  • Take a sitz bath two to three times daily for 15 to 20 minutes. Sit in warm water, covering just your hips and buttocks. This can help reduce itching and irritation. You can take a sitz bath in a few inches of water in a regular tub, but small tubs that fit over toilet seats are also available for this purpose at pharmacies. Following a sitz bath, gently pat the area dry to avoid further irritation. (2)
  • Use ice packs on the area to relieve swelling and pain.
  • Don’t push too hard or strain while passing a bowel movement.
  • Clean your anus after each bowel movement by gently patting (rather than wiping) with moistened pads, such as baby wipes. Using hard, dry toilet paper, which may contain fragrance, can cause further irritation.
  • Keep the area clean by bathing or showering daily with warm water. After bathing, gently pat the area dry. You can even use a hair dryer to dry the area. (2)
  • Sit on cushions rather than hard surfaces to help lessen the swelling of hemorrhoids and keep new ones from forming.
  • Take over-the-counter (OTC) pain relievers, such as acetaminophen, ibuprofen, or aspirin as needed. (3)

Hemorrhoid Medication: When, Why, and Which Ones to Use

A number of OTC creams, ointments, and pads that contain hydrocortisone or witch hazel are available to help relieve pain, swelling, and itching caused by hemorrhoids. According to the National Institute of Diabetes and Digestive and Kidney Diseases, doctors recommend using over-the-counter products for a week. If symptoms persist beyond a week, follow up with your doctor. (3)

Creams and suppositories containing hydrocortisone can cause the skin to weaken and thin, so they should not be used for more than a week. If any of these creams cause a rash or dry skin around the area, contact your healthcare provider. (2)

Medical Procedures for Hemorrhoids That Won’t Go Away

If hemorrhoids don’t go away with home treatments, medical procedures may be necessary.

If you visit your doctor with painful external hemorrhoids — those located around the outside of the anus that are visible from the outside — your doctor may offer to excise them (cut them off) during the office visit. You will be given a shot with a local anesthetic to numb the area before the procedure.

Doctor’s office procedures for internal hemorrhoids — those located inside the rectum — may include:

Rubber Band Ligation This is the most commonly performed hemorrhoid procedure in the United States. (2) In this procedure, which can be used for grades 1, 2, and 3 internal hemorrhoids (grading is based on the appearance of hemorrhoids and their degree of prolapse, or how much they protrude through the anus), the doctor places a small rubber band around the base of the hemorrhoid, which can be done when your doctor performs an anoscopy examination to better visualize the hemorrhoids. The rubber band cuts off the hemorrhoid’s blood supply.

The procedure typically causes the hemorrhoid to shrink and fall off within about a week, though several short visits to your doctor may be necessary to completely get rid of the hemorrhoid. Minor bleeding and a feeling of pressure may occur, but this can usually be alleviated with OTC pain relievers. This procedure is not associated with significant recovery time. (4)

Sclerotherapy In this quick, often painless procedure, the doctor injects a chemical into the hemorrhoidal blood vessel, causing scar tissue to form in order to shrink the hemorrhoid. (3) The principle behind this is the same as rubber band ligation, though it’s not always as effective. This procedure can be used in patients taking blood thinners, for whom rubber band ligation may not be recommended. (4)

Infrared Coagulation In this office-based procedure, the doctor uses an intense beam of infrared light to shrink the hemorrhoid. Heat from the light causes the formation of scar tissue, cutting off the blood supply to the hemorrhoid. (3) This procedure is generally quick and painless, with few complications, but can take several sessions to work completely. (4)

When Hemorrhoid Surgery May Be Necessary

Very few people — fewer than 10 percent of all adults who see a doctor because of symptomatic hemorrhoids — will require a surgical operation. (4)

Surgery to remove hemorrhoids is called a hemorrhoidectomy.

Your doctor may recommend a hemorrhoidectomy if:

  • You have large external hemorrhoids
  • You have both internal and external hemorrhoids
  • An internal hemorrhoid has prolapsed (popped out through the anus)

In a hemorrhoidectomy, the doctor makes a small incision to remove the hemorrhoid and surrounding tissue before closing the wound with stitches.

