Post-inflammatory hyperpigmentation isn’t a true acne scar. In some cases, it even fades away all by itself without treatment. Topical retinoids help speed the process up and work to fade away even stubborn marks.

Topical retinoids only work on dark marks. Pitted or depressed scars need more than just topical creams.

Laser Treatments

Laser treatments come in two forms: ablative and non-ablative. Ablative lasers resurface the skin by removing outer layers. Non-ablative lasers create changes in the dermis without causing damage to the skin’s surface.

Ablative Lasers

The most common ablative lasers used to treat acne scars are the carbon dioxide and erbium-YAG lasers. These lasers burn skin tissue in a controlled manner to a specific depth. The usual result is that “new” skin is smoother, atrophic scars are reduced in-depth, and the overall look of scarring is softened.

Skin generally heals within two weeks but can remain red for a period of time after healing. The redness fades over the course of several weeks to several months.

Non-Ablative Lasers

Non-ablative lasers tighten the skin and stimulate new collagen formation. They are often called lunchtime lasers because they can be done quickly, such as over a lunch hour. Unlike ablative lasers, there is minimal to no downtime.

These lasers are most beneficial for mild acne scarring and pigmentation problems, rather than deep, pitted scars. However, pulsed dye lasers are a form of non-ablative laser that are used to improve raised scars and keloids.

Side Effects

Hypopigmentation, or the loss of skin color, is a possible side effect of laser treatment, especially in darker skin tones. Your dermatologist can tell you if you’re a good candidate for laser treatment.

Punch Techniques

Punch techniques including punch excision, punch elevation, and punch grafting are used to treat ice pick scars and other depressed scars. A small punch tool, which is often described as a tiny, circular cookie cutter, is used to cut the scar from the skin. After the scar is excised, the skin is sutured closed.

A small scar resulting from the treatment may be left, but it is generally less obvious than the original one. The new scar can grow fainter with time, or be more speedily faded using resurfacing techniques such as microdermabrasion or laser treatment.

After a scar is excised, a skin graft can be used to fill the void. The graft is usually taken from the skin behind the ear. Again, punch grafts leave their own scars. But they are less noticeable than pitted scars and can be resurfaced more easily.

For deep boxcar scars whose bases are comprised of normal-looking skin, the punch elevation technique may be used. During punch elevation, only the base of the scar is excised.

The dermatologist then elevates the base to the skin’s surface, where it is attached with sutures, steri-strips, or skin glue such as Dermabond. Unlike with skin grafting, the skin after punch elevation has the same tone and texture as the surrounding tissue.

Subcutaneous Incision

Subcutaneous incision, also known as subcision, is used to treat rolling acne scars, as well as some depressed acne scars. Subcision is a simple surgical procedure performed under local anesthesia.

A needle or small scalpel is inserted to run parallel to the skin’s surface. The instrument cuts the bands of tissue that tether the skin to deeper structures. The skin visibly lifts once these bands have been released, smoothing the skin surface’s appearance.

Dermabrasion

Dermabrasion is performed under local anesthesia at your dermatologist’s office. A rapidly rotating wire brush abrades the top layers of the skin. After healing, the look of scars has softened and pitted scars are reduced in depth.

Dermabrasion is used to treat depressed boxcar scarring. This procedure may make ice pick scars look more prominent because while narrow at the skin’s surface, they often widen near the base. Once considered the gold standard in acne scar treatment, dermabrasion is used less often in favor of laser resurfacing techniques.

Dermabrasion may cause pigmentation changes in darker skin tones.

Microdermabrasion

Not to be confused with dermabrasion, microdermabrasion is a cosmetic procedure that is performed at day spas as well as dermatologists’ offices.

During a microdermabrasion treatment, a machine discharges ultra-fine aluminum oxide crystals through a tube onto the skin. The crystals are simultaneously vacuumed away. A series of treatments is needed.

As only the surface skin cells are removed, microdermabrasion works best on hyperpigmentation. This treatment won’t do anything for pitted or depressed scarring.

Dermal Fillers

Another treatment option for depressed acne scars is the use of dermal fillers. A filler substance is injected into the acne scar, elevating the base of the scar so it is more even with the skin’s surface. Results are not permanent, so treatment will need to be repeated after several months.

Substances used as dermal fillers include human and bovine collagen, hyaluronic acid, and fat transferred from the patient’s own body. Your dermatologist will help you decide which filler will be most appropriate for you.

