Tea and Coffee Lower Blood Pressure

MILAN, Italy — A large French retrospective analysis provides good news for caffeine lovers: investigators showed that drinking tea or coffee was associated with a small but statistically significant reduction in systolic and diastolic blood pressure. In addition, drinking tea and coffee was also associated with a significant reduction in pulse pressure and heart rate, although the heart-rate reductions were greater with tea.

Presenting the results at the European Society of Hypertension (ESH) 2013 Scientific Sessions, Dr Bruno Pannier (Centre d’Investigations Préventives et Cliniques, Paris, France) said that other studies have suggested a relationship between coffee and tea consumption and blood pressure, but these analyses haven’t been conclusive. Some have suggested a benefit, while others found no relationship between tea/coffee consumption and blood pressure.

Presenting the data on 176 437 subjects aged 16 to 95 years of age who had a checkup at their center between 2001 and 2011, Pannier explained that the analysis was simply based on a questionnaire asking participants how much coffee or tea they drank per day. Individuals were classified into three groups: those who drank no coffee/tea, those who drank one to four cups, and those who drank more than four cups.

Overall, coffee is consumed more frequently than tea, although there were differences between the sexes, said Pannier. Men were more likely to drink coffee, while women were more commonly tea drinkers. Coffee consumption was also significantly associated with tobacco consumption, higher cholesterol levels, and higher scores on stress and depression indexes. Tea consumption, on the other hand, was associated with lower cholesterol levels but similarly high scores on the stress and depression measurements.

After adjustments that included these and other potential confounding variables, both coffee and tea consumption was associated with a significant reduction in systolic and diastolic blood pressure, as well as other variables.

Speaking during the session, Pannier explained that the group did not differentiate between green, black, or herbal tea consumption, which is one of the limitations of the analysis. In addition, the questionnaire is not sophisticated enough to address estimates in the caffeine content of the coffee consumed in France.

That said, Pannier believes that tea is a major source of flavonoids in the diet, and these compounds can improve vasodilation. “The vasorelaxing compounds included in these beverages might be involved in these results, something that has been suggested by the experimental data,” he said.

Is coffee good for you or not?

Just a couple of calories a cup, good old black coffee packs quite a punch. It wakes you up, boosts your metabolic rate and decreases the risk of some diseases.

Not that habitual coffee drinkers need convincing, but evidence of its health benefits stacks up quickly:

  • Higher consumption of coffee – caffeinated and decaf alike – was associated with a lower risk of total mortality, including deaths attributed to heart disease, nervous system diseases and suicide.

More specifically, habitual coffee drinking has been linked to a lower risk of coronary heart disease in women.

For health-conscious coffee lovers then, the most important question isn’t, “Is it good for you?” but rather, “How do you take it?”

If you dress your coffee up too much with cream and sugar, you risk negating the health benefits.

“We know that sugar has adverse effects,” said Penny Kris-Etherton, a nutrition professor at Penn State University. “Even if you add sugar and don’t exceed your calorie needs, you’re still negating some of the benefits because sugar is a negative food ingredient.”

That warning goes double for even fancier coffee drinks. The federal dietary guidelines say three to five cups of coffee per day can be part of a healthy diet, but that only refers to plain black coffee.

“They’re not talking about these large Frappuccinos that have at least 800 calories a beverage,” Kris-Etherton said. “Very quickly, calories can add up, and weight gain will create negative effects on cardiac risk.”

Despite its benefits, caffeine also can be dangerous if consumed in excess.

“We all know how important sleep is,” Kris-Etherton said. “You don’t want to disrupt normal sleep habits and good sleep because you’ve had too much caffeine, so if you want to include coffee in your diet, be sure to think about timing.”

Anyone who’s had one cup too many knows that heart-fluttering feeling that comes next; for some people, those jitters may be a warning sign.

“Some people are slow metabolizers of caffeine,” Kris-Etherton said. “It’s a genetic predisposition. Some people can experience jitters, palpitation, insomnia – sort of like those energy drinks that give you a big boost.”

Caffeine also is addictive, and cutting back too quickly can cause withdrawal symptoms, especially terribly harsh headaches.

