Erectile Dysfunction: Treatment & Care

If you think you have ED, a good first step is to talk with your doctor. The treatment you need – ranging from lifestyle changes to medication to surgery – will depend on what’s causing it.

    1. ED Tests: What to Expect at the Doctor

With so many possible causes for ED, your doctor has a number of tests they can use to figure out the best treatment for you.

    2. ED Treatment Overview

Erectile dysfunction can be treated at any age. Treatment depends on your overall health and the underlying cause of the problem.

    3. What to Ask Your Doctor About ED

It can be embarrassing to discuss sexual matters with your doctor. Yet a doctor’s appointment is  neccessary if you want to get treatment for ED. Here’s how to prepare for your doctor visit.

    4. ED Vacuum Devices

A vacuum constriction device (VCD) is an external pump with a band on it that a man with erectile dysfunction can use to get and maintain an erection.

    5. Penile Implants for ED

A penile prosthesis or implant is an effective treatment for erectile dysfunction. Penile implant surgeries take about an hour and are typically done in an outpatient center.

    6. ED Surgery

Vascular reconstructive surgery is one way to improve blood flow to the penis to help a man with erectile dysfunction get and keep an erection.

    7. Cialis, Levitra, Staxyn, and Viagra to Treat ED

You may already know most of the names: Cialis, Levitra, Staxyn, Stendra, Viagra. Learn more about these medications that can help men with erectile dysfunction.

    8. Alprostadil for ED

Alprostadil, available as an injection or a suppository, is a drug that makes the blood vessels expand. That boosts blood flow throughout the body, including the penis, so it helps men with ED have an erection.

    9. New ED Treatments

Future treatments for erectile dysfunction focus on providing medications that are more effective, work rapidly, and have fewer, if any, side effects than currently available treatments.

    10. Testosterone Replacement Therapy for ED

Inadequate production of testosterone is not a common cause of erectile dysfunction. But, when ED does occur due to decreased testosterone production, testosterone replacement therapy may improve the problem.

    11. Alternative Remedies for ED

If you have erectile dysfunction, there are many ways your doctor can help you treat it. But you may want to consider complementary or alternative therapies, such as nutritional supplements, herbal remedies, acupuncture, and talk therapy. Just be sure to talk to your doctor first.

    12. Yohimbe Bark Supplements for ED

You can buy extract of the bark of the yohimbe tree – long used as an aphrodisiac in western Africa — as a dietary supplement. Its promoters claim it can help erectile dysfunction. But the science shows mixed results.

    13. Horny Goat Weed (Epimedium) and ED

Horny goat weed is an herb that has been used in China for centuries to treat low libido, erectile dysfunction, fatigue, and other conditions. Learn how its extract may help increase blood flow to the penis.

    14. Quit Smoking to Help ED

Men who smoke are about twice as likely to develop ED as nonsmokers. Smoking hampers circulation to all areas of the body, including the genitals, making it tougher to get and keep an erection.

For Many Men, Impotence Is Treatable Without Drugs – WebMD

TUESDAY, April 22, 2014 (HealthDay News) — A new study reminds men with erectile dysfunction that there’s help out there that doesn’t require a prescription: diet, exercise and other lifestyle changes.

Losing weight, eating better, getting more active, drinking less alcohol and getting better sleep can all help reverse problems that contribute to impotence, according to a new study published recently in the Journal of Sexual Medicine.

Erectile dysfunction and low sexual desire are often linked to the development of heart disease. The researchers discovered that a large proportion of men were able to naturally overcome erectile dysfunction with heart-healthy changes — no pharmaceutical help necessary.

What’s more, focusing on lifestyle change helps ensure an overall healthier and longer life, the researchers added.

While one of the biggest factors contributing to impotence is advancing age, other factors seem to play an even greater role in the development of the problem, explained study lead author Dr. Gary Wittert. Besides, because a significant number of men maintain erectile function into advanced age, it’s unlikely that getting older, in and of itself, is the cause of sexual dysfunction, he said.

Instead, impotence seems to be typically related to an unhealthy lifestyle.

“It is always worth reducing obesity, improving nutrition and getting more exercise — firstly, because health and well-being improve and overall cardiovascular risk and risk of diabetes will be reduced,” said Wittert, a professor and director of Freemasons Foundation Centre for Men’s Health at the University of Adelaide, in Australia.

What’s the connection between impotence and heart health? “An erection is a hydraulic event dependent on the dilation of blood vessels that carry blood to the penis,” explained Wittert. “These blood vessels are similar to those that supply blood to the heart muscle.”

Although other issues such as nerve damage and hormone abnormalities can also lead to erectile dysfunction, the failure of the blood vessels to dilate properly is one of the more common causes, Wittert said. “This is an early abnormality in the pathway to more serious heart disease.”

For the study, data was collected from more than 800 randomly selected Australian men, 35 to 80 years old at the beginning of the study, with follow-up five years later. Sexual desire was assessed using a standard questionnaire that addressed interest in engaging with another person in sexual activity, interest in engaging in sexual behavior by oneself, and no interest in sexual intimacy.

Erectile function was also assessed using a standard scoring system. The researchers took factors such as height, weight, blood pressure, hand grip strength, amount of body fat, age, education, marital status, occupation and smoking behavior into account. Depression, the probability of obstructive sleep apnea, medication usage, diet and alcohol consumption, and physical activity were also assessed, as were blood levels of glucose, triglycerides (an unhealthy blood fat) and cholesterol.

People whose health habits and lifestyle improved during the study period tended to see an improvement in sexual function, Wittert’s team reported. And the reverse was true: those whose health habits and lifestyle deteriorated during the five years were more likely to experience impotence.

One expert said the study carries valuable lessons for men worried about their sexual health.

“As we get older, there are some natural things we just can’t change. The message from this study is, don’t get a prescription, but get exercise. Get rid of the fat. Work on the depression,” said Dr. David Samadi, chairman of the department of urology at Lenox Hill Hospital, New York.

Samadi, who was not involved in the research, warned that a prescription is not as good as a fundamental lifestyle change. “Long-term, medication is not the answer unless you take care of the high blood pressure or high cholesterol or diabetes,” he said. “Medication works well for those who cannot make the necessary changes, but drugs should not be the first line of treatment.”

Yet Wittert, the researcher, isn’t against using medication to treat sexual dysfunction. However, he said he tries to encourage men to tackle their lifestyle issues at the same time. He recommends using drugs to initially solve the problem, and then begin to modify lifestyle and risk factors. Healthier living can make impotence drugs more effective or make them less necessary, and a better lifestyle also tends to increase sexual desire, Wittert said.

Both experts agree that there are many indirect causes of sexual dysfunction and low sex drive. The best bet is to prevent or treat the underlying disease, they said.

8 Natural Cures for Erectile Dysfunction

Erectile dysfunction (ED), or impotence, is a major challenge for many men today regardless of their age — young, middle-aged, or old. Because erectile dysfunction may be caused by many factors — a health condition, emotional or relationship problems, some kinds of medication, smoking, drugs, or alcohol — an erectile dysfunction cure is possible.

Though erectile dysfunction treatment options may include ED medication and surgery, there are also noninvasive erectile dysfunction remedies that may help.

According to Mayo Clinic, men with erectile dysfunction may experience some or all of these persistent symptoms:

  • Reduced sexual desire
  • Trouble getting an erection
  • Trouble keeping an erection

In fact, one common reason many younger men visit their doctor is to get erectile dysfunction medication. Often, men with erectile dysfunction suffer with diabetes or heart disease, or may be sedentary or obese, but they don’t realize the impact of these health conditions on sexual function. Along with erectile dysfunction treatment, the doctor may recommend managing the illness, being more physically active, or losing weight.

The causes of erectile dysfunction are many, including physical problems like diabetes, heart disease, obesity, high blood pressure, high cholesterol, and even sleep disorders. Medication taken for health conditions can result in erectile dysfunction, as can alcohol and smoking.

But erectile dysfunction is also caused by mental health problems, such as anxiety, depression, stress, and trouble with relationships. That’s why visiting a medical doctor for erectile dysfunction treatment is so valuable.

