As a runner, musician, and equestrian with fibromyalgia, I’m no stranger to muscle pain and soreness. My hands and wrists especially ached during college when I played the flute for countless hours a day, and back pain has plagued me for most of my adult life. To cope, I tried various muscle rubs and quickly amassed a collection of products — many didn’t work well, but I ended up loving some.

Choosing a quality muscle rub can be tricky. You want a product that works immediately, but that also lasts for hours. Many formulas are greasy, while others are powerfully scented. The muscle rubs in this list are effective, non-greasy, and have minimal fragrance.

When trying a new product, it’s important to make sure that you aren’t allergic to any of its ingredients. Always follow the product’s usage directions and test it on a small area of your skin first. Muscle rubs can be used in conjunction with other pain-relieving methods, but don’t apply muscle rub before using a heating pad or wrapping or bandaging the affected area.

While muscle rubs can ease away short-term muscle soreness, always consult your doctor for long-term pain management or in the event of acute injury.

Amazon.com: Aspercreme with Lidocaine Maximum Strength Pain Relief Cream, 4.7 oz. : Everything Else

I was unaware that Lidocaine had become an OTC product, but when I found out, I researched the products that Amazon carried and selected Aspercreme with 4% Lidocaine to use on my arthritic hand.
My hands will hurt to the point where I’ve got silent tears running down my face at night when I’m wanting to just go to sleep. I just get defeated by that much throbbing, aching pain, and if you ask any doctor you’ll get “It’s just arthritis, everyone has it” I’ve tried to sleep with my hands out, fingers splayed because curling them up causes fresh kinds of pain.
So, I eagerly awaited the arrival of the Aspercreme with Lidocaine and I have to admit, I am more pleased than I thought I would be. It’s not a miracle, and it doesn’t wipe out all the arthritic pain in my hands, but it does allow me to get the pain to a point where it doesn’t keep me from going to sleep and the pain doesn’t wake me up all night, especially if a hand tucks under my cheek while I’m sleeping. It takes the pain from a level 6 to a level 3 in about 20 minutes.
I tried it out on other areas, and in the area of my trochanteric bursitis (outside hip) using Aspercreme with Lidocaine allows me to sleep on my side without waking up from pain -something that hasn’t happened in the last 10 years. If I press on the area, I can still feel pain, but the top level of pain is gone. Using it on my back -more arthritis (thanks Mom & Dad for those genes) I don’t feel much relief at all. It could be because that pain is “referred pain” or the source of pain is unreachable. I did feel a little relief on my shoulders -both had rotator cuff surgery and get stiff & sore.

The biggest negative about this product is the cap on the bottle. My hands can NOT manipulate it, you have to press down & turn, both actions are not always possible with my hands. I’ve had to change all the door knobs to levers because grasping, closing, manipulating are no longer easy for me to do.

So, Aspercreme, you should rethink your cap. What good is a pain relief product if someone can’t get to it? This is why you lost stars.

Topical Relief for Muscle Pain

Muscle creams, also called muscle rubs, are available over the counter (OTC) and are used to ease muscle and joint pain caused by arthritis or injuries. They contain a wide variety of active ingredients. Some have active medicines that penetrate the skin and enter the body while others contain counterirritants that create a cool or hot sensation as a distraction from pain points. Common OTC muscle creams include BenGay, Icy Hot, Biofreeze, and Tiger Balm.

Arnica

Arnica is an herb, and its flowers are commonly used in medicine. It contains active chemicals that can aid in reducing swelling and decreasing pain and act as an antibiotic.

Arnica is commonly used for pain associated with osteoarthritis, sore throat, and surgery, to name a few. So far, only its use for managing osteoarthritis pain is supported by scientific evidence. One study showed that an arnica gel provided similar pain relief to topical ibuprofen (an NSAID) for osteoarthritis. Even so, topical arnica products have a similar, if not worse, side effect profile compared with topical NSAIDs.

Common products containing Arnica include:

  • Boiron Arnicare Gel 2.6 Ounce Topical Pain Relief Gel
  • Arnicare Cream
  • Arnica-Menthol Organic Magic Balm
  • Arnica + K Cream

While Arnica is commonly used in food and homeopathic treatment, there are some concerns related to its safety. Be sure to use only as directed. Eating the Arnica plant, topical creams, or ointments, or the plant extracts or oil can result in serious poisoning. Additionally, don’t use Arnica if you’re pregnant, breastfeeding, have allergies to ragweed or related plants, on broken skin, have digestive issues, have a high heart rate, or before surgery.

Arnica can be unsafe when taken by mouth. Speak with your doctor and follow the dosage recommendations on any products containing Arnica.

Capsaicin

Capsaicin is a chemical compound derived from chili peppers. It’s commonly used to relieve minor pain in muscles and joints caused by arthritis, backaches, muscle strains, bruises, cramps, and sprains. This substance works by affecting nerve cells in the skin that are associated with pain, which results in decreased activity of these nerve cells and a reduced sense of pain.

Capsaicin is potent and is in small doses in muscle rubs. Capsaicin creams, gels, oils, and other products contain between 0.025% and 0.075% of the substance. Capsaicin can also be applied by a doctor at a higher concentration after the application of a topical anesthetic.

Over-the-counter options include:

  • Capzasin-P: A capsaicin 0.1% topical analgesic cream
  • Zostrix: A capsaicin 0.033% topical analgesic cream
  • Zostrix Maximum Strength: A capsaicin 0.075% topical analgesic cream

This chemical can cause burning or stinging pain, and ingestion can lead to nausea, vomiting, abdominal pain, and diarrhea. If the chemical comes in contact with your eyes, it can be extremely painful and even cause conjunctivitis or blepharospasm.

Menthol

Menthol is a common active ingredient in topical pain relief products. It provides a cold sensation once applied. Similar to capsaicin, it desensitizes nerve cells in the skin. The substance can feel nice on the skin, but it cannot alleviate inflammation. Instead, it works to override the pain sensation with a cooling sensation.

Menthol can be paired with other ingredients to add to its cooling power. It’s found in the following OTC products:

  • BenGay Cold Therapy
  • Icy Hot Naturals
  • Icy Hot Advanced Relief Patch
  • Salonsip Gel Patch
  • Aspercreme Heat Pain Relieving Gel
  • Aspercreme Max
  • BenGay Zero Degrees
  • Mentholatum Sports Pain Relief Cold
  • Tiger Balm Ultra

Menthol and capsaicin cause local heat. When using creams that contain these ingredients, wear gloves or judiciously wash your hands, since accidentally getting it in your eyes can lead to serious irritation.

Lidocaine

Lidocaine is an anesthetic (numbing medicine) that works by blocking the transmission of nerve signals. It is used to relieve itching as well as pain and discomfort from eczema, minor burns, scrapes, insect bites, hemorrhoids, and other conditions. It is also used to numb areas of the body before a surgery.

Common brands of topical lidocaine include:

  • Lidotrex
  • Regenecare
  • Renuu Patch
  • Vexa-D
  • Vopac

If lidocaine is ingested, it can cause numbness of the mouth and throat, which can lead to trouble swallowing or choking. If a large amount is ingested, it can also get into the bloodstream, leading to serious symptoms like seizures, coma, or cardiac arrest. Consult a doctor before using lidocaine.

Turmeric

Turmeric might be a familiar name to you. It has a chemical called curcumin, which can help decrease swelling, especially in arthritis. One study found that topical administration of a curcumin 5% ointment significantly reduced knee pain in older adults with knee osteoarthritis.

Muscle cream products that contain turmeric include:

  • Jointflex Pain Relief Cream with Turmeric
  • Biofreeze Pain Relief Cream
  • Everyday Medical Turmeric Pain Relieving Cream

Turmeric is a natural ingredient. Therefore, there are limited concerns about the safety of the substance. Do not take turmeric products if you are allergic to turmeric or any ingredients contained in this drug.

How to Apply Muscle Cream

Before using a muscle cream, speak with your doctor or pharmacist to make sure it’s safe for you.

When applying a muscle cream, the first step is washing and drying your hands. Next, read the packaging completely to note the dose recommendation, directions, and warnings.

