The nicotine gum is a type of nicotine replacement therapy (NRT) that can be used to help you stop smoking. Nicotine gum helps to reduce cravings and feelings of withdrawal by replacing some of the nicotine you would normally get from smoking. Nicotine gum is often used together with the nicotine patch, which is a long-acting type of NRT, to help you quit. How to use nicotine gum – the “Park and chew method”

Nicotine gum looks like regular gum, but you don’t use it in
the same way you would use normal chewing gum. Here’s how to use it correctly –
it’s called the park and chew method. There are three simple steps:

Step one: Use the gum when you feel like smoking. Even
better, try to anticipate when a craving might strike, and use the gum before
the craving takes hold.

Step two: Pop the gum into your mouth and start chewing.
When you feel a tingling sensation, park the gum under the tongue or between
your gum and cheek. Park it until the tingling stops and then start chewing
again. 

Step three: Repeat. Park and chew the gum for up to 30
minutes or until the flavour is gone and the urge to smoke has eased.
Then, throw the gum away. 

Some types of acidic
drinks like coffee, fruit juice, soft drinks or beer can affect how well the
gum works. Wait for 15 minutes after having these drinks before using the gum.
The gum also won’t work as well if you take it while eating or drinking.

How the nicotine gum helps

Nicotine gum helps by replacing some of the nicotine you
would normally get from smoking; it can help to manage cravings. Using the
nicotine gum can also help to manage some of the other symptoms of nicotine
withdrawal when you quit such as difficulty concentrating, frustration,
restlessness and anxiety.

What dose should I use?

If your doctor has directed you to use this medication, remember that he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

Serious side effects are more likely if you continue to smoke while using this product.

Recommendation for Long-Term Nicotine-Replacement Therapy May Be on the Way

As a nicotine-gum addict, Mike Metzger used to worry about the warning on its label: “Stop using the nicotine gum at the end of 12 weeks.”

But that was before a prominent smoking-cessation scientist, K. Michael Cummings, told him to keep chewing the gum as long as it helped him stay off cigarettes. So 15 years after quitting smoking, Mr. Metzger, a 63-year old retired New York telephone worker, still chews 16 pieces of nicotine gum a day.

Public health officials are backing away from recommending against the long-term use of nicotine gum, lozenges and patches. The Food and Drug Administration is considering whether to eliminate the 12-week warning and instead recommend nicotine-based quit-smoking aids for extended—perhaps even permanent—use. Anti-tobacco activists and

PLC, the U.S.’s leading marketer of nicotine-replacement therapy, or NRT, are also backing the potential move.

“There really doesn’t appear to be any great harm” with long-term use of NRT, says Mr. Cummings, chair of the department of health behavior in the cancer-prevention division at the Roswell Park Cancer Institute in Buffalo, N.Y.. “You’re better off chewing gum or wearing a patch than smoking.”

By giving former smokers the most addictive chemical in cigarettes—nicotine—NRT is designed to ease the difficulty of quitting. In more than a quarter century on the market, nicotine gum, lozenges and patches have amassed a strong safety record. Even among most long-term NRT users, the hazards of nicotine pale beside the dangers of smoking, which is the number one cause of lung cancer and the most preventable cause of cardiac death.

Nicotine itself is not believed by scientists to raise the risk of developing cancer. It may raise the risk of cardiovascular disease, however. It can elevate heart rate, constrict blood vessels and raise blood pressure. Some people have adverse skin reactions to nicotine patches.

Like other addictive substances, nicotine essentially hijacks the brain’s reward system, conditioning users to associate intake with pleasure. But while other addictive drugs tend either to stimulate or relax users, nicotine can do both, depending on the user’s mood, giving it a particularly tight grip.

Sudden withdrawal from the drug can induce intense cravings, nervousness, irritability, confusion and insomnia, among other symptoms.

