How Is Hyperglycemia Diagnosed, Exactly?

If you have insulin resistance, prediabetes, type 1 diabetes, gestational diabetes, or type 2 diabetes, your body isn’t able to use the hormone insulin correctly. Insulin is critical for regulating blood sugar levels because it ferries blood sugar, or glucose, to our cells and muscles for immediate energy or to store for later use.

Typically, your doctor will diagnose you with insulin resistance, prediabetes, or diabetes after seeing that your blood sugar levels are abnormal. Often, the test they’ll use is the hemoglobin A1C, or A1C test for short, says Gregory Dodell, MD, an assistant clinical professor of medicine, endocrinology, diabetes, and bone disease at the Icahn School of Medicine at Mount Sinai in New York City.

A1C is a two- to three-month average of your blood sugar levels, says Dr. Dodell, explaining that the test measures the percentage of sugar that’s attached to your red blood cells.

Here’s what your A1C result means:

  • Under 5.7 percent: normal
  • 5.7 to 6.4 percent: prediabetes
  • Over 6.5 percent: diabetes

If your A1C is over 6.5 percent on two or more separate occasions, you likely have diabetes.

Dodell says your doctor may also test your blood sugar levels through a fasting glucose test. Here’s what those results mean:

  • Under 100 milligrams per deciliter (mg/dL): normal

These symptoms can worsen if you don’t treat high blood sugar. Too-high blood sugar levels can even lead to a life-threatening condition called diabetic ketoacidosis, also called diabetic coma.

During ketoacidosis, your body breaks down large amounts of fat at once, and as a result, ketones are excreted and sent to your urine. But when there are too many ketones, your body can’t keep up with this excretion process, causing ketones to accumulate in your bloodstream. Signs of ketoacidosis from high blood sugar can include dry mouth, fruity-smelling breath, nausea, and shortness of breath. You may even have anxiety and vomiting.

Another possible complication of high blood sugar is called hyperglycemic hyperosmolar syndrome. With this condition, your blood glucose readings may be 600 mg/dL or higher. It occurs when you do have enough insulin, but your body’s not using it right. The glucose isn’t used up by your body, and instead transfers to your urine. Hyperglycemic hyperosmolar syndrome is considered a life-threatening emergency. Signs may include dehydration and even coma.

High blood sugar can be seen in various forms of diabetes, including type 1, type 2, and gestational.

People with type 1 diabetes experience high blood sugar because their bodies don’t produce insulin. On the other hand, if you have type 2 diabetes or gestational diabetes, you might have adequate insulin, but your cells and muscles can’t absorb it properly due to insulin resistance.

But you can also develop hyperglycemia if you don’t have diabetes. In these cases, episodes of hyperglycemia are usually temporary. Your blood sugar may rise after you eat a large meal or as a result of a high-endurance workout. Emotional stresses and illnesses can also cause these fluctuations.

First, know you can’t just cut all carbs or avoid foods with sugar to prevent blood sugar spikes. After all, healthy foods such as fruits and veggies also contain carbs and sugar. The value of carbs differs according to their complexity, and sugar in food is different from blood sugar.

That said, you should try to avoid these foods to help prevent blood sugar spikes:

  • White bread, pasta, and rice
  • Packaged and processed snacks, such as chips, pretzels, cookies, and candy
  • Soda or juice
  • Fast food
  • Foods high in saturated fat, such as hot dogs, bacon, and sausage
  • Foods high in trans fats, like margarine and certain packaged baked goods
  • Foods that can raise your cholesterol, like liver, red meat, and full-fat dairy

Tracking your carb intake, especially if you have diabetes, is also critical for avoiding blood sugar spikes.

To help keep your blood sugar levels steady, the Mayo Clinic also recommends eating fish twice a week, as well as concentrating on plant-based fats and fiber-rich foods regularly.

Because eating large meals is a common cause of blood sugar spikes, it’s best to eat smaller meals throughout the day, rather than three big meals. Cutting down on overall portion sizes can also help.

When you exercise, your body uses glucose as its primary energy source. This, in effect, will help bring down your blood sugar levels. Working out regularly will lower your A1C.