Apply the cream to the affected area or where you feel pain. Some muscle creams can be applied up to four times per day. Be sure to follow the directions and do not use more than recommended.

Wash your hands after application, and store the cream in a safe and cool place.

Possible Side Effects

The effects of muscle creams vary depending on the ingredients, but generally, there are some particular side effects to keep in mind:

  • Redness, warmth, stinging, or burning
  • Rare but serious side effects such as blistering, swelling, severe redness, increased or unusual pain, nausea, vomiting, ringing in the ears
  • A serious allergic reaction, which is rare but may happen with symptoms like a rash, itching, swelling, dizziness, and trouble breathing

If you have any of the above symptoms, call your doctor right away.

If aches and pains are getting in the way of you living your best life, then consider recruiting the assistance of a pain relief cream. Pain relief creams can temporarily quell inflammation or reduce pain when applied directly to the skin.

“Not all pain is the same,” said Susan Bard, M.D., a dermatologist at Vive Dermatology, explaining that different pain relievers meet different needs. Some help reduce inflammation while others are more suitable for open wounds like cuts and scrapes.

Here are the best pain relief creams on the market, based on your needs.

Final Verdict

Penetrex’s Therapy Cream (view at Amazon) is loaded with natural ingredients that soothe pain effectively. And since the anti-inflammatory cream is non-greasy and non-staining, you can apply it without worrying about making a mess. It’s also suitable for sensitive skin, unlike creams that rely on counterirritants.

What to Look for in a Pain Relief Cream

Active Ingredients

Different pain relief creams rely on different active ingredients to get the job done. Be sure to select a pain relief cream that suits your specific type of pain. And exercise caution with salicylate- or lidocaine-based creams, as it is possible to overuse these pain-relieving agents.

Natural Ingredients

“Try to stay as natural as possible,” said Dr. Jaliman. Natural ingredients can help you minimize the risk of irritation, especially if you have allergies or sensitive skin.

Fragrance

“As with any topical product you apply to the skin, be cautious in using products that have fragrances,” said Dr. Zeichner. Fragrances may cause skin irritation, especially for people with sensitive skin.

FAQs

How do pain relief creams work?

Pain relief creams tend to work in one of four ways. Anti-inflammatory pain relief creams use soothing ingredients like arnica to reduce swelling. Meanwhile, counterirritating pain relief creams draw on ingredients like menthol and camphor to cool the skin.

Some pain relief creams use salicylates, a type of non-steroidal anti-inflammatory drug (NSAID), to block a key enzyme in the inflammation process. And other pain relief creams numb the skin with a local anesthetic called lidocaine.

Can I use pain relief cream while pregnant?

You can use pain relief creams powered by lidocaine safely so long as you follow the directions on the product label.

Scientific studies have yet to confirm whether camphor is safe for use during pregnancy, so it’s best to be cautious and avoid pain relief creams that feature it.

There are no human studies to determine whether menthol is truly safe during pregnancy, but the risk is considered low when the menthol concentration in a pain relief cream is low. If you want to use a methol-powered pain relief cream, consult with your doctor first.

While oral arnica consumption is problematic during pregnancy, topical use is generally recognized as safe. However, if you have concerns, consult with your doctor before using these types of pain relief creams.

Lastly, you should avoid NSAID-based pain relief creams during your third trimester. Methyl salicylate, a common active ingredient in these products, is absorbed into the body via the skin and can cause harm to the baby and complications during delivery.

The 6 Best Muscle Pain Relief Creams & Gels

There’s nothing quite like starting your day off with an aching back or leg muscles that are practically screaming in pain from yesterday’s workout (or whatever caused your achy muscles). When soreness occurs, you want to have one of the best muscle pain relief creams on hand so you can soothe your aches immediately.

Though there are many ointments, gels, creams, and even patches out there, many of which work the same way. Counterirritant ingredients like menthol and camphor produce cool and hot sensations. This provides temporary relief because the cold and warmth block pain signals from traveling to your brain. While the relief is only temporary, because you’re treating the pain topically as opposed to popping an aspirin, the results are nearly instant. Plus, the act of massaging the product into the skin helps to increase blood flow, which in turn promotes healing and relaxes muscles.

Another popular ingredient in muscle pain relief creams and gels is methyl salicylate, which is a counterirritant and a pain reliever whose structural properties are actually similar to aspirin. While some common ingredients like menthol and arnica are plant-based, many of these creams still contain additional ingredients meant for pain relief, which means they shouldn’t be used every day for long periods of time – especially if you’re trying to mask the pain of a more serious injury. And just as important, make sure to read and follow each one’s individual directions.

Ahead, here are some of the best muscle pain relief creams, whether you’re in need of a post-workout treatment or relief from minor joint pain and stiffness.

1

The Best Pain Relief Gel

A favorite among dancers like New York City Ballet’s Sara Mearns, Tiger Balm was a longtime staple in my own dance bag growing up. The new Active Muscle Gel formula was designed to help relieve tired and achy muscles after you work out. Since it’s a gel, it’s less greasy than a traditional balm or cream formula, and it can be applied more easily over larger muscles. Like Tiger Balm’s classic ointment (which is also incredibly effective for pain relief), the gel contains menthol to give a cool then hot sensation on muscles, and it’s formulated with methyl salicylate for easing aches and pains.

2

 A Natural Pain Relief Cream

Made with natural botanical ingredients like arnica, for pain relief, and sunflower oil to moisturize skin, Kneipp’s cream formula soothes sore muscles, relieves achy joints and can even help reduce inflammation and bruising. The formula is not pore-clogging, so you can use it anywhere (including on your back, without the risk of breakouts), vegan, and free of icky synthetic ingredients like parabens, preservatives, and mineral oil. It absorbs into skin without leaving behind any sticky or greasy residue, and can also be used on bug bites to help quell swelling.

3

The Best Roll-On Pain Relief Gel

This 3-ounce roll-on gel is perfect for traveling or storing in your gym bag without having to worry about spills. It also means a no-mess application if you’re on-the-go and don’t have time to massage a traditional cream onto your sore muscles. The BioFreeze Pain Relief Roll-On Gel uses menthol to hit sore muscles with a cooling sensation that blocks pain signals. Effective at relieving sore muscles, backaches, sore joints, and arthritis, the brand also boasts that it’s the number-one clinically-recommended topical pain relief brand.

Many Amazon users note that they tried BioFreeze at the recommendation of a chiropractor or nurse aid, and they noticed an instant relief. One writes, “For anyone who has pain of any type, this will be your best friend. I suffer from severe sciatica nerve pain, [and] BioFreeze helped me control the pain.”

4

An Organic Way To Relieve Sore Muscles

If you’re concerned about the ingredients in your muscle relief cream, Dr. Bronner’s Arnica-Menthol Balm is free of synthetic ingredients and it’s certified organic, to boot. A blend of arnica, camphor, menthol, and peppermint oils helps to soothe aches and pains, while organic beeswax and oils like avocado, coconut, olive, jojoba, and hemp moisturize the skin. Because of this, it’s actually a multi-purpose balm that can be used to treat dry hands and cuticles, and even help heal skin that’s just been tattooed.

5

Best Pain Relief Gel Minus The Scent

Most pain-relieving creams have a telltale smell that’s equal parts strong and unpleasant. Icy Hot Vanishing Scent is a gel formula whose menthol scent dissipates within minutes after application. The tube comes with a sponge applicator to keep your hands from getting messy, and it can be used several times a day to relieve pain from arthritis, backaches, muscle strains, sprains, bruises, and cramps.

6

Best Imported Salve For Pain Relief

One Bustle beauty editor found this pain-relieving salve while on a trip to Germany and swears by it. Amazon users also rave that the Apothekers Origin salve works quickly and doesn’t stain clothes or smell too strongly. Since the EU has stricter laws when it comes to ingredients used in skin care products, this formula is made with all natural plant-based ingredients, like valerian, camphor, peppermint, and arnica, and no synthetic chemicals. This also gives it a less intense and more pleasant forest-like scent. One reviewer who has been using the salve for 25 years calls it the “best product ever” and writes, “It’s a miracle for aches, pain, joints, and arthritis … no greasiness and no strong odor.” Another fan called it a “miracle in a jar.”

