You may be reluctant to exercise for fear that it’ll aggravate your symptoms, but research shows that regular moderate exercise lessens pain and improves function.

While the pain and fatigue associated with fibromyalgia may make exercise and daily activities difficult, it is crucial to be physically active. Research has repeatedly shown that regular aerobic exercise improves pain, function and overall quality of life.
Won’t exercising make my pain flare up?
You may be reluctant to try exercise for fear that it will make your pain worse. But starting low and going slow helps keep symptoms from flaring up. You may want to start with walking two minutes a day and gradually work your way up to 30 minutes two or three times a week.
It’s crucial to pace yourself. If you do too much on your good days, you may have more bad days. If an exercise causes you increased pain, reduce the time or intensity of that exercise next time.
What type of exercise should I do?
Different types of exercises can help in different ways. Examples include:
- Flexibility exercises. Stretching exercises can reduce stiffness and improve range of motion. They can be a good way to start and end your exercise routine, to help prevent strain injuries.
- Strengthening exercises. Strong muscles help support your joints and can help reduce fatigue. Your muscles get stronger when they push or pull against a force, such as pulling on elastic bands or lifting weights.
- Aerobic exercises. This type of activity increases your breathing and heart rate, and can improve your stamina. Low-impact aerobic exercises — such as fast walking, biking or swimming — appear to be the most helpful to people with fibromyalgia.
Is water exercise a good choice?
Many people find water exercise to be easier on their joints because the buoyancy of the pool water reduces weight-bearing stress. Some types of these exercises use the resistance of the water to improve your strength and balance. Exercising in warm water can also help relax your muscles and reduce pain.
What are some other options?
Although they are less studied, mind-body practices may help improve symptoms and overall well-being. Yoga and tai chi are practices that combine meditation, slow movements, deep breathing and relaxation. Both have been found to be helpful in controlling fibromyalgia symptoms.

Fibromyalgia and Weight Gain
A particularly frustrating problem for many fibromyalgia patients is the tendency to gain weight. We eat less but notice little, if any, difference. And it’s difficult to exercise more because of the pain. According to Mark Pellegrino, MD, in his book Fibromyalgia: Up Close and Personal, it’s not unusual for someone to gain 25 – 30 pounds the first year after developing fibromyalgia.
The Link Between FM and Weight Gain
Why does fibromyalgia trigger weight gain in so many people? There are a number of contributing factors:
Lack of Sleep. One of the primary symptoms of FM is the inability to get the deep, stage 4 restorative sleep we need. Recent studies have shown that people who do not get adequate amounts of sleep are more likely to gain weight. Lack of sleep causes a reduction metabolism and an increase in appetite, particularly a craving for high-carb and high-sugar foods.
Neuroendocrine Abnormalities. Research has revealed several hormone deficiencies in fibromyalgia, such as serotonin, growth hormone, cortisol and thyroid, which can contribute to decreasing the body’s metabolism. There may also be an increased sensitivity to insulin, causing the body to go into a fat-storing mode.
Decreased Activity. Because of their level of pain, most FM patients are not able to increase their exercise to the levels needed to increase metabolism rates and burn the calories required for significant weight loss.
Medication. A number of medications commonly given to treat the symptoms of FM — particularly antidepressants — have the side effect of weight gain.
Weight-loss Strategies
Since our pain makes it difficult for us to increase our exercise enough to burn more calories and our slow metabolism means eating less doesn’t usually help much, what can we do to lose weight? Dr. Pellegrino has developed a food plan designed to improve our metabolism and calorie-burning abilities by providing us with the right “quality” of food. He recommends:
Eat a High-protein, Low-carbohydrate Diet. Focus on good proteins like lean meats, eggs, dairy products, tofu, soy meat substitutes and legumes. Limit carbohydrate intake to vegetables and fresh fruits. Include good fats, such as plant oils, fish oils, almonds and avocados. If you need a sweetener, use a natural sweetener like Stevia or Xylitol. Avoid sugar and other sweets, breads and pastas, rice, potatoes, partially hydrogenated oils, carbonated drinks, and alcohol except in moderation.
Eat Proteins First. Never eat carbohydrates — even good carbs — by themselves. Always eat a protein first. When you eat a protein first, the protein digestive enzymes are activated, which then slows the absorption of carbohydrates.
Eat Until You’re Full. At meals, eat until you’re full, but not stuffed. Eat slowly, chewing your food well. You should eat five or six times a day. It can be three regular meals and two small snacks, three small meals and two larger snacks, or five small meals. Experiment and see what works best for you. If you have irritable bowel syndrome, you may find that you do better eating small portions more frequently.
Give Yourself a Break. Follow this diet strictly Monday through Friday, then allow yourself to splurge a little on the weekends. Knowing you can indulge a little on weekends should make it easier to stick to the diet through the week.
Check With Your Doctor
Before starting any new diet or exercise program, it is always a good idea to check with your doctor. You might also want to talk with your doctor about testing you for things like thyroid, yeast or fungal overgrowth (Candida), hypoglycemia, and hormone deficiencies, since these things can also contribute to weight gain.

