A boil is a skin infection that starts in a hair follicle or oil gland. At first, the skin turns red in the area of the infection, and a tender lump develops. After four to seven days, the lump starts turning white as pus collects under the skin.

  • The most common places for boils to appear are on the face, neck, armpits, shoulders, and buttocks. When one forms on the eyelid, it is called a sty.
  • If several boils appear in a group, this is a more serious type of infection called a carbuncle.

Causes of Boils

Most boils are caused by a germ (staphylococcal bacteria). This germ enters the body through tiny nicks or cuts in the skin or can travel down the hair to the follicle.

These health problems make people more susceptible to skin infections:

  • Diabetes
  • Problems with the immune system
  • Poor nutrition
  • Poor hygiene
  • Exposure to harsh chemicals that irritate the skin

Symptoms of Boils

A boil starts as a hard, red, painful lump usually about half an inch in size. Over the next few days, the lump becomes softer, larger, and more painful. Soon a pocket of pus forms on the top of the boil. These are the signs of a severe infection:

  • The skin around the boil becomes infected. It turns red, painful, warm, and swollen.
  • More boils may appear around the original one.
  • A fever may develop.
  • Lymph nodes may become swollen.

When to Seek Medical Care

  • You start running a fever.
  • You have swollen lymph nodes.
  • The skin around the boil turns red or red streaks appear.
  • The pain becomes severe.
  • The boil does not drain.
  • A second boil appears.
  • You have a heart murmur, diabetes, any problem with your immune system, or use immune suppressing drugs (for example, corticosteroids or chemotherapy) and you develop a boil.
  • Boils usually do not need immediate emergency attention. However, if you are in poor health and you develop high fever and chills along with the infection, a trip to a hospital’s emergency room is needed.

Your doctor can make the diagnosis with a physical exam. Many parts of the body may be affected by this skin infection, so some of the questions or exam may be about other parts of your body.

Boils Treatment — Home Remedies

  • Apply warm compresses and soak the boil in warm water. This will decrease the pain and help draw the pus to the surface. Once the boil comes to a head, it will burst with repeated soakings. This usually occurs within 10 days of its appearance. You can make a warm compress by soaking a wash cloth in warm water and squeezing out the excess moisture.
  • When the boil starts draining, wash it with an antibacterial soap until all the pus is gone and clean with rubbing alcohol. Apply a medicated ointment (topical antibiotic) and a bandage. Continue to wash the infected area two to three times a day and to use warm compresses until the wound heals.
  • Do not pop the boil with a needle. This could make the infection worse.

Medical Treatment for Boils

If there are concerns about the seriousness of the infection, additional blood tests will be performed. The doctor may prescribe antibiotics if the infection is severe. If the boil is drained, a culture may be done to determine the type of bacteria causing the infection and to assess if an appropriate antibiotic was given.

Next Steps — Follow-up

Whether the boil is drained at home or is lanced by a doctor, you will need to clean the infected area two to three times a day until the wound is healed. Apply an antibiotic ointment after washing and cover with a bandage. If the area turns red or looks as if it is getting infected again, contact your doctor.

Preventing Boils

Help prevent boils by following these guidelines:

  • Carefully wash clothes, bedding, and towels of a family member who is infected with boils.
  • Clean and treat minor skin wounds.
  • Practice good personal hygiene.
  • Stay as healthy as possible.

Outlook

Most boils will disappear with simple home treatment.

Hidradenitis suppurativa: MedlinePlus Genetics

In most cases, the cause of hidradenitis suppurativa is unknown. The condition probably results from a combination of genetic and environmental factors. Originally, researchers believed that the disorder was caused by the blockage of specialized sweat glands called apocrine glands. However, recent studies have shown that the condition actually begins with a blockage of hair follicles in areas of the body that also contain a high concentration of apocrine glands (such as the armpits and groin). The blocked hair follicles trap bacteria, leading to inflammation and rupture. It remains unclear what initially causes the follicles to become blocked and why the nodules tend to recur.

Genetic factors clearly play a role in causing hidradenitis suppurativa. Some cases have been found to result from mutations in the NCSTN, PSEN1, or PSENEN gene. The proteins produced from these genes are all components of a complex called gamma- (γ-) secretase. This complex cuts apart (cleaves) many different proteins, which is an important step in several chemical signaling pathways. One of these pathways, known as Notch signaling, is essential for the normal maturation and division of hair follicle cells and other types of skin cells. Notch signaling is also involved in normal immune system function. Studies suggest that mutations in the NCSTN, PSEN1, or PSENEN gene impair Notch signaling in hair follicles. Although little is known about the mechanism, abnormal Notch signaling appears to promote the development of nodules and lead to inflammation in the skin. Researchers are working to determine whether additional genes, particularly those that provide instructions for making other γ-secretase components, are also associated with hidradenitis suppurativa.