Topical Pain Relief: Menthol and Lidocaine

Topical pain relief treatments are a great temporary pain relief alternative to over-the-counter pain medication. It is a good option for patients who cannot take pain relief medication due to a sensitive stomach. Topical pain relief is fast-acting and can be applied directly onto the targeted area, unlike other forms of pain relief.

Below we’ve listed two ingredients that are commonly found in topical pain relief treatments:

Menthol

Menthol is made from mint extracts and provides a soothing cooling sensation. It is often used to temporarily relieve minor pain from arthritis, muscle strains, backaches, and other bothersome injuries.

Point Relief® ColdSpot® gel, spray, and roll-on are fast-acting options that provide cooling temporary pain relief to treat aches, pains, and strains. This product is formulated with a higher menthol concentration than the leading brand. It contains all-natural ingredients such as menthol, arnica and ilex. With a multitude of different product offerings, ColdSpot® can work anywhere for many different patients.

Lidocaine

Lidocaine is a local numbing medication used to reduce pain and discomfort by blocking nerve signals in your body. It can be used to treat skin irritations such as sunburn, insect bites, minor cuts and burns. Lidocaine is fast acting and temporarily numbs an affected area.

For hours of pain relief, the Point Relief® LidoSpot™ patch adheres and stays firmly in place to the treatment area, even when performing physical activity. It is fast acting, fits anywhere for pain relief, and works for up to 12 hours. This patch is designed to block pain impulses from being transmitted. It uniquely contains the highest possible amount of lidocaine (4%) without a prescription and 1% menthol for cooling pain relief.

These topical pain relief treatments are all great for fast acting, temporary pain relief. Consult with your primary care physician before trying any of these products. For more information on our pain relief products, contact us today!

Which is the Best Muscle Rub? / Fitness

When you’re sore and tired from a heavy lifting session, there are few things that will make you feel as wonderful as a good rub down with your favorite muscle rub.

But do you know which is the best muscle rub?

Tiger Balm

For an all-round effective analgesic cream, you just can’t beat Tiger Balm. It’s much more common in Asia (where I grew up), but you can find it in just about any pharmacy around the country. The Chinese heat rub is made with menthol, camphor, mint oil, cassia oil, and other herbal ingredients, and the formula on which the rub is based has been around for more than 1,500 years. After all that time, they’ve definitely got it right, and few rubs can release muscle tension as well as burning hot Tiger Balm! The smell is strong, but it’s actually a pleasant one.

Biofreeze

The beauty of Biofreeze is that it’s a medication-strength cream without the medicated smell or the need for a prescription. Many chiropractors and pain specialists recommend its use, and it’s an OTC pain relief gel you can find at almost any pharmacy–not to mention online. It contains menthol as the active ingredient, but it also has aloe, calendula, green tea, arnica, camphor, and lemon balm to help soothe your muscles. It works for a long time, and it doesn’t have too bad of a smell.

Icy Hot

Icy Hot will turn your skin red, which means that it’s stimulating circulation to the area to which it is applied. The initial coolness will help to reduce the pain, while the heat that follows soon after will get the blood flowing. Its primary ingredient is menthol, but it contains other natural ingredients that work nicely to chill and heat your muscles. You’ll find the heat of the cream can actually be quite painful, so make sure not to apply it too close to cuts or wounds.

Vicks Vapo-Rub

Vicks Vapo-Rub is more commonly used as a cough relief treatment, but it’s great on those sore, tired muscles as well. It is an oil-based medication, with cedarleaf oil, turpentine oil, nutmeg oil, and petroleum jelly being some of the inactive ingredients in the rub. However, it has camphor, menthol, and eucalypthol in the rub, so it’s going to help relieve the tension from those tired muscles. The smell isn’t too bad either!

P-K-5 Pain Reliever

I have never personally tried this rub, but I’ve heard that it’s surprisingly effective for dealing with muscle pain. It contains albumin proteins (from eggs), herbs, oils, plant extracts, minerals and vitamins, and it’s supposed to be one of the best pain relief rubs for those who don’t like the smell of the mentholated ones. It is a hypoallergenic cream that won’t cause a strong skin reaction (not uncommon with the others on this list), so it’s a good option for those with sensitive skin.

I’m partial to Tiger Balm, because I’ve tried it and found that it works a lot better than Icy Hot, Vicks Vapo-Rub, and Biofreeze. However, many people have found that the others work just as well, so any of these 5 muscle rubs are going to be your best option!

Warming compresses – one of the methods of Chinese medicine – Rossiyskaya Gazeta

Traditional Chinese medicine specialists believe that heat improves blood circulation, which helps to relieve pain. Therefore, in Chinese medicine, warming compresses are widely used. No complicated rules or special preparations are required here – you just need ordinary hot water and a towel.

Why are warm compresses so good? In case of a cold or runny nose, you can make a warming compress for the nose and breathe well with warm steam – in a few minutes the nasal congestion will pass.The compress warms and moisturizes the nasal cavity, facilitates nasal breathing.

Dark circles under the eyes that appear after a sleepless night are due to poor circulation. Of course, to get rid of them, you can use an expensive eye cream, but it’s much easier to make a regular compress. The warmth will improve blood circulation around the eyes. It is best to do this compress within 15 minutes before bedtime. It will help not only eliminate circles under the eyes, but also get rid of dry eye syndrome.

And to improve sleep, put a towel in warm water, squeeze a little and rub your back along the spine, paying special attention to the cervical and thoracic regions. Rub for five minutes. This will help relieve tension, muscle and soft tissue spasm, you will relax and sleep soundly all night.

Photo: China Media Corporation

A long time spent at the computer leads to aching pain in the cervical spine. It is recommended to warm these places with a hot towel.The lateral areas between the ear and the collarbone should also be covered, such a compress should be kept for 10 minutes.

Warming up with a hot towel also helps in stretching the tendons of the cervical spine. Place a hot towel on the sore spot, while doing slow rotational movements with your neck.

Warming up the lumbar region with a hot towel will also relieve pain and relax the muscles of the lumbar region. For those suffering from lower back pain, it is better to give up soft sofas and sleep on a hard bed.For lumbar pain, it is recommended to first apply a cold lotion, and then a warming compress. However, it is better to keep the lower back warm at all times.

For menstrual pain, Chinese doctors advise applying a warming compress to the lower abdomen – it will help get rid of blood stagnation and soothe the pain.

Of course, a warming compress is not a panacea. And he will not help in all cases and not for everyone. For people with sensitive skin, the elderly, children suffering from diabetes or paralysis, it is better not to do warming compresses without the special recommendation of a doctor.Particular attention should be paid to the temperature of the towel: it should not be too hot. The temperature should be between 40-50 degrees. The heating process takes about 20 minutes. If a warming compress is applied to a sensitive or painful surface, the compress should be cooler. During warming up, it is necessary to monitor the condition of the skin at the heating site at all times. For those suffering from skin diseases and those with inflammation on the skin, hot compresses are contraindicated, as heat exacerbates symptoms and promotes the growth of bacteria.

Not all pain can be relieved by warming up. It is used mainly for “cold diseases”, the common symptoms of which are excess “internal heat”, fear of heat, bad breath, and constipation. But after soft tissue damage, for example, when muscles are stretched, hot compresses are not recommended for the first 24-48 hours.

A warming compress will help with discomfort and pain, but it is far from a miracle cure. He plays an auxiliary, and not the main, role in the treatment.

Treatment of chest osteochondrosis in Yekaterinburg

Osteochondrosis of the thoracic spine is a degenerative-dystrophic process that affects the intervertebral discs. The disease is characterized by a chronic course and develops more often after 25 years. Contribute to the development of osteochondrosis – back injuries, heavy physical exertion, stoop, sedentary work, obesity, anatomical features, and other diseases of the musculoskeletal system.

Treatment of thoracic osteochondrosis is aimed at slowing the progression of the disease.The methods of therapy used depend on the severity of the symptoms and the neglect of the degenerative processes.