A hemorrhoidectomy is performed in an operating room and can be done under general anesthesia, in which you are unconscious and unable to feel pain.

It can also be done with a spinal anesthetic block while you are awake (similar to an epidural injection given during childbirth), or with a combination of relaxing and sedating drugs given intravenously (IV) and a local anesthetic injected around the anus.

Hemorrhoidectomy is associated with postoperative pain, but the procedure is successful for 95 percent of cases. (2)

More recently, another option aimed at avoiding the postoperative pain of a hemorrhoidectomy has become available, called a stapled hemorrhoidopexy. A circular stapling device pulls the hemorrhoidal tissue upward and to its normal position, stapling it in place. The staples eventually fall out over time. (4)

According to a study published in October 2015 in the journal Techniques in Coloproctology, in the long term, patient satisfaction, resolution of symptoms, quality of life, and functional outcome appeared similar among patients after stapled hemorrhoidopexy and hemorrhoidectomy. (5) But other sources note that stapling has been associated with a greater risk of recurrence and rectal prolapse. (6)

Pain is to be expected following hemorrhoid surgery, and it may take two to four weeks before you’re back to your normal self. Your doctor will likely prescribe you pain medication, and sitting in a sitz bath two to three times a day for 10 to15 minutes can also help. You may experience some bleeding with bowel movements for a few weeks after surgery. If the bleeding doesn’t stop after the bowel movement, call your surgeon. (4)

Are laxatives effective for the treatment of symptomatic hemorrhoids in adults?

Evidence-Based Answer

Fiber has a consistent beneficial effect in the treatment of symptomatic hemorrhoids for up to three months’ follow-up as measured by overall symptoms and bleeding.

Practice Pointers

Hemorrhoid treatment options include medical management, rubber-band ligation, sclerotherapy, coagulation, and surgical hemorrhoidectomy depending on the type of hemorrhoid and the frequency and severity of symptoms. The goal of first-line medical management is to minimize constipation and associated straining. Clinical practice guidelines recommend the use of fiber despite inconclusive evidence about its effectiveness in improving symptoms.

Alonso-Coello and colleagues reviewed the literature and identified seven randomized controlled trials comparing the effectiveness of fiber versus placebo in adults 23 to 71 years of age with symptomatic hemorrhoids. The trials studied several types of fiber including ispaghula husk, Plantago ovata or psyllium, sterculia, and unprocessed bran for a treatment duration of one to 18 months. Study size ranged between 28 and 92 participants with a mean of 50. Six of the seven trials assessed the degree of improvement of individual symptoms (e. g., bleeding, pain, itching, prolapse) or overall symptoms measured at six weeks’ and three months’ follow-up. One study examined rubber-band ligation plus fiber versus rubber-band ligation alone for third-degree hemorrhoids (defined as hemorrhoids that prolapse with straining but are reducible) and measured recurrence rate and the need for repeat procedures at 18 months.

The results of five studies reporting overall symptoms were pooled and showed a 53 percent reduction in the risk of persistent symptoms or lack of improvement. Of those taking fiber, 16 to 40 percent did not improve compared with 23 to 61 percent of those taking placebo. The four studies that reported bleeding as an individual outcome found a trend or a significant difference in favor of the fiber group. Pooled analysis of the two studies evaluating pain or discomfort showed a nonsignificant trend in favor of fiber. Likewise, the pooled analysis of three studies showed a nonsignificant difference between fiber and placebo for persistent pro-lapse. The two studies that evaluated itching did not find a significant difference between the groups. The one study examining rubber-band ligation plus fiber versus rubber-band ligation alone reported that the number of long-term recurrences was fewer overall in the group that received fiber (15 versus 45 percent, respectively) at 18 months’ follow-up.

The most common side effects with fiber were gastrointestinal symptoms, typically starting at the study onset, and these generally were not severe enough for participants to discontinue fiber. The rate of side effects varied considerably among studies, with some studies reporting no side effects and others reporting up to a 50 percent incidence of gastrointestinal bloating.

