Pain management – types, causes, treatments

Pain is one of the most common symptoms in all areas of medicine. It can cause severe pain to the patient, disturbs sleep, does not allow to engage in work and daily activities, fully communicate with loved ones, reduces the quality of life and the effectiveness of the treatment of the underlying disease.

Currently, there is a special direction in world health care – pain medicine . It studies the causes and mechanisms of pain, methods of effectively dealing with them. This is a very rapidly developing field of medicine. In the West, the term pain management – “pain management” .

In modern medicine, a concept has been adopted that can be briefly described by the phrase “without pain. ” The patient must not endure pain. It needs to be eliminated, and for this modern doctors have many options, starting with a wide range of medications and ending with special surgical interventions.

Why does pain occur?

Pain is a universal response of the human and animal body to damage to the body, discomfort at the physical and emotional levels. All causes of pain can be divided into three large groups:

  • Injuries: wounds, fractures, dislocations, burns, bruises, etc.
  • Various diseases. Most often, patients turn to doctors with complaints of headaches, pain in the neck, back and lower back, joints. Pain syndrome is accompanied by chronic diseases of internal organs and the nervous system, infections, malignant tumors. Almost every adult has experienced a toothache at least once in their life.
  • Surgical interventions. Any surgeon can tell you how important high-quality pain relief for patients in the postoperative period is.

In some areas of medicine, pain management is a particularly pressing issue. For example, in oncology, where many patients with advanced malignant tumors experience chronic excruciating pain.

What are the types of pain?

First of all, pain is divided into acute and chronic . Acute pain occurs in response to a single tissue injury, usually does not last long and responds well to treatment. Usually the cause of acute pain is obvious. Typical examples: broken bone, cut finger.

Chronic pain is called pain that bothers for more than three months. It is not always possible to immediately understand its causes, and it is often difficult to cope with it.

If pain is caused by irritation of nerve endings in internal organs, bones, muscles, it is called nociceptive (from the word “nociceptor” – the so-called pain receptors). When the nervous system is affected, neuropathic pains occur . Examples of neuropathic pain include interstitial cystitis, irritable bowel syndrome, and fibromyalgia.

There are such common types of pain as:

  • Headaches: migraine, vascular, cluster, tension headaches.
  • Radicular – as a result of irritation of the nerve roots.
  • Sciatica – with damage to the sciatic nerve.
  • Phantom pain occurs in amputated limbs.
  • Pain in malignant tumors – occurs as a result of various causes.
  • Psychogenic pain is diagnosed when, during the examination, the patient does not find a disease that could cause pain.

By the nature of the pain, they are very different: aching, pressing, stabbing, shooting (sometimes the patient describes his sensations as an “electric shock”), burning, “dagger”.

According to statistics, women visit doctors with complaints of pain more often than men

Modern methods of pain management

In order to prescribe effective pain relief, the doctor must understand the causes of pain, correctly assess its intensity, mechanisms of occurrence, and the psycho-emotional state of the patient. There are many different methods of pain management, in parallel, it is always necessary to treat the underlying disease. Usually anesthesia is prescribed by the attending physician: neurologist, surgeon, traumatologist, etc.

The SOVA clinic employs experienced medical specialists who know how to quickly and effectively deal with various types of pain syndromes. Book an appointment with our doctor.

It is important for the patient to understand the goals of treatment. Often it is impossible to completely eliminate chronic pain, and then the doctor will try to reduce it as much as possible.

Medications

The most commonly used drugs for pain management are from the group NSAIDs (non-steroidal anti-inflammatory drugs) : ibuprofen, analgin, naproxen, diclofenac, aspirin, ketorol, and paracetamol. According to indications, glucocorticoids (drugs of hormones of the adrenal cortex), antidepressants, tranquilizers, sleeping pills and other drugs can be included in the treatment regimen.

Severe pain that cannot be managed with NSAIDs is treated with narcotic analgesics.

For example, experts from the World Health Organization (WHO) have developed a special pain relief ladder for cancer patients:

  • Mild pain is treated with NSAIDs.
  • Moderate pain – weak narcotic analgesics in combination with NSAIDs.
  • Severe pain – with powerful narcotic analgesics.

If the treatment at the previous stage ceases to help, they move on to the next.

IMPORTANT! Any pain medications, even over-the-counter medicines, can cause side effects. The more often and in larger doses you take the medicine, the higher the risk that negative effects will occur. If the pain bothers you for a long time, you should not self-medicate. Contact your doctor.

Blockades

In some cases, when drugs from the NSAID group do not help, pain can be stopped with the help of blockades. During this procedure, the doctor “chips” the place, which is the source of pain, with an anesthetic and / or glucocorticoid solution. Usually after that the pain passes quickly, the effect can last up to several months. Most often, blockades are used for pathologies of the spine, lesions of the nerve roots.

Non-drug methods

Many non-drug methods help reduce pain, thanks to them you can improve the patient’s condition and reduce the dose of drugs:

  • Exposure to heat or cold. A common example is an ice pack that is applied to the site of a bruise.
  • With pains caused by some reasons, therapeutic exercises, walking, stretching exercises, yoga help to cope.
  • In diseases of the musculoskeletal system are effective: massage, osteopathy, manual therapy.
  • Acupuncture (acupuncture) is an alternative medicine method, but some studies have shown that it has an analgesic effect.
  • Physiotherapy – eg electrophoresis with drugs, magnetotherapy.
  • Transcutaneous Electrical Nerve Stimulation (TENS) is a procedure during which special electrodes are applied to the skin and a weak current is applied.

Surgical methods

In cases where the above methods of treatment do not help, doctors may suggest some surgical interventions:

  • Neurolysis – removal of the scar and other pathological formations that impair nerve function.
  • The nerve can be destroyed by electrocoagulation or chemicals, so that it can no longer conduct pain impulses.
  • Stereotactic surgery is a procedure during which a precisely focused radiation is applied to the problem area.
  • Special methods of stimulation of the brain and spinal cord using electrodes.

Psychotherapy

Pain can be reduced by working with the psycho-emotional state of the patient. Psychotherapists help with this. Cognitive behavioral therapy has been shown to be effective in some cases. During the sessions, the specialist helps the patient get rid of negative mindsets, change the perception and attitude towards pain.

As you can see, doctors have a large arsenal of tools to help keep pain under control.