The positive effects of regular exercise are unmistakable. According to the American Diabetes Association, working out can lead to blood sugar-reducing effects for up to 24 hours.

Work with your healthcare provider or hire a personal trainer to figure out which exercises are best for you, based on your goals and your overall fitness level. Some exercises can affect your blood sugar more than others. For instance, high-intensity workouts, such as running or spinning, burn more glucose, while longer bouts of low-intensity exercises, like walking, burn more fat. Either option can help increase insulin sensitivity. You should also measure your sugar before and after each workout.

Diabetes Medication That Can Help You Control Your Blood Sugar

If you’ve been diagnosed with diabetes, your doctor will likely discuss medication options to help bring your blood sugar down. If you have prediabetes, on the other hand, you’ll probably rely on diet and lifestyle changes to help stabilize your numbers, though in some cases, you may need medication, Dodell says.

Diabetic ketoacidosis is treated with emergency insulin and fluids administered intravenously. But the goal with blood sugar control is to prevent this type of medical emergency from happening in the first place. If your blood glucose readings are consistently higher than usual, you may not be getting enough insulin. Talk to your doctor about adjusting your dosage. You should also tell them about any other prescription or over-the-counter medication you take, as these could affect your blood sugar, too. Corticosteroids for inflammation are just one example.

If you’re receiving emergency medical treatment for high blood sugar complications, your doctor may administer fluids and electrolytes.

Random blood sugar values are often not that useful to your provider and this can be frustrating to people with diabetes. What is prediabetes?

Prediabetes is a warning sign that type 2 diabetes (T2D), which affects 1 in 3 American adults, might be in your future. Typically, this means that your blood sugar is higher than normal, but not yet high enough for a type 2 diagnosis. 

Luckily, prediabetes does not have any serious symptoms. Think of it as a call to action. “The way I explain prediabetes to my patients is that your body is struggling to keep your blood glucose levels in a healthy range,” says nutritionist Lauren Antonucci RD. “You shouldn’t panic, but you should start making real changes in your diet and lifestyle to prevent your blood sugar from rising and turning into type 2.”

What causes prediabetes?

Insulin is a hormone made by your pancreas that helps your body convert glucose into fuel. When you eat a meal, the carbohydrates in your food are converted into glucose, otherwise known as blood sugar. 

That glucose stays in your bloodstream until your pancreas releases insulin. The insulin acts like a key that opens your cells to allow the glucose to enter, where it’s then used to fuel your body. Without insulin or when insulin is not working effectively, that glucose stays stuck in your bloodstream and accumulates, causing your blood sugar to rise.

During type 2 diabetes, your pancreas can still produce insulin, but that insulin gradually becomes less effective at helping the glucose into your cells. 

Prediabetes is an indicator that your body is beginning to stop using insulin as efficiently as it should. In other words, your body is starting to become resistant to insulin. When your body resists insulin, the glucose levels in your blood rise, which is how you develop prediabetes and, if that process is not stopped and allowed to progress, type 2 diabetes.

What are prediabetes symptoms to watch out for?

“If prediabetes was easy to detect, you wouldn’t have people go years and years before getting diagnosed with type 2 diabetes,” says Antonucci. 

Typically, adults experience either no symptoms at all, or the insulin resistance symptoms are so gradual or slight they may go unnoticed for years. Sometimes, though, there are warning signs. These include:

  • Increased thirst
  • Increased hunger
  • Fatigue
  • Unexplained weight loss, even if eating more
  • Frequent urination

The last symptom happens because excess sugar in your bloodstream triggers your body to make more urine in order to flush the glucose out. The more you urinate, the more likely you are to become dehydrated, which can lead to a cycle of increased hunger and thirst signals.

Diabetes symptoms men experience can also cause sexual issues such as erectile dysfunction (ED) and decreased sex drive.

These are some of the prediabetes risk factors and warning signs to watch for.

How do doctors diagnose prediabetes?

