- Laser. A laser is used to clear new and deep seated breakouts.
- Deroofing. Breakouts are turned into scar tissue (a procedure called deroofing).
- Draining abscesses. Abscesses can be drained to provide some relief.
- Excision (cutting out) of the lesions and skin grafting. In this type of surgery, larger areas of the skin and scar tissue are removed. A skin graft from a healthy area of the body may be needed to replace the lost skin.
What are the complications of hidradenitis suppurativa?
Hidradenitis suppurativa occurs in cycles throughout a patient’s lifetime, with periods of breakouts and healings. In some cases, the disease progresses and causes abscesses (pus-containing areas of infection) to form deep within skin tissue. Continued healing and scarring can cause that area of the skin to become thick and in extreme cases, the scarring makes the affected area firm and difficult to move. Continued healing and scarring can also cause fistulas to form inside the body. Fistulas are hollow passages that are painful and require surgery to repair.
What are the outcomes after treatment for people with hidradenitis suppurativa?
Some people with the disease have recurrent outbreaks for the rest of their lives. Some people who treat the disease early with surgery have no further symptoms. In rare cases, hidradenitis suppurativa clears up without any treatment.

Conventional beauty wisdom suggests that beauty is about embracing your imperfections and loving yourself as you are. But there are some beauty problems that are unseemly, weird, and almost never discussed in beauty journalism. Rare is the magazine newsstand cover that screams, “7 Ways to Pluck Your Chin Hair!” This week, we’ll be addressing a lesser-heard-from beauty problem every day, starting with this one: buttne.
As straightforward as the name implies, buttne is literally acne on your ass. “Dude, it’s a thing,” my friend told me when I asked about her embarrassing beauty problems. It’s a cruel world that permits congested butt pores to exist. Dermatologists I spoke to confirmed that it is, indeed, a real thing — and it’s getting worse as more and more patients seek dermatologists for treatment. “I have seen quite a bit of acne on the buttocks as of late,” said Dr. Carlos Charles, a dermatologist practicing in New York. Another, Dr. Elizabeth Tanzi, agrees: “We are seeing a lot more breakouts on the butt.” Below, everything you need to know to spot (and treat) acne on your bum.
1. How can you tell whether you have buttne?
Red bumps on your butt: not good overall. But sometimes what you think is butt acne could be something else. If they’re tiny red bumps centered around hair follicles, they could instead be hair-infected follicles. If that’s the case, try loosening up your clothes. Try to wear cotton underwear (hey, it’s normcore), which is more breathable than nylon or silk.
2. What’s causing it?
As everyone has been telling you, your yoga pants aren’t meant for everyday wear. Your Lululemons might be to blame. As Dr. Tanzi explains, “People tell me, ‘I’m always in my yoga pants’. And I’m like, that’s not meant for every day!” You know it’s serious when a doctor tells you to give up the yoga pants. It works like this: Your yoga pants are tight. They’re sweaty. They stick to your butt when you sit or even when you’re not sitting. So does bacteria and sweat, which clog pores. Dr. Charles puts it this way: “Sweat that sits on the body for extended periods of time leads to the overgrowth of bacteria that can exacerbate acne.”
3. Did you try to make your butt shine as brightly as Kim Kardashian’s?
Don’t. Save the Paper butt reenactment for Photoshop (or do it with water). A too-slick moisturizer like a body or butt oil could clog pores in an already-sensitive area. Your butt needs air, not shine.
4. Did you try to pop it?
You can pop that booty but don’t pop that blemish. Just as you should (not) with acne on the face, you generally shouldn’t try to pop acne, as doing so badly can cause infection.