The American Gastroenterological Association recommends adequate water and fiber intake as the main-stay of medical management and suggests that topical steroids and analgesics also may be useful in relieving hemorrhoidal symptoms.1

Lifestyle and Diet Changes

Being constipated can cause haemorrhoids and irritate them further. It is important to be eating a healthy diet, which includes plenty of fibre and drink plenty of fluids to keep bowel motions soft and regular. MEDICINAL

Laxatives

If your haemorrhoids are caused by constipation, over-the-counter laxatives can be used in the short term to help. There are a variety of laxatives available that act in different ways. You can get bulking laxatives such as Fybogel, which work in the same way as increasing fibre in the diet; osmotic laxatives, which work by softening the stool making it easier to pass, and stimulant laxatives such as senna, which encourages peristalsis and stimulates the gut to pass the stool. Make sure that you speak to your local pharmacist or GP about which type of laxative is best for you.

Creams, Ointments and Suppositories

There are many over-the-counter creams, ointments and suppositories that you can buy to help relieve the discomfort and itching of haemorrhoids. Your pharmacist will be able to advise on a suitable treatment to try at home. Some of the topical treatments can also promote tightening to shrink the vessel.

NON SURGICAL

Sclerotherapy

Sclerotherapy involves injecting a solution into the haemorrhoid, which will cut off the blood supply and eventually force the haemorrhoid to shrivel up and disappear.

Banding

Banding involves putting a very tight elastic band around the base of the haemorrhoid to cut off the blood supply. After a week or so, the haemorrhoid should drop off. You will probably not notice this as it will likely happen with a bowel movement.

Infrared Coagulation/ Laser Treatment

This treatment uses infrared light or a laser beam to painlessly burn and seal the haemorrhoid cutting off the blood supply to the vessel. This method is most useful for prolapsed haemorrhoids.

SURGICAL

Haemorrhoidectomy

If your haemorrhoids are very severe, painful and the bleeding is causing an issue then you may be offered surgery to remove them. A haemorrhoidectomy is the traditional method of removing haemorrhoids, which involves cutting open the anus. This operation is done under general anaesthetic and can be quite painful to recover from.

Stapled Haemorrhoidopexy

A stapled haemorrhoidopexy is sometimes used as an alternative to a haemorrhoidectomy. This operation involves stapling part of the anorectum to reduce the blood supply and eventually cause them to shrink. This operation does carry more complications than a traditional haemorrhoidectomy but the recovery the time is quicker.

Haemorrhoidal Artery Ligation

This is an operation done under general anaesthetic to reduce the supply to the haemorrhoids. It involves inserting a small ultrasound probe that emits high frequency waves that allows the surgeon to locate the blood vessels.

What are hemorrhoids? — Hemorrhoids are swollen veins in the rectum. They can cause itching and pain. Sometimes they can also make you bleed during a bowel movement.

In some cases, you can see or feel hemorrhoids around the outside of the rectum. In other cases, you cannot see them because they are hidden inside the rectum.

Should I see a doctor or nurse? — You should see a doctor or nurse if you have any bleeding or if your bowel movements look like tar. Bleeding could be caused by something other than hemorrhoids, so you should have it checked out.

If you do have hemorrhoids, your doctor or nurse can suggest treatments. But there some steps you can try on your own first.

What can I do to keep from getting more hemorrhoids? — The most important thing you can do is to keep from getting constipated. You should have a bowel movement at least a few times a week. When you have a bowel movement, you also should not have to push too much. Plus, your bowel movements should not be too hard.

Being constipated and having hard bowel movements can make hemorrhoids worse. Here are some steps you can take to avoid getting constipated or having hard stools:

  • Eat lots of fruits and vegetables. They have fiber, which helps to increase bowel movements.
  • Take fiber powders, wafers, or pills. You should get 20 to 35 grams of fiber a day.
  • Take medicines called stool softeners such as docusate sodium (sample brand names: Colace, Dulcolax) or bulk-forming laxatives. Bulk-forming laxatives include psyllium seed (sample brand names: Metamucil, Konsyl), methylcellulose (sample brand name: Citrucel), polycarbophil (sample brand name: FiberCon), and wheat dextran (sample brand name: Benefiber). These medicines increase the number of bowel movements you have. They are safe to take and they can prevent problems later.

What can I do to reduce my symptoms? — Some people feel better if they soak their buttocks in 2 or 3 inches of warm water. You can do this up to 2 to 3 times a day for 10 to 15 minutes. Do not add soap, bubble bath, or anything to the water.