Diabetes is a measure of levels of sugar in your bloodstream. Physicians check this in two different ways: 

1. A standard blood glucose test

This requires you to fast before a blood test, so your PCP or endocrinologist can check the level of glucose in your bloodstream at any given time. Ideally, fasting glucose levels should be 99 mg/dL or lower. If your fasting blood glucose levels are between 100 and 125mg/dL, it could be considered prediabetes, says Martha McKittrick RD. “I like to have people get a second test done, to make sure it’s accurate,” she says. A reading higher than 126 mg/dL indicates diabetes.

2. An A1C blood test 

The test for A1C, or hbA1c, is a non-fasting blood test that measures the average amount of glucose in your blood stream over a period of three months and is measured in percentages. A healthy percentage is under 5.7%. A result between 5.7 and 6.4% suggests prediabetes. Anything higher than 6.5% indicates diabetes.

A person who is prediabetic may not always show symptoms. In fact, the majority don’t. Oftentimes, an asymptomatic person will only get a prediabetes diagnosis if their general physician gives them a blood glucose test or checks their hbA1C readings and finds an atypical result. 

For this reason, one of the most important things you can do is get a physical exam with bloodwork once every year. “Your bloodwork can tell you if your glucose levels are too high, and whether or not you’re heading in the right direction as far as controlling them before you develop type 2 diabetes,” Antonucci says.

Although most people don’t show noticeable symptoms of prediabetes, if you are experiencing fatigue, excessive hunger, excessive thirst, and frequent urination, it’s better to be safe than sorry. Make an appointment to get your glucose levels checked ASAP.

What are the risk factors and warning signs of prediabetes? 

Your risk of developing prediabetes increases when you have any of the following:   

  • Unhealthy diet. Diets high in sugar and processed foods raise your blood pressure and increase your level of LDL, or “bad cholesterol” in addition to your blood glucose, putting you in danger of developing prediabetes.
  • Excess weight. If you’re medically overweight, you’re at a high risk for developing prediabetes. This is especially true if you carry a lot of extra weight in your abdomen, which is known as visceral fat and can play a role in insulin resistance.
  • A sedentary lifestyle that includes little to no physical exercise.  Being very inactive contributes to insulin resistance and weight gain, both of which can then lead to prediabetes. Aim to move around at least 30 minutes per day, even it is just to go on a walk.
  • A family history of type 2 diabetes. Prediabetes has a hereditary factor. If someone in your immediate family has T2D—or has had T2D in the past — you are more likely to develop prediabetes and type 2 diabetes yourself. Your race and ethnicity also affect your likelihood of developing prediabetes. There is a genetic component, but research has shown that racial disparities in who develops diabetes have more to do with socio-economic status and the accessibility of health care and healthy food in your community than your genes.
  • A history of gestational diabetes. Diabetes during pregnancy can signal a higher risk for prediabetes and type 2 diabetes later on.
  • Smoking.  The nicotine in cigarettes decreases the sensitivity of your cells to insulin, which raises the glucose in your blood. Other chemicals in cigarettes cause inflammation and make it harder for those cells to absorb insulin.

Is prediabetes reversible?

Absolutely! You can turn back your prediabetes by adopting lifestyle changes, including:

What are the treatments for prediabetes?

What is a prediabetes diet?

A diet for prediabetes looks a lot different than a typical American one. “I like to tell my clients to reserve half of your plate for vegetables, a quarter of your plate for protein, and a quarter of your plate for healthy carbohydrates, such as whole grains and fruit,” says McKittrick. 

A person with prediabetes should not eat processed carbohydrates that are high in sugar, but instead get carbohydrates from whole grains, fruits, legumes, and starchy vegetables (in moderation). “If you want to cut out carbs, you certainly can, but you don’t have to,” McKittrick says. “They taste good, they give you energy, and usually it’s not sustainable to give them up completely. ”

High Blood Sugar in the Morning: what to do

The facts are clear: many with diabetes experience high blood sugar in the morning. Indeed, this occurs for more than half of people with diabetes, according to this study.

What’s less clear is why this happens: there are a couple of different theories. But that doesn’t mean that you can’t do something about it.

