair and Skin Growth

Are you frustrated with dry skin, cracked lips, or dull hair? Natural dietary supplements or vitamins for hair and skin may be the answer you’re looking for. But while there’s no shortage of vitamins for skin and hair on the market, not all of them are created equal.

Let’s look at biotin, one of the hair vitamins found in many foods and available over the counter in supplement form at pharmacies and supermarkets. Some findings show that cigarette smoking may cause a deficiency in biotin, with symptoms that include:

  • Loss of hair color
  • Red scaly rash around the eyes, nose, and mouth
  • Thinning of the hair

Biotin has been found to be “likely effective” in treating biotin deficiency and is safe when used in recommended amounts (read the supplement label). Could this be the hair growth vitamin you’ve been looking for? As with any vitamin for skin or hair, always consult with your physician before you try it.

Omega-3 fatty acids are another vitamin for hair. Omega-3s may boost the shine in your hair and keep your tender scalp from flaking. A study published in September 2017 in the journal Biochimica et Biophysica Acta found that dietary supplementation with fish oil — filled with omega 3 fatty acids — could have therapeutic value to many inflammatory skin conditions.

Another study, published in March 2015 in the Journal of Cosmetic Dermatology, revealed that taking omega-3 and omega-6 supplements for six months, along with antioxidants, acts efficiently against hair loss and improves hair density.

It’s not uncommon for women of childbearing age to have anemia due to blood loss during menstrual periods, resulting in hair loss. Iron deficiency is a usual cause of anemia. Iron replacement is generally done through vitamin and mineral supplements. Your doctor can do a simple test to check for anemia.

Zinc also has antioxidant properties and is vital to your body’s resistance to infection and for tissue repair. High doses of zinc are toxic, though, so talk to your doctor about your diet to see if you need to supplement.

Losing Your Hair? It May Be Your Iron Intake / Nutrition

People have been trying to find a cure for hair loss for years. Adequate nutrition is essential for healthy hair and there is insufficient evidence that taking an iron supplement will stop hair loss. However, there is evidence that individuals with hair loss have lower iron stores than those who do not have hair loss. There is also evidence suggesting anemia is associated with hair loss. Iron deficiency can cause anemia. Never take an iron supplement without your doctor’s supervision because too much iron can be toxic and, according to the Iron Disorders Institute, can actually be a cause for hair loss.

Iron Deficiency and Hair Loss

Treating iron deficiency when you are not anemic is controversial according to researchers of a study titled “The diagnosis and treatment of iron deficiency and its potential relationship to hair loss” published in 2006 in “Journal of the American Academy of Dermatology”. But they stated in this study that they believe treating iron deficiency enhances treatment for hair loss. This makes sense, as all the nutrients in your body work in synergy to keep your cells healthy including your hair follicles.

Iron is a mineral in your body, mainly found in hemoglobin, a protein that makes up your red blood cells and carries oxygen to all of the cells in your body. Your doctor can check if you are anemic by checking your hemoglobin. Iron deficiency can cause iron deficiency anemia, in which case your doctor would also check for low ferritin to confirm the anemia is caused by iron deficiency. Although there is no sufficient evidence that iron deficiency independently causes hair loss, if you have hair loss, you may have low ferritin.

Dietary Iron

It is always better to consume nutrients from food sources rather than from supplements. The recommended dietary allowance for iron is eight milligrams a day for men over 18 and 18 milligrams a day for women over 18. For men and women over 50 the recommended dietary allowance is eight milligrams.

Dietary iron is found in two forms, heme iron and non-heme iron. Heme iron is found in the animal sources of iron such as beef, chicken, tuna, and turkey and is easier for your body to absorb. Non-heme iron is plant iron and is found in plant sources of iron such as fortified cereals, beans and lentils, spinach, and bread. Non-heme iron is not as easily absorbed by your body. Vitamin C and meat proteins increase your absorption of this iron. Calcium, tannins such as in tea, phytates such as in whole grains and legumes, and polyphenols found in many plant foods with antioxidant properties, can reduce non-heme iron absorption.

Do flat irons damage hair?

Foster said that flat irons do not damage hair. However, improper use, like utilizing a temperature too high for your hair type, can “run the risk of damage.” Bertin agreed, adding that heat damage is “irreparable and has to be grown out.” That means it’s safest to start with a lower temperature and increase if necessary. He said the cuticle, or outer layer of a hair strand, can “very easily” become damaged by heat over 356 degrees Fahrenheit or 180 degrees Celsius, “if care isn’t taken to do quick passes on a low or medium temperature with a flat iron.” Damaging that cuticle layer will make your hair more porous, which means it will struggle to hold on to moisture, explained Bertin. When heat is applied to hair, he noted, its moisture content begins evaporating, which causes dryness and breakage.

Although hair straighteners are safe to use, hairstylists we consulted advised against using flat irons — or any hot tool, for that matter — every day. “Even with the best heat protectants in the game, heat, whether that is from a flat iron or a hair dryer, is not something you should be exposing your hair to every day,” added Bertin. He said those with afro-textured hair should consider flat ironing their hair at most once a week because “our hair is naturally more fragile and dry, so flat ironing any more than that is a huge no-no.” Depending on the health and dryness of straighter hair types, you can get away with flat ironing more often, but Bertin advised against everyday use if you want healthy hair.

Having a beard was not consistently significant in assessing a man’s attractiveness, however, those with rich facial hair appeared to be more influential, in line with previous studies.

Men prone to baldness are distinguished by the sensitivity of the hair follicles, inherited through the maternal line.

Moreover, this happens to men in all countries of the world, in all ethnic groups without exception.

If being bald were so bad, this phenomenon would disappear over time. The fact that it is so widespread may even speak of its usefulness, but what is it? And if so, why only men suffer from baldness?

“There is a pattern in nature: if a man has something that a woman does not have, that something will serve as a signal,” says Frank Muscarella, a psychologist at Barry University. This topic interested him back in the 1990s.

Most of these male-only features have a similar purpose.

Then they used these images in their experiment. Muscarella showed them to 202 psychology students, who were equally male and female, and asked them to rate the attractiveness of men and aspects of their personality.

Bald and balding men did not appear to be as attractive as the others, but in one category of characteristics they were far ahead of the rest.

They were considered smarter, more influential, competent, educated, honest, responsive, as well as having a higher social status. All of these traits are collectively known as social maturity.

Muscarella suggested that baldness developed as a signal of high social status – something that some women cannot resist.

Curiously, bald men were also considered significantly less aggressive.