Researchers have studied many other possible risk factors for hidradenitis suppurativa. Obesity and smoking both appear to increase the risk of the disorder, and obesity is also associated with increased severity of signs and symptoms in affected individuals. Studies suggest that neither abnormal immune system function nor hormonal factors play a significant role in causing the disease. Other factors that were mistakenly thought to be associated with this condition include poor hygiene, the use of underarm deodorants and antiperspirants, and shaving or the use of depilatory products to remove hair.

Overview

Boils (furuncles) are painful pus-filled bumps on the skin resulting from the deep infection of a hair follicle. The infection is usually caused by a type of bacteria called Staphylococcus aureus (“staph”). Many people are “carriers” of the staph germ, meaning that it normally lives on their skin or in their nose without doing them any harm. Tiny breaks in the surface of the skin (such as those caused by friction or scratching), however, can help the germ gain entry into and infect the hair follicle, resulting in a boil.

Boils may resolve with simple self-care measures, but the infected fluid (pus) needs to drain in order for them to heal completely. Many boils drain of their own accord, or they can be lanced by a health care professional. Antibiotics may also be prescribed. Untreated boils can enlarge or grow together to form a giant multi-headed boil (carbuncle). Rarely, the infection in the skin can get into the bloodstream, leading to serious illness.

Who’s at risk?

Boils are most common in teenagers and young adults.

People that are particularly prone to developing boils include:

  • Athletes participating in contact sports or using shared equipment.
  • Individuals with a weakened immune system, such as persons with HIV, diabetics, and those taking certain medications such as the types of medications used to prevent rejection of a transplanted organ or to treat cancer (chemotherapy).
  • Individuals with another skin condition that may lead to scratching or other injury to the skin (eczema, scabies).
  • Staph carriers.
  • People who are obese.
  • Individuals with poor nutrition.
  • People living in close quarters with others (military barracks, prison, homeless shelters).

Signs and Symptoms

A red-to-purple, tender lump on an area of the skin that also has hair. The most common areas for boils to occur are places where there is friction and/or places that tend to be sweaty, such as the buttocks, armpits, groin, neck, shoulders, and face. The skin surrounding the lump may look swollen and red. The center of the lump eventually becomes filled with yellow or white pus that you will be able to see (called “coming to a head”). The pus is a mixture of bacteria and infection-fighting white blood cells.

Self-Care Guidelines

Warm compresses applied to the area for 20 minutes at least 3–4 times a day may ease the discomfort and help encourage the boil to drain. If the boil starts to drain, wash the area with antibacterial soap and apply some triple antibiotic ointment and a loose bandage. Repeat this process of cleansing and bandaging the area 2–3 times a day until the skin is healed.

Boils can be very contagious. Do not share clothing, towels, bedding, or sporting equipment with others while you have a boil. Wash your hands frequently with antibacterial hand soap to avoid spreading the infection to others.

Use an antibacterial soap on boil-prone areas when showering, and dry your skin thoroughly after bathing. Avoid tight-fitting clothing and activities that cause a great deal of sweating.

Do not pop the boil yourself with a pin or needle. Doing so may make the infection worse.

When to Seek Medical Care

See your doctor if:

  • You have multiple boils or if the boil(s) increases in size or number.
  • You have a fever or chills, severe pain, or otherwise feel unwell.
  • The boil fails to drain.
  • The area of redness surrounding the boil begins spreading.
  • You have diabetes, a heart murmur, a problem with your immune system, or are taking immune-suppressing medications when you develop a boil.
  • You have had repeated outbreaks of boils.

If you are currently being treated for a skin infection that has not improved after 2–3 days of antibiotics, return to your doctor.