Obtaining nicotine from gum, lozenge or patch doesn’t replace entirely the pleasure of smoking any more than popping a tablet of caffeine—the addictive element in coffee—could equal the pleasure of a fresh-brewed cup of cappuccino. But jolts of nicotine can reduce the physical symptoms of withdrawal while quitters battle longings for the taste of tobacco, the sensation of smoke and various rituals that accompany the habit. (Going off NRT can also lead to withdrawal symptoms.)

Studies, typically funded by NRT-manufacturers, have shown that those who use the products are about twice as likely to quit smoking after 12 weeks, compared to those who don’t use NRT. But studies of those who successfully quit outside of NRT clinical trials often show no NRT advantage over the cold-turkey approach.

About one in five American adults smokes—a rate that has remained virtually unchanged since 2004.

NRT makers offer different dosages for heavy and light smokers, and recommend that the frequency of use decline over time, for instance, from a piece of gum every hour or two in the first six weeks of quitting to every four to eight hours in weeks 10 through 12.

Glaxo products—which include Nicorette gum and lozenges and NicoDerm patches—account for more than half of the $835 million market in annual U.S. retail sales of over-the-counter smoking-cessation aids, according to market-research firm Euromonitor International.

Those skeptical of any move to eliminate warnings on long-term NRT use note that many public-health officials and anti-tobacco agencies have received funding from NRT’s makers. “There’s no money to be made in cold turkey quitting, even though cold turkey is how the vast majority of former smokers kicked the habit,” says

John Polito,

editor of WhyQuit, a popular blog that endorses cold-turkey cessation from tobacco.

The FDA has faced increasing pressure in recent years to loosen its rules on quit-smoking aids. Some public-health groups have filed petitions with the agency urging changes.

Another impetus is the landmark 2009 law signed by President Barack Obama that, for the first time, empowered the FDA to regulate tobacco products. The law, which is partly aimed at reducing smoking’s toll on the nation’s health, instructs the agency to consider rewriting the warnings on nicotine-replacement products, which range from eight to 12 weeks, depending on the type.

The warnings date back to when nicotine gum first received FDA approval back in 1984. They weren’t based on hard science connecting health hazards to long-term nicotine addiction, but were set to mirror the length of use in clinical trials.

Britain’s Royal College of Physicians, which sets health standards in the U.K., concluded in a 2007 report on nicotine addiction that “medicinal nicotine is a very safe drug” and “there are no grounds to suspect appreciable long-term adverse effects on health.”

Many other countries have less-stringent policies on NRT. Canada, Germany, Japan and the U.K. have approved recommended-use periods of anywhere from six to 12 months.

A few studies have examined the use of NRT beyond the current recommendations. Last year, researchers at the University of Pennsylvania published a study in Annals of Internal Medicine that found that smokers who use a nicotine patch for 24 weeks may be more likely to stay off cigarettes than those treated for eight weeks.

However, the study reported no difference in the quit rates of the two groups when they were checked at the one-year mark, revealing the fact that many people began smoking again. The senior author of the study, funded by federal grants, has served as a consultant for GlaxoSmithKline.

Barbara Gregg says that without nicotine gum she likely would have returned to smoking. Ms. Gregg, a 41-year-old executive assistant in the University of Minnesota’s department of medicine, smoked for 22 years before she quit by going cold-turkey in 2007. She started chewing the gum about six months later, after experiencing nicotine cravings.

In 2009, before the start of her mother’s funeral, she kept her father company as he smoked a cigarette in the parking lot.

“I can guarantee you that if I didn’t have nicotine gum with me, I absolutely would have bummed a cigarette off my dad and picked back up again,” she says.

My story

Like many startup founders, I smoked cigarettes.I didn’t smoke every day, and sometimes only to relieve stress or fatigue. Of course, I understood that it was harmful.

It all started at a party: after a few glasses of alcohol, I started asking friends and strangers for cigarettes. Then I did not consider myself a smoker. I thought that overall I was in good health: I exercised almost every day and rarely bought cigarettes.

