The relationship between metformin and weight is unclear, but several theories provide possible explanations for weight fluctuations. Reduced hunger is one documented side effect of metformin, according to the Mayo Clinic.

For example, metformin may affect hunger cues. In one small study, 12 women with obesity and type 2 diabetes who were not taking insulin were randomly given two dose levels of metformin — 850 milligrams (mg) or 1,700 mg — or a placebo three times a day for three days, with each participant going through the study three times so they had each dose and the placebo. Participants received a meal test on each third day and rated their hunger level before eating. Researchers found that hunger levels in the metformin group were significantly lower, especially after the participants took the 1,700 mg dose compared with when they were taking the lower metformin dose or placebo.

If you take metformin, it might not appear as if you’re eating less with this drug. But the number of calories you’re currently consuming for breakfast, lunch, and dinner might be lower than your normal food intake. This subtle change in appetite could be responsible for a gradual decline in weight.

The most common symptoms are vomiting, diarrhea, and nausea, which affects about 2 to 63 percent of people on the drug. Discomfort can be so severe that you may not feel like eating much and subsequently consume fewer calories. The extended release forms of metformin have documented fewer side effects in studies.

Is Long-Term Weight Loss Possible on Metformin?

Even though metformin can help you shed pounds, the amount you lose might be far less than expected. On average, weight loss after one year on the drug is only 6 pounds, according to the Diabetes Care study.

Can People Without Diabetes Take Metformin?

What if you don’t have type 2 diabetes? Can you still take metformin for weight loss? This is an excellent question, and the short answer is yes. Sood explains that metformin has been used off-label, or outside its intended purpose, for weight management.

In fact, a review published in December 2018 in the European Journal of Clinical Pharmacology examining the efficacy of metformin for weight loss in overweight and obese people without diabetes concluded that the drug could significantly decrease body mass index in both adolescents and adults.

But because this drug is only available by prescription and hasn’t been approved for weight loss by the Federal Drug Administration, your doctor will likely first recommend some of these traditional strategies for weight loss:

If you don’t have type 2 diabetes, your doctor may sign off on this drug if you’re obese and struggling to lose weight. But again, metformin isn’t a substitute for healthy eating and regular exercise, which are keys to sustainable weight loss. These habits also encourage weight loss if you have type 2 diabetes but can’t take metformin.

“For those patients with diabetes who can’t take metformin, due to chronic kidney disease or other concerns, diet and lifestyle modifications will still be of utmost importance, whether they’re prescribed alternate medication or not. “Make it a point to stand up every hour or so and walk around the room, the yard, or the block. Little bits of physical activity over the day add up, resulting in the burning of more calories as a result,” she says.

The Bottom Line on Metformin and Weight Loss

Here are seven things you need to know if you or a loved one has been prescribed metformin as part of a type 2 diabetes treatment plan.

1. Which Batches of Metformin Are Affected by the Recall?

Certain batches of metformin-ER are being recalled, and the FDA hasn’t indicated that metformin-IR, the most commonly prescribed type of the drug, is impacted.

As the American Academy of Family Physicians (AAFP) notes, the recall impacts roughly 19.5 million prescriptions, or one-quarter of all metformin drug prescriptions, which average 78 million annually, according to a 2017 report from the Agency for Healthcare Research and Quality.

Robert Gabbay, MD, PhD, chief scientific and medical officer for the American Diabetes Association (ADA), estimated that these numbers are likely similar in 2020, though he noted the ADA does not track metformin prescription trends.

Jeremy Kahn, a spokesperson for the FDA, said the agency does not have information on the percentage of metformin-ER inventory affected by the recall. “Metformin-ER is not in shortage,” he added.

2. What Should I Do if I Take Metformin to Manage Diabetes?

Check to see if your metformin is part of the recall. If it is, don’t abruptly stop taking your meds without speaking with your doctor first, the FDA urges in an October 2020 statement. Doing so can send your blood sugar soaring, and that is risky in and of itself.

“Patients should continue to take their metformin until their pharmacist supplies a different manufacturer’s product or their doctor prescribes a different treatment,” says Matt Petersen, vice president of medical information and professional engagement for the ADA, in an official release.

As for what to expect when you contact your provider? It’ll depend on your individual health, but he or she may prescribe a different med that is not affected by the recall or potentially metformin-IR. “The solution in this instance is quite simple,” says Caleb Alexander, MD, the codirector of the Johns Hopkins Center for Drug Safety and Effectiveness in Baltimore: “Patients can simply be switched to a shorter-acting formulation.”

Dr. Gabbay agrees, noting that a switch to metformin-IR is possible because it regulates blood sugar the same way metformin-ER does. “ER can reduce side effects in some patients (stomach upset, diarrhea) compared to regular metformin, though in all patients those side effects tend to reduce with time,” Gabbay says. “Dosing of ER versus IR is somewhat different, so again, a switch must be managed by a patient’s primary care provider.”