If ignored, erectile dysfunction can lead to complications, such as an unsatisfactory sex life, low self-esteem, high anxiety, and relationship problems. Getting a partner pregnant also becomes difficult if a man suffers with erectile dysfunction.

A study published in May 2014 in The Journal of Sexual Medicine found that some men can reverse erectile dysfunction with healthy lifestyle changes, such as exercise, weight loss, a varied diet, and good sleep. The Australian researchers also showed that even if erectile dysfunction medication is required, it’s likely to be more effective if you implement these healthy lifestyle changes.

For men searching for ED medication, there are many newer erectile dysfunction treatments (all prescription drugs) that help you get an erection firm enough to have sex, and most have few side effects.

Commonly prescribed erectile dysfunction treatments include:

How can I treat erectile dysfunction?

You can work with a health care professional to treat an underlying cause of your erectile dysfunction (ED). Choosing an ED treatment is a personal decision. However, you also may benefit from talking with your partner about which treatment is best for you as a couple.

Lifestyle changes

Your health care professional may suggest that you make lifestyle changes to help reduce or improve ED. You can

You can seek help from a health professional if you have trouble making these changes on your own.

Go to counseling

Talk with your doctor about going to a counselor if psychological or emotional issues are affecting your ED. A counselor can teach you how to lower your anxiety or stress related to sex. Your counselor may suggest that you bring your partner to counseling sessions to learn how to support you. As you work on relieving your anxiety or stress, a doctor can focus on treating the physical causes of ED.

How do doctors treat erectile dysfunction?

Change your medicines

If a medicine you need for another health condition is causing ED, your doctor may suggest a different dose or different medicine. Never stop taking a medicine without speaking with your doctor first. Read about which medicines make it more likely that you’ll develop ED.

Prescribe medicines you take by mouth

A health care professional may prescribe you an oral medicine, or medicine you take by mouth, such as one of the following, to help you get and maintain an erection:

All of these medicines work by relaxing smooth muscles and increasing blood flow in the penis during sexual stimulation. You should not take any of these medicines to treat ED if you are taking nitrates to treat a heart condition. Nitrates widen and relax your blood vessels. The combination can lead to a sudden drop in blood pressure, which may cause you to become faint or dizzy, or fall, leading to possible injuries.

Also talk to your health care professional if you are taking alpha-blockers to treat prostate enlargement. The combination of alpha-blockers and ED medicines also could cause a sudden drop in blood pressure.

A health care professional may prescribe testosterone if you have low levels of this hormone in your blood. Although taking testosterone may help your ED, it is often unhelpful if your ED is caused by circulatory or nerve problems. Taking testosterone also may lead to side effects, including a high red blood cell count and problems urinating.

Testosterone treatment also has not been proven to help ED associated with age-related or late-onset hypogonadism. Do not take testosterone therapy that hasn’t been prescribed by your doctor. Testosterone therapy can affect how your other medicines work and can cause serious side effects.

A health care professional may prescribe you an oral medicine to help you get and maintain an erection.

Prescribe injectable medicines and suppositories

Many men get stronger erections by injecting a medicine called alprostadil into the penis, causing it to become filled with blood. Oral medicines can improve your response to sexual stimulation, but they do not trigger an automatic erection like injectable medicines do.

Instead of injecting a medicine, some men insert a suppository of alprostadil into the urethra. A suppository is a solid piece of medicine that you insert into your body where it dissolves. A health care professional will prescribe a prefilled applicator for you to insert the pellet about an inch into your urethra. An erection will begin within 8 to 10 minutes and may last 30 to 60 minutes.

Discuss alternative medicines

Some men say certain alternative medicines taken by mouth can help them get and maintain an erection. However, not all “natural” medicines or supplements are safe. Combinations of certain prescribed and alternative medicines could cause major health problems. To help ensure coordinated and safe care, discuss your use of alternative medicines, including use of vitamin and mineral supplements, with a health care professional. Also, never order a medicine online without talking with your doctor.

How will side effects of erectile dysfunction medicines affect me?

ED medicines that you take by mouth, through an injection, or as a pellet in the urethra can have side effects, including a lasting erection known as priapism. Call a health care professional right away if an erection lasts 4 hours or longer.

A small number of men have vision or hearing loss after taking oral ED medicines. Call your health care professional right away if you develop these problems.

Prescribe a vacuum device

A vacuum device causes an erection by pulling blood into the penis. The device has three parts:

  • a plastic tube, which you put around your penis
  • a pump, which draws air out of the tube, creating a vacuum
  • an elastic ring, which you move from the end of the tube to the base of your penis as you remove the tube

The elastic ring maintains the erection during intercourse by preventing blood from flowing back into your body. The elastic ring can remain in place up to 30 minutes. Remove the ring after that time to bring back normal circulation and to prevent skin irritation.

You may find that using a vacuum device requires some practice or adjustment. Using the device may make your penis feel cold or numb and have a purple color. You also may have bruising on your penis. However, the bruises are most often painless and disappear in a few days. Vacuum devices may weaken ejaculation but, in most cases, the devices do not affect the pleasure of climax, or orgasm.

Recommend Surgery

For most men, surgery should be a last resort. Talk with your doctor about whether surgery is right for you. A urologist performs surgery at a surgical center or hospital to

  • implant a device to make the penis erect
  • rebuild arteries to increase blood flow to the penis

Implanted devices. Implanted devices, known as prostheses, can help many men with ED have an erection. Implants are typically placed by a urologist. The two types of devices are

  • inflatable implants, which make your penis longer and wider using a pump in the scrotum
  • malleable implants, which are rods that allow you to manually adjust the position of your penis

You usually can leave the hospital the day of or day after the surgery. You should be able to use the implant 4 to 6 weeks after the surgery.

Once you have either implant, you must use the device to get an erection. Possible problems with implants include breaking and infection.

Artery reconstruction. Surgery to repair arteries can reverse ED caused by blockages that stop blood flow to the penis. Usually men younger than 30 are the best candidates for this type of surgery.

Erectile Dysfunction (ED): Causes, Diagnosis & Treatment

Overview

Dr. Drogo Montague discusses treatment options for erectile dysfunction.

What is erectile dysfunction (ED)?

Erectile dysfunction (ED) is the inability to get and keep an erection firm enough for sexual intercourse. Estimates suggest that one of every 10 men will suffer from ED at some point during his lifetime. It is important to understand that in most cases, ED is a symptom of another, underlying problem. ED is not considered normal at any age, and may be associated with other problems that interfere with sexual intercourse, such as lack of desire and problems with orgasm and ejaculation.

How common is erectile dysfunction?

Approximately one in 10 adult males will suffer from ED on a long-term basis.

Many men do experience occasional failure to achieve erection, which can occur for a variety of reasons, such as drinking too much alcohol, stress, relationship problems, or from being extremely tired.

The failure to get an erection less than 20% of the time is not unusual and typically does not require treatment. However, the failure to achieve an erection more than 50% of the time generally means that there is a problem and treatment is needed.

ED does not have to be a part of getting older. While it is true that some older men may need more stimulation, they should still be able to achieve an erection and enjoy intercourse.

Symptoms and Causes

What causes erectile dysfunction (ED)?

ED can be caused by a number of factors, including:

  • Vascular disease: Blood supply to the penis can become blocked or narrowed as a result of vascular disease such as atherosclerosis (hardening of the arteries).
  • Neurological disorders (such as multiple sclerosis): Nerves that send impulses to the penis can become damaged from stroke, diabetes, or other causes.
  • Psychological states: These include stress, depression, lack of stimulus from the brain and performance anxiety.
  • Trauma: An injury could contribute to symptoms of ED.

Chronic illness, certain medications, and a condition called Peyronie’s disease can also cause ED. Operations for the prostate, bladder, and colon cancer may also be contributing factors.

What medications could cause erectile dysfunction (ED)?

Erectile dysfunction (ED) is a common side effect of a number of prescription drugs. While these medications may treat a disease or condition, in doing so they can affect a man’s hormones, nerves or blood circulation, resulting in ED or increasing the risk of ED.

