Psoriasis Dos and Don’ts: Baths, Vaseline, and More

If you’re living with psoriasis, you probably already know how frustrating and challenging it can sometimes be to cope with the condition. Symptoms like itchy, scaling skin can affect your ability to perform daily activities, take a toll on your emotional health, and impact your quality of life.

The good news: There are ways to make life with psoriasis easier. Follow these dos and don’ts to help get your symptoms under control.

Psoriasis Dos

Do talk to a dermatologist. Make an appointment with a dermatologist who specializes in treating psoriasis — he or she will be aware of the latest developments regarding treatment plans. Be prepared to discuss the details of your condition with your doctor, including when you first noticed it, what your symptoms are, any situations that seem to make your symptoms worse, and what treatments have and have not worked for you in the past.

Do moisturize. Dry skin is more susceptible to outbreaks of psoriasis, so keep your skin well lubricated. After bathing or showering, seal in moisture by applying a generous amount of moisturizing cream or ointment to your skin. Vaseline, Cetaphil cream, and Eucerin cream are a few commonly available moisturizers reported to provide good results. Avoid lightweight lotions, which don’t contain enough emollients.

If over-the-counter products don’t help, your doctor may prescribe a moisturizing cream that contains medication.

Be especially diligent about moisturizing during the winter months, when cold outdoor weather and overheated buildings are a particularly drying combination. “In psoriasis, the epidermis builds up rapidly, producing a thick scale,” says James W. Swan, MD, professor of medicine in the division of dermatology at Loyola University Medical Center in La Grange Park, Illinois.

When the skin is hydrated, the scales soften and fall away, alleviating itch and dryness. “But not using anything on the skin for three days will allow the scale to get very thick,” says Dr. Swan.

Do take a soak. Soaking in a warm (not hot) bath for 15 minutes can help loosen scales and help reduce the itching and inflammation caused by psoriasis. Adding sea salt, oatmeal, bath oil, or a bath gel containing coal tar to the water can further soothe and moisturize your skin. If you live or vacation in an area with mineral or salt baths, take a dip in one. Both are associated with relieving psoriasis.

Do get some sun. For reasons experts still don’t fully understand, psoriasis lesions often diminish when exposed to ultraviolet light. So while sunbathing is discouraged for most people because of the risk of skin cancer, it can be helpful for those with psoriasis. The trick is to make sure that only the areas affected by psoriasis are exposed.

Cover unaffected skin with clothing or a sunscreen with an SPF (sun protection factor) of at least 30. Limit sun exposure to 15 minutes, and be careful to avoid sunburn, which will only make matters worse. It may take several weeks to see an improvement. Avoid tanning beds, which don’t produce the same healing effect and may actually be harmful.

Your doctor may also recommend ultraviolet light therapy, either in the doctor’s office or at home. According to Swan, “One of the gold standards for treatment of psoriasis is phototherapy,” which involves exposing the skin to ultraviolet light on a regular basis and under medical supervision. According to the National Psoriasis Foundation, UVB light in particular penetrates the skin and slows the growth of affected skin cells.

“Ultraviolet B (UVB) light reduces the inflammatory cells from the skin that is causing psoriasis,” says Swan. “It also slows the cell proliferation that results in the scaling.”

Do reach out. Having psoriasis isn’t just physically tough — it can be difficult emotionally as well. Feelings of depression, frustration, and isolation are common. Body image issues related to the appearance of psoriasis lesions are normal. While it may feel as if you’re the only person struggling with this condition, in fact the World Health Organization reports that at least 100 million people are affected worldwide.

Discuss your feelings about the disease with your family, friends, and doctor. In-person and online support groups for those with psoriasis can also provide support and help you remember that you’re not alone. Psoriasis organizations, such as the National Psoriasis Foundation, can connect you with others who are living with psoriasis, as well as keep you informed about research developments and opportunities to get involved in fundraising walks and other events.

Psoriasis Don’ts

Don’t overdo it. The best way to handle psoriasis is to do so gently. Avoid the temptation to scratch or scrub lesions, which will only irritate them, making them worse. Try not to pick at scales, which can cause bleeding and increase your risk of infection. Instead, talk with your doctor about creams and ointments that can gently remove the thick scale. Bathing in very hot water or using abrasive cleaners can also make your symptoms flare up.

Don’t stress out. Some people with psoriasis say their condition worsens when they’re under stress. Avoid stressful situations when you can, and take extra steps to take care of yourself — such as eating well, exercising, and getting enough sleep — when you can’t avoid stress. Hypnosis, relaxation, meditation, biofeedback, and other stress management techniques may also help.