Steroid Treatments

Steroid treatments are often used for raised hypertrophic and keloid scars. Steroids may be injected directly into the scar tissue, causing the tissue to shrink or flatten, softening the scar tissue and improving the overall appearance.

Corticosteroid creams and impregnated tapes (which are infused with a corticosteroid, applied over the scar and left on for several hours at a time) may also be used to treat hypertrophic scars.

Acne scars are of great concern to those who are affected. Acne scarring often has profound effects on self-confidence and can even affect people’s behavior. Fortunately, by using a combination of treatment options, we are able to produce improvement in the appearance of most body and facial acne scars.

There are many different types of acne scars. Some are sharply dimpled “pock marks”, some are larger and flatter with varying degrees of depression, and others have a “rolling” appearance. Some acne scars are firm and elevated while others are red. Therefore, it is not surprising that there is no single acne scar treatment or procedure that will give optimal improvement of acne scars. A combination of surgical, laser treatments, and sometimes medical procedures can be employed to achieve ideal outcomes.

Non-Surgical Removal of Acne Scars

Some scars can be treated with injections or laser treatments. Bellafill is a filler used to counteract the effects of acne scarring by using collagen and medical-grade plastic beads to fill depressed acne scars. Newer laser and energy-based technologies such as non-ablative fractional skin resurfacing and eMatrix skin resurfacing, can also improve the appearance of scarred skin without the longer recovery times associated with traditional laser skin resurfacing.

Surgical Removal (Excision) of Acne Scars

When treating ice pick type scars, surgical excision is usually required.  There are no lasers or technology-based approaches which will provide adequate correction of the scars.  Ice pick scars are frequently found on the cheeks and temples.  Removal of ice pick type acne scars is performed in the office with the aid of local anesthetic injections.  Sedation is not required.  As the scars are removed, sutures are used to repair the skin, converting the deep ice pick scar to a smoother, less depressed scar.  Large numbers of ice pick scars can be removed during a single visit.  Typically, only 1-2 stitches are required at each surgical site.  These stitches stay in place for 1 week.  Some patients who have larger areas of depressed scarring will require either removal and repair of the entire area or elevation of the depressed scars using a procedure known as dermal grafting.  Dermal grafting refers to the use of one’s own tissue as a natural filler that is placed in a pocket created under the depressed area.  When performing dermal grafting, tissue is obtained from ice pick type scars that are being removed or from donor sites such as the area behind the ear.

Subcision Treatment Method for Acne Scar

The scars that are not deep enough to require removal, but are too deep to be expected to respond well to laser treatment alone, are treated with a technique known as subcision.  Subcision is a procedure that uses a tiny blade to release the scar from the underlying skin. This allows the depressed area to become somewhat shallower as the firm attachments holding the scar to the deeper tissues are released.  Stitches are not required following subcision. Bruising and swelling may occur following both removal and subcision of acne scars.

Acne Scar Treatments

Acne scars can be treated with many different methods depending on how severe they are, the number, and the locations. While we would love for complete acne scar removal, the real goal of the best acne scar treatment is dramatic improvement.

Using a laser on the skin can stimulate collagen and elastic fibers. This means that the skin can become tighter and the scars will shrink and become less deep. Acne scars on the face can be blended with the surrounding skin. This treatment is ideal for patients with a broad area of scarring and discolorations. We use a fractional CO2 laser to treat the scar areas as needed.

There is no pain with this procedure. After the fractional CO2 laser resurfacing, there may be a light sunburn appearance for 2 days. Depending on your preference, you can still attend work or school. The skin cells will be stimulated to start making more collagen. The Erbium laser can also be combined to get even better results! The effects are best seen in 3-6 months as it takes time for your body to grown new collagen and elastic fibers.

Platelet-Rich Fibrin Matrix (PRFM)/Platelet-Rich Plasma (PRP)

This will immediately allow the atrophic scarring to elevate. To prevent the scar from tethering again, facial filler can be used. Depending on the location, dermal fillers such as Juvederm, Restylane, or Versa may be used. Fat removed the belly or thighs (liposuction) can also be performed in the office and those fat cells can transferred to improve the acne scars. Individual scars are treated in a particular session. This is a great acne scar treatment for the most severe acne scars.

A detailed and comprehensive discussion of acne scars starts with causes of scarring, prevention of scarring, types of scars, and focal acne scar treatment.