There are also medicines that you can get without a prescription. They are usually creams or ointments that you rub on your anus to relieve pain, itching, and swelling. Some hemorrhoid medicines come in a capsule (called a suppository) that you put inside your rectum. Others come in a cream that comes in a bottle with a nozzle that you put inside your rectum. It is OK to try these medicines. But do not use medicines that have hydrocortisone (a steroid medicine) for more than a week, unless your doctor or nurse approves.

What if the self-care steps do not work? — If you still have symptoms after trying the steps listed above, you might need treatments to destroy or remove the hemorrhoids.

One popular treatment is called “rubber band ligation.” For this treatment, the doctor ties tiny rubber bands around the hemorrhoids. A few days later the hemorrhoids shrink and fall off. The doctor can also use lasers, heat, or chemicals to destroy hemorrhoids. But if none of these options works, there is always surgery to remove the hemorrhoids.

Hemorrhoid symptoms

Hemorrhoid symptoms can vary depending on the location of your hemorrhoid. Common symptoms of a hemorrhoid can include:

  • Swelling around your anus
  • Irritation or itching on or near your anus
  • Pain or discomfort on or near your anus
  • Fecal leakage
  • Painful bowel movements
  • Bleeding, which is typically painless, during bowel movements. This bleeding might show up on tissue or in the toilet.
  • A lump on or near your anus that’s sensitive or painful

You can develop a hemorrhoid internally (inside your rectum) or externally (around your anus) External hemorrhoids can form blood clots, which causes a thrombosed hemorrhoid — which can be especially painful, but normally goes away without treatment any time from within several days to two weeks.

What Causes Hemorrhoids?

Hemorrhoids are normal part of our anatomy but several risk factors may increase your chances of developing symptoms. These include:

  • Chronic constipation

  • Bright red blood on the surface of a stool: This often means that the bleeding is from the anus or just inside. This can be caused by hemorrhoids or anal fissures. It can also be caused by more serious problems like cancer and polyps.
  • Blood mixed with a stool: This usually means that there is a disease or problem inside the rectum or higher up in the colon (intestines). Examples are colon cancer, colon polyps, diverticulosis, and ulcerative colitis.

  • Moderate: small blood clots, passing blood without stool, or toilet water turns red

Mild Rectal Bleeding

  1. What You Should Know:
    • Rectal bleeding is never normal. However, it is not always serious.

    • You can treat mild constipation at home.
    • Here is some care advice that should help.
  2. Eat a High Fiber Diet:
    • Slowly increase the amount of fiber in your diet. This will help soften your stools. Fiber works by holding more water in your stools.
    • Eat more fresh fruit and vegetables. They are great sources of natural fiber. This includes peas, prunes, citrus, apples, beans, and corn.
    • Eat more foods made from whole grains. Foods made from whole grains also have fiber. Examples are bran flakes, bran muffins, graham crackers, oatmeal, brown rice, and whole wheat bread. Popcorn is also a good source of fiber.

Almost everyone will get hemorrhoids at some point in their life. Most of the time, symptoms go away on their own, after a few days, even without treatment. On occasion, your hemorrhoid condition is complex and needs a doctor’s attention. That’s where we can help.

Hemorrhoids are enlarged or swollen veins in the bottom of the rectum or the anus. They do not usually cause serious health problems. But they can be annoying and uncomfortable.

Hemorrhoids are common, occurring in both men and women. They are more common in people:

  • Pelvic tumors
  • Pregnancy, both during or after
  • Sendenaty people, or those who sit for prolonged periods of time
  • Those who have diarrhea or constipation
  • Who are older

Treatment consists of dietary changes and having regular bowel habits. If your hemorrhoids are large, do not go away on their own or you have many hemorrhoids, you may need surgery.