If you experience ED and think that it may be a result of the medication you are using, do not stop taking the medication. If the problem persists, contact your doctor and he or she may be able to prescribe a different medication. Common medications that may list ED as a potential side effect include:

  • Diuretics (pills that cause increase urine flow).
  • Antihypertensives (high blood pressure drugs).
  • Antihistamines.
  • Antidepressants.
  • Parkinson’s disease drugs.
  • Antiarrhythmics (drug for irregular heart action).
  • Tranquilizers.
  • Muscle relaxants.
  • Nonsteroidal anti-inflammatory drugs.
  • Histamine h3-receptor antagonists.
  • Hormones.
  • Chemotherapy medications.
  • Prostate cancer drugs.
  • Anti-seizure medications.

Other substances or drugs that can cause or lead to ED include these recreational and frequently abused drugs:

  • Alcohol.
  • Amphetamines.
  • Barbiturates.
  • Cocaine.
  • Marijuana.
  • Methadone.
  • Nicotine.
  • Opiates.

These drugs not only affect and often suppress the central nervous system, but can also cause serious damage to the blood vessels, leading to permanent ED.

What prescription drugs may cause erectile dysfunction?

Erectile dysfunction (ED) is a common side effect of a number of prescription drugs. While these medications may treat a disease or condition, in doing so they can affect a man’s hormones, nerves or blood circulation. The result may be ED or an increase in the risk of ED.

If you have ED and think that it may be a result of the medication you are using, do not stop taking the medication. If the problem persists, contact your doctor and he or she may be able to prescribe a different medication. Common medications that may list ED as a potential side effect include:

  • Alcohol.
  • Amphetamines.
  • Barbiturates.
  • Cocaine.
  • Marijuana.
  • Methadone.
  • Nicotine.
  • Opiates.

Aside from the well-known complications that the use and abuse of these drugs can cause, ED is not often mentioned. However, use of these drugs is a risk factor for ED. These drugs not only affect and often times slow down the central nervous system, but can also cause serious damage to the blood vessels, leading to permanent ED.

How are depression and erectile dysfunction related?

For some men, depression can accompany the condition of erectile dysfunction (ED). It is common for men with ED to feel angry, frustrated, sad, unsure of themselves, or even less “manly.” Such feelings may lead to a lack of self-esteem and, in severe cases, to depression.

Depression that accompanies ED is treatable. The first step in addressing your concerns about ED-related depression is to be honest with yourself, your partner, and your doctor. After depression has been brought out into the open, coping with it will be easier and less stressful.

Diagnosis and Tests

How is erectile dysfunction (ED) diagnosed?

Because there are a variety of causes for ED, there are several different tests your doctor may use to diagnose the condition and determine its cause. Only after the cause of ED is determined can it be effectively treated.

Before ordering any tests, your doctor will review your medical history and perform a thorough physical examination. The doctor will also “interview” you about your personal and sexual history. Some of these questions will be very personal and may feel intrusive. However, it is important that you answer these questions honestly. The questions asked may include:

  • What medications or drugs are you currently using? This includes prescription drugs, over-the-counter drugs, herbals, dietary supplements and illegal drugs.
  • Have you had any psychological problems such as stress, anxiety and depression?
  • When did you first notice symptoms of ED?
  • What are the frequency, quality and duration of any erections you have had?
  • What are the specifics of the circumstances under which ED first occurred?
  • Do/did you experience erections at night or during the morning?
  • What sexual techniques do you use?
  • Are there problems in your current relationship?

The doctor may also wish to interview your sexual partner since your partner may be able to offer in sight about the underlying causes.

After your physical examination and discussion, your doctor may then order any one of the following tests to further diagnose your condition:

  • Complete blood count (CBC): This is a set of blood tests that, among other things, can detect the presence of anemia. Anemia is caused by a low red blood cell count and can cause fatigue, which in turn can cause ED.
  • Liver and kidney function tests: These blood tests may indicate whether ED may be due to your kidneys or liver functioning improperly.
  • Lipid profile: This blood test measures the level of lipids (fats), like cholesterol. High levels may indicate atherosclerosis (hardening of the arteries), which can affect blood circulation in the penis.
  • Thyroid function test: One of the thyroid hormones’ functions is to regulate the production of sex hormones, and a deficiency in these hormones may contribute to or cause ED.
  • Blood hormone studies: Testosterone and/or prolactin levels in the blood may be measured to see if abnormalities in either of these sex hormones are present.
  • Urinalysis: Analysis of urine can provide a wealth of information, including information on protein, sugar and testosterone levels. Abnormal measurements of these substances can indicate diabetes, kidney disease or a testosterone deficiency, all which can cause ED.
  • Duplex ultrasound: This is perhaps the best test for evaluating ED. An ultrasound uses high-frequency sound waves to take “pictures” of the body’s tissues. For people with ED, an ultrasound may be used to evaluate blood flow and check for signs of a venous leak, atherosclerosis (hardening of arteries) or tissue scarring. This test is performed both while the penis is erect (usually induced by an injection of a drug that stimulates erection) and also while it is soft.
  • Bulbocavernosus reflex: This test evaluates nerve sensation in the penis. During the test, your doctor will squeeze the head of your penis, which should immediately cause your anus to contract. If nerve function is abnormal, there will be a delay in response time.
  • Nocturnal penile tumescence (NPT): This test measures a man’s erectile function while he is sleeping. Normally, a man will have five or six erections while asleep. A lack of these erections may indicate there is a problem with nerve function or circulation to the penis. The test uses two methods, the snap gauge method and the strain gauge method. The snap gauge method is performed by wrapping three plastic bands of varying strength around the penis. Erectile function is then measured based on which of the three bands breaks. The strain gauge method works by placing elastic bands around the tip and base of the penis. If the penis becomes erect during the night, the bands stretch, measuring the changes in penile circumference.
  • Penile biothesiometry: This test involves the use of electromagnetic vibration to determine sensitivity and nerve function. A decreased sensitivity to these vibrations may indicate nerve damage.
  • Vasoactive injection: During this test, an erection is produced by injecting special solutions that cause the blood vessels to dilate (enlarge) allowing blood to enter the penis.
  • Dynamic infusion cavernosometry: This test is used for men with ED who have a venous leak. During this test, fluid is pumped into the penis at a predetermined rate. By measuring the rate at which fluid must be pumped to attain a rigid erection, doctors can determine the severity of the venous leak.
  • Cavernosography: Used in conjunction with the dynamic infusion cavernosometry, this test involves injecting a dye into the penis. The penis is then X-rayed so that the venous leak can be seen.
  • Arteriography: This test is given to people who are candidates for vascular reconstructive surgery. A dye is injected into the artery believed to be damaged and X-rays will be taken.

Before you are given any of these tests, your doctor will explain what is involved. If you have any questions, do not hesitate to ask your doctor.

Management and Treatment

What doctors treat erectile dysfunction?

The type of medical specialist who treats ED will depend on the cause of the problem. Based on your family’s medical history, as well as your own medical history and current health, your doctor may treat you with oral medications (Viagra®, Levitra®, Cialis®).

If these options fail, you may be referred to a urologist who can assist with other non-surgical options such as vacuum device or injections or surgical treatment options. If needed, your doctor may also refer you to a psychologist specializing in sexual dysfunction.

How is erectile dysfunction treated?

ED can be treated in many ways, including:

  • Oral medications.
  • Sex therapy.
  • Penile injections.
  • Vacuum devices.
  • Intraurethral medication.
  • Surgery (penile implant).

Each type has its own pros and cons. Discuss your options with your doctor to determine the best treatment for you.

The first step to treating ED is to find the underlying cause. Then the appropriate treatment can begin. There are a number of non-surgical and surgical options that can help a man regain normal sexual function.

What non-surgical treatments are there for erectile dysfunction (ED)?

Education and communication

Education about sex, sexual behaviors, and sexual responses may help a man overcome his anxieties about sexual dysfunction.

Talking honestly with your partner about your needs and concerns may also help to overcome many barriers to a healthy sex life.

Medication

Medications such as sildenafil (Viagra®), vardenafil (Levitra®), or tadalafil (Cialis®) may help improve sexual function in men by increasing blood flow to the penis. Men who are on medicines that contain nitrates such as nitroglycerine should not take oral ED medications. The combination of nitrates and these specific medications can cause low blood pressure (hypotension).

The most common side effects of these medications are indigestion, nasal congestion, flushing, headaches and a temporary visual disturbance.