“Those migraines are pretty bad,” she said. “If you are drinking a lot and then go cold turkey, the effects will be greater than if you have less caffeine and taper off.”

It’s worth noting that kids shouldn’t drink coffee, Kris-Etherton said. The American Academy of Pediatrics recommends that, in general, kids avoid caffeine-containing beverages.

Kris-Etherton also cautioned that brewing methods can affect cardiovascular risk. For example, she said, paper filters remove a compound called cafestol that increases LDL cholesterol (the harmful type), so unfiltered coffee could pose a higher health risk.

“Most people drink filtered coffee,” she said. “But you know, French presses are so popular too, and that may not be good for you, especially if you drink a lot of coffee.”

Still, Kris-Etherton said, research shows that the health benefits of coffee – even decaf – seem to outweigh the risks: “Just pay attention to how you’re feeling after consuming coffee. Get in sync with how your body’s feeling.”

When we stand up from sitting or lying down, our blood vessels respond to gravity by narrowing to prevent our blood pressure falling. This ensures a steady supply of oxygenated blood to the brain.

Orthostatic hypotension is a form of low blood pressure. It happens when the blood vessels do not constrict (tighten) as you stand up. It is usually a symptom of an underlying disorder rather than a disease in itself. The condition is also known as postural hypotension.

Everyone is likely to experience a mild form of postural hypotension at some time. It’s the lightheaded feeling you may get if you leap out of bed very quickly.

Symptoms of orthostatic hypotension

The drop in blood pressure reduces the flow of oxygenated blood to the brain. Symptoms may include:

  • Blurry vision
  • Dizziness
  • Light-headedness
  • Mental confusion
  • Nausea
  • Muscle tremors
  • Fainting.

Blood pooling explained

Gravity dramatically affects blood flow whenever you stand up from sitting or lying down – on average, about 800ml of blood temporarily pools in the blood vessels of the legs. Sensors called baroreceptors located in the three major arteries – the aorta and the two carotid arteries – detect this drop in blood pressure.

In a healthy person, the cardiovascular system and the autonomic nervous system quickly respond by increasing the heart rate and directing blood vessels in the legs and abdomen to constrict (tighten). These measures maintain adequate blood pressure to the brain.

This compensatory mechanism to increase blood pressure does not occur, or may be delayed, in a person with orthostatic hypotension. Their blood pressure remains low, which triggers symptoms. Blood pressure typically returns to normal once the person sits or lies down again, but this depends on the severity of the underlying cause.

Causes of orthostatic hypotension

Orthostatic hypotension usually results from an underlying disorder. It is not a disease in itself. Some of the many causes of orthostatic hypotension include:

  • Fever
  • Prolonged bed rest
  • Excessive amounts of alcohol
  • Some medications, such as some diuretics or antihypertensive (high blood pressure) medications
  • Dehydration due to vomiting, diarrhoea or both, as in gastroenteritis
  • Certain conditions, such as anaemia, diabetes, varicose veins or adrenal insufficiency
  • Nervous system disease, such as Parkinson’s disease or neuropathy
  • Heart problems, including irregular heart beat (heart arrhythmia), congestive heart failure, aortic stenosis or heart attack
  • Spinal cord conditions such as syringomyelia
  • Shy Drager syndrome, a degenerative disorder of the brain and spinal cord that affects functioning of the autonomic nervous system
  • Significant blood loss.

Other causes of dizziness

Remember there are other causes of dizziness, light-headedness and nausea, apart from postural hypotension. One example is ear disease. It is always important to seek medical advice for any unexplained symptoms.

Old age is an important risk factor

The incidence of orthostatic hypotension increases with age. One US study found that every elderly participant had some degree of orthostatic hypotension. Factors that seem to increase the risk in elderly people include:

  • Digestion – when you eat and then digest food, an increased blood supply is sent to your intestines. Orthostatic hypotension is more likely to occur about 15 to 45 minutes after eating a meal (this is known as post-prandial hypotension).
  • Standing – if you stand up for a long time, it causes a slow but steady drop in blood pressure.
  • Frailty and chronic sickness – these are more common in old age.