Signs of high Blood Sugar in the Morning

High blood sugar is significant when it’s too high. That’s when it’s called hyperglycemia. Hyperglycemia is usually defined as above 126 mg/dL or 7.0 mmol/L in a period of fasting (before eating). This will be the clearest sign of high blood sugar in the morning.

Before breakfast, you usually would have had a period of at least eight hours without eating (because you’re sleeping). As such, hyperglycemia during a period of fasting is particularly important here: you’ll know that the high blood sugar wasn’t caused by any recent meals.

Other signs of high blood sugar in the morning will be the usual symptoms of hyperglycemia:

  • Nausea
  • Feeling faint or weak
  • Thirst
  • Tiredness
  • Blurred vision
  • Increased urination

Of course, it’s normal to feel somewhat groggy and thirsty when waking. It’s important to distinguish between hyperglycemia and the I-hate-Mondays morning tiredness. And the best way to do that is to test your blood sugar.

Why is my Blood Sugar high in the Morning?

High blood sugar in the morning is mysterious to the extent that it may be a “phenomenon”. Specifically: the dawn phenomenon.

This is where hormones (like adrenaline) are released in the early hours of the morning. In turn, the hormones make the liver release its store of glycogen (which becomes glucose), thus raising blood sugar levels.

It’s not exactly known why this happens, but these hormones are usually released to give extra energy. So, the theory is that the body releases extra glucose in order to give you extra energy to wake up and start the day.

Obviously, this extra glucose doesn’t help those with diabetes, but at least the body’s trying. It’s the thought that counts, right?

There may also be another potential reason for high blood sugar in the morning: the Somogyi phenomenon (it’s a Hungarian name, in case you were wondering). This can occur as a result of insulin use.

It’s theorised that taking insulin before bed can cause the body to have a rebound effect whilst sleeping. If insulin continues to lower blood sugar during sleep, and it’s not being treated, then the body responds in stress mode: it releases hormones (like adrenaline again) to trigger a rise in blood sugar levels.

But for those with diabetes, this doesn’t stabilise the blood sugar. That extra glucose can’t be used without diabetes medication, and so, the blood sugar gets too high instead.

There could be other reasons for high blood sugar in the morning, such as too many carbs before bed or requiring a different level of insulin. We can look at what to do about that in a moment.

How to Lower Blood Sugar in the Morning

As with any time of the day, lowering blood sugar will usually mean taking medication.

If you’re taking other medication for diabetes, just make sure you’re taking the right amount at the right time of day. With insulin, you might want to use insulin to lower your blood sugar back to a stable level.

More generally, stress and diabetes leads to higher blood sugar levels – so it is worth getting enough sleep. 

Somogyi phenomenon

With the Somogyi phenomenon, you’ll want to take the opposite steps because you want to prevent low blood sugar during the night. It may seem somewhat counterintuitive, but you might need to take less insulin, or eat more carbs, or do less exercise before bed.

It can be difficult to determine what caused your morning high blood sugar. And so, many of these steps will be trial-and-error until you find the right fit for you. Make sure to seek advice from your healthcare professional when going through this trial-and-error.

Last but not least, keep checking your blood sugar levels before bed: that’s the best way to get an idea of what your blood sugar is up to. High and Low Blood Sugars

High blood sugar may occur within hours or may develop over several days. You may have one or all of the following symptoms:

  • High blood sugar levels (over 200)
  • Extreme thirst
  • Fatigue/sleepiness
  • Using the bathroom a lot
  • Grouchiness
  • Pain in stomach with or without nausea and vomiting

Causes of high blood sugar include:

  • Too much food or eating the wrong foods
  • Not enough diabetes pills or insulin
  • Illness or infection
  • Not enough exercise (Do not exercise if your sugar is above 240)
  • Stress

What to do to prevent hyperglycemia:

  • Always take your diabetes medicine unless your doctor tells you not to.
  • Test your blood sugar every four to six hours.
  • If you usually take insulin, test your urine for ketones. Call your doctor if moderate to large ketones are present.
  • If your blood sugar is over 150, drink 8 ounces of sugar-free liquid every one to two hours.
  • Call your doctor if you are vomiting and unable to eat or drink liquids.