Mechanical aids

Aids such as vacuum devices and penile constriction rings serve as erectile aids for some men.

A vacuum constriction device (above) is a cylinder that is placed over the penis. The air is pumped out of the cylinder, which draws blood into the penis and causes an erection. The erection is maintained by slipping a band off of the base of the cylinder and onto the base of the penis. The band can stay in place for up to 30 minutes. The vacuum device can be safely used to treat most causes of erectile failure. Lack of spontaneity, discomfort, and cumbersomeness of the device seem to be the biggest concerns of patients.

Penile injection therapy (intracavernosal injection therapy)

Men are taught how to inject medications directly into the erection chambers of the penis to create an erection. Injection therapy is effective in treating a wide variety of erection issues caused by blood vessel, nerve and psychological conditions.

Using a tiny needle and syringe, the man injects a small amount of medicine into the side of his penis. The medicine relaxes the blood vessels, allowing blood to flow into the penis. This treatment has been widely used and accepted since the early 1980s. The three most common medicines are prostaglandin E1 (alprostadil), papaverine (Papacon®), and phentolamine (Regitine®).

The most common side effects are pain and penile scarring (fibrosis). In extremely rare cases, patients with cerebral and vascular disease or severe cardiovascular diseases might not be able to tolerate the dizziness and high blood pressure occasionally caused by injection therapy.

A painful erection that lasts longer than two to three hours is called priapism and may occur with injection therapy. This can be lessened with proper dosing and by following the treatment guidelines.

Psychology and sex therapies

Psychological causes may contribute to erectile failure even when there is a clear organic cause.

Therapy with a trained counselor can help a person address feelings of anxiety, fear or guilt that may have an impact on sexual dysfunction.

Sex therapy can be beneficial to most men when counseling is provided by a skilled sex therapist. Sex therapy also helps a man’s partner accept and cope with the problems.

A patient whose ED has a clear psychological cause should receive sex therapy counseling before any invasive treatments are pursued.

Hormone

Low hormone levels may play a role in ED. Hormone replacement in the form of topical gels, creams, patches, injections and pellets are only used after physician evaluation.

What are surgical treatment options for erectile dysfunction (ED)?

Penile prosthesis surgery

Inflatable penile prostheses are implanted during outpatient surgery. Once they are part of a man’s body, they enable him to have an erection whenever he desires. The use of a prosthesis preserves penile sensation, orgasm and ejaculation for most men.

The most commonly used penile implant consists of a pair of inflatable cylinders that are surgically implanted in the erection chambers of the penis. The cylinders are connected through tubing to a reservoir of fluid under the lower abdominal muscles, and to a pump inside the scrotal sac.

To inflate the penile prosthesis, the man compresses the pump a number of times to transfer fluid from the reservoir to the cylinders. This causes the penis to become erect. When inflated, the prosthesis makes the penis stiff and thick, which is very similar to a natural erection.

A penile prosthesis does not change the sensation on the skin of the penis or a man’s ability to achieve orgasm or ejaculate. Pressing on a deflation valve attached to the pump returns the fluid to the reservoir, which returns the penis to a flaccid state.

The surgical procedure is performed through one or two small incisions that are generally well hidden. Other people will be unable to tell that a man has an inflatable penile prosthesis. Complications following surgery are not common, but primarily include infection and mechanical device failure.

Approximately 95% of penile implant surgeries are successful in producing erections that enable men to have sexual intercourse. Moreover, patient satisfaction questionnaires show that up to 90% of men who have undergone penile implants say they would choose the surgery again, and overall satisfaction ratings are higher than those reported by men using oral medication or penile injection therapy.

Prevention

Can erectile dysfunction (ED) be prevented?

For people who are at risk of developing ED due to personal behavior, steps may be taken to try to prevent its occurrence. However, other causes may not be preventable.

A number of studies now suggest a link between ED and obesity, high cholesterol, hypertension, diabetes and heart disease.

The following recommendations may help prevent ED or improve the problem if it is already present:

  • Eat a healthy diet. A diet that limits saturated fat intake and includes several portions of fruits, vegetables and whole grains can benefit men with ED.
  • Reduce cholesterol. High cholesterol can harden, narrow or block the arteries (atherosclerosis) leading to the penis. Men can lower cholesterol through diet, exercise and medication.
  • Maintain a healthy weight.
  • Exercise regularly. Regular exercise may reduce the risk of ED. Choose exercises that you enjoy and will make a regular part of your day. In addition to reducing the risk of ED, exercise also can help you manage stress. Check with your doctor before starting any exercise program.

Treating Erectile Dysfunction: 5 Options Beyond ED Pills

TIPS TO LIVE BY

June 15, 2021

What happens in the bedroom typically stays in the bedroom, with one major exception being when things aren’t going well in there.

“In terms of sexual health, erectile dysfunction (ED) is the most common problem men report to their doctor,” says Dr. Nathan Starke, urologist specializing in men’s health at Houston Methodist. “Most often, men are prescribed an oral medication such as ‘the blue pill’ or one of its closely related alternatives.”

But treating ED doesn’t have to begin and end with oral medications — which can come with unwanted side effects. In addition, it’s unsafe for some men to take these medications, including those who have severe heart disease/heart failure or low blood pressure, as well as those who take nitrate drugs to treat chest pain. And, finally, these medications often fail to work in some men altogether.

“Fortunately, there are several other ways to treat ED,” adds Dr. Starke.

5 ways to treat ED that eliminate the need to take a pill

For men who don’t want to take a pill or for those who simply can’t, Dr. Starke is here to explain your options.

1. VED (Vacuum Erection Device)

Often called a penile pump, a VED works by manually pulling blood into your penis using suction. Once the erection is formed, a tension ring is slipped to the base of the penis, which helps maintain an erection for about 30 minutes.

“While many men do not love the concept of suction and ring compression to get and maintain erections, it is a reliable option that works for many couples to restore erection function without medication,” says Dr. Starke.

Side effects of using a penile pump include:

  • Mild bruising
  • Restriction of ejaculation
  • The penis feeling cold to the touch when used for an extended period

2. Testosterone replacement

“ED is commonly caused by temporary stress or chronic cardiovascular conditions that reduce blood flow to the penis. But, low testosterone can contribute to issues with forming and maintaining an erection,” says Dr. Starke. “Your doctor can help you understand if your testosterone levels are low and whether starting testosterone treatment may help alleviate your ED symptoms.”

Though testosterone replacement is not recommended as a first-line, single treatment for ED, your doctor may recommend it or try it in combination with another ED treatment option, depending on your individual symptom profile.

3. Urethral suppository

Intraurethral therapy involves inserting a small drug pellet into the tip of the penis.

“With this therapy, it only takes about 10 minutes for an erection to form — which can then last anywhere from 30 to 60 minutes,” says Dr. Starke. “However, side effects of using a urethral suppository can include a burning sensation or very minor bleeding from the tip of the penis.”

4. Penile injections

“Though a universally scary concept, penile injections involve a medicine that improves penile blood flow much better than the oral alternatives. The medicine is injected at the base of the penis using a very small needle. This is a self-injection, of course, although we routinely teach patients proper technique and perform the first injection in the clinic,” says Dr. Starke.

Side effects of this treatment, include:

  • Minor pain or bleeding at the injection site
  • Prolonged erection that can last longer than desired
  • Formation of scar tissue within the deeper tissues of the penis, primarily seen in long-term users of this therapy

5.

Penile implant

A penile implant involves a 30- to 45-minute surgical procedure that places a water-based device into the erection chambers of the penis, which enables you to reliably control exactly when and how long an erection lasts. Dr. Starke says this is the closest option to a cure that exists for ED.

“A penile implant does involve a short surgical procedure, so whether or not you’re willing or able to undergo the procedure can be a determining factor in whether it’s the right option for you,” explains Dr. Starke. “For men who do choose an implant, the vast majority (greater than 90% of men and their partners) report great satisfaction with it.”

Addressing underlying health conditions and minimizing stress can also reduce symptoms of ED

“ED isn’t always caused by increasing age, and it rarely occurs alone. In fact, it’s often linked to — and even caused by — one or more underlying health conditions that a man may not even know he has,” says Dr. Starke. “In particular, high blood pressure, heart disease and anxiety or depression can contribute to the symptoms of ED.”

