Overview

Warts are growths of the skin and mucous membranes (the mouth or genitals) that are caused by over 100 types of the human papillomavirus (HPV). The virus causes thickening of the top skin layer. A plantar wart occurs on the sole of the foot. It can look and feel like a callus. Although most types of warts are usually harmless and painless, plantar warts may be tender to walk on. Plantar warts can be small, or they can grow to cover most of the sole of the foot. They are usually painless and go away on their own, sometimes taking a few months to resolve (but can take up to 2 years).

Warts are usually acquired from person-to-person contact. The virus is not highly contagious but can cause an infection by entering through a small break in the skin. In the same way, warts can be spread to other places on your own body. The virus is rarely transferred by touching an object used by an infected person. It is a good idea to avoid sharing towels or other intimate objects and to wear shoes in public places.

Who’s at risk?

Warts can affect people of any age, but they are most common in those aged 12–16 years. It is estimated that 20% of schoolchildren and about 10% of the general population have warts. Those with HIV or organ transplants or on chemotherapy have a higher incidence of warts due to their weakened immune system.

Signs and Symptoms

The most common locations for plantar warts include:

  • Soles of the feet, especially the weight-bearing areas (the heels and balls of the feet)
  • Undersides of the toes

Plantar warts may occur singly or in clusters. They appear as thick, rough, callus-like thickenings on the soles of the feet. In addition, plantar warts often have multiple small black “dots” at the surface, which are actually tiny blood vessels.

Plantar warts are usually tender.

Infection with plantar warts can be described as:

  • Small plantar warts: one or few small, painless lesions
  • Moderate plantar warts: multiple lesions, which may be uncomfortable
  • Giant plantar warts: the warts cover a large part of the sole and may cause significant discomfort and pain

Self-Care Guidelines

Because warts can resolve on their own, it is not necessary to treat all warts. Additionally, treating warts may not always destroy them, nor will it necessarily keep other warts from appearing. Treatment can be painful and cause scars and might need to be repeated, so it should only be done in cases where the warts are highly bothersome or interfere with daily life.

  • Duct tape applied daily to the affected area seems to work for unknown reasons. The tape should be very sticky and kept on for a few days. Between changes of duct tape, the wart should be soaked in warm water, and any loose skin should be removed every few days with a mild abrasive, like a pumice stone or emery board.
  • Over-the-counter wart removers have a high percentage of salicylic acid and work by dissolving away the layer of skin infected with the virus. This treatment needs to be used daily and can sometimes be irritating if it touches unaffected skin around the wart; 40% salicylic acid self-stick pads appear to be one of the most effective types. Cut out a pad that covers the wart and then apply duct tape over the pad to secure it. Keep on for 2–3 days. Remove the pad and tape and scrub away the top layer of dead skin, now white-colored, with an emery board or pumice stone. Reapply a new 40% salicylic acid pad and duct tape. Keep removing dead skin and reapplying every few days until the wart is gone. Many plantar warts will respond in 1–2 months.
  • Over-the-counter freezing medications are available but have not been found to be very effective.
  • Family members should avoid sharing personal items such as towels.

When to Seek Medical Care

Make an appointment with a dermatologist or another physician if you have:

  • Painful or bleeding warts.
  • Rapidly spreading or multiplying warts.
  • Warts that interfere with daily life and are not responsive to self-care.

People with diabetes or a circulatory disorder should have their warts treated by a physician.

Treatments Your Physician May Prescribe

Once you have been diagnosed with a plantar wart, your physician may try one or more of the following treatments:

  • Freezing with liquid nitrogen (cryosurgery)
  • Burning with an electric needle (electrocautery)
  • Using a laser to disrupt the blood supply of the warts
  • Application of cantharidin, podophyllin, tretinoin, or salicylic acid
  • Injection with Candida antigen, an allergy-causing protein, or with bleomycin, a chemotherapy drug, directly into the warts

Plantar warts can be very stubborn, and effective treatment may take many months.

What causes plantar warts? 

A virus called the human papillomavirus (HPV) can cause warts to develop on your skin. If you develop a wart on the sole of your foot, you have a plantar wart. Plantar warts come in different forms, appearing as a single bump or in clusters. 

Some of the signs you have a plantar wart include:

  • A fleshy, rough, or grainy growth on the sole of your foot
  • A bump or dark spot that appears under hard skin  
  • A growth that makes the skin on your foot change
  • Tenderness, pain, or irritation that arises when you stand or walk

Sometimes plantar warts can grow inward into the deeper layers of your skin rather than out from the surface. This happens due to the pressure your weight places on the bottoms of your feet. 

