What is Orajel?

Orajel contains benzocaine, a local anesthetic (numbing medication). It works by blocking nerve signals in your body.

Orajel numbs the skin or surfaces inside the mouth and used for the temporary relief of pain from sore throat, canker sores, cold sores, fever blisters, minor irritation or injury of the mouth and gums.

Orajel is also use for temporary relief of sore gums due to teething in children 2 years of age and older.

Warnings

Orajel used in the mouth may cause a condition in which the oxygen in your body tissues can become dangerously low. This is a potentially fatal condition called methemoglobinemia (met-HEEM-oh glo-bin-EE-mee-a). Do not use this medicine if you have ever had methemoglobinemia.

GET EMERGENCY MEDICAL HELP IF YOU HAVE SYMPTOMS OF METHEMOGLOBINEMIA: headache, tired feeling, confusion, fast heart rate, and feeling light-headed or short of breath, with a pale, blue, or gray appearance of your skin, lips, or fingernails.

An overdose of benzocaine can cause fatal side effects if too much of the medicine is absorbed through your skin and into your blood. Use the smallest amount needed.

Do not use Orajel on a child younger than 2 years old.

Before taking this medicine

Do not use Orajel if you have ever had methemoglobinemia in the past.

Ask a doctor or pharmacist if this medicine is safe to use if you have:

asthma, bronchitis, emphysema, or other breathing disorder;
heart disease; or
if you smoke.

Ask a doctor before using Orajel if you are pregnant or breast-feeding.

If you apply Orajel to your chest, avoid areas that may come into contact with the baby’s mouth.

How should I use Orajel?

Use Orajel exactly as directed on the label, or as prescribed by your doctor. Read all medication guides or instruction sheets.

Your body may absorb more benzocaine if you use too much, if you apply it over large skin areas.

Use the smallest amount needed to numb the skin or relieve pain. Do not use large amounts of Orajel. Do not cover treated skin areas with plastic wrap without medical advice.

Do not use Orajel to treat large skin areas or deep puncture wounds. Avoid using the medicine on skin that is raw or blistered, such as a severe burn or abrasion.

Call your doctor if your symptoms do not improve or if they get worse within the first 7 days of using Orajel. Also call your doctor if your symptoms had cleared up but then came back.

If you are treating a sore throat, call your doctor if the pain is severe or lasts longer than 2 days, especially if you also develop a fever, headache, skin rash, swelling, nausea, vomiting, cough, or breathing problems.

Store at room temperature away from moisture and heat. Do not freeze.

What happens if I miss a dose?

Since Orajel is used when needed, you may not be on a dosing schedule. Skip any missed dose if it’s almost time for your next dose.

What happens if I overdose?

Seek emergency medical attention or call the Poison Help line at 1-800-222-1222. An overdose of benzocaine topical applied to the skin can cause life-threatening side effects such as uneven heartbeats, seizure (convulsions), coma, slowed breathing, or respiratory failure (breathing stops).

What should I avoid while using Orajel?

Avoid eating within 1 hour after using Orajel on your gums or inside your mouth.

Avoid getting Orajel in your eyes. Avoid swallowing the gel while applying it to your gums or the inside of your mouth.

Get emergency medical help if you have signs of an allergic reaction to Orajel: hives; difficult breathing; swelling of your face, lips, tongue, or throat.

Stop using this medicine and call your doctor at once if you have:

severe burning, stinging, or sensitivity where the medicine is applied;
swelling, warmth, or redness; or
oozing, blistering, or any signs of infection.

Common Orajel side effects may include:

mild stinging, burning, or itching where the medicine is applied;
skin tenderness or redness; or
dry white flakes where the medicine was applied.

What other drugs will affect Orajel?

Benzocaine topical is used to reduce pain or discomfort caused by minor skin irritations, sore throat, sunburn, vaginal or rectal irritation, ingrown toenails, hemorrhoids, and many other sources of minor pain on a surface of the body. Benzocaine is also used to numb the skin or surfaces inside the mouth, nose, throat, vagina, or rectum to lessen the pain of inserting a medical instrument such as a tube or speculum.

Benzocaine topical should not be used to treat teething pain in infants, and is not approved for use in a child younger than 2 years old. There are many brands and forms of benzocaine topical available. Not all brands are listed on this leaflet.

Fatal overdoses have occurred when numbing medicines were used without the advice of a medical doctor (such as during a cosmetic procedure like laser hair removal). Be aware that many cosmetic procedures are performed without a medical doctor present.

a personal or family history of methemoglobinemia, or any genetic (inherited) enzyme deficiency;
asthma, bronchitis, emphysema, or other breathing disorder;

How should I use benzocaine topical?

Use the smallest amount needed to numb the skin or relieve pain. Do not use large amounts of benzocaine topical. Do not cover treated skin areas with a bandage or plastic wrap without medical advice.

