Most of us experience diarrhea a couple of times a year, whether it’s from a bacterial infection, a food allergy or simply something you’ve eaten that disagrees with you. Diarrhea usually is not serious and often disappears within a day or so.

Mom’s advice is still the
best when it comes to home treatments for diarrhea: Eat chicken soup and
saltines and follow the BRAT diet – bananas, rice, applesauce and toast.

You might think that eating
will make your diarrhea worse, but your food choices can help to ease your
symptoms and ensure your health doesn’t worsen as a result.

Salty
and sweet

The biggest danger with a short bout of diarrhea is dehydration, or the loss of water and nutrients from the body’s tissues. You could become dehydrated if you have diarrhea more than three times a day and aren’t drinking enough fluids. Dehydration can cause serious complications if it isn’t treated.

The best way to guard against dehydration is to consume liquids that contain salt and sugar, says digestive disease specialist Donald Kirby, MD, Director of Cleveland Clinic’s Center for Human Nutrition. The salt slows down the fluid loss, while the sugar helps your body to absorb the salt.

“We tend not to want people
to have salt in general, but when you’re having significant diarrhea, you want
to make sure you’re getting salt and enough sugar to keep out of the emergency
room,” Dr. Kirby says.

Over-the-counter rehydration solutions such as Pedialyte® will do the job, Dr. Kirby says. Or, follow Mom’s advice and have some chicken soup with saltine crackers or pretzels.

The
BRAT diet

Here’s another bit of good advice from Mom for treating diarrhea – eat the BRAT diet: bananas, rice (white), applesauce and toast.

When your health is good, physicians usually recommend whole-grain, high-fiber foods. But high-fiber foods could spell trouble when you have diarrhea. The BRAT foods are low-fiber and can help to make your stools firmer. Bananas also are high in potassium and help to replace nutrients your body has lost because of diarrhea.

Dr. Kirby says you also can
add oatmeal, boiled or baked potatoes (peeled), or baked chicken with the skin
removed.

“These are simple foods that
people often tolerate very well,” Dr. Kirby says.

Probiotics

Another way to help your gut recover from a diarrheal infection is to consume probiotics — a food or dietary supplement that contains live bacteria, which replaces or adds to the beneficial bacteria usually found in the gastrointestinal tract.

While it’s wise to avoid dairy products when you have diarrhea, Dr. Kirby says, there’s one notable exception — yogurt or kefir, a fermented milk drink, that contain probiotics. These can restore the beneficial bacteria that your body flushes out with diarrhea. Just make sure the yogurt or kefir are low in sugar, as higher levels of sugar can potentially worsen symptoms or diarrheal losses (that’s water and electrolytes) in some patients.

It’s time to call the doctor
when diarrhea lasts longer than a few days, Dr. Kirby says. Also see your
physician if you experience severe pain or your stool contains significant
amounts of blood or pus.

Drinking Milk When You Have Diarrhea

If you have diarrhea, you may feel that you can’t trust your own bowels. Stools come out watery and too frequently. You may even feel dehydrated and fatigued from the loss of fluids and minerals. Different foods and drinks appear to trigger diarrhea in different people. For example, you may have an increased sensitivity to milk when you have diarrhea because of the lactose it contains.

Lactose

When you have diarrhea, you may find it difficult to properly digest lactose-rich foods such as milk, according to the National Digestive Disease Information Clearinghouse. Even if you’re not normally lactose intolerant, diarrhea that’s caused by a virus can make you sensitive to milk products — sometimes for up to 6 weeks after the diarrhea has cleared. Because you need to produce more of the enzyme lactase to digest lactose, this can cause more watery stools or an irritated bowel.

Children

Diarrhea can have a serious effect on children, so it’s important to feed them the right foods during a bout of persistent diarrhea. In a 1996 study published in the journal “Pediatrics,” researchers measured whether a dry cereal diet or a milky cereal diet worked best for children with diarrhea. They found that the symptoms of diarrhea generally lasted a similar amount of time. That means that moderate amounts of milk may have no negative effects in children with diarrhea.

