Medicines That Treat Acute & Chronic Sinusitis

 

Sinusitis is common. But many people who have it use treatments that aren’t likely to help. So before you run to the drugstore, you’ll want to be sure you know how to handle this condition.

The sinuses are small cavities in the skull that are normally filled with air. They make mucus, which helps keep the nasal passages clear of allergens and pollutants.

Sinusitis is an inflammation of the tissue that lines these cavities. In some cases, this swelling blocks off the sinuses, trapping mucus and air inside them. This can cause pain and pressure. Sometimes, it can lead to a bacterial infection.

Do you know the two types of sinusitis?

Acute vs. Chronic

Acute sinusitis lasts up to 4 weeks, but typically improves in a week to 10 days.

Chronic sinusitis may last up to 3 months – and can linger for years. Its cause can be hard to pin down — and hard to treat.

Acute sinusitis usually occurs from a cold, and sometimes from allergies. The cause of chronic sinusitis isn’t always as obvious. Risks for chronic sinusitis include year-round allergies, a weak immune system, frequent colds, and cigarette smoking.

Sometimes, chronic sinusitis is due to problems with the structure of the nasal passages, or a growth such as a nasal polyp that keeps the sinuses from draining normally.

Acute and chronic sinusitis have similar symptoms:

  • Facial pressure and pain
  • Thick discolored mucus
  • Congestion

Treatment

The key is to figure out the cause. For instance, if your sinusitis is due to allergies, then decongestants alone will probably not help much.

If you have symptoms for more than 10 days, check in with your doctor. With a good exam — and sometimes imaging tests, like X-rays, CT scans, or MRIs — you may be able to figure out exactly what’s causing the problem.

Often, the best sinusitis treatment is a combination of different approaches — typically medication plus self-care.

Medicines

Antibiotics. If your doctor thinks a bacterial infection is to blame, they may prescribe antibiotics. For acute sinusitis, you typically take them for 10-14 days. For chronic sinusitis, it might be longer.

Antibiotics only help with bacterial infections. They won’t help if your sinusitis is caused by viruses or other problems. 

Painkillers. Many people with sinusitis take over-the-counter (OTC) pain medicines, like ibuprofen or acetaminophen, to ease discomfort. Follow the instructions on the label, and don’t take them for more than 10 days. Check with your doctor to see which one is right for you.

Decongestants. These meds lower the amount of mucus in the sinuses. Some are available as nasal sprays. Others are pills. If you use decongestant nasal sprays for more than 3 days, they may actually make you more congested. Follow the instructions on the label.

Allergy medicines. Many cases of sinusitis are due to uncontrolled allergies. If you’ve never been diagnosed with allergies, it might be worth doing some allergy testing to see if you have them. If you do, medication (like antihistamines) and avoiding your triggers will help. Another option is to get allergy shots, a long-term treatment that gradually makes you less sensitive to the things that set off your symptoms.

Steroids. In some cases, your doctor may prescribe inhaled steroids to bring down the swelling in the sinus membranes. For tough cases of chronic sinusitis, you may need to take steroids by mouth.

Surgery. Occasionally, if you have chronic sinusitis or acute sinusitis that keeps coming back, an operation may be the best choice. The surgeon can remove blockages and enlarge the sinus passages, which makes it easier for them to drain.

Home Remedies

While medicines can help, many cases of sinusitis go away on their own without any medical treatment. If you often get the condition, many of these same approaches will help you prevent it, too.

Humidify. Use a humidifier in rooms where you spend a lot of time. Follow the instructions for regular cleaning.

Breathe in steam vapors. You can either run the shower and sit in the bathroom, or breathe in steam from a bowl of warm (but not too hot) water. The steam vapors ease congested and swollen nasal passages.

Use warm heat. Put a warm, wet towel on your face. It can take off some of the pressure.

Try a nasal saline solution. While they don’t contain medicine (saline is salt water), they can help keep your nasal passages moist.

Flush out your sinuses.Nasal irrigation with salt water can clear out mucus (and other debris) and keep your sinuses moist. You can use bulb syringes or neti pots, for example. Use distilled, sterile, or previously boiled water to make up the irrigation solution. Rinse the device after each use and let it air dry.

Drink lots of fluids. They’ll help thin the mucus, which reduces the blockage in your sinuses. Cut down on alcohol, which makes the swelling worse.

Rest. When you’ve got a sinus infection, take it easier than normal. Get plenty of sleep and give your body a chance to recover.

Home treatments may do the trick for some people. But call your doctor if you’ve had sinus symptoms for more than a few days or if they’re really bad. The sooner you start the right treatment, the sooner you’ll feel better again.