Symptoms of osteochondrosis

The main clinical sign of thoracic osteochondrosis is pain in the thoracic spine. In the early stages, there is practically no pain, arises after a prolonged forced position or lifting weights.

With the development of degenerative processes, pain becomes permanent, motor activity is limited. Additional symptoms join:

  • There is pressure on the back and chest, making it difficult to inhale and exhale;
  • when the nerve roots are squeezed or irritated, intercostal neuralgia develops;
  • Tension of the muscles of the back;
  • with stenosis of the spinal canal and compression of the spinal cord, conduction disturbances of sensitivity (numbness) may develop from the level of damage to the thoracic region and the underlying part of the trunk and lower extremities, paresis (weakness) of the lower extremities.Disruption of the pelvic organs (violation of urination and defecation).

Dorsalgia what is it?

The terms refer to pain in the thoracic spine. Dorsalgia is characterized by chronic or acute pain in the thoracic spine, aggravated by movement.

Diagnostics: MRI or CT of the thoracic spine

How is thoracic osteochondrosis treated?

For the treatment of chest osteochondrosis, conservative therapy is used, including drugs, physiotherapy, exercise therapy, massage, manual therapy, balneotherapy.

The main goal of therapy is to relieve pain and eliminate inflammatory processes, restore the function of the spinal roots, improve blood circulation, relieve muscle spasm.

Surgical intervention is performed in case of a herniated disc with damage to the spinal cord.

Preparations

Medication for breast osteochondrosis:

  • Non-steroidal anti-inflammatory drugs – relieve pain, relieve inflammation and swelling of soft tissues.Prescribed drugs such as diclofenac, nimesulides, ibuprofen, meloxicam, ketololac, lornoxicam, etoricoxide, etc. For local use – fastum gel, voltaren gel, dolobene gel, etc.
  • Muscle relaxants for reducing muscle tone resulting from acute pain (baclofen, tizanidine, toperisone).
  • Vascular drugs that improve blood circulation in the affected area (pentoxifylline, aminophylline).
  • Diuretics, relieve edema (Mannitol, furasemide).
  • Thioctic acid preparations (thioctacid, berlition, octolipen, etc.) to restore sensitivity.
  • B vitamins (preparations including a complex of B vitamins: B1, B6, B 12) – to normalize metabolism.
  • Medical and medical blockade for the removal or reduction of pain syndrome (acute, chronic) with anesthetics (novocaine, lidocaine), with corticosteroid drugs (diprospan, dexamethasone).
  • Anticholinesterase drugs.Improving the conduction of nerve impulses, increasing muscle strength. Ipidacrine preparations (neuromidin. Axamon).
  • Hondoprotectors – the drugs are based on collagen, chondroitin sulfate, glucosamine, which are necessary to slow down degenerative processes (arthra, don, teraflex, etc.).

Physiotherapy

Physiotherapy methods relieve swelling, inflammation, improve blood flow in the affected area, reduce or stop pain.
Magnetolaser, magnetotherapy, CMT with novocaine, ultrasound with hydrocortisone, etc.

Balneotherapy : Mud applications, ozokerite applications on the affected areas, radon baths.

Exercise therapy and massage

Physiotherapy is the main method of treating osteochondrosis. Gymnastics is performed regularly for 10-15 minutes several times a day. To enhance the effectiveness, a massage course is prescribed. The procedures help to relax the muscles, reduce pressure on the vertebrae, improve metabolic processes and blood flow. Exercise therapy can be performed independently at home, also in a polyclinic (individual and group lessons).The massage is performed by a qualified specialist in a polyclinic.

Manual therapy

the method is carried out with the help of the doctor’s hands using various techniques of isometric relaxation (muscle relaxation), improving blood flow in the affected area and restoring the full range of motion in the thoracic spine, and as a result, relieving pain.

Effective exercises for the treatment of thoracic osteochondrosis:

  • and. p. – lying on your back, hands are pressed to the shoulders, legs are bent at the knees; twisting it is necessary to try to reach with the right elbow to the left knee and vice versa;
  • and.p. – lying on your stomach, arms extended upward, at the same time raising straight legs and arms, bending in the back;
  • and. p. – sitting with straight legs on the floor, it is necessary to bend in front, reaching with the right palm to the left foot, with the left palm to the right foot;
  • exercise “plank” – you should rest your socks and palms on the floor, tighten the muscles of the abdomen and buttocks, try not to sag in the lower back, stand for 30-60 seconds a day;
  • hanging on the horizontal bar.

Acupuncture

The technique consists in influencing biologically active points of the body with thin needles.The procedure reduces the manifestation of pain and inflammation. Acupuncture is not painful, and minor discomfort may occur.

What to do in case of an exacerbation?

During the period of exacerbation, severe pain appears, radiating along the intercostal space. At this stage, it is necessary to comply with bed rest, reduce the mobility of the spine. The elimination of symptoms is carried out with analgesics prescribed by the attending physician.

Prevention of osteochondrosis

To prevent the disease and slow down degenerative processes, it is recommended:

  • weight correction;
  • 90,040 cycling, jogging, swimming, yoga and other sports;

    90,040 daily walks;

  • intake of vitamin complexes and chondroprotectors;
  • reduction of back strain;
  • timely treatment of diseases of the musculoskeletal system.

90,000 Osteochondrosis.7 simple rules to help you keep your back healthy

Head of the neurosurgical department No. 3 of the Krasnodar Regional Clinical Hospital No. 1 Igor Basankin gives advice that will be useful to the majority.

Osteochondrosis is a very common pathology that is accompanied by pain and serious discomfort. Treatment of this disease does not always help to completely get rid of its manifestations, so prevention is especially relevant here.

What leads to osteochondrosis

Igor Basankin

Internal causes: disorders of intrauterine formation of the spine, genetic predisposition, metabolic disorders, hormonal pathologies, overweight, age-related changes.

The external reason, by and large, is one – improper load on the spine.

In this situation, in places of excessive pressure, a change in the structure of the cartilage tissue occurs.

This happens with a sedentary lifestyle; traumatic back injuries; scoliosis, kyphosis and other back pathologies; constant carrying of weights, flat feet; poor quality pillow or mattress, bad habits – excessive drinking, smoking.

How to recognize a disease

Recurrent or persistent pain in the dorsal region, neck, numbness in the limbs, or constant tension in the spinal muscles. Shooting pains in the back, chest, or lower back.Pain when raising arms up. Pain when tilting or turning the head, dizziness, pulling pains in the arm and / or leg. Noise in ears. Sudden bouts of nausea.

7 ways to make your life easier

Important: For all the actions listed below to be effective, they must be comprehensive

Learn to keep a straight posture. Ideal posture is the position of the body when the head is in line with the back.The head is raised high, the shoulders are straightened, the chest is raised, the stomach is pulled in. The gait should have been smooth and springy. Show discipline, achieve perfect results.

Organize your workplace. Many neglect this obvious advice, but in vain. After all, the condition of the muscles of the neck and shoulder girdle has a direct impact on the spine. Prevention of cervical osteochondrosis is impossible if the shoulders at the desk are tense all the time.

The chair must be equipped with a comfortable backrest and armrests. It is important that the keyboard is below the elbows, the angle between the forearm and shoulder is about 120 degrees. Position the monitor so that your eyes are facing the top edge of the monitor.

Keep your back straight while working at the computer. Try to take a break every 40-50 minutes to walk and exercise a little. It is best to do special gymnastics, since during a long stay in one position, the neck often swells.

Correct driving position. Motorists often tune the driver’s seat not as recommended, but in the way they want. After a while, they begin to complain of osteochondrosis. Therefore, adjust the back of the chair so that not only the shoulders touch it, but the entire lower back. It should snuggle up against the seat and not be suspended. Do not tilt the backrest too far back, but fix it in an upright position. Use special lumbar cushions designed for the driver’s seat.

Keep your chin slightly raised during the ride. Watch your knees: they should be higher than the hips. Try to stop every 1.5-2 hours: get out of the car and do some lower back exercises. This will restore the tone of the back muscles.