Before talking about scars, focal acne scar treatment, acne scar removal, laser treatment for acne scars, or hyperpigmentation acne scars treatment, a word about spots that may look like scars but are not scars in the sense that a permanent change has occurred: Even though they are not true scars and disappear in time, they are visible and can cause embarrassment. Macules or “pseudo-scars” are flat, red or reddish spots that are the final stage of most inflamed acne lesions. After an inflamed acne lesion flattens, a macule may remain to “mark the spot” for up to 6 months. When the macule eventually disappears, no trace of it will remain — unlike a true scar.

Post-inflammatory pigmentation is discoloration of the skin at the site of a healed (or healing) inflamed acne lesion. It occurs more frequently in darker-skinned people, but occasionally is seen in people with white skin as well. Early treatment by a dermatologist may minimize the development of post-inflammatory pigmentation. Some post-inflammatory pigmentation may persist for up to 18 months, especially with excessive sun exposure. Hyperpigmentation acne scars treatment like chemical peeling may hasten the disappearance of post-inflammatory pigmentation.

Causes of Acne Scars

Focal acne scar treatment or acne scar removal may depend on the cause and type of scar.

In the simplest terms, scars form at the site of an injury to tissue. They are the visible reminders of injury and tissue repair. In the case of acne, the injury is caused by the body’s inflammatory response to sebum, bacteria, and dead cells in the plugged sebaceous follicle. Two types of true scars from acne exist (which we’ll discuss further momentarily): depressed areas such as ice-pick scars, and raised thickened tissue such as keloids.

When tissue suffers an injury, the body rushes its repair kit to the injury site. Among the elements of the repair kit are white blood cells and an array of inflammatory molecules that have the task of repairing tissue and fighting infection. However, when their job is done, they may leave a somewhat messy repair site in the form of fibrous scar tissue or eroded tissue.

White blood cells and inflammatory molecules may remain at the site of an active acne lesion for days or even weeks. In people who are susceptible to scarring, the result may be an acne scar. The occurrence and incidence of scarring is still not well understood, however. There is considerable variation in scarring between one person and another, indicating that some people are more prone to scarring than others. Scarring frequently results from severe inflammatory nodulocystic acne that occurs deep in the skin. But scarring also may arise from more superficial inflamed lesions. Still, nodulocystic acne is the most likely to result in scars.

The life history of scars is also not well understood. Some people bear their acne scars for a lifetime with little change in the scars without some form of acne scar treatment like laser acne scar removal, but in other people, the skin undergoes some degree of remodeling and acne scars diminish in size with time.

People also have differing feelings about acne scars. Scars of more or less the same size that may be psychologically distressing to one person may be accepted by another person as “not too bad.” The person who is distressed by scars is more likely to seek acne scar removal or focal acne scar treatment to moderate or remove the scars.

Preventing Acne Scars

The occurrence of scarring is different in different people. It is difficult to predict who will scar, how extensive or deep the scars will be, and how long scars will persist. It is also difficult to predict how successfully scars can be prevented by effective acne scar treatment such as hyperpigmentation acne scars treatment, laser treatment for acne scars or other acne scar removal procedures.

Nevertheless, the only sure method of preventing or limiting the extent of scars is to treat acne early in its course, and keep treating it for as long as necessary. The more that inflammation can be prevented or moderated, the more likely it is that scars can be prevented. Any person with acne who has a known tendency to scar should be under the care of a dermatologist or cosmetic specialist.

Types of Acne Scars

There are two general types of acne scars, defined by tissue response to inflammation: scars caused by increased tissue formation, which are usually raised; and scars caused by loss of tissue, which appear as indentations.

Scars caused by increased tissue formation are called keloids or hypertrophic scars. The word hypertrophy means “enlargement” or “overgrowth.” Both hypertrophic and keloid scars are associated with excessive amounts of the cell substance collagen. Overproduction of collagen is a response of skin cells to injury. The excess collagen becomes piled up in fibrous masses, resulting in a characteristic firm, smooth, usually irregularly shaped scar. The typical keloid or hypertrophic scar is 1 to 2 millimeters in diameter, but some may be 1 centimeter or larger. Keloid scars tend to “run in families” — that is, abnormal growth of scar tissue is more likely to occur in susceptible people, who often are people with relatives who have similar types of scars. Hypertrophic and keloid scars persist for years, but may diminish in size over time.