5. Do you use a medicated body wash?
You should. Put aside the good-smelling body washes for now. The solution for treating butt acne isn’t much different from face acne. Both Dr. Charles and Dr. Tanzi suggest trying topical over-the-counter solutions, like a body wash with salicylic acid or benzoyl peroxide. Try Murad’s Clarifying Body Spray, Neutrogena’s Body Clear Body Wash, Phisoderm Anti-Blemish Wash, Topix Benzoyl Peroxide Wash (10 percent benzoyl peroxide), or the Body Shop’s Tea Tree Wash (for a natural solution). If using a body wash containing benzoyl peroxide, switch to white towels. “It will bleach your towels, but it works really nicely,” Dr. Tanzi explains. Depending upon the severity of the acne, look for washes with acid percentages of 3 percent and up. A dermatologist can also write you a prescription for an antibacterial cream like Clindamycin and in more extreme cases, even prescribe Accutane.

Is It Hidradenitis Suppurativa or Something Else?
Hidradenitis suppurativa (HS) is a rare condition that causes painful red bumps to form on or under your skin. The bumps can break open and ooze fluid.
It can be hard to diagnose, because red bumps can also be a sign of pimples or other common skin problems. And the symptoms of HS can change. Your skin can go from clear to a severe breakout in just a few days.
Although there’s a good chance that it’s not HS, you should see a doctor to find out, because if you do have it, you need treatment to stop it from getting worse. Otherwise, it could leave scars on your skin and cause serious infections.
These skin diseases can look like hidradenitis suppurativa:
Acne
It’s easy to confuse HS with acne, because the bumps look a lot alike in both conditions.
You’re much more likely to have pimples. About 85 out of every 100 teens and young adults get acne. In comparison, less than 1 out of 100 get HS.
Acne starts when pores and hair follicles become clogged up with oil, dead skin cells, dirt, and bacteria. Hidradenitis suppurativa starts when a protein called keratin and other substances build up in the hair follicle and plug it up. It can cause a bacterial infection.
One way to tell the difference is by where the breakouts are. Acne usually forms on the:
- Face
- Neck
- Back
- Chest
- Shoulders
Hidradenitis suppurativa often shows up in skin folds, such as the:
- Underarms
- Breasts
- Groin
- Buttocks
- Stomach
- Nape of the neck
- Backs of the ears
Sometimes HS can appear on your face or back. The bumps heal and then come back.
You can pop acne and release the pus. HS bumps break open on their own and release a foul-smelling liquid.
Cysts
These are small fluid-filled sacs on the skin. The fluid inside is white or yellow, and thick. It’s made from broken-down keratin.
A clogged follicle or injury to the skin causes cysts to form.
Cysts can appear in some of the same places as acne, including the:
- Face
- Scalp
- Behind the Ears
- Neck
- Chest
Hidradenitis suppurativa bumps often go away, only to return. They can spread to other areas. Cysts go down, but can get re-inflamed. They don’t spread unless something else causes them.
Folliculitis and Boils
Folliculitis is an infection of the hair follicles. Bacteria or fungi get trapped in the follicles, which swell up into red or pus-topped bumps. You may also hear folliculitis called razor burn or hot tub rash.
The bumps are itchy or painful. They can break open and spread to other areas of your skin.
HS can also look like boils, which are a painful red or pus-filled bump. Boils often form on parts of the skin where hair grows, such as on your:
- Face
- Neck
- Armpits
- Shoulders
- Buttocks
- Eyelids — this is called a stye
HS tends to be long lasting and hard to treat, while folliculitis and boils tend to respond well to treatment with antibiotics and don’t usually come back.
Diagnosis
See a doctor if:
- The bumps on your skin hurt
- They don’t go away in a few weeks
- Your skin clears up and then bumps form again
- You have breakouts on many parts of your body
Your doctor will first ask about your symptoms and look at the affected areas of skin.
Three signs can help your doctor tell whether you have it and not another skin condition:
- Painful red bumps deep in your skin in more than one area
- The bumps are in areas where skin rubs against skin, like the underarms, groin, and buttocks.
- The bumps went away but then came back.
The doctor might remove a sample of the fluid from inside the bumps. The fluid will go to a lab and be tested for infection.
Basic Grooming: How to Beat the Butt Rash
These hot summer months can cause a lot of swamp ass, and therefore rashes that result from that radioactive concoction of sweat and bacteria brewing between them cheeks. To help you keep your own personal Grand Canyon fresh, I asked four very different butt rash experts how it happens and, more importantly, how to make it go away.