Unpasteurized Milk

In some cases, milk may actually be the source of the diarrhea. Bacteria may grow in unpasteurized milk. For example, a common cause of diarrhea symptoms in America is the bacteria Campylobacter jejuni, which triggers campylobacteriosis, according to Colorado State University Extension. Certainly you should avoid any unpasteurized milk when you have diarrhea to avoid further possible contamination, particularly if you’re pregnant.

Considerations

You may find that yogurt triggers fewer symptoms during a bout of diarrhea than milk does. Yogurt contains less lactose. The University of Maryland Medical Center suggests taking lactase enzyme tablets before drinking milk or dairy. If you know that you’re lactose intolerant even when you don’t have diarrhea, then you should take extra care to avoid all milk and milk products, which will likely make your diarrhea symptoms much worse.

  • bananas

  • plain white rice
  • bread or toast
  • boiled potatoes
  • unseasoned crackers

These foods may be especially helpful on the first day of dealing with diarrhea. Eating many small meals throughout the day can help keep the digestive system from becoming overworked.

Probiotics

Probiotic foods, such as yogurt and kefir, may help in some cases, but in others, probiotics may irritate the digestive system even more.

Probiotics aid digestion by improving the balance of good and bad bacteria in the gut. However, dairy products can irritate the digestive system, so a person may wish to try non-dairy sources of probiotics, such as miso or sauerkraut.

Liquids are also vital to recovery. People with diarrhea must drink plenty of water throughout the day and should drink an additional cup of water after every loose bowel movement.

Drinking plenty of water helps prevent dehydration and flush any toxins out of the body.

Sugary foods and artificial sweeteners

Sugars that pass into the colon may disrupt the already sensitive bacteria there, making diarrhea worse. This includes fruit juices and high-sugar fruits.

People with diarrhea should also avoid artificial sweeteners, as some can have a laxative effect.

High fiber foods

It may also help to avoid too much fiber. Fiber helps keep the digestive system active. Usually, this is a good thing, but when the body is trying to recover from diarrhea, fiber may make symptoms worse.

Insoluble fibers are the main culprit and can be found in foods such as:

  • whole grains, such as wheat, rice, and barley
  • whole grain bread or baked goods
  • grain cereals
  • nuts and seeds

Soluble fibers, such as the pectin found in apples and bananas, can actually help a person recover from diarrhea, but a person should still try to limit their intake at least for the first day of symptoms.

Other foods not to eat

Other foods that can irritate the bowels during diarrhea include:

  • onion and garlic
  • most processed, packaged food
  • raw vegetables
  • foods that produce gas in the intestines, such as cabbage, broccoli, and cauliflower
  • citrus fruits
  • fatty meats, including sardines, pork, and veal
  • dairy products

Caffeinated beverages, such as coffee, tea, and soda, may overstimulate the digestive system and make symptoms worse.

While many sports drinks contain electrolytes that may help with dehydration, they often contain added sugars or artificial sweeteners. Coconut water or electrolyte-enhanced water are good alternatives.

Other diarrhea treatments include bismuth subsalicylate (Pepto Bismol) and Ioperamide (Imodium). These drugs can help relieve occasional cases of diarrhea and may be helpful to have on hand in a medicine cupboard.

Pepto Bismol and Imodium are available for purchase over-the-counter or online.

People with diarrhea should also ensure they get enough rest, as putting the body in stressful situations while dealing with diarrhea may make matters worse.

Limit physical activity while diarrhea symptoms are presenting, as strenuous activity may put the body at a greater risk of dehydration.

Hydration is also vital when managing diarrhea. Drink plenty of water throughout the day.

Blood or mucus may appear in the stool in more serious cases of diarrhea. This is often accompanied by a fever and requires a visit to the doctor as soon as possible.

Untreated diarrhea can also lead to serious complications, including dehydration. A person with severe diarrhea may require hospitalization and intravenous electrolytes.

Anyone experiencing a fever of higher than 102°F or severe abdominal pains should visit a doctor.

Parents or guardians should carefully monitor any children with diarrhea. If symptoms do not clear up with 24 hours, they should call the doctor for guidance.

If a child shows any signs of dehydration, seek immediate medical care. Signs include:

  • dry mouth
  • dry diaper for longer than 3 hours
  • weight loss
  • crying without tears

Any additional symptoms should be reported to a doctor as soon as possible for diagnosis and treatment.

