Anybody who has suffered, or is suffering from, scalp psoriasis will know that it’s very difficult to deal with. One day you might think your scalp is healthy, the next – for an undefined reason – your relationship with the autoimmune disorder becomes closer than you’d like. 

“Psoriasis is an inflammatory dermatosis that affects between two and five per cent of the UK population,” explains Tracey Walker, director of education at the Institute Of Trichologists. “Scalp involvement is common and sometimes it is only seen there. It can appear at any age but does have peaks in the teens and in the sixties.”

“Scalp psoriasis is an inflammatory condition that affects the scalp skin causing the excessive and rapid growth of skin cells. Psoriasis scalp applications

Coal tar scalp applications are typically available over the counter without a prescription. Scalp applications containing vitamin D analogues or steroids typically require a prescription from your doctor.

If you’re still willing to experiment or aren’t happy with your current choice of shampoo, this guide is for you!

Finding the right shampoo can alleviate those more intense symptoms and make it easier to enjoy the day or sleep. In this guide, we look at the best psoriasis shampoos and their key attributes. Tar-Based Shampoos

Coal tar is one of the oldest and most commonly used treatments for psoriasis. Tar works by:

• Slowing the growth of skin cells, preventing some of the excess buildup

• Don’t use conditioner afterwards. If your hair is prone to dryness, try to find a coal tar shampoo with added coconut oil or other oils.
• Read the instructions on the bottle regarding how often to use the coal tar shampoo. You can use normal shampoo on off days if you like.
• If your doctor says it’s right for you, you can use a moderate-strength topical steroid solution overnight after using your coal tar shampoo. This is often an effective combination.

Efficacy of Iralfaris shampoo in the treatment of scalp psoriasis: a videodermoscopy evaluation prospective study in 70 patients

Aim:

This work has the aim to test the sensibility of VSCAPSI method in the evaluation of effectiveness of a medicated shampoo for the treatment of scalp psoriasis. Psoriasis is a chronic inflammatory skin disease histologically characterized by proliferation and loss of differentiation of keratinocytes, angiogenesis with vasodilatation and increased permeability, and inflammation. Scalp involvement is a common clinical feature of psoriasis, that is present in the 25% of patients who suffer of it. Videodermoscopy (VD) permits a magnified view of the surface components of the epidermis and papillary dermis, which are not visible to the naked eye, together with the ability to capture digitally the viewed images and to store them for later use. Moreover videodermoscopy is a non-invasive technique, used to analyze cutaneous peripheral microcirculation. Therefore VD could be an useful tool in evaluating the efficacy of treatments for scalp psoriasis. The clinical benefit of currently available medicated shampoos for the treatment of scalp psoriasis is restricted, due to their limited efficacy, low cosmetic appeal and safety and tolerability problems. Therefore effective and safe products are needed especially for the long term management of scalp psoriasis. A specific shampoo designed for the scalp hygiene in psoriatic patients has been recently developed. This shampoo contains urea, glycolic acid, salicylic acid, icthyol pale and laureth 9 (polidocanol). Aim of the study was to evaluate in a 12-week prospective monocenter, open-study the efficacy and tolerability of an emollient, keratolytic shampoo (Iralfaris shampoo ISDIN, Barcelona; Ir-S) applied three times a week in patients with scalp psoriasis. The efficacy of the shampoo has been valuated with VSCAPSI.

Methods:

Seventy subjects with mild to moderate/severe scalp psoriasis were enrolled in the trial, after their informed consent. Efficacy was assessed using a specific and validated videodermoscopy scalp psoriasis severity index (VSCAPSI) score, performed at baseline, after 45 and 90 days. Patients were evaluated for itching.

Results:

VSCAPSI score at baseline was 8. 5. Ir-S induced a significant reduction of VSCAPSI score in comparison with baseline value both after 45 and 90 days of treatment. The use of shampoo lead to a progressive reduction of VSCAPSI score getting a score of 4.2 at T45 and a score of 4.0 at T90 (P=0.001 vs. baseline). The use of Ir-S has significantly reduced the percentage of patients reporting itching sensation.