Remember to call your doctor if your blood sugar stays above the goals you’ve been given for 24 hours.

Low blood sugar (usually less than 70mg/dL) usually occurs quickly and may require that someone help you get treatment. Symptoms include:

  • Sweating
  • Shaking
  • Fast and/or pounding heartbeat
  • Hunger
  • Irritability/Confusion
  • Fainting

Causes of low blood sugar include:

  • Too much insulin or too many diabetes pills
  • Not enough food or missing a meal
  • Too much exercise, or extra physical activity

What to do to prevent hypoglycemia:

Try to test your blood sugar. If you are too shaky, eat or drink a fast-acting carbohydrate such as:

  • 4 ounces juice (orange, apple, cranberry or grape)
  • 6 ounces (1/2 can) of a regular soft drink
  • 8 ounces skim milk
  • 3 glucose tablets
  • 8 Sweet Tart candies
  • 2 tablespoons raisins

Test blood sugar every 15 to 30 minutes until it is above 100. Eat a second fast-acting carbohydrate if needed. If your next regular meal is more than two hours away, eat a snack such as half a sandwich or three peanut butter or cheese crackers. If you are unconscious, someone should call 911 immediately. You should not be given anything by mouth if you are unconscious.

Be sure to tell your doctor about this episode in case your medication needs to be adjusted.

Medication Used to Correct a Low Blood Sugar

This medicine is available in a kit for a low blood sugar emergency. Symptoms of a low blood sugar emergency are:

  • unconsciousness (cannot wake the patient)
  • patient is awake but unable to eat
  • the patient is having a seizure
  • the patient’s blood sugar is still less than 70 even after eating or drinking food containing sugar.

Most often, glucagon is injected into the muscle. This type of glucagon is a powder in a bottle to which sterile water is added from a pre-filled syringe. The same syringe is then used to give the injection. Do not mix the glucagon with water unless you are going to inject the glucagon.

Some glucagon can be given as a puff through the nose. Talk to your healthcare provider about which type may be best for you and your family. A family member or friend should know where you keep your glucagon kit and how to administer the medication. Read the directions that come with the kit with your friend or family member. That way you will be prepared if a low blood sugar emergency happens.

Administering Glucagon in an Emergency

If a diabetic person is unable to be woken up, call 911 immediately.

It is important to act quickly when someone with diabetes becomes unconscious or will not wake up.

  1. Turn him/her on his/her side. He/she may vomit when waking up; turning him/her on his/her side will stop him/her from choking on vomit.
  2. Administer the glucagon according to the directions that came with the kit.
  3. Feed the patient as soon as he/she wakes up. Try small sips of fruit juice or regular pop. If tolerated, follow with a snack of a carbohydrate and a fat, such as peanut butter crackers.

Call 911 even if the patient wakes up.

**Glucagon will not revive someone who is unconscious because of HIGH blood sugar.

Check the expiration date on your glucagon kit and ask your doctor for a new prescription if your kit has expired.

  • Not everyone with diabetes needs to check their blood sugar every day. But some people may need to check it many times a day.
  • If you have type 1 diabetes, check your blood sugar at least 4 times a day.

Usually, you will test your blood sugar before meals and at bedtime. You may also check your blood sugar:

  • The time of day
  • Your blood sugar level
  • The amount of carbohydrates or sugar you ate
  • The type and dose of your diabetes medicines or insulin
  • The type of exercise you do and for how long
  • Any unusual events, such as feeling stressed, eating different foods, or being sick

Many glucose meters let you store this information.

You and your provider should set a target goal for your blood sugar levels for different times during the day. If your blood sugar is higher than your goals for 3 days and you don’t know why, call your provider.

Random blood sugar values are often not that useful to your provider and this can be frustrating to people with diabetes. Often fewer values with more information (meal description and time, exercise description and time, medicine dose and time) related to the blood sugar value are much more useful to help guide medicine decisions and dose adjustments.
