So, if you just had a stressful day, or have one coming up, it might be completely normal to have an action-packed dream (about being in a fight, or being chased), and wake up with fists clenched. For instance, when a baby clenches its fists for more than three months, this can signal a neurological disorder. While fist clenching is part of your baby’s development, it does not hurt seeking a second opinion when you find it unusual.

Among the things that you can do include, learn how to express your feelings, avoid or manage stress, stay hydrated, sleep on comfortable beddings, and most importantly, seek professional advice.

“My Tingling Hands Wake Me Up at Night!”

My first recommendation to these patients is to avoid bending the wrist and elbow for prolonged periods at night.  How is this done? A simple wrist splint worn to bed can keep the wrist in a neutral position, which maximizes the diameter of the carpal tunnel, and minimizes chances of that nerve being pinched and awakening you. Similarly, a towel placed around the elbow, held with either tape or an Ace bandage, can be used to comfortably keep the elbow straight at night.

I tell patients, “If you don’t irritate the nerve for 6-8 hours while you’re sleeping at night, it tends to be much less irritable to you throughout the day.”

There are several non-surgical treatment options for carpal and cubital tunnel syndrome, such as splinting, anti-inflammatories, postural changes, and nerve gliding exercises. When these fail, surgery can oftentimes be very beneficial.

A research study published recently in the Journal of Hand Surgery assessed the quality of sleep in patients before and after carpal tunnel release surgery. In a study of 398 individuals with carpal tunnel syndrome, they found that carpal tunnel release surgery significantly improved quality of sleep as rated by seven aspects.  After carpal tunnel surgery, patients had:

  1. Less difficulty falling asleep
  2. Easier time staying asleep
  3. Fewer problems waking up early
  4. More satisfaction with their sleep quality
  5. Reduction in sleep problems affecting quality of life
  6. Less worries about their sleep problem
  7. Improvement in daily functioning, concentration, and memory

The benefits were seen almost immediately after surgery, and all 7 of these variables were significantly improved as soon as 2 weeks after surgery.

Again, oftentimes carpal tunnel syndrome (and cubital tunnel syndrome) can be managed successfully without surgery. Unfortunately, non-operative management is not always successful.  Surgical release of the compressed and pinched nerve can quickly decrease symptoms, and, it turns out, may make you feel much more rested.

1. Palmar Grasp Reflex

Babies have a very strong grasping reflex called the “Palmar grasp which prevents them from opening their hands. In case you tickle your baby’s palm or offer him your finger to hold, he is likely to close his fingers around it and clutch it tightly.

2. Cerebral Palsy

Cerebral palsy could also be one of the causes of why a baby clenches his fists all the time. In case a baby’s fists are clenched all the time and his body is stiff, it could be a sign of a neurological disorder, wherein the baby’s brain has difficulty directing the various muscles to perform different functions. Rigid or spastic movements, poor motor control, and muscle weakness are other symptoms of the condition.

3. Vestigial Primitive Reflex

Newborn babies during their initial days of life tend to remember and keep the position they held while curbed inside the uterus. They often hold their arms near their bodies and have their hands clenched into fists. It may take them several weeks before they clench and unclench their fists.

4. Evolutionary History of Humans

Scientists studying human evolution offer a logical explanation for why do babies clench their fists when feeding. A popular view is that they do it because the monkeys did it. Modern humans who seem to share a common ancestry with some species of upright-walking apes learnt the vice-like grip from baby apes who hold on to their mom’s thick hair for survival as they hang in precarious positions while their moms swing from treetops.

What Would Happen If Babies Do Not Clench Their Fists?

If babies don’t clench their fists when they are in the womb, they would perhaps try to pinch or grab. Furthermore, babies have fingernails which could hurt a mother. The most fragile part of the womb is the thin amniotic sac which surrounds the baby. It is a nurturing membrane made up of complex biochemicals. Any pricking or scratching of this flimsy sac can lead to the release of hormones and chemicals that may induce premature labour. Thus, a baby’s clenched fist can be a way of preventing the baby’s fingernails from harming the amniotic sac.

Researchers cite a congenital issue called amniotic band syndrome as a plausible explanation for a baby not clenching his fists. Sometimes thin strands from amniotic sac may get peeled away to float with the amniotic fluid which is around the baby. The strands may wrap around the toes, fingers or even arms of the baby causing serious injury to the limbs including amputation. The reason why some tiny bands get stripped away from the amniotic sac is not known yet. But some doctors propose that a baby’s lack of appropriate palmar grasp reflex can be a reason of amniotic band syndrome. And hence, a baby may not be able to clench his fists.

When Do Babies Unclench Their Fists and Start Using Their Hands?

Most moms like to know for how long babies can clench their fists. The answer is that newborn babies usually clench their fists for the initial few months after their birth due to palmer grasp reflex. By the age of 3–4 months, they gradually begin unclenching their fists. You may see them relaxing their tight fists and opening their hands as their nervous system slowly matures. They may also try and reach for toys with open hands that you tie them in their crib. And by the age of 6-7 months, they learn to grasp, hold, and release objects.

It is normal for babies to clench their fists but slowly they learn to unclench their fists and grasp and hold other objects. Many of the problems that hand therapists treat, such as carpal tunnel syndrome, cubital tunnel syndrome, thoracic outlet syndrome, arthritis, or even tendonitis can be exacerbated by pressure while we are sleeping. This pressure can worsen symptoms like swelling, numbness, tingling, and pain.

How does this happen?

Sometimes the positions in which we sleep place our spine, joints, and soft tissues under compression, tension, or both. Our nerves are especially sensitive to this. For example, when we sleep in a “fetal position,” the nerves in our wrists and elbows can be compressed at the point where our arms and wrists bend. They can also be put under tension which can cause irritation and aggravate symptoms like numbness, tingling, and/or pain. Another example is when a pillow causes the neck to be bent in a position that can compress the nerves that travel from the spine into the arms and hands.

How can I prevent it?

Fortunately, this does not always mean that we need to go buy an expensive mattress or pillow. There are some simple changes that we can make at night to better position ourselves for success during the day. See below for some helpful tips with photos.

Back Sleepers

For back-sleepers, the goal is to maintain the natural curves in your spine from head to your hips. You want a pillow that naturally cradles the neck without placing the head too high or too low. This protects the nerves that travel from the neck into our arms and hands. A small pillow under each elbow allows the shoulder to naturally fall back against the mattress, the arms to rest in a neutral position, and lessens the urge to put your arms up over your head. This decreases pressure or tension on the nerves as they travel through the shoulder and elbow. A pillow under your knees maintains the curve at the base of the spine and decreases pressure on your lower back and knees. This also can alleviate pressure on the nerves that arise from our lower spine and travel into our legs and feet.

Side Sleepers

For side-sleepers, the goal is to maintain the spine in a level line from neck to hips. Your pillow should cradle the neck without positioning the head too high or too low. On the mattress side, your shoulder blade should be lying flat on the bed rather than having your arm curled underneath you. Your top arm should be lying on and cradling a pillow to prevent compression in your shoulder, elbow, and wrist. A pillow between your legs decreases pressure on the low back, hips, and knees.