Natural Home Remedies for Sinus Infection and Pain

1. Heat Up (or Steam Up) Your Face

One of the most effective home remedies for sinus infections is to warm up and moisturize your sinus passageways.

“Inhaling steam helps to soothe the sinus tissue and gives you the feeling of clearing them out a little,” Dr. Del Signore says.

You can simply stand in the shower or even sit in the bathroom when the shower is running. You can also place a warmed washcloth over your nose and cheeks while you lie on your bed.

For the most potent steam treatment, boil a pot of water, then take it off the heat. Tent a towel over your head and bend over the pot to inhale the steam. Be careful not to start out too close to the hot water and to keep your eyes closed. As the liquid cools, you can move in a little, but only to the point where it remains comfortable.

You might add a drop or two of essential oils; eucalyptus oil can help open the nose, while lavender essential oil or chamomile essential oil will calm you.

2. Irrigate Your Sinuses to Help Ease Symptoms and Prevent Sinus Infections

Nasal irrigation is basically a method of using a saltwater solution to force out germs and plugged-up mucus residing in the sinus passages. Other terms for this are nasal wash, nasal douche, or lavage. Some people refer to it by one of the popular devices used to get the water in, a “neti pot.”

A small number of studies has found irrigation can improve symptoms, including one review published in September 2016 in the Canadian Medical Association Journal.

Experts caution that it is important to use distilled or sterile water (you can sterilize tap water yourself by boiling for 3 to 5 minutes, then cooling) to avoid the rare possibility of introducing a parasite into your sinus passageways.

3. Yoga Can Help Drain Mucus From Sinus Passageways

If you are in the midst of a sinus infection, a supported yoga pose where your head is elevated will help you feel better without putting too much pressure on your sinuses, says Leslie Kazadi, a certified yoga therapist who teaches around Los Angeles and online at YogisAnonymous.com.

How to Try Supported Reclined Cobbler’s Yoga Pose

  1. Place a bolster or rolled up blanket under your back and lie on your bed or floor.
  2. Bend your knees and bring the soles of your feet together; you can place yoga blocks or rolled towels under your knees to make this more comfortable.
  3. Relax your arms out to your sides. Remain here for as long as is comfortable.
  4. Come out of the pose by rolling off the bolster or blanket and onto your side, then pressing your hands against the floor to sit up.

4. Consider Using a Supplement, Such as the Enzyme Bromelain

Bromelain is a mixture of enzymes found in the pineapple plant that is sold as a dietary supplement. According to the National Center for Complementary and Integrative Health (NCCIH), you can get it as a powder, cream, tablet, or capsule, sometimes in combination with other ingredients.

According to a research published in the journal Laryngoscope, bromelain has been studied for sinusitis because it is thought to be effective in taming inflammation. A small number of double-blind studies has found bromelain improves sinus symptoms more than a placebo, the review found.

Research published in Alternative Medicine Review indicated that oral doses of bromelain are typically from 500 to 1000 milligrams (mg) per day, but some people take 2000 mg.

Although bromelain is natural, that doesn’t mean there can’t be side effects. The NCCIH cautions that some people experience allergic reactions, GI issues, menstrual problems, and an increased heart rate.

5. Try Quercetin — a Powerful Herb You’ve Likely Never Heard Of

Quercetin is a natural plant component found in everything from onions and apples to green tea and red wine. Like many plant ingredients, it is an antioxidant. For sinus problems, quercetin has also been found to stabilize the cells in the body that release histamine — the chemical that stimulates mucus secretion in the sinuses.

The Alternative Medicine Review article recommends quercetin as helpful for sinusitis, suggesting a typical oral dose of 400 to 500 mg taken three times per day.

6. Drinking Liquids Help Ease Sinus Pain and Loosens Congestion

Staying hydrated keeps your sinuses moist so you feel better, and it also decreases the thickness of sinus mucus so it flows out more easily, Del Signore says.

“Everyone is guilty of not drinking enough water,” he says, recommending people get from six to eight 8-ounces glasses every day.

Steer clear of too many caffeinated or alcoholic drinks, which can cause dehydration.

How to Tell if These Remedies Are Not Working

You will know if these remedies are effective because you will begin to feel better and your sinuses will be less congested.

However, unlike with antibiotics where symptoms start to diminish quickly, natural remedies typically take longer to work. So you should continue to do these remedies regularly for at least a week or two before determining if they are working.

When To See a Doctor for Sinus Issues

If your symptoms persist more than one to two weeks, you should consult with your physician.