Aside from the various treatment options for ED, Dr. Starke points out that simply taking steps to better your heart health and mental health may help alleviate your symptoms.

Impotence treatments – MyDr.com.au

Impotence, also known as erectile dysfunction or ED, is a very common problem, affecting up to half of 40-70 year old men in Australia.

Treatment techniques for impotence have varied through the years, from external steel mechanical attachments, to static electricity attached to the penis and testicles, to simple aphrodisiacs such as oysters. Until as recently as 1970, erectile failure was almost always seen as being due to psychological causes and was usually treated with psychotherapy.

Since then, the medical causes contributing to impotence have been recognised and the treatment of impotence has been revolutionised, providing a range of options which are far more acceptable and very much more successful.

Treatment options for impotence

Treatment choices for erectile dysfunction include:

  • medicines;
  • self-injection therapy;
  • devices such as vacuum pumps;
  • penile implant surgery;
  • hormone therapy; and
  • counselling.

If you have erectile dysfunction, the treatment your doctor recommends will depend on the severity of symptoms and the underlying cause of your impotence.

Your doctor will want to check that any conditions that could be contributing to or causing erectile dysfunction are being treated.

Your doctor may also suggest that you make some lifestyle adjustments, such as:

  • increasing the amount of physical activity you get;
  • losing weight if you are overweight;
  • reducing the amount of alcohol you drink;
  • quitting smoking; and
  • not taking illicit drugs.

These lifestyle recommendations can help improve impotence related to several causes and improve your health in general.

Medicines for erectile dysfunction

The first tablet available for erectile dysfunction, sildenafil (brand name Viagra), has been largely responsible for helping to bring the topic of erectile dysfunction out into the open. Similar medications — tadalafil (Cialis) and vardenafil ( Levitra) — are also available. These medicines all work in a similar way, although there is some difference in how long their effect lasts. Sildenafil, tadalafil and vardenafil belong to a group of medicines called phosphodiesterase type 5 (PDE5) inhibitors because they block the PDE5 enzyme.

How do oral medicines help treat erectile dysfunction?

PDE5 inhibitors help in the process of getting and keeping an erection by working on chemicals in the body that are involved in erections. These medicines work by stopping PDE5 from breaking down an erection-producing chemical called cyclic guanosine monophosphate (cGMP). cGMP helps to relax the smooth muscle cells in the penis’s erectile tissue, allowing more blood to flow into the penis to cause an erection. When PDE5 is temporarily blocked by these medicines, it can’t break down the erection producing cGMP, so an erection can be achieved and maintained. PDE5 inhibitors can be used in the treatment of erectile dysfunction that is due to physical or psychological causes.

Medications such as Viagra, Cialis and Levitra will work only if you are sexually stimulated. They are not aphrodisiacs and won’t increase your sex drive.

Side effects of PDE5 inhibitors

Side effects of these medicines can include headaches, flushes, blocked nose, indigestion and dizziness.

In rare situations, sildenafil and vardenafil can cause a distortion of vision or change in colour vision.

Tadalafil has been associated with back pain.

Who can take medicines for impotence?

PDE5 inhibitors cannot be taken by all men, so your doctor will need to evaluate your suitability before prescribing either of these medications.

Men taking nitrates (often used to treat angina) should never take phosphodiesterase type 5 (PDE5) inhibitors. PDE5 inhibitors should also not be taken with some medicines used to treat high blood pressure.

PDE5 inhibitors may also not be suitable for men with certain heart conditions or low blood pressure. Check with your doctor to find out if this type of medication may be suitable for you.

Self-injection therapy

Self-injection therapy delivers a medicine called alprostadil (brand name Caverject), also known as prostaglandin E-1, to the erectile tissue of the penis. Prostaglandin E-1 occurs naturally in the body and helps increase the blood flow to the penis to cause an erection. Unlike the PDE5 inhibitors, alprostadil will cause an erection whether the penis is stimulated or not.

Self-injection therapy is usually recommended if PDE5 inhibitor medicines are not suitable or have not been effective in the treatment of erectile dysfunction.

How to use self-injection therapy

Alprostadil is injected into either of the 2 cigar-shaped chambers of the penis known as the corpora cavernosa, which run along the length of the penis, one on either side. Your doctor or urologist (specialist in problems with male reproductive organs and the urinary tract) will give you instructions on how to do this.

Alprostadil should produce an erection in 5 to 20 minutes and, generally, the erection will last for 30 to 60 minutes.

You should not use alprostadil more than once in a 24-hour period, and you should use it no more than 3 times a week.

Don’t try to use more than the recommended dose of alprostadil, as your erection may last longer than is medically safe.

Who can use self-injection therapy?

You should ask your doctor if alprostadil is suitable for you. Your doctor will also be able to tell you how much alprostadil to use, depending on your condition and whether or not you are taking any other medications, and also how to use alprostadil properly.

People with certain illnesses, such as leukaemia and sickle cell anaemia, or who have a penile implant or Peyronie’s disease, where the penis may be scarred and produces erections that are not straight, should not use alprostadil.

Men for whom sexual activity is not advised should not use alprostadil.

Side effects of injection therapy

The most common side effects of alprostadil include pain in the penis or bruising in the penis at the site of injection. Fibrosis (the development of fibrous tissue) can also develop following injections into the penis.

The most serious side effect is priapism (a persistent erection), which is a medical emergency. Your doctor will inform you of what to do if you have an erection that persists for 2 hours or more. It is very important that you follow your doctor’s instructions and inform them that you have experienced this side effect.

Vacuum erection devices

Vacuum erection devices work by creating a vacuum, which increases blood flow to the penis, producing an erection.

The penis is lubricated and placed inside a hollow plastic chamber. Air is pumped out of the chamber, either manually or by a battery powered pump. This creates a vacuum which pulls blood into the penis to cause an erection. This takes about 5 minutes.

Once the penis is erect, the man fits a rubber ring around the base of his penis to keep the blood trapped inside the penis when the cylinder is removed. After intercourse, the ring can be removed to return the penis to a limp state.

Vacuum erection devices avoid surgery and can be used as often as required. However, they may be difficult to use, and many men and their partners feel they take much of the pleasure and spontaneity away from sexual activities. Vacuum pumps are not suitable for men who have problems with blood clotting, or blood disorders such as leukaemia.

Penile implant surgery for impotence

Penile implant surgery is not a common procedure but in some cases it may be the most appropriate treatment for erectile dysfunction.

The procedure involves placing an implant inside the penis, along its length, so that it can become erect. The implant may be a pair of semi-rigid rods or a pair of inflatable cylinders.

The inflatable implants allow the penis to look and feel limp (flaccid) or erect, depending on how much the cylinders are inflated. The cylinders in an inflatable implant are hollow, and the man gets an erection by squeezing a pump located in his scrotum to fill the cylinders with salt water (saline) stored in a reservoir implanted in his lower abdomen. A release valve drains the saline out of the cylinders and back into the reservoir.

With the semi-rigid, malleable rod type of implant, the rods run along the length of the penis and can be bent upwards to produce an erect penis, or downwards when an erect penis is not required.

Like all surgery, there are some risks, such as infection or bleeding. If you have had surgery and have severe pain, fever, swelling or excessive bleeding, you should contact your doctor as soon as possible.

Vascular surgery for erectile dysfunction

In cases where a man’s anatomy prevents blood flow into or out of the penis, vascular surgery may be an option. This treatment option is rarely recommended, and is usually only successful in younger men.

If there is a blockage that prevents blood from flowing into the penis, a doctor may recommend an operation that bypasses the blocked blood vessels, using a length of vein or manufactured tubing, to allow more blood to flow into the penis and help produce an erection.

If the problem is that blood leaks back out of the penis, this can be corrected by tying off the major veins that drain the penis, a procedure known as venous ligation.

Hormone treatments for impotence

In a small number of men, blood tests may show abnormally low levels of testosterone, the male sex hormone. In such cases your doctor might prescribe a course of testosterone injections or a testosterone implant. The supplements can help boost sex drive as well as increasing the ability to have erections. Testosterone gel or patches, applied daily to the skin, are another option.

