When blood vessels in and around the anus become inflamed, swollen tissues called piles or “hemorrhoids” result.

Hemorrhoids are actually a normal part of physiology. They assist in closing the anal canal and contribute to up to 20% of the anus’s resting pressure. They are also believed to somehow differentiate between stool, liquid, and gas.

This raises numerous questions about passing gas with hemorrhoids. Can hemorrhoids cause gas buildup? Do hemorrhoids make you fart?

Hemorrhoids can create a feeling of increased pressure in the anus, prompting people with hemorrhoids to ask, do hemorrhoids make you fart? Because inflamed hemorrhoids often alter bowel activity, they can lead to abdominal bloating and gas. The cross purposes between trying to minimize hemorrhoid pain and regularly relieving oneself can leave those experiencing both swollen hemorrhoids and excessive gas simultaneously in a difficult position.

What helps relieve gas from hemorrhoids?

The main cause of excessive gas is an improper balance of soluble and insoluble fiber, which affects stool and digestive processes and can create bacterial overgrowth. Whether bacterial overgrowth is causing gas can be determined by a breath test.

Bacteria located in the colon are there to digest food — but when the stool fails to pass through the body, the bacteria continue digesting it, which creates a buildup of gas.

The best way to relieve abdominal gas depends heavily on the individual experiencing it and the factors influencing their digestive health.

How to avoid gas buildup with hemorrhoids?

As mentioned, relieving oneself whenever necessary and not delaying bowel movements is important both for the relief of painful hemorrhoids and excessive gas. Methods for reducing gas include lifestyle changes, such as avoiding gum and refraining from talking while eating and drinking. Diet itself is also a major factor.

Diet to reduce gas

Some people experience gas differently, and everyone’s microbiome is different. The National Institute of Health recommends limiting certain types of carbohydrates that don’t fully digest. Once these kinds of carbohydrates reach the large intestine, they require much more bacterial activity to fully digest food and turn it into the stool.

These foods include:

  • High fructose corn syrup

  • Candy and gum — especially those containing sorbitol, erythritol, and other sweeteners ending in “-ol”

  • Dairy products

  • Legumes

  • Whole grains

  • Cruciferous vegetables

The NIH also suggests trying a FODMAP diet, which helps zero in on those particular carbohydrates that are hardest to digest for the individual. They also stress the importance of ruling out other potential causes of gas, such as celiac disease, irritable bowel syndrome, and dietary allergens.

In addition to altering diet, it can help to simply sit and eat more slowly rather than “eating on the run.”

Other causes of abdominal gas buildup

Discomfort caused by gas can also be caused by other factors and conditions, including smoking, acid reflux, and intestinal obstructions.

Certain cancers are sometimes associated with intestinal obstructions. In particular, colon and rectal cancer can cause gas and anal bleeding. Because these conditions are more serious and have similar symptoms to hemorrhoid issues, care should be taken not to mistake hemorrhoids for cancer and vice versa.

Aside from possible pain and swelling, symptoms of swollen hemorrhoids include itching and small amounts of bright red blood in the stool. Larger amounts of blood — more than several drops — could indicate other, more concerning issues.

Note that hemorrhoids can occur externally, on or around the anus, or internally, in the anal canal. If internal hemorrhoids continue to swell, they could be pushed out, especially through excess strain.

These “prolapsed” hemorrhoids are often about the size of a grape and are very tender. They may return inside the anus or be carefully pushed back in.

Hemorrhoids can also become “thrombosed,” where a blood clot is present. In rare cases, hemorrhoids can rupture, causing more severe bleeding. Anal bleeding and pain are symptoms of several other conditions, however, so it’s extremely important to receive medical attention if you experience ongoing anal bleeding of any amount.

Without continual aggravation, hemorrhoids most often disappear on their own in around one week. The main treatments for hemorrhoids involve home care, but clinical treatments may be necessary if they worsen.