Outlook

Many cases of diarrhea last only a few days and respond well to home treatments. Eating a simple diet, increasing fluid intake, and using over-the-counter medications as necessary can help reduce symptoms quickly.

In cases where the body does not respond to these treatments after 2 or 3 days, a person should visit their doctor for diagnosis and treatment.

Lactose Intolerance Symptoms, Tests, and Diagnosis

Thirty minutes have passed since you ate a bowl of ice cream, and now your stomach is cramping and gassy. You feel like you might have diarrhea. Does this sound like you? Or, you had milk, mashed potatoes, or even candy almost 2 hours ago and have these symptoms. Does that sound like you? If either does, you could have lactose intolerance.

Lactose is the main sugar in milk and most other dairy products. Your small intestine makes the enzyme lactase to help you digest that sugar. When you’re lactose intolerant, you don’t make enough lactase to digest lactose well.

You can’t cure lactose intolerance, but if you change what and how you eat, you may get rid of your symptoms.

Ease Your Symptoms

Millions of Americans have symptoms of lactose intolerance:

You can use trial and error to find out what foods cause symptoms, and in what amount. And then check in with your doctor for a diagnosis. You may be sensitive to small amounts of foods that have lactose, or you may only have symptoms if you eat a lot of lactose foods. Your symptoms may be severe or mild. Lactose intolerance is different for everyone.

Find the Culprits (Hint: It might not just be dairy.)

Milk and dairy products are the best-known lactose foods, but there are many others. Some nondairy products have a protein called casein, which can have traces of lactose. To avoid symptoms from lactose intolerance, read food labels carefully. When shopping or cooking, look for these ingredients that have lactose:

  • Curds
  • Dry milk solids
  • Milk
  • Milk byproducts
  • Dry milk powder
  • Whey

If you are highly sensitive to lactose, you may need to avoid foods such as:

  • Baked goods
  • Bread, baking, and pancake mixes
  • Breakfast cereals
  • Certain types of candy, such as milk chocolate
  • Instant foods (breakfast drink mixes, mashed potatoes, soups, and meal replacement drinks)
  • Margarine
  • Nondairy creamers (liquid and powdered)
  • Nondairy whipped topping
  • Processed meats (bacon, hot dogs, sausage, and lunch meats)
  • Protein and meal replacement bars
  • Salad dressing

Get a Diagnosis

Your doctor may ask you to keep a diary of the foods you eat, to note when you have symptoms, and to stop eating an offending food to see if your symptoms go away. To help make a diagnosis, some doctors simply look at your symptoms and whether avoiding dairy products for 2 weeks relieves them.

To confirm the diagnosis, your doctor may do other tests, such as:

Hydrogen breath test. Normally, people have very little hydrogen in their breath. If your body doesn’t digest lactose, though, hydrogen builds in your intestines, and after a while it’s in your breath. This test measures how much hydrogen is in your breath after you have a lactose-loaded drink several times in a few hours. If your levels are high 3 to 5 hours later, your body does not digest lactose well.

Lactose tolerance test. When your body breaks down lactose, it releases sugar into your blood. This tests how much sugar is in your blood. After you fast, a small sample of blood is taken. Then, you drink a liquid that is high in lactose. Two hours later, you give another blood sample. Because lactose causes blood sugar levels to rise, your blood sugar levels in this sample should be higher. If you’re lactose intolerant, you’ll have just a low rise in blood sugar. But this test is rarely done any more.

How to Manage Lactose Intolerance

You can’t change how well your body digests lactose, but you can cut or even stop your symptoms.

Talk with your doctor or a registered dietitian who can help you plan a healthy diet that keeps you feeling good. Keep a food diary to help you learn how much (if any) dairy you can eat without having symptoms. Many people don’t need to stop eating all dairy.

If you make small changes in what you eat, you may be able to prevent symptoms by helping your body digest dairy foods easier.

Don’t eat dairy alone. It’s easier for your body to digest lactose when you eat it with other foods. So try having small amounts of milk or dairy foods with meals.

Choose easier-to-digest dairy products. Some people find it easier to digest dairy products like cheese, yogurt, and cottage cheese.

Use lactose-free or reduced-lactose milk and dairy products. You can find dairy products with most of the lactose removed, or lactase added, at many grocery stores.