Who gets plantar warts? 

The virus that causes warts, HPV, is highly contagious. Anyone who comes in contact with HPV may develop plantar warts; however, some people are more susceptible than others. As with all viruses, some people react more strongly to HPV and develop warts more easily, including:

  • Teenagers and children
  • People who have weakened immune systems
  • People with a history of plantar warts

In addition, if you frequent warm, damp places, like locker rooms and indoor swimming pools, you have a higher chance of contracting the virus and getting a plantar wart — especially if you don’t wear foot coverings. People with small cuts and tears on their feet also have an increased risk of getting plantar warts because the openings allow the virus easy entry into your body.  

How can I get rid of plantar warts?

The best way to get rid of frustrating and contagious plantar warts is by scheduling an appointment with a foot specialist. It’s possible to wait and let the wart resolve on its own; however, it can take years for the virus that causes plantar warts to run its course. You can also try over-the-counter (OTC) medications and remedies, which take time and deduction to work effectively.

Because warts are so contagious, the best way to eradicate these unwanted visitors is by visiting a podiatrist. What Causes Warts?

Got foot warts? Nearly everyone will have a wart at some point in their lives. Warts are typically small, hard skin growths caused by an infection with humanpallilloma virus. Foot warts are treatable. Foot warts are among the most common dermatologic conditions podiatrists treat. Read on to learn about the causes of warts.

An HPV Infection

Common warts are caused by by an HPV infection. Over 100 types of HPV exist. Certain types of HPV infection cause cervical cancer. Some types of HPV infection cause foot warts, while others cause warts that appear on the face, neck, or hands. Of the 100 types of HPV, about 60 cause common warts on areas such as the hands or feet.

Skin-To-Skin Contact

Wart viruses are contagious. You can get foot warts from skin-to-skin contact with people who have warts. However, not all HPV strains are highly contagious. You can get the wart virus by touching an object that another person’s wart touched, such as clothing, towels, shoes, or exercise equipment.

Breaks in Your Skin

HPV infects the top layer of skin and usually enters the body in an area of damaged or cut skin. Cuts from shaving can provide an avenue for infection. Getting a scrape can also bring on common warts. Foot warts are very common in swimmers whose feet are scratched by rough pool surfaces.

A Weak Immune System

In most cases, your immune system defeats an HPV infection before it creates a wart. Someone with a weakened immune system is more vulnerable and therefore more likely to develop warts. Immune systems can be weakened by HIV or by immunosuppressant drugs used after organ transplants.

The many faces of HPV

Recognizing plantar warts

To figure out whether you’ve been infected by HPV, it’s useful to recognize the signs of plantar warts, which are so named because warts typically develop on the plantar (bottom) side of your foot.

There are two different types of plantar warts — mosaic and solitary. Mosaic warts are groups of growths that develop close together while solitary warts, as the name implies, are single growths.

Whatever their configuration, the hallmarks of plantar warts include:

  • A fleshy and/or rough outcropping of skin
  • Patches of thick skin that look like a callus
  • Black dots at the center of the wart, which are tiny spots of blood
  • Pain and discomfort, especially when you apply pressure to the wart

Plantar warts most often form on weight-bearing areas, around the balls of your feet or on your heels, which is why they can cause so much discomfort.

Treating plantar warts

If you suspect you’ve developed plantar warts, it’s always a good idea to have us check them out. This is especially true if you have a pre-existing condition like diabetes as any problem in your feet is cause for concern.

If we confirm the presence of plantar warts, we can take several approaches to clear the infection, which include:

  • Injections that target the virus

If you’ve observed small, new growths on the sole of your child’s foot, you don’t have to panic. Your child has probably been exposed to HPV, which is a virus that can infect the skin and develop into plantar warts. Plantar warts are typically seen in patients between the ages of 12 and 16, and the HPV virus can infect numerous areas of the body, including the soles of the feet. 

 

They are often spread via direct contact, most often when the patient walks barefoot in public showers. It’s important to remember that the strain of HPV that causes plantar warts do not spread to other parts of the body.

 

The gestation period for HPV on the bottom of the foot generally ranges from 1 to 20 months. This makes it difficult to determine when and where the virus was contracted. Once it does begin to develop, it can go undetected, as many patients don’t experience symptoms.