Balanced diet. An unbalanced diet is one of the most important factors in the development of osteochondrosis. Therefore, for prevention purposes, avoid sweets, salty foods, white bread, fatty foods.A special danger is white refined sugar, it is he who leaches calcium from the bones.

Eat foods rich in vitamins C, E, B3 (PP): vegetables, fruits, seafood, fish, legumes, nuts, chicken, eggs. They will provide high elasticity to your cartilage tissue. Do not forget about low-fat dairy products – kefir, yogurt, cottage cheese. Take vitamin complexes. Lose excess weight.

Physical activity.Physiotherapy (exercise therapy) is an excellent tool for the prevention of osteochondrosis. With its help, you will strengthen your muscle corset and ligamentous apparatus. All exercises included in the exercise therapy complex are physiological: during their execution, the muscles do not overwork.

Swimming is highly recommended – water helps to relax the muscles and increase the range of motion of the joints.

Another effective method is Nordic walking. During such walks, the main muscle groups are involved, energy consumption increases, and the functioning of the respiratory and heart organs improves.

Massage. It will tone your muscles and relieve excessive muscle tension. Massage is especially important in the prevention of chest osteochondrosis. Usually this procedure is performed in courses. Also learn to self-massage and do it every day for 10-15 minutes.

Proper sleep. Throughout the day, the intervertebral discs are subject to the load of the whole body. Only at night is it possible to completely relax and assume a free position.That is why the prevention of osteochondrosis of the spine consists in organizing the correct sleep regimen.

The base of the bed should be flat and firm. Avoid sleeping in hammocks, cots, or air mattresses.

The mattress should have good elasticity and medium firmness. Thanks to this, it will be possible to maintain the physiological curves of the spine in a natural position. A good solution is to buy an orthopedic mattress: it will allow you to maximally relax the muscles of the whole body and reduce the degree of irritation of the nerve fibers.

Keep your pillow small, rectangular, and moderately firm. Orthopedic pillows will help eliminate pain in the neck area and will serve as an excellent means of preventing cervical osteochondrosis.

HEALING BACK MASSAGE 1200 rubles. Mytishchi

Therapeutic back massage is widely used in the treatment of many diseases of the musculoskeletal system. This is the same essential element as medication, physiotherapy and health exercises.But in order for massaging the back to lead to the desired result, certain rules must be followed. Otherwise, massage will not be beneficial, but harm.

Massage against back pain
Therapeutic back massage has a particularly positive effect on the condition of the muscular system. Intense exposure allows you to relieve a person from pain in the spine. Along with stretching and relaxation, contraction and contraction of muscles, elasticity, flexibility, and good mobility return to the tissues.Regularly visiting an experienced specialist, you can improve your posture, relieve pain in the muscles, and restore elasticity to flabby muscles. The procedure helps to activate the lymphatic drainage system, when exposed to the body, lactic acid and other chemical waste products are removed. It is these substances that contribute to the manifestation of pain, discomfort in the ligaments and joints. Head massage becomes especially effective from this point of view. Indeed, a huge number of lymph nodes are located on the neck, back of the head and even the face of a person.

It has been known since ancient times that the spine is the basis of human life, health and longevity. According to medical statistics, about 70% of those seeking medical care at hospitals and clinics are people who complain of back pain. Having felt the first symptoms, in no case should you delay, self-medicate, rubbing in anesthetic ointments and gels, in the hope that the pain will soon go away by itself. Back pain is the first symptom that speaks of dystrophic disorders in various parts of the spine.Catching up on “poultices”, rubbing in anesthetic, you only relieve back pain, without eliminating the underlying problem, which in the future can lead to serious diseases of the spine.

One of the most effective methods of treating diseases of the spine and back pain is therapeutic massage. Carrying out a course of therapeutic back massage will help improve the blood supply to the intervertebral discs, improve blood circulation in the spine, and the conduction of nerve endings. Therapeutic back massage strengthens hypotrophied muscles and helps prevent muscle atrophy.Helps reduce pain. And most importantly, it gives a stable healing effect. Therapeutic back massage is prescribed in courses of 10-15 procedures, depending on the disease. As a rule, by the time of treatment, many diseases of the spine already have a chronic form, chronic dystrophic changes, so it is advisable to do massage courses at least a couple of times a year. Ideally, once every three months.

Back massage is a unique procedure that helps relieve fatigue, tension, and improves mood.Classic back massage is the most common type of massage that is used to relieve pain, for diseases of internal organs, to normalize the psycho-emotional state, etc. There are some secrets of proper back massage, which will be discussed in this article.

Preparatory phase
During the massage, all muscles should be relaxed as much as possible. To do this, lie on your stomach (your head is turned to the right or left), put a flat pillow under your stomach, and a roller under your legs.
It is advisable to use a special cream or massage oil for back massage. A small amount of one of these products is applied both to the skin of the patient’s back and to the hands of the massage therapist.

How is a classic back massage done?
The classic back massage technique is based on eight massage techniques: stroking, rubbing, kneading, squeezing, moving, shaking, vibrating and shaking. Each of the techniques is aimed at a specific effect on the skin, blood circulation, nervous system, adipose tissue.
The massage is carried out along the lymphatic vessels to the large lymph nodes. Basically, all movements are directed from the bottom up. The spine itself and lymph nodes cannot be massaged directly.

So, let’s consider a variant of the sequential process of carrying out a classic back massage:
Stroking. With two closed palms, draw in the direction from the lower back along the spine, spreading your hands to the sides of the shoulder blades. Movements should be smooth, sliding, without jerking or pressure.Repeat the technique 5 – 7 times.
Rubbing. This is a more intense technique that can be performed with weights (placing one hand on the other). Rubbing is performed with the base of the palm in a straight, circular or spiral manner in all directions. Repeat 3-4 times, then perform the same number of strokes.
Kneading. At a slow pace, without sharp pressure, perform spiral movements with the knuckles upward from the waist, spreading the arms to the sides of the shoulder blades.Repeat 3-4 times, grasping different parts of the back.
Sawing. With the outer edges of the palms, make a sawing motion on one and the other side of the back. After that, make 3 – 4 strokes.
Rolling. Gently grasp the area of skin between the thumbs and the rest of the fingers of both hands. Moving forward and fingering with your thumbs, move the “wave” from the waist to the neck. Repeat 2-3 times on each side of the back, grasping different areas, then rub the back with your palms.
Patting. Slightly relaxing the hands, tap with palms all over the back.
Finish the massage with the same technique that was performed at the beginning.

Can back massage be harmful?
When starting the procedure, it is worth remembering that massage can bring not only benefits, but also harm. There are several contraindications to back massage:

  • increased body temperature;
  • acute inflammatory processes in the body;
  • dermatological diseases;
  • hypertensive crisis;
  • blood diseases;
  • gastrointestinal disorders;
  • severe heart disease, etc.

If you decide to do a massage, follow the technique of its implementation (if pain occurs, massage should be stopped). Careless massage can damage nerve or muscle tissue. Therefore, it is best to entrust this process to a specialist.

Performing therapeutic back massage for osteochondrosis.
Osteochondrosis is a chronic disease of the spine, which is often the cause of back pain and is divided into types: osteochondrosis of the cervical spine, osteochondrosis of the thoracic spine, osteochondrosis of the lumbosacral spine, widespread osteochondrosis (several departments can be affected at once).

Symptoms of osteochondrosis: constant sharp or aching pain. Pain can worsen with physical exertion, lifting weights. The range of motion decreases. Muscle spasms appear. Feeling numb and aching in the limbs.

Therapeutic back massage is one of the main types of treatment for osteochondrosis. It improves blood circulation, relieves back pain. Promotes the restoration of muscle and spine function. Correctly performed therapeutic massage procedures do not harm the body.Gives a stable therapeutic effect.

