Most people assume that a breakout of pimples on the butt is acne. In fact, butt acne isn’t really acne. Acne is a skin condition involving clogged pores. It causes small bumps that can look like acne. Folliculitis is inflammation of the hair follicle, which can also look like acne. The inflammation can be the result of irritation or infection. Contact dermatitis is another possible cause of a butt breakout.

Keratosis pilaris is very common and tends to run in families. Experts don’t really know what causes it. Folliculitis is the result of damage to the hair follicle that then causes irritation or allows germs to infect it. The damage can come from friction on the skin, wearing tight clothing, or shaving. While keratosis pilaris bumps usually don’t hurt or itch, folliculitis can be uncomfortable or painful.

“Butt acne” treatment depends on the underlying cause of the pimples. If it’s keratosis pilaris, you probably have similar bumps on the back of the upper arms or thighs. Moisturizing the skin right after showering can help soften the skin. An exfoliant cream containing lactic acid or similar ingredients can encourage the skin to turnover faster and prevent plugged follicles.

Treatment of folliculitis depends on whether or not it’s infectious. Folliculitis from irritation may improve if you remove the source of irritation, such as shaving or wearing tight clothing. Applying warm compresses and hydrocortisone cream may help soothe the area and reduce inflammation.

What Is Keratosis Pilaris, And Why Does It Look Like Body Acne?

Has your skin been extremely itchy or irritated now that it’s much colder outside? When you remove those wool clothing layers, are there tiny bumps on your arms or thighs that just won’t go away? If you answered “yes” to these questions, you may keratosis pilaris.

Often mistaken for acne, keratosis pilaris is a chronic skin condition that forms white or red bumps when hair follicles are plugged with keratin buildup. It usually appears on the cheeks, upper arms, thighs, outer legs, buttocks and back. According to board-certified dermatologist Linda Honet, it’s an annoying ailment that’s relatively harmless but without continuous treatment, it just comes right back.

While a pimple is usually more painful and not itchy, Honet explains that with keratosis pilaris you have “multiple little bumps with dry plugs in the hair follicle that can be raw or red.”

“People who have family members with asthma, atopic dermatitis, eczema, hay fever or seasonal allergies have a higher instance of KP than people who do not,” says Honet. “KP often does get worse in the winter because the humidity falls and the ambiance moisture in the air sucks away moisture in the skin. Then, it gets itchy.”

The primary way that family practitioners or dermatologists like Honet treat keratosis pilaris is by moisturizing. “Often these are very heavy-duty moisturizers that have alpha hydroxy acids in them like glycolic acid or lactic acid because they are chemical exfoliators,” says Honet. “They actually de-plug the hair follicles, smooth out the skin and impart more moisture.”

As a fan of the Gwyneth Paltrow-backed skincare line Restorsea, Honet was able to self-test the effectiveness of the brand’s new Revitalizing Scalp Treatment in treating KP. The derm explained that an off-label remedy like this scalp treatment is often used, especially if it does no harm to other body parts.

Honet believes what really makes this product stand out is how gentle it is in comparison to the mechanical exfoliators that are often used to de-plug hair follicles, such as bath puffs or Clarisonic brushes. She also recommends body washes that are formulated with alpha hydroxy ingredients. “Glytone makes one that’s nice with glycolic acid, and Eucerin has a whole line that’s called Professional Repair that’s formulated for extremely dry skin,” says Honet.

Maintaining a consistent and healthy skincare regimen is key to minimizing symptoms of keratosis pilaris. You’ll want to use a gentle cleanser or exfoliator when bathing and not use hot water. Be sure not to scrub too hard because it can lead to more irritation. While people usually gravitate toward steamy showers and baths during this season, Honet points out that it actually strips your skin of any natural oils that you’re making. Instead, use lukewarm water and keep your bathing time brief. Then, get out of the shower while your skin is still damp and apply a treatment. “It’ll seal in the moisture and allow the medication to be more effective,” says Honet.

Needless to say, it makes all subsequent Spin classes seriously awkward, both in front of other women in the locker room (even though I know I shouldn’t care) and on the bike. (Let’s just say it’s not a time you want to do tap backs.)

Determined to put an end to this cycle, and help out anyone else who might need it, I grilled Bard for her best advice on how to get rid of butt acne. Here, the four tips she swears by, because I know we’ve all been there.

1. Give the tight clothes a rest.
In case you needed another excuse to embrace the sweats-and-stilettos trend, loose pants are your best bet for avoiding breakouts. “To prevent folliculitis, I encourage patients not to wear tight, friction-inducing clothing, such as tight jeans, and to change out of your sweaty gym clothes as soon as possible,” says Bard. Opt for cotton underwear over nylon or spandex to give the skin on your butt a chance to breathe.