One essential treatment for reducing the blood vessel inflammation that causes hemorrhoids is to reduce swelling and strain and encourage the muscles around the anus to relax. Similarly, softening one’s stool helps relieve the strain involved in hemorrhoids. To these ends, common treatments for hemorrhoids flare-ups include:

  • Reducing lower body strain and exertion

  • Laying down on one’s side

  • Wearing loose-fitting clothing

  • Refraining from scratching or touching the area

  • Softening the stool (but not with laxatives)

  • Taking a sitz bath (possibly with Epsom salt — although exercise caution, as if the hemorrhoids have broken open, it could sting)

  • Squatting over a toilet instead of sitting on it

Other treatments for external hemorrhoids

Keeping external hemorrhoids clean can be challenging, especially if they are extremely tender. Even if not too painful, it’s important not to make them worse with excessive friction — and yet they must be kept clean to prevent infection, especially if the skin starts breaking.

Some find soothing, chemical-free wipes useful in keeping the area clean and soothing the discomfort. Many such wipes contain witch hazel or aloe vera. There are also over-the-counter creams.

Outpatient procedures such as rubber band ligation and infrared photocoagulation are also options, as is surgical removal. These can result in a recurrence of symptoms, but they provide more immediate relief. Some clinical procedures require low-intensity recovery periods, while others are less invasive and allow patients to return to their normal functions.

Other treatments for internal hemorrhoid

Internal hemorrhoids can be somewhat harder to treat. Treatments for internal hemorrhoids include internal suppositories with soothing ingredients, such as witch hazel and coconut oil, as well as pharmaceutical compounds.

If internal hemorrhoid has prolapsed outside the anus, it can become “strangulated,” where the anal muscles cut off blood flow. The prolapsed hemorrhoid may go in on its own — otherwise, it can be gently pressed back into the anus. Relaxing the anal muscles should help.

Like external hemorrhoids, internal hemorrhoids can be treated through clinical and surgical procedures if they become debilitating or painful enough.

When to seek clinical treatments for hemorrhoids?

If hemorrhoid symptoms persist without relief for more than a week, even with over-the-counter products, consider consulting a doctor to determine other possible causes. A professional diagnosis may help you understand whether your hemorrhoid symptoms correlate with other issues that could be more serious.

Also, seek a diagnosis if you suspect blood-clot-related hemorrhoids (aka a “thrombosed” hemorrhoid), as blood clots affect 900,000 Americans yearly, killing 100,000. External thrombosed hemorrhoids are more likely with “acute constipation” and those with a history of prolonged straining.¹

Hemorrhoids are a normal part of the physiology of the anus. But even though they help maintain the right level of anal pressure, there is no evidence that hemorrhoids directly cause gas buildup.

If hemorrhoids and gas coincide, their relationship can contribute to the potential discomfort of hemorrhoids, and it’s possible they are both being caused by another undiagnosed condition, for which they are both symptoms.

While hemorrhoids haven’t been found to cause gas, they can affect gas levels indirectly by making pressure on the anus painful and altering bowel movement activity. Those suffering from painful hemorrhoids may experience painful bowel movements, not relieving themselves as soon as they normally would.

This leads to stool that doesn’t move through the body quickly enough, which causes gas to build up as bacteria continue to break down stool more than necessary.

As a result, hemorrhoids can indirectly lead to more gas buildup. This can be mitigated by loosening one’s stool, reducing strain, treating the inflammation, and altering one’s diet to prevent as much gas from building up.

Is It Hemorrhoids or Something Else?

It’s not uncommon to have hemorrhoids — swollen veins around the outside of the anus or the lower part of your rectum that can cause painful burning or itching. In fact, 75 percent of people will get hemorrhoids at some point in their lifetime, according to the National Institute of Diabetes and Digestive and Kidney Diseases. They can often be treated with over-the-counter remedies, and you may not even need to see a doctor.

“Hemorrhoids are part of being human and play an important role in continence,” says Kinga Skowron Olortegui, MD, a colorectal surgeon and assistant professor at UChicago Medicine. “The problem is when they act out, but it’s often the pushing or straining due to constipation that makes them swell. So treating the constipation with a fiber supplement is one of the most effective treatments.”

But some symptoms of hemorrhoids, like rectal bleeding, where you may find bright red blood on your toilet paper or see blood in the toilet after a bowel movement, may also be caused by other diseases, some of them serious, like colon cancer.

So how, then, do you know when to see a doctor?

“If patients have pain or bleeding after their bowel movements have normalized, they should have it checked by a doctor,” says Dr. Olortegui.