Don’t ignore flare-ups. Psoriasis is a lifelong condition, and one that tends to wax and wane over time. But that doesn’t mean you just have to live with it. If your psoriasis returns after a period of being under control, schedule a visit with your doctor to find out why, and to decide what can be done to treat it. 

Don’t give up. One of the most frustrating things about treating psoriasis is that something that works well for one person may not work at all for another. It may take some time to find the right therapy or combination of therapies that works best for you. Be patient and don’t give up. It’s important to be consistent with your treatment plan, day in and day out, even when your symptoms aren’t so bad. With psoriasis, slow and steady wins the race.

How to Shower or Bathe if You Have Psoriasis

Stress, a cold or other infection, or dry weather can trigger a psoriasis flare. As your immune system overreacts, you’ll notice those telltale scaly psoriasis lesions — likely on your elbows, knees, or scalp — that can cause your skin to feel especially sensitive, irritated, and itchy. Soaking in a warm bath or shower can help soothe your skin by hydrating and softening these lesions. But frequent, hot baths or showers can dry skin and worsen psoriasis.

It’s important to note that your bathing and over-the-counter skin-care regimen won’t actually heal individual lesions, because psoriasis is an inflammatory disease that originates from the immune system. “It’s not something that we think about as a direct result of dry skin and bathing,” says Rashmi Unwala, MD, a dermatologist specializing in psoriasis at Cleveland Clinic in Ohio. “Focusing on skin care can be detrimental to patients. Just switching their products won’t make a difference for the internal disease and delays care.” It’s essential to see a doctor to diagnose and treat the underlying condition with a variety of safe and effective topical and systemic medicines.

With that said, your bathing routine during a psoriasis flare can impact how your skin feels. Here’s how to get the most out of your shower or bath.

1. Limit showers to 5 minutes and baths to 15 minutes once per day

Showering or bathing too often can increase the amount of moisture you lose through your skin, making it dry and irritated. “It can make already inflamed skin feel even worse,” says Dr. Unwala. She suggests bathing once a day and limiting baths to no more than 15 minutes and showers to 5 minutes.

2. Make sure the water is lukewarm, not hot

When your skin is irritated, keep the water temperature of your baths and showers warm, not hot. “Hot water will make skin drier and more irritated,” says Unwala.

3. Add coal tar, ground oatmeal, or bath oils to bathwater

Baths can help gently remove some psoriasis scales without damaging underlying skin or worsening individual lesions. “Especially if a person is extremely irritated and struggling with itch, baths can soothe and relieve discomfort,” says Unwala.

She suggests adding a coal tar solution to your bath, which can help ease itching and scaling and reduce some of the inflammation of the psoriasis, according to the National Psoriasis Foundation. You can also add bath oils to moisturize the skin as well as oatmeal, which helps moisturize by sealing the cracks in dry skin. If you have questions about which bath products to use, talk to your doctor or dermatologist.

4. Use gentle, moisture-rich body wash made for sensitive skin

Instead of bar soap, use a gentle, fragrance-free cleanser or bodywash to avoid irritating your skin. “There’s a theoretical risk that because you have broken [and inflamed] skin, you could develop an allergy to some products,” says Unwala.

Many cleansers, shampoos, and lotions labeled for psoriasis contain coal tar. “It’s one of the most effective over-the-counter ingredients for psoriasis,” says Unwala. She adds that cleansers with salicylic acid can help gently work away psoriasis lesions on the skin without trauma. Lactic acid, urea, and phenols are also used as scale lifters, or keratinolytics. Talk to your doctor or dermatologist about which ingredients to look for in your skin-care products.

5. Skip the washcloths and scrubbing beads

It might be tempting to use a loofah, a washcloth, or scrubbing beads to slough off psoriasis lesions. But excessive exfoliation with abrasive materials can inflame skin, potentially causing the Koebner phenomenon, in which minor skin injuries or infections lead to psoriasis flares.

“Trauma to the skin will lead to more psoriasis. Vigorous rubbing and scrubbing can lead to enlargements of areas of psoriasis,” explains Unwala. Instead, use your hands to gently apply your cleanser and rinse thoroughly.

6. If you have scalp psoriasis, gently massage shampoo and conditioner into your hair

The same principles apply if you have scalp psoriasis. Rather than rubbing shampoo into your hair, gently massage and rinse. Ask your doctor whether a medicated shampoo that contains coal tar to help ease inflammation could work for you.