3. What’s the Difference Between Metformin-IR and Metformin-ER?

Metformin-IR, which is sold under the brand name Glucophage, is generally split up throughout the day, though you may take only one dose per day to start, notes the discount drug website GoodRX. On the other hand, metformin-ER, which is sold under the brand name Glucophage XR, is often taken one time per day (however, in some cases, your doctor may okay splitting up dosing). It depends.

4. What Is NDMA and Why Might Exposure to It Be Harmful?

NDMA is a chemical that has been associated with liver damage in humans, and animal studies have linked exposure to an increased risk of cancer and stillbirths, according to the Centers for Disease Control and Prevention (CDC).

The chemical was once used to make rocket fuel, and is still sometimes produced as a byproduct of a variety of manufacturing processes, including the production of medicines, according to the CDC.

Kahn, the FDA spokesperson, said that the agency “has no information to provide on specific adverse events being linked directly to NDMA impurities.”

NDMA is classified as “probably carcinogenic to humans” by the International Agency for Research on Cancer. But that doesn’t mean you should panic. More on this later.

5. How Did Metformin Become Contaminated With NDMA?

The short answer is: Regulators still don’t know, though the cause under ongoing investigation, according to the FDA. They are asking all manufacturers of ER metformin — including companies not covered by the current recall — to test batches for NDMA before selling these drugs in the United States.

The FDA says it does not know how long NDMA has been present in the recalled metformin.

6. Is My Cancer Risk Higher if I’ve Been on Metformin-ER for a While?

Probably not, but it’s not certain. The FDA recalled certain batches of metformin after finding levels of NDMA “above the acceptable intake limit,” which is 0.096 micrograms.

Scientific research has produced mixed results on whether NDMA poses a cancer risk in humans, says Thomas Marron, MD, PhD, an assistant professor in oncology at Mount Sinai in New York City and a medical reviewer for Everyday Health.

While preclinical studies have suggested that NDMA exposure may elevate cancer risk, larger population studies in humans, such as a September 2018 study in the BMJ, have suggested that NDMA does not cause cancer in humans, Dr. Marron says.

Health agencies including the FDA and the World Health Organization’s (WHO) International Agency for Research on Cancer also note that the levels in medicines contaminated with NDMA likely don’t contain enough of this impurity to cause cancer in humans.

What’s more, the FDA says it doesn’t expect NDMA to be harmful at low levels, even when someone is exposed over a long period of time, such as 70 years. The agency notes that samples of the contaminated metformin appear to have similar NDMA concentration to that of grilled and smoked meats.

Most often, people are exposed to NDMA by consuming these foods, along with cheese, or fermented foods like beer, according to the CDC. Certain cosmetics and cleaning products may also contain NDMA.

Whether NDMA poses a cancer risk from these consumer items is also uncertain, Marron explains. “It’s (NDMA) in foods like beer, cheese, smoked meats, all things associated with Western diets where there are higher rates of cancer in general,” he says. “It’s very difficult to parse out the different contributing factors, even when you have large data sets,” such as in the aforementioned BMJ study.

7. What if I’m Taking a Form of Metformin-ER Not Covered by the Recall?

You should continue to check the list of recalled metformin on the FDA’s website to ensure your medication is safe to use.

After all, the number of companies and batches of metformin-ER covered by recalls in the United States have been expanding. If your drug isn’t there now, you can check back later to be sure it isn’t added in the future. You should also check with your pharmacy to see how they would notify you if the batch of metformin-ER you’re prescribed gets added to recall lists.

17-Year-Old Loses 118 Pounds, Putting Diabetes in Remission

“I had bad cramps, intense leg cramps to the point where I couldn’t sleep,” Tayé recalls. “I kept walking around, which was making my mom mad, but I couldn’t sleep.” He constantly begged for water and yet began vomiting. Those are among the common symptoms of DKA, which also include frequent urination, fruity-smelling breath, and difficulty breathing, per the ADA.

“When we took him to the doctor I was asked if he had ever been tested for sugar, for diabetes. I thought, Why would we do that? He’s 15,” explains Stewart. “But I saw the look on the nurse’s face, and I could see that she was worried.” The nurse told them rush to Riley Hospital for Children in Indianapolis. The boy was in such pain that he was no longer able to walk, so hospital workers had to wheel him in.

Once there his blood glucose level was measured at a stratospheric 694 milligrams per deciliter (mg/dL) (a normal reading is below 200 mg/dL, per the CDC). “They said we’re keeping him because he has DKA and I said, ‘What is that?’ I never thought in a million years they would say my son has DKA and diabetes, and will rely on insulin for the rest of his life,” says Stewart. Upon his release from the hospital, Tayé was put on three to four shots of short-acting insulin with meals, one shot of long-acting insulin at bedtime, and twice-daily oral doses of metformin.