Community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) is a strain of staph bacteria resistant to antibiotics in the penicillin family, which have been the cornerstone of antibiotic therapy for staph and skin infections for decades. CA-MRSA previously infected only small segments of the population, such as health care workers and persons using injection drugs. However, CA-MRSA is now a common cause of skin infections in the general population. While CA-MRSA bacteria are resistant to penicillin and penicillin-related antibiotics, most staph infections with CA-MRSA can be easily treated by health care practitioners using local skin care and commonly available non-penicillin-family antibiotics. Rarely, CA-MRSA can cause serious skin and soft tissue (deeper) infections. Staph infections typically start as small red bumps or pus-filled bumps, which can rapidly turn into deep, painful sores. If you see a red bump or pus-filled bump on the skin that is worsening or showing any signs of infection (ie, the area becomes increasingly painful, red, or swollen), see your doctor right away. Many people believe incorrectly that these bumps are the result of a spider bite. Your doctor may need to test (culture) infected skin for MRSA before starting antibiotics. If you have a skin problem that resembles a CA-MRSA infection or a culture that is positive for MRSA, your doctor may need to provide local skin care and prescribe oral antibiotics. To prevent spread of infection to others, infected wounds, hands, and other exposed body areas should be kept clean, and wounds should be covered during therapy.

Treatments Your Physician May Prescribe

The pus inside of a boil needs to be drained thoroughly before the body can completely clear the infection. If the boil does not drain itself, your doctor may wish to perform a simple procedure. In this procedure, a sterile needle or small blade is used to “nick” the skin over the top of the boil, and the pus is allowed to drain out. The area will then be cleaned and bandaged, and you will be sent home with instructions to wash, apply antibacterial ointment, and re-bandage the area several times daily as discussed above. You may also be prescribed a course of antibiotics to be taken by mouth.

Your physician may choose to collect a swab of the pus for laboratory analysis and swabs from other areas of the body (nose, armpits, and/or anus and genital area) to determine if you are a carrier of staph. If you are a carrier, your doctor may prescribe a topical medication applied to the inside of the nose and/or oral antibiotics for several days. These measures can help prevent a recurrence of the boil(s) and decrease the possibility that you may unknowingly spread the germ to others. If your doctor prescribes antibiotics, be sure to take the full course of treatment to avoid the development of bacterial resistance to the antibiotic.

What are Boils and Carbuncles?

Boils and carbuncles are skin infections usually caused by Staphylococcus aureus bacteria (staph). These staph infections form pockets in the skin that are filled with pus, a fluid that includes bacteria, dead skin cells and infection-fighting white blood cells. Whether the pocket of pus is called a boil or a carbuncle depends on its location and size:

  • A boil, also called a furuncle, begins as a painful infection of a single hair follicle. Boils can grow to be larger than a golf ball, and they commonly occur on the buttocks, face, neck, armpits and groin.

  • A carbuncle is a deeper skin infection that involves a group of infected hair follicles in one skin location. Carbuncles often are found on the back of the neck, shoulders, hips and thighs, and they are especially common in middle-aged or elderly men. People with diabetes are more likely to develop carbuncles.

Symptoms

A boil looks like a red, swollen, painful bump under the skin. As the infection gets worse, a whitish tip, also called a point or head, can appear at the center of the boil. This tip is usually the area from which the boil’s pus will drain. A carbuncle looks like a cluster of interconnected boils.

Whenever you have a boil or a carbuncle, you also can have a fever and feel generally sick. A fever is more likely with a carbuncle than with a single boil.

Diagnosis

Your doctor can diagnose a boil or carbuncle by examining your skin. If you get several boils within a short period of time, your doctor may do blood tests to check for diabetes or other medical conditions that can increase your risk of repeated infections.

Expected Duration

In many otherwise healthy people, a small boil will form a white tip (come to a head) and drain within five to seven days. However, very large boils or carbuncles can last longer and may not drain on their own. These may need to be drained by a physician, and you may need to take antibiotics.

Prevention

If you have an area of skin that is prone to boils or carbuncles, keep the area clean and dry, and avoid wearing tight clothing that doesn’t allow the skin to breathe. Washing daily with antibacterial soap also can help. At the earliest sign of irritation or a bump at a hair follicle, use warm compresses to open up the blocked pore and drain any early infection. If you develop signs of inflammation or infection at a hair follicle (folliculitis) as a result of shaving, you should avoid shaving in that area to prevent bacteria from being spread to other parts of the skin.

Treatment

Small boils can be treated with moist heat (usually a warm, wet washcloth) applied for 20 to 30 minutes, 3 or 4 times a day. This will help the boil drain on its own. Once the boil drains, cover it with a clean bandage to protect the skin and absorb draining pus. Wash the affected area daily with antibacterial soap to prevent the infection from spreading. Anyone who helps care for the infected area should also wash his or her hands thoroughly with antibacterial soap.