Complementary medicines for erectile dysfunction

There is a lack of scientific evidence supporting the effectiveness of complementary therapies for the treatment of impotence.

Always check with your doctor before taking any herbal medicines or supplements for impotence. These formulations may contain ingredients that can interact with other medicines or cause dangerous side effects.

Counselling for men with impotence

Erectile dysfunction often has physical causes, but sometimes there is a psychological basis for erection problems. Often this is a form of performance anxiety. A man may have had an episode of erectile dysfunction due to some passing cause like fatigue, stress, relationship difficulty or intoxication. This may have led to embarrassment or a feeling of failure. Even if the physical cause does not remain, future attempts to have sex may trigger memories of this embarrassment and acute anxiety that it will happen again. This anxiety itself is capable of causing erectile dysfunction, and so a man may get trapped in a self-reinforcing cycle of anxiety and erectile dysfunction. In these instances, seeing a GP, counsellor or psychologist can be very helpful.

Stress, anxiety, depression and low self-esteem, in fact, almost all significant emotional problems, can have a major effect on sexuality. So do many chronic physical illnesses, even if they don’t directly affect genital function. Counsellors and psychologists can assist with these and a wide range of other sexual and relationship problems and can also help female partners suffering from sexual problems. They are particularly skilled in helping patients to overcome guilt or anxiety relating to sexual abuse, and in helping couples to sort out relationship difficulties. Simple problems can be dealt with in a few visits, but more complex problems may require several months or even years of therapy.

Your doctor may be able to recommend a psychologist or counsellor who specialises in sexual and relationship problems.

City Clinical Hospital №31 – Is impotence treated?

Weakness of the stronger sex

70% of all male failures in bed are by no means organic, but purely psychological reasons. But even if the origins of this innermost problem lie only in the physiology of a man, then not everything is hopeless.

Since 1988, the offensive term “impotence” has been renamed into a scientific, and therefore neutral definition – erectile dysfunction (ED). Nevertheless, the essence of the phenomenon, of course, has not changed.Let’s define in terms: ED is defined as the inability of a man to achieve or maintain an erection sufficient for normal intercourse. Unlike previous definitions, this is a more “vague” term, which is associated with differences in methods and diagnostic criteria, age composition, etc. ED has many underlying causes and in most cases is just a symptom of other conditions. Diabetes mellitus, arterial hypertension, damage to the cardiovascular system, damage to the cardiovascular system, smoking is determined in almost 80% of cases of ED.Harmonious disorders – testosterone deficiency – in 30%.

There is no norm in sex

ED can be temporary and permanent. For a week or two, prolonged stress, past illnesses, overwork, etc. can disrupt an erection. Persistent inability to fulfill marital duties (more than several months) requires immediate examination. However, most often doctors have to deal with pseudo impotence, the causes of which have nothing to do with physiology, but are hidden deep in the male psyche. It is known that representatives of the stronger sex react extremely painfully to the slightest failures in sex. Just a couple of indelicate remarks are enough – and a healthy young man in the prime of years runs the risk of becoming a complete “impotent”. And the norm in such a delicate area is so conditional that it is absolutely impossible to give specific “correct” figures and indicators. It all depends on age, temperament, individual characteristics, field of activity, life circumstances, personal relationships of a person, etc.d.

In our country, there is still no culture of referring to doctors (narrow specialists). Often men solve intimate problems on the basis of the recommendations of the pharmacy workers, the advice of friends, the recommendations of psychiatrists, venereologists, etc. You need to contact an urologist-andrologist.

Psychogenic ED can be treated more effectively if you work not with one “victim”, but with a couple as a whole. In addition to psychotherapy, modern drugs are used, the effectiveness of which is much higher than that of the drugs of the previous generation.

If there is only a diagnosis

It is not uncommon for a man to have a morning erection with psychogenic ED and there are no problems with masturbation. When psychology has nothing to do with it, there is no normal erection under any circumstances. And then in men’s health you need to understand in detail.

Among the organic causes of erectile dysfunction, the most common is vascular pathology: either a weak flow of arterial blood to the penis, or blood stagnation. In the first case, an erection is achieved with great difficulty, and its quality is poor. In the second version, on the contrary, the male organ quickly comes into a state of full combat readiness, but this state ends so quickly that sometimes the would-be lover does not have time to get down to business. Such violations can occur when exposed to occupational hazards, for example, among drivers or households – irregular working hours, systematic overheating, hypothermia, high frequency currents, vibrations, quality of power. With diseases: endarteritis, atherosclerosis of large arteries, varicose veins, as well as as a result of injuries of the genitals and nearby tissues.

Patients and their spouses should be aware of the negative impact of forced interruptions in sexual activity on the well-being of men and women in general.

Doppler ultrasound is used for diagnostics. The man is handed an erotic magazine, with which he retires in a separate room, then with sensors attached to the penis, his blood vessels are measured.Revealed vascular pathologies are treated with surgery or medication.

Do not drop your dignity!

Among the ailments associated with erectile dysfunction, the most frequent are hypertension and diabetes mellitus. As well as neurological and endocrine diseases. Androgens (from Greek male + produce) i.e. male reproductive organs are produced in the testes and adrenal cortex. Testosterone is most active. It affects physical, sexual and emotional performance. A decrease in its concentration in the blood in men can lead to certain clinical manifestations, such as a decrease in sexual activity and performance.

To diagnose these disorders, it is necessary to pass tests for hormones and, if necessary, conduct an MRI of the brain. But chronic prostatitis, contrary to popular belief, does not in itself lead to impotence, but causes such severe pain that men lose the desire for intimacy.

Most of the problems leading to a worsening or even lack of erection can be solved with adequate treatment. But there are other pathologies, such as sclerosis of the corpora cavernosa of the penis. In this disease, the delicate elastic tissue of the cavernous bodies of the genital organ is replaced by coarse connective tissue that is unable to provide an erection. The reasons are trauma to the penis, repeated injections into the organ and even too zealous, many hours of love exercises, in which a man tries with all his might to delay ejaculation as long as possible. Sex giants, keep in mind that everything is good in moderation!

The best prevention of impotence is a healthy lifestyle, sports, careful attitude to alcohol and other harmful substances, regular sex life without prolonged abstinence and sexual excesses.

Love until the grave

Some drugs can interfere with erection. The effect of “contrasex” is possessed by antihypertensive drugs, but also some quite harmless drugs, for example, antihistamines – psychotropic, say, the well-known diphenhydramine. Therefore, it should not be drunk for more than 5 days.

Also, do not self-medicate with drugs that improve male strength. Overdose leads to general depletion of the body. Cores should be especially careful: taking Viagra together with nitroglycerin threatens them with sad consequences.

The accumulated experience in the treatment of ED, the rational use of drugs and technologies, allows many patients to restore sexual activity, the quality of erections with a minimum risk of side effects of treatment.

  3. Publications

Surely, this question tormented more than one generation of men, each of whom sooner or later faced problems in the intimate sphere. We tried to find the answer to it together with the chief physician of the clinic “Health Academy”, urologist-andrologist Alexander Shaptilay.

– In medical language, the inability of a man to achieve and maintain an erection is called “erectile dysfunction.” And it is quite common today. Suffice it to say that about four out of ten men of sexually active age – from 20 to 50 years old – face this problem to a greater or lesser extent, – Alexander Viktorovich cites sad statistics.

According to him, there are two main groups of causes of erectile dysfunction. It is believed that in 30–40% of cases it is of psychogenic nature, and in 60–70% it is organic. That is, behind the violation of potency is some kind of disease, having cured or treated which, you can improve the potency itself.

– For example, diseases of the circulatory system are now very common: arterial hypertension, ischemic heart disease, atherosclerosis. They, as well as diabetes and alcoholism, can cause insufficient “patency” of the arteries through which blood enters the penis in response to sexual arousal, – says the doctor. – And varicose veins or venous insufficiency of the vessels of the penis can cause an accelerated outflow of blood through the veins. A hormonal cause of erectile dysfunction is also possible: either a lack of the male hormone testosterone, or an excess of female hormones, which is possible with some severe liver or brain diseases.