2. Wash up after workouts.
Not only should you always shower after working out, but it’s important you pay attention to your skin back there while you do it. To cleanse, skip the basic body wash and reach for an antibacterial soap or benzoyl peroxide wash like Neutrogena’s Clear Stubborn Acne Cleanser. Some other tips: Make sure you wash your hair first and body last so the dirty suds from your scalp won’t drip down and clog your follicles. What is folliculitis?

Folliculitis means an inflamed hair follicle due to any cause. The result is a tender red spot, often with a surface pustule.

Folliculitis may be superficial or deep. It can affect anywhere there are hairs, including chest, back, buttocks, arms, and legs. Acne and its variants are also types of folliculitis.

What causes folliculitis?

Folliculitis can be due to infection, occlusion (blockage), irritation and various skin diseases.

Folliculitis due to infection

Swabs should be taken from the pustules for cytology and culture in the laboratory to determine if folliculitis is due to an infection.


Bacterial folliculitis is commonly due to Staphylococcus aureus. If the infection involves the deep part of the follicle, it results in a painful boil. Recommended treatment includes careful hygiene, antiseptic cleanser or cream, antibiotic ointment, or oral antibiotics.

Spa pool folliculitis is due to infection with Pseudomonas aeruginosa, which thrives in warm water. Gram-negative folliculitis is a pustular facial eruption also due to infection with Pseudomonas aeruginosa or other similar organisms. The most common yeast to cause a folliculitis is Pityrosporum ovale, also known as Malassezia. Malassezia folliculitis (pityrosporum folliculitis) is an itchy acne-like condition usually affecting the upper trunk of a young adult. Treatment includes avoiding moisturisers, stopping any antibiotics and using topical antifungal or oral antifungal medication for several weeks.

Candida albicans can also provoke a folliculitis in skin folds (intertrigo) or the beard area. It is treated with appropriate topical or oral antifungal agents.


Ringworm of the scalp (tinea capitis) usually results in scaling and hair loss, but sometimes results in folliculitis. In New Zealand, cat ringworm (Microsporum canis) is the commonest organism causing scalp fungal infection. Other fungi such as Trichophyton tonsurans are increasingly reported. Treatment is with an oral antifungal agent for several months.

Viral infections

Folliculitis may be caused by the herpes simplex virus. This tends to be tender and resolves without treatment in around ten days. Severe recurrent attacks may be treated with aciclovir and other antiviral agents.

Herpes zoster (the cause of shingles) may present as folliculitis with painful pustules and crusted spots within a dermatome (an area of skin supplied by a single nerve). It is treated with high-dose aciclovir.

Molluscum contagiosum, common in young children, can present with follicular umbilicated papules, usually clustered in and around a body fold. Molluscum may provoke dermatitis.

Parasitic infection

Folliculitis on the face or scalp of older or immunosuppressed adults may be due to colonisation by hair follicle mites (demodex). This is known as demodicosis.

The human infestation, scabies, often provokes folliculitis, as well as non-follicular papules, vesicles and pustules.

Folliculitis due to irritation from regrowing hairs

Folliculitis may arise as hairs regrow after shaving, waxing, electrolysis, or plucking. Swabs taken from the pustules are sterile — there is no growth of bacteria or other organisms. In the beard area irritant folliculitis is known as pseudofolliculitis barbae.

Irritant folliculitis is also common on the lower legs of women (shaving rash). It is frequently very itchy. Treatment is to stop hair removal, and not begin again for about three months after the folliculitis has settled. To prevent reoccurring irritant folliculitis, use a gentle hair removal method, such as a lady’s electric razor. Avoid soap and apply plenty of shaving gel, if using a blade shaver.

Folliculitis due to contact reactions


Paraffin-based ointments, moisturisers, and adhesive plasters may all result in a sterile folliculitis. If a moisturiser is needed, choose an oil-free product, as it is less likely to cause occlusion.


Coal tar, cutting oils and other chemicals may cause an irritant folliculitis. Avoid contact with the causative product.

Topical steroids

Overuse of topical steroids may produce a folliculitis. Perioral dermatitis is a facial folliculitis provoked by moisturisers and topical steroids. Perioral dermatitis is treated with tetracycline antibiotics for six weeks or so.

Folliculitis due to immunosuppression

Eosinophilic folliculitis is a specific type of folliculitis that may arise in some immune-suppressed individuals such as those infected by human immunodeficiency virus (HIV) or those who have cancer.

Folliculitis due to drugs

Folliculitis may be due to drugs, particularly corticosteroids (steroid acne), androgens (male hormones), adrenocorticotrophic hormone (ACTH), lithium, isoniazid (INH), phenytoin and B-complex vitamins. Treatment with protein kinase inhibitors (epidermal growth factor receptor inhibitors) and targeted therapy for metastatic melanoma (vemurafenib, dabrafenib) nearly always results in folliculitis.

Folliculitis due to inflammatory skin diseases