Anthony Rossi, dermatologist: That area doesn’t get a lot of airflow, especially in between the butt cheeks. That can prompt irritation, moisture buildup and rashes — just all over irritation. Keeping things cool and dry is the first line of defense. If you know you’re prone to this, you should be wearing loose-fitting, breathable fabrics — cotton is very breathable, but spandex is going to trap moisture there.
We also tell people, after they workout, they should shower and change. If it’s really bad, you can use zinc oxide butt paste, like baby diaper paste. I tell my patients to use that, since it prevents the skin from rubbing against itself and prevents chafing. It’s antibacterial, too, so that’s really helpful. I also tell people to carry around wipes, which cool down the skin and get rid of moisture and bacteria.
Melanie Mari, trained manscaper at Bare Skin Studio: Keep the area clean and washed! Literally scrub your butt with some sort of exfoliant, especially after shaving it down or waxing it. You can use a basic plain washcloth to exfoliate your butt — the big plus with exfoliating after shaving or waxing your butt is that it helps untrap hairs that would get stuck otherwise. Wearing cotton or something moisture-wicking helps, because when you’re sweating, the sweat can get trapped underneath your skin, and that’s part of what causes irritation and “buttne.” Baby fresh ointment helps a lot down there, too.
Ethan, a fitness (and butt!) model: You may be a little disappointed with my reply, because when it comes to looking after my butt, I don’t actually do any skincare, just basic body wash in the shower. I always shower as soon as I can after exercise, though. Underwear-wise, I wear briefs instead of boxers, as boxers can bunch up and make things a little too warm when exercising.
Devin Feldman, a cyclist who knows a thing or two about butt rashes: A big thing for prevention — and it sounds so obvious — but you need to keep your shorts clean! If you don’t keep them clean, you’re just asking for some sort of infection.
Another thing is to avoid sitting down for a long time, especially on hard surfaces, when your butt is sweating. The big thing is ventilation, so if you’re sitting on a hard surface, it’s 90 degrees out and really humid, you have to stand up every once in a while. When you get home, putting some baby powder down there also goes a long way. It kinda dries everything out and keeps it from rubbing together too hard.
This tip is especially helpful for when you go to the beach — the sand and saltwater will kill you [or at least your butt] — but definitely rinse super well after any activity, and bring a change of clothes if you can. Are Your Lululemons Giving You a Rash on Your Butt?
I have been taking a new fun dance class called DanceBody that is very popular in Miami and New York. You get really sweaty in the class and, because I am a dermatologist, I get asked a lot of questions about the skin issues that result.
Lately, there have been a lot of questions about an itchy rash on the buttocks that lasts about 24 to 36 hours after sweaty exercise. All of the cases I have seen in my dermatology practice have been after wearing Lululemon leggings, so at first, I thought this was a reaction to the fabric. After doing some research, I found that this rash is probably due to an infection called pityrosporum folliculitis.
What Is Pityrosporum Folliculitis?
This infection is caused by a type of yeast called pityrosporum, and has been very common lately. What happens is this yeast makes its way down into the hair follicles, where it multiplies and causes an itchy rash. Like many other microbiota, pityrosporum is commonly found on healthy skin. However, when it becomes overgrown, it can cause this unpleasant rash. So what makes this yeast overgrow? It likes warm, moist environments, which Miami certainly has. Then, when you add tight-fitting clothing like leggings and sweaty exercise, you create the perfect environment for the yeast to thrive.
Interestingly enough, this problem has been so common lately that it has been discussed in a closed Facebook group of board-certified dermatologists. We have found that the rash clears up with topical steroids, but will come back. It does not get better after treatment with oral doxycycline or topical benzoyl peroxide and retinoids, so it is not acne. It is also not a reaction to the fabric or bacteria that is on the skin from exercising, although we are still not sure why it is more common with Lululemon leggings. It could just be because so many people wear them, or that we happen to see it with this particular legging.