During the singing session, I try to find out the symptoms that bother you. You tell me if there are back pains. Which side of your back hurts, in which part of the spine. Do you have numbness in your limbs? Or maybe the pain radiates to the internal organs. It is desirable to describe in more detail the picture of the disease. Also, if you have recent X-rays or MRI scans, it is advisable to ask your doctor for a transcript and description, and bring them to me for the first session.Massage techniques for osteochondrosis begin to be performed with light stroking movements. Stroking relaxes the back muscles. Then rubbing is performed, with the help of which we achieve an increase in blood flow in the tissues, warm up the muscles. Next, we move on to kneading. Reception of kneading leads to an increase in the tone of the muscles of the back, increases the blood supply to the tissues.

Usually, with osteochondrosis, I recommend not to be limited to a certain area – cervical, thoracic, lumbosacral, but to perform massage techniques all over the back, for greater efficiency.

Sometimes people call, complain of back pain and ask for a couple of sessions. I always try to explain that two or three sessions will have no effect. It’s just wasted money and wasted time, mine and yours. As a rule, degenerative changes in the spine are already chronic. In order to really achieve a therapeutic effect, you need to do at least ten sessions.

Carrying out therapeutic massage for sciatica.
Radiculitis is a disease of the peripheral nervous system that occurs against the background of degenerative lesions of the roots of the spinal nerves.

Symptoms of sciatica: As a rule, with radiculitis, persistent or recurrent dull back pain appears. Wandering pains can radiate from the neck to the arm, from the lumbar region to the legs. Sensitivity is impaired – numbness in the limbs, slight tingling, burning. There is also weakness in the muscles of the back or lower extremities. Sometimes this weakness is isolated, i.e. affects only certain muscle groups.

As a result of damage to the roots of the spinal nerves, impulses are disrupted, and the muscles gradually stop working normally.In the absence of timely treatment, muscle atrophy or flaccid paresis (paralysis) occurs.

With sciatica, complex treatment is prescribed. Medication: anti-inflammatory and vitamin preparations. With severe back pain – novocaine blockade. Also, therapeutic massage procedures, electrophoresis, acupuncture, physiotherapy exercises, etc. are prescribed.

When you first meet, tell me in detail what worries you. Are there any back pain, “lumbago” in the lumbar region.Does the pain from the back spread to the buttocks, thigh, lower leg. Please provide a description of the X-ray and MRI scans (if available). Massage procedures for radiculitis are prescribed after the end of the acute phase of the disease, when the inflammatory process turns into a subacute and chronic phase. Massage can be local (affect only certain muscle groups) or general. With sciatica, I recommend not to be limited only to local back massage, but to perform a general one, when the upper and lower extremities are also worked out, which allows you to improve blood supply and innervation throughout the body.

The first two, three sessions are made more gentle. We exclude any harsh techniques. We reduce the pressure of the masseur on the back muscles. We make the session 5-10 minutes shorter. Gradually, from 2-3 sessions, the intensity of movements and the load on the muscles increases. With sciatica, a back massage session lasts about 30 minutes. General massage session lasts 50-60 minutes

Basic massage techniques for lumbago and sciatica.
Lumbago and sciatica are different diseases, but they have similar symptoms, so it is easy to confuse them.Sciatica is an inflammatory process in the sciatic nerve caused by an infection or inflammation of the femur. Lumbago is an attack of acute pain in the lumbar spine, most often caused by osteochondrosis or intervertebral hernia.

Symptoms of lumboischialgia: pain in the lower back, radiating to one or both legs. It is possible to spread along the buttock or along the posterior outer surface of the leg, lower leg. The pain comes on suddenly and abruptly. It can be provoked by an uncomfortable position, great physical activity, and sudden movement.The appearance of such signs as a feeling of chilliness in the affected area, a feeling of heat and tingling is also possible.

One of the most common and effective methods of treating this disease is therapeutic massage procedures. They help relieve tension, reduce muscle tone, make them more relaxed, get rid of the clamping of the sciatic nerve, make the limbs more flexible and light. When performing a therapeutic massage, blood circulation improves, the nutrition of soft tissues and bones improves.Puffiness and numbness are removed. The outflow of lymph is normalized and the release of biologically active substances is stimulated, which help to reduce the inflammatory process.

At the first meeting, tell the symptoms. If you have – bring pictures. During massage procedures, techniques such as stroking, patting and rubbing are used. In the first days of the procedure, softer techniques are used that do not cause discomfort to the patient and do not cause unpleasant sensations.After light stroking, a shallow kneading of the back muscles is performed.

In order to completely get rid of the symptoms of the disease, it is recommended to carry out at least 10 sessions. Although the patient can feel the improvement after the first two sessions. However, you should not give up other procedures that will help to consolidate the effect and get rid of relapses. The optimal number of sessions for these diseases is 15. Therapeutic back massage for lumbago and sciatica not only helps to reduce pain, but also improves the general condition of the body.This will help you recover as much as possible.

Herniated disc.
Herniated discs are the result of degenerative changes in the intervertebral discs. Such changes may be associated with metabolic disorders in the body. May be the result of previous injuries or increased physical exertion on the spine. They can also be a consequence of the neglected, during not cured osteochondrosis. As a result of these degenerative changes, the structure of the intervertebral disc is destroyed, which leads to protrusion (protrusion) or prolapse (prolapse) of its nucleus.

When a spinal hernia is detected, complex treatment is recommended. It is important to wait until the pain syndrome is reduced with the help of medication. In especially difficult cases, surgical intervention is possible.

Therapeutic massage is indicated in the recovery phase. You can not carry out massage procedures in the early days of the course of the disease. It can cause spasms of the muscles in the back, increase pain. A prerequisite is that massage procedures should be performed only by a qualified specialist with medical education.Improper performance of massage techniques can lead to serious complications, up to and including surgery. During the session, the masseur excludes strong rubbing and kneading. It is impossible to carry out massage techniques in the place of localization of the hernia, but only in the area of the surrounding tissues. A sign that the massage is being performed correctly is a feeling of relaxation and warmth in the muscles.

The technique is determined each time individually by the massage therapist, depending on the clinical situation.This takes into account the location of the hernia, the symptoms, the course of the disease, the presence of concomitant diseases, the individual characteristics of the patient. It is possible to use different methods of therapeutic massage: point, segmental-reflex, classic, etc. The duration of the procedures is also individually set, which can vary from several minutes to an hour.

One of the most important conditions for the treatment of a hernia of the spine is a systematic approach. This means that massage procedures alone cannot completely eliminate the disease and its symptoms.It is important to combine them with other means of treatment: medicines, physiotherapy exercises, physiotherapy. The task, with the help of massage techniques, is to relieve the pain symptom, improve blood circulation and lymph flow, saturation of tissues and bones with nutrients, decrease the tone and tension of the back muscles.

Indications and general rules
Therapeutic back massage is indicated for various inflammatory and degenerative diseases of the spinal column and the muscles, ligaments and nerve fibers located here.
Among these diseases:

  • osteochondrosis;
  • the initial stages of disc hernias;
  • deforming spondylarthrosis;
  • sciatica, sciatica;
  • the initial stages of lateral curvature of the spine – scoliosis;
  • myositis.

Massaging the muscles of the back and neck leads to the following:

  • pain decreases or disappears;
  • tense muscles relax;
  • relaxation is combined with an increase in tone, strengthening of the muscle frame;
  • local blood circulation improves;
  • inflammatory foci resolve;
  • local metabolic processes are enhanced.

And the very smooth touch of the masseur’s hands provides general relaxation and arouses positive emotions. In addition, massage is not only a treatment, but also, to some extent, a diagnosis. During the massage, the most painful areas are identified. As a rule, these places correspond to the location of pathological foci.

Method
Today, no one knows for sure who and when first developed therapeutic massage. But, one way or another, the technique of massage procedures has been developed over the years.But the basic principles of massage have remained virtually unchanged. All massage effects should be carried out gently, while the patient should not experience any pain or discomfort. Back massage is done while lying on your back. Immediately before massaging, the masseur kneads the hands to make them warm, flexible and sensitive. A special massage gel is applied to the surface of the back, lower back and neck in order to facilitate the sliding of the masseur’s hands over the skin surface.