7. Dry your skin by blotting your body with a towel

When you’re done bathing, avoid rubbing your towel on your skin or hair. Instead, gently pat yourself dry, leaving your skin slightly damp. Use towels washed in a fragrance-free detergent to avoid potentially irritating your skin.

8. Apply a fragrance-free moisturizer after getting out of the bath or shower

As soon as you’re done bathing, apply prescription topicals to any affected areas and then apply moisturizer to damp skin. Some experts recommend choosing a thick cream or ointment over a lotion to lock in moisture. If your skin is extra dry, you may want to consider using an oil.

Some ointments and moisturizers created for psoriasis also contain coal tar or salicylic acid, which can help calm inflammation and gently remove plaques. “Choose something that doesn’t have an exfoliant and isn’t irritating to the skin,” says Unwala. “Anything that burns or stings is going to make things worse.”

And remember, bathing can only do so much to help your skin during a psoriasis flare. It’s important to take your medications as prescribed and reach out to your doctor if your psoriasis isn’t improving or is getting worse. Adjusting your treatment to address the underlying cause is the only way to effectively treat psoriasis.

What the researchers found was that in routine clinical practice, sBPT was actually more effective than phototherapy alone with about 35 treatments, and results were sustained at a follow up of six months. Additionally, both therapies were considered safe options for treating psoriasis.

A systemic review from 2012 found the water from the Dead Sea is helpful for psoriasis and other rheumatic conditions, including psoriatic arthritis, a type of inflammatory arthritis that affects some people living with psoriasis. Additionally, Dead Sea saltwater is considered is a safe complementary treatment.

A 2017 report in the Journal of Education and Health reports on the use of Dead Sea salts for psoriasis and other skin conditions. Two common active ingredients, salicylic acid and tar, are approved by the U.S. Food and Drug Administration (FDA) as treatments for psoriasis.

Salicylic Acid

Salicylic acid is classified as a keratolytic, or peeling agent. It works by causing the outer layer of skin to shed. It is a common and effective treatment for a wide variety of skin problems. As a psoriasis treatment, it acts as a scale lifter, helping to soften and remove psoriasis scales.

Strong salicylic acid preparations can cause irritation if left on the skin for too long. Further, the body may absorb too much salicylic acid if used over large areas of the skin. Salicylic acid may also weaken hair shafts and make them more likely to break, leading to temporary hair loss. Please consult your health care professional if you experience any complications.

Coal Tar

Tar derived from coal and wood (e.g., juniper and pine) are used for medicinal purposes. However, coal tar is most commonly used to treat psoriasis. Tar can help slow the rapid growth of skin cells and restore the skin’s smooth appearance. In addition, it can help reduce the inflammation, itching and scaling of psoriasis. Tar products can vary dramatically from brand to brand. Generally, the higher the concentration of tar, the more potent the product.

Tar can irritate, redden and dry the skin. You should test a tar product on a small area of the skin first. If reddening occurs, try applying the tar on top of a moisturizer. Tar can stain clothing, bed linen and light-colored hair. Tar can also make skin more sensitive to sunlight, so be sure to wash it off thoroughly, use sunscreen and monitor your sun exposure. Tar remains active on the skin for at least 24 hours, so limit your time outdoors as you will be at an increased risk of sunburn during this period.

Studies show some of the chemicals in coal tar may cause cancer, but only in very high concentrations, such as in what is used in industrial paving. If you are using tar regularly, make sure to follow a regular skin cancer checkup schedule.

California requires OTC coal tar shampoos, lotions and creams that contain more than 0.5 percent coal tar to be labeled with cancer warnings. However, the FDA maintains that OTC products with coal tar concentrations between 0.5 percent and 5 percent are safe and effective for psoriasis, and there is no scientific evidence that the tar in OTC products is carcinogenic.

If there ever was a medical condition that could convince Sherlock Holmes to get out of the business, it’s plaque psoriasis. Psoriasis symptoms are obvious—after all, it’s hard not to notice that maddening itch, the inflammation, and those bothersome silvery scales that usually occur on the elbows, knees, trunk, and scalp. But when it comes to finding its cause or cure, that’s even more of a mystery than Watson’s first name.

What is known about psoriasis is that it causes skin cells to go hyper. A normal skin cell takes about a month to mature, but in those with psoriasis, this process takes only 3 or 4 days. These skin cells are poorly developed, and they can’t shed fast enough. Instead, they pile up—forming raised, scaly “plaques” that itch and leave skin below red and inflamed. But instead of the proverbial heartbreak, there is reason to take heart. While there’s no cure as yet, you can control psoriasis and lessen its impact on your life.