“Riley sent me to classes to learn what to do,” says Stewart. “I was so terrified I could not do what was needed. I lay there and prayed about it, asking God to give me the knowledge, patience, and strength.” Worried that her son would face a future of diabetes complications such as limb amputations, as well as the high cost of insulin, Stewart was determined to wean him off insulin.

“I remember after they got discharged from the hospital and meeting Michelle for the first time,” says his endocrinologist. “We talked about the options and what life with diabetes is going to look like. And she said, ‘Dr. Nabhan, we’re going to win this.’”

A Diet and Lifestyle Makeover to Stabilize Blood Sugar

Seven weeks later and 20 pounds lighter, as a new lifestyle including exercise and a diabetes-friendly diet took hold, the insulin injections were history. That may seem like a brief time to be on insulin, but it is in keeping with best practices, says Mary Pat Gallagher, MD, the director of the Robert I. Grossman, MD, and Elisabeth J. Cohen, MD, Pediatric Diabetes Center at NYU Langone in New York City. Dr. Gallagher was not involved in Tayé’s care. “With type 2 diabetes, the goal is to get you off insulin within three months.” If weight loss, insulin, and metformin aren’t enough for a child or teen to reach their target blood sugar levels, adding liraglutide (also known as Victoza or Saxenda) is recommended.

At first, getting more exercise seemed like a daunting challenge. Tayé enjoys video games — especially Mario & Sonic — and preferred staying inside even before the COVID-19 pandemic kept him homebound. Stewart decided to bring the exercise indoors, insisting on an hour of exercise per day: typically, 45 minutes on the treadmill and 50 sit-ups. Twice a month, Tayé gets a break from the treadmill — as long as he does 100 sit-ups. His newfound fitness is reaping dividends as life emerges from lockdown. Exercise helps the body use insulin more efficiently, helping lower A1C. A review published in March 2020 in Mayo Clinic Proceedings suggests that regular exercise may also help reduce reliance on type 2 diabetes medications.

“Now I can play basketball with my friends,” says Tayé. “Before, I couldn’t play for more than five minutes, but now I can do it for a long time.” It’s easier to stick with activities you enjoy, he advises other young people. “Try to find something you are comfortable with and make fun out of it.

Exercise options abound. “There are multiple exercise videos that you can stream on your TV and do cardio in your house,” says Nabhan. “You can jump rope, you can dance, or if you have stairs in your home, go up and down the stairs. You really can be creative with what you do.” She advises that parents have their children ages 12 and older vaccinated against COVID-19 so they can exercise safely with other kids.

Hand-in-hand with exercise were the changes Stewart made to how she prepared meals to keep her son on a low-carbohydrate, low-fat, portion-controlled diet. “I had the measuring cups and measured everything, even down to the condiments. I counted the carbs in every teaspoon of ketchup,” says Stewart. She also kept a food journal for Tayé and sent weekly reports to his healthcare team.

Stewart furthermore found healthier ways to prepare Tayé’s favorite foods. She ditched grease frying, bought an air fryer, and relied more on oven baking. “He likes barbecue, so I would bake the chicken — use only 2 tablespoons of sauce and smear it on the chicken, then bake it in the oven instead.

Her tip for parents who are trying to help a child with type 2 diabetes lose weight and lower their blood sugar: “It’s important to eat and take meds and exercise on a schedule. Being consistent trains the stomach to be hungry at the same time each day, and that cuts down on snacking in between meals.”

A Commitment to Stay on a Healthy Path

Now entering his final year of high school, Tayé looks forward to studying computer science and dreams of having his own company one day. With that new life ahead will come the responsibility to maintain a healthy weight and blood sugar level on his own. His mother has played a large role in his success thus far, observes Nabhan. Going forward, “My hope is that he has seen what diabetes can do to you, and what it means to be diabetes-free. I do think he will have the ability to be a little bit more flexible with his diet, but he cannot lose this focus and live free as if he doesn’t have diabetes. We know that diabetes can come back with a vengeance.”

Once he makes the transition to college, it’s important for him to make his own schedule and stick to it, says Dr. Gallagher. College class and study schedules are more variable than those in high school, so it will fall to a college student with type 2 diabetes to create consistency on their own. “Make sure you get dressed and get out of your room, go to the gym or do whatever physical activity you like every single day.” She also advises reaching out to those who manage the campus meal plan to find out what diabetes-friendly options they provide.

Meanwhile, Tayé encourages other young people do what he has done to take back his health. “Just motivate yourself,” he says. “Find a goal and set it for yourself, whatever inspires you to lose the weight.”