You’ll want to check with your doctor even sooner if you have a severe case of a sinus infection — including a high fever, swelling around the eyes, and red and inflamed skin, among other symptoms, Del Signore says.

In this case, or if your symptoms do not clear up within two weeks, your doctor will likely prescribe an antibiotic.

You probably don’t give too much thought to your ability to breathe through your nose — until you wake up with a sinus infection and that ability is gone!

Sinus infections, sometimes called acute sinusitis, can steal your joy pretty quickly.

A lot of viruses start with nasal symptoms. If you have a scratchy throat, congestion, and a runny nose, you’ve probably got a virus on your hands.

What’s Happening in My Body?

Most sinus infections come from colds that start in the nose. (Doctors and nurses actually call the viruses that cause these “rhinoviruses,” because rhino means nose!)

A lot of sinus infections are caused by coronaviruses. (The SARS-CoV-2 virus that causes COVID-19 is a more severe type of coronavirus, but studies have shown that an estimated 15% of common colds originated from less serious coronaviruses.)

These viruses replicate in the nose. Your immune system then kicks off an inflammatory response to help kill the virus. This can cause swelling in the sinuses, leading to your symptoms.

Unlike bacterial infections, viral infections don’t respond to antibiotics and usually just need to run their course. But you don’t have to take it lying down!

Home Remedies for Sinus Infections

While you wait for the virus to finish doing its thing, you can try some home remedies for sinus infections to make yourself more comfortable.

Saline Spray and Sinus Rinses

Using a saline solution, rinse your nose and sinuses with a neti pot or other irrigation system.

You can buy saline solution over the counter or make your own at home with distilled water and saline packets. Saline spray also comes in small bottles that you can spray directly into your nose.

Sinus rinses like these work by flushing mucus out of the nose and sinuses to keep things flowing freely.

Saline also has a natural decongesting effect, meaning it shrinks your swollen nasal tissues and makes it easier for you to breathe through your nose!

Over the Counter Medications

For help alleviating sinus pressure and pain, try ibuprofen and a decongestant. Some medications include a combination of pain reliever and decongestant. (Advil Cold and Sinus is one example.)

Mild antihistamines like Claritin and Allegra are helpful in moderation. Strong antihistamines like Benadryl are often way too drying. Be careful not to take antihistamines too often.

Even though you want your nose to stop running, too many antihistamines can make it more likely that you’ll get sinus infections in the future.

Drugs like Mucinex and Flonase can also be helpful in thinning the mucus in your nose and sinuses. (Don’t use Mucinex-D if you have high blood pressure, though!)

If you use Afrin, make sure you stop after three days. Though it can seem like a miracle drug, using it more than three days in a row can lead to something called rebound congestion. In other words, when you stop using the Afrin after continuous use, your nose becomes more congested than it was in the first place!

Vapor Rub

Vapor rub products like Vicks usually contain mentholatum, which can help clear out your sinuses. Breathing mentholatum works similarly to eating spicy foods. You can apply vapor rub to your chest or add it to a humidifier.

Take Your Vitamins

Supplementing your diet with vitamins, especially vitamin C and vitamin D, can give a boost to your immune system to help it kick infection to the curb.

Vitamin C can be taken in chewable, liquid, or pill form. Vitamin D is usually taken in capsule or oil form.

Closely follow all directions on the bottle from the manufacturer.

Steam It Out

Steam is a great natural way to reduce the symptoms and discomforts of a sinus infection. The moist heat can open up your sinuses and help you breathe better.

Try the steam method by taking a hot shower, or just turn the shower on and sit in the steamy bathroom for 15 or 20 minutes.

Adding a humidifier or vaporizer to your bedroom at night can also help you breathe easier while you sleep.

Stay Hydrated

Fluid intake is one of the best ways to keep the mucus flowing so it doesn’t clog up your sinuses.

Be sure to drink plenty of water. You can also try an electrolyte beverage such as Gatorade.

Do These Things Work for Kids?

Sinus infections make everyone miserable, children as well as adults!

Most of the tips above also work for kids, but antihistamines aren’t recommended for young children.

Young children can’t blow their noses well, so helping them thin and clear the mucus from their nose is beneficial.

A nasal aspirator or a product like the Nose Frida can be a lifesaver in helping young children breathe through their nose.

A warm bath can help clear the sinuses and take a child’s mind off their illness.

Kids are often fascinated by the condensation that forms on surfaces in a steamy room. To occupy them while a hot shower runs in the bathroom, try tracing pictures with them on the mirror!

When Should I See a Doctor?

