Once you have them, keloids are notoriously difficult to eliminate and have a very high chance of re-growing once they are surgically cut out. This is because the body is likely to respond in the same exaggerated way to this surgery as it did to the initial injury.

No home remedies have been shown to get rid of keloids once they have formed. But, there are a few things that people can do at home after a skin injury to help prevent keloids from forming or minimize their appearance.

Silicone sheeting or gel

Silicone is one of the most widely used scar remedies, and it has been proven to help shrink some raised and keloid scars. Silicone has a low risk of adverse effects and is easy to use.

Reviews published in Aesthetic Plastic Surgery and the Journal of Cutaneous and Aesthetic Surgery confirm that applying silicone sheeting or gel as advised can provide an effective way to shrink existing raised scars or help keep them from forming.

A review in American Family Physician states that steroid therapy works best on newer keloids and when combined with surgery to remove part of the scar. It can also be used in conjunction with cryotherapy.

Imiquimod cream

This cream is used to treat a variety of skin lesions, including superficial basal cell skin cancers. It appears to work well on the skin after a keloid has been removed. A review in the Journal of Oral and Maxillofacial Surgery found that the cream reduced the chances of a keloid coming back.

Radiation treatment

A newer treatment that is showing promising results involves a short course of radiation therapy, which is given immediately after surgically cutting out the keloids. Several studies in the plastic surgery and dermatology literature show a high rate of cure, or at least improvement, in keloid scars in people treated by this method.

Because keloids are difficult to treat, it is best to take steps to prevent them immediately after an injury, surgery, or piercing. People who are prone to keloids may wish to avoid tattoos and piercings altogether. If someone requires surgery for any reason, they should make their doctor aware of any history of keloids so prevention can begin soon after the surgery.

Though keloids do not pose a health risk, they can be emotionally and mentally damaging. Some treatments are available, but no single treatment works for all. People will need to discuss keloid removal options with their doctor and determine the best course of action to take.

A keloid (say “KEE-loyd”) is a scar that grows bigger and wider than the original injury. Keloids most commonly grow on the breastbone, shoulder, upper chest and back, earlobes, and face.

Keloids do not become cancer. But they can be bothersome or painful enough that you seek treatment. Keloids often grow back after treatment.

It’s possible to prevent a keloid from forming if you take steps to protect the skin after it is damaged.

What causes a keloid?

Keloids can form where the skin is damaged, such as by a surgery cut, a piercing, a burn, chickenpox, or acne. Thick tissue grows up and out from the healing area, making the scar bigger than the original injury. For some people, even a scratch can lead to keloids.

Keloids do run in families, and they rarely grow in light-colored skin. Experts think that keloids may be linked to a gene that is linked to dark skin pigment.

What are the symptoms?

Keloids look like firm, raised, hard scars. They grow larger over time. Their colors vary from slightly pink to very dark.

Keloids can rub against your clothes and become irritated, itchy, or painful. When exposed to the sun, they may turn darker than the rest of your skin. The dark color may stay.

How is it treated?

There is no sure cure for keloids, but treatment sometimes improves how they look and feel. It is common for keloids to grow back after treatment.

When trying to treat a keloid, your doctor may need to use more than one type of treatment. Based on a keloid’s size and location, and how soon it is treated, your doctor may:

  • Freeze it. This is called cryotherapy. It is best used for small keloids, such as from acne. Cryotherapy can lighten the skin.
  • Inject it with medicine.
    • A corticosteroid is the most commonly used medicine for reducing keloids. It is most likely to work well with cryotherapy or right after surgery.

To prevent keloids after a minor skin injury, start treating it right away. This may help it heal faster and with less scarring. Using the following tips to treat the area may help prevent keloid growth.

  • Cover a new wound with a thin layer of petroleum jelly, such as Vaseline, and a nonstick bandage. Hold the bandage in place with tape so that there is even pressure on the wound. Wash the area with soap and water every day.
  • After a wound is healed over, use a silicone gel bandage. Keep even pressure on the area. This may prevent keloid growth. Keep the bandage on the skin for 12 to 24 hours a day for 2 to 3 months. (It takes 3 months for a keloid to grow).

What Is It?

What are keloid scars

Keloids are raised, bumpy, firm or rubbery scars that result from an abnormal healing response to skin injury. Unlike other scars, keloids outgrow the original wound boundaries. This means that an injury as small as a pinprick, such as from an insect bite, spot or ear piercing, can become a large keloid scar. No one yet knows the underlying cause of the abnormal healing that leads to keloid scars; it is an area of ongoing research.

Keloid facts

  • Keloids are composed of thick scar tissue and are the result of an over-production of collagen in the scar during healing.
  • They can continue to grow over a period of months or even years.
  • Their colour can be very different to that of surrounding skin, varying from flesh-coloured to pink, red or dark brown.
  • Keloids are more common in darker skin types.
  • Keloid scars can bleed and become infected.

What treatments are available?