In addition, according to Alexander Shaptilei, many groups of drugs have a side effect in the form of a decrease in potency – most cardiological drugs, drugs for lowering blood pressure, for the treatment of disorders of the nervous system, antiandrogens, drugs for chemotherapy. And, of course, sexually transmitted diseases, as well as problems with the prostate, affect male strength.

When the reason is in the head

Recently, in addition to the causes of organic causes of impotence, doctors have begun to deal with the mental origins of the disease.

– Today this form of pathology is very common. It often affects young people who are afraid of the first sexual intercourse. Television, video and especially the Internet provide open access to pornographic scenes, after watching which guys no longer know how to approach a live girl. They are afraid that nothing will work out – nothing will work out. But this form is well treated, – Alexander Shaptilei reassures.

But when adult men begin to lose potency due to nervousness, it is much worse. Most often this happens during a midlife crisis: hard work, daily troubles, a small salary, a crisis in the country, everything in life is bad, hormones are weakening, the wife seems old and ugly … And then there is this chronic fatigue. I came home from work all squeezed out like a lemon, tried to have sex with my wife – it didn’t work. The wife called him a “weakling”, hung labels – that’s it, the diagnosis is assured. In the future, the disintegration of the family.

– Although psychogenic factors all over the world are quite common causes of male impotence, in our country, unfortunately, their treatment is not taken seriously. I mean that psychotherapy becomes the main method of treatment in this case, and the urologist, if necessary, must solve “mechanical” problems, ”says the doctor. – Today urologists are forced to deal with such “psychotherapy”, sometimes successfully, sometimes not.But nevertheless, this form is fertile according to the forecast, because with the right approach it is well treated.

Impotence is not a sentence

Experts assure that with the current diagnostic capabilities, it will not be difficult for a competent doctor to establish the true cause of a decrease in potency. And according to the reason to prescribe treatment. If it is prostatitis, prostatitis will be treated. If this is a hormonal deficiency – a hypoandrogenic state, then the patient will receive preparations of the male hormone testosterone. If the problem is in the vessels, the patient will be prescribed vasoactive drugs, agents that selectively improve the blood flow of the penis. Impotence that has arisen against the background of a serious somatic illness – diabetes mellitus or arterial hypertension – is also impossible to eliminate without treating the underlying disease.

– Only God gives a 100% guarantee against impotence, and I will say this: with the competent approach of the doctor and the patient’s desire to help, it is almost always possible to cure it. But it is imperative to start treatment with the general normalization of the patient’s life, – says Alexander Viktorovich.

The whole truth about impotence in the words of a urologist (18+)

Medical center “Medlux” in a series of articles raises urgent health problems. The first material was devoted to the correct planning of pregnancy, the second – to the peculiarities of menopause. This time urologist Andrey Mungalov told the news agency “Chita.Ru “about the causes, symptoms and diagnosis of impotence.
Andrey Mungalov – urologist with 24 years of experience. He was the first in the Trans-Baikal Territory to use stone crushing.

In 1996 he invented an apparatus for the treatment of prostatitis, and in 2004 – a method for preserving men’s health “School of Prostatitis and Erectile Dysfunction Prevention”. In September 2014, he continued to study the hormonal aspect of andrological diseases.

The term “impotence” originated in 1655. However, sexual impotence, which he means by himself, appeared much earlier and continues to torment men to this day.Nowadays, impotence is scientifically correctly called erectile dysfunction.

The risk of developing this disorder increases with age. In men over 60, impotence occurs four times more often than in patients from 40 to 60 years old. According to medical statistics, over 30% of men aged 18–40 suffer from sexual dysfunctions to one degree or another.

Symptoms of impotence

Doctors identify six main criteria on the basis of which a man can be considered impotent:

• if he is unable to experience sexual desire;
• if he is unable to achieve a sufficient erection, although he feels sexual desire;
• if he is unable to insert the penis into the vagina after the appearance of an erection;
• if he is unable to perform frictional movements for the required time;
• if he is unable to achieve ejaculation as a result of frictional movements;
• if he is unable to experience an orgasm as a result.

Types of impotence

Impotence is of two types – organic and psychogenic.

Organic impotence is a physiological disorder. These include, for example, urological diseases, endocrine disorders, and vascular diseases. This pathology progresses slowly. Periodically, an erection does not occur, gradually failures become more frequent. With organic impotence, sexual attraction to a partner, relative erection and ejaculation during intercourse sometimes persist, but spontaneous night and morning erections are absent.

Psychogenic erectile dysfunction is a mental disorder in the form of neurosis, depression with a concomitant neurotic state. This disease occurs suddenly with the preservation of spontaneous night and morning erections. However, the problems are episodic. Treatment of this type of impotence requires the initial identification and elimination of its cause.

In many cases, impotence results from a combination of emotional and physiological factors. If a man experiences small changes in his sexual responses, for example, due to vascular insufficiency, he may be very frightened by this circumstance. The resulting anxiety can cause more serious difficulties in comparison with minor physiological changes.

A separate issue is the relationship between impotence and prostatitis. In some, prostatitis gives rise to impotence, in others, impotence gives rise to prostatitis. In still others, both diseases develop against the background of metabolic syndrome. Each case must be dealt with individually.

Diagnostics

Doctors offer a list of questions to help determine if the problem is medical or emotional.By answering them, a person will get an approximate idea of what difficulties may be associated with. Also, the answers will help the attending physician in the diagnosis.

1. Are there no erections at all or do they arise, but then disappear?

If, over the course of several years, erections gradually became more and more unstable and easily disappeared soon after onset, and now they are not observed at all, then the likely cause is a slowly progressive physical problem.

If the erection is excellent, but quickly disappears at the beginning of intercourse, then the matter may be increased anxiety about sexual performance.

If an erection largely depends on active physical stimulation and disappears immediately after its termination, then the reason is most likely physical in nature.

If an erection occurs normally, and gradually disappears during intercourse, this blood leaves the cavernous bodies of the penis, going into the systemic circulation. This process is completely normal and is not a symptom of the disease.

2. Does a man wake up with a good erection?

Normally, three or four erections should occur during a night’s sleep, lasting approximately 20 minutes. This happens during REM sleep or dreaming. If a man notes a fairly regular night or morning erection, this is a reliable sign of the safety of the erectile mechanism and its ability to function normally. Otherwise, you should be alert and continue the examination.

3. Does the occurrence of an erection depend on the situation?

Having answered this question, one can understand whether external factors or medical problems affect erectile function. After all, it is perfectly normal to experience difficulties due to fatigue, stress, or anxiety about sexual performance.

4. Does the partner help and how effective is her help?

If a full-fledged erection occurs in a man only as a result of additional stimulation provided by a partner, then he is healthy and everything is in order with his sexuality. The need for stimulation is completely normal and common in adulthood.

On the other hand, if an erection does not occur even after stimulation, this indicates either a medical problem or a high level of anxiety blocking natural sexual responses.

5. Do you get an erection during masturbation?

If you can get a good erection with the help of masturbation, this proves that everything is fine with sexual reactions. Masturbation alone creates ideal conditions for testing the performance of the reproductive system, because it eliminates unnecessary anxiety and embarrassment that arise in the presence of a partner.

If the test results are satisfactory, but the man experiences difficulties during normal intercourse, this may be due to two reasons. The first is that sexual anxiety, anger, or resentment block the normal course of sexual responses. Another reason is the woman’s ignorance about what kind of sexual stimulation her partner needs.

6. Is there a connection between impotence and an emotionally traumatic situation?

A stressful situation can be the cause of impotence. In this case, difficulty with an erection is part of the body’s natural response.Unfortunately, early failures can trigger the development of anxiety about sexual performance, which will later become the main cause of prolonged impotence.

7. Does a man suffering from erectile dysfunction have depression?

Depression is a common cause of sexual dysfunction. If a person is sad, dejected and exhausted, and has sleep or appetite disturbances, then sexual problems are just another symptom of depression.

8. How does a man react when problems arise? How does the partner react?

If a partner is able to support and help in difficult times, and the man himself calmly and philosophically reacts to difficulties with an erection, anxiety does not play a decisive role in this situation. Otherwise, it is the emotional reaction that can lead to a worsening of the situation.

9. Does the person take medications that can cause erection problems?

Many drugs have a negative effect on sexual function. However, taking these medications may not be the only cause of dysfunction. Therefore, medical diagnostics is indispensable.