Determining the Diagnosis

When symptoms can be caused by more than one disease, doctors call it a differential diagnosis. Here are seven different causes for hemorrhoid symptoms you need to know about:

  • Colon Cancer, Rectal, or Anal Cancer “If there’s bleeding or a bump that’s growing, we want check for cancer,” says Olortegui. It’s a growing concern, especially since colon cancer rates are rising in people under 50, according to a March 2020 study published in CA: A Cancer Journal for Clinicians. These cancers can occur near the rectum and cause bleeding and discomfort that are similar to hemorrhoid symptoms. Colon cancer symptoms may include persistent bleeding, a change in bowel habits or bowel movement shape, lower abdominal pain, and unexpected weight loss. Anal cancer is much less common than colon or rectal cancer. Signs of anal cancer include bleeding around the anus, a growing lump, pain in that area, or discharge. If any of those symptoms are prevalent, you should speak to your doctor.
  • Inflammatory Bowel Disease (IBD) This condition, which includes ulcerative colitis and Crohn’s disease, can cause rectal bleeding and discomfort. Both types of IBD are long-term diseases where the sphincter may become inflamed, producing symptoms similar to hemorrhoids. Other symptoms of IBD may include cramps, diarrhea, weight loss, and fever. Keep in mind that people with IBD also have hemorrhoids because of their abnormal bowel habits.
  • Anal Fissures “Anal fissures are small tears, like paper cuts, in the anal canal that can act very much like hemorrhoids,” says John Hall, MD, a colorectal surgeon and surgeon in chief and chairman of the department of surgery at Tufts Medical Center in Boston. Anal fissures can cause pain, burning, and bleeding. They can occur at any age and, like hemorrhoids, are usually caused by constipation. They usually clear up with home treatments similar to those used for hemorrhoids. However, if the bleeding doesn’t clear or if its accompanied by a growing lump, talk to your doctor.
  • Perirectal Abscess The abscess is a pus-filled bump found near your anus or rectum and can occur when one of your anal glands gets clogged and infected. Symptoms like pain and being uncomfortable can mimic hemorrhoids, but abscesses tend to be more painful, constantly throbbing, and larger than a hemorrhoid.
  • Pruritis Ani “Also known as anal itching, it’s easily mistaken for hemorrhoids, but it’s just an itch that often clears up with some dietary changes,” explains Olortegui. “It’s often triggered by spicy or acidic foods.” Pruritis ani causes an intense urge to scratch. It may result from too much moisture or a food sensitivity. Treatment involves keeping the area dry, avoiding scrubbing, and using a prescription ointment or cream.
  • Pinworms Commonly found in children, pinworms are tiny parasites that hatch in the small intestine, then move on to live in the large intestine for about one or two months. The female worms then leave the body to lay their eggs on the skin surrounding the anus. While most people with a pinworm infection generally don’t exhibit symptoms, those who do experience anal itching, a symptom similar to hemorrhoids.
  • Anal Warts Similar to genital warts, one of the many sexually transmitted diseases (STDs) that can grow in the anal area and cause symptoms of bleeding and discomfort. Anal warts and genital warts are caused by human papilloma virus (HPV), and they should always be treated because they will continue to grow and increase the chances of getting anal cancer.

Getting Help for Hemorrhoid Symptoms

You should seek treatment for hemorrhoid symptoms if:

  • You have rectal bleeding for the first time.
  • You have heavy rectal bleeding.
  • You have rectal bleeding that is not responding to home care.
  • You see blood in your stool.
  • You have other hemorrhoid symptoms, such as pain, pressure, itching, and burning, that do not respond to home care after a few days.
  • You have hemorrhoid symptoms along with other symptoms such as fever, weight loss, abdominal pain, or a change in bowel habits.
  • You have a bump that seems to be growing.
  • You have continued pain after bowel movements are normal again

“If there’s bleeding, we’re going to do a colonoscopy to rule out some dangerous conditions,” says Olortegui. “If we can rule out cancers, then we evaluate the condition with an anoscopy [a short tube used to examine the anal area] to give us a clear picture of the rectum and anal canal. That way we can make sure the hemorrhoids look okay or we can determine if surgical intervention is warranted.”