There are a number of treatments for keloid scars.These include:

Conservative management

This involves application of silicone gel or sheets, use of steroid tape or sheets, pressure garments and pressure earring.  A nurse can guide you in how best you can massage and manage your scar during the healing phase.

Intralesional Steroid Injection

Steroid is administered directly into the scar tissue every six to eight weeks. This helps flatten and soften the scar, it helps reduce discomfort such as pain and itchiness. The number of injections needed varies, larger and or older keloid scars require longer period of treatment.

Intralesional Excision (Surgery)

This surgical procedure can be done to reduce the size of larger keloid scars before commencing regular steroid treatment. The scar tissue is removed from within the keloid boundaries, without harming normal surrounding skin to avoid producing further scaring.  This can be done as a minor procedure or as a day case. Further steroid treatment is required following this procedure.

Keloid scars: Advanced therapies and research to break the ‘overhealing’ cycle | Dermatology

For most people, when they cut themselves shaving the wound will become slightly inflamed, scab over, and maybe leave a tiny scar. But for approximately 4% to 6% of patients, the healing process goes too far, resulting in thick, raised scars known as keloids.

Keloids can arise from cuts, burns, chickenpox, acne, and piercings. Patients who develop keloids, might wind up with scars much larger in width and height than the original injury. Over months or years, scars can grow from the size of a pencil top eraser to a grapefruit or larger. 

Patients tend to first notice symptoms between ages 10 and 30, with the 20s as the prime age to develop keloids. The condition can range from mildly annoying to socially limiting. Research suggests that keloids affect patients’ quality of life similarly to psoriasis. While some patients don’t mind their scars, others are very bothered by them, finding it impossible to camouflage facial keloids. Others have told us keloids have compelled them to keep their arms and legs covered.

Scarring can also cause physical issues. Some patients experience limited mobility if the scars cross major joints, such as the shoulders, elbows, or knees. Approximately 75% of patients who keloid have intermittent itching, while about half have scar-related pain. 

My UT Southwestern dermatology colleagues and I are working to identify genetic factors that increase the risk of keloid scarring. Our hope is to get to the root cause of keloids to improve standards of care and potentially prevent excessive scarring.

What causes keloid scars?

Keloids are the result of excessive collagen production. Collagen is a substance produced by a type of skin cells called fibroblasts. These cells are found in the dermis, the deeper level of the skin. Patients who keloid have more dermis – and more cells that generate dermis – than patients who do not keloid.

In other words, their bodies ‘overheal’ with excessive scar tissue.

Classification of cicatricial formations

Excessive scars, subdivided into:

  1. Hypertrophic.
  2. Keloid.

They are a common and overlooked problem. Clinical hypertrophic and keloid scars vary in size and location relative to the original wound.

Features of hypertrophic scars

Hypertrophic scars are red in color, rise above the level of the skin and rarely itch. They can be linear (for example, surgical and traumatic scars), or widespread (for example, burn scars).Common hypertrophic scars are located in the upper back of the anterior chest wall and at the site of vaccination with the Calmette-Guerin vaccine (BCG). Such scars are dense and rise above the level of the skin, but rarely – by more than 4 mm. Most often, they appear at the site of tension in the wound.

Wounds located along the lines of least stretch of the skin usually heal normally, but those located elsewhere are subject to tension and are prone to pathological scarring. Scars that extend over the joints or are localized in the folds of the skin are particularly prone to hypertrophy.Hypertrophic scars often widen over time due to the tension in the wound.

A hypertrophic scar that develops after surgery usually forms within a few weeks after surgery, increases in size within 3-6 months and then, after a static period, undergoes regression and matures within 2 years. Redness and irritation pass, and as it develops backward, the scar becomes softer, and its surface becomes uneven as a result of contraction.

A mature hypertrophic scar, which has acquired its final appearance, appears to be convex and flagellated. A common serious consequence of hypertrophic scar formation is contracture. Contracture occurs as a result of an incorrectly proceeding continuously contractile wound healing process. The scar is shortened, which increases the tension in the muscles, tendons and ligaments, and limits the mobility of the corresponding part of the body. Contracture usually occurs on the flexor surface of the joints.

Characteristic signs of keloid scars

Keloid scars most often form on the auricles, shoulders, chest, forearms and upper back, they can be of different sizes in diameter up to 30 cm or more. These scars can develop over several months or even years after the injury. They sometimes develop after inflammatory conditions such as chickenpox and folliculitis. Sometimes a keloid scar can appear spontaneously, without prior injury.Keloid scars appear in the form of dense rounded growths of irregularly shaped scar tissue, outgrowing the original boundaries of the damage, but rarely penetrating the subcutaneous fat layer.

The growth of keloid scars in the direction of the surrounding tissues depends on their location. For example, a scar located on the auricle often grows in the form of large lobes. And keloid scars that appear in the middle of the chest are usually butterfly-shaped. In consistency, scars can range from soft and loose to as hard as tough rubber.

Keloid scars are more difficult to treat than hypertrophic scars, and they are more prone to recurrence after surgical treatment; recurrence of keloid scars can occur within 2 years after their removal by surgery.