Switch to dairy-free products. There are many nondairy options, such as almond, rice, or soy milks. Special note about infants and young children: When babies have symptoms of lactose intolerance (although it’s rare in children under age 6), pediatricians advise changing from cow’s milk formula to soy milk formula until the symptoms go away, then slowly adding cow’s milk formula and dairy products back into their diets.

Take a lactase enzyme replacement. These are available over the counter in pills or capsules. Take the advised dose with your first drink or bite of dairy to help prevent lactose intolerance symptoms.

Chances are at some point in your life you’ve experienced diarrhea.

Just how common is it? Well, according to the Canadian Digestive Health Foundation’s latest statistics, over-the-counter retail sales for diarrhea medications and remedies were $50 million in 2008 – nearly double what it was in 2003.

Diarrhea is unpleasant, to say the least, but it’s a digestive problem that can be helped by tweaking your diet.

“First, it’s important to address the underlying cause of the loose bowel movements,” Andrea D’Ambrosio, registered dietitian with Dietetic Directions and spokesperson with Dietitians of Canada, says. “For example, some people are sensitive to certain types of foods like lactose or gluten and may experience loose bowels from consumption. However, others experience ‘acute’ or short-term diarrhea when taking certain medications or undergoing cancer treatment, surgery or even a very anxiety-producing situation.”

So if relief from diarrhea is what you’re looking for, D’Ambrosio offers up a list of five foods that can help with diarrhea management, as well as five foods to steer clear of.

Foods that help

1. Oatmeal

Oatmeal is an example of a food containing soluble fibre and may be helpful in managing diarrhea, D’Ambrosio says.

“Soluble fibre is a type of fibre that works by attracting water and turns it into a gel during digestion,” she explains. “This slows the digestion process and can help adding bulk to bowel movements.”

2. Psyllium husks

Another type of soluble fibre, psyllium husks can also help with diarrhea.

“It is imperative to increase water with soluble fibre to ensure that you don’t alternate to constipation since the fibre needs water to absorb to move through the digestive tract,” D’Ambrosio explains.

3. White rice

White rice is a common food that is generally well tolerated for those with diarrhea.

“The reason is because it is low fibre, which helps in not promoting bowel movements,” she says. “This allows the bowels to form instead of promoting them to move.”

For example, brown rice would be higher in insoluble fibre, D’Ambrosio points out, which helps food pass more quickly through the stomach.

4. Bananas

One serving of banana is actually half a banana, D’Ambrosio explains, and they are a good source of soluble fibre.

“However, it is important to address the underlying cause of the diarrhea,” she says. “For example, if an individual has fructose malabsorption, consuming a whole banana or an over-ripe banana could worsen diarrhea.”

Bananas are also a great source of potassium, she says, which is an electrolyte that needs to get replaced when you’re dealing with loose bowel movements.

5. Broth-based soups

These types of soups are helpful for replenishing the losses of salt and potassium from loose bowels, D’Ambrosio says.

“They are generally well-tolerated by the body and can help in maintaining electrolyte balance which can shift with changes in hydration and output of liquid.”

Foods that may not help

1. Fatty or greasy foods

For some people, greasy foods worsen diarrhea, D’Ambrosio says.

Avoid fried foods, or foods high in fat like bacon, sausage, pizza and pastries which may worsen symptoms.

2. Caffeine

Caffeine acts as a bowel stimulant and may aggravate diarrhea in some, she says.

3. Milk

If someone has underlying lactose intolerance, they will need to determine how much they are able to tolerate without symptoms, D’Ambrosio says.

“For many with lactose intolerance, yogurt and cheese are easier to tolerate because they have lower amount of lactose.”

4. Sorbitol or sugar-free foods

“If an individual has sugar-free candies or gums or certain ‘diet products,’ the sorbitol or xylitol ingredients can act as laxatives and can worsen diarrhea,” according to D’Ambrosio.

5. Bran

If you have diarrhea, you will want to minimize the amount of insoluble fibre, which adds bulk to the stools and promotes movement, D’Ambrosio says.

When to see your doctor

“It is important to note these foods will not cause diarrhea for every individual,” D’Ambrosio warns. “Which is why it’s important to address with your doctor how often you have been experiencing loose bows and when it occurs.”