Back massage begins with stroking, and ends with it. Stroking involves gentle circular motions along the spine up to the neck. Movements can be not only circular, but also zigzag. In this case, one rule should be strictly observed. Although massage is performed to treat ailments of the spine, the massage does not affect the line of the spinal column. This is one of the main differences between massage and manual therapy.

The next step is a deep stroking or weight massage.Burdening involves strengthening the hand by placing the other hand on it. It is deep stroking that allows you to identify the most painful places. After deep stroking, rubbing the massaged areas with the edge or surface of the palm, fingertips. Then kneading is carried out.

Some techniques involve massaging with fists. But fist massage requires a thorough knowledge of the technique from the massage therapist. Weak efforts will not lead to the desired result.And large impacts will cause pain.

Massage for osteochondrosis of the cervical spine
This type of massage is somewhat similar to kneading dough – the skin is gathered into a fold, and this fold moves along the line of the spine. It follows that in a simplified version, back massage looks like this – stroking, rubbing, kneading and then stroking again. During the massage, other types of influences can also be applied – vibration, tapping, patting with the edge of the palm, fingers, and a hand folded in a “boat”.Each type of impact takes about 2-3 minutes, and the total duration of the massage is no more than 15 minutes.

Massage for scoliosis requires special skills and experience. The essence of this pathology is that the muscles on the concave side of the curvature are pathologically tense, and on the convex side they are too relaxed. And therefore, massaging on both sides pursues opposite goals – to relax one muscle group and strengthen another. Scoliosis massage should only be performed by a qualified specialist. Incorrect technique will only aggravate the degree of curvature.And with a maximum 4 degree of lateral curvature, massage is generally contraindicated.

Contraindications
In any case, do not forget about contraindications.
These include:

  • exacerbation of an existing disease of the spine, accompanied by severe pain, severe sensory and movement disorders;
  • exacerbation of any other disease;
  • any infectious diseases, incl.h. influenza, ARVI;
  • high fever for any reason;
  • severe chronic decompensated diseases of the cardiovascular and respiratory system;
  • tuberculosis;
  • any malignant tumors;
  • skin diseases, wounds, abrasions, hematomas in the area of massaged areas;
  • some types of blood clotting disorders;
  • pregnancy;
  • mental illness.

Therefore, before starting the massage, you should consult with doctors (therapist, neuropathologist) and undergo the laboratory and instrumental studies recommended by them.

Pharmacotherapeutic group

Selective beta1-blocker.

ATX code

C07AB07

Pharmacological properties

Pharmacodynamics

Bisoprolol is a selective beta1-blocker without its own sympathomimetic activity, does not have a membrane stabilizing effect.Reduces the activity of blood plasma renin, reduces myocardial oxygen demand, reduces the heart rate (HR) (at rest and during exercise). It has antihypertensive, antiarrhythmic and antianginal effects. Blocking in low doses
beta1-adrenergic receptors of the heart, reduces the formation of cyclic adenosine monophosphate (cAMP) from adenosine triphosphate stimulated by catecholamines, reduces the intracellular flow of calcium ions, has a negative chrono-, dromo-, batmo- and inotropic effect (inhibits conductivity and excitability, slows down atrioventricular conduction) (AV).When the dose is increased above the therapeutic dose, it has a beta2-adrenergic blocking effect.
The total peripheral vascular resistance at the beginning of the drug application (in the first 24 hours) slightly increases (as a result of a reciprocal increase in the activity of alpha-adrenergic receptors), after 1-3 days it returns to the initial one, and with prolonged use it decreases.
The antihypertensive effect is associated with a decrease in the minute blood volume, sympathetic stimulation of peripheral vessels, a decrease in the activity of the sympathoadrenal system (of great importance for patients with initial hypersecretion of renin), restoration of sensitivity in response to a decrease in blood pressure (BP) and an effect on the central nervous system (CNS) …With arterial hypertension, the effect occurs after 2-5 days, a stable effect – after 1-2 months.
The antianginal effect is due to a decrease in myocardial oxygen demand as a result of a decrease in heart rate, a slight decrease in contractility, lengthening of diastole, and improved myocardial perfusion.
The antiarrhythmic effect is due to the elimination of arrhythmogenic factors (tachycardia, increased activity of the sympathetic nervous system, increased cAMP content, arterial hypertension), a decrease in the rate of spontaneous excitation of sinus and ectopic pacemakers, and a slowdown in AV conduction (mainly in the antegrade and, to a lesser extent, in the retrograde directions through AV node) and along additional paths.When used in medium therapeutic doses, as opposed to non-selective
beta-blockers, has a less pronounced effect on organs containing beta2-adrenergic receptors (pancreas, skeletal muscles, smooth muscles of peripheral arteries, bronchi and uterus) and on carbohydrate metabolism, does not cause retention of sodium ions in the body.
The maximum effect of the drug is achieved 3-4 hours after ingestion. Even with the appointment of bisoprolol once a day, its therapeutic effect lasts for 24 hours, due to the 10-12-hour half-life (T1 / 2) from blood plasma.As a rule, the maximum decrease in blood pressure is achieved 2 weeks after the start of treatment.

Pharmacokinetics

Suction
Bisoprolol is almost completely (more than 90%) absorbed from the gastrointestinal tract. Its bioavailability due to insignificant metabolization during “primary passage” through the liver (at about 10%) is about 90% after oral administration. Food intake does not affect absorption. Bisoprolol demonstrates linear kinetics, and its plasma concentrations are proportional to the dose taken in the range from 5 to 20 mg.The maximum concentration in blood plasma is reached after 2-3 hours.
Distribution
Bisoprolol is widely distributed. The volume of distribution is 3.5 l / kg. Protein binding

Emergency care for painful lumbosacral radicular syndromes

The prevalence of neurological manifestations of spinal osteochondrosis in the structure of diseases of the nervous system is very high. Especially often in the work of practitioners there are vertebrogenic lumbosacral radicular syndromes (Brotman M.K., 1975; Limansky Yu.P., Macheret E.L., Vaschenko E.A. et al., 1988; Vinnichuk S.M., Dubenko E.G., Macheret E.L. et al., 2001; Macheret E.L., Dovgy I.L., Korkushko A.O., 2006; Skoromets A.A., Skoromets A.P., Skoromets T.A., 2010). Emergency care in the treatment of patients with vertebrogenic lumbosacral radicular syndromes is extremely relevant in the work of practitioners (neurologists, therapists, family doctors).

In the conditions of the neurological department, we observed and treated 300 patients (210 men, 90 women) with vertebrogenic lumbosacral radicular syndromes.The patients’ age was from 23 to 65 years. All patients were divided into 3 groups: I – with L4-5-radicular syndrome (176 patients), II – with S1-radicular syndrome (74 patients), III – with L4-5 – S1-radicular syndrome (50 patients).

Upon admission to the neurological department, patients usually complained of lower back pain radiating to the leg. The patients associated the occurrence of pain with physical work, hypothermia, unsuccessful movement in the lumbar spine, and their combination.Pain usually occurred immediately after exposure to these factors or after 1–2 days. Usually, the pain began in the lower back, and then spread to the leg (from the side of the root lesion). In addition to the characteristic “pattern” of pain, in a number of cases, patients complained of a decrease in skin sensitivity in the affected area.

Examination revealed muscle tension in the lumbosacral region, smoothness of the lumbar lordosis, scoliosis in the lumbar region, limitation of movement in the lumbar spine, pain on palpation of the vertebral and paravertebral points of the lumbosacral region, along the nerve trunks on the leg from the side of the lesion.With L 4-5 -root syndrome, a decrease in the knee reflex was usually noted, with S1 – the Achilles reflex, with L 4-5 – S 1 – a decrease in the knee and Achilles reflexes on the side of the lesion. In 73% of cases (in addition to reflex disorders), pain hypesthesia was observed in the affected area. Less common was an increase in the corresponding knee and Achilles reflexes, pain sensitivity on the side of the same name with the pain component. Often, patients experienced pain when moving, walking, while sleeping.

Most patients showed positive symptoms of tension (86% of cases). With L 4-5 -radicular syndrome, weakness of the extensors of the toes or only the first toe was often noted.

