Pfizer will terminate a trial of the anti-obesity tablet drug lotiglipron due to potential liver-related side effects. The company will now focus on another investigational drug in the same group, danuglipron. Meanwhile, Pfizer’s competitors in the development of an oral weight loss drug reported on their developments. Eli Lilly has published the results of a phase II trial of orforglipron, and Novo Nordisk continues to expand its semaglutide-based portfolio and is preparing to file with the FDA for approval of an oral form of the drug with an increased dosage and potency equal to the blockbuster Wegovy injection.

Lotiglipron from Pfizer. Pharmacokinetic data from a phase I study and laboratory data from a phase I and an ongoing phase II study revealed elevated levels of liver enzymes in patients who took lotiglipron once daily. Elevated enzyme levels often indicate damage to liver cells, but the company said that no patients complained of liver-related symptoms or side effects.

However, on June 26, the company announced its decision to stop the study of the drug. The announcement sent Pfizer stock down 3.6% by that evening.

Danuglipron from Pfizer. Pfizer’s hopes lie with another oral anti-obesity drug, danuglipron. Preliminary data from a phase II study showed that weight loss in patients with type II diabetes who took danuglipron twice daily for 16 weeks was up to 4.17 kg. The drug is currently undergoing a stage 2b study in obese non-diabetic patients. CT results are expected by the end of 2023.

Pfizer added that it is also developing a version of danuglipron that patients can take once a day instead of twice.

Pfizer CEO Albert Bourla said the obesity pills could end up making the company $10 billion a year.

Orforglipron by Eli Lilly. On June 23, an American company announced the publication of interim results from a phase II study in The New England Journal of Medicine. Overweight patients who took orforglipron once daily lost 14.7% of their body weight after 36 weeks.

Semaglutide from Novo Nordisk. A Danish company published data at a diabetes conference on June 25 showing that a new drug with increased doses of semaglutide helped overweight adults lose 15.1% over 68 weeks in an advanced CT. The company also said it plans to file for FDA approval before the end of 2023.

Novo Nordisk tablets are an oral version of semaglutide, the active ingredient in Ozempic and Wegovy’s popular injectable weight loss products.

Lotiglipron, danuglipron, and semaglutide are in a class of drugs called glucagon-like peptide-1 agonists. They mimic a hormone produced in the gut called GLP-1, which signals the brain when a person is full.

Novo Nordisk already has FDA-approved and RF registered semaglutide in tablet form, sold under the brand name Rebelsas, for the treatment of type II diabetes. But the highest dose of the drug is 14mg, while the company’s experimental obesity pills contain a much higher dose of 50mg.

New evidence suggests that high-dose semaglutide tablets may be as effective as Novo Nordisk’s blockbuster weekly injections of Wegovy in the US. According to the CT, taking Wegovy results in about 15% weight loss after 68 weeks. However, according to the Danish company, pills would serve as a much more convenient way to treat obesity.

In addition to Rebelsas, injectable Ozempic was also sold in Russia based on semaglutide. At the end of 2022, the company notified Roszdravnadzor of plans to stop the supply of both drugs. Ozempic has already disappeared from pharmacies and is supplied only on a discount, but its supply will continue until the end of 2023, like Rebelsas.

In March 2023, Eli Lilly entered into an agreement with the Swiss Swixx BioPharma to transfer the rights to import and sell its products in Russia. In May of this year, it became known that Novo Nordisk could transfer the supply of drugs in Russia to a third-party distributor.