Conclusion:

The treatment was safe and well-tolerated with an high cosmetic acceptance. Ir-S is a good alternative to other medicated shampoo in the treatment of mild to moderate scalp psoriasis. Moreover, in the treatment of severe scalp psoriasis, it can lead to an improvement if associated with topical medications.

Etrivex Shampoo is the first and only short contact formulation developed for the treatment of scalp psoriasis. The shampoo contains 0.05% clobetasol propionate as the main component, as well as cleansing, foaming, conditioning and other components that allow you to effectively use the healing power of IV generation glucocorticosteroids, providing cosmetic hair care.The article presents its own successful experience of treating 15 patients with psoriasis of the scalp.

Etrivex shampoo is the first and the only short contact drug, developed for the treatment of scalp psoriasis.The main component of the shampoo is clobethasole propionate (0.05%), other components are cleaning and foamy agents, conditioners, etc., due to which the therapeutic efficiency of fourth-generation glucocorticoids is fully utilized in parallel with cosmetic care of the hair. Etrivex shampoo was effectively used in therapy of our 15 patients with psoriasis of the hairy part of the head.

The lesion of the scalp presents particular difficulties for the choice of topical therapy, and the presence of rashes on open areas of the skin and the aesthetic unattractiveness of lesions significantly reduce the quality of life of patients [1]. One of the most common chronic inflammatory diseases of the scalp is scalp psoriasis and seborrheic dermatitis. Up to 3% of the world’s population suffers from psoriasis, i.That is, about 80 million people, with the same frequency of women and men, and seborrheic dermatitis affects 1 to 3% of all immunocompetent adults (more often men than women) [2].

According to the literature, the incidence of psoriasis of the scalp in comparison with other dermatoses of similar localization ranges from 40 to 80%. In about 80% of patients with psoriasis with lesions of smooth skin, lesions of the scalp were also observed, however, in 25% of cases of psoriasis of the scalp occurs in isolation [1, 2].

Psoriasis is one of the most common chronic inflammatory systemic dermatoses with predominant skin lesions and, probably, genetically determined immune disorders, which are realized under the influence of a number of trigger factors, manifesting an inflammatory process in the dermis, accompanied by persistent impairment of proliferation and morphological differentiation of keratinocytes [1].

The average area of ​​the scalp is about 700 cm2, while the average surface area of ​​the hair in women is about 6 m2, and in men – about 1 m2, in addition, the anatomical and physiological features of the skin determine a certain specificity of the development of psoriatic eruptions in this zone …Diagnosis of the disease is based on the characteristic clinical manifestations: an abundance of sharply delimited erythematous-squamous rashes of a silvery-white color with clear boundaries, raised, constantly itchy and abundantly flaky, infiltrated large plaques and chalk-

papules, often significantly extending far beyond the boundaries of hair growth (spreading to the skin of the face, temporal region, auricles, eyebrows, behind the ear region and chest). The rash is usually symmetrical. One of the features of the clinical manifestations of psoriasis on the scalp is a diffuse lesion of large areas of the skin, accompanied by abundant peeling, and there may not be typical efflorescences. However, in most cases, a characteristic clinical feature of the lesion of this zone is the presence of a scalloped edge of the affected area, which is well defined by the border of hair growth on the forehead and temporal areas and is a bright pink, sharply delimited, garland-shaped corolla of psoriatic plaque protruding above the level of healthy skin.On palpation of these elements, a positive “step” symptom is determined, which distinguishes the process from seborrheic dermatitis. Depending on the nature and volume of the spread of rashes on the scalp, there are 3 degrees of severity of the process (Table 1).

Effect of scalp psoriasis on the quality of life of patients

Features of the psychoemotional state of this category of patients have been studied for a long time, and today there are two views on the role of mental factors – psychogenicity in the genesis of the development and progression of dermatosis.On the one hand, they can act as trigger provoking factors in the manifestation of exacerbations of psoriasis, on the other hand, for most patients the very fact of the presence of a skin disease is perceived as a severe mental trauma, especially when the process is localized in visible areas of the skin, since it imposes significant restrictions on the usual lifestyle, professional activity, social activity [3]. It is known that in almost 48% of patients psoriatic eruptions occupy most of the head (scalp, face, ears) and 82% of patients suffering from dermatosis experience a constant feeling of discomfort [3-5]. Constant irritation and flaking of the scalp determine the unpresentable appearance of patients, which causes deep social isolation and maladjustment. Problems with work are noted in 75% of cases [6], persistent depression – in 54% of cases [3, 7], decreased sexual activity – in 50%, suicidal thoughts – in 20% of cases [7].