It’s especially important to address with your doctor if the diarrhea is happening on a regular basis, she adds.

“It is important to discover the underlying cause for your diarrhea and to know that each individual may have different triggers and foods that help.”

If left untreated, diarrhea can lead to dehydration and possibly malnutrition, D’Ambrosio says.

Diet for Diarrhea Only (Infant/Toddler)

The main goal while treating diarrhea is to prevent dehydration. This is the loss of too much water and minerals from the body. When this occurs, body fluids must be replaced. This is done by giving your child small amounts of liquids often. You can also give oral rehydration solution. Oral rehydration solution is available at pharmacies and most grocery stores. Don’t use sports drinks—they are not good enough. In general, for mild diarrhea, the child can continue to eat.

 

If your baby is breastfed:

  • Keep breastfeeding. Feed your child more often than usual.

  • If diarrhea is severe, give oral rehydration solution between feedings.

  • As diarrhea decreases, stop giving oral rehydration solution and resume your normal breastfeeding schedule.

If your baby is bottle-fed:

  • Give small, frequent amounts of fluid. An ounce or two (30 to 60 mL) every 30 minutes may improve symptoms. Start with 1 teaspoon (5 mL) every 5 minutes and increase gradually as tolerated.

  • Give full-strength formula or milk. If diarrhea is severe, give oral rehydration solution between feedings.

  • If giving milk and the diarrhea is not getting better, stop giving milk. In some cases, milk can make diarrhea worse. Try soy or rice formula.

  • Don’t give apple juice, soda, or other sweetened drinks. Drinks with sugar can make diarrhea worse. Sports drinks are not the same as oral rehydration solutions. Sports drinks have too much sugar and not enough electrolytes to correct dehydration.

  • If your child is doing well after 24 hours, resume a regular diet and feeding schedule.

  • If your child starts doing worse with food, go back to clear liquids.

If your child is on solid food:

  • Keep in mind that liquids are more important than food right now. Don’t be in a rush to give food.

  • Don’t force your child to eat, especially if he or she is having stomach pain and cramping.

  • Don’t feed your child large amounts at a time, even if he or she is hungry. This can make your child feel worse. You can give your child more food over time if he or she can tolerate it.

  • If you are giving milk to your child and the diarrhea is not going away, stop the milk. In some cases, milk can make diarrhea worse. If that happens, use oral rehydration solution instead.

  • If diarrhea is severe, give oral rehydration solution between feedings.

  • If your child is doing well after 24 hours, try giving solid foods. These can include cereal, oatmeal, bread, noodles, mashed carrots, mashed bananas, mashed potatoes, applesauce, dry toast, crackers, soups with rice noodles, and cooked vegetables.

  • Don’t feed your child high fat foods.

  • Don’t feed your child high sugar foods including fruit juice and sodas.

  • For babies over 4 months, as they feel better, you may give cereal, mashed potatoes, applesauce, mashed bananas, or strained carrots during this time. Babies over 1 year may add crackers, white bread, rice, and other starches.

  • If your child starts doing worse with food, go back to clear liquids.

  • You can resume your child’s normal diet over time as he or she feels better. If at the diarrhea or cramping gets worse again, go back to a simple diet or clear liquids.

Follow-up care

Follow up with your child’s healthcare provider, or as advised. If a stool sample was taken or cultures were done, call the healthcare provider for the results as instructed.

Call 911 if your child has any of these symptoms:

When to seek medical advice

Call your child’s healthcare provider right away if any of these occur:

  • Belly pain that gets worse

  • Constant lower right belly pain

  • Repeated vomiting after the first two hours on liquids

  • Occasional vomiting for more than 24 hours

  • Continued severe diarrhea for more than 24 hours

  • Blood in stool

  • Refusal to drink or feed

  • Dark urine or no urine, or dry diapers, for 4 to 6 hours in an infant or toddler, or 6 to 8 hours in an older child, no tears when crying, sunken eyes, or dry mouth

  • Fussiness or crying that cannot be soothed

  • Unusual drowsiness

  • New rash

  • More than 8 diarrhea stools within 8 hours

  • Diarrhea lasts more than one week on antibiotics

  • Fever (see Children and fever, below)

Fever and children

Always use a digital thermometer to check your child’s temperature. Never use a mercury thermometer.

