Scar after caesarean section – WMT high-tech clinic in Krasnodar
The number of births by caesarean section increases every year and reaches about 40%, so women are more likely to deal with the problem of an inconsistent (thinned) scar on the uterus. The main cause of scar failure is chronic sluggish metroendometritis, the main symptoms are prolonged postmenstrual spotting and pain in the lower abdomen.
DIAGNOSIS
Diagnosis of an incompetent scar on the uterus at the stage of pregnancy planning is especially difficult, since it is difficult to make a correct diagnosis only on the basis of ultrasound results. Therefore, the WMT clinic uses the entire range of diagnostic measures to clarify the diagnosis:
- Ultrasound of the small pelvis (“niche” in the projection of the scar, the thickness of the scar is less than 3 mm, the absence of vascularization in CDI indicates failure)
- Hydrosonography with anechoic contrast. The study is carried out on the 4-8th day of the cycle. A saline solution is injected into the uterine cavity, which allows obtaining objective information about the state of the scar, the nature of the blood flow in the scar, and the reserve of intact myometrium.
- Office hysteroscopy. It is performed on the 4-6th day of the menstrual cycle, retractions in the scar area, the absence of blood vessels are detected, which indicates a pronounced predominance of fibrosis. It also allows you to identify volumetric formations in the scar area, endometriosis of the scar and changes in the scar zone with the formation of multiple cavities (niches) in the myometrium.
- MRI of the pelvis with an assessment of the condition of the scar.
TREATMENT OF FAULTY SCAR
When the diagnosis of an incompetent scar on the uterus is confirmed at the stage of preconception preparation, the surgeons of the WMT clinic perform a reconstructive plastic surgery – metroplasty. The operation can be performed by laparotomy (open), laparoscopic and vaginal access. Preference is given to laparoscopic access. During surgical intervention, economical excision of the connective tissue scar and layer-by-layer restoration of the uterine wall are performed.
Indications for surgical treatment:
- Thinning of the myometrium in the area of the scar 3 mm or less.
- Thinning of the scar on the uterus in combination with endometriosis and foreign bodies (encapsulated ligatures) in the structure.
Assessment of the consistency of the scar after metroplasty is carried out after 1, 3, 6 and 12 months using ultrasound. Only after receiving data on the satisfactory condition of the scar, you can plan a pregnancy.
ULTRASONIC SIGNS OF A POSITIVE RESULT OF THE OPERATION:
- The position of the scar corresponds to the area of the operation.
- There are no deformations, “niches”, areas of retraction from the side of the serous membrane and the uterine cavity in the scar zone.
- The thickness of the myometrium in the region of the lower uterine segment is equal to the thickness of the anterior wall of the uterus.
- Adequate blood flow in the scar area.

At the preoperative appointment, the surgeon-gynecologist will select the most appropriate method of performing the operation, taking into account all the factors individually for each patient.
Currently, an increasing number of pregnancies end in operative delivery by Caesarean section. In this regard, the frequency of such a pathology as the failure of the scar on the uterus has increased. It is almost impossible to avoid this complication by performing certain actions – it all depends on the woman’s body, her reaction to the suture material, the location of the uterus and the situation that arose earlier, which led to the use of surgical delivery.
A uterine scar after a caesarean section may or may not be successful. A wealthy scar has sufficient thickness and does not contain any cavities (niches). Such a scar is quite elastic and can stretch and withstand the load during pregnancy and childbirth. An inconsistent scar is distinguished by a small thickness, the formation of recesses (niches). Need to treat an inconsistent scar on the uterus? The answer depends on the patient’s future plans for childbearing, on the symptoms that bother her. The decision is made individually in each case!
Pregnancy with an incompetent uterine scar is associated with a risk of uterine rupture during pregnancy and childbirth, rotation of the placenta into the scar area.
All these conditions threaten the life of the patient and the fetus. Also described is the perforation of the uterus in the niche area when an intrauterine device is inserted into the uterine cavity. Diagnosis of insolvency of the scar on the uterus is complex and requires the use of a complex of instrumental studies. Ultrasound is an important method for assessing the course of reparative processes and evaluating the viability of the scar of the operated uterus. Ultrasound criteria for a complete failed scar on the uterus: complete or partial (thinning of the myometrium up to 2 mm) defect of the myometrium in the form of a niche (depression) in the projection of the scar on the uterus. Also characteristic is the heterogeneity of the scar and a decrease in blood flow (vascularization) in the scar area, which indicates the presence of coarse scar tissue. Magnetic resonance imaging (MRI) of the pelvis is a fairly accurate method for assessing the condition of the scar on the uterus after cesarean section. MRI allows you to determine the size of the niche and the thickness of the uterine wall in the area of the scar, assess the condition of the periuterine tissue and the severity of the scar process in the vesicouterine space.
If a failure of the scar on the uterus is detected or it is suspected, according to the above studies, a diagnostic hysteroscopy is indicated. With the insolvency of the scar, the presence of a niche, retraction in the area of the scar is noted. The paucity of the vascular pattern and the whitish color of the scar tissues are often noted, which indicates the predominance of the connective tissue component. Sometimes you can find the remnants of suture material and endometrioid cysts in the scar area. This pathology is treated only by surgery. The most effective method of scar plastic surgery is through laparotomy access. Laparotomy is a surgical intervention in which all manipulations are carried out through an incision on the anterior abdominal wall of a woman. laparotomy is accepted by the surgeon and depends on many factors, such as comorbidities, previous surgical interventions and the presence of a postoperative scar in the patient.
In most cases, a transverse suprapubic approach (according to Pfannenstiel) is used to perform uterine scar plasty. After entering the abdominal cavity, the gynecologist surgeon examines and evaluates the internal organs, highlights the uterus, evaluates the scar.
Complete or partial excision of an incompetent scar on the uterus within healthy tissues, if necessary, separation of adhesions. After that, the uterine wall is restored with several rows of sutures. Absorbable material is used to suture the uterine wall.
All removed tissues must be sent for histological examination.
After inspection and cleansing (sanation) of the abdominal cavity, control of the absence of bleeding, the instruments are removed. The anterior abdominal wall is sutured.