Hemorrhoids are a common proctological problem. The disease consists in hypertrophy of the cavernous veins located in the rectum, and is accompanied by a number of unpleasant symptoms. Hemorrhoids are characterized by rectal bleeding, pain in the anus. At the heart of the pathology is stagnation of blood in the venous plexuses, an increase in hemorrhoids in size. The disease occurs against the background of various congenital and acquired factors, causes a lot of anxiety to patients, but can be stopped with the help of modern methods of treatment.

The human anal canal has a complex structure, there are hemorrhoidal venous plexuses, similar in structure to the cavernous tissue. They take part in the retention of gases and feces. Under the influence of a genetic defect, expressed by a weak vascular wall, as well as as a result of stretching and traumatization of the plexuses, hemorrhoidal disease develops. Anything that can cause an increase in pressure in the vessels located in the pelvic area provokes hemorrhoids. Usually, the disease begins to form from the internal plexuses, then it passes to the external ones. When the structural frame is damaged, irreversible prolapse of nodes occurs – this is typical for stage 3-4 hemorrhoids.

Development of hemorrhoids

Both women and men are equally affected by hemorrhoids. Increasingly, this diagnosis is received by people at a fairly young age, so it is important for everyone to know about the causes of hemorrhoids, symptoms, methods for its diagnosis and methods of treatment.

Causes of hemorrhoids

So, the following reasons can provoke the development of the disease:

  • Constant constipation and need to push when going to the toilet
  • Lack of movement due to sedentary work, limited mobility and lifestyle in general
  • Excessive physical activity associated with weight lifting, horseback riding, cycling
  • Bad habits, such as excessive drinking of alcoholic beverages
  • An unbalanced diet, in which there is clearly a lack of dietary fiber and whole foods, and semi-finished products, fatty, spicy, smoked foods predominate.

Sedentary lifestyle is one of the causes of hemorrhoids

In general, the way of life of most modern people can be described by the word “physical inactivity”. Comfort surrounds us everywhere: on the way to work and home, in the office and at home. Sedentary work, unwillingness to play sports – all this sooner or later leads to a deterioration in the condition of the venous plexuses. Leaves its mark and nourishment. With a lack of fiber, many face problems when going to the toilet. Chronic constipation is a concern for a very large percentage of the population. Dense fecal masses injure the structures of the rectum, hence the hemorrhoidal bleeding. In general, we can say that hemorrhoids is a complex problem, it does not happen that one factor causes the development of the disease in a patient. Understanding what provoked the deterioration of the condition is the key to a well-built treatment strategy. In order to apply for a diagnosis in a timely manner, you need to know what symptoms are characteristic of hemorrhoids.

Symptoms of hemorrhoids in men and women

Most proctological diseases are quite insidious, the fact is that they may not manifest themselves as some characteristic symptoms until the moment when the disease no longer passes to a more serious stage. It happens the same way with hemorrhoids. In the first stage, patients often do not recognize the discomfort in the anus and rectum that occurs from time to time. Further, more significant symptoms usually join, for example, the sensation of a foreign body in the anus, a feeling as if the intestines were not completely emptied when visiting the toilet. This happens against the background of an increase in internal hemorrhoids in size and because the folds in the anal canal are displaced.

After going to the toilet, the patient may find blood on toilet paper, stool or linen. Bleeding can be both insignificant and profuse, literally in the form of a jet of blood.

In addition to blood, mucus may be discharged from the anus. This, in turn, causes tissue irritation, which can lead to anal itching and a burning sensation in this delicate area.

Enlarged hemorrhoids resemble bumps in appearance. Such a phenomenon occurs as a result of the fact that the blood flow to the plexuses has increased, but she could not move back. Fallen nodes at the initial stage of the disease return to their place on their own, at later ones you will need to do it with your hands, and at completely neglected ones only a doctor can help.

Hemorrhoids

Pain often accompanies progressive hemorrhoids. This happens due to traumatization of the nodes, swelling and irritation of the skin around the anus, as well as thrombosis of the nodes and their infringement. For advanced cases, pain will be an almost constant phenomenon. Sometimes patients experience dull or sharp pain even while walking or sitting.