Do not use benzocaine topical to treat large skin areas or deep puncture wounds. Avoid using the medicine on skin that is raw or blistered, such as a severe burn or abrasion.

To treat minor skin conditions, apply a thin layer of benzocaine topical to the affected area. If using the spray, hold the container 6 to 12 inches away from the skin. Do not spray this medication onto your face. Spray it instead on your hands and then rub it onto the face, avoiding contact with your eyes.

Clean the area with soap and water before applying benzocaine topical. Wash your hands before and after inserting a rectal suppository.

Do not take a rectal suppository by mouth. It is for use only in your rectum.

Remove the wrapper, but avoid handling the suppository too long or it will melt. Lie on your back with your knees up toward your chest. Gently insert the suppository into your rectum about 1 inch, pointed tip first.

Stay lying down for a few minutes while the suppository melts. You should feel little or no discomfort. Avoid using the bathroom for at least an hour.

Call your doctor if your symptoms do not improve or if they get worse within the first 7 days of using benzocaine topical. Also call your doctor if your symptoms had cleared up but then came back.

Is it safe to use Orajel or Anbesol for teething?

The FDA has strengthened its warnings about benzocaine, a medication used to numb pain that is included in some over-the-counter teething products. Though it’s rare, swallowing benzocaine can cause a dangerous condition called methemoglobinemia, in which not enough oxygen is carried in the blood.

The latest FDA warning states that benzocaine products should not be used in children younger than 2 years. It also calls on manufacturers to stop marketing benzocaine products for teething and to add a warning about methemoglobinemia to the product label.

What are some ways that you can relieve your child’s teething pain, without using numbing medications like Orajel or Anbesol?

Gently massage your child’s gums.
Give your child something firm and cool to chew on. Make sure it is clean, and large enough that they can’t choke on it, and supervise them closely. Some possibilities are a chilled teething ring or pacifier; a cool, wet washcloth; a chilled cucumber or carrot; or hard, unsweetened teething crackers. Avoid using frozen items. They may be too hard or uncomfortably cold.
Comfort and distract your child. Sing a favorite song, read a book, rock your child, or gently message them. Breastfeeding can also help.

The US FDA orders a recall on Baby Orajel and other teething painkillers that include benzocaine — Quartz

Twenty-seven years after the US Food and Drug Administration (FDA) approved a mild oral painkiller for over-the-counter use, the agency is reversing its stance.

On May 22, the FDA issued a warning about over-the-counter products containing benzocaine, citing “serious safety concerns” and a “lack of efficacy for teething.” The painkiller can be found in Baby Orajel, a gel that can be spread over the gums, as well as in other sprays, gels, and lozenges.

In children under two, benzocaine can cause a condition known as methemoglobinemia, where oxygen levels in the blood are abnormally low. Symptoms include a gray or bluish appearance, especially in the lips and nail beds, as well as rapid breathing and an elevated heart rate. They can occur a couple of minutes to several hours after receiving a dose of a product containing benzocaine. In some cases, methemoglobinemia can be fatal.

The FDA’s warning is an update to a statement the regulatory agency made seven years ago. At that time, the FDA was aware that methemoglobinemia was a rare reaction to benzocaine; it had received a total of 319 reports of this particular complication, not limited to children. Seventy-two of them occurred between 2006 and 2011. In that statement, the FDA said it planned to monitor benzocaine products, and would update advisories as needed.

In addition to warning consumers this week, the FDA sent a letter (pdf) to companies that sell these products, request that they discontinue any lines marketed for teething relief. According to the letter, benzocaine products marketed to adults—including Orajel, Anbesol, Cepacol, and Chloraseptic—are still considered safe, but need to contain two additional warnings: one telling users that the product shouldn’t be used for children under two, or for teething, and another about the risks of methemoglobinemia.

Even before the latest warning, the American Academy of Pediatrics did not endorse these types of teething painkillers. ;)) and I have the same problem. Advil works well if you can use it but I can’t.Exederin worked well when I was still able to take it, it’s another drug I can’t take. I was prescribed a viscous 2% lidocaine, and although it works for 15-20 minutes, the pain returns when it goes away. I found that it works best, trust me, is a listerine gargle in my mouth for about a minute. Make sure it also contains methyl salicylate. I think this is what makes it work. When I do this and hold a heating pad (covered and over low heat) on the part of my mouth that hurts, it relieves most of the pain.

I know this is an old thread, but after reading 5 million messages since I was in agony for 5 weeks with a lot of pain… the swollen jaw throbbed continuously that every time I thought it would pass, it just moved to a new place in my mouth .. he was on minocycline and percocet, and no relief. actually got bad enough, I cut my gums with a precision razor to relieve the pressure ..