Treatment of patients with psoriasis of the scalp presents great difficulties, since, in addition to these features, the process is characterized by a more pronounced inflammatory component than on smooth skin [8].

Thus, in psoriasis of the scalp, there is a twofold increase in the production of interleukin-8, defensins P4, calcium-binding protein S100 by hair follicles (Table 2).

It has been established that three members of the S100 family of proteins are involved in the work of the body’s nonspecific defense system. Proteins S100 – a group consisting of more than 20 calcium-binding proteins with low molecular weight, the molecules of which contain domains that bind Ca2 + ions.The S100 protein genes are located on chromosome 1 (C21). Two proteins – S100A8 (calgranulin A; MNR8), S100A9 (calgranulin B; MRP14) – are expressed in granulocytes, monocytes and macrophages at the early stages of their differentiation, and one protein – S100A12 (calgranulin C) – is localized in granulocytes. cell cytoplasm, are involved in the regulation of processes that determine cell differentiation, the exchange of fatty acids (including arachidonic), the functioning of the cytoskeleton associated with phagocytic activity and migration of immunocytes.

Having sufficient efficacy, almost all topical agents for the treatment of psoriatic efflorescence of the scalp can be used by the patient independently, however, the undoubted problem is the need for their long exposure.The surface of the scalp is difficult to access for drugs, therefore, the application of traditional ointments and solutions of long-term contact requires compliance with certain household restrictions and conditions, the violation of which reduces the effectiveness of therapy and leads to patients refusing treatment. Low cosmetic acceptability of most of them is one of the most unpleasant aspects of therapy, adversely affecting their compliance [4].

The existing drugs are quite diverse, however, topical glucocorticosteroids (TCS) are first-line drugs and are included in the clinical guidelines for the management of patients with this dermatosis.An urgent problem in our country is the uncontrolled use of TGCS of a strong class of long-term contact, due to which their atrophogenic properties can be realized.

Analogs of vitamin D3, salicylic acid, tar, zinc and drug combinations are widely used, many of which are fatty and have an unpleasant pungent odor, staining and irritating substances [1].

Preparations of vitamin D3 analogs (cal-cypotriol) are characterized by a slow onset of action, possible development of skin irritation at the application sites (erythema, itching and burning sensation), hypercalcemia and hypercalciuria, and can also lead to persistent skin hyperpigmentation. In addition, being inactivated at low pH, they do not combine with the whole

by a number of traditional topical preparations, primarily with salicylic acid, which complicates their use [9]. Zinc preparations with prolonged use cause excessive dryness of the skin at the sites of application.Tar preparations have an unpleasant specific odor, a potential carcinogenic effect, and can also color the hair, scalp and clothing [9]. In this regard, 54.6% of patients are not satisfied with the results of traditional therapy using these agents and are always looking for more effective and cosmetically acceptable topical agents [2].

The methods of phototherapy (UV and PUVA) are often accompanied by phototoxic reactions (erythema, severe pruritus, hyperpigmentation), in addition, their potential carcinogenic effect is actively discussed [9].

Thus, experiencing significant inconveniences and not getting the quick desired effect from standard topical therapy, patients do not adhere to the prescribed treatment regimen in 40-73% of cases and in 73% of cases express concern about possible adverse events associated with the use of long-term contact with TCS [3 ].

In this regard, the search for highly effective topical drugs, the properties of which would not reduce the adherence of patients to therapy, seems to be very relevant in modern clinical practice.A modern topical agent must be highly effective and safe

at minimal exposure, easy to apply to the scalp and has optimal cosmetic properties in the form of colorlessness and odorlessness [4]. In addition, the possibility of long-term therapeutic and prophylactic care of the scalp during remission is of great importance for the patient, which will allow them to control the disease, determining their quality of life [7].