For infants and toddlers, be sure to use a rectal thermometer correctly. A rectal thermometer may accidentally poke a hole in (perforate) the rectum. It may also pass on germs from the stool. Always follow the product maker’s directions for proper use. If you don’t feel comfortable taking a rectal temperature, use another method. We are inexorably approaching warm sunny days – the time of trips to the sea, to the village to the grandmother, the time of sandpits, the time of fruits and vegetables, and, unfortunately, the time when our children are most often prone to intestinal infections.

A few important points for mom:
Acute intestinal infections in children are a dangerous disease. And the younger the child, the more dangerous. Every year, OCIs claim the lives of hundreds of thousands of children. The insidiousness of these diseases is that water is simply needed for treatment. Rehydration (replenishment of water losses) and diet therapy are the mainstay of treatment for all AEIs, and most deaths occur in countries where clean drinking water is scarce.

The key symptom in AEI is diarrhea (lat.- expiring) or, more simply, diarrhea. To put it even more clearly: the body loses water and becomes dehydrated. This symptom – diarrhea, will always be present in acute intestinal infections, while the rest – temperature, abdominal pain, flatulence, vomiting, etc., all may be present, and may be expressed to one degree or another.
The causative agents of OCI are microorganisms: viruses, microbes, protozoa. To a small child who is breastfed, they enter through the mouth. Most often, in a contact-household way – from the mother’s hands, a toy, a dummy fell, was lifted – and given to the child.
Once in the gastrointestinal tract, the pathogen begins to actively multiply and release a toxin that “paralyzes” part of the intestinal cells, disables them, and blocks the channels through which these cells “let” water from the intestines into the body.
And what’s going on? All the water that the child drinks is in transit. This is one of the ways of harmful microbial exposure. The second is that the intestine is a tube, in the wall of which there are muscle fibers. The toxins of the pathogens of OCI act in such a way that these fibers contract quickly – quickly (the intestine hums, what we call increased peristalsis) and quickly and quickly drives water to the exit, not allowing it to be absorbed in those parts of the intestine that have not yet been damaged by the microbe.And the third mechanism is directly damage to the mucous membrane. Replenishing the loss of water and salts, and diet therapy is the basis of OCI treatment!

In some cases, heavy artillery will be needed to give specific antimicrobial therapy to defeat the pathogen. It is clear that self-medication for diarrhea is not worth it. How to replace water?

In some cases, it is necessary to replace part of the diet, sometimes go completely and keep the lactose-free mixture for another week or two of the recovery period after infection – this is the time during which the lactase enzyme in the intestine is completely restored.After that, you can gradually return to the usual dairy diet.

But what about older children who have lures or a common table?
This is exactly the moment when industrial lures will just come in handy. They are the basis of diet therapy. If the child receives them, we continue. These are dairy-free, gluten-free cereals, meat purees, vegetable broccoli.Juices can be removed, but fruit puree – especially blueberries – can be left in the diet.
If you went to a common table, during intestinal infections, adjust his diet, taking into account industrial complementary foods. The diet with OI changes – the child eats fractionally, in small portions – the philosophy is the same as when replenishing fluid – so that he does not vomit and does not transit through the intestines. Therefore, we give food fractionally, often in small portions, in a heated form.

I don’t know how it’s in other cities and countries, but I’ve heard many times from mothers a story that can be called a classic: when a mother complained to a pediatrician that the child had diarrhea (by the way, most often it’s not even diarrhea, but the norm for a particular baby on breastfeeding), and if this stool is foamy and green, then the further development of events is something like this: an ambulance call with immediate sending to an infectious diseases hospital, where a bunch of tests will be prescribed for mother and child (sowing milk for sterility, feces tests for dysbiosis), in which they will definitely find the “culprit” – for example, staphylococcus, which lives normally in everyone, but in this situation he “got caught”.And then – the appointment of antibiotics to the child (with a bunch of concomitant pharmaceutical junk), a ban on breastfeeding and the appointment of an artificial lactose-free mixture. If the mother persists, then she will be allowed to breastfeed, but – attention: no more than 1 time in 2 hours and no longer than 5 minutes, and before / after each feeding – a lactose-free mixture is required!