 

Because of the increased thickness of the skin on the sole of the foot, in its early stages, the infected area is often mistaken for a callus.

 

Warts can go away on their own, but not all of them will be resolved without treatment. OTC treatments can be tried in addition to keeping feet dry and clean to eliminate moisture. However, for warts that are have been there for a long time, or have become painful, you’ll need to see a podiatrist.

 

Depending on the length and severity of the infection time, there are several treatment options that can be utilized. The method used will be based on your podiatrist’s opinion and assessment of your child’s infection. Laser treatment is a very effective treatment method that is accurate, clean, and painless.

  • The formation of a callus, which indicates that the wart is growing inward
  • Tiny black dots developing in the wart, also known as wart seeds
  • Difficulty walking or standing due to foot pain

Overview

Plantar warts are small growths that usually appear on the heels or other weight-bearing areas of your feet. This pressure may also cause plantar warts to grow inward beneath a hard, thick layer of skin (callus).

Plantar warts are caused by HPV. The virus enters your body through tiny cuts, breaks or other weak spots on the bottom of your feet.

Most plantar warts aren’t a serious health concern and usually go away without treatment eventually. You may want to try self-care treatments or see your doctor to have the warts removed.

Plantar warts are caused by the same type of virus that causes warts on your hands and fingers. But, because of their location, they can be painful.

Symptoms

Plantar wart signs and symptoms include:

  • A small, fleshy, rough, grainy growth (lesion) on the bottom of your foot, usually the base of the toes and forefoot or the heel
  • Hard, thickened skin (callus) over a well-defined “spot” on the skin, where a wart has grown inward
  • Black pinpoints, which are commonly called wart seeds but are actually small, clotted blood vessels
  • A lesion that interrupts the normal lines and ridges in the skin of your foot
  • Pain or tenderness when walking or standing

When to see a doctor

See your doctor for the lesion on your foot if:

  • The lesion is bleeding, painful or changes in appearance or color
  • You’ve tried treating the wart, but it persists, multiplies or recurs
  • Your discomfort interferes with activities
  • You also have diabetes or poor sensation in your feet
  • You also have a weakened immune system because of immune-suppressing drugs, HIV/AIDS or other immune system disorders
  • You aren’t sure whether the lesion is a wart

Causes

Plantar warts are caused by an infection with HPV in the outer layer of skin on the soles of your feet. They develop when the virus enters your body through tiny cuts, breaks or other weak spots on the bottoms of your feet.

HPV is very common, and more than 100 kinds of the virus exist. But only a few of them cause warts on the feet. Other types of HPV are more likely to cause warts on other areas of your skin or on mucous membranes.

Transmission of the virus

Each person’s immune system responds differently to HPV. Not everyone who comes in contact with it develops warts. Even people in the same family react to the virus differently.

The HPV strains that cause plantar warts aren’t highly contagious. So the virus isn’t easily transmitted by direct contact from one person to another. But it thrives in warm, moist environments. Consequently, you may contract the virus by walking barefoot around swimming pools or locker rooms. If the virus spreads from the first site of infection, more warts may appear.

Risk factors

Anyone can develop plantar warts, but this type of wart is more likely to affect:

  • Children and teenagers
  • People with weakened immune systems
  • People who have had plantar warts before
  • People who walk barefoot where exposure to a wart-causing virus is common, such as locker rooms

Complications

When plantar warts cause pain, you may alter your normal posture or gait — perhaps without realizing it. Eventually, this change in how you stand, walk or run can cause muscle or joint discomfort.

Prevention

To reduce your risk of plantar warts:

  • Avoid direct contact with warts. This includes your own warts. Wash your hands carefully after touching a wart.
  • Keep your feet clean and dry. Change your shoes and socks daily.
  • Avoid walking barefoot around swimming pools and locker rooms.
  • Don’t pick at or scratch warts.
  • Don’t use the same emery board, pumice stone or nail clipper on your warts as you use on your healthy skin and nails.

Diagnosis

In most cases, your doctor can diagnose a plantar wart with one or more of these techniques:

  • Examining the lesion
  • Paring the lesion with a scalpel and checking for signs of dark, pinpoint dots — tiny clotted blood vessels
  • Removing a small section of the lesion (shave biopsy) and sending it to a laboratory for analysis

Treatment

Most plantar warts are harmless and go away without treatment, though it may take a year or two. If your warts are painful or spreading, you may want to try treating them with over-the-counter (nonprescription) medications or home remedies. You may need many repeated treatments before the warts go away, and they may return later.