Acne vs. Pityrosporum Folliculitis
Pityrosporum folliculitis often resembles acne, but it is unrelated. In fact, misdiagnosing and treating this infection as acne can make it worse. The main difference in symptoms between these two common skin conditions is that acne is rarely itchy, whereas pityrosporum folliculitis does cause itching. To further complicate matters, these two conditions can and often do occur together, so it is best to consult a board-certified dermatologist for an accurate diagnosis and treatment recommendations.
How to Get Rid of Rash after Exercising
As mentioned above, if you have developed a rash after exercising and wearing tight-fitting clothing like Lululemons, schedule a visit to your dermatologist’s office. He or she can prescribe you a few different things to get rid of this rash:
- Ketoconazole shampoo. Your doctor will give you specific instructions, but you typically wet your hair then apply the shampoo and work into a lather. Gently massage it into your hair, then rinse with warm water and dry with a soft towel. Your doctor may ask you to repeat this every other day until the rash clears up.
- Ketoconazole cream. If the rash is on your body and not your forehead or scalp, your doctor may prescribe you ketoconazole cream instead.
- Oral itraconazole. In some more severe cases, oral itraconazole may be combined with the topical treatments mentioned above. Follow your doctor’s treatment instructions and be sure to follow-up with him or her to keep track of your progress.
How Can I Prevent Getting a Rash from My Leggings?
If you notice this rash frequently, even after properly treating it, you might need to consider switching your workout gear to something that fits more loosely. If you’re in love with your Lululemons and prefer to work out in leggings, remember these tips:
- Change out of your workout clothes directly after exercising.
- Ideally, take a shower immediately after your workout. Don’t go to brunch or run errands in between.
- If you don’t have access to a shower, use wet cleansing wipes on sweaty areas of your body after a workout and change your clothes.
In Summary
If you get a rash on your buttocks or other particularly sweaty areas of your body after a good workout, a yeast infection called pityrosporum folliculitis could be the culprit. Before assuming this itchy rash is acne, talk to your dermatologist, because the proper treatment is key to getting rid of it. Do you suffer from bumne?
Bumne sounds like it should be added to the list of “body acne” alongside chestne and bacne. However, bumne isn’t actually acne despite what its name leads you to believe. Lumpy, bumpy skin and dry, red rashes on your butt are most likely the result of folliculitis or keratosis pilaris – two skin conditions that stem from inflammation or blockages within our hair follicles (read: pores).
I can see where the “bum acne” confusion comes from – especially since keratosis pilaris is classed as an acneiform disorder, which means it mimics acne but is nothing of the sort.
So, let’s chat about these skin conditions that are affecting our peaches. As I mentioned, folliculitis is the inflammation of your hair follicles. You can develop folliculitis anywhere you grow hair – it’s not just reserved for your butt. It appears on the skin as a rash of shallow red bumps which can blister, develop a whitehead and be sore, itchy or tender as well. The main cause of folliculitis is a fungal or bacterial infection in the upper part of the hair follicle, but shaving, wearing restrictive clothing and excessive sweating can also trigger the skin condition.
Or perhaps your bumne stems from keratosis pilaris, otherwise known as “chicken skin”. The patches of teeny hard bumps slightly resemble the rough-textured skin of chicken, hence the name, and can crop on your arms and thighs as well as your butt. It can feel dry or itchy and the colour of the bumps can vary, so don’t worry if yours are white, red, skin-toned or darker.
To get nerdie, keratosis pilaris occurs when our hair follicles get clogged with a build-up of keratin, a structural protein that’s found in our hair, skin and nails. This creates a “keratin plug” which blocks the opening of the hair follicle which looks like a hardened lump on the skin.
I would give you the same advice whether you’re suffering from keratosis pilaris or folliculitis: try to wear loose clothing and breathable cotton underwear. To prevent a build-up of bacteria, clean the affected area with an anti-bacterial body wash immediately after working out or if you’ve been sweating a lot.
Mild cases of folliculitis should clear up naturally, whereas keratosis pilaris can take its time – luckily there are steps you can take to speed up the process. You can use skincare products that’ll gently exfoliate, regulate and hydrate the areas affected by keratosis pilaris or folliculitis.