When I was younger, I was more often at such parties, like many of my friends. We believed that in our environment there were only a couple of people who were “real” smokers, because they smoked every day. But we were all smokers. Like my friends, the morning after another party I woke up with a feeling of discomfort: smell, cough, headache.

Once my wife said that it was time to become more responsible and quit a bad habit.I promised her that I would not smoke anymore. Such a promise is easy to make, but difficult to keep. So I started looking for anything that could help me.

First there was “nicotine replacement therapy”, it is also called NRT. Many different nicotine chewing gum products were sold in pharmacies.

From the instructions, I realized that I needed to take chewing gum with 2 mg of nicotine in a strip, since I did not smoke a cigarette within 30 minutes after waking up.But with this concentration, there was little effect. Then I took a chewing gum that contained 4 mg of nicotine.

The same evening I drank with friends, I wanted to smoke. I had to try hard to get the chewing gum – its packaging was inconvenient. The gum itself turned out to be tough and tasteless, but then I received a dose of nicotine and decided that I would continue to chew them.

Later the chewing gum has run out. And I realized that it was much easier to buy a pack of cigarettes: it was inconvenient to get to the pharmacy where the chewing gum was sold, it was not in the minimarket, I could not buy it online.

Regular nicotine chewing gum: inconvenient packaging, unpleasant taste

I thought that it would be easier to quit smoking with my friends, so I suggested that they get rid of the bad habit with me.But everyone refused with the words: “One day I’ll just quit”, “I don’t want to quit, I don’t need it”.

Some friends said that they had already tried nicotine gum, but they did not like the packaging, and the product itself caused nausea. Others doubted that it worked at all.

I told them that nicotine chewing gum is not as bad as cigarettes, but I realized that their dissatisfaction was justified.The old nicotine gum was created as a medicine. They had to be taken for 12 weeks, after which the person must quit smoking. No one thought that their packaging should be easy to open, and the gum itself should be pleasant to the taste.

This method of fighting smoking was not suitable for me and my friends. So we started looking for something better.

John Coogan is the co-founder and CTO of Soylent.He worked in consumer goods like me, but he also had experience building e-commerce platforms.

Sami Hamdush holds a UCLA degree in biophysics and biochemistry and was the head of the development department at Soylent. There, he applied his knowledge of biotechnology to everything from formulation development and clinical trial management to regulatory compliance and manufacturing.

Science

Nicotine is a chemical stimulant. Humanity has been using nicotine in various forms for thousands of years. Even if there were bans or restrictions on the use of nicotine, people still wanted to experience the sensations it gives.

Like any psychotropic substance, nicotine has its advantages and disadvantages. Research shows that nicotine can improve cognitive activity in the brain, help control hunger, but on the other hand, it can be addictive.

However, not all forms of nicotine intake can be addictive.Studies have shown that people who chew nicotine gum are less likely to be addicted. One reason is that the absorption of nicotine from chewing gum is slower than smoking.

When inhaled, nicotine enters the brain even faster than after injection. Another reason is that nicotine chewing gum lacks certain enzymes, such as monoamine oxidase, which is present in tobacco, and reinforces the habit by “rewarding” the brain for smoking cigarettes.

Also, addiction is more likely to occur in those who smoke frequently and in children whose brains are still developing. Almost 95% of addicted smokers started smoking before age 21, which is why large tobacco companies (and companies selling vapes) target young people.

But despite the fact that smoking causes one in five deaths each year, nicotine has nothing to do with it.According to Scott Gottlieb, FDA spokesman, the problem is “the delivery mechanism, not the nicotine itself.”

The fact that smokers do not know the scientific facts about nicotine is bad in itself. But most non-tobacco nicotine products are also unpleasant to consume.

Opportunity

I have not smoked for over three years. This I am proud of. I no longer smoke, do not use other tobacco products, but continue to chew Lucy.