If your self-care approaches haven’t helped, talk with your doctor about these treatments:

  • Stronger peeling medicine (salicylic acid). Prescription-strength wart medications with salicylic acid work by removing layers of a wart a little bit at a time. They may also stimulate your immune system’s ability to fight the wart.

    Your doctor will likely suggest you apply the medicine regularly at home, followed by occasional office visits.

  • Freezing medicine (cryotherapy). Cryotherapy done at a doctor’s office involves applying liquid nitrogen to the wart, either with a spray or a cotton swab. This method can be painful, so your doctor may numb the area first.

    The chemical causes a blister to form around your wart, and the dead tissue sloughs off within a week or so. Cryotherapy may also stimulate your immune system to fight viral warts. You may need to return to the doctor’s office for repeat treatments every two to four weeks until the wart disappears.

    Some studies suggest that cryotherapy combined with salicylic acid treatment is more effective than just cryotherapy, but further study is needed.

Surgical or other procedures

If salicylic acid and freezing medicine don’t work, your doctor may recommend one or more of the following treatments:

  • Other acids. Your doctor shaves the surface of the wart and applies trichloroacetic acid with a wooden toothpick. You’ll need to return to the doctor’s office for repeat treatments every week or so. Side effects include burning and stinging. Between visits, you may be asked to apply salicylic acid to the wart.
  • Immune therapy. This method uses medications or solutions to stimulate your immune system to fight viral warts. Your doctor may inject your warts with a foreign substance (antigen) or apply a solution or cream to the warts.
  • Minor surgery. Your doctor cuts away the wart or destroys it by using an electric needle (electrodesiccation and curettage). This procedure can be painful, so your doctor will numb your skin first. Because surgery has a risk of scarring, this method usually isn’t used to treat plantar warts unless other treatments have failed.
  • Laser treatment. Pulsed-dye laser treatment burns closed (cauterizes) tiny blood vessels. The infected tissue eventually dies, and the wart falls off. This method requires repeat treatments every three to four weeks. The evidence for the effectiveness of this method is limited, and it can cause pain and potentially scarring.
  • Vaccine. HPV vaccine has been used with success to treat warts even though this vaccine is not specifically targeted toward the wart virus that causes the majority of plantar warts.

Lifestyle and home remedies

Many people have removed warts with these self-care tips:

  • Peeling medicine (salicylic acid). Nonprescription wart removal products are available as a patch or liquid. Usually, you’re instructed to wash the site, soak it in warm water, and gently remove the top layer of softened skin with a pumice stone or emery board. Then after the skin has dried, you apply the solution or patch. Patches are usually changed every 24 to 48 hours. Liquid applications are generally used daily. You may need repeated applications on a regular basis over several weeks to months to see results.
  • Freezing medicine (cryotherapy). Nonprescription medicines that freeze the wart include Compound W Freeze Off and Dr. Scholl’s Freeze Away. The Food and Drug Administration cautions that some wart removers are flammable and shouldn’t be used around fire, flame, heat sources (such as curling irons) and lit cigarettes.
  • Duct tape. Using duct tape to remove warts is a harmless but unproven approach. To try it, cover the wart with silver duct tape, changing it every few days. Between applications, soak the wart and gently remove dead tissue with a pumice stone or emery board. Then leave the wart open to the air to dry for a few hours before covering it with tape again.

You’ll likely start by seeing your primary care doctor. He or she may refer you to a specialist in disorders of the skin (dermatologist) or feet (podiatrist). The following tips can help you prepare for your appointment.

Bring a list of all medications you take regularly — including over-the-counter (nonprescription) medications and dietary supplements — and the daily dosage of each.

You may also want to list questions for your doctor, such as:

  • If I have a plantar wart, can I start with at-home care?
  • If I proceed with home treatment, under what conditions should I call you?
  • If the first treatment doesn’t work, what will we try next?
  • If the lesion isn’t a plantar wart, what tests do you need to do?
  • How long will it take to get results?
  • How can I prevent warts?

What to expect from your doctor

Your doctor may ask you questions such as:

  • When did the lesion first appear?
  • Has it changed in size or appearance?
  • Is your condition painful?
  • Have you had warts before?
  • Do you have diabetes or poor sensation in your feet?