The reasoning behind the ban on breastfeeding is something like this: “If you continue to breastfeed your baby, the disease will get worse and longer”, “If you breastfeed for more than 5 minutes, the baby will overeat and the intestinal flora will not be able to recover”, “Breast milk contraindicated in a child during diarrhea, because lactose is poorly absorbed. “

Let’s figure it out. We will not analyze the situation when the child does not have any intestinal infection, but only has frequent stools (even green) – it is clear that no treatment is needed, except for changing the mode of attachments to the breast (more details here and here).

Consider a situation when a baby really has an acute intestinal infection (AII) caused, for example, by viruses (by the way, this happens much more often than bacterial AII, and antibiotic therapy is not prescribed).

OCIs are one of the serious problems of pediatrics, because they annually claim 3 million lives of children in the world (about 80% of them are children under the age of 2 years). According to official statistics, 50-60 thousand cases of infectious diarrhea in children are registered in Ukraine every year. 20-30 children die annually from acute intestinal infections in our country. The main method of treating AEI of any etiology is the prevention or treatment of dehydration of the child’s body with the help of timely and adequate rehydration therapy (these are measures aimed at restoring water balance).Moreover, it is recommended to give preference to oral rehydration – that is, DRINKING the child (usually with special solutions for oral rehydration). [1]

The World Health Organization believes that in the case of breastfeeding, the baby should continue to receive breast milk. In the global strategy of WHO and Unicef ​​to reduce child mortality [3], WHO experts recommend

Continue breastfeeding with diarrhea and fever in children, regardless of the severity of the condition and the degree of dehydration of the child, and give the necessary rehydration solutions in addition to breast milk.

Chief pediatric infectious disease specialist of the Ministry of Health of Ukraine, Head of the Department of Pediatric Infectious Diseases of the National Medical University, Professor S.A. Kramarev also recommends continuing breastfeeding for intestinal infections in children and explains this in his book [4]: ​​

“Breastfeeding should be maintained despite diarrhea. This is because human milk lactose is well tolerated by children with diarrhea. In addition, human milk contains epithelial, transformable and insulin-like growth factors.These substances contribute to a faster recovery of the intestinal mucosa in children. Also, human milk contains anti-infectious factors such as lactoferrin, lysozyme, lgA, bifidum factor ”.

According to the Order of the Ministry of Health of Ukraine No. 803 dated 10.12.2007

“In case of AEI, a child who is only breastfed does not stop feeding, the duration of one feeding increases, and the frequency of feeding increases.” (This is about “no longer than 5 minutes, no more than once every 2 hours)

That is, breastfeeding should be maintained, despite diarrhea, and the frequency and duration of applications are INCREASED!

Now regarding the duration of the disease.If the child is on mixed or artificial feeding, then at the time of illness it is necessary to replace the usual mixture with a specialized one – with a reduced lactose content and partially split cow’s milk protein. Why?

“In case of impaired absorption of carbohydrates and the development of secondary lactase deficiency against the background of viral, watery diarrhea, there is anxiety, bloating, regurgitation, splashing foamy stools after each feeding.At the same time, the early introduction into the diet of adapted formulas that contain lactose from cow’s milk can worsen the child’s condition and increase the duration of diarrhea. In addition, cow’s milk contains proteins that are allergenic to the baby’s body.

It is not recommended to use soy-based milk formulas in an acute period of diarrhea. Established increased sensitivity of the intestinal mucosa of children to soy protein in diarrhea. This increases the risk of developing proteinaceous enteropathy. “

Please pay attention: for children exclusively breastfed, no lactose-free formulas need to be introduced – they are needed only by those children who received artificial formulas before the infection!

In addition, breast milk, in addition to all the necessary and useful substances, also contains antibodies to this specific pathogen, which caused an intestinal infection in the child. How does this happen? It is known that the mother and the child live in the same microflora – which means that the mother also encountered the same pathogen that caused OCI in the baby – and her body immediately began to produce antibodies from this microbe (or they are already in her blood, if she has encountered him before).And the mother passes these antibodies with milk to her baby. That is, breast milk from food and drink also turns into a cure for this particular disease!