These home remedies for sinus infections are great for acute infections in the upper respiratory system.

But if five days or so pass and you’re not seeing any improvement, or if things are getting worse, it’s probably time to see an ENT doctor.

If you feel a lot more pressure behind your cheekbones and eyes or your nasal drainage has changed to yellow or green-tinged, it’s likely your sinus cavity has gotten infected from the blockage.

If you have symptoms like a fever, body aches, chills, chest congestion, or a cough, something more serious could be going on. Especially in this era of COVID-19, it’s a good idea to get checked out just to be safe.

What is a sinus infection?

Sinusitis is the medical term for inflammation of the sinus cavities that are located in your cheeks, behind your forehead, and on either side of your nose. The inflammation traps mucus inside your sinuses, preventing the natural flow from the back of your throat to your stomach.

When this is caused by bacteria or viruses, it is called a sinus infection. However, sinusitis can also be caused by chronic allergies or nasal polyps.

The common symptoms of a sinus infection include:

It’s also important to keep your humidifier clean to prevent bacteria and fungi from building up in the tank and being released into the air, potentially causing lung problems. To clean your humidifier, follow the manufacturer’s instructions. The EPA recommends that you empty the tank and dry all surfaces of the humidifier every day.

3. Breathe in steam

Similar to a humidifier, breathing in hot, moist steam can help open up your nasal passages and relieve pain and pressure. You can breathe in straight steam or try some herbs to thyme, basil, and eucalyptus to boost symptom relief.

You can try steam inhalation in a couple of ways:

Steam therapy

For steam therapy, you can simply take a hot shower and inhale the steam. Another method is to drape a towel over your head and inhale the steam from a bowl of hot water for about 10 minutes, up to four times a day.

However, a 2012 article in the British Journal of General Practice notes that this method has no proven benefit and may risk burn injuries, especially for children.

Steam vaporizer

Another option is to use a steam vaporizer that uses electricity to heat water and create steam.

“If you have very thick mucus, it might help make it thinner,” Abi Hachem says. “You should place it relatively close to you, not in one room when you’re in another room.”

Steam vaporizers should not be used around children or pets, since they could be scalded by the steam or by hot water if it tips over.

Although steam vaporizers aren’t as likely as humidifiers to have a mineral buildup, they should still be cleaned each day by emptying the tank and drying all surfaces.

4. Use a neti pot

You can also rinse and help clear your nasal passages using a neti pot. Known as nasal irrigation, this helps loosen mucus and removes dust, pollen, and other debris.

In fact, a 2016 randomized controlled trial found that nasal irrigation is more effective than steam inhalation for improving the symptoms of sinusitis. Participants who used nasal irrigation daily showed improvement after six months, while participants who used steam inhalation did not show improvement.

To safely use a neti pot, follow these steps:

  1. Lean over a sink and tilt your head sideways so your forehead and chin are almost level. This will help prevent the solution from flowing from your nose into your mouth.

  2. Insert the spout into your highest nostril. The solution will drain out through your lower nostril. As you do this, Abi Hachem suggests you say the letter “K” to help elevate your palate and prevent the solution from dribbling down your throat.

  3. Tilt your head in the opposite direction and repeat these steps.

After each use, rinse the neti pot with distilled water and let it air dry. It’s important to be sure only to use distilled water or tap water that’s been boiled for one minute and then cooled. Tap water contains bacteria that can remain inside your nasal passages and lead to rare, possibly fatal infections.

You can also purchase a saline solution or make your own. Adding salt to the water prevents burning or irritation when the solution passes through your nasal membranes.

5. Drink lots of water and get adequate rest

“I recommend drinking a lot of water because it thins the mucus,” Abi Hachem says. While there’s no evidence about the most effective quantity, he recommends drinking at least eight eight-ounce glasses of water each day.

You can also try drinking hot liquids like tea or soup to temporarily help relieve your symptoms. But you should avoid drinking alcohol, as it’s dehydrating and can worsen your congestion. If you’re taking an antibiotic for your sinus infection, drinking alcohol could increase side effects like upset stomach and drowsiness.

It’s also important to get about seven to nine hours of sleep each night to help your body rest and recover. “You want to get enough sleep and follow a normal sleep cycle so that all the hormones in your body are maxing out your immune system function,” Abi Hachem says.

Because getting a good night’s sleep can be difficult due to the pain and congestion, you can do the following to help you get more rest:

  • Use a neti pot before going to bed to help clear your nasal passages.

  • Sleep with your head raised by an extra pillow to help prevent mucus from pooling in your sinuses.