Symptoms of fibromyalgia
Most often, when complaining about the condition, the patient says: “everything hurts.” This is because pain can occur anywhere in the body. This, in fact, is the main symptom. It is usually dull and aching and lasts for three months. In this case, painful sensations can change the location. In addition, the patient also complains about:
- Constant fatigue and rapid fatigue, restless sleep;
- memory problems and difficulty concentrating;

Disease diagnosis
Fibromyalgia is difficult to diagnose because many diseases have similar symptoms. For example, hypothyroidism, vitamin D deficiency, hypercalcemia, polymyositis (muscle disease), Paget’s disease (bone disease), as well as oncology and infectious diseases (hepatitis, AIDS, etc.). Therefore, research and advanced analyzes are carried out, first of all, to exclude these pathologies. Specifically, there are no separate methods for establishing a diagnosis of fibromyalgia; it can be suspected based on the patient’s complaints and his physical examination.Experts identify 18 tender points at the attachment points of the tendons, which can cause pain when pressed. Treatment methods

Neuralgia, muscle pains, parasthesia, irritation of the intestines and bladder are my lifelong companions from adolescence.However, these states did not torment me much, since one was rarely superimposed on the other, and relief came after a short time. To this were added unpleasant paraesthetic disorders of the hearing system (ringing, stuck, burning sensation in the ears, most often right), the back of the head (a feeling of goose bumps), tension and pain of the facial muscles, sometimes the hands become numb and tingling. Betamethasone injections help to alleviate the symptoms, the organic cause of all these disorders was found.Rheumatologists, traumatologists, neurologists, neurosurgeons shrug their shoulders and just prescribe to try new drugs. It gets much worse after physical therapy. Yoga did not help either. All this was superimposed on irritable bowel syndrome and pain in the lower back, radiating to the muscles of the legs, with a tingling sensation and numbness in the feet. Unpleasant and painful sensations at the level of different ligaments and joints. I went through an additional gynecologist and endocrinologist, a course of antibiotics and high doses of vitamin D. I have always been skeptical about the existence of such a disease, but from personal experience I can say that endless pains and problems with digestion and discomfort in the pelvic organs, in addition to all these paraesthetic disorders, myalgias and arthrialgias, all this greatly disrupts performance and mood …
The generally accepted protocol for the treatment of fibromyalgia is Antidepressants and Lyrica (the active ingredient is Pregabalin). I cannot morally accept such treatment.Once in despair, I tried pregabalin, but quit before reaching the required therapeutic dose due to side effects.
Betamethasone shots and corticoids (methylprednisolone / prednisone) help me, this is strange for my diagnosis. Corticosteroids have not been scientifically proven to relieve the symptoms of fibromyalgia. The first reaction is severe drowsiness, impaired reaction and coordination, decreased mood. Such a dose, of course, did not relieve any pain.
The next appointment is Valium (active ingredient diazemam), treatment protocol 5 mg once a day for 2 weeks. The reaction is excellent, the effect is long-lasting, it is enough for me to take it every other day. But it is not recommended to use it for a long time. You need to pause.
And in these pauses I decided to give a second chance to Sirdalud, his side ones are still easier than those of Lyrica.I decided to try the dose escalation route, starting at 1 mg per day, and building up 1 mg every day until I got to the correct dosage of 6 mg per day. And only from 6 mg per day the pain becomes less. 6 mg Sirdaluda have less analgesic effect than 5 mg Valium, and more side effects. With a gradual increase in the dose, the manifestations of drowsiness became less, but during work and driving you notice how much worse the reactions and coordination become, which does not happen while taking Valium.Another side effect that came to light after a two-week intake was skin reactions in the form of a rash on the chest, increased acne, also dry mouth and constant thirst, slight irritation of the gastrointestinal tract. With a decrease in the dose, the pain begins to intensify.
It seems that today there are no ideal drugs, so you need to weigh the benefits and adverse reactions well. I have not tried antidepressants, but I have tried Lyrica, Gabapentin, Tramadol, all sorts of non-steroidal anti-inflammatory drugs, and so far my choice has settled on Sirdaluda and Valium.
Therefore, I will recommend Sirdalud to those who suffer from moderate to severe muscle pain, but I take two points off due to side effects.

Duloxetine for the treatment of painful neuropathy, chronic pain or fibromyalgia

Does duloxetine work to treat pain generated by nerves when damaged by disease or pain caused by fibromyalgia?
Relevance
Duloxetine is a medicine used to treat depression and urinary incontinence (urinary leakage) and may also be helpful for certain types of pain.Pain can occur spontaneously when there is damage to the nerves that carry pain information to the brain (neuropathic pain). When nerve damage occurs outside the spinal cord, it is called peripheral neuropathy. Another type of pain, nociceptive pain, occurs when nerves sense damage to other tissue (such as a pinprick in the skin). Some types of pain of unknown origin occur without visible damage to the nerve or tissues. This type of pain occurs, for example, with fibromyalgia. The aim of this review was to evaluate the benefits and harms of duloxetine for the treatment of neuropathic pain and chronic pain of all types.
Characteristics of research
We reviewed all published scientific literature and found 18 trials with a total of 6407 participants that were of sufficient quality to be included in this review. Eight trials examined the effect of duloxetine on painful diabetic neuropathy and six on pain associated with fibromyalgia. Three trials examined the painful physical symptoms associated with depression and one small study looked at duloxetine for pain in stroke or for spinal cord disease (central pain).
Key results and quality of evidence
The usual dose of duloxetine is 60 mg. There was moderate quality evidence that duloxetine at this dose reduces pain in painful diabetic peripheral neuropathy and fibromyalgia.