How do hemorrhoids progress

The patient may have a pathology of the hemorrhoidal plexuses, but not know about it until the moment when the disease goes into the acute stage. The course of the disease usually consists of alternating these two conditions: remission and exacerbations. If it was possible to diagnose hemorrhoidal disease at an early stage, then most likely the treatment will take a minimum of time and will not become a problem. In here, chronic neglected hemorrhoids cause a lot of inconvenience, to the point that it does not allow a person to fully live, work, play sports and lead a normal lifestyle. Hemorrhoids are dangerous not only by the deterioration in the quality of life, but also by their complications. Many proctological problems can develop against the background of this disease, for example, paraproctitis often becomes a complication of acute hemorrhoids, and anal fissures are found in many patients during examination. In general, the symptomatic picture of acute and chronic hemorrhoids has many intersections with other diseases of the proctological spectrum, so if you experience any discomfort in the anus, you should consult a specialist as soon as possible.

The form of the disease is internal, external and combined. In the first case, the plexuses located directly under the mucous membrane inside the rectum suffer, in the second – around the anus. If the patient has enlarged and inflamed nodes both inside and outside, then he is diagnosed with “combined hemorrhoids”.

Stages of hemorrhoids

In total, doctors distinguish four stages of this disease. At the first, very initial, patients may notice unusual discomfort in the rectum. On examination, the doctor will see that the nodes are enlarged, but do not fall out, while the vascular pattern of the mucosa will be bright. As the disease progresses, the symptoms progress and intensify. At the second stage, bleeding from the anus joins – the pathological nodes are injured by solid feces, which provokes the release of blood. In addition, mucous discharge is possible, the perianal area is often irritated, the tissues there may swell and itch. In the process of emptying the intestines in patients with the second stage of hemorrhoids, problem nodes may fall out, but they are reduced on their own. The third stage of the disease is characterized by a deterioration in health, bleeding more often, while they can be more abundant, and pain is stronger. Not only straining during defecation, but also any physical stress, for example, lifting a heavy object or even a sharp change in body position, can contribute to the loss of nodes at this stage of the development of the disease. The last stage of hemorrhoids is associated with the constant discomfort of the patient, the nodes are no longer set into place either by themselves or with the help of hands, itching and pain interfere with normal life, fecal incontinence is added, since the sphincter ceases to close normally. Often it is painful for patients even just to sit. It is worth mentioning such an important aspect of hemorrhoids as the psychological state of the patient. Running hemorrhoids makes patients feel insecure, makes them shy, prevents them from leading a normal life. In some cases, people may even lose their ability to work. Finding a disease in its infancy is the best guarantee that it will be possible to eliminate its root cause and cope with the consequences.

How is hemorrhoids diagnosed

Diagnosis and treatment of hemorrhoidal disease is the area of ​​expertise of a coloproctologist. Many use another name for this specialty – proctologist. In order for the appointment to be comfortable for both the patient and the doctor, you need to pay attention to preparing for it. Before a visit to the clinic, you will need to clean the intestines, it is most convenient to do a microclyster for this. They can be bought at any pharmacy. As with any other medical appointment, it all starts with a consultation. The coloproctologist will interview the patient: what worries you, how long ago the symptoms appeared, what diseases she has in history, what lifestyle the person leads. The more information that can be collected at this stage, the better – this will help to accurately establish the diagnosis and gain an understanding of the causes of the problem.

Proctologist’s examination

The next part of the visit to the proctologist is the examination itself. The doctor will examine the perianal area and part of the rectum. To do this, the patient will be asked to sit in a special chair, take a pose lying on his side, take a knee-elbow position. The doctor first examines the outside of the anus and the tissues around it. The specialist assesses the shape of the anus and the degree of its closure, the color of the tissues, the presence and size of hemorrhoidal cones, their color. Next, conduct a finger test. To get more data, the proctologist will need to perform anoscopy or sigmoidoscopy. This is an endoscopic instrumental study that does not cause significant discomfort to the patient, but at the same time allows you to detect pathological changes inside the rectum, examine the nodes, their condition, and notice anal fissures. During the examination of the patient, the doctor may decide that additional diagnostics are needed to clarify the diagnosis. In this case, they are prescribed to do an ultrasound, colonoscopy, donate blood, urine and feces. Also, consultations of other specialists, for example, a gastroenterologist or an oncologist, may sometimes be necessary.

As a result of all diagnostic measures, the coloproctologist can determine the form of hemorrhoids and its stage. This determines what the treatment will be, whether the patient will need rehabilitation after it.

Methods of treatment of hemorrhoids