Causes of hemorrhoids

In general the cause of hemorrhoids is increased pressure on the veins in the pelvis and rectal area. The cause of this can be:

  • Excessive straining found with chronic constipation
  • Excessive straining found with chronic diarrhea
  • Pregnancy, due to pressure in the pelvis from the baby in the uterus
  • Obesity
  • Prolonged daily sitting like with truck drivers or other sedentary professions

Symptoms of hemorrhoids

The most common symptoms of hemorrhoids include the following:

  • Painless rectal bleeding, usually is a small amount
  • Anal itching or pain, due to irritation of the skin surrounding the anus
  • Tissue bulging around the anus, some people can see or feel hemorrhoids on the outside of the anus
  • Leakage of feces or difficulty cleaning after a bowel movement

Rectal bleeding

Many people with hemorrhoids notice bright red blood on the stool, in the toilet or on the tissue after a BM. The amount of blood is usually small. Yet even a small amount of blood can cause the water to appear bright red. This can be frightening.

Less common is heavy bleeding.

While hemorrhoids are one of the most common reasons for rectal bleeding, there are other, more serious causes. It is not possible to know what is causing rectal bleeding unless you have an examination. You should seek medical attention if you see bleeding after a bowel movement.

Rectal pain

Hemorrhoids can become painful. If you develop severe pain, call your healthcare provider immediately. This may be a sign of a serious problem.

Testing for hemorrhoids

Often your doctor can determine if you have hemorrhoids by asking about your symptoms and doing a physical examination.

Your doctor will do a digitial rectal examination, an exam using a gloved finger inside your rectum. You may need to do an anoscopy. This is where your doctor inserts a short, lighted scope into your anus and exams the inside of your anal canal. You may need to have a Colonoscopy or sigmoidoscopy for further testing.

Hemorrhoids hidden inside the rectum are “internal” hemorrhoids. You cannot see them, but they can cause symptoms.

Hemorrhoids that you can see or feel are “external” hemorrhoids.

Hemorrhoid types

External hemorrhoids are visible on the outside of the anus and originate in the lower part of the anus. These can become inflammed and the blood inside the veins can become clotted. This is a thrombosed, or clotted, hemorrhoid.

Internal Hemorrhoids are generally not visible on the outside. This is because they originate higher up in the anal canal. Internal hemorrhoids more commonly cause bleeding after a bowel movement.

If internal hemorrhoids become large and severe, they can push out through the anus, becoming visible. This can be very painful. This is especially true if the hemorrhoid becomes trapped in the anal muscle and cannot go back inside.

Classification of Internal Hemorrhoids:

  • Grade 1 (minor) – A hemorrhoid is present but only visualized by a doctor with ansocopy or colonoscopy. The hemorrhoid does not extend out the anus.
  • Grade 2 – The hemorrhoid(s) extends out of the anus with a bowel movement or with straining. After your BM, the hemorrhoid goes back inside on its own.
  • Grade 3 – The hemorrhoid(s) extends out of the anus with a bowel movement or with straining. You have to manually push the hemorrhoid back inside the anus. If you have this, you should seek medical attention, but it is not urgent.
  • Grade 4 (severe) – A hemorrhoid(s) extends outside the anus and are not able to be manually pushed back inside. If you have this seek medical attention immediately. There are significant potential complications.

Hemorrhoid treatment

Most of the time, hemorrhoid symptoms go away after a few days even without treatment. If not, treatment focuses on relieving the pain.

After hemorrhoid surgery

This is generally a same-day surgery. Most people are able to return to work and other activities in about 1-2 weeks. You may have a small amount of bleeding on the dressing or after having a bowel movement. This can last for a couple weeks. You should contact your doctor if the bleeding is more than a few Tablespoons per day.

You may resume your usual diet immediately after surgery.

It is very important to not allow yourself to become constipated after surgery. To avoid constipation, we recommend:

  • Starting a diet of high fiber the morning after surgery. Bran cereal, wheat or rye bread, fresh fruits, and vegetables are ideal.
  • Mixing one tablespoon of Metamucil with eight ounces of water twice a day. You should start this on the morning after surgery.
  • Taking Dulcolax stool softener (100mg) twice daily. This starts on the day before your surgery, and until you are no longer taking pain medication.
  • If you go 48 hours without a bowel movement (BM), take two tablespoons of Milk of Magnesia. Continue taking every 6 hours until your first BM, and then stop.
  • Call the office if you go more than 2 days without a BM or if you are having abdominal pain or abdominal distension.
  • Drink plenty of water and juice and eat fresh fruits and vegetables.