With a long course of lumbosacral radicular syndromes, in a number of cases (13%), flabbiness, hypotrophy of the leg muscles from the side of the lesion, a subjective feeling of coldness of the foot, an objective decrease in pulsation by a.dorsalis pedis (11% of cases) were observed.

It should be noted that the course of lumbosacral radicular syndromes was different: lighter in group I, heavier in group II and even more severe in group III. Faster regression of radicular syndrome was also noted in group I, slower in group II and even slower in group III.

For the urgent treatment of lumbosacral radicular syndromes, analgin was used (50%, 2.0 ml intramuscularly once a day). For disturbances in nighttime sleep, analgin was injected intramuscularly before bedtime (often in combination with intramuscular injection of 1% diphenhydramine, 1.0 ml).In addition to analgin, diclofenac sodium was administered intramuscularly (3.0 ml intramuscularly every other day, usually in the morning) or dicloberl. In addition, they prescribed nimesil (1 powder 2 times a day) or piroxicam (1 tablet 2 times a day) after meals.

Plasmol (1.0 ml subcutaneously into painful points once a day) or vitreous body (2.0 ml once a day into painful points) was prescribed as an integral element of the complex treatment. For dehydration, furosemide (lasix) 40 mg was used – 1/2 tablet 3 times a day (against the background of a potassium-sparing diet or taking asparkam (panangin) 1 tablet 3 times a day.

Instead of oral administration of furosemide, you can use furosemide 2.0 ml intramuscularly 1-2 times a day or intravenously drip (in saline). The drug L-lysine escinat (5.0–10.0 ml per 200.0 ml of physiological solution) has proven itself in this regard by intravenous drip 1 time per day. In case of peripheral venous insufficiency with radiculitis, troxevasin was prescribed (1 capsule 2 times a day).

As a vasoactive agent, a solution of aminophylline (2%, 5.0-10.0 ml per 200.0 ml of physiological solution) was used intravenously dropwise 1-2 times a day or Actovegin solution (intramuscularly, 2.0 ml or intravenously, 2.0 –5.0 ml per 200.0 ml of saline) once a day.

There was a good positive effect from the combined intravenous drip application of Actovegin (5.0 ml per 200.0 ml of saline) and xefocam orally or in the form of blockades.

At night, suprastin (1 tablet) or diphenhydramine (1% solution of 1.0 ml intramuscularly) was usually prescribed to improve sleep. Finlepsin (or carbamazepine) has a good analgesic effect – 200 mg in 1 tablet. The drug was used 1 / 2-1 tablet 3 times a day. Neuralgin has an even more powerful analgesic effect.It was used according to the following scheme: on the 1st day – 300 mg in the evening, on the 2nd day – 300 mg 2 times a day (day and evening), starting from the 3rd day – 300 mg 3 times a day inside. Attention is drawn to the possibility of dizziness in patients with the use of these drugs, especially in elderly patients or in elderly patients. Patients should be warned in advance about the possibility of side effects when using these drugs.

For anxiety, irritability of patients, small tranquilizers were used: seduxen (relanium, sibazon), gidazepam, phenazepam.With persistent pain syndrome, with the appearance of a depressive syndrome, amitriptyllin (1 / 2-1 tablet 2-3 times a day), rexitin (10 mg) in the morning were successfully used.

For analgesic purposes and to restore conduction along the nerve trunks, intramuscular administration of a complex of vitamins of group B (solution of vitamin B 1 1.0 ml each, solution of vitamin B 6 1.0 ml each, solution of vitamin B 12 each 200 ). The introduction of vitamins B 1 , B 6 , B 12 was usually carried out alternating sequentially on days.Vitamins of group B were more often used for long-term course of radicular syndromes, in the presence of trophic muscle, skin vegetative-trophic disorders.

For the reduction purpose, we used the preparation nucleo C.M.F. forte (1 tablet 2 times a day or intramuscularly 1 time a day) for 10 days, as well as a solution of proserin according to the scheme, increasing its dose daily by 0.2 ml to 1.0 ml, and then by 1.0 ml every day, subcutaneously once a day for 10-15 days. A similar effect was exerted by the drug Neuromidin, 1 tablet 2 times a day (or intramuscularly, 1.0 ml 1 time a day) for 10-15 days.

In the complex of treatment for statodynamic changes in the spine, with increased tension of the back muscles, the drug mydocalm has proven itself, which was used in a dose of 150 mg (1 tablet) 3 times a day (or 1.0 ml intramuscularly) for 10-15 days.

It was found that the effectiveness of the drug increases when combined with small tranquilizers (sibazon, gidazepam).

As a vasoactive drug, pentoxifylline has proven itself well (intravenous drip, 5.0 ml per 200.0-400.0 ml of saline 1 time a day or orally 1 tablet 3 times a day after meals).For a vasoactive effect, nicotinic acid can also be used (1% solution intramuscularly according to the scheme from 1.0 to 10.0 ml once a day, adding 1.0 ml daily to 10.0 ml, and then in the opposite direction), and also a solution of solcoseryl, 2.0 ml intramuscularly or intravenously drip, 2.0-5.0 ml per 200.0 ml of saline. If a solution of nicotinic acid is undesirable in the presence of gastric ulcer or duodenal ulcer, the use of a solution of solcoseryl for these diseases is not contraindicated.It should be remembered that the use of trental is contraindicated in patients suffering from gastric ulcer or duodenal ulcer.

With pronounced painful radicular syndromes, we noted a good effect from the use of blockades in painful points with hydrocortisone, dexamethasone, and diprospan. You should be aware of the side effects of these drugs and use them with caution. For blockades, you can use the injection into painful points, paravertebral into the exit site of the corresponding roots of a solution of novocaine (0.5-2.0%) or lidocaine (2%) in combination with a solution of vitamin B 12 (200-500  1 time per day) – 3-5 blockades daily or every other day.

In complex treatment, rubbing with Finalgon, Nikoflex ointment was also used; fastum gel; olfen plaster; applicators Kuznetsova, Lyapko; lying on a mattress, under which a wooden board is placed. The use of massage (manual or using a Seragem bed), diadynamic currents, phonophoresis (usually with hydrocortisone ointment), darsonval, magnetic laser therapy, superficial acupuncture, physiotherapy exercises, treatment on Evminov’s board were recommended when the pronounced pain syndrome had already somewhat decreased in its manifestation, the patient could calmly endure the prescribed procedures.

In the first days after hospitalization, we usually limited the use of these procedures to patients, since at this time they did not tolerate them well. Depending on which group (I, II or III) the patients were assigned to, the effect of the treatment was different. The most effective treatment was in patients of group I with L 4-5 -radicular syndrome. Usually, during the first 3-4 days after hospitalization, their acute pains diminished, their sleep and mood improved. In patients of group II with S 1 -radicular syndrome, the effect of treatment was less pronounced than in patients of group I, and an improvement in their condition was observed after 5–8 days (in some cases and later).Treatment was less effective in patients of group III. However, in group III, if the L 4-5 root lesion prevailed, then the effectiveness of treatment was higher, and regression of pain and objective neurological symptoms occurred more quickly than in those cases when the S 1 root lesion prevailed. In general, the defeat of the S 1 root (in groups II and III) aggravated the prognosis of the disease (both in terms of the rate of subjective and objective neurological regression, and in terms of recovery in general).

It should be pointed out that when prescribing treatment for patients, one should take into account their individual characteristics, the characteristics of the course of the disease, the presence of concomitant chronic diseases, the patient’s age, and his psychological characteristics. All these points can be of significant importance when prescribing treatment for patients. It is impossible to approach the treatment of patients in a stereotyped manner, without taking into account these features. This reduces the effect of the treatment. Only correctly selected, pathogenetically grounded treatment, especially in cases of S1-root lesion, gives a rather pronounced and quick effect.In necessary cases (with insufficient effectiveness of conservative treatment), neurosurgical intervention is recommended.

The results of the observation and treatment of patients were statistically processed using the Fisher method (F). In group I, the effectiveness of treatment was 92%, in group II – 83%, in group III – 79%.