Acne scars associated with loss of tissue — like scars that result from chicken pox — are more common than keloids and hypertrophic scars. Scars associated with loss of tissue include “ice-pick” scars, which usually occur on the cheek. They are usually small, with a somewhat jagged edge and steep sides — like tiny versions of wounds from an ice pick. Ice-pick scars may be shallow or deep, and may be hard or soft to the touch. Soft scars can be improved by stretching the skin, but hard ice-pick scars cannot be stretched out. Depressed fibrotic scars are usually quite large, with sharp edges and steep sides. The base of these scars is firm to the touch. Ice-pick scars may evolve into depressed fibrotic scars over time. Soft scars, whether superficial or deep, are soft to the touch. They have gently sloping rolled edges that merge with normal skin. They are usually small, and either circular or linear in shape.

Atrophic macules are usually small when they occur on the face, but may be a centimeter or larger on the body. They are soft, often with a slightly wrinkled base, and may be bluish in appearance due to blood vessels lying just under the scar. Over time, these scars change from bluish to ivory white in color in light-skinned people, and become much less obvious.

Follicular macular atrophy is more likely to occur on the chest or back of a person with acne. These are small, white, soft lesions, often barely raised above the surface of the skin — somewhat like whiteheads that didn’t fully develop. This condition is sometimes also called “perifollicular elastolysis.” The lesions may persist for months to years.

Treatments for Acne Scars

Many treatments are available for acne scars through dermatologic surgery. The type of focal acne scar treatment selected should be the one that is best for you in terms of your type of skin, the cost, what you want the treatment to accomplish, and the possibility that some types of acne scar removal or treatment may result in more scarring if you are very susceptible to scar formation.

A decision to seek dermatologic surgical acne scar treatment depends not just on the severity of your scars, but the way you feel about scars. Do your acne scars psychologically or emotionally affect your life? Are you willing to “live with your scars” and wait for them to fade over time? These are personal decisions only you can make, which can help your doctor decide as well about your treatment options, whether laser treatment for acne scars or hyperpigmentation acne scars treatment.

Scar treatments that are currently available include:

• Dermal filler injection. Hyaluronic acid, a normal substance of the body, is injected under the skin to “stretch” and “fill out” certain types of superficial and deep soft scars. Dermal fillers for focal acne scar treatment usually do not work as well for ice-pick scars and keloids. Benefits from this acne scar removal injection usually last up to 6 months, but then they must be repeated to maintain the appearance.
• Autologous fat transfer. For this type of acne scar removal, fat is taken from another site on your own body and prepared for injection into your skin. The fat is injected beneath the surface of the skin to elevate depressed scars. This method of autologous (meaning from your own body) fat transfer is usually used to correct deep contour defects caused by scarring from nodulocystic acne. Because the fat is reabsorbed into the skin over a period of 6 to 18 months, the acne scar treatment procedure usually must be repeated. Longer-lasting results may be achieved with multiple fat-transfer procedures.
• Dermabrasion. Under local anesthetic, a high-speed brush or fraise is used to remove surface skin and alter the contour of scars. With this acne scar removal option, superficial scars may be removed altogether, and deeper scars may be reduced in depth. Dermabrasion does not work for all kinds of scars; for example, it may make ice-pick scars more noticeable if the scars are wider under the skin than at the surface. In darker-skinned people, dermabrasion may cause changes in pigmentation that require additional acne scar removal treatment.
• Microdermabrasion. This new technique is a surface form of dermabrasion. Rather than a high-speed brush, microdermabrasion uses aluminum oxide crystals passing through a vacuum tube to remove surface skin. Only the very surface cells of the skin are removed, so no additional wound is created. For this type of focal acne scar treatment, multiple procedures are often required but scars may not be significantly improved.
• Laser Treatment for Acne Scars. Lasers of various wavelength and intensity may be used to recontour scar tissue, and reduce the redness of skin around healed acne lesions. The type of laser used is determined by the results that the laser acne scar removal treatment aims to accomplish. Tissue may be removed with more powerful instruments such as the carbon dioxide laser. In some cases, a single laser acne scar removal procedure is all that will be necessary to achieve permanent results.
• Skin Surgery. Some ice-pick scars may be removed by “punch” excision of each individual scar. In this focal acne scar treatment procedure, each scar is excised down to the layer of subcutaneous fat; the resulting hole in the skin may be repaired with sutures or with a small skin graft. Subcision is a technique in which a surgical probe is used to lift the scar tissue away from unscarred skin, thus elevating a depressed scar.