Diagnosis of orthostatic hypotension

Tests used to diagnose orthostatic hypotension include:

  • Medical history, including medical conditions and drugs taken on a regular basis
  • Physical examination
  • Measuring blood pressure when lying down versus standing up
  • Blood tests to check, for example, blood sugars or adrenal hormones
  • Echocardiography, an imaging scan of the heart, to check for certain heart conditions
  • Other tests, depending on individual factors.

Treatment of orthostatic hypotension

Treatment depends on the cause. Options may include:

  • Fluids to treat dehydration
  • Management of diabetes, such as regular insulin injections
  • Changes in medication or altered doses if drugs are the cause (sometimes, however, stopping or altering the dose of a particular medication may cause more harm than good and must be carefully considered in consultation with your doctor)
  • Medication, surgery or both to treat heart conditions
  • Medication to increase blood volume or pressure, including corticosteroids
  • Medications to treat orthostatic hypotension including pyridostigmine, a drug used in the treatment of myasthenia gravis
  • A range of treatments, since orthostatic hypotension may have two or more causes
  • In some cases, a lower body pressure suit is required.

Self-help suggestions

Be guided by your doctor, but general suggestions that may help you to manage orthostatic hypotension include:

  • Prop your head up with pillows when you’re in bed to reduce the likelihood of orthostatic hypotension when you get up.
  • Get up slowly from a sitting or lying position to allow your blood vessels time to adjust.
  • Wear support stockings to reduce the pooling of blood in the legs, which helps to boost blood pressure throughout the body.
  • Eat small, frequent meals instead of three large meals per day.
  • Drink plenty of fluids throughout the day to prevent dehydration.
  • Limit alcohol consumption. Alcohol can cause dehydration and dilation of blood vessels.
  • Consider drinking small but regular doses of caffeine, which boost blood pressure. A cup of coffee or tea with each meal tends to reduce the severity of orthostatic hypotension. Remember, however, that caffeinated drinks can cause dehydration, so moderation is the key.
  • Try other drinks that contain caffeine, like hot chocolate or cola, if you don’t like coffee or tea.
  • Avoid vigorous exercise, hot baths, saunas and any hot environment because heat causes blood vessels to widen (dilate).
  • Avoid standing without moving for long periods of time.
  • Avoid ascending quickly to high altitudes.

Researchers Say 4 Cups A Day Does The Deed

Researchers at the Preventative and Clinical Investigations Center in Paris, France observed the blood pressure of almost 200,000 men and women between the ages of 16 and 95 for 10 years and recorded their blood pressure, pulse pressure, and heart rate.

The findings revealed that those who avoided coffee and tea consumption all together had the highest rates of blood pressure, pulse pressure, and heart rate. And, those who drank tea the most often had the best health reports. Even coffee drinkers fared better than those who didn’t drink coffee at all.

The participants were divided into three groups; those who abstained from coffee and tea completely, those who drank between one and four cups a day, and then those who drank more than four cups. The results ran counter to most medical advice, such as that of the National Health Services, which recommends people with high blood pressure to cut down their caffeine intake.

The results also revealed heavy tea drinkers benefited the most and had the lowest systolic and diastolic blood pressure readings, as well as the lowest pulse pressure and heart rate. Diastolic blood pressure is exerted on the walls of arteries around the body and in between heartbeats when the heart is relaxed, while systolic blood pressure is the pressure exerted when the heart is beating. Researchers think the flavonoids found in tea could help relax blood vessels, which ultimately decreases blood pressure and calms the nervous system.

The authors of the study say the findings are scientifically significant; however, people with high blood pressure shouldn’t start downing six cups a day in hopes of a natural medication. The difference between those who drank a moderate one to four cups and those who drank more was slim. Nevertheless, if you’re choosing between coffee or tea, there isn’t a large margin; those who drank large quantities of coffee every day only had slightly higher blood pressure reading than tea drinkers.

The debate on whether or not caffeine consumption is actually good for you continues. Dr. Bruno Pannier from the Preventative and Clinical Investigations Center presented the study’s findings to the European Society of Hypertension in Milan and said more research needs to be done to explain the pros and cons of tea and coffee drinking. Pannier also made clear that the study made no distinction between black tea, herbal tea, and green tea — each with different effects according to previous studies.