If the symptom is of great concern, see a doctor. In doing so, try to eliminate stressors from your life. Good rest and good sleep can put the body in order if the cause of impotence lies in an emotional reaction.

But, although until recently it was generally accepted that impotence is associated exclusively with psychological factors, it is not so.The development of the latest medical technologies and the advances in the pharmaceutical industry have significantly expanded the understanding of the possibilities of diagnosis and treatment of men suffering from erectile dysfunction. Nowadays, specialists use many methods, in addition to psychological or surgical intervention.

When you visit your doctor, discuss the questions you answered before your appointment. This will help the specialist navigate the problem and understand what should be paid attention to – hormonal levels, the presence of vascular disorders, inflammatory diseases, or other possible causes.

Impotence can develop for quite natural reasons. As men age, the level of testosterone in the blood begins to decrease, which leads to impaired erectile function. But given that testosterone levels can decrease not only due to age-related reasons, the control of this hormone is necessary if problems with potency arise.

Impotence resulting from anxious experiences is just as real and requires the same serious study and treatment as impotence, the cause of which is a disease. It is equally important to determine if there is a lack of physical stimulation at the root of the problem, which can be quickly and easily corrected.

For a more accurate diagnosis, and even more so for the correct choice of treatment method, a complete medical examination will be required. It is also necessary in order to exclude diabetes that can cause impotence. Indeed, in addition to the main inconveniences, erectile dysfunction can be a harbinger or an early symptom of many diseases. It has become especially alarming recently, when erectile dysfunction disorder began to develop not only in older men, but also in young people who are absolutely healthy physically. Therefore, in no case should you self-medicate. Only an experienced doctor will correctly assess the current situation and prescribe a treatment that will return you to a full sexual life.

Erectile dysfunction-impotence treatment in Mytishchi | MC “Orange”

Erectile dysfunction is the inability to achieve or maintain an erection necessary for normal intercourse. Pathology can be caused by various reasons, and it is not uncommon. In most cases, it is diagnosed in men over 40 years of age. Lack of erection does not allow for normal sexual intercourse, which means it is one of the causes of male infertility. It has illness and another side effect – psychological. She is very hard for men. This is why successful erectile dysfunction treatment is so important.

The prognosis for such a pathology depends on its type, causes, clinical picture and other factors. Despite the fact that modern urology successfully copes with diseases that were considered incurable a few decades ago, it is not always possible to cure impotence. For example, age-related erectile dysfunction in older men is hardly treatable.

Having problems with erection? Make an appointment with a urologist at the MC “Orange” by phone +7 (495) 646-80-03. In the early stages, erectile dysfunction is treatable in many cases.

Causes of erectile dysfunction

All the reasons leading to the development of ED can be divided into several groups:

  • Psychogenic. They occur most often and can be associated with both a lack of attraction due to age-related changes, and with situational problems. The lack of an erection in itself is stressful for a man, which further increases the influence of psychogenic factors, since the fear of failure is added to them. Treatment of erectile dysfunction caused by psychogenic factors is primarily aimed at eliminating psychological problems.
  • Organic. This group of reasons includes various diseases that have a negative effect on potency. Arterial hypertension, hormonal disorders, obesity, diseases of the genitourinary system, spinal injuries can make a normal erection impossible. The situation is aggravated by the fact that many drugs used to treat common diseases negatively affect the ability of men to have a normal erection.

One of the most important tasks that must be solved in order for the treatment of erectile dysfunction in men to be successful is to identify the causes of the problem, since without eliminating them it can return.

Symptoms

Regardless of the reasons, the symptoms of erectile dysfunction are always the same and it is simply impossible to confuse it with other diseases.Even with sexual desire, an erection does not occur.

It is imperative to see a doctor if you encounter the following problems:

  • In the morning or during intercourse, the penis is erect but not firm enough.
  • Due to insufficient erection, it becomes impossible to complete intercourse.
  • Ejaculation occurs prematurely.
  • No erection at all.

Diagnostics and treatment

The diagnostic stage is very important, as it allows you to collect information about the patient’s health, to identify the causes that led to impotence. For diagnostics, complex methods are used, an additional examination is prescribed by specialists of other profiles. Only after that, the doctor will select a treatment program.

Sometimes, in order to get rid of the problem, it is enough to eliminate the reasons due to which erectile dysfunction occurred, the price of refusing to be examined and trying to carry out symptomatic treatment is a complete loss of the opportunity to have a normal sex life.

Different methods are used for treatment:

  • Conservative. This refers to the intake of medicines and is complemented by the use of other methods of treatment.
  • Vacuum therapy. During the procedure, the doctor ensures the blood flow using a vacuum. This stimulation of blood circulation allows you to restore the ability to erect.
  • Revascularization. This is a microsurgical intervention that allows you to create an anastomosis between the arteries and thus restore normal blood circulation.
  • Endophalloprosthetics. It is used in cases where it is necessary to restore the rigidity of the penis. To do this, during the operation, the doctor implants implants into the cavernous bodies.
  • Venous surgery. This method is used in combination with traditional medications.

Medical Center “Orange” is a diagnostic and medical institution. We use traditional methods of treatment, vacuum therapy. If serious surgical intervention is required, the doctor will refer you to another clinic in Moscow, where similar services are provided.

And if you do not treat this problem properly, it can really end up in complete impotence (sexual impotence).

Medicines for the treatment of impotence

The problem of sexual weakness is being successfully treated. In addition to medical methods, the treatment of erectile dysfunction with folk remedies has long been used. The collection of medicinal plants is effectively used for the treatment and prevention of impotence.

Some of them are:

Prepare a mixture of dry medicinal herbs from stinging nettle, mint and flax seeds. All ingredients must be taken in equal amounts. Pour two tablespoons of this collection into a thermos and pour two glasses of boiling water. It is better to prepare the infusion in the evening, then it will be ready by morning. Drink one glass in the morning and in the evening, regardless of the meal. The course lasts 21 days. The break is from one week to two months. Sometimes a couple of courses are enough. This collection helps men avoid premature ejaculation.

The next collection includes only 2 ingredients: immortelle flowers – 2 parts, calendula flowers – 1 part. Pour one tablespoon of this mixture with a glass of boiling water. It should be insisted before cooling, preferably without access to light, for example, in a ceramic teapot. Take 1 tbsp. spoon 3-4 times a day, between meals. The course can last up to 30 days, then a break – 30 days. This folk remedy for impotence is quite effective. In some cases, only 2-3 courses may be needed.

Yarrow is readily available in every pharmacy. A decoction is prepared from it, which helps to cope with dysfunction. To do this, pour 200 g of dry yarrow herb with a glass of water, boil for no more than 20 minutes over very low heat. Cool and drain. Take this broth 1 tablespoon 3-4 times a day.

Herbs for impotence give no less effect than drugs, and provide a lasting result. In addition, no side effect is observed from them.

Alcohol tinctures for impotence

Alcohol tinctures are also known for the treatment of sexual weakness in men. They are indicated for use only if the man does not intend to get behind the wheel, otherwise there may be trouble with representatives of the traffic police. It should be borne in mind that alcoholic tinctures are prepared precisely from vodka or diluted alcohol (up to 40 degrees), since if the degree is higher, the healing substances will not pass from the raw material into the tincture. They are simply preserved in the feedstock. Traditional methods of treating impotence with alcoholic tinctures suggest making them from plant rhizomes.

The method of their preparation is the same. One part of the raw material is filled with 3 parts of vodka and infused for two weeks in a dark place. The container with the tincture should be shaken periodically. Take 25-30 drops after meals. For the preparation of the tincture use: calamus root; lovage root; ginseng root; galangal root. The use of these funds for impotence gives a lasting result.

What is the difference between the use of medicinal herbs and drugs for impotence? The use of plant materials has a beneficial effect on the entire body, therefore, not the disease disappears, but its cause. While some drugs for erectile dysfunction act like a whip on a driven horse. The only drawback in the treatment of impotence with folk remedies is that recovery occurs gradually. Patience must be stocked up. Sometimes it will take more than one course of taking the drug, but 2-4, and in some cases, the treatment can last 6-12 months.

Take care of yourself and be healthy!