Large boils and carbuncles may be treated with antibiotics. In many cases, the doctor will drain the infected area through a small incision. This will relieve pain, speed recovery and limit scar formation. If the infection is completely drained, antibiotics may not be necessary. If the infection is deep, your doctor may fill the empty pocket that contained the pus with a strip or piece of sterile gauze. The gauze can keep the incision open, which will allow pus to continue to drain. The pocket can then heal slowly, becoming more shallow over time until it is a superficial wound. You may need to return to the doctor a few times to have the gauze and dressing changed.

When To Call A Professional

Call your doctor whenever you have a carbuncle, a large boil or a boil that doesn’t improve after a week of warm compress treatment as described above. If you have diabetes, you should call your doctor even if you develop a small boil because you are more prone to developing serious infections. Ask to be seen immediately if a boil of any size:

  • Develops in an infant

  • Is located on the face, rectum, groin or spine

  • Produces fever or severe pain

  • Interferes with movement

  • Causes swelling, red streaks or other discoloration in nearby skin

If you have been treated with antibiotics for a boil or carbuncle and your infection does not appear to be responding within one to two days, contact your doctor again. Your infection may be caused by a type of bacteria that is resistant to several commonly used antibiotics. This bacteria is called community-acquired methicillin-resistant S. aureus (community-acquired MRSA). To treat this type of infection, your doctor needs to know that your first antibiotic was not helpful. You may need to take a different kind of antibiotic.

If you have had several episodes of boils within a short period of time, visit your doctor. Your doctor can check whether an undiagnosed medical illness is affecting your body’s ability to fight infections.

Prognosis

Most small boils heal without leaving a scar. In general, the larger the boil or carbuncle, the greater the chance that it will leave a scar. Because of this, you should see a doctor if you have a boil on your face. Antibiotic treatment and surgical drainage can help limit scar formation.

What are boils?

A boil is a red, swollen, painful bump under the skin. It often looks like an overgrown pimple. Boils are often caused by infected hair follicles. Bacteria from the infection form an abscess, or pocket of pus. A boil can become large and cause severe pain.

Boils most often happen where there is hair and rubbing. The face, neck, armpits, breasts, groin, and buttocks are common sites.

How do you treat a boil?

You can sometimes care for a boil at home.

  • Do not squeeze, scratch, drain, or open the boil. Squeezing can push the infection deeper into the skin.
  • Gently wash the area with soap and water twice a day. Dry it well.
  • Put warm, wet cloths on the boil for 20 to 30 minutes, 3 or 4 times a day. Do this as soon as you notice a boil. The heat and moisture can help the boil to open and drain, but it may take 5 to 7 days. A warm compress or waterproof heating pad placed over a damp towel may also help.
  • Keep using heat for 3 days after the boil opens. Put a bandage on it so the drainage does not spread. Change the bandage every day.
  • If the boil is draining on its own, let it drain. Keep cleaning it twice a day with soap and water.
  • To help keep the infection from spreading, do not share towels and washcloths with other people.

Your doctor may want to cut a small opening in the boil so that the pus can drain out. This is called lancing the boil. He or she will numb the area first. Sometimes gauze is placed in the cut so that it stays open and keeps draining.

Your doctor may also prescribe antibiotics to stop the infection. Take your antibiotics as directed. Do not stop taking them just because you feel better or the boil looks better. You need to take the full course of antibiotics.

When should you call a doctor?

Call your doctor if:

  • The boil is on your face, near your spine, or near your anus.
  • A boil is getting larger.
  • You have any other lumps near the boil, especially if they hurt.
  • You are in a lot of pain.
  • You have a fever.
  • The area around the boil is red or has red streaks leading from it.
  • You have diabetes and you get a boil.
  • The boil is as large as a ping-pong ball.
  • The boil has not improved after 5 to 7 days of home treatment.
  • You get many boils over several months.

How can you prevent boils?

If you often get boils in the same spot, gently wash the area well with soapy water every day. Always dry the area well. Do not wear tight clothing over the area.

If you have many boils, your doctor may prescribe a cream or ointment that you put inside your nose. This is because the bacteria that usually cause boils sometimes live inside the nose and then spread to other areas, including your skin. Your doctor may also advise you to take antibiotics for a longer time than normal. These medicines may help keep boils from coming back.

What are boils?

Boils are painful, red, pus-filled lumps in your skin caused by an infection of hair follicles. One infected hair follicle is a furuncle, and a group of infected follicles joined together is a carbuncle. They can occur anywhere, but most often in hairy areas that sweat or rub, such as your face, neck, armpits, groin and buttocks.

You can look after small boils on your own, but you may need to see your doctor for treatment of large boils. If a boil becomes worse or spreads or you develop a fever, you should see your doctor.