The modern algorithm for the treatment of scalp psoriasis according to the recommendations of the European Academy of Dermatology and Venereology (2009) includes the use of short contact formulations in the form of shampoo [1] (Fig.1).

The most important active substances in the treatment of scalp psoriasis are GCS. Strong evidence of efficacy has been obtained with the use of clobetasol propionate, a topical agent belonging to the class of very potent drugs in this group [1].

No-forming, conditioning and other components that allow you to effectively use the healing power of IV generation GCS, providing cosmetic hair care (tab.3).

A number of comparative studies have shown that the efficacy of clobetasol propionate shampoo is statistically significantly higher than the efficacy of calcipotriol solution (p = 0.016) and 1% tar shampoo (p <0.001) [4]. By the end of the course of therapy, 84% of patients showed a significant improvement or improvement in their condition (a decrease in the initial DISHS index by more than 50%). The most pronounced positive dynamics was observed in relation to such clinical symptoms of the disease as itching and flaking. So, by the end of the 2nd week of therapy 90%

patients noted the complete absence of itching and flaking. From the point of view of subjective perception of the effectiveness of treatment and ease of use of the drug, all patients gave a good or excellent assessment of this form of the drug.

We included 0.05% Etrivex shampoo in the course of treatment of 15 patients diagnosed with psoriasis vulgaris with lesions of the scalp as a mono-means of topical therapy. All patients (9 women and 6 men, mean age 29.2 ± 2.4 years) had typical psoriatic eruptions of mild and moderate severity, as well as not only a long history of the disease (more than 5 years), but also extensive experience in the use of various methods of treatment of the scalp. In addition, 6 of 15 patients had typical eruptions of psoriasis vulgaris on the smooth skin of the trunk and extremities, and 1 patient had psoriatic arthritis. Treatment with shampoo was carried out daily for 4 weeks with control of the dynamics of DITTTS

Method of application of shampoo Etrivex:

1. Shampoo is applied with massaging movements on dry scalp once a day, thoroughly rubbed into the affected skin.

2. Incubate for 15 minutes without covering the head.

3. Foam and wash off with water and / or regular shampoo (with neutral pH) (Fig. 2).

By the end of the 2nd week of topical therapy with Etrivexom, 80% of patients with moderate severity of the process showed a significant clinical improvement. The most pronounced positive dynamics was observed in relation to such clinical symptoms as hyperemia, infiltration, itching and desquamation (Fig. 3, a, b), while in patients with mild severity, the regression of rashes was almost complete. In general, by the end of the 2nd week of course therapy, 90% of patients noted a complete absence of itching and peeling. After carrying out the recommended therapeutic course of topical therapy with Etrivex, all patients are recommended a maintenance course with a regimen of application 2 times a week. The majority of the patients we observed (12 out of 15) continued to use the medicinal shampoo as maintenance therapy 2 times a week, while we did not observe fresh rashes for the next 3 months (Fig.4, a, b). Currently, dynamic monitoring of patients continues, however, patients already note an extension of the remission period against the background of prophylactic use of Etrivex, in contrast to previously used methods.

In terms of the compliance of the medicinal product in the form of a shampoo, all 15 patients gave a positive assessment of the form and efficacy of the medicinal product. In addition, some patients noted the positive effect of the shampoo on the rapid regression of elements that were accidentally exposed to water when it was washed off (hands, neck, décolleté and back).

It was due to the short exposure time (15 minutes per day) on the skin of class IV tHCS that a pronounced clinical improvement was achieved. During treatment and during the observation period (4 months), we did not register cases of skin atrophy and telangiectasias, which are often complications of therapy with long-term contact with TCS [4].

The use of active medicinal products in the form of shampoo in topical psori therapy regimens –

daily application of the drug should not exceed 4 weeks. As a maintenance therapy, the shampoo should be applied 2 times a week for 6 months to maintain the therapeutic effect and prevent relapses.In the event of exacerbations against the background of maintenance therapy, repeated courses of treatment with daily application of Etrivex shampoo can be carried out.

Thus, Etrivex shampoo, containing 0.05% clobetasol propionate, is the first effective and cosmetically adapted short contact therapeutic agent for the treatment of scalp psoriasis.
