Purpose of venous catheters and their types

A standard venous catheter is usually a long, flexible tube of small diameter, one end of which is inserted into the vein and the other end is brought out. The catheter is installed under sterile conditions and is used for long-term infusion of drugs into the patient’s veins.

Venous catheters are of two types – peripheral and central. The peripheral venous catheter is intended for the introduction of drugs into peripheral veins: the area of ​​the head, neck and arms. The central venous catheter is placed in the central veins: femoral, subclavian and jugular.

Catheters for intravenous administration are also distinguished by the material of manufacture. The simplest option is polyethylene, however, it is not suitable for long procedures. Therefore, for patient safety, it is recommended to buy Teflon and polyurethane IV catheters.

8. A critically ill patient requires access to the circulatory system.
9. Anesthesia needs to be provided to a patient

A central venous catheter is used when it is necessary to administer a large amount of drugs to the patient, as well as substances that irritate the vascular walls (for example, chemotherapy drugs). Also, catheterization of the central vein is performed when, due to the physiological characteristics of the patient, it is not possible to carry out catheterization of peripheral veins.

Cervical insemination

Traditionally, insemination takes place in the cervical way. The catheter is inserted into the cervix, the narrow part that connects the vagina and uterus. This is the place where the seed is located, the contracting movements of the cervix move it towards the uterus. In fact, traditional insemination is, to some extent, insemination carried out by the sow herself.

Therefore, it is extremely important to properly detect heat to ensure that the sow is in heat. The gilt and sow should be stimulated with a boar held in front of them in such a way that insemination is as close as possible to natural mating. The catheter must be properly positioned in the cervix and avoid excessive pressure.

What does it mean to “avoid hard pressure on the catheter?”

In order to make insemination faster and easier, hands-free insemination is becoming more and more popular in Spain and other countries. Care must be taken with these systems: in conventional insemination, when the catheter is inserted, the sow contracts the cervix and clamps the catheter head for a few seconds or even minutes. Indeed, if at this moment we pulled the catheter back, we would feel that it does not come out, i.e. blocked.

After a few minutes, after the neck has adapted to the catheter, it relaxes. Any employee conducting insemination has seen many times: the catheter is inserted, but the semen does not pass, then suddenly, without any pressure, the sow begins to quickly “absorb” the semen. This is because the cervix relaxes, allowing it to pass through, as if in natural mating.

If we are working with the hands-free insemination technique (Photo 1), we apply pressure to the catheter, which is usually stiff enough to adapt to the semen dose retention device, and in doing so, we apply force to the catheter. As long as the neck is contracted, this is not a problem if the catheter is inserted correctly, but once the neck is relaxed, if the catheter is under pressure, at an angle greater than 90º in relation to the ground, it can come out back. Therefore, the catheter can move, which means that the semen can go into the vagina and not into the cervix, which means that the reverse movement of the semen is possible. In addition to this problem, there is the fact that in self-insemination, usually several sows are inseminated at the same time, which means that the probability of not noticing the return flow of semen is higher, therefore, the risk of avoiding this problem by introducing a catheter increases.

Is it correct that the catheter remains inside the sow for some time after insemination?

It must be remembered that cervical insemination is insemination by “filling”. A large amount of semen is located in the cervix, and from there it enters the uterus. For this reason, it is advisable to leave the catheter in the cervix for a few minutes, it acts as a plug while the sow moves the semen into the uterus. However, this happens within a few minutes of insemination, so if we leave the catheter on, it’s only for a short time – never an hour or more, which sometimes happens.

If we leave the catheter, we should use a catheter with a plug on the end or “close” it, as they do on some farms, by bending and fixing it with plasticine or in some other way. The type of catheter used also matters. Ribbed or foam catheters have a larger diameter, so it fills the cervix more and closes it well. Coiled catheters are less likely to block the cervix as they are narrower and leave more space between the catheter and the cervical wall, which encourages back flow of semen if the catheter is left inside after insemination

Hygiene during insemination

Undoubtedly, the whole process of insemination must be carried out in a hygienic manner. It is necessary to keep the pen under the sow clean, clean the vulva, open it wide so that dirt does not get inside with the catheter, do not touch the head of the catheter with your hands, etc.

Yet, as with any disease, doing the same things will lead to different results on different farms. Some farms can work well in relatively clean conditions, others require maximum cleanliness. It depends on the infectious load, the immune system of the sows and especially the time of insemination. Farms that are inseminated multiple times at the start and end of the heat are more at risk of insemination-associated infections.

Is one type of catheter better than another?

There is no ideal catheter. They all work well when used correctly. However, it is necessary to understand that they are different and to some extent are introduced differently.

Coiled catheters mimic the shape of a boar’s penis and must be inserted with a twisting motion. This is a softer and less traumatic insertion. I find them the most suitable for the Iberian breed of sows, as they don’t like any kind of contact.

Ribbed or foam catheters must be inserted in one motion. When, on the contrary, farmers introduce them in several movements, the sow becomes restless and reacts with a contraction of the cervix. As a consequence, when the cervix is ​​fully or partially closed, the catheter will be located in the vagina or dangle freely at the beginning of the cervix.

In my experience, misplacement of the catheter is one of the most common mistakes on farms, often resulting in no pregnancy.

Insemination of a sow that has been stimulated with a boar wire in front of her, insertion of a catheter according to its type, adherence to the necessary hygiene procedures, attention to detail, all this is extremely important if we want to achieve maximum results from artificial insemination.