What are the symptoms of boils?

Symptoms include:

  • a red lump with a white or yellow centre
  • pain
  • feeling unwell and having fevers, particularly with a large boil

What are the causes of boils?

Boils are usually caused by infection with the bacteria Staphylococcus aureus. This bacteria usually lives on your skin without causing harm, but can infect hair follicles if the skin is broken. The infection can spread to other parts of your body, or to other people.

Anyone can develop a boil. You are more at risk if:

How are boils treated?

Boils should be kept clean and covered.

Pus needs to drain before a boil will heal. This may happen by itself, but sometimes treatment is needed.

For small boils, you can put a warm compress on the boil several times a day. This may help the pus to drain.

For larger boils and carbuncles (larger than 5cm), see your doctor. They may need to make a cut in the boil to help the pus drain, and you may need antibiotics. Large boils and carbuncles can scar.

See your doctor if the boil is on your face, if it is getting worse quickly, if it is very painful or if it hasn’t healed in 2 weeks.

What not to do

It is important not to squeeze or pop boils, as this can be very painful and can spread the infection.

Looking after yourself

If you have boils, you can help them heal more quickly and prevent them from spreading if you:

  • wash your hands regularly, especially after touching your boil
  • eat well and bath or shower regularly
  • keep any skin cuts clean and protected
  • don’t share personal items such as razors or towels
  • wash clothes, linen and towels in hot water

Symptoms of a staphylococcus infection can range from irritated skin, to painful lumps and swelling, to fever, racing heart, and confusion if the bacteria enter your bloodstream.

Some people carry staph bacteria on their skin or in their nose but never experience any symptoms from it.

But when the bacteria enter the body through a break in the skin, an infection — in which the bacteria multiply, potentially causing illness and tissue damage — can result.

Recognizing the early signs and symptoms of a staph infection is important, so you can seek help and get appropriate treatment before the infection spreads or worsens. (1)

Skin Staph Infection Symptoms

Staph infections that affect the skin usually cause less serious symptoms than those that invade other body parts. But skin infections can turn dangerous if they progress or aren’t treated. The following types of skin infections may be caused by staph:

Folliculitis

Folliculitis, which happens when hair follicles become infected, looks like small red bumps or pimples that may be pus-filled. They can cause itchiness, burning, and pain. Sometimes the blisters break open and crust over. (2)

Boils

Boils are a deeper, more severe version of folliculitis. They typically start as red, swollen bumps about the size of a pea that are painful to the touch. A boil may enlarge and gradually turn white as pus collects under the skin. Boils may rupture and drain spontaneously or may need to be lanced and drained by a doctor. (1) A boil is also a form of a skin abscess.

Skin Abscess

A skin abscess is a pus-filled pocket of infection that develops under your skin, similar to a boil but deeper in the body. The surrounding area may be painful, red, warm, and swollen. You might also have a fever and chills. (3)

Cellulitis

Symptoms of cellulitis include redness, swelling, warmth, and pain where the infection is located. Cellulitis most often occurs on a person’s leg. As it spreads, you may feel feverish and sick. Additionally, you might have small, red spots around the reddened skin and blisters that burst open.

Impetigo

Impetigo causes red sores that break open and ooze. After a few days, the blisters form a yellowish-brown crust. This infection is sometimes painful, but not always. Most cases of impetigo develop on the face, hands, or feet.

Staphylococcal Scalded Skin Syndrome (SSSS)

SSSS most commonly affects babies and young children and is most commonly caused by toxins released by staph bacteria. Symptoms include a rash, fever, and blisters that break easily, leaving moist, painful areas of skin. Large sections of the top layer of skin may also peel off, giving the appearance of skin that’s been scalded, or burned. Kids with this infection might also be irritable and tired. SSSS is a medical emergency and needs prompt treatment. (1,4)

Wound Infections

Wound infections usually crop up two or more days after you’ve had surgery or sustained an injury. They cause redness, pain, swelling, warmth, and pus drainage at the site of infection. You might also have a fever. (5)

Symptoms of Invasive Staph Infections

Invasive staph infections can lead to severe problems that might require immediate medical attention. Of course, symptoms vary depending on the type of infection, your overall health, and the location of the infection.

Sepsis

Sepsis is a life-threatening complication of an infection. Symptoms of sepsis usually include fever, chills, fast breathing, a high heart rate, and disorientation. (6)

Your doctor will diagnose you with sepsis if you have an infection and at least two of the following symptoms:
